Objective In order to increase the supervision of asarone injection and ensure the safety of clinical use, the samples of asarone injection in circulation in the market were tested. Methods In this paper, according to the national drug standards approved by the State Food and Drug Administration, the quality of 26 batches of asarone injection sampled by the institute was investigated, and the project evaluation of the variety traits, identification, pH value, color, related substances, loading, visible foreign bodies and content determination was completed. Results 26 batches of asarone injection were tested according to the standard, and all items were in conformity with the regulations. Conclusion The overall quality of this variety is good, but the properties of light degradation are observed. It is suggested to further optimize the production and use conditions, reduce the exposure time and avoid the formation of impurities.

目的 为加强对细辛脑注射液的监管,保障人民的临床用药安全,故对市场现行流通使用的细辛脑注射液进行抽样检测。方法 本文依照国家食品药品监督管理总局批准的国家药品标准,考察了本机构抽样的26批次细辛脑注射液的质量状况,完成了该品种性状、鉴别、pH值、颜色、有关物质、装量、可见异物及含量测定的项目评价。结果 抽检的26批次细辛脑注射液依标准检测,各项目均符合规定。结论 评价该品种整体质量较好,但考察到该品种有见光降解的性质,建议进一步优化生产、使用条件,减少曝光时长,避免杂质生成。

目的 为加强对细辛脑注射液的监管,保障人民的临床用药安全,故对市场现行流通使用的细辛脑注射液进行抽样检测。方法 本文依照国家食品药品监督管理总局批准的国家药品标准,考察了本机构抽样的26批次细辛脑注射液的质量状况,完成了该品种性状、鉴别、pH值、颜色、有关物质、装量、可见异物及含量测定的项目评价。结果 抽检的26批次细辛脑注射液依标准检测,各项目均符合规定。结论 评价该品种整体质量较好,但考察到该品种有见光降解的性质,建议进一步优化生产、使用条件,减少曝光时长,避免杂质生成。

Objective In order to increase the supervision of asarone injection and ensure the safety of clinical use, the samples of asarone injection in circulation in the market were tested. Methods In this paper, according to the national drug standards approved by the State Food and Drug Administration, the quality of 26 batches of asarone injection sampled by the institute was investigated, and the project evaluation of the variety traits, identification, pH value, color, related substances, loading, visible foreign bodies and content determination was completed. Results 26 batches of asarone injection were tested according to the standard, and all items were in conformity with the regulations. Conclusion The overall quality of this variety is good, but the properties of light degradation are observed. It is suggested to further optimize the production and use conditions, reduce the exposure time and avoid the formation of impurities.

细辛脑(asarone), 化学名为 2,4,5-三甲氧基-1-丙烯苯, 又名 $\alpha$ -细辛脑、 $\alpha$ -细辛醚 [ 1 - 2 ] ,是中药石菖蒲的主要有效成分之一[ 3 - 4 ],临床上主要用于止咳、祛痰、平喘、镇静、解痉、抗惊厥、降血脂和降胆固醇等[ 5 - 6 ]。由于只是从中药中提取细辛脑,难以满足临床需求, 因此, 现在临床上使用的细辛脑注射液均为全合成制品[ 7 - 9 ]。近年来,临床上常有使用细辛脑注射液引发的不良反应 [ 10 - 15 ] ,为加强对细辛脑注射液的监管, 评估药物有效性, 保障人民的临床用药安全, 本机构对市场现行流通使用的细辛脑注射液进行了抽样, 依照制剂国家药品监督管理局标准进行检验, 评估细辛脑注射液的质量状况, 为临床用药提供参考。
LC-20A 高效液相色谱仪 (岛津企业管理 (中国) 有限公司)、CP225D 电子天平(北京赛多利斯仪器系统有限公司)、T9S 紫外分光光度计(北京普析通用仪器有限责任公司)、Metrohm pH 计(瑞士万通中国有限公司)
α-细辛脑对照品(中国食品药品检定研究院,批号 100298-201203, 含量: 100.0%)、甲醇为色谱纯、乙醇为分析纯、水为超纯水。
本次抽样涉及 10 个地市的 26 批检品, 包括流通环节 19 批,使用环节 5 批,生产环节 2 批,共涉及国内 5 家生产企业 $\left({\mathrm{A}\sim \mathrm{E}}\right),6$ 个批准文号,占总批准文号的 13%。规格:(1) $2\mathrm{\;{mL}}: 8\mathrm{\;{mg}}$ (23 批次);(2) $5\mathrm{\;{mL}}: {16}\mathrm{\;{mg}}$ (3 批次)。
细辛脑注射液现行质量标准为国家食品药品监督管理总局批准的国家药品标准 WS-10001-(HD-0437)-2002-2016。
本品应为无色至淡黄色的澄明液体。
26 批次细辛脑注射液均为无色至淡黄色的澄明液体。
其中 $2\mathrm{\;{mL}}: 8\mathrm{{mg}}$ 规格各厂家均使用无色低硼硅玻璃安瓿瓶包装, $5\mathrm{\;{mL}}: {16}\mathrm{{mg}}$ 规格均使用棕色低硼硅玻璃安瓿瓶包装, 将上述两种不同颜色安瓿瓶样品去除包装贴纸, 在自然光下照射 7 天, 可以观察到无色安瓿瓶包装的样品颜色变黄明显。
(1) 标准规定
供试品溶液主峰的保留时间应与对照品溶液主峰的保留时间一致。
(2) 测定结果
26 批次细辛脑注射液制成的供试品溶液主峰的保留时间均与对照品溶液主峰的保留时间一致。图谱见 图 1

取本品适量,用乙醇稀释制成每 $1\mathrm{\;{mL}}$ 中约含细辛脑 8 $\mu \mathrm{g}$ 的溶液,应在 ${313}\mathrm{\;{nm}}\pm 2\mathrm{\;{nm}}$${258}\mathrm{\;{nm}}\pm 2\mathrm{\;{nm}}$ 的波长处有最大吸收。
(3) 测定结果
26 批次细辛脑注射液,其中在 ${313}\mathrm{\;{nm}}\pm 2\mathrm{\;{nm}}$ 的波长处吸收,有 1 批次为 ${311}\mathrm{\;{nm}},6$ 批次为 ${312}\mathrm{\;{nm}},{18}$ 批次为 ${313}\mathrm{\;{nm}},1$ 批次为 ${314}\mathrm{\;{nm}}$ ; 在 ${258}\mathrm{\;{nm}}\pm 2\mathrm{\;{nm}}$ 的波长处吸收, 有 1 批次为 ${256}\mathrm{\;{nm}},{21}$ 批次为 ${257}\mathrm{\;{nm}},4$ 批次为 ${258}\mathrm{\;{nm}}$ ; 均符合规定,批间均一性较好。
pH 值应为 5.0~6.5。
26 批细辛脑注射液的 $\mathrm{{pH}}$ 值均在标准规定的区间范围内, B 企业批间差异分别为 1.0, RSD%达到 7.3%, 差异较大,考虑产品在生产工艺方面存在一定的问题, $\mathrm{C}$ 企业样品批间差异较小,产品质量相对较好, $\mathrm{A}\text{、}\mathrm{D}\text{、}\mathrm{E}$ 企业样品数量较少,这里不做批间比较。结果见 表 1
取本品,或取本品适量,用水稀释制成每 $1\mathrm{\;{mL}}$ 中含细辛脑 ${3.2}\mathrm{{mg}}$ 的溶液,与黄色 2 号标准比色液比较,不得更浓。
直接取$5\mathrm{\;{mL}}: {16}\mathrm{{mg}}$规格细辛脑注射液,或$2\mathrm{{mL}}: 8\mathrm{{mg}}$规格细辛脑注射液${4.0}\mathrm{\;{mL}}$${5.0}\mathrm{\;{mL}}$容量瓶用水定容至刻度摇匀,将上述溶液置《中国药典》2020 年版标准比色液中配套的空玻璃安瓿瓶中, 与标准比色液中黄色 2 号标准比色液同置白色背景前, 平视观察。26 批细辛脑注射液所显颜色均浅于黄色 2 号标准比色液颜色。
供试品溶液的色谱图中, 如有与杂质 I 保留时间一致的色谱峰, 其峰面积乘以 1.5 后不得大于对照溶液主峰面积 (1.0%), 其他单个杂质峰面积不得大于对照溶液主峰面积的 0.5 倍( 0.5 %),各杂质峰面积 ( 杂质 I 峰面积乘以 1.5 ) 的和不得大于对照溶液主峰面积的 1.5 倍(1.5%)。
26 批细辛脑注射液中有 13 批检出杂质 $\mathrm{I}$ ,含量为 0.04%~0.28%,有 15 批检出其他杂质,含量为 0.05%~0.3%, 26 批细辛脑注射液根据标准要求均符合规定,合格率 100%, E 厂家 3 批次检品均未检出杂质I,该厂家包装使用棕色低硼硅玻璃安瓿瓶,提示避光包装可以减少杂质I生成。 结果见 表 2
根据上述实验结果进一步测试, 将性状测定项下在自然光下照射 7 天处理的细辛脑注射液按有关物质项下供试品溶液的配制方法配制供试品溶液并进行有关物质测定, 在含量测定条件下,对上述光照样品中主成分 $\alpha$ -细辛脑进行了标示含量测定, 经计算观察到 26 批细辛脑注射液中总杂质含量均明显增加,大于 10%的有 18 批占 69%, 其中杂质I增加明显;标示含量测定结果中,仅有 3 批标示含量符合含量测定规定的 93.0%~107.0%范围,占 11.5%,结果见 表 3
26 批次细辛脑注射液各取 5 支,每支的装量均不得少于其规定的标示装量。
26 批次细辛脑注射液测定结果均符合规定, $2\mathrm{{mL}}$ 规格样品平均装量为 ${2.1}\mathrm{\;{mL}},5\mathrm{\;{mL}}$ 规格样品平均装量为 ${5.3}\mathrm{\;{mL}}$
26 批次细辛脑注射液各取 20 支,供试品中均不得检出明显可见异物。
取 26 批次细辛脑注射液均除去安瓿瓶标签,保证安瓿瓶没有破损, 擦拭安瓿瓶外壁, 在暗室中, 将无色和棕色安瓿瓶包装的供试品分置光照度为 ${1500}\mathrm{{lx}}$${3000}\mathrm{{lx}}$ 的光源下按照《中国药典》2020 年版中“灯检法”检定,均未检出明显可见异物。
本品含细辛脑 $\left({{\mathrm{C}}_{12}{\mathrm{H}}_{16}{\mathrm{O}}_{3}}\right)$ 应为标示量的 93.0%~107.0%。
26 批次细辛脑注射液含细辛脑为标示量的 94%~100.9%, 平均标示含量为 97.8%,均符合规定, RSD 为 2.1%。其中 C 厂家极差较大, 且 RSD 也较大, 考虑产品在生产工艺中存在不稳定因素,建议在制备工艺及储存条件上优化工艺。
经检验, 本机构抽检的 26 批细辛脑注射液, 按照规定检验标准完成检验, 所检项目均符合规定, 产品整体质量较好。但在检验过程中发现, 该品种对光照敏感, 随着放置时间延长,细辛脑注射液中杂质含量会增加,建议在生产、使用过程中优化生产条件,减少曝光时长。
