Objective To analyze the molecular epidemiological characteristics, virulence gene profiles, and distribution of antibiotic resistance genes of Vibrio parahaemolyticus (VP) isolated from foodborne diseases patients in Dalian City, Liaoning Province. Methods A total of 63 VP clinical isolates were collected from Dalian City in 2023. The whole genome sequencing, multilocus sequence typing, virulence genes and drug resistance genes were analyzed, and the clinical epidemiological data were combined for comprehensive analysis. Results July-September was the main epidemic period of VP. The results of sequence type (ST) showed that ST3 was the main epidemic group. The 63 strains carried both tlh and vcrD, and the carrying rate of tdh and vcrD2 virulence genes was 95.24%. All strains carried blaCARB-47, CARB-20, tet (35), tet (34) and CRP resistance genes, and one ST3 strain also carried aminoglycosides, sulfonamides and other resistance genes. There was a certain correlation between virulence gene carrying type and ST type, but there was no obvious clustering feature in the distribution of drug resistance genes. All isolates from patients with systemic symptoms carried tdh and vcrD2. Conclusion ST3 is the main epidemic group of VP in Dalian, Liaoning Province, which is widely distributed and has a high proportion of virulence genes. Speical attention should be paid to the risk of drug resistance transmission of ST3. This study provides a molecular epidemiological basis for the prevention and control of VP.

目的 分析辽宁省大连市食源性疾病患者来源的副溶血性弧菌(Vibrio parahaemolyticus, VP)分子流行特点, 毒力基因及耐药基因的分布特征。方法 收集了2023年大连市63株VP临床分离株, 借助全基因组测序, 多位点序列分型、毒力基因以及耐药基因分析, 同时结合临床流行病学数据进化全面分析。结果 7—9月为VP的主要流行周期。序列分型(sequence type, ST)结果显示, ST3型为主要流行群。63株菌同时携带tlhvcrD, tdhvcrD2毒力基因的携带率为95.23%。所有菌株均携带blaCARB-47CARB-20tet (35)、tet (34)以及CRP耐药基因, 其中1株ST3型菌株还携带氨基糖苷类、磺酰胺类等耐药基因。毒力基因携带类型和ST型存在一定关联, 但耐药基因分布没有明显的聚类特征。全身症状患者的分离株均携带tdhvcrD2结论 ST3型是辽宁省大连市VP的主要流行群, 广泛分布且携带毒力基因比例较高。并密切关注ST3型的耐药性传播风险。本研究为VP的防控提供了分子流行病学依据。

目的 分析辽宁省大连市食源性疾病患者来源的副溶血性弧菌(Vibrio parahaemolyticus, VP)分子流行特点, 毒力基因及耐药基因的分布特征。方法 收集了2023年大连市63株VP临床分离株, 借助全基因组测序, 多位点序列分型、毒力基因以及耐药基因分析, 同时结合临床流行病学数据进化全面分析。结果 7—9月为VP的主要流行周期。序列分型(sequence type, ST)结果显示, ST3型为主要流行群。63株菌同时携带tlhvcrD, tdhvcrD2毒力基因的携带率为95.23%。所有菌株均携带blaCARB-47CARB-20tet (35)、tet (34)以及CRP耐药基因, 其中1株ST3型菌株还携带氨基糖苷类、磺酰胺类等耐药基因。毒力基因携带类型和ST型存在一定关联, 但耐药基因分布没有明显的聚类特征。全身症状患者的分离株均携带tdhvcrD2结论 ST3型是辽宁省大连市VP的主要流行群, 广泛分布且携带毒力基因比例较高。并密切关注ST3型的耐药性传播风险。本研究为VP的防控提供了分子流行病学依据。

Objective To analyze the molecular epidemiological characteristics, virulence gene profiles, and distribution of antibiotic resistance genes of Vibrio parahaemolyticus (VP) isolated from foodborne diseases patients in Dalian City, Liaoning Province. Methods A total of 63 VP clinical isolates were collected from Dalian City in 2023. The whole genome sequencing, multilocus sequence typing, virulence genes and drug resistance genes were analyzed, and the clinical epidemiological data were combined for comprehensive analysis. Results July-September was the main epidemic period of VP. The results of sequence type (ST) showed that ST3 was the main epidemic group. The 63 strains carried both tlh and vcrD, and the carrying rate of tdh and vcrD2 virulence genes was 95.24%. All strains carried blaCARB-47, CARB-20, tet (35), tet (34) and CRP resistance genes, and one ST3 strain also carried aminoglycosides, sulfonamides and other resistance genes. There was a certain correlation between virulence gene carrying type and ST type, but there was no obvious clustering feature in the distribution of drug resistance genes. All isolates from patients with systemic symptoms carried tdh and vcrD2. Conclusion ST3 is the main epidemic group of VP in Dalian, Liaoning Province, which is widely distributed and has a high proportion of virulence genes. Speical attention should be paid to the risk of drug resistance transmission of ST3. This study provides a molecular epidemiological basis for the prevention and control of VP.

副溶血性弧菌(Vibrio parahaemolyticus, VP)作为一种广泛存在于河口及海洋生态系统中的革兰氏阴性嗜盐菌, 其主要感染途径是通过摄入生食的海产品[1-2]。当人体免疫力低下时, 该病原菌可能引发创伤感染并进一步发展为全身性败血症[3-5]。随着海产品流通网络的不断扩展, 该菌正从沿海地区向内陆扩散, 引发的食源性疾病发病率已超越沙门氏菌和志贺氏菌, 连续多年成为我国散发性腹泻的首要致病菌[6-7]。在辽宁省大连市、营口市等沿海区域, 副溶血性弧菌已成为危害最严重的食源性病原, 中毒事件频发[8-9]
全基因组测序(whole genome sequencing, WGS) 技术的普及为微生物研究提供了新的技术手段。通过对测序数据的生物信息学分析, 能够快速解析细菌群体的基因型特征[10-12]。本研究基于WGS技术获得的副溶血性弧菌全基因组序列, 综合运用多位点序列分型(multilocus sequence typing, MLST)、毒力基因检测及耐药基因分析等方法, 系统比较不同菌株的遗传特征, 并探讨各分析结果之间的内在联系, 旨在揭示辽宁地区副溶血性弧菌的流行规律与致病特性。通过系统评估2023年辽宁省食源性疾病监测中分离的副溶血性弧菌的流行趋势、传播路径、临床分离株与食品分离株的遗传相似性分布情况, 以及菌株的致病性特征和分子流行规律, 为制定精准的防控策略提供科学依据。
食源性疾病病例定义为由食品或怀疑由食品引起的以腹泻症状为主诉的就诊病例, 24 h排便≥3次, 且粪便性状异常, 如稀便、水样便、黏液便或脓血便等; 同时排除服用抗生素或不恰当服用化学物质导致腹泻的病例[13]。收集2023年辽宁省大连市各哨点医院肠道门诊食源性疾病病例粪便标本, 由医院实验室负责病原检测, 最终经辽宁省疾控中心复核鉴定, 共获得63株副溶血性弧菌。
SSNP-9600A全自动核酸提取仪(江苏硕世生物科技有限公司); IMH100-SSS恒温培养箱(美国赛默飞世尔科技公司); Milli-Q纯水仪(德国默克密理博公司); VITEK2全自动微生物生化鉴定系统(法国梅里埃生物公司)。
3%碱性蛋白胨水(alkaline peptone water, APW)、3%胰蛋白胨大豆琼脂(tryptic soy agar, TSA)(北京陆桥技术有限公司); 弧菌显色培养基(法国科马嘉公司); 细菌DNA提取试剂盒(江苏硕世生物科技有限公司)。以上试剂均经过质控并且在有效期内使用。
利用3% APW选择性增菌液对副溶血性弧菌磁珠进行增菌, 将增菌液划线接种科马嘉显色平板进行筛选, 挑取可疑菌落接种于3% TSA平板进行纯化培养, 将纯培养后的菌落利用全自动微生物生化鉴定系统进行确证实验。
通过全自动核酸提取仪提取副溶血性弧菌核酸。利用二代测序平台Illumina平台, 构建的样品插入片段小于400 bp, 进行PE150 (双端测序)测序, 即双端测序, 单端测序读长为150 bp。使用SOAP de novo (v2.04)[14-15]、SPAdes[16]和ABySS[17]组装软件对二代测序后的优化序列进行多个Kmer参数的拼接, 得到最优的连续克隆系组装结果, 然后将测序片段比对到重叠群上。根据测序片段的双端和重叠关系, 对组装结果进行部分拼接和优化, 形成支架。
将测序结果经拼接组装后, 上传至基因组流行病学中心MLST分析平台(https://cge.food.dtu.dk/services/MLST/), 得到菌株的MLST型。确定7等位基因序号构成的序列型(sequence type, ST)。
将测序所获得的基因组在病原菌毒力因子数据库(virulence factor data base, VFDB)[18]中进行毒力基因的比对, 获得毒力因子基因的注释并进行统计分析, 以了解tlh、tdh、vcrD、vcrD2 4种毒力基因的携带情况。
使用ABRicate v0.5 (https://github.com/tseemann/abricate) 软件, 将基因组序列与ResFinder v4.0[19]数据库中的耐药基因进行比对, 查找菌株所携带的耐药基因, 参数设置中基因覆盖度及一致性阈值均为80%。
利用BioNumerics 8.0软件, 将63株菌的7个等位基因序号构成的ST字符型数据, 通过非加权分组平均法(unweighted pair group method with arithmetic mean, UPGMA), 对63株菌的MLST型别进行聚类分析, 生成相似度矩阵和最小生成树。
2023年大连地区, 因副溶血性弧菌导致的食源性疾病均集中在7—9月, 其中8月患者数量最多为38人, 占比60.32%, 其次为7月份, 共19人, 占比30.16%。见表1
63名患者食用污染副溶血性弧菌的食品类别, 其中因食用水产动物及其制品(包括熟制和生食等)而致病的患者人数最多(47.62%), 其次是食用肉与肉制品的患者(22.22%)。见表2
进食场所的患者分布, 其中在酒店进食的人数最多(36.51%), 其次为在家庭进食的人数(34.92%)。见表3
患者购买受污染食品的场所分布, 其中酒店占比最高(28.57%), 其次为农贸市场(19.05%)。见表4
63株副溶血性弧菌分离株均携带5种相同的耐药基因, 分别是β内酰胺酶类耐药基因blaCARB-47CARB-20, 四环素耐药基因tet (35)和tet (34), 多重耐药外排泵基因CRP。而编号为ln041116-2023-00071的分离株, 除携带以上5种耐药基因以外, 还携带氨基糖苷类耐药基因aadA1AAC(6’)-Ib7, 苯酚类耐药基因catB8catA3, 磺酰胺类耐药基因sul2sul1以及β内酰胺酶类耐药基因blaPER-1, 该分离株的ST型为ST3型, 携带tlhtdhvcrDvcrD2 4种毒力基因。
经VFDB数据库比对, 63株副溶血性弧菌均携带tlhvcrD毒力基因, 不携带trhtoxRman7vcrD1毒力基因, tdhvcrD2基因的携带率均为95.23%。见表5
表6所示, 63株副溶血性弧菌分离株的毒力基因携带类型, 共存在2种形式, 一种是tlh+, trh-, tdh+, vcrD+, vcrD2+, 共60株, 占比95.23%, 另一种毒力基因携带类型为tlh+, trh-, tdh-, vcrD+, vcrD2-, 有3株。
63株菌被分为5个ST型, 最多的ST型为ST3, 有57株, 占比90.48%, ST121和ST527型均为2株, 占比均为3.17%, ST332和未分型ST型均有1株, 占比均为1.59%。经过本次结果比对, ST3、ST332和ST527型的分离株均携带tlhtdhvcrDvcrD2毒力基因, 而ST121和未分型ST型的分离株携带tlhvcrD毒力基因, 而不携带tdhvcrD2毒力基因(见表7)。
通过ST型聚类图显示, 共聚类成4簇, 第一簇中有ST3和ST121型, 但两者距离较远, 相似性较小, ST332、ST527和未分型ST型各成一簇, 相互之间关联较小。从最小生成树图可以看出, 5种ST型分布较分散, 相互间的距离较远, 差异较大。见图12
进食不同食品类别患者的分离株中, 进食多种食品、粮食类及其制品、乳与乳制品、蔬菜及其制品、水果类及其制品和饮料与冷冻饮品患者的分离株中, 均100%携带tlhtdhvcrDvcrD2毒力基因, 而进食肉与肉制品患者的分离株中, tdhvcrD2基因的携带率均为92.86%, 进食水产动物及其制品患者的分离株tdhvcrD2基因的携带率均为93.33%, ST332型单独存在于进食多种食品的患者分离株中, 未分型ST型单独存在于进食肉与肉制品的患者分离株中, ST527型同时存在于进食水产动物及其制品和饮料与冷冻饮品的患者分离株中, ST121型只存在于进食水产动物及其制品的患者分离株中, 而ST3型广泛存在于进食各类食品的患者分离株中。见表8
在不同地区县进食的患者分离株中, 在金州区、沙河口区、瓦房店市、西岗区和长海县进食的患者分离株, 均100%携带tlhtdhvcrDvcrD2毒力基因, 而甘井子区患者的分离株, tdhvcrD2基因的携带率均为92.31%, 中山区患者的分离株, tdhvcrD2基因的携带率均为93.33%, 高新园区患者的分离株不携带tdhvcrD2基因, 高新园区分离株的ST为未分型ST型。
ST121型在甘井子区和中山区进食的患者中均有分离, 未分型ST型的菌株分离于在高新园区进食的患者, ST527型菌株分离于在沙河口区进食的患者, ST332型菌株分离于中山区进食的患者, 而ST3型的菌株除在高新园区未分离出, 在其他各区县均有分离。见表9
患者的消化系统症状以水样便和稀便为主, 其中以水样便症状的占比较多, 稀便症状的患者较少, 水样便中每天排便5次的患者最多, 占比19.05%, 其次为排便10次的患者, 为15.87%。结果显示, 每天排8次水样便的患者, 有既往病史的比例较高, 为40%, 其次为每天排5次和7次水样便的患者, 分别为16.67%。不同消化系统症状的ST型, 均以ST3型为主。各消化系统症状的患者分离株, 均携带tlhvcrD毒力基因, 而tdhvcrD2的携带情况, 每天排便5次水样便的患者分离株携带率分别为91.67%, 每天排便4次水样便的患者分离株携带率分别为77.78%, 其他菌株均携带tdhvcrD2毒力基因。见表10
患者绝大多数无明显全身症状, 占比为80.95%, 部分患者存在全身症状, 以发热、乏力和脱水为主, 其中以乏力症状的患者最多, 占比7.94%, 结果显示, 有全身症状的患者均无既往病史。发热症状患者分离株的ST型, 包括ST3和ST332型, 乏力症状患者分离株的ST型包括ST3和ST527型, 脱水症状患者分离株的ST型均为ST3型, 而无全身症状患者的分离株包括4种ST型。存在全身症状患者的分离株均携带tdhvcrD2毒力基因, 而无全身症状患者的分离株, tdhvcrD2的携带率均为94.12%。见表11
本研究通过对2023年大连市63例副溶血性弧菌感染病例的流行病学特征进行系统性分析表明, 所有感染事件均集中在7—9月, 这一现象与既往文献报道的夏季高温环境下海产品冷链运输保障不足导致致病菌污染风险升高的结论高度契合[20-23]。该季节性分布规律与我国其他区域的监测结果具有一致性, 可能与高温高湿环境更利于微生物增殖的特性密切相关。监测数据同时显示, 受污染食品主要集中在水产及其制品、肉与肉制品等高蛋白易腐品类, 这与辽宁省历史监测数据呈现的污染规律保持一致[24]。为有效防控食源性风险, 需持续强化食品生产加工环节的卫生监管, 严格执行体系要求, 确保相关产品符合食品安全国家标准。从暴露场所分析来看, 酒店餐饮场所作为重要的感染传播媒介, 应被列为重点监管对象。建议建立食材溯源管理制度、从业人员健康档案和操作规范培训机制, 通过全流程质量控制预防群体性食源性疾病发生。在供应链源头管控方面, 应着重加强农贸市场等初级农产品交易场所的卫生管理, 建立致病菌污染预警机制, 切断污染食品进入流通环节的潜在路径。
在既往耐药基因研究中[25], 辽宁省水产养殖环境分离的副溶血性弧菌已被证实广泛携带blaCARB基因。本研究进一步发现, 临床患者来源的所有VP分离株均携带该耐药基因, 这一现象表明该基因在辽宁省VP分离株群体中呈现高度流行态势。值得关注的是, 现代水产养殖业中抗生素的长期大量使用可能构成重要的选择压力, 尤其是亚抑菌浓度抗生素的持续暴露, 极有可能通过诱导基因突变或促进耐药基因横向转移等机制, 推动该耐药基因在VP种群中的快速传播与稳定遗传[26-28]
本研究主要检测副溶血性弧菌中tlh、tdh、vcrD、vcrD2 4种毒力基因的携带情况, 其中vcrD基因作为三型分泌系统(T3SS)的重要组成部分, 其主要功能包括诱导宿主细胞自噬、形态学改变及细胞膜完整性破坏[29-32]tdh编码的耐热直接溶血素蛋白通过形成极性四聚体结构, 与真核细胞膜结合后引发跨膜离子失衡, 被认为是导致腹泻症状的关键毒力因子。而tlh基因产物则通过调控细胞凋亡、膜溶解及坏死等多种病理过程参与感染进程[33], 被认为是副溶血弧菌的特异性基因, 所有的副溶血弧菌都会产生tlh基因。本研究中, 63株副溶血性弧菌均100.00%携带tlhvcrD毒力基因, tdhvcrD2基因的携带率均为95.23%。这与临床来源的副溶血性弧菌分离株中90%为tdh一致[34]。在毒力基因与消化系统症状的关联性上, 无法推断本研究中的毒力基因和消化系统症状之间的具体关系, 应加大样本量, 扩大监测地区, 增加监测年限, 以探究毒力基因对消化系统症状的影响情况。而在全身症状方面, 由于存在全身症状患者的分离株均完全携带tdhvcrD2毒力基因, 而无全身症状患者的分离株不完全携带tdhvcrD2毒力基因, 推断不携带tdhvcrD2毒力基因的菌株与较轻的全身症状有一定关联性, 但需小鼠毒力实验进一步证明。
通过对63株副溶血性弧菌进行多位点序列分型, 结果显示共分为5个ST型, 最多的ST型为ST3, 占比90.48%, ST121和ST527型占比均为3.17%, ST332和未分型ST型占比均为1.59%, 由此推断2023年大连市流行的副溶血性弧菌主要为ST3型。通过ST型聚类图和最小生成树图可以看出, 各ST型相似性较低, 差异较大, 但多种ST型可以存在于同一类食品类别中, 也可在同一区县内流行, 部分ST型可能与特定食品类别、地区相关。结合毒力基因结果, ST3、ST332和ST527型的分离株均携带tlhtdhvcrDvcrD2毒力基因, 而ST121和未分型ST型的分离株携带tlhvcrD毒力基因, 而不携带tdhvcrD2毒力基因, 这一现象暗示毒力基因组合与序列型别之间可能存在潜在关联性, 但该结论仍需扩大样本量进行验证, 以明确两者间的统计学相关性。再结合耐药基因结果, 各ST型的分离株携带耐药基因的情况基本一致, 虽各ST型亲缘关系较远, 但其携带的耐药基因没有明显的聚类和独立的特征。将ST型与临床症状相结合, 各ST型引起的消化系统症状和全身症状无明显的特点, 需增加样本量进一步探究各ST型引起的临床症状是否存在一定的趋势和聚类特点。
本研究对2023年大连食源性疾病中的副溶血性弧菌展开分析。结果显示, 7—9月是流行高峰期, ST3型为主要流行株, 且广泛分布, 毒力基因携带率较高。不同食品类别、地区县的患者分离株ST型存在差异, 部分ST型与特定食品类别、地区相关。消化系统症状以水样便居多, 各症状ST型以ST3为主, 毒力基因与消化系统症状关系待明确。全身症状中多数患者无明显症状, 有症状者以乏力为主。因此, 应加强夏季监管, 关注ST3型菌株的耐药风险, 为防控提供依据。
