The microbiota in the gastrointestinal tract of humans and animals exhibite diverse characteristics, with microbial communities residing in different ecological niches displaying distinct spatial differences, often characterize by high host specificity. In addition to the compositional differences observe along the longitudinal axis of the gastrointestinal tract, microbes also form discrete characteristic communities within specific microenvironments. This review explored how the physical environment of the gastrointestinal tract affaceded the spatial formation and maintenance of microbial communities, the role of diet in shaping the spatial distribution of gut microbiota, and the importance of this spatial heterogeneity in maintaining organismal health. Focusing on the spatial heterogeneity of gastrointestinal microbiota enables a more comprehensive understanding of the adaptive strategies and biological functions of microbes in different locations and local environments in the gastrointestinal tract. This approach contributed to a better understanding of the role of the gut microbiota in health and disease, providing new theoretical foundations for maintaining gut health, preventing and treating diseases, and developing personalized medical strategies.

人类和动物消化道内的微生物群落呈现出多样化的特征, 不同生态位栖息的微生物群落具有空间集合的差异, 通常具有高度的宿主特异性。除了沿胃肠道纵轴呈现出的微生物组成差异特征外, 微生物还会在特定环境形成离散的特征群落。本文探讨了胃肠道环境如何在空间上影响微生物群落的形成和稳态, 膳食影响胃肠道微生物空间分布的机制, 以及这种空间异质性在维持机体健康中的重要性。通过关注胃肠道微生物的空间异质性, 能够更全面地探索微生物在胃肠道不同位置和局部环境中的适应策略及其生物学功能, 有助于理解胃肠道微生物群在健康与疾病中的作用, 为肠道健康的维护、疾病的预防与治疗以及个体化医疗方案的制定提供了新的理论依据。

The microbiota in the gastrointestinal tract of humans and animals exhibite diverse characteristics, with microbial communities residing in different ecological niches displaying distinct spatial differences, often characterize by high host specificity. In addition to the compositional differences observe along the longitudinal axis of the gastrointestinal tract, microbes also form discrete characteristic communities within specific microenvironments. This review explored how the physical environment of the gastrointestinal tract affaceded the spatial formation and maintenance of microbial communities, the role of diet in shaping the spatial distribution of gut microbiota, and the importance of this spatial heterogeneity in maintaining organismal health. Focusing on the spatial heterogeneity of gastrointestinal microbiota enables a more comprehensive understanding of the adaptive strategies and biological functions of microbes in different locations and local environments in the gastrointestinal tract. This approach contributed to a better understanding of the role of the gut microbiota in health and disease, providing new theoretical foundations for maintaining gut health, preventing and treating diseases, and developing personalized medical strategies.

人类和动物的胃肠道里存在着数万亿的细菌、真菌、古菌及病毒等微生物, 它们共同构成了一个庞大且复杂的生态系统, 并且通过与宿主的相互作用建立起精密的共生关系[1]。越来越多的研究表明, 调控肠道微生物的组成和功能, 不仅能够改善胃肠道健康, 还可以有效缓解甚至治愈相关疾病。哺乳动物胃肠道按照解剖结构可分为胃、小肠、盲肠和结肠, 这一结构类似于自然环境中的生态系统, 在不同的区域和微环境中栖息着特定的微生物群落。在个体中, 饮食[2]、药物[3]、疾病状态[4]和卫生[5]等外部因素是影响胃肠道微生物群落组成和多样性的主要原因。除此之外, 胃肠道内微生物的分布还受到pH[6]、氧分压[7]、蠕动剪切力[8]、抗菌肽[9]、细菌黏附[10]因素的调控, 这些因素共同决定了微生物在胃肠道不同空间位置上的定植, 使得不同部位的微生物群落在功能上呈现出显著差异[11]。这些区域性差异不仅体现了肠道微生物对宿主环境的适应性, 也揭示了它们在肠道功能维持、代谢调控以及免疫反应中的关键作用。
目前, 胃肠道微生物学的研究主要通过粪便样本来解析微生物群的组成与功能。这种方法系统揭示了胃肠道微生物与宿主健康之间的重要联系, 但它忽视了内部环境的空间复杂性和胃肠道微生物及其群落的空间异质特征。因此, 深入研究胃肠道微生物的空间分布, 不仅有助于深入揭示微生物在胃肠道的分布规律及其功能特征, 还能深化对微生物生态学及其与宿主相互作用的理解。通过关注胃肠道微生物的空间异质性, 能够更全面地探索微生物在胃肠道不同位置和局部环境中的适应策略及其生物学功能。这对于解释微生物群落如何通过分区协作来维持肠道稳态具有重要意义。本文探讨胃肠道微生物群的分布特点及其对宿主健康的影响, 重点分析不同胃肠道区域内微生物群落的差异以及这些差异如何受到膳食等多种因素的影响, 揭示它们在维持宿主健康、调节免疫和代谢方面的重要作用。本文将为肠道微生物空间分布提供新的见解, 并为肠道健康的维护、疾病的预防与治疗以及个体化医疗方案的制定提供了新的理论依据。
图1所示, 哺乳动物消化道包括胃、小肠(包括十二指肠、空肠和回肠)、盲肠和结肠, 这些部位各自构成了独特的生态位, 为微生物群落的定植提供了特定的栖息环境。沿着消化道纵轴方向的生理变化, 尤其是化学和营养梯度的逐步变化, 以及宿主局部免疫活动的差异, 这些因素共同塑造了细菌群落的纵向分布及组成特征。
对于微生物来说, 胃部环境通常被认为是“恶劣”的栖息地[13], 高度酸性的内部环境[14]、抗菌酶以及免疫因子的作用阻碍了胃部微生物群的生长[15]。尽管如此, 胃部仍然生存着特定的微生物群落, 主要由变形菌门(Proteobacteria)、厚壁菌门(Proteobacteria)、拟杆菌门(Bacteroidetes)组成。幽门螺杆菌(Helicobacter pylori)是在胃部最早被发现的微生物, 它能够通过自身趋化性运动, 穿透黏液层, 定植在较为中性的黏液层深处的胃黏膜腺体内部[16-17], 并通过水解尿素产生尿素酶来调节局部酸度[18], 为自身定植提供保护的同时改变常驻微生物的栖息地, 这种微生态的变化通常会导致慢性萎缩性胃炎、胃溃疡和胃癌的发生[16]。胃部的微生物群远比之前认识的更加多样化, 随着分子生物学的发展和新一代测序技术的出现, 研究人员对胃部微生物群的组成和功能有了更深入的了解, 揭示了胃中更多元的微生物种类及功能, 例如, 乳杆菌(Lactobacillus)作为益生菌, 能够抑制小鼠幽门螺杆菌感染, 保护胃黏膜, 对胃部健康具有积极作用[19]
小肠包括十二指肠、空肠和回肠, 是肠道中最长的部分, 营养物质在此处完成大部分的消化和吸收过程, 其独特的生理结构使得内部微生物群落分布呈现出显著空间异质性。由胃进入十二指肠, 此时内容物由于胃酸的存在仍然呈酸性[20]。另外, 十二指肠组织的高度血管化特性[21]以及小肠中绒毛的存在[22]大大增加了黏膜表面积, 进而导致该区域在胃肠道中拥有最高的氧分压[23]。加之高浓度胆汁的抗菌作用, 微生物种类和丰度较低, 主要以能够适应高酸度和胆汁的环境的乳杆菌属(Lactobacillus)和双歧杆菌属(Bifidobacterium)为主[24]。从十二指肠到回肠, pH逐渐升高至7.5左右[25], 氧分压逐渐降低, 并且由于胆汁逐渐稀释及肠道内容物较长的停留时间[26], 微生物的生长环境变得更加温和适宜, 微生物的丰度和多样性显著增加, 特别是拟杆菌属(Bacteroides), 它们能够有效分解复杂多糖[27],并通过发酵产生短链脂肪酸(short-chain fatty acids, SCFAs)、吲哚衍生物和共轭脂肪酸等代谢产物。这些代谢产物不仅为肠上皮细胞提供能量, 还在增强肠道屏障功能方面发挥重要作用[28]。值得注意的是, 与结肠微生物群相比, 小肠微生物群表现出更明显的波动性, 在短时间内甚至同一天的上午和下午也被检测出存在差异[29]。此外, 小肠微生物群失衡会引起小肠细菌过度增长、小肠真菌过度增长以及肠易激综合征等肠道疾病[30]
盲肠和结肠肠腔处于低氧状态, 为拟杆菌、梭状芽孢杆菌(Clostridium)等厌氧微生物的长期定植提供了理想条件。
盲肠是小肠和结肠的过渡区域, 几乎无法检测到氧气[31], pH较小肠发生下降, 保持在6~7之间[7]。结肠内容物较慢的流动速度减缓了细菌的流动速度[8], 导致细菌通过结肠的传输时间比通过其他肠道区域时间更长, 这使得结肠拥有丰富的微生物群落。膳食纤维和复杂多糖在此经过肠道微生物的发酵, 产生SCFAs, 使结肠成为微生物最为丰富的区域[32]。在结肠中, 微生物群以拟杆菌门、厚壁菌门和假单胞菌门(Proteobacteria)为主, 特别是拟杆菌科(Bacteroidaceae)和肠杆菌科(Enterobacteriaceae), 在结肠微生物群中占主导地位。而小鼠结肠中鼠尾菌科占主导地位, 同时拟杆菌科和普雷沃氏菌科(Prevotellaceae)的丰度也较高[33]。此外, 厚壁菌门和拟杆菌门的比值增加被认为可能与代谢疾病的发生密切相关, 尤其是肥胖等代谢疾病的发生, 对宿主的健康有重要影响[34]。此外, 结肠微生物群的失衡也与肠道炎症性疾病的发生密切相关, 大肠杆菌(Escherichia coli)的丰度增加则通常被认为与结肠炎症相关[35]。结肠炎症可能导致肠道屏障功能受损, 增加肠道通透性, 从而引发或加剧慢性炎症性肠病等疾病。另一方面拟杆菌科能有效分解复杂碳水化合物产生SCFAs, 对调节免疫维持肠道健康至关重要[36]。基于16S rRNA扩增子测序的研究揭示了结肠中微生物丰度的梯度模式, 例如拟杆菌属和肠杆菌科的丰度在结肠远端的丰度较高, 而肠球菌属(Enterococcus)和链球菌属(Streptococcus)的丰度则相对较低, 这一现象反映了微生物群落的纵向生物地理模式以及其在空间分布上的显著异质性[37]
肠道微生物群与宿主之间相互作用的关键区域是肠腔界面, 这一界面不仅包括肠腔与肠上皮细胞的接触区域, 还涉及到微生物群落与肠道上皮细胞之间的互动。结肠上皮细胞通过增殖、迁移和分化形成隐窝, 并分泌黏液, 进一步构建了微生物群落的栖息环境, 使得微生物群在横截面上呈现出显著的异质性。隐窝上的杯状细胞不断分泌O-糖基化Muc2形成凝胶状黏液层[38], 将肠道上皮组织与肠腔内容物分隔开。与小肠不同的是, 结肠黏液层明显分为两层(图2): 内层致密坚固, 几乎没有细菌, 可以作为细菌无法穿透的物理屏障[40]; 外层则松散, 是肠道细菌的主要栖息环境, 共同维持微生物群的空间结构。有研究表明, 与肠腔群落相比, 黏膜中放线菌门(Actinobacteria)和变形菌门高度富集[41], 且在相邻黏膜样本之间表现出组成差异[42]。黏液的黏度从近端结肠到远端结肠逐渐增加, 这使得黏液相关的细菌在近端结肠的丰度高于远端结肠[43]。黏液层的异质性导致了微生物群落的空间结构化, PÉDRON等[44]使用激光捕获显微切割技术细分结肠组织, 描绘离散区域微生物群落的组成, 观察到在结肠肠腔和黏液微环境之间存在种间差异。利用荧光原位杂交和16S rRNA测序相结合的方法, DUNCAN等[45]发现一个类似生物膜的密集厌氧微生物群落嵌入在黏液层中。随着技术的进步, 未来对微生物群的理解将更多的来自于关注肠道生态系统内的“微栖息地与微生物间”以及“微生物与宿主间”的空间组织关系。
微生物生态位是指微生物在栖息环境中所占据的特定位置或扮演的功能角色, 涉及微生物与其生存环境之间的相互作用、资源使用、空间占据以及与其他生物体的关系。在肠道这一复杂的微生态系统中, 微生物群落通过争夺有限的空间、营养物质以及与宿主免疫系统的相互作用来维持生态平衡[46]。每个微生物群落在胃肠道内都扮演着独特的角色, 微生物之间的相互作用对于维持机体健康至关重要。在健康个体中, 共生菌通过调节葡萄糖酸盐的可用性, 促进大肠杆菌和克雷伯氏菌属(Klebsiella)在肠道内的定植, 从而减少小鼠肠道炎症[47]。乳杆菌属和双歧杆菌属则降了低肠道环境的pH, 创造一个对沙门氏菌(Salmonella)等病原体生长不利的环境[48]。它们之间形成了相互依存的生态网络, 抑制病原微生物的入侵, 维持胃肠道生态的稳定。然而, 当肠道微生物组的平衡被打破时, 机会病原体便可侵入并占据空缺的生态位进行定植, 进而改变肠道环境并产生潜在有害的失调代谢物, 这些变化可能引起一系列疾病的发生, 例如艰难梭菌(Clostridioides difficile), 通常存在于正常的肠道微生物群中, 但当肠道微生态遭到破坏时, 它就会变得致病, 破坏细胞骨架和结肠上皮屏障的完整性, 诱导异常的炎症反应和细胞死亡[49]
肠道微生物群的个体差异性与饮食中的营养成分之间存在着错综复杂的关系, 饮食作为肠道微生物群落的重要调节因子, 通过多种途径影响着微生物的生长、定植以及群落结构。饮食中的营养成分不仅是肠道微生物的能量来源, 还影响其代谢产物的类型, 进而在肠道内产生深远的生态效应。结肠是肠道中微生物群落最为丰富的区域, 许多微生物依赖于发酵上消化道无法分解的膳食纤维等复杂多糖, 产生SCFAs等多种代谢产物来维持肠道稳态[50]。地中海饮食模式下, 肠道中拟杆菌丰度增加, 它拥有丰富的多糖降解基因, 能够通过不同的多糖降解途径, 降解并利用膳食纤维等复杂多糖[51]。而当膳食纤维缺乏时, 微生物的能量获取来源发生改变, 某些微生物的丰度下降, 而嗜黏蛋白阿克曼菌(Akkermansia)和多形拟杆菌(Prevotella)在人体和小鼠的黏液层中表现出较高的丰度[52], 它能够转变自身代谢途径, 利用黏液中黏蛋白的复杂O-糖基作为营养来源实现在小鼠肠道中竞争性定植[53]。类似地, 在小鼠的肠道炎症期间, 随着黏液不断被利用, 黏液降解菌产酸拟杆菌的丰度也相应增加[54]。因此, 具备利用黏液作为碳源的能力使得一些细菌能够在宿主体内长期稳定地定植。除此之外, 动物蛋白含量高的饮食会增加拟杆菌属、另枝菌属(Alistipes)、嗜胆菌属(Bilophila)的丰度, 同时减少乳杆菌属、罗氏菌属(Ruminococcus)[55]。而西方饮食饮食中高脂肪、低纤维的饮食结构会增加厚壁菌门和拟杆菌门的比值, 引起肠道屏障损伤, 发生肠道菌群的易位, 从而使微生物群的空间分布出现不平衡[56-57]
肠道中的抗菌肽在维持肠道微生态平衡、调节免疫反应以及防御病原入侵等方面发挥着至关重要的作用。它们通过隔离关键生长营养物质、破坏细菌膜等机制发挥多种抗菌活性, 抑制病原菌的定植和繁殖。潘氏细胞和肠上皮细胞是抗菌肽的主要来源, 特别是定位于小肠隐窝中的潘氏细胞, 它们与杯状细胞产生的黏蛋白共同作用, 在肠上皮细胞与肠道微生物之间形成了一道物理屏障。这些抗菌肽与带负电荷的细菌膜相互作用并破坏其结构, 从而抑制细菌定植[58]。不同类型的抗菌肽在肠道中的浓度和分布有所不同, 通常在小肠近端拥有较高的浓度, 沿小肠纵轴逐渐降低, 这也导致了远端肠道的细菌丰度和多样性相对较高。凝集素Reg IIIγ通过结合并破坏革兰氏阳性菌的肽聚糖, 防止微生物入侵肠道上皮组织[59]。MIKI等[60]通过对肠道横切片分析发现, Reg IIIβ表达不足会通过阻碍正常肠道微生物群稳态的恢复, 使肠道鼠伤寒沙门菌感染持续时间延长。此外, 潘氏细胞分泌的α-防御素直接与细菌膜结合, 而中性粒细胞释放的β-防御素激活局部免疫反应进一步强化肠道的抵抗力, 限制病原细菌接近上皮[61]
研究表明膳食的组成与抗菌肽的表达水平密切相关, 高纤维饮食通过发酵产物SCFAs激活肠道上皮细胞及免疫细胞中的特定受体, 促进防御素和肽聚糖类抗菌肽的表达, 从而增强肠道的抗菌能力[62]。多酚类化合物通过抗氧化作用调节肠道免疫功能, 提升抗菌肽的合成[63]。高脂饮食会改变肠道微生物群的组成, 则与潘氏细胞分泌的抗菌肽的表达减少相对应[64]。因此, 膳食成分通过直接或间接作用调节肠道抗菌肽的合成, 维持肠道的免疫稳态和肠道屏障完整。
黏附力是细菌能否定植的关键因素。黏附素是细菌表面一类具有特异性结合能力的分子, 其化学成分主要为蛋白质、多糖、糖脂或糖蛋白。鼠李糖乳杆菌(Lactobacillus rhamnosus GG)菌毛蛋白SpaC与黏附有关并能和黏蛋白特异性结合[65]。PINZÓN-ARANGO等[66]发现食用蔓越莓产品影响大肠杆菌的菌丝和脂多糖等表面特性, 从而减少对上皮细胞的粘附。细菌表面蛋白也与细菌的黏附有关。此机制首先在嗜酸乳杆菌中被发现[67]。DE-SANDOZEQUI等[68]发现纤维杆菌门(Fibrobacteres)通过自身的S-层蛋白可与胞外基质进行黏附实现定植。受体是细菌实现黏附的重要部分。宿主细胞表面黏黏附素受体的类型和数量决定了细菌黏附的位置和数量, 不同的细菌对应不同的受体[69], 幽门螺杆菌通过黏附素和宿主黏蛋白之间的结合实现在胃中的定植[70], SPAULDING等[71]发现清除大肠杆菌的FimH受体不会影响肠道内其他微生物的定植。肠道黏附受体多数分布在肠道黏液层, 受黏液的分泌和流动速度的影响, 由肠上皮细胞分泌较低的分泌速度及流速, 利于固有菌群的保留[69]。此外, 膳食成分也对细菌的黏附性产生了影响, 例如, VIENNOIS等[72]发现膳食乳化剂羧甲基纤维素(carboxymethyl cellulose, CMC)和聚山梨醇酯80 (polysorbate 80, P80)以剂量依赖的方式增加了黏附侵袭性大肠杆菌(adherent-invasive Escherichia coli, AIEC)黏附相关基因的表达, 并导致宿主炎症信号的激活, 从而诱导肠道炎症。LI等[73]在体外糖酵解实验过程中发现, 富含膳食纤维的大豆种皮经发酵后产生了丰富的丙酸盐和正丁酸盐, 从而增强了植物乳杆菌(Lactobacillus plantarum)和长双歧杆菌(Bifidobacterium longum)的黏附能力和增殖效率。
胃肠道可被视为一条动态的“河流”, 其流动速度、营养物质、氧气含量和pH等环境因素沿其轴向发生显著变化, 形成不同的生态位, 进而塑造具有高度空间异质性的微生物群落分布格局。这种空间异质性不仅反映了微生物群落的生态位分化, 还对宿主健康产生深远影响。
胃部作为“河流”的上游, 微生物群落主要由需氧菌组成, 包括放线菌门、变形菌门和梭杆菌门(Fusobacteria)。其中幽门螺杆菌感染不仅与胃炎、胃溃疡和胃癌的发生密切相关, 还可以通过改变胃内pH和调节宿主免疫反应, 使得酸敏感细菌更容易进入肠道, 间接影响下游肠道微生物的组成[74]。质子泵抑制剂的使用会改变胃内微生物群落结构, 并通过口腔来源的链球菌易位进一步影响肠道微生物组成和多样性[75]。随着“河流”向下流动进入小肠, 内部环境发生变化, 小肠微生物群落结构逐渐变得复杂, 主要由乳杆菌目(Lactobacillales)、链球菌科(Streptococcaceae)、双歧杆菌属等组成, 它们参与胆汁酸的代谢和转化, 调节宿主的消化、免疫功能、胆汁酸稳态和营养代谢[76]。作为胃和结肠之间的过渡区域, 小肠微生物通过SCFAs和胆汁酸代谢物等, 降低结肠的pH, 抑制病原菌的生长, 同时促进有益菌的繁殖[77]。盲肠和结肠可以被视为“河流”的下游, 肠道中微生物最为丰富的区域, 类似于“热带雨林”, 具有极高的生物多样性。拟杆菌门、厚壁菌门和假单胞菌门是结肠微生物群的主要成员, 这些微生物通过发酵膳食纤维等复杂碳水化合物产生SCFAs, 不仅为肠上皮细胞提供能量, 还在调节免疫反应和维持肠道屏障功能中发挥重要作用。结肠微生物群的失衡与多种疾病的发生密切相关, 包括炎症性肠病、结直肠癌、肥胖和2型糖尿病等, 其微生物群落的分布和功能对宿主的整体健康具有深远影响。
在“河流”两岸的“河床”, 存在着宿主与微生物之间的复杂相互作用。肠道上皮细胞通过分泌黏液形成黏液层, 将肠道上皮组织与肠腔内容物分隔开。黏液层的异质性不仅影响微生物群落的分布, 还通过调节微生物与宿主细胞的相互作用, 维持肠道稳态。研究表明, 黏液层的完整性对于防止病原体入侵和维持肠道屏障功能至关重要[78]。膳食可以被视为“河流”的“支流”, 通过引入不同的营养物质和代谢产物, 影响微生物群落分布和功能。不同饮食结构还通过影响肠道中抗菌肽的产生, 调节肠道免疫功能, 从而引起肠道微生物群的改变。这些相互作用共同维持肠道生态的平衡, 并影响宿主的健康。
从胃部到结肠, 微生物群落的分布和功能随着环境因素的变化而呈现出显著的异质性。这种空间分布的异质性不仅反映了微生物对宿主环境的适应性, 也揭示了微生物群落在维持肠道稳态、代谢调控和免疫反应中的关键作用。
近年来肠道微生物的空间分布研究取得了显著进展, 但仍面临诸多挑战。目前的研究主要集中在宏观层面, 例如不同肠段微生物群落的组成和丰度的对比, 而对微生物在胃肠道内微观空间分布及其相互作用机制的研究相对不足。未来的研究需要借助先进的技术手段和跨学科合作, 以更高分辨率揭示微生物在肠道中的功能分区和时空演化规律。结合荧光原位杂交、激光共聚焦显微镜等高分辨率成像技术和空间转录组学, 可以揭示微生物在黏液层、隐窝和肠绒毛等微环境中的定植模式和功能分工。深入研究宿主-微生物相互作用的分子机制, 特别是微生物如何通过代谢产物和信号分子与宿主细胞进行交流, 将为理解微生物在肠道屏障功能和免疫调节中的作用提供新视角。膳食作为影响微生物群落的重要因素, 具体调控机制仍需结合多组学技术进行精准研究, 例如膳食纤维和多酚类化合物如何通过调节微生物代谢途径和基因表达影响群落组成和功能, 仍需进一步探索。此外, 可以利用空间限制性抗生素、同位素标记技术等手段验证微生物空间分布与疾病之间的因果关系。
未来的研究还需借助肠道芯片、类器官模型和人工智能算法等技术创新, 突破现有研究的局限性, 全面解析微生物群落的生态学特征及其与宿主健康的关联, 为肠道健康的维护、疾病的预防与治疗以及个体化医疗方案的制定提供新的理论依据和实践指导。
