The weapons in modern warfare are different from those in traditional. Along with the increasing use of precision-guided and high-energy explosive weapons, the combat environment and traumatic condition of eye injuries have changed dramatically. Traditional War Injury Treatment System of the Chinese PLA has not been able to better meet the needs of modern warfare. In order to improve the abilities of treating and protecting from combat-related eye injuries, this paper reviews the research advances in the combat-related eye injuries in 21st century through the aspects of epidemiology, combat environment, injury factors, treatment system and protection work. The present situation of the research on the combat-related eye injuries in Chinese PLA is analyzed, and the research directions in the future are prospected.

现代战争中,随着精确制导、高能爆炸性武器使用的增多,眼战伤的受伤环境、伤情伤势发生了巨大变化。我军多年来无战伤救治实践,传统的三区七级防护与救治体系难以更好地适应现代战争的需求。为提高我军眼战伤的救治及防护水平,该文对21世纪以来外军现代局部战争中的眼战伤流行病学、受伤环境、致伤因素、眼战伤救治体系及防护等的研究进展进行综述,分析我军眼战伤防护救治研究的现状,并对未来眼战伤的研究方向进行展望。

现代战争中,随着精确制导、高能爆炸性武器使用的增多,眼战伤的受伤环境、伤情伤势发生了巨大变化。我军多年来无战伤救治实践,传统的三区七级防护与救治体系难以更好地适应现代战争的需求。为提高我军眼战伤的救治及防护水平,该文对21世纪以来外军现代局部战争中的眼战伤流行病学、受伤环境、致伤因素、眼战伤救治体系及防护等的研究进展进行综述,分析我军眼战伤防护救治研究的现状,并对未来眼战伤的研究方向进行展望。

The weapons in modern warfare are different from those in traditional. Along with the increasing use of precision-guided and high-energy explosive weapons, the combat environment and traumatic condition of eye injuries have changed dramatically. Traditional War Injury Treatment System of the Chinese PLA has not been able to better meet the needs of modern warfare. In order to improve the abilities of treating and protecting from combat-related eye injuries, this paper reviews the research advances in the combat-related eye injuries in 21st century through the aspects of epidemiology, combat environment, injury factors, treatment system and protection work. The present situation of the research on the combat-related eye injuries in Chinese PLA is analyzed, and the research directions in the future are prospected.

在现代战争中,随着高能、高速、精确制导爆炸性武器使用的增多,眼战伤的发生率也在逐步提高[1]。在19世纪的大部分战争中,只有不到1%的伤员遭受了眼战伤(1861—1865年美国南北战争0.57%;1894年第一次中日战争1.20%[2])。而在20世纪的战争中,眼战伤比例迅速上升(1914—1918年第一次世界大战1.54%~2.25%;1950—1953年朝鲜战争4.10%;1955—1975年越南战争5.00%~9.00%[2])。在1991年的海湾战争中,一所战地医院的数据显示眼战伤发生率已经达到了13.00%[3](图1[2])。
现代战争武器及作战模式的变化带来了眼部伤情的变化,其防护与救治体系也须应时而变。本文对21世纪以来外军主要局部战争中的眼战伤伤情及防护救治的主要研究结果进行综述,以期为我军战时卫勤保障工作提供参考。
一项研究调查分析了2001—2011年美军在伊拉克自由行动及持久自由行动(Operation Iraqi Freedom and Enduring Freedom,OIF/OEF)中后送伤员视力低于20/200的情况,有239例(265只眼)的最终视力低于20/200,在265只眼中,闭合性眼球损伤有56只眼(21.1%);无Ⅰ区损伤,Ⅱ区损伤1只眼,占1.8%(1/56),Ⅲ区损伤55只眼,占98.2%(55/56)[4]。一项针对伊拉克和阿富汗战争中的闭合性眼战伤的研究报道,2006—2009年,在46例(83只眼)遭遇了爆炸伤的军人中,20例(43.5%)的25只眼(30.1%)并发了闭合性眼球损伤;其中Ⅰ区损伤11例(13只眼),Ⅱ区损伤12例(16只眼),Ⅲ区损伤13例(15只眼)[5]。由此可见,在现代战争中,闭合性眼战伤的比例虽然不高,但累及视网膜、视神经等Ⅲ区结构的损伤比例较高,对视力影响较大。在对爆炸伤士兵的救治中,军医不能忽视对其眼部的详细检查,及时诊断出闭合性眼战伤可为抢救伤员视力赢得时机。
美国沃尔特·里德陆军医疗中心(Walter Reed Army Medical Center,WRAMC)的眼战伤数据库(Walter Reed Ocular Trauma Database)收录了2001—2011年OIF/OEF中后送伤员的890只伤眼的病历数据,其中166只(18.6%)伤眼存在球内异物[6];184只眼遭受了角膜或巩膜损伤,其中169只眼为开放性眼外伤,15只眼为闭合性角膜挫伤[7]。可见开放性眼外伤的比例远高于闭合性眼外伤。2003年3月—2006年10月,WRAMC共救治了61例(65只眼)遭受了眼球穿通伤的美国军人,最初接受玻璃体切除术的19例伤眼至随访结束,其中位数视力仅为指数,只有4例伤眼的视力恢复到20/200以上[8]。Vlasov等[4]发现,从OIF/OEF撤离的视力低于20/200的239例军人的265只伤眼中,206只(77.7%)为开放性眼外伤。可见,开放性眼战伤是士兵最主要的致盲损伤。战场上眼部防护装备的改进及外科治疗水平的提高对防治开放性眼战伤至关重要。
除了体征明显的开放性眼战伤及闭合性眼战伤,爆炸相关的创伤性脑损伤(traumatic brain injuries,TBI)后遗留的视觉功能障碍也是影响军人生活质量的一个重要因素,但往往被军医忽视。Magone等[9]调查分析了31例无眼外伤的爆炸性轻度脑损伤(mild traumatic brain injuries,mTBI)伤员的长期(≥12个月)视力预后情况,发现其随访末的平均矫正视力为20/20,尽管视力非常好,但视觉障碍却很常见(21例,67.7%),其中最常见的主诉是畏光(55%)和阅读困难(32%)。一项基于远程医疗数据库(Expeditionary Medical Encounter Database)的研究评估了OIF/OEF中与爆炸相关的脑损伤后视觉障碍的发生率,结果发现与其他受伤的军人相比,中度TBI[OR=1.58,95%置信区间(CI)=1.02~2.45]及重度至致命TBI(OR=14.26,95%CI=7.00~29.07)的伤员视觉障碍发生率更高[10]。因此,在接诊战场后送的遭受了TBI的伤员后,军医需要对其进行全面的视功能检查。
军事技术的更新改变了战争方式,卫勤保障工作需要围绕这些致伤因素的变化做出改进,才能降低伤残率。研究现代战争中眼战伤的致伤因素可以协助军队开发或改进防护装备,更好地帮助军医救治伤员。
有研究调查了塞内加尔军队在2005年执行作战任务期间造成眼部受伤的主要武器,发现37例后送到该国首都达喀尔三级医院眼科的伤员共有46只眼发生了眼战伤。其致伤因素中,RPG7火箭炮占62%,地雷爆炸占13.5%,进攻性手榴弹爆炸占10.8%,突击步枪占5.7%,火焰喷枪占8%。称为“叛乱武器”的RPG7火箭炮经常用于非洲地区的冲突中,其造成的眼伤预后常常不乐观[11]。Mader等[12]分析了OIF/OEF行动中眼部损伤的类型及原因,发现弹药的爆炸碎片造成的眼部伤害占所有眼伤的82%,最常见的单一伤害原因是简易爆炸装置的爆炸损伤(51%)。
在战场上,防弹眼镜的使用减少了弹片导致眼部受伤的机会。然而在现代战争中,重型火力武器的爆炸威力之大,以至于目前的防弹眼镜不能完全防止眼部损伤,而只能降低损伤的严重程度[12]。更加不幸的是,防护型战斗眼镜并未在战斗中被普遍使用,许多士兵的双眼完全暴露在重火力对抗的环境下。Thach等[13]报道在OIF的第一年,85%的眼外伤士兵不知道或未规范地使用眼部保护装备。另一项研究显示,在2003年3月—2006年10月的OIF行动中,只有24%的士兵在受伤期间佩戴眼罩,有34%的士兵没有佩戴眼罩,42%的伤员不知道受伤时其眼部是否处于被保护状态[14]。有研究者调查了在战场上不愿佩戴防护眼镜的士兵,主要原因包括认为其过于笨重成为行动中不必要的负担,在战斗中眼镜会起雾,以及抱怨军方发放的眼镜外观不时髦等[15]。可见,现有的眼部防护装备无论从防护效能还是外观上都需进一步改进。
战场防护装备的设计与研发需紧密围绕士兵的实战状态。准确确定穿透性眼部及面部伤口的常发位置对于个人防护设备的设计至关重要。Breeze等[16]分析了2005年1月1日—2009年12月31日遭受面部或眼部穿透伤的所有英国军人的病历及验尸记录,结果显示1187例战伤军人中,113例(9.5%)为眼伤;不佩戴防护眼镜的军人因爆炸性碎片而发生眼战伤的可能性是佩戴防护眼镜军人的36倍;在整个面部损伤中,下1/3为最常受伤的部位(60%),其次是上1/3(24%)。尽管不能很好地保护下面部,防护眼镜仍可将整个面部(包括骨骼及软组织)的伤害降低15%(P<0.01)。将用于下巴固定的绑带及头盔套的现有材料更改为具有防弹保护的材料,可进一步降低头面部战伤的发生率。Bailoor等[17]采用三维流固耦合计算机模型评估弹道及激光防护眼罩在人眼遭受爆炸载荷作用时的防护效能,结果显示,爆炸波在穿过防护眼罩后的能量显著降低,且防护眼罩为之后爆炸波的膨胀提供了空间。
在战场上,由于躯体及颅脑等部位的创伤更加致命,致使眼伤的救治往往不会被优先考虑,且急救方法不规范;而且眼科手术通常在后方医院进行,无法保证其时效性。眼战伤造成的视力损失可直接导致士兵失去作战能力,造成战斗减员。
在近几年的现代战场上,眼战伤的救治体系有了显著的改善。美军在OIF/OEF行动中,由随队医务人员在战场上对受伤士兵进行急救及分类,之后会迅速安排医疗后送。伤员通常由直升机或军用救护车送往战斗支援医院。战斗支援医院可根据军队的前进、战场的转移而重新安置,避免了伤员后送距离过长的问题,从而能更好地支持前线的需求。该级医院拥有一支包括了眼科医师在内的外科专家团队,在维持伤员生命体征稳定后,眼科急诊手术可由经验丰富的眼科医师在几个小时内开展[18]。我军传统的三区七级救治体系建立在20世纪既往战争卫勤保障经验的基础上,往往在第五级野战医院甚至第六级的基地医院才会配备眼科医师,前四级医院均不具备开展眼科显微手术的条件[2]。未来战争的分级救治形式不会有根本变化,但其理论需要创新,严格的线性后送模式往往会失去抢救眼战伤伤员视力的最佳时机,对无其他合并重伤的伤员,可打破分级治疗的界限,越级后送至有专科救治能力的后方医院[2]。应加强眼战伤的时效救治、专科救治,强调眼科首次显微手术前伸,并贯穿眼战伤救治的全过程[1]。美军的移动式战斗支援医院以及海陆空立体后送模式值得我军借鉴。
我国多年来无战伤救治实践,最近的战争即为1979年2月及1984年4月的西南边境作战。耿美香等[19]在战后总结了一线医院在眼战伤处理中暴露的问题:(1)在开放性眼外伤的眼球内放置引流条、包扎纱布与眼内容物粘连、清创不彻底、眼内容物挖除过多,以及不能及时诊断处理眶尖综合征等不当的急救操作对伤员的眼球造成了毁灭性的二次伤害;(2)一线军医对伤员的分类错误使轻伤员占用了紧急后送资源,而重伤员得不到及时救治;(3)后送时效低,20%左右的伤员在受伤72 h后才能抵达医院。可见,以我军当时的眼战伤救治能力无法高质量地完成现代战争的卫勤保障任务。
针对眼战伤的救治,近些年我军的临床及科研人员已经开展了许多工作。耿美香团队研发了眼战伤急救包并将其应用在边防部队的卫生队[20]。该急救包将敷料与滴眼液一体化结合,解决了直接将纱布盖在眼内容物表面时造成的粘连问题。丁美宁等[21]研制的一体式眼战伤冲洗装置相较于传统的注射器结膜囊冲洗法,具有流速可控、操作简单、耗时较短等优点,能为眼战伤的一线救治提供保障。张颖等[22]对眼创伤评分法在军人眼外伤中的应用价值进行了研究,提出在战场环境中,非眼科专业的一线军医也可通过该评分法快速准确地对伤员进行分类,合理安排后送。邱怀雨等[23]调查了和平时期团级及团级以下部队医院的军人眼外伤救治情况,发现只有极少数的眼外伤伤员在团级以下的卫生队或卫生所接受了初步处理,绝大部分为直接转诊。由此可见,我军基层医院军医的眼外伤救治知识储备及实践能力均需进一步加强。《中华战创伤学(第4卷)眼部战创伤》《眼创伤学》《眼创伤诊疗指南》等专著全面系统地为军医救治眼部战创伤提供了理论与实践指导[2,24-25]。2019年全军眼科专业委员会又出台了《我军眼战伤专科前救治专家共识》,归纳总结了眼战伤一线救治与后送规范,适用于战地现场人员自救互救及无眼伤专科救治能力的各类救治机构的军医遵循[26]
现代战争武器的革新造成了眼战伤发病率的增高及伤势的加重。结合我军及外军的作战经验,我军已经在现代战争眼战伤伤员分类及急救包研发等领域取得了初步的成果,并提出了眼科专科治疗前移、加强眼科时效治疗的理念。适应高技术战争的眼战伤后送与救治体系须应时而变。在未来的研究中,救治方面,眼科专科治疗介入的具体时机需要深入研究;显微外科手术的开展时机、专科手术设备及手术专家团队的部署方案及部署可行性有待进一步明确;更适合眼战伤前线急救及后送的医疗器材研发与改进工作仍须得到重视。防护方面,高效能防护装备应随着战争武器的更新而不断改进。提升防护装备对激光武器、高能高速爆炸性武器及沙尘、高温、高寒、辐射等恶劣战场环境的防护能力,不断改善防护效能的测试方法,将会成为重要的研究方向。我军要吸取上世纪战争眼战伤救治的经验教训,借鉴外军现代战争的眼战伤救治新理念及新技术,加强学科间的合作研究,以降低眼战伤的发生率、致盲率,降低战斗减员率,更好地保障打赢现代化战争。
