目的探讨收缩压(SBP)纵向轨迹对新发慢性肾脏病(CKD)的影响。方法选取符合标准的13 985名参加健康体检的开滦集团在职及离退休职工进行前瞻性队列研究，通过SAS Proc Traj程序确定研究对象前3次健康体检的SBP纵向轨迹，生成5个不同的SBP轨迹组：低-稳定轨迹组(106.42~109.43 mmHg，n=2927)、中-稳定轨迹组(120.63~124.46 mmHg，n=6916)、中-高轨迹组(132.22~142.97 mmHg，n=2716)、高-中轨迹组(156.77~134.15 mmHg，n=668)及高-稳定轨迹组(154.65~158.98 mmHg，n=758)。采用Log-Rank检验比较不同SBP轨迹组随访期间新发CKD的累积发病率，采用多因素Cox回归模型分析SBP纵向轨迹对新发CKD的影响。结果 13 985例受试者的年龄为(49.5±11.0)岁，其中男10 712例(76.6%)，女3273例(23.4%)。随访时间为(6.8±1.0)年。新发CKD的累积发病率为13.9%(1187/13 985)，其中低-稳定轨迹组、中-稳定轨迹组、中-高轨迹组、高-中轨迹组、高-稳定轨迹组分别为7.7%(147/2927)、12.2%(514/6916)、17.8%(291/2716)、21.9%(93/668)、35.4%(142/758)，呈逐渐升高趋势，经Log-Rank检验差异有统计学意义(χ²=177.561，P<0.001)。多因素Cox风险比例回归模型分析结果显示，与低-稳定轨迹组比较，中-高轨迹组、高-中轨迹组、高-稳定轨迹组均为新发CKD的危险因素。多因素调整后，其HR值及95%CI分别为1.36(1.04~1.77)、1.62(1.08~2.43)、2.06(1.40~3.02)，且SBP纵向轨迹对新发CKD的影响优于基线SBP水平。结论 SBP纵向轨迹水平升高是新发CKD的危险因素。

Objective To investigate the relationship of longitudinal trajectory of systolic blood pressure (SBP) to new-onset chronic kidney disease (CKD). Methods A prospective cohort study was performed with a total of 13 985 on job and retired Kailuan Group employees who met the standard and participated in health examination. SAS Proc Traj program was used to identify the longitudinal trajectories of SBP of the first three times health examination, and 5 different SBP trajectory groups formed: low-stability trajectory group (106.42-109.43 mmHg, n=2927), medium-stability trajectory group (120.63-124.46 mmHg, n=6916), medium-high trajectory group (132.22-142.97 mmHg, n=2716), high-medium trajectory group (156.77-134.15 mmHg, n=668) and high-stability trajectory group (154.65-158.98 mmHg, n=758). Log-rank test was performed to compare the differences of new-onset CKD cumulative incidence among different SBP trajectory groups during follow-up; Multi-Cox regression model was used to analyze the effect of different SBP longitudinal trajectories on new-onset CKD. Results The mean age of 13 985 subjects was (49.5±11.0)years, including 10 712 males (76.6%) and 3273 females (23.4%), the mean follow-up time was (6.8±1.0) years. The cumulative incidence of CKD was 13.9% (1187/13 985). The cumulative incidences of CKD in low-stability trajectory group, medium-stability trajectory group, medium-high trajectory group, high-medium trajectory group and high-stability trajectory group were 7.7% (147/2927), 12.2% (514/6916), 17.8% (291/2716), 21.9% (93/668), and 35.4% (142/758), respectively, the incidence of CKD increased with elevated stability trajectory (log-rank χ²=177.561, P<0.001). Multi-Cox regression model analysis showed that with adjusted confounding factors, compared with the subjects in low-stability trajectory group, SBP longitudinal trajectories were the risk factors for new-onset CKD in medium-high trajectory group, high-medium trajectory group and high-stability trajectory group, and the HR and 95%CI were 1.36 (1.04-1.77), 1.62 (1.08-2.43) and 2.06 (1.40-3.02), respectively. And the influence of SBP longitudinal trajectory on new-onset CKD was better than that of baseline SBP level. Conclusion Elevated longitudinal trajectory of SBP is the risk factor for new-onset CKD in Kailuan group population.

近年来，慢性肾脏病(chronic kidney disease，CKD)患病人数迅速增加，全球范围内CKD的患病率约为13.4%^[1-2]。我国成人CKD患病率高达10.8%，但CKD患者的患病知晓率仅为12.5%^[3]，已成为我国主要的公共卫生问题^[4]。多项研究表明，高血压与CKD的发生相关，是CKD的主要病因之一^[5-7]。长期队列研究发现，随着诊室血压水平升高，终末期肾脏病(end stage renal disease，ESRD)的发生率也明显增高^[8]。前瞻性队列研究表明，累积收缩压(systolic blood pressure，SBP)增加是估算肾小球滤过率(estimated glomerular filtration rate，eGFR)降低的危险因素，同时血压累积暴露得分每增加1分，CKD发病风险增加6%^[9]。但是CKD的发生发展需要经历较长的时间，以往许多研究只关注单一时点的血压测量值或累积血压水平与终点事件之间的关系，不足以准确反映长期的血压水平对CKD的影响^[5-7,9]。因此，需要实现“单一时点血压或累积血压水平暴露”到“血压动态变化轨迹”的转变。

轨迹模型是一项基于多次重复测量数据的分析技术，该技术遵循同质分组法对数据进行分组以评估长时血压变化趋势，研究不同血压轨迹对靶器官损害的影响^[10-11]。有回顾性研究显示，在山东多中心健康管理人群中，血压变化轨迹与CKD发病风险相关^[12]，但是鲜有一般人群血压轨迹与CKD关系的前瞻性队列研究报道。为此，本研究依据开滦研究(注册号：CHiCTR-TNC-11001489)资料，应用轨迹模型分析血压动态变化轨迹与新发CKD之间的关系。

1 资料与方法

1.1 资料来源

2006—2007年度由开滦总医院、开滦林西医院、开滦赵各庄医院、开滦唐家庄医院、开滦范各庄医院、开滦吕家坨医院、开滦荆各庄医院、开滦林南仓医院、开滦钱家营医院、开滦马家沟医院、开滦医院分院共计11家医院对开滦集团在职及离退休职工进行了第1次健康体检，共有101 510名职工参加，其中男81 110名，女20 400名。此后于2008—2009、2010—2011、2012—2013、2014—2015、2016—2017年度按照第1次健康体检时间顺序对上述人群分别进行第2~6次健康体检，参与体检的医务人员及体检地点相同，健康体检项目相同。该研究已通过开滦(集团)有限责任公司医院医学伦理委员会审批([2006]医伦字5号)。已告知被检者其体检结果将会用于心脑血管疾病及相关疾病的危险因素及干预研究(开滦研究)，被检者均同意并签署知情同意书。

1.2 资料收集

流行病学调查内容、一般体格检查、人体测量学指标等具体内容参见本课题组已发表文献^[13]。血压测量方法：测量前30 min内受试对象禁止吸烟、饮茶或咖啡，背靠静坐15 min。使用经校正的汞柱式血压计测量右侧肱动脉血压。收缩压读数取柯氏音第Ⅰ时相，舒张压读数取柯氏音第Ⅴ时相。连续测量3次，每次测量间隔1~2 min，取均值。质量控制：为避免信息偏倚，统一培训体检、检测人员，各协作医院按体检计划书严格执行体检规程。购置同一型号汞柱式血压计、体重秤，统一校正，对测量人员进行培训，统一测量手法，每一医院原则上由固定人员进行测量。统一购置、使用同一批检测试剂，使用相同型号生化仪检测，由专业人员随批质控。

1.3 相关疾病定义及诊断标准

1.3.1 eGFR

采用两种族慢性肾脏病流行病学合作研究公式(chronic kidney disease epidemiology collaboration equation，CKD-EPI)法计算^[14]。女性：肌酐≤62 μmol/L时，eGFR=144×(Scr/0.7)^–0.329×(0.993)^年龄；肌酐>62 μmol/L时，eGFR=144×(Scr/0.7)^–1.209×(0.993)^年龄。男性：肌酐≤80 μmol/L时，eGFR=141×(Scr/0.9)^–0.411×(0.993)^年龄；肌酐>80 μmol/L时，eGFR=141×(Scr/0.9)^–1.209×(0.993)^年龄。

1.3.2 相关定义

吸烟定义为近一年内平均每天至少吸烟1支；饮酒定义为近一年平均饮白酒(乙醇含量>50%)100 ml/d；体育锻炼定义为锻炼次数≥3次/周，持续时间≥30 min/次。

1.3.3 诊断标准

糖尿病诊断标准^[15]：2006—2007年度健康体检空腹血糖(fasting blood glucose，FBG)≥7.0 mmol/L或FBG<7.0 mmol/L但服用降糖药物或注射胰岛素治疗者；高血压诊断依据2010年中国高血压防治指南的诊断标准^[16]：2006—2007年度健康体检时收缩压≥140 mmHg和(或)舒张压≥90 mmHg，或正在服用抗高血压药物者；CKD诊断依据2002年美国国家肾脏基金会制定的慢性肾脏病临床实践指南(kidney disease outcome quality initiative，K/DOQI)标准^[17]，本研究CKD定义为：以第3次体检(2010—2011年度)结束为随访起点，第6次体检(2016—2017年度)结束为随访终点，随访过程中，每2年对该人群进行一次健康体检，任意一次健康体检时eGFR<60 ml/(min·1.73 m²)和(或)尿蛋白阳性者诊断为CKD。

1.4 纳入标准及排除标准

纳入标准：①2006—2007、2008—2009、2010—2011年度健康体检时血压、肌酐及尿常规资料完整者；②同意参加本研究并签署知情同意书者。排除标准：①2006—2007、2008—2009、2010—2011年度健康体检时eGFR<60 ml/(min·1.73 m²)和(或)尿蛋白阳性者和(或)尿毒症患者及正在接受肾替代治疗的患者；②2006—2007、2008—2009、2010—2011年度健康体检时有恶性肿瘤、心脑血管病史者；③因各种原因均未参加2012—2013、2014—2015、2016—2017年度健康体检者；④2012—2013、2014—2015、2016—2017年度健康体检时血肌酐及尿常规资料均缺失者；⑤6次体检时SBP极值(≥250 mmHg)者。

1.5 分组

本研究截止到2016—2017年度健康体检结束，以8年为暴露期，应用SAS Proc Traj程序确定研究对象于2006—2007、2008—2009、2010—2011年度3次健康体检时SBP的纵向轨迹，生成5个不同的SBP轨迹组：低-稳定轨迹组、中-稳定轨迹组、中-高轨迹组、高-中轨迹组及高-稳定轨迹组。

1.6 生化指标检测

受试者空腹至少8 h，于体检当日晨起抽取肘静脉血5 ml，离心后取上清检测FBG、血肌酐、三酰甘油(triglyceride，TG)、总胆固醇(total cholesterol，TC)、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol，HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol，LDL-C)。FBG检测采用己糖激酶法，血糖在5.55 mmol/L时变异系数(coefficient of variation，CV)≤2%，线性上限为33.3 mmol/L，试剂盒购自中生北控生物科技股份公司。血肌酐测定采用酶法，试剂购自上海名典生物工程公司，批内CV<10%，批间相对极差<10%，线性范围为44~106 μmol/L，采用日立7600型全自动生化仪测定。操作按试剂说明书严格进行，与卫生部质控结果一致，由专业检验师进行操作。其余检测指标的具体测定方法参见本课题组已发表文献^[13]。尿常规采用尿干化学法+尿沉渣检测法，H12-MA尿液分析试纸及DIRUI N-600尿液常规检测分析仪均购自长春迪瑞医疗科技股份有限公司。尿常规中尿蛋白测定采用半定量法，阴性代表<0.15 g/L，trace代表0.15~0.29 g/L，(+)代表0.3~3.0 g/L，(++)代表3.0~10.0 g/L，(+++)代表>10.0 g/L。

1.7 统计学处理

每次健康体检数据均由各医院终端录入，通过网络上传至开滦医院计算机室服务器，形成Oracle10.0g数据库，由程序导出形成DBF格式文件，采用SAS 9.4统计软件进行分析。轨迹模型通过SAS Proc Traj程序完成^[18-19]。应用CNORM模式先确定SBP纵向轨迹组数，再进一步确定每组轨迹的多项式阶。根据贝叶斯信息准则(Bayesian information criterions，BIC)及分组后的平均概率(AvePP)来选择最佳轨迹模型和评估轨迹的拟合程度，保证每组所占比例在5%以上，最终确定5组SBP轨迹，分别以2、2、2、2、2位多项式阶的轨迹模型为最佳模型。计量资料以$\bar{x}±s$表示，多组间比较采用单因素方差分析，组间进一步两两比较采用LSD-t检验；计数资料以例(%)表示，组间比较采用χ²检验。采用Log-Rank检验比较不同SBP轨迹组新发CKD的累积发病率。采用多因素Cox风险比例回归模型分析SBP轨迹对新发CKD的影响。P<0.05为差异有统计学意义。

2 结果

2.1 研究队列确定

2006—2007年度参加健康体检者共101 510名，该研究队列同时完成2008—2009、2010—2011年度健康体检且3次体检时血压、血肌酐及尿常规资料均完整者共42 972例，排除3次健康体检时eGFR<60 ml/(min·1.73 m²)和(或)尿蛋白阳性者和(或)尿毒症患者及正在接受肾替代治疗的患者共10 575例，排除3次健康体检时有恶性肿瘤、心脑血管病史者1716例，排除随访期间因各种原因未参加2012—2013、2014—2015、2016—2017年度健康体检者15 155例，排除随访期间健康体检时血肌酐及尿常规资料均缺失者1312例，排除前3次体检时SBP极值(≥250 mmHg)者229例，最终纳入13 985例研究对象进行统计分析(图1)。

2.2 不同SBP轨迹组一般资料比较

本研究以2010—2011年度健康体检资料为基线资料，13 985例研究对象的年龄为(49.5±11.0)岁，男10 712例(76.6%)，女3273例(23.4%)。其中低-稳定轨迹组2927例，SBP稳定在106.42~109.43 mmHg；中-稳定轨迹组6916例，SBP稳定在120.63~124.46 mmHg；中-高轨迹组2716例，SBP由132.22 mmHg升至142.97 mmHg；高-中轨迹组668例，SBP由156.77 mmHg降至134.15 mmHg；高-稳定轨迹组758例，SBP稳定在154.65~158.98 mmHg(图2)。

与低-稳定轨迹组比较，其他4组的年龄、SBP2006、SBP2008、SBP2010、BMI、FBG、TC、LDL-C、HDL-C、hs-CRP均明显升高，差异有统计学意义(P<0.001)；其他4组中男性、吸烟、饮酒者比例，高血压、糖尿病的患病比例及服用降压药、降糖药的比例明显增高，体育锻炼者比例逐渐缩小，差异有统计学意义(P<0.001，表1)。

2.3 不同SBP轨迹组的新发CKD情况

随访至第6次健康体检结束，随访时间(6.8±1.0)年，新发CKD的累积发病率为13.9%(1187/13 985)，低-稳定轨迹组、中-稳定轨迹组、中-高轨迹组、高-中轨迹组、高-稳定轨迹组新发CKD的累积发病率分别为7.7%(147/2927)、12.2%(514/6916)、17.8%(291/2716)、21.9%(93/668)、35.4%(142/758)，组间比较逐渐升高，差异有统计学意义(χ²=177.561，P<0.001)。

2.4 多因素Cox风险比例回归分析新发CKD的影响因素

以是否发生CKD为因变量(是=1，否=0)，以不同SBP轨迹组为自变量(以SBP低-稳定轨迹组为参照组)，进行多因素Cox风险比例回归模型分析。结果显示：与低-稳定轨迹组比较，中-稳定轨迹组、中-高轨迹组、高-中轨迹组、高-稳定轨迹组均为新发CKD的危险因素，HR值及95%置信区间(CI)分别为1.60(1.33~1.92)、2.49(2.04~3.04)、3.13(2.42~4.06)、4.59(3.64~5.79)。

根据调整因素的不同，建立4个回归模型。模型1为单因素分析；模型2在模型1基础上进一步校正年龄、性别；模型3在模型2基础上进一步校正吸烟、饮酒、体育锻炼、FBG、TC、BMI、hs-CRP、服用降压药史、服用降糖药史等混杂因素，模型4在模型3的基础上校正了2006年基线体检的收缩压值。经多因素调整后，模型4结果显示：与低-稳定轨迹组比较，中-稳定轨迹组、中-高轨迹组、高-中轨迹组、高-稳定轨迹组仍显著增加新发CKD的发病风险，HR值及95%CI分别为1.17(0.95~1.44)、1.36(1.04~1.77)、1.62(1.08~2.43)、2.06(1.40~3.02)。见表2。

2.5 敏感性分析

为进一步减少CKD其他危险因素对结果的影响，分别去除队列中糖尿病者、BMI>28 kg/m²者及服用降压药物、降糖药物者后进行敏感性分析，经多因素调整后，结果与表2的分析结果一致(表3)。其中模型1去除糖尿病患者；模型2去除BMI>28 kg/m²者；模型3去除服用降压及降糖药物者。模型1—3均为多因素模型，调整年龄、性别、吸烟、饮酒、体育锻炼、FBG、TC、BMI、hs-CRP、服用降压药史、服用降糖药史等混杂因素。

3 讨论

本研究人群的基线SBP为(123.42±16.84) mmHg，高于理想的SBP水平(<120 mmHg)^[15]。根据纵向SBP轨迹分组，本研究观察人群的SBP处于理想水平(低-稳定轨迹组)者比例仅为20.9%(2927/13 985)，近50%(6916/13 985)的研究对象处于正常高值血压水平(中-稳定轨迹组)，另外有近20%(2716/13 985)的正常高值血压水平者进展为高血压(即中-高轨迹组)，其中超过5%(758/13 985)的研究对象始终高于150 mmHg，血压控制不理想。

一项基于中国人群的早期SBP轨迹与中年亚临床肾损害关系的研究结果显示：低-稳定组的亚临床肾功能损害患病率为7.85%，而中-增长组为43.62%^[20]。来自山东多中心健康管理人群的研究显示，SBP低-稳定组的CKD发病密度为6.65/1000人年，而极高水平波动组为25.82/1000人年^[12]。本研究对象为一般人群，随访(6.8±1.0)年，新发CKD的累积发病率为13.9%，随着SBP轨迹由低-稳定轨迹组升高至高-稳定轨迹组，新发CKD的累积发病率由7.7%升至35.4%，与上述研究结果一致。但既往研究均为特殊人群，结论推广时存在一定的局限性，本研究对象为一般人群，随访时间长，结果可靠，更具有推广意义。

本研究发现，校正了性别、年龄等混杂因素后，与SBP低-稳定轨迹组比，中-高轨迹组、高-中轨迹组、高-稳定轨迹组均显著增加新发CKD的发病风险。中-高轨迹组SBP增加10 mmHg，CKD发病风险是低-稳定轨迹组的1.36倍。高-中轨迹组SBP水平由156 mmHg下降至134 mmHg，CKD的发病风险较高-稳定轨迹组降低了44%。因此积极控制SBP水平在135 mmHg以下，可明显降低CKD的发病风险。周苗^[12]的研究发现，与SBP低水平稳定组比较，SBP高水平稳定组及极高水平轨迹组的CKD发病风险明显升高，其HR(95%CI)分别为1.82(1.16~2.84)和3.40(1.97~5.86)。还有研究发现，在非高血压及糖尿病人群中，SBP高-稳定轨迹可明显增加肾损害的发病风险^[21-22]，均与本研究结果一致。本研究还发现，与低-稳定轨迹组比较，中-稳定轨迹组的SBP维持在120.63~124.46 mmHg(均<125 mmHg)，随访7年后新发CKD的发病风险虽无统计学差异，但仍增加了17%，说明血压只要高于理想水平即可增加CKD的发病风险。Meta分析结果表明，与理想血压人群相比，高血压前期人群发生终末期肾病的风险增加59%(RR=1.59，95%CI 1.39~1.91)；亚组分析结果显示，高血压前期中血压高值组(130~139 mmHg)发生终末期肾病的风险是理想血压人群的2倍以上(RR=2.02，95%CI 1.70~2.40)^[23]。因此，对高血压前期人群也应尽早积极控制其血压水平，血压降至理想水平有助于减少CKD的发生。既往研究发现，血压轨迹在预测疾病风险方面优于基线血压水平^[24-25]。本研究也发现，校正了基线SBP后，SBP纵向轨迹对新发CKD的影响仍有意义，表明SBP纵向轨迹对新发CKD的影响优于基线SBP水平。本研究采用了轨迹模型的统计学方法分析年度间多次测量的纵向SBP长时轨迹对新发CKD的影响，且排除了血压变异等混杂因素的影响，使结果更加稳定、可靠。

高血压对肾脏有长期的损害作用，可导致肾血管持续收缩，出现血管硬化及玻璃样变，引起肾血流量减少，肾小球滤过率降低。肾小球高压可直接损伤肾小球基底膜结构，还可减少舒血管活性物质的生成，激活肾脏局部肾素-血管紧张素系统，收缩肾脏小动脉，最终导致肾小球硬化及间质纤维化，从而造成肾功能损害^[26]。血压增高还可直接传递到肾小球循环，造成肾小球内高压、高灌注状态，导致肾脏自我调节功能障碍，进而出现肾脏血流动力学异常^[27]。肾脏损害后，又会促进高血压的进展，导致血压继续升高，形成恶性循环，造成CKD进一步进展^[28]。

本研究也存在不足之处：入选的研究对象为开滦集团公司的职工，男性多于女性，男女比例不均衡可能会对结果有一定影响。CKD的发生受多种因素影响，包括肥胖、糖尿病、服用药物等，为此笔者在进行多因素分析时校正了空腹血糖、BMI、服用药物等混杂因素，还进一步分别排除糖尿病、肥胖患者及服用降压药物、降糖药物者后进行了敏感性分析，结果与调整前一致。本研究中检测尿蛋白采用的是半定量的试纸条法，未进行24 h尿肌酐排泄率检测，可能导致CKD的诊断误差，但已有的研究表明，利用半定量试纸条法诊断CKD具有较高的敏感性(93.3%)和特异性(91.6%)^[29]。本研究为大规模的流行病学调查研究，利用半定量试纸条法检测简便、快捷、经济，且队列相对稳定，随访时间长，研究结果对CKD的防治具有重要意义。
