Objective To explore the effects of mid-term (4 weeks) and long-term (8 weeks) simulated microgravity on calcification of the common carotid artery in the rats. Methods Hindlimb-unweighted (HU) rat model was used to simulate the effects of weightlessness on the cardiovascular system. Seventy-five male SD rats were randomized into the control group, 4-week HU group (HU4w), and 8-week HU group (HU8w). After the common carotid arteries were separated, the quantification of calcium content and the alizarin red staining were used to detect the calcium salt deposition in the common carotid arteries. The alkaline phosphatase (ALP) activity of common carotid arteries was measured. Western blotting and the real-time polymerase chain reaction(RT-PCR) were performed to measure the protein and mRNA levels of Runx2, Msx2, BMP2, osteocalcin, and SM22α. Results Compared with the control group, the calcium content and the calcium salt deposition were increased, meanwhile, the ALP activity was enhanced in the common carotid arteries of 4-week and 8-week HU rats. Besides, the protein and mRNA levels of Runx2, Msx2, BMP2, osteocalcin were increased, while those of SM22α were decreased in the common carotid arteries of 4-week and 8-week HU rats (P<0.05 or P<0.01). Interestingly, there was no statistically significant difference of above tests between the HU4w and HU8w groups. Conclusion Mid- and long-term simulated microgravity induced the osteogenic transdifferentiation of smooth muscle cells and calcification in the common carotid artery of rats, while the degree of calcification did not change significantly with the prolonged period of simulated microgravity.

目的 探讨中期(4周)及长期(8周)模拟失重对大鼠颈总动脉钙化的影响。方法 采用尾部悬吊后肢去负荷模型模拟微重力对心血管系统的影响。选取雄性SD大鼠75只,随机分为对照组(n=25)、4周尾部悬吊组(HU4周组,n=25)和8周尾部悬吊组(HU8周组,n=25)。模拟失重完成后,分离并收集大鼠颈总动脉组织,采用钙含量测定和茜素红染色方法检测钙盐沉积,进行碱性磷酸酶(ALP)活性测定,通过Western blotting和实时荧光定量PCR检测Runt相关转录因子2(Runx2)、肌节同源盒基因同系物2(Msx2)、骨形态发生蛋白2(BMP2)、骨钙素(osteocalcin)、平滑肌22α(SM22α)蛋白和mRNA的表达水平。结果 与对照组比较,4周和8周模拟失重大鼠颈总动脉的钙含量增加、钙盐沉积增多、ALP活性增强,Runx2、Msx2、BMP2、osteocalcin的蛋白和mRNA表达增加,SM22α蛋白和mRNA表达降低(P<0.05或P<0.01),但上述指标在HU4周日与HU8周两组间差异无统计学意义。结论 中期和长期模拟失重可引起大鼠颈总动脉平滑肌细胞向成骨样细胞表型转化及动脉钙化,且钙化程度不因模拟失重时间延长发生明显变化。

目的 探讨中期(4周)及长期(8周)模拟失重对大鼠颈总动脉钙化的影响。方法 采用尾部悬吊后肢去负荷模型模拟微重力对心血管系统的影响。选取雄性SD大鼠75只,随机分为对照组(n=25)、4周尾部悬吊组(HU4周组,n=25)和8周尾部悬吊组(HU8周组,n=25)。模拟失重完成后,分离并收集大鼠颈总动脉组织,采用钙含量测定和茜素红染色方法检测钙盐沉积,进行碱性磷酸酶(ALP)活性测定,通过Western blotting和实时荧光定量PCR检测Runt相关转录因子2(Runx2)、肌节同源盒基因同系物2(Msx2)、骨形态发生蛋白2(BMP2)、骨钙素(osteocalcin)、平滑肌22α(SM22α)蛋白和mRNA的表达水平。结果 与对照组比较,4周和8周模拟失重大鼠颈总动脉的钙含量增加、钙盐沉积增多、ALP活性增强,Runx2、Msx2、BMP2、osteocalcin的蛋白和mRNA表达增加,SM22α蛋白和mRNA表达降低(P<0.05或P<0.01),但上述指标在HU4周日与HU8周两组间差异无统计学意义。结论 中期和长期模拟失重可引起大鼠颈总动脉平滑肌细胞向成骨样细胞表型转化及动脉钙化,且钙化程度不因模拟失重时间延长发生明显变化。

Objective To explore the effects of mid-term (4 weeks) and long-term (8 weeks) simulated microgravity on calcification of the common carotid artery in the rats. Methods Hindlimb-unweighted (HU) rat model was used to simulate the effects of weightlessness on the cardiovascular system. Seventy-five male SD rats were randomized into the control group, 4-week HU group (HU4w), and 8-week HU group (HU8w). After the common carotid arteries were separated, the quantification of calcium content and the alizarin red staining were used to detect the calcium salt deposition in the common carotid arteries. The alkaline phosphatase (ALP) activity of common carotid arteries was measured. Western blotting and the real-time polymerase chain reaction(RT-PCR) were performed to measure the protein and mRNA levels of Runx2, Msx2, BMP2, osteocalcin, and SM22α. Results Compared with the control group, the calcium content and the calcium salt deposition were increased, meanwhile, the ALP activity was enhanced in the common carotid arteries of 4-week and 8-week HU rats. Besides, the protein and mRNA levels of Runx2, Msx2, BMP2, osteocalcin were increased, while those of SM22α were decreased in the common carotid arteries of 4-week and 8-week HU rats (P<0.05 or P<0.01). Interestingly, there was no statistically significant difference of above tests between the HU4w and HU8w groups. Conclusion Mid- and long-term simulated microgravity induced the osteogenic transdifferentiation of smooth muscle cells and calcification in the common carotid artery of rats, while the degree of calcification did not change significantly with the prolonged period of simulated microgravity.

航天飞行过程中,当航天员返回地面1G重力环境或受到其他立位应激时,会出现严重的心血管失调现象,主要表现为立位耐力不良和运动能力降低[1]。航天飞行后的立位耐力不良涉及血容量减少、神经体液调节等复杂机制,其中人体流体静压消失和血液头向转移引起的动脉重建可能是其重要机制之一[2-3]。最近研究证实,长期航天飞行可引起颈总动脉的僵硬度明显增加,此改变影响颈总动脉的功能,进而可能影响航天员在轨、着陆时的工作能力,甚至对生命安全造成威胁[4]
血管钙化是在多种因素作用下发生的与骨发育相似的、主动的、高度可调控的复杂病理过程,主要表现为血管壁僵硬度增加和顺应性降低。由于钙化促进因素与抑制因素的失衡,多种血管细胞尤其是血管平滑肌细胞出现向成骨样细胞表型的转化,在此过程中平滑肌细胞收缩蛋白标志分子,如平滑肌22α(smooth muscle 22α,SM22α)等表达减少,骨形成相关转录因子包括Runt相关转录因子2(runt-related transcription factor 2,Runx2)、肌节同源盒基因同系物2(muscle segment homeobox 2,Msx2)和骨基质蛋白包括骨钙素(osteocalcin)、骨形态发生蛋白2(bone morphogenetic protein 2,BMP2)、碱性磷酸酶(alkaline phosphatase,ALP)等表达增加,导致富含钙磷的基质囊泡释放增多,最终引起钙盐在血管壁的过度沉积[5]。已有研究证实,血管钙化与骨代谢紊乱呈正相关,血管钙化患者通常患有严重的骨质疏松[6]。在微重力环境下,航天员承重骨骨质显著丢失,骨钙以每月1.0%~1.6%的速度减少[7]。微重力环境下骨钙的丢失和颈总动脉僵硬度的增加是否预示着某种程度的血管钙化,目前尚不清楚。本实验采用模拟失重大鼠模型研究失重对心血管系统的影响,观察中期(4周)及长期(8周)模拟失重是否引起大鼠颈总动脉钙化,旨在为微重力环境下心血管失调的研究提供新的思路。
雄性SD大鼠75只,8周龄,体重180~200g(空军军医大学实验动物中心提供)。钙含量检测试剂盒、茜素红购自美国Sigma公司,ALP活性检测试剂盒购自中国南京建成生物工程研究所,Runx2兔多克隆抗体、BMP2兔多克隆抗体、osteocalcin鼠单克隆抗体、SM22α兔多克隆抗体购自英国Abcam公司,Msx2兔多克隆抗体购自美国Santa Cruz公司,β-actin兔多克隆抗体购自美国Proteintech公司,辣根过氧化物酶(HRP)标记的山羊抗兔、山羊抗小鼠IgG抗体购自中国北京中杉金桥生物公司,蛋白裂解液M-PER、BCA试剂盒、4%~12% Bis-tris预制胶、电泳缓冲液和转膜缓冲液购自美国Thermo公司,PVDF膜、ECL发光液购自美国Millipore公司,PCR引物购自中国生工生物工程(上海)股份有限公司,RNA提取试剂盒、反转录试剂盒、实时荧光定量PCR试剂盒购自日本TaKaRa公司。其余试剂均为国产分析纯试剂。实验过程符合国家及单位有关动物管理和使用的规定。
采用尾部悬吊后肢去负荷模型模拟失重对心血管系统的影响[8-9]。大鼠适应性饲养1周后进行实验,按照体重配对原则随机分为对照组、4周尾部悬吊组(HU4周组)和8周尾部悬吊组(HU8周组),每组25只。在模拟失重期间,大鼠始终保持尾部悬吊、后肢去负荷及–30°头低位状态,前肢自由活动,可自由进食和饮水。在HU8周组大鼠尾部悬吊4周后,开始进行HU4周组大鼠的尾部悬吊处理。对照组大鼠在同一动物室内单笼饲养,不进行尾部悬吊处理。动物室室温20~24 ℃,每昼夜保持12 h光照与12 h黑暗循环交替。模拟失重完成后,腹腔注射戊巴比妥钠(40 mg/kg)麻醉并放血处死大鼠,分离颈总动脉,称量左侧后肢比目鱼肌质量以验证模拟失重的效果。
取颈总动脉(约10 mg)于60 ℃彻底烤干,称重,加入浓硝酸300 μl,于180 ℃硝化并使硝酸挥发干净,加入去离子水数次至pH为中性后将水烤干,冷却后加入钙复溶液(含27 nmol/L氯化钾和27 μmol/L氯化镧)复溶管壁内的钙,过夜后收集至EP管中。采用钙试剂盒测定溶液中的钙离子浓度,通过血管干重标准化计算单位干重的钙含量。
造模完成后,经左心室灌注4%多聚甲醛进行原位固定,分离颈总动脉并置于4%多聚甲醛溶液继续固定24 h,石蜡包埋,制成5 μm厚度切片。切片经二甲苯脱蜡、梯度乙醇脱水后,浸入茜素红溶液5 min,蒸馏水冲洗,脱水、二甲苯透明、中性树胶封片,显微镜下观察。
将颈总动脉转移至玻璃匀浆器中,加入生理盐水,于冰上进行匀浆,4 ℃离心(2500 r/min,10 min),吸取上清。采用BCA试剂盒测定样本的蛋白浓度,采用ALP活性检测试剂盒测定样本的ALP活性,具体操作参照说明书进行,通过蛋白含量标准化计算ALP活性。
将冻存的血管组织置于含100 g/L蛋白酶抑制剂的蛋白裂解液中,于冰上进行匀浆,4 ℃离心(12 000 r/min,10 min),小心吸取上清,采用BCA试剂盒进行蛋白定量。制备蛋白样品并进行电泳,每条泳道上样量20 μg。电泳结束后,采用半湿转法将蛋白从凝胶转移至PVDF膜上。将PVDF膜浸入5% BSA溶液中室温封闭4 h,随后加入一抗(Runx2抗体,1:1000;Msx2抗体,1:500;BMP2抗体,1:1000;osteocalcin抗体,1:500;SM22α抗体,1:1000;β-actin抗体,1:1000),4℃孵育过夜,洗膜后加入HRP标记的山羊抗兔或山羊抗小鼠IgG抗体(1:10 000),室温孵育90 min,洗膜,使用ECL发光液发光显影,采用Tanon 4200凝胶成像系统采集蛋白表达信号。
采用RNA提取试剂盒匀浆、裂解颈总动脉组织,提取组织中的总RNA,测定总RNA浓度。根据反转录试剂说明书,将提取的RNA反转录生成cDNA。随后以cDNA为模板,用TB Green进行实时定量PCR,每组设3个复孔。引物序列:Runx2,正义5'-CTCTTCCCAAAGCCAGAGCG-3',反义5'-ACCA TCCTGGAAGGAGACCG-3';Msx2,正义5'-GCAGG AGGCGGAACTGGAAAAG-3',反义5'-TGGGAAGC ACAGGTCTATGGAAGG-3';BMP2,正义5'-AAGC GTCAAGCCAAACACAAACAG-3',反义5'-CCAGTC ATTCCACCCCACATCAC-3';osteocalcin,正义5'-AAT AGACTCCGGCGCTACCT-3',反义5'-CTGGAAGCCA ATGTGGTCCG-3';SM22α,正义5'-TGGCGTGATT CTGAGCAAGTTGG-3',反义5'-AATTGAGCCACCT GTTCCATCTGC-3';β-actin,正义5'-TCAGGTCATC ACTATCGGCAAT-3',反义5'-AAAGAAAGGGTGTAA AACGCA-3'。PCR反应条件:95 ℃预变性30 s;95 ℃ 5 s、60 ℃ 30 s,40个循环。熔解曲线分析:65~95 ℃。以β-actin为内参,采用2–ΔΔCt法计算mRNA的相对表达水平。
采用SPSS 20.0软件进行统计分析。定量资料若符合正态性,则采用$\bar{x}±s$进行描述,若数据同时满足正态性和方差齐性,则多组间比较采用单因素方差分析(One-way ANOVA),组间多重比较采用Tukey's post hoc检验。P<0.05为差异有统计学意义。
实验期间各组大鼠生长状况良好,体重持续增长。尾部悬吊结束时,三组大鼠的体重差异无统计学意义。与对照组相比,HU4周组及HU8周组大鼠的比目鱼肌湿重、比目鱼肌湿重/体重均明显降低(P<0.05或P<0.01);与HU4周组相比,HU8周组大鼠的比目鱼肌湿重、比目鱼肌湿重/体重亦明显降低(P<0.05,表1)。
茜素红染色结果显示,对照组大鼠颈总动脉未见钙盐沉积特征,HU4周组及HU8周组大鼠颈总动脉可见沿弹力纤维分布的深红色钙盐沉积(图1A)。钙含量检测结果显示,与对照组相比,HU4周组及HU8周组大鼠颈总动脉的钙含量均明显增加(P<0.01);而HU4周组及HU8周组比较差异无统计学意义(P>0.05,图1B),与茜素红染色结果一致。
颈总动脉ALP活性检测结果显示,与对照组相比,HU4周组及HU8周组大鼠颈总动脉的ALP活性明显增强(P<0.01或P<0.05);与HU4周组相比,HU8周组大鼠颈总动脉的ALP活性略有降低,但差异无统计学意义(图2)。
Western blotting检测结果显示,与对照组相比,HU4周组及HU8周组大鼠颈总动脉的Runx2、Msx2、BMP2和osteocalcin蛋白表达均明显增加(P<0.05),SM22α蛋白表达明显降低(P<0.05);而HU4周组与HU8周组大鼠颈总动脉上述各蛋白的表达差异均无统计学意义(图3)。
实时荧光定量PCR检测结果显示,与对照组相比,HU4周组及HU8周组大鼠颈总动脉Runx2、Msx2、BMP2和osteocalcin的mRNA表达水平均明显上调(P<0.01),SM22α的mRNA表达水平明显下调(P<0.01);与HU4周组相比,HU8周组大鼠颈总动脉Runx2的mRNA表达水平上调(P<0.01),而其他因子的mRNA表达水平两组比较差异无统计学意义(图4)。
本研究通过检测4周和8周模拟失重大鼠颈总动脉钙化相关指标的变化,探讨了中期和长期模拟失重是否可以引起大鼠颈总动脉钙化。结果显示,4周和8周模拟失重可引起大鼠颈总动脉钙含量增加、钙盐沉积增多,ALP活性增强,Runx2、Msx2、BMP2、osteocalcin蛋白和mRNA表达增加以及SM22α蛋白和mRNA表达降低。
颈总动脉作为大脑血流供应的重要通路,对维持脑部氧气和营养物质的供应起着至关重要的作用。颈总动脉结构和功能的异常与脑部缺血症状如头痛、头晕、记忆力减退、思维能力降低甚至缺血性脑卒中等存在密切联系。在微重力环境下,颈总动脉的功能和结构发生重建[2]。地面模拟实验证实,模拟失重大鼠的动脉功能和结构重建主要涉及局部肾素-血管紧张素系统、一氧化氮通路、离子通道、氧化应激、炎症反应及线粒体损伤等[10-13]。最近研究发现,航天飞行6个月后航天员颈总动脉的僵硬度增加17%~30%,其增加程度相当于正常血管10~20年的老化进程[4],这可能会对航天员的健康造成一定威胁,但其发生机制仍不清楚。
血管钙化是高血压、糖尿病、动脉粥样硬化、慢性肾病、衰老等多种心血管疾病普遍存在的病理改变,主要表现为血管壁僵硬度增加和顺应性降低,容易引发动脉血栓形成和斑块破裂,是导致心血管事件的重要危险因素。根据发生部位的不同,血管钙化可分为内膜钙化、中膜钙化和心脏瓣膜钙化。内膜钙化与动脉粥样硬化密切相关,呈点状、局限性分布;中膜钙化常见于高血压、慢性肾病损伤的血管,钙盐在平滑肌细胞周围或沿弹力层分布[14-16]。本研究发现4周和8周模拟失重大鼠的颈总动脉钙含量显著增加,并可见沿弹力纤维分布的钙盐沉积,提示中期和长期模拟失重可导致大鼠颈总动脉发生钙化;长期模拟失重并未显著改变钙盐沉积的程度,使得血管钙化仍然存在,此结果与长期航天飞行后航天员颈总动脉僵硬度增加的表现一致。微重力环境和模拟失重条件下,颈总动脉跨壁压升高、血流速度增加[3],该血流动力学变化对血管壁产生的机械应力刺激可能是引起颈总动脉钙化的关键因素。
血管钙化的细胞来源具有多样性,血管平滑肌细胞、周细胞、钙化血管细胞,以及血管壁内的间充质干细胞都具有向成骨样细胞转化的潜能,其中平滑肌细胞向成骨样细胞表型转化是血管钙化的主要细胞来源[17]。在高血压、糖尿病、氧化应激、炎症等致钙化因素作用下,平滑肌细胞收缩蛋白标志分子表达减少,而成骨样细胞表型转化相关因子表达增加,使平滑肌细胞转化为具有合成和分泌功能的成骨样细胞,释放基质囊泡,从而在局部形成有利于钙盐沉积的微环境[18]。本研究检测了ALP的活性以及其他成骨样细胞表型转化相关蛋白和mRNA的表达,发现4周和8周模拟失重大鼠颈总动脉的ALP活性均显著增强,Runx2、Msx2、BMP2和osteocalcin蛋白和mRNA表达均显著增加,提示中期和长期模拟失重均可引起颈总动脉平滑肌细胞由收缩表型向成骨样细胞表型转化。另外,本研究比较了中期和长期模拟失重对颈总动脉平滑肌细胞表型转化的影响,发现与中期模拟失重相比,长期模拟失重除了上调Runx2基因表达以外,并未显著改变其他成骨样细胞表型转化相关蛋白和基因的表达,而长期模拟失重引起的Runx2基因和蛋白表达变化不一致可能与转录后蛋白修饰有关,其确切机制仍有待进一步研究。
综上所述,本研究发现中期和长期模拟失重均可引起大鼠颈总动脉平滑肌细胞由收缩表型向成骨样细胞表型转化,促进钙盐沉积并引起动脉钙化,但其钙化程度并未随着模拟失重时间的延长而发生明显变化,推测微重力环境下颈总动脉钙化呈现相对稳定和持续的状态,这可能与致钙化因素、钙化抑制因素的平衡调节有关,而延长模拟失重时间是否会改变颈总动脉钙化程度目前尚不明确。在后续研究中,将深入探讨模拟失重大鼠颈总动脉钙化的相关影响因素及其关键分子机制,为航天后立位耐力不良防护措施的研究提供新的思路。
