Objective To evaluate the efficiency and safety of single ProGlide pre-suture technique for hemostasis of vein puncture point after removal of the sheath tube in patients undergoing percutaneous intervention with large-caliber femoral vein sheath. Methods A total of 26 patients were enrolled in present retrospective analysis, including 18 patients receiving extracorporeal membrane oxygenation and 8 patients undergoing radiofrequency ablation operation for arrhythmia. Clinical evaluation and vascular ultrasound examination were performed for all the subjects after operation to observe, record and evaluate the success rate of single ProGlide pre-suture technique and the incidence of complications at venipuncture points during hospitalization. Results Twenty males and 6 females with average age of (66.5±11.7) years were recruited in present study, including 10 cases of hypertension (38.5%),7 cases of diabetes (26.9%), 4 cases of chronic renal insufficiency (15.4%), 2 cases of chronic obstructive pulmonary disease (7.7%)and 17 cases of peripheral arterial disease (65.4%). The achievement ratio of single ProGlide pre-suture technique was 96.2%, only one patient required hemostasis by manual compression after removal of the sheath and suture. No complications related to venipuncture occurred during hospitalization period. Conclusion In percutaneous intervention, rapid, safe and effective hemostasis of vein puncture point can be achieved with single ProGlide pre-suture technique after removal of the large-caliber vein sheath.

目的 评估在置入大直径股静脉穿刺鞘的经皮介入手术中,应用单把ProGlide血管缝合器预缝合技术行拔管后穿刺点止血的安全性和有效性。方法 本研究为回顾性研究。共收集26例应用单把ProGlide血管缝合器预缝合技术行拔管后穿刺点止血的患者,其中接受经皮体外膜肺氧合治疗者18例,心律失常经导管射频消融手术者8例。所有患者均在术后接受临床评估及血管超声检查,观察技术成功率及住院期间静脉穿刺点的并发症发生率。结果 26例中,男20例、女6例,年龄(66.5±11.7)岁,合并高血压10例(38.5%)、糖尿病7例(26.9%)、慢性肾功能不全4例(15.4%)、慢性阻塞性肺疾病2例(7.6%)、外周动脉疾病17例(65.4%)。单把ProGlide血管缝合器预缝合技术成功率为96.2%。仅1例接受体外膜肺氧合治疗的患者在拔管缝合后,由于仍存在明显的出血而转为人工按压止血。住院期间所有患者均未发生静脉穿刺相关并发症。结论 在经皮介入手术中,单把ProGlide预缝合技术可实现大直径静脉鞘管拔除后穿刺点的快速、安全和有效止血。

目的 评估在置入大直径股静脉穿刺鞘的经皮介入手术中,应用单把ProGlide血管缝合器预缝合技术行拔管后穿刺点止血的安全性和有效性。方法 本研究为回顾性研究。共收集26例应用单把ProGlide血管缝合器预缝合技术行拔管后穿刺点止血的患者,其中接受经皮体外膜肺氧合治疗者18例,心律失常经导管射频消融手术者8例。所有患者均在术后接受临床评估及血管超声检查,观察技术成功率及住院期间静脉穿刺点的并发症发生率。结果 26例中,男20例、女6例,年龄(66.5±11.7)岁,合并高血压10例(38.5%)、糖尿病7例(26.9%)、慢性肾功能不全4例(15.4%)、慢性阻塞性肺疾病2例(7.6%)、外周动脉疾病17例(65.4%)。单把ProGlide血管缝合器预缝合技术成功率为96.2%。仅1例接受体外膜肺氧合治疗的患者在拔管缝合后,由于仍存在明显的出血而转为人工按压止血。住院期间所有患者均未发生静脉穿刺相关并发症。结论 在经皮介入手术中,单把ProGlide预缝合技术可实现大直径静脉鞘管拔除后穿刺点的快速、安全和有效止血。

Objective To evaluate the efficiency and safety of single ProGlide pre-suture technique for hemostasis of vein puncture point after removal of the sheath tube in patients undergoing percutaneous intervention with large-caliber femoral vein sheath. Methods A total of 26 patients were enrolled in present retrospective analysis, including 18 patients receiving extracorporeal membrane oxygenation and 8 patients undergoing radiofrequency ablation operation for arrhythmia. Clinical evaluation and vascular ultrasound examination were performed for all the subjects after operation to observe, record and evaluate the success rate of single ProGlide pre-suture technique and the incidence of complications at venipuncture points during hospitalization. Results Twenty males and 6 females with average age of (66.5±11.7) years were recruited in present study, including 10 cases of hypertension (38.5%),7 cases of diabetes (26.9%), 4 cases of chronic renal insufficiency (15.4%), 2 cases of chronic obstructive pulmonary disease (7.7%)and 17 cases of peripheral arterial disease (65.4%). The achievement ratio of single ProGlide pre-suture technique was 96.2%, only one patient required hemostasis by manual compression after removal of the sheath and suture. No complications related to venipuncture occurred during hospitalization period. Conclusion In percutaneous intervention, rapid, safe and effective hemostasis of vein puncture point can be achieved with single ProGlide pre-suture technique after removal of the large-caliber vein sheath.

经皮介入手术具有创伤小、治疗效果好、术后恢复时间短等优点,在多种心脏疾病的治疗中被越来越广泛的应用[1]。然而,很多新技术比常规介入手术需要更大的血管通路,如经皮主动脉瘤腔内隔绝术需要12F~24F的动脉鞘、二尖瓣钳夹术需要24F静脉鞘、经皮体外膜肺氧合技术需要15F~24F的动静脉鞘。因此,如何实现术后穿刺点的快速止血并减少血管并发症成为这些新技术所面临的最大挑战。ProGlide是一种基于外科缝合理念的经皮血管缝合装置。2007年的一项研究报道了在接受动脉瘤腔内修补术的患者中应用该缝合器的预缝合技术,即在大尺寸鞘管插入前先置入两把ProGlide血管缝合器,并将缝线留置在体外,待手术结束拔除鞘管时,再收紧缝线完成血管止血[2]。目前该技术被广泛用于介入手术动脉通路的止血。然而,静脉通路穿刺点止血的方法仍较为有限,普遍以人工按压以及“8”字缝合等传统方法为主[3],增加了医护人员的工作量,且术后仍需加压包扎,延长了患者卧床时间、降低了舒适度。本研究旨在探讨利用单把ProGlide缝合器预缝合技术是否可安全、有效地实现大直径鞘管(14F~19F)撤除后股静脉穿刺点的快速止血。
回顾性分析空军军医大学第一附属医院心血管内科2019年3月—2020年6月接受经皮介入手术需要穿刺股静脉并置入大直径鞘管的26例,其中经皮穿刺建立体外膜肺氧合18例,心律失常射频消融手术8例。所有患者均采用单把ProGlide缝合器预置技术对静脉穿刺点进行止血,术后第2天接受临床评估及血管超声检查,以排除股静脉穿刺点相关并发症。本研究通过空军军医大学第一附属医院伦理委员会审批。
手术均由经验丰富的心血管介入医师在心内科导管室或床旁实施。经皮穿刺股总静脉后置入6F鞘管,实施单把ProGlide(雅培公司,美国)预缝合技术,操作步骤按说明书(图1)及文献[2]报道进行。由于股静脉压力较动脉低,导致ProGlide出血孔常无回血,对判断缝合器是否进入血管造成困难。这时应尽可能将血管缝合器插入血管,从缝合器侧孔注入2 ml造影剂以判断缝合器位置。确定缝合器位置合适后即可释放缝合器,最后更换6F鞘管为手术需要的大直径鞘管。待手术结束后,助手在撤除鞘管后按压穿刺点,术者则收紧体外的ProGlide预置缝线来实施血管穿刺点的止血。
①有效性终点:定义为技术成功率,即在拔除股静脉鞘管并收紧预先留置的ProGlide缝线后,可在≤2 min内实现穿刺点的止血,无需人工压迫或其他方法止血;②安全性终点:定义为住院期间静脉穿刺部位的相关并发症,包括穿刺部位感染、血肿、出血、腹膜后血肿、静脉闭塞、急性或慢性静脉功能不全。
采用SPSS 19.0软件进行数据处理。本研究为一项对单个患者队列的描述性研究,仅采用描述性分析。符合正态分布的定量资料以$\bar{x}±s$表示;计数资料以例数(%)表示。
26例患者中,男20例,女6例,年龄(66.5±11.7)岁。合并高血压10例(38.5%)、糖尿病7例(26.9%)、慢性肾功能不全4例(15.4%)、慢性阻塞性肺疾病2例(7.6%)、外周动脉疾病17例(65.4%,表1)。
单把ProGlide血管预缝合技术应用于股静脉穿刺点止血的技术成功率为96.2%,缝合效果见图2。仅有1例接受体外膜肺氧合治疗的患者在撤除股静脉19F鞘管、收紧ProGlide缝线后,穿刺点仍有明显出血,在人工按压15 min后出血停止,给予弹力绷带加压处理。术后所有患者均接受临床评估及血管超声检查,住院期间均未出现股静脉穿刺部位血栓、狭窄、闭塞、血肿等血管并发症。
本研究发现,在接受穿刺股静脉实施介入手术的患者中,如需置入大直径鞘管时,利用ProGlide血管缝合器预缝合技术可实现术后穿刺点的快速止血(≤2 min)。尽管部分患者围术期需进行抗凝和抗血小板治疗,在一定程度上增加了出血风险,但所有患者术后均未发生严重的出血。仅1例接受体外膜肺氧合治疗的患者在拔管并收紧缝线后,股静脉穿刺点仍有明显的出血。在经过评估后,该患者接受了人工按压(15 min)并以弹力绷带加压包扎以完成止血。考虑可能是该患者比较肥胖,影响了缝合器的预置效果,从而导致技术失败。尽管如此,包括该患者在内的所有患者术后均接受临床评估及血管超声检查,住院期间未发现血栓、闭塞、血肿等血管并发症。
近年来,血管缝合器或闭合装置广泛用于股动脉穿刺点的快速止血,此类器械的使用可避免人工压迫,且可促使患者尽早下床活动。其中,ProGlide(一种基于外科缝合理念的血管缝合器)使介入医师可通过经皮方法来缝合穿刺血管[4]。有研究发现,与外科方法相比,ProGlide用于股动脉缝合具有更低的输血率、更短的住院周期、更低的住院花费[5]。最初,ProGlide的适应证是<8F鞘管拔除后的血管缝合。2007年的一项研究描述了一种ProGlide预缝合技术,该技术在大直径鞘管插入股动脉前先置入两把缝合器,类似于外科切开插管前对血管进行的预先荷包缝合。两把缝合器之间呈30°展开,可用于最大24F的血管鞘拔管后的止血[2]。该技术逐渐广泛用于动脉瘤腔内治疗、经导管主动脉瓣置换等需要大直径血管鞘的介入手术。
目前,血管缝合器或闭合器在静脉通路中的应用鲜见报道。人工压迫及皮下“8”字缝合为静脉穿刺点的传统止血方法,从临床实践来看,其主要缺点为医护人员工作量的增加及患者卧床时间的延长。同时,有研究报道,静脉通路采用皮下“8”字缝合方法后的严重出血发生率为4%,中等程度出血发生率为13.3%[6],且低体重患者由于缺乏皮下组织使该方法更易失败。此外,如手术需要在同侧置入动脉鞘和静脉鞘,由于穿刺点距离较近而很难使用“8”字缝合。而临床常用的一些血管闭合器同样存在局限性。一项研究使用Angio-seal血管闭合器对常规右心导管插入术后股静脉穿刺点进行止血,但该装置无法实现预置缝合技术[7]。另一种血管封堵器MynxGrip同样被用来行5F~7F静脉鞘管拔除后的止血,结果显示,与人工按压相比,两组的出血、血栓等并发症发生率差异均无统计学意义;而MynxGrip可明显减少止血所需时间[8]。另一项多中心、随机对照研究共纳入204例接受射频消融术的患者,对比Cardiva Mid-Bore封堵器与人工按压6F~12F鞘管拔除后静脉穿刺点止血的效果,结果显示,该血管封堵器在止血效果、减少患者卧床时间、提升患者满意度上均优于人工按压,且不增高并发症发生率[9]。然而,上述3种装置并不适用于12F以上静脉鞘管拔除后的穿刺点止血。
国外有研究将双把ProGlide缝合器应用于二尖瓣钳夹术后24F静脉鞘管拔除后的止血,最终42例均成功止血,术后1年随访未发现静脉狭窄、血栓等并发症,提示该装置具有较好的安全性及有效性[10]。本研究所采用的单把ProGlide预缝合技术是在该装置股动脉缝合经验的基础上发展而来的。与动脉相比,静脉具有管壁薄、压力小、弹性低的特点。因此,我们推测与动脉预缝合需要双把缝合器不同,实现股静脉术后穿刺点(14F~18F)的快速、有效地止血可能仅需单把缝合器即可。本研究最终结果也显示该技术的成功率可达96.2%,且术后并发症发生率极低,提示此方法在减少缝合器使用数量、降低医疗费用、保证医疗安全的同时,还可以显著缩短手术时间、减轻医护人员工作量。
综上所述,本研究结果显示,单把ProGlide血管闭合器预缝合技术用于经皮介入手术静脉穿刺点的止血是安全、有效的。未来可扩大样本量,开展随机对照研究,以比较人工按压或“8”字缝合与本方法在股静脉穿刺点止血效果、并发症发生率、患者满意度及卧床时间等方面的差异。
