Objective To explore the relationship between platelet function and disease progression and prognosis in patients with cerebral hemorrhage without antiplatelet agents. Methods A retrospective analysis was performed on the clinical data of 129 patients with intracerebral hemorrhage who were admitted to the Neurosurgical Department, the First Affiliated Hospital of Chongqing Medical University from January 2015 to December 2019. These patients were divided into two groups: rebleeding group after intracerebral hemorrhage (n=53) and non-rebleeding group (n=76). The relation of thrombelastograph parameters including R value, K value, Angle of view, and the inhibition rates of arachidonic acid (AA) and adenosine diphosphate (ADP) under the condition without use of antiplatelet drugs at admission and their correlation with Glasgow Coma Scale score were compared between the two groups. Results The AA inhibition rate (P=0.015) and ADP inhibition rate (P=0.025) in rebleeding group were statistically higher than those in non-rebleeding group. AA inhibition rate of platelet function (P=0.022) and ADP inhibition rate (P=0.030) in rebleeding death group were statistically higher than those in survival group. AA inhibition rate (r=–0.183,P=0.038) and ADP inhibition rate (r=–0.175, P=0.047) were negatively correlated with Glasgow Coma Scale score. Conclusions Thrombelastographic detection of platelet function can be used as an early warning indicator to assist the assessment of the risk of rebleeding and prognosis in patients with intracerebral hemorrhage.

目的 探讨未使用抗血小板药物的脑出血患者的血小板功能与疾病进展及预后的关系。方法 回顾性分析2015年1月—2019年12月重庆医科大学附属第一医院神经外科收治的129例脑出血患者的临床资料,将其分为脑出血再出血组(53例)与未再出血组(76例),比较两组入院未使用抗血小板药物情况下的血栓弹力图指标,包括R值、K值、Angle角、花生四烯酸(AA)抑制率、二磷酸腺苷(ADP)抑制率等与再出血及预后的关系,以及与格拉斯哥昏迷量表评分的相关性。结果 再出血组AA抑制率和ADP抑制率均明显高于未再出血组,差异有统计学意义(P=0.015、0.025);再出血组死亡患者AA抑制率和ADP抑制率均明显高于生存组,差异有统计学意义(P=0.022、0.030);AA抑制率和ADP制率与格拉斯哥昏迷量表评分呈负相关(r=–0.183,P=0.038;r=–0.175,P=0.047)。结论 血栓弹力图血小板功能可作为预警指标辅助评估脑出血患者的再出血风险及预后情况。

Objective To explore the relationship between platelet function and disease progression and prognosis in patients with cerebral hemorrhage without antiplatelet agents. Methods A retrospective analysis was performed on the clinical data of 129 patients with intracerebral hemorrhage who were admitted to the Neurosurgical Department, the First Affiliated Hospital of Chongqing Medical University from January 2015 to December 2019. These patients were divided into two groups: rebleeding group after intracerebral hemorrhage (n=53) and non-rebleeding group (n=76). The relation of thrombelastograph parameters including R value, K value, Angle of view, and the inhibition rates of arachidonic acid (AA) and adenosine diphosphate (ADP) under the condition without use of antiplatelet drugs at admission and their correlation with Glasgow Coma Scale score were compared between the two groups. Results The AA inhibition rate (P=0.015) and ADP inhibition rate (P=0.025) in rebleeding group were statistically higher than those in non-rebleeding group. AA inhibition rate of platelet function (P=0.022) and ADP inhibition rate (P=0.030) in rebleeding death group were statistically higher than those in survival group. AA inhibition rate (r=–0.183,P=0.038) and ADP inhibition rate (r=–0.175, P=0.047) were negatively correlated with Glasgow Coma Scale score. Conclusions Thrombelastographic detection of platelet function can be used as an early warning indicator to assist the assessment of the risk of rebleeding and prognosis in patients with intracerebral hemorrhage.

脑出血,指非外伤性脑内血管破裂导致脑实质内血液聚集,年发病率为12~15/10万人,占我国所有脑卒中的18.8%~47.6%,在脑卒中所有亚型中居第二位。脑出血发病凶险,急性期病死率高,发病30 d内病死率高达35%~52%,致残率达75%[1-4]。研究显示,凝血功能障碍导致血肿面积扩大是脑出血病情恶化的重要因素之一[5]。因此,凝血功能监测对脑出血患者病情判断及预后评估十分重要。血栓弹力图(thromboelastography,TEG)是一种动态反映凝血因子活性、血小板聚集功能和纤溶过程的检测方法[6]。血小板功能检测作为TEG检测的一部分,目前多用于评估使用抗血小板药物(阿司匹林、氯吡格雷等)后的血小板功能和凝血状态[7],以指导临床用药,但对未使用抗血小板药物患者的临床指导意义尚无文献报道。本研究旨在分析未使用抗血小板药物的脑出血患者血小板功能检测指标与疾病发展及预后的关系,探讨其在临床诊治及预后评估中的应用价值。
纳入2015年1月—2019年12月重庆医科大学附属第一医院神经外科收治的脑出血患者129例。纳入标准:(1)急性起病;(2)局灶神经功能缺损症状(少数为全面神经功能缺损),常伴有头痛、呕吐、血压升高以及不同程度的意识障碍;(3)头颅CT或MRI显示出血灶;(4)入院即完善TEG血小板功能检测。排除标准:(1)出血原因为脑外伤、动静脉畸形、烟雾病、高血压、特发性血小板减少性紫癜、血友病等;(2)TEG检测前患者服用影响血小板功能或血液系统的药物,包括华法林、阿司匹林、氯吡格雷、氨甲环酸等;(3)非血管性脑出血;(4)合并严重心、肝、肾疾病的患者。
根据疾病进展情况,将患者分为再出血组(53例)与未再出血组(76例)。再出血的诊断标准为患者在病情好转或稳定的情况下,突发头痛、恶心呕吐、抽搐、意识障碍等症状加重或再出现,复查CT显示颅内出血较前增加[8]。根据疾病预后,将再出血患者分为生存组与死亡组。生存组为出院时恢复良好、病情稳定的患者;死亡组为脑出血后再出血导致死亡的患者。
收集患者入院后即检测的TEG指标。R值:反映凝血因子活性;K值、Angle角:共同反映纤维蛋白原活性水平;最大振幅(maximum amplitude,MA)值:反映血小板功能(80%)和纤维蛋白原水平(20%);EPL、LY30:均为最大血凝块形成后30 min内血块溶解率,反映纤维蛋白溶解水平;花生四烯酸(arachidonic acid,AA)抑制率、二磷酸腺苷(adenosine diphosphate,ADP)抑制率:通过100%—(MAADP or MAAA)—MAfibrin/(MAthrombin—MAfibrin)×100%计算得出相应抑制率,分别反映AA、ADP诱导的血小板功能抑制程度。
入院后取前臂肘静脉血3 ml,分别置于枸橼酸抗凝管和肝素抗凝管中,15 min至2 h内完成检测。所有检测均采用美国Haemoscope公司生产的血栓弹力图分析仪TEG5000完成。同时从电子病历系统中调取所有患者的临床资料,根据1.2分组标准进行分组。
采用SPSS 25.0软件进行统计分析。利用Kolmogorov-Smirnov检验进行正态性检验。符合正态分布的计量资料以$\bar{x}±s$表示,组间比较采用t检验,不符合正态分布则以M(Q1Q3)表示,组间比较采用Mann-Whitney U检验。计数资料以百分比(%)表示,组间比较采用χ2检验。P<0.05为差异有统计学意义。不同组变量间与格拉斯哥昏迷量表(Glasgow coma scale,GCS)评分的相关性采用Spearman检验。检验水准 α=0.05。
129例脑出血患者中,未再出血组76例,其中男42例,女34例,平均47.9岁,死亡4例,有高血压和糖尿病史者分别为49例和27例;再出血组53例,其中男32例,女21例,平均53.3岁,死亡31例,有高血压和糖尿病史者分别为36例和17例。两组性别、年龄、高血压史、糖尿病史差异无统计学意义(P>0.05)。再出血组病死率较未再出血组明显升高,差异有统计学意义(χ²=28.984,P=0.000)(表1)。
未再出血组与再出血组R值、K值、Angle角、MA值、EPL值、LY30值比较差异均无统计学意义(P>0.05)。再出血组AA抑制率和ADP抑制率明显高于未再出血组,差异有统计学意义(P=0.015、0.025,表2)。
生存组与死亡组患者R值、K值、Angle角、MA值、EPL值、LY30值比较差异均无统计学意义(P>0.05)。死亡组AA抑制率和ADP抑制率明显高于生存组,差异有统计学意义(P=0.022、0.030,表3)。
再出血组GCS评分低于未再出血组,差异有统计学意义(Z=–2.622,P=0.009)。再出血死亡组患者GCS评分低于生存组,差异有统计学意义(Z=–2.540,P=0.011)。血小板功能检测中,AA抑制率和ADP抑制率越高,患者GCS评分越低,预后越差,二者与患者GCS评分呈明显负相关(r=–0.183,P=0.038;r=–0.175,P=0.047,图1)。
脑出血,指突发的非外伤性脑实质内出血,分为原发性脑出血和继发性脑出血。原发性脑出血无明确病因,50%以上由高血压引起,约30%与脑淀粉样血管病变有关。继发性脑出血一般有明确的病因,多由脑动静脉畸形、脑动脉瘤、口服抗凝药物等因素导致[9]。脑出血起病急,病情变化快,早期病死率高。研究显示,急性脑出血后常继发出血时间延长、纤溶过程异常等凝血功能障碍,早期患者凝血功能异常及血肿扩大是导致神经功能损害、病情恶化的重要原因[8]。脑出血患者D-二聚体显著升高,且与疾病严重程度和早期预后相关[10-11],凝血指标可作为判断患者预后的依据之一[12-13]
TEG是一种通过检测血栓黏弹力动态变化,反映全血状态下凝血因子活性、血小板聚集功能、纤维蛋白原功能和纤维溶解功能的凝血功能检测方法,其结果与传统凝血四项有良好的相关性,而灵敏度优于凝血四项检测[14],可动态反映全血状态下的凝血全貌,目前已广泛应用于临床。其中,普通高岭土检测用于评估未使用抗血小板药物患者的凝血状态,在脑血管疾病的诊治中有指导作用,R值越低,表明凝血因子活性越低,脑外伤病情越重,预后越差[15],K值、Angle与MA值与患者脑出血量明显相关,可作为监测脑出血患者凝血功能的良好指标[16]。血小板功能AA抑制率、ADP抑制率分别用于评估使用抗血小板药物后花生四烯酸、二磷酸腺苷通道的血小板抑制率,可指导用药方案的选择。临床上通过AA抑制率、ADP抑制率能有效评估阿司匹林、氯吡格雷等抗血小板药物在急性缺血性脑卒中、冠心病中的疗效[17-18]。但目前国内外有关TEG血小板功能检测在未使用抗血小板药物患者中的应用报道较少。何秋光等[19]探究了血小板功能检测在动脉瘤蛛网膜下腔出血中应用,发现ADP抑制率与疾病Hunt-Hess分级、Fisher分级呈正相关,且再出血患者AA抑制率、ADP抑制率较未再出血患者显著增高。但血小板功能检测在未使用抗血小板药物的脑出血患者中的应用及其临床意义目前暂无研究报道。脑出血早期的病情严重程度及预后与凝血状态关系密切,因此本研究分析了未使用抗血小板药物的脑出血患者血小板功能检测与疾病进展及预后的关系,并探讨其临床意义。
本研究分析了在未使用抗血小板药物的情况下,129例脑出血患者TEG血小板功能指标及其与疾病发展、预后的关系。将所有纳入患者分为未再出血组(76例)与再出血组(53例),两组年龄、性别、高血压史、糖尿病史差异无统计学意义,具有可比性。再出血组的病死率(58.5%)明显高于未再出血组(5.3%),差异有统计学意义。两组血小板功能检测中,再出血组AA抑制率和ADP抑制率显著高于未再出血组,提示AA抑制率和ADP抑制率能较好地预测脑出血患者发生再出血的风险,AA抑制率和ADP抑制率越高,AA通道和ADP通道诱导的血小板聚集功能越差,发生再出血的风险越大。这与何秋光等[19]在蛛网膜下腔出血患者中TEG血小板功能检测的报道一致。
对TEG血小板功能与再出血组患者预后的关系进行分析发现,死亡组AA抑制率和ADP抑制率明显高于生存组,入院GCS评分明显低于生存组,差异有统计学意义,且AA抑制率和ADP抑制率与患者入院GCS评分呈明显负相关。GCS是目前公认且临床常用的评定脑出血严重程度的评分量表,评分越低,表明脑出血严重程度越高[20]。本研究GCS评分结果与既往报道一致,且AA抑制率和ADP抑制率与其具有良好的相关性,可辅助GCS评分判断脑出血的预后,AA抑制率、ADP抑制率越高,GCS评分越低,则预后越差。但本研究为回顾性研究,样本量有限,所得结果有待进一步通过大样本前瞻性试验进行论证。
综上所述,凝血功能障碍是脑出血早期出血量扩大、病死率增高的重要因素,TEG检测血小板功能可作为预警指标辅助评估脑出血患者的再出血风险及预后情况,AA抑制率和ADP抑制率越高,发生再出血的风险越大,病死率越高。
