Objective To compare the clinical biochemical features, metabolic abnormalities, difference of hormone level and related factors between male and female patients with subclinical Cushing's syndrome (SCS). Methods The clinical data of 118 SCS patients (38 males and 80 females), admitted in the Department of Endocrinology, the First Medical Center of Chinese PLA General Hospital from January 2010 to January 2020, were retrospectively analyzed. The clinical characteristics, biochemical indices, metabolic disturbance, circadian rhythm of plasma adrenocorticotropic hormone (ACTH) and cortisol levels (ACTH-F),ACTH and cortisol levels at 08:00 am. after 1 mg dexamethasone suppression test (1 mg DST) and classical low-dose dexamethasone suppression test (low-dose DST) were compared between male and female SCS patients. Logistic regression analysis of F00:00, 1 mg DST-F08:00 and low-dose DST-F08:00 levels was performed for SCS patients of different genders. Results The BMI, triglyceride level and the proportion of abnormal glucose metabolism were obviously higher in male SCS patients than those in female patients[(28.08±8.30) kg/m2 vs. (25.57±3.76) kg/m2, P=0.026; (1.68±1.04) mmol/L vs. (1.35±0.69) mmol/L, P=0.045 and 60.5% vs. 37.5%, P=0.019], while no statistical difference existed between male and female patients in age, course of SCS, blood pressure levels, morbidity of hypertension, bone density and bone metabolic markers etc. The 1mg DST-F08:00 and low-dose DST-F08:00 were significantly higher in female than those in male SCS patients, and the significant difference still existed after adjusting for age, onset age, BMI and course of disease. Meanwhile, no significant effect was detected on F00:00, 1mg DST-F08:00 and low-dose DST-F08:00 between premenopausal and postmenopausal female patients. Conclusion The BMI, triglyceride level and the proportion of abnormal glucose metabolism were lower in female than in male SCS patients, but the F00:00, 1mg DST-F08:00 and low-dose DST-F08:00 were higher in female than in male SCS patients, and such differences still existed after adjusting for age, onset age, BMI and course of disease.

目的 比较不同性别亚临床库欣综合征(SCS)患者的临床生化特点、代谢异常情况、激素水平差异及相关影响因素。方法 回顾性分析2010年1月—2020年1月因肾上腺意外瘤(AI)在解放军总医院第一医学中心内分泌科住院诊断的SCS患者118例,其中男38例,女80例,比较不同性别患者的临床特点、生化指标、代谢异常、血促肾上腺皮质激素-皮质醇(ACTH-F)节律、午夜1 mg和经典小剂量地塞米松抑制试验(DST)等检测结果的差异,并行logistic回归分析SCS患者中性别与午夜0:00时皮质醇(F00:00)、1 mg地塞米松抑制实验后晨8:00时皮质醇(1 mg DST-F08:00)及小剂量地塞米松抑制实验后晨8:00时皮质醇(小剂量DST-F08:00)之间的关系。结果 男性SCS患者体重指数[BMI,(28.08±8.30) kg/m2 vs. (25.57±3.76) kg/m2P=0.026]、三酰甘油水平[(1.68±1.04) mmol/L vs. (1.35±0.69) mmol/L,P=0.045]及糖代谢异常比例(60.5% vs. 37.5%,P=0.019)均明显高于女性患者,而年龄、SCS病程、血压水平、高血压患病率、骨代谢指标、骨密度等两组间差异无统计学意义。女性SCS患者F00:00、1 mg DST-F08:00及小剂量DST-F08:00均明显高于男性患者(P<0.05)。以年龄、起病年龄、BMI、SCS病程为校正因素行logistic回归分析后,上述指标性别间差异仍有统计学意义(P<0.05)。以是否绝经对女性患者进行分层分析,校正年龄、起病年龄、BMI、SCS病程后的logistic回归分析结果显示,是否绝经对女性患者F00:00、1 mg DST-F08:00、小剂量 DST-F08:00水平无明显影响。结论 女性SCS患者BMI、三酰甘油及糖代谢异常比例低于男性;但其F00:00、1 mg DST-F08:00、小剂量DST-F08:00水平高于男性,且不受年龄、发病年龄、BMI及病程等因素的影响。

目的 比较不同性别亚临床库欣综合征(SCS)患者的临床生化特点、代谢异常情况、激素水平差异及相关影响因素。方法 回顾性分析2010年1月—2020年1月因肾上腺意外瘤(AI)在解放军总医院第一医学中心内分泌科住院诊断的SCS患者118例,其中男38例,女80例,比较不同性别患者的临床特点、生化指标、代谢异常、血促肾上腺皮质激素-皮质醇(ACTH-F)节律、午夜1 mg和经典小剂量地塞米松抑制试验(DST)等检测结果的差异,并行logistic回归分析SCS患者中性别与午夜0:00时皮质醇(F00:00)、1 mg地塞米松抑制实验后晨8:00时皮质醇(1 mg DST-F08:00)及小剂量地塞米松抑制实验后晨8:00时皮质醇(小剂量DST-F08:00)之间的关系。结果 男性SCS患者体重指数[BMI,(28.08±8.30) kg/m2 vs. (25.57±3.76) kg/m2P=0.026]、三酰甘油水平[(1.68±1.04) mmol/L vs. (1.35±0.69) mmol/L,P=0.045]及糖代谢异常比例(60.5% vs. 37.5%,P=0.019)均明显高于女性患者,而年龄、SCS病程、血压水平、高血压患病率、骨代谢指标、骨密度等两组间差异无统计学意义。女性SCS患者F00:00、1 mg DST-F08:00及小剂量DST-F08:00均明显高于男性患者(P<0.05)。以年龄、起病年龄、BMI、SCS病程为校正因素行logistic回归分析后,上述指标性别间差异仍有统计学意义(P<0.05)。以是否绝经对女性患者进行分层分析,校正年龄、起病年龄、BMI、SCS病程后的logistic回归分析结果显示,是否绝经对女性患者F00:00、1 mg DST-F08:00、小剂量 DST-F08:00水平无明显影响。结论 女性SCS患者BMI、三酰甘油及糖代谢异常比例低于男性;但其F00:00、1 mg DST-F08:00、小剂量DST-F08:00水平高于男性,且不受年龄、发病年龄、BMI及病程等因素的影响。

亚临床库欣综合征  /  性别  /  皮质醇  /  地塞米松抑制试验

Objective To compare the clinical biochemical features, metabolic abnormalities, difference of hormone level and related factors between male and female patients with subclinical Cushing's syndrome (SCS). Methods The clinical data of 118 SCS patients (38 males and 80 females), admitted in the Department of Endocrinology, the First Medical Center of Chinese PLA General Hospital from January 2010 to January 2020, were retrospectively analyzed. The clinical characteristics, biochemical indices, metabolic disturbance, circadian rhythm of plasma adrenocorticotropic hormone (ACTH) and cortisol levels (ACTH-F),ACTH and cortisol levels at 08:00 am. after 1 mg dexamethasone suppression test (1 mg DST) and classical low-dose dexamethasone suppression test (low-dose DST) were compared between male and female SCS patients. Logistic regression analysis of F00:00, 1 mg DST-F08:00 and low-dose DST-F08:00 levels was performed for SCS patients of different genders. Results The BMI, triglyceride level and the proportion of abnormal glucose metabolism were obviously higher in male SCS patients than those in female patients[(28.08±8.30) kg/m2 vs. (25.57±3.76) kg/m2, P=0.026; (1.68±1.04) mmol/L vs. (1.35±0.69) mmol/L, P=0.045 and 60.5% vs. 37.5%, P=0.019], while no statistical difference existed between male and female patients in age, course of SCS, blood pressure levels, morbidity of hypertension, bone density and bone metabolic markers etc. The 1mg DST-F08:00 and low-dose DST-F08:00 were significantly higher in female than those in male SCS patients, and the significant difference still existed after adjusting for age, onset age, BMI and course of disease. Meanwhile, no significant effect was detected on F00:00, 1mg DST-F08:00 and low-dose DST-F08:00 between premenopausal and postmenopausal female patients. Conclusion The BMI, triglyceride level and the proportion of abnormal glucose metabolism were lower in female than in male SCS patients, but the F00:00, 1mg DST-F08:00 and low-dose DST-F08:00 were higher in female than in male SCS patients, and such differences still existed after adjusting for age, onset age, BMI and course of disease.

近年来,随着影像学检查手段的不断进步,肾上腺意外瘤(adrenal incidentaloma,AI)检出率逐年增高。国外学者关于AI的相关研究发现,分泌皮质醇的肿瘤在功能性肿瘤中的占比最高且是临床上面临的最大挑战[1]。Comlekci等[2]回顾性分析了376例AI患者,其中41例为亚临床库欣综合征(subclinical Cushing's syndrome,SCS),且女性明显多于男性(4.1:1)。目前,有关库欣综合征(Cushing's syndrome,CS)的研究较多[3],但关于SCS的研究较少,进一步针对不同性别SCS患者的分析研究则更少。尽管SCS的诊断尚存在许多争议[4],但多数学者认为午夜0:00时皮质醇水平(F00:00)、1 mg地塞米松抑制实验(1 mg dexamethasone suppression test,1 mg DST)后晨8:00时皮质醇水平(1 mg DST-F08:00)及小剂量地塞米松抑制实验(low dose dexamethasone suppression test,low-dose DST)后晨8:00时皮质醇水平(low-dose DST-F08:00)对SCS的诊断具有重要价值。本研究收集了近10年解放军总医院第一医学中心内分泌科因AI就诊、经完善系列检查后被临床诊断为SCS的118例患者的临床资料,回顾性分析不同性别患者的临床特点和上述三项指标的性别差异。
纳入2010年1月—2020年1月因与肾上腺无关的疾病行影像学检查或常规体检时发现AI,并就诊于解放军总医院第一医学中心内分泌科的118例SCS患者的病例资料进行回顾性分析,其中男38例,女80例。本研究经解放军总医院第一医学中心伦理委员会批准(S2020-493-01)。目前SCS确诊试验及诊断最佳切点尚未统一,2016年欧洲内分泌学会(European Society of Endocrinology,ESE)和欧洲肾上腺肿瘤研究网络(European Network for the Study of Adrenal Tumours,ENSAT)联合发布的AI诊治指南[5]推荐采用1 mg DST作为一线筛查试验。本研究对所有入组患者进行1 mg DST后如有异常结果,补充行小剂量DST,采用1 mg DST-F08:00≥50 nmol/L(敏感性75%~100%,特异性67%~72%)作为诊断截点[6]。所有患者均告知上述检查注意事项并签署知情同意书。SCS诊断标准:(1)缺乏高皮质醇血症的典型临床表现外貌体征,如多血貌、紫纹、皮肤淤斑等;(2)实验室检查血促肾上腺皮质激素-皮质醇(adrenocorticotropic hormone-cortisol,ACTH-F)节律紊乱、24 h尿游离皮质醇(24 h urinary free cortisol,24 h UFC)、F00:00升高等两项以上异常且不能被1 mg DST抑制(1 mg DST-F08:00≥50 nmol/L)。纳入标准:经临床及内分泌功能评估为SCS者。排除标准:(1)有酗酒、劳累、创伤、抑郁病史,有严重肝肾功能异常,就诊前半年内有外源性糖皮质激素(包括口服药剂、外用软膏、可疑中草药等)或其他影响下丘脑-垂体-肾上腺轴功能药物应用史;(2)经临床或术后病理诊断考虑为原发性醛固酮增多症、嗜铬细胞瘤、肾上腺皮质癌、肾上腺转移癌、先天性肾上腺皮质增生症、节细胞神经瘤、神经鞘瘤、肾上腺血肿、肾上腺无功能瘤及其他未明确诊断者。
所有患者行身高、体重等检查并计算体重指数(body mass index,BMI)。影像学检查包括肾上腺彩超、肾上腺CT或MRI。骨密度测量采用美国HOLOGIC,Discovery-Wi型双能X线骨密度仪。血糖、血脂、血钾等生化指标采用全自动生化分析仪检测。行75 g口服葡萄糖耐量试验评估糖代谢情况,计算稳态模型胰岛素抵抗指数(homeostasis model assessment-insulin resistance,HOMA-IR),公式为:HOMA-IR=空腹血糖(fasting plasma glucose,FPG,mmol/L)×空腹胰岛素(fasting insulin,FINS,μU/ml)/22.5]。ACTH、血尿皮质醇采用化学发光法测定,ACTH测定在Immulite 2000分析仪上完成,血、尿皮质醇测定在ADVIA centaur CP分析仪上完成。
分析118例SCS患者的影像学资料及肾上腺手术情况;比较不同性别SCS患者的临床生化特点、代谢异常情况、激素水平差异,并采用logistic回归分析性别与皮质醇激素水平之间的关系。考虑女性患者皮质醇水平可能因绝经而受到影响,进一步以是否绝经对女性患者进行分层,比较未绝经与已绝经女性SCS患者的临床指标,并采用logistic回归分析女性SCS患者是否绝经与皮质醇激素水平之间的关系。
采用Empower States统计软件(www.empowerstats.com)及R 4.0软件进行统计分析。计量资料呈正态分布者以$\bar{x}±s$表示,两组间比较采用t检验;呈偏态分布者以M(Q1Q3)表示,两组间比较采用秩和检验;计数资料以例(%)表示,两组间比较采用χ2检验。纳入年龄、发病年龄、BMI及病程作为校正因素行logistic回归分析,以评估SCS患者的性别与F00:00、1 mg DST-F08:00、小剂量DST-F08:00水平之间的关系;同时纳入年龄、发病年龄、BMI及病程作为校正因素行logistic回归分析评估女性SCS患者是否绝经与F00:00、1 mg DST-F08:00、小剂量DST-F08:00水平之间的关系。P<0.05为差异有统计学意义。
118例SCS患者中男性与女性比例为1:2.1。男性患者BMI、三酰甘油水平、糖代谢异常比例均明显高于女性(P<0.05);女性患者F00:00、1 mg DST-F08:00及小剂量DST-F08:00水平均明显高于男性(P<0.05);而两组年龄、发病年龄、病程、血压、总胆固醇、低密度脂蛋白、血钾、HOMA-IR水平及合并高血压、骨密度异常比例等差异无统计学意义(表1)。
118例SCS患者中,43例(36.4%)为左侧肾上腺病变,47例(39.8%)为右侧肾上腺病变,28例(23.7%)为双侧肾上腺病变。左侧、右侧及双侧肾上腺病变的男性患者占比(分别为29.0%,34.2%,36.8%)与女性(分别为40.0%,42.5%,17.5%)比较,差异无统计学意义(P=0.068)。男性患者肾上腺肿瘤最大直径[(23.45±10.42) mm]与女性[(24.38±10.04) mm]比较差异无统计学意义(P=0.658)。同时,118例中有20例于本院行手术治疗并复诊评估,术后病理均回报为肾上腺皮质腺瘤,未常规行进一步免疫组化检查;复诊评估的20例中有11例ACTH-F正常,F00:00水平<50 nmol/L,余9例中8例1 mg DST后晨8:00点皮质醇抑制到50 nmol/L以下,1例因F00:00水平仅轻度升高(51.7 nmol/L)未进一步行1 mg DST,综上,基本支持术前SCS诊断。
分别纳入年龄、发病年龄、BMI、病程作为校正因素并建立多个模型,行logistic回归分析显示,经不同模型校正后,女性患者F00:00、1 mg DST-F08:00、小剂量DST-F08:00仍明显高于男性患者(表2)。
以是否绝经对女性患者进行分层,结果发现,绝经后女性发病年龄高于绝经前女性,病程更长,收缩压、空腹血糖及空腹胰岛素水平更高,而F00:00、1 mg DST-F08:00及小剂量DST-F08:00水平差异无统计学意义(表3)。此外,分别以年龄、发病年龄、BMI、病程为校正因素并建立不同模型后行logistic回归分析,结果显示,未经校正及经不同模型校正后,未绝经与已绝经女性患者F00:00、1 mg DST-F08:00、小剂量DST-F08:00差异均无统计学意义(表4)。
既往研究发现,多种内分泌疾病存在明显的性别差异[7]。文献报道,CS患者中女性比例明显高于男性,男女比例约为1:2.5,且女性患者比例随时间推移逐年增长,其增长速率高于男性[8-9]。有研究指出,在SCS患者中也存在女性患者较男性多的现象[10]。张毅等[11]回顾性分析了1994—2013年解放军总医院内分泌科1173例不同性别AI患者的病因构成,发现SCS患者女性比例高于男性,男女比例接近1:2。本研究中男女患者比例为1:2.1,与上述研究结果一致。
CS是由多种病因所致的以慢性高皮质醇血症为特征的一种临床综合征,表现为满月脸、水牛背、皮肤紫纹、淤斑、向心性肥胖等,而SCS患者通常皮质醇分泌增多不明显,无典型的CS症状和体征。以往曾有学者认为,非ACTH依赖的CS及SCS系同一疾病的不同阶段[12],然而越来越多的研究发现,这两种疾病可能为涉及不同基因改变的不同疾病[13-14]。本研究通过对比不同性别SCS患者的皮质醇自主分泌情况,发现女性SCS患者F00:00、1 mg DST-F08:00及小剂量DST-F08:00水平均明显高于男性,提示女性SCS患者皮质醇自主分泌增多。然而,女性与男性SCS患者24 h UFC水平无明显差异,考虑可能与该指标在临床评价下丘脑-垂体-肾上腺轴功能方面的诊断价值有限有关。24 h UFC理论上记录了患者全天尿中排出的总皮质醇量,可在一定程度上反映血中皮质醇的整体分泌水平,但其本身也会受到患者饮水量、尿量、肾功能等因素的影响,且轻度CS患者24 h UFC水平可在正常范围内,故而其对SCS的筛查敏感性欠佳、价值有限,临床上往往与其他几项试验联合应用。此外,样本量有限所造成的偏倚也是24 h UFC未显示出性别差异的可能原因。
皮质醇分泌增多可引起机体脂肪重新分布、胰岛素抵抗、水钠潴留及成骨细胞活性降低等一系列改变。有研究指出,SCS患者较健康人群更容易出现肥胖、高血脂、糖尿病、高血压、骨质疏松、心血管事件,且病死率也增高[15],因此,指南[5]建议对1 mg DST-F08:00>50 nmol/L的患者进行代谢异常的排查。然而,本研究虽然提示女性SCS患者具有更高的皮质醇自主分泌水平,但男性患者的BMI、三酰甘油水平及糖代谢异常比例反而较女性患者高,推测其原因为虽然女性皮质醇水平高于男性,但在现有病程长度下不足以造成代谢指标的明显差异,因而无法颠覆吸烟等具有性别差异的生活习惯对代谢的影响。
女性SCS患者具有更高的皮质醇自主分泌水平的原因,不除外与女性雌激素及孕激素水平更高有关。类固醇受体是核受体超家族中的一个亚家族,类固醇激素在生理状态下通过与细胞内类固醇受体结合而发挥作用。类固醇受体家族中包含糖皮质激素受体、孕激素受体、雌激素受体、雄激素受体和盐皮质激素受体,上述受体在结构及作用机制方面有较高相似度[16]。类固醇受体的配体结合域与孕激素受体有55%的同源性,与雌激素受体有30%的同源性[17]。孕激素、雌激素可具有一定程度的糖皮质激素样活性,且较皮质醇对某些糖皮质激素受体亚型有更高的亲和力[18],故推测女性雌激素、孕激素可结合并占据皮质醇受体而导致游离皮质醇水平升高。同时,雌激素可使肝脏合成皮质类固醇结合球蛋白增多,最终使血清总皮质醇水平升高。此外,有研究发现雌孕激素受体与肿瘤细胞的生长有关[19]。本研究结果显示,绝经后与绝经前女性SCS患者F08:00、1 mg DST-F08:00及小剂量 DST-F08:00水平差异虽无统计学意义,但绝经后女性较绝经前女性的上述各项指标均有下降的趋势,未显示出统计学差异不排除与研究人群样本量相对有限有关,如扩大样本量,有望予以进一步佐证。
综上所述,本研究发现,女性SCS患者F00:00、1 mg DST-F08:00及小剂量DST-F08:00水平明显高于男性,且该差异不受是否绝经的影响。但由于本研究为单中心、回顾性研究,样本量有限,研究结果可能存在偏倚,未来仍需多中心、大规模和长期前瞻性研究提供进一步的临床证据。
