Objective To analyze the expression of chemerin and Sprouty1 proteins in common psoriasis lesion tissues and their correlation with the severity of the disease. Methods Sixty-eight patients with common psoriasis who received treatment at the dermatology outpatient clinic of the Wuxi Fifth People's Hospital and the Affiliated Hospital of Jiangnan University from February 2018 to November 2020 were selected as the experimental group, and another 68 healthy individuals who underwent physical examination at the dermatology outpatient clinics of the two hospitals during the same period were selected as the control group. The skin lesions of the trunk or limbs of patients with psoriasis vulgaris and normal skin tissues of healthy subjects were collected. Immunohistochemistry and Western blotting were used to detect the expression of chemerin and Sprouty1 protein in skin tissues, and RT-PCR was used to detect the relative expression of chemerin and Sprouty1 mRNA in skin tissues, and Pearson correlation was used to analyze the correlation between the expression of chemerin and Sprouty1 protein with the psoriasis area and severity index (PASI). Results The results of immunohistochemical assay showed that chemerin and Sprouty1 proteins were expressed in the skin tissues of both groups. Chemerin protein was strongly expressed in the adipose tissue of the skin lesions in experimental group, and in the subcutaneous adipose tissue of the control group with a small amount of expression; Sprouty1 protein was expressed in the cell membrane of the granular layer cells of the skin lesions in experimental group with a small amount of expression, and in control group, Sprouty1 protein was strongly expressed in the cell membrane and cytoplasm of the whole epidermal layer cells. The relative expressions of chemerin mRNA (1.478±0.14 vs. 0.858±0.08, P<0.001) and protein (1.138±0.12 vs. 0.789±0.06, P<0.001) were significantly higher in experimental group than in control group, and the relative expressions of Sprouty1 mRNA (0.837±0.04 vs. 1.346±0.09, P<0.001) and protein (0.646±0.05 vs. 1.212±0.12, P<0.001) were significantly lower than that of control group. Pearson correlation analysis showed that the relative expression of chemerin protein was positively correlated with PASI score (r=0.536, P<0.05), and the relative expression of Sprouty1 protein was negatively correlated with PASI score (r=-0.619, P<0.05). Conclusion The relative expression of chemerin and Sprouty1 protein in the lesion tissue of common psoriasis correlated with PASI score.

目的 分析寻常型银屑病皮损组织中chemerin和Sprouty1蛋白表达与病情严重程度的相关性。方法 选取2018年2月—2020年11月在无锡市第五人民医院和江南大学附属医院皮肤性病科门诊接受治疗的68例寻常型银屑病患者作为实验组,以同期在两家医院皮肤性病科门诊体检的68名健康者作为对照组。采集寻常型银屑病患者躯干或四肢的皮损组织及健康者的正常皮肤组织,采用免疫组化及Western blotting检测皮肤组织中chemerin、Sprouty1蛋白相对表达量,RT-PCR检测皮肤组织中chemerin、Sprouty1 mRNA相对表达量,采用Pearson相关分析法分析chemerin、Sprouty1蛋白相对表达量与银屑病皮损面积和严重程度指数(PASI)的相关性。结果 免疫组化检测结果显示,两组皮肤组织中均有chemerin、Sprouty1蛋白的表达。Chemerin蛋白在实验组皮损脂肪组织中呈高表达,在对照组皮下脂肪组织中少量表达;Sprouty1蛋白在实验组皮损颗粒层细胞膜中少量表达,在对照组表皮全层细胞的细胞膜和细胞质中呈高表达。实验组chemerin mRNA(1.478±0.14 vs. 0.858±0.08,P<0.001)和蛋白(1.138±0.12 vs. 0.789±0.06,P<0.001)相对表达量明显高于对照组,Sprouty1 mRNA(0.837±0.04 vs. 1.346±0.09,P<0.001)和蛋白(0.646±0.05 vs. 1.212±0.12,P<0.001)相对表达量明显低于对照组。Pearson相关分析显示,chemerin蛋白相对表达量与PASI评分呈正相关(r=0.536,P<0.05),Sprouty1蛋白相对表达量与PASI评分呈负相关(r=-0.619,P<0.05)。结论 寻常型银屑病皮损组织中chemerin及Sprouty1蛋白相对表达量与PASI评分存在相关性。

目的 分析寻常型银屑病皮损组织中chemerin和Sprouty1蛋白表达与病情严重程度的相关性。方法 选取2018年2月—2020年11月在无锡市第五人民医院和江南大学附属医院皮肤性病科门诊接受治疗的68例寻常型银屑病患者作为实验组,以同期在两家医院皮肤性病科门诊体检的68名健康者作为对照组。采集寻常型银屑病患者躯干或四肢的皮损组织及健康者的正常皮肤组织,采用免疫组化及Western blotting检测皮肤组织中chemerin、Sprouty1蛋白相对表达量,RT-PCR检测皮肤组织中chemerin、Sprouty1 mRNA相对表达量,采用Pearson相关分析法分析chemerin、Sprouty1蛋白相对表达量与银屑病皮损面积和严重程度指数(PASI)的相关性。结果 免疫组化检测结果显示,两组皮肤组织中均有chemerin、Sprouty1蛋白的表达。Chemerin蛋白在实验组皮损脂肪组织中呈高表达,在对照组皮下脂肪组织中少量表达;Sprouty1蛋白在实验组皮损颗粒层细胞膜中少量表达,在对照组表皮全层细胞的细胞膜和细胞质中呈高表达。实验组chemerin mRNA(1.478±0.14 vs. 0.858±0.08,P<0.001)和蛋白(1.138±0.12 vs. 0.789±0.06,P<0.001)相对表达量明显高于对照组,Sprouty1 mRNA(0.837±0.04 vs. 1.346±0.09,P<0.001)和蛋白(0.646±0.05 vs. 1.212±0.12,P<0.001)相对表达量明显低于对照组。Pearson相关分析显示,chemerin蛋白相对表达量与PASI评分呈正相关(r=0.536,P<0.05),Sprouty1蛋白相对表达量与PASI评分呈负相关(r=-0.619,P<0.05)。结论 寻常型银屑病皮损组织中chemerin及Sprouty1蛋白相对表达量与PASI评分存在相关性。

Objective To analyze the expression of chemerin and Sprouty1 proteins in common psoriasis lesion tissues and their correlation with the severity of the disease. Methods Sixty-eight patients with common psoriasis who received treatment at the dermatology outpatient clinic of the Wuxi Fifth People's Hospital and the Affiliated Hospital of Jiangnan University from February 2018 to November 2020 were selected as the experimental group, and another 68 healthy individuals who underwent physical examination at the dermatology outpatient clinics of the two hospitals during the same period were selected as the control group. The skin lesions of the trunk or limbs of patients with psoriasis vulgaris and normal skin tissues of healthy subjects were collected. Immunohistochemistry and Western blotting were used to detect the expression of chemerin and Sprouty1 protein in skin tissues, and RT-PCR was used to detect the relative expression of chemerin and Sprouty1 mRNA in skin tissues, and Pearson correlation was used to analyze the correlation between the expression of chemerin and Sprouty1 protein with the psoriasis area and severity index (PASI). Results The results of immunohistochemical assay showed that chemerin and Sprouty1 proteins were expressed in the skin tissues of both groups. Chemerin protein was strongly expressed in the adipose tissue of the skin lesions in experimental group, and in the subcutaneous adipose tissue of the control group with a small amount of expression; Sprouty1 protein was expressed in the cell membrane of the granular layer cells of the skin lesions in experimental group with a small amount of expression, and in control group, Sprouty1 protein was strongly expressed in the cell membrane and cytoplasm of the whole epidermal layer cells. The relative expressions of chemerin mRNA (1.478±0.14 vs. 0.858±0.08, P<0.001) and protein (1.138±0.12 vs. 0.789±0.06, P<0.001) were significantly higher in experimental group than in control group, and the relative expressions of Sprouty1 mRNA (0.837±0.04 vs. 1.346±0.09, P<0.001) and protein (0.646±0.05 vs. 1.212±0.12, P<0.001) were significantly lower than that of control group. Pearson correlation analysis showed that the relative expression of chemerin protein was positively correlated with PASI score (r=0.536, P<0.05), and the relative expression of Sprouty1 protein was negatively correlated with PASI score (r=-0.619, P<0.05). Conclusion The relative expression of chemerin and Sprouty1 protein in the lesion tissue of common psoriasis correlated with PASI score.

寻常型银屑病是一种易复发的慢性免疫炎症性皮肤病,临床主要表现为红色丘疹、斑块,可发生于全身,最常见于四肢伸侧和骶尾部,具有家族遗传倾向,常伴发代谢综合征[1],其确切病因目前尚未明确,可能与遗传、免疫因素有关[2]。研究发现,肥胖是最易促发或加重银屑病的因素之一,二者在基因水平上存在关联[3]。脂肪因子chemerin是肥胖相关炎症的生物标志物,属于多功能性蛋白质,在免疫细胞趋化、脂肪细胞代谢及血糖稳态过程中起着重要作用[4]。有研究发现,chemerin在银屑病患者血清中异常表达[5]。寻常型银屑病患者表皮角质形成细胞的增殖分化异常,而Sprouty1与肌肉干细胞的增殖、分化有关,可能是角质形成细胞的负反馈调节因子[6]。目前关于银屑病患者血清中chemerin的研究较多,但其皮损组织中chemerin、Sprouty1的相关研究较少。本研究旨在分析寻常型银屑病患者皮损组织中chemerin、Sprouty1的表达与银屑病皮损面积和严重程度指数(psoriasis area and severity index,PASI)的相关性,以期为寻常型银屑病的发病机制和治疗研究提供理论依据。
选取2018年2月—2020年11月于无锡市第五人民医院和江南大学附属医院皮肤性病科门诊接受治疗的68例寻常型银屑病患者作为实验组。纳入标准:(1)符合《中国银屑病诊疗指南(2018完整版)》[7]的诊断标准;(2)年龄22~61岁;(3)自愿参与本研究;(4)近1个月未使用免疫抑制剂和糖皮质激素。排除标准:(1)心、脑、肝、肾等器官功能严重障碍者;(2)合并其他皮肤疾病者;(3)合并类风湿关节炎、干燥综合征、系统性硬化、肌炎、强直性脊柱炎、银屑病关节炎、痛风等其他免疫性疾病者。以同期在两院体检的68名健康者作为对照组。本研究符合《赫尔辛基宣言》相关准则,通过无锡市第五人民医院和江南大学附属医院伦理委员会审批(JNDXFSYYLL 2018-006),研究对象均签署知情同意书。
免疫组化试剂盒购自上海莼试生物技术有限公司;二氨基联苯胺(diaminobenzidine,DAB)显色试剂盒购自上海基因科技有限公司;Trizol试剂购自武汉普诺赛生命科技有限公司;chemerin抗体、Sprouty1抗体、内参3-磷酸甘油醛脱氢酶(glyceraldehyde-3-phosphate-dehydrogenase,GAPDH)试剂盒购自英国Abcam公司;反转录聚合酶链反应(reverse transcription-polymerase chain reaction,RT-PCR)试剂盒、Western blotting检测试剂盒购自上海吉玛制药技术有限公司。XHC-SV1型显微镜购自北京东方华测科学技术中心;LWD300-38LFT型无穷远倒置荧光显微镜购自西安测维光电技术有限公司;紫外分光光度计购自北京友仪四方科技发展有限公司。
采集寻常型银屑病患者躯干或四肢的皮损组织及健康者的正常皮肤组织各1.0 cm×0.5 cm×1.5 cm,分成3份,1份采用10%多聚甲醛溶液固定,脱水、石蜡包埋、切片(厚4 μm),行免疫组化检测,余2份分别于-80 ℃液氮中保存,行Western blotting和RT-PCR检测。
将切片脱蜡水化,室温下滴加3% H2O2溶液,避光孵育10 min,磷酸盐缓冲液(PBS)冲洗5 min×3次,分别滴加chemerin抗体(1:200)、Sprouty1抗体(1:100),4 ℃孵育16 h;加入生物素标记的二抗(1:100),37 ℃孵育30 min,PBS冲洗3 min×3次;滴加辣根标记液,37 ℃孵育30 min,PBS冲洗3 min×3次;DAB显色40 s,苏木精复染2 min,95%乙醇置换去水,中性树胶封片,镜检,拍照观察chemerin、Sprouty1的表达情况。
提取皮损组织和正常皮肤组织蛋白,经十二烷基硫酸钠-聚丙烯酰胺凝胶电泳(sodium dodecyl sulfate-polyacrylamide gel electrophoresis,SDS-PAGE)分离,将蛋白转移至PVDF膜上,5%脱脂奶粉封闭1 h,用10 mmol/L Tris-HCl缓冲液(pH为8.3)洗涤,分别加入兔抗chemerin(1:100)、Sprouty1(1:100)、GAPDH(1:100)一抗过夜,Tris-HCL缓冲液洗涤;加入辣根过氧化物酶(horseradish peroxidase,HRP)二抗(1:200),2 h后洗膜,增强型化学发光试剂(ECL)底物显影后,利用分子成像系统分析条带灰度值,以目的条带灰度值与内参GAPDH灰度值的比值表示蛋白相对表达量。
将冻存组织研磨后用Trizol试剂提取总R NA,紫外分光光度计测定其浓度、纯度及完整性。参照RT-PCR试剂盒说明书步骤扩增cDNA,扩增条件:95 ℃预变性30 s;95 ℃变性5 s,60 ℃退火30 s,60 ℃延伸1 min,共40个循环;循环结束后60 ℃延伸5 min,4 ℃保存。每个样品的Ct值以β-actin作为内参进行校正,采用2-ΔΔCt法计算chemerin、Sprouty1 mRNA相对表达量。引物序列来自Sanger数据库,由上海赛百胜基因技术有限公司合成。Chemerin引物:正义5'-CCCGGCTTCAGGATTTAC-3',反义3'-GCTGGACCATGACTGAGTTG-5';Sprouty1引物:正义5'-ATGGATCCCCAAGGTCAACA-3',反义3'-CGAGGAGCAGGTCTTCCCAC-5';β-actin引物:正义5'-AAGTATGACAACAGCCTCAAG-3',反义3'-TCCACGATACCAAAGTTGTC-5'。
采用PASI[8]对患者病情进行评估。由同一专家分别对患者的红斑(E)、鳞屑(D)、浸润(I)程度和皮疹面积进行记录,并计算PASI值。PASI评分(h)=0.1×(Eh+Dh+Ih)评分×A;PASI评分(u)=0.2×(Eu+Du+Iu)评分×A;PASI评分(t)=0.3×(Et+Dt+It)评分×A;PASI评分(i)=0.4×(Ei+Di+Ii)评分×A;PASI总分=PASI评分(h)+PASI评分(u)+PASI评分(t)+PASI评分(i)。其中h、u、t、i分别代表头部、上肢、躯干、下肢,A表示皮损面积。
采用SPSS 22.0软件进行统计分析。计量资料以$\bar{x}±s$表示,组间比较采用配对t检验;计数资料以例(%)表示,组间比较采用χ2检验;采用Pearson相关分析法进行相关性分析。P<0.05为差异有统计学意义。
实验组男39例,女29例,年龄25~61岁;对照组男35例,女33例,年龄22~57岁。两组性别、年龄、体重指数等一般资料比较差异无统计学意义(P>0.05,表1)。
68例寻常型银屑病患者中,37例(54.4%,37/68)皮损发生在上肢,其中19例(51.4%,19/37)累及头皮;余31例(45.6%,31/68)皮损发生在下肢,其中24例(77.4%,24/31)累及背部,皮损大小不一,以红色斑块、结节为主,上层覆盖厚层银白色鳞屑。
免疫组化检测结果显示,两组皮肤组织中均有chemerin、Sprouty1蛋白表达。Chemerin蛋白在实验组皮损脂肪组织中呈高表达,在对照组皮下脂肪组织中少量表达;Sprouty1蛋白在实验组皮损颗粒层细胞的细胞膜中少量表达,在对照组表皮全层细胞的细胞膜和细胞质中呈高表达(图1)。
RT-PCR和Western blotting检测结果显示,实验组皮肤组织中chemerin mRNA (1.478±0.14 vs. 0.858±0.08,P<0.001)和蛋白(1.138±0.12 vs.0.789±0.06,P<0.001)相对表达量明显高于对照组,Sprouty1 mRNA(0.837±0.04 vs. 1.346±0.09,P<0.001)和蛋白(0.646±0.05 vs. 1.212±0.12,P<0.001)相对表达量明显低于对照组(图2)。
Pearson相关分析结果显示,68例寻常型银屑病患者皮损组织中chemerin蛋白相对表达量与PASI评分呈正相关(r=0.536,P<0.05),Sprouty1蛋白相对表达量与PASI评分呈负相关(r=-0.619,P<0.05,图3)。
寻常型银屑病是银屑病最常见的类型之一,易伴发多种代谢性疾病,可能与患者的不良生活方式及疾病本身的多种因素有关。寻常型银屑病与代谢性疾病存在共同的危险因素和发病机制,其中肥胖是代谢综合征发生的源头,多项研究显示,寻常型银屑病患者的肥胖率高于普通人群,且PASI评分与体重指数呈正相关,而肥胖常伴随着脂肪的堆积,可增加炎性脂肪细胞因子的分泌,引起胰岛素抵抗及相关并发症[9]
Chemerin是脂肪因子家族的新成员,在人脂肪组织中呈高表达,具有趋化白细胞的功能,可募集表达chemerin受体的炎性细胞,参与代谢综合征及相关疾病的病理生理过程[10]。研究发现,肥胖人群血清chemerin水平与白细胞介素-6(IL-6)等炎性因子关系密切,且可影响与银屑病发病相关的树突细胞、单核细胞、巨噬细胞等固有免疫效应细胞,被认为是银屑病早期病变的标志物[11]。杨雪松等[12]分析了寻常型银屑病患者血清chemerin水平与PASI评分的相关性,发现PASI评分与血清chemerin水平存在正相关关系。本研究结果显示,银屑病患者皮损组织与健康对照皮肤组织中均有chemerin的表达,但银屑病患者皮损组织中chemerin蛋白和mRNA相对表达量均高于对照组,且chemerin蛋白相对表达量与PASI评分呈正相关,与余泽莹等[13]的研究结果相似,提示chemerin蛋白可能参与了寻常型银屑病发病机制中的免疫应答及表皮异常增殖。
研究显示,银屑病是由角质形成细胞与浸润免疫细胞之间的相互作用引起的,但影响角质形成细胞增殖的因素及其与银屑病发病机制的关系仍不明确[14]。Sprouty1蛋白是MAPK信号通路的负性调节蛋白,可负性调节生长因子诱导的细胞增殖、分化、运动和存活[15]。Weng等[16]发现,银屑病患者皮损处MAPK活性明显升高,被激活的MAPK发生磷酸化,可在表皮中促进细胞生长和增殖,并调节免疫应答,从而参与银屑病的发生和发展。本研究结果显示,银屑病患者皮损组织与健康对照皮肤组织中均有Sprouty1的表达,但银屑病患者皮损组织中Sprouty1蛋白和mRNA相对表达量低于对照组,且Sprouty1蛋白相对表达量与PASI评分呈负相关,与李锦意等[17]的研究结果基本一致,提示Sprouty1蛋白在角质形成细胞与银屑病发病机制中发挥作用,但仍需进一步研究证实。
综上所述,寻常型银屑病皮损组织中chemerin、Sprouty1蛋白异常表达,其相对表达量与PASI评分存在相关性,该结果为开展寻常型银屑病的通路和靶向治疗研究提供了理论基础,但后续仍需进一步探究chemerin及Sprouty1蛋白在银屑病皮损组织中发挥调控作用的具体机制,以期为银屑病的治疗提供新的方向。
