Objective To analyze the disease burden of benign prostatic hyperplasia (BPH) in China from 1990 to 2019. Methods Data were obtained from the Global Burden of Disease Study 2019. The prevalence, incidence, years lived with disability(YLD) and their corresponding age-standardized rate were used to compare the burden of BPH between China and the world from 1990 to 2019. In addition, a comparative analysis of YLD among 6 urinary diseases was also performed. Results In 2019,the prevalent cases and incidence cases of BPH in China was approximately 20.34 million and 2.84 million, respectively, and the standardized prevalence rate and standardized incidence rate were 2220.85/100 000 and 280.64/100 000, respectively. In addition,the number of YLD due to BPH was approximately 0.41 million and the standardized YLD rate was 44.04/100 000. Compared with 1990, the prevalent cases, incident cases and YLD increased by 125.06%, 122.38% and 123.93% respectively, while the standardized prevalent rate, standardized incident rate and standardized YLD rate decreased by 14.69%, 10.09% and 14.83%, respectively. In 1990 and 2019, the prevalence rate and YLD rate of BPH in China were the highest in the age group ≥80 years old and increased with age.The incidence rate increased in the age group under 79 years old, but decreased in the age group ≥80 years old. In 2019, the YLD of BPH accounted for 61.15% of the 6 urinary diseases, which was 4.11 times that of prostate cancer. Conclusion The disease burden of BPH in China remains at a high level in 2019. Health concerns and guidance on prevention and control should be strengthened for middle-aged and elderly men.

目的 分析1990—2019年中国良性前列腺增生(BPH)的疾病负担。方法 利用2019年全球疾病负担(GBD 2019)数据库，采用患病率、发病率、伤残损失寿命年(YLD)及其标化率对1990—2019年中国与全球BPH疾病负担进行对比分析，并通过中国YLD数据对6种泌尿系统疾病进行对比分析。结果 2019年中国BPH患病例数为2034.30万，标化患病率为2220.85/10万；发病例数为283.65万，标化发病率为280.64/10万；YLD为40.62万人年，标化YLD率为44.04/10万。与1990年相比，中国患病例数、发病例数及YLD分别增高了125.06%、122.38%和123.93%，标化患病率、标化发病率及标化YLD率分别降低了14.69%、10.09%和14.83%。1990年与2019年我国BPH患病率和YLD率在≥80岁年龄组最高且随着年龄的增长呈上升趋势；发病率在79岁以下年龄组呈上升趋势，在≥80岁年龄组反而降低。2019年BPH的YLD占6种泌尿系统疾病总和的61.15%，是前列腺癌的4.11倍。结论 2019年我国BPH的疾病负担仍维持在较高水平，应注重对中老年男性健康的关注及防控指导。

目的 分析1990—2019年中国良性前列腺增生(BPH)的疾病负担。方法 利用2019年全球疾病负担(GBD 2019)数据库，采用患病率、发病率、伤残损失寿命年(YLD)及其标化率对1990—2019年中国与全球BPH疾病负担进行对比分析，并通过中国YLD数据对6种泌尿系统疾病进行对比分析。结果 2019年中国BPH患病例数为2034.30万，标化患病率为2220.85/10万；发病例数为283.65万，标化发病率为280.64/10万；YLD为40.62万人年，标化YLD率为44.04/10万。与1990年相比，中国患病例数、发病例数及YLD分别增高了125.06%、122.38%和123.93%，标化患病率、标化发病率及标化YLD率分别降低了14.69%、10.09%和14.83%。1990年与2019年我国BPH患病率和YLD率在≥80岁年龄组最高且随着年龄的增长呈上升趋势；发病率在79岁以下年龄组呈上升趋势，在≥80岁年龄组反而降低。2019年BPH的YLD占6种泌尿系统疾病总和的61.15%，是前列腺癌的4.11倍。结论 2019年我国BPH的疾病负担仍维持在较高水平，应注重对中老年男性健康的关注及防控指导。

Objective To analyze the disease burden of benign prostatic hyperplasia (BPH) in China from 1990 to 2019. Methods Data were obtained from the Global Burden of Disease Study 2019. The prevalence, incidence, years lived with disability(YLD) and their corresponding age-standardized rate were used to compare the burden of BPH between China and the world from 1990 to 2019. In addition, a comparative analysis of YLD among 6 urinary diseases was also performed. Results In 2019,the prevalent cases and incidence cases of BPH in China was approximately 20.34 million and 2.84 million, respectively, and the standardized prevalence rate and standardized incidence rate were 2220.85/100 000 and 280.64/100 000, respectively. In addition,the number of YLD due to BPH was approximately 0.41 million and the standardized YLD rate was 44.04/100 000. Compared with 1990, the prevalent cases, incident cases and YLD increased by 125.06%, 122.38% and 123.93% respectively, while the standardized prevalent rate, standardized incident rate and standardized YLD rate decreased by 14.69%, 10.09% and 14.83%, respectively. In 1990 and 2019, the prevalence rate and YLD rate of BPH in China were the highest in the age group ≥80 years old and increased with age.The incidence rate increased in the age group under 79 years old, but decreased in the age group ≥80 years old. In 2019, the YLD of BPH accounted for 61.15% of the 6 urinary diseases, which was 4.11 times that of prostate cancer. Conclusion The disease burden of BPH in China remains at a high level in 2019. Health concerns and guidance on prevention and control should be strengthened for middle-aged and elderly men.

良性前列腺增生(benign prostatic hyperplasia，BPH)是老年男性最常见的疾病之一[1-2]，随着年龄增长其患病率逐渐增高[3]。BPH是导致下尿路症状(lower urinary tract symptoms，LUTS)的首要因素，全球约1/4的男性存在LUTS困扰[4]。BPH虽为非致死性疾病，但其导致的全球疾病负担日益加重[5]。据统计，美国30岁以上男性中有1500万人受BPH/LUTS的影响[6]，45岁以上男性中有45%会发展成BPH，70岁以上男性中则高达80%[7-8]。目前我国60岁及以上老龄人口约为2.54亿，占总人口的17.9%，预计到2050年将突破4亿，随着人口老龄化程度进一步加深，老年男性健康将成为我国重要的公共卫生问题[9]。对BPH进行流行病学研究有利于科学指导男性健康防控。本研究基于2019年全球疾病负担(Global Burden of Disease Study 2019，GBD 2019)数据库，对1990—2019年我国BPH疾病负担进行分析，以期为BPH相关防控提供决策依据。
1 资料与方法

1.1 资料来源
本研究数据来源于GBD 2019数据库中1990—2019年中国及全球的BPH数据。GBD 2019采用统一、可比的方法，对各种疾病的疾病负担进行了多指标综合估计[10-12]，量化评估了全球204个国家和地区的主要疾病、伤害和健康危险因素的发病率及疾病负担。采用国际疾病分类(ICD-9和ICD-10)对所研究疾病进行分类，并对垃圾代码进行合理的重新分配，使估计结果更加准确，具体方法已在文献中报告[13-15]。所有数据通过GBD官方网站在线查询工具获得(http://ghdx.healthdata.org/gbd-results-tool)。为进一步分析我国BPH的疾病负担，在GBD数据库中另选择5种泌尿系统疾病(前列腺癌、膀胱癌、尿石症、尿道感染和间质性肾炎、男性不育症)进行对比分析。
1.2 BPH定义
BPH定义为前列腺组织的良性增生，前列腺体积增加或平滑肌张力增大导致的膀胱出口梗阻(bladder outlet obstruction，BOO)是导致LUTS的主要原因[3]。根据ICD-10，BPH的疾病编码为N40、N40.0、N40.1、N40.2、N40.3和N40.9。
1.3 观察指标
采用患病率、发病率、伤残损失寿命年(years lived with disability，YLD)及其标化率对1990—2019年中国与全球BPH疾病负担进行对比分析；按照GBD 2019中BPH的年龄分组对中国BPH进行分析，分别为：15~49岁、50~54岁、55~59岁、60~64岁、65~69岁、70~74岁、75~79岁和≥80岁；通过中国YLD数据对6种泌尿系统疾病进行对比分析。
1.4 统计学处理
GBD 2019采用DisMod-MR 2.1对非致死性疾病的疾病负担进行估计，具体方法及估算过程参见文献[13]。YLD通过患病率和后遗症有关的残疾权重的乘积进行估算，具体方法参见文献[13]。标化率(age-standardized rate，ASR)的计算公式为：ASR=(∑Ai=1aiwi)/(∑Ai=1wi)×100 000，其中ai代表ith年龄组的年龄标化率，w代表所选参考标准人群中同一ith年龄组的人数(或权重)，A代表年龄组的数量。

2.1 1990—2019年中国和全球BPH的疾病负担
1990年我国BPH发病例数为127.55万，标化发病率为312.14/10万；2019年我国BPH发病例数为283.65万，标化发病率为280.64/10万。与1990年相比，我国患病例数、发病例数及YLD分别增高了125.06%、122.38%和123.93%，标化患病率、标化发病率及标化YLD率分别降低了14.69%、10.09%和14.83%。
2019年中国和全球BPH患病例数、发病例数及YLD较1990年均有增长，而标化患病率、标化发病率及标化YLD率略有下降。2019年我国BPH患病例数(2034.30万)、发病例数(283.64万)及YLD(40.62万/年)低于全球水平，而标化患病率(2220.85/10万)、标化发病率(280.64/10万)及标化YLD率(44.04/10万)接近全球水平(表1)。
2.2 1990—2019年中国不同年龄组BPH的发病率、患病率及YLD率
如表2所示，75~79岁年龄组的发病率最高，1990年约为2810.28/10万，2019年约为2638.89/10万。与1990年相比，2019年各年龄段的发病率均降低。
患病率最高的年龄段为≥80岁，与YLD率所呈现的趋势相似。YLD率在≥80岁年龄组最高，1990年和2019年的YLD率分别为625.76/10万和569.64/10万。YLD率随着年龄的增长呈上升趋势(表2)。
2.3 1990—2019年中国6种泌尿系统疾病的YLD
1990—2019年中国6种泌尿系统疾病的YLD均持续增长(表3)。1990年YLD位居前3的疾病分别为BPH、男性不育症和尿石症，2019年分别为BPH、前列腺癌和男性不育症。1990年，BPH的YLD为17.47万人年，约为男性不育症的2.77倍，占泌尿系统疾病的60.18%；2019年，BPH的YLD为40.62万人年，是前列腺癌的4.11倍，占泌尿系统疾病的61.15%(图1)。

本研究分析了1990—2019年我国与全球BPH的患病例数、发病例数、YLD及其标化率，比较分析了中国不同年龄组BPH患者及各泌尿系统疾病的疾病负担，结果显示，1990—2019年中国和全球BPH的患病例数、发病例数、YLD均呈上升趋势，而标化患病率、标化发病率及标化YLD率均下降；2019年中国BPH患病例数、发病例数及YLD均低于全球水平；相较1990年，2019年中国BPH发病率、YLD率与患病率均下降。在各年龄段中，BPH发病率最高的为75~79岁，而患病率与YLD率则在80岁及以上人群中最高。1990—2019年中国6种泌尿系统疾病的YLD均呈上升趋势，而BPH的疾病负担一直居于首位。
随着中国人口老龄化的加速，BPH已成为社会重点关注的健康问题[4,16]。我国BPH的发病及患病情况显示，人口基数大和老龄化可能是导致发病例数及患病例数不断攀升的原因之一，而随着医疗水平、教育水平和健康意识的不断提高，BPH的发病率不断下降。此外，地域及种族因素也可能是中国BPH疾病负担低于全球水平的原因之一。已有研究发现，日本、中国和印度男性的前列腺体积明显小于澳大利亚或美国男性[17]，亚洲男性BPH的发病率低于白人男性[18]。BPH的伤残负担多由相关并发症引起，如急性尿潴留、反复尿路感染及肾功能损害等[19]，其并发症不仅严重影响了患者的生活质量，也带来了沉重的经济负担[20]。短期来看，BPH的手术治疗费用高于药物治疗；长远来看，BPH作为慢性进展性疾病，其导致的间接费用、隐形费用及药物治疗费用等明显高于手术治疗费用[20-21]。因此，随着人口不断老龄化，BPH的治疗费用会进一步提升，其疾病负担将会进一步加重。
BPH/LUTS症状的控制及其并发症的预防对于老年BPH患者至关重要。在老年群体中，BPH增加了跌倒、抑郁的风险并严重影响了患者的生活质量[19]。此外，老年男性患者存在行动不便及电子设备使用困难的问题，可能导致其出现症状甚至相关并发症时不能及时就医[22]。针对老年患者开发简单易用的移动软件或网上就诊平台可便于其进行自我管理和及时诊治。尽管BPH通常不会危及生命，但疾病进展所引起的症状和并发症对患者的影响却不容忽视[23]。老年患者常合并一种或多种慢性疾病，手术风险及治疗效果的不确定性也会影响治疗方案的制定。为此，建议开展老年男性健康公开课，普及泌尿生殖系统疾病预防知识，以利于BPH的早期防控。
本研究存在以下局限性：(1)数据全部来源于GBD 2019数据库，虽然GBD团队定期对数据和估算模型进行更新，但对于中国的估计可能仍然不够准确[11-12,24]，因此建议未来在我国开展BPH相关流行病学调查进行验证。(2)BPH作为非致命性疾病，GBD数据库可以使用的评估指标有限，可能无法准确评估其疾病负担。仅使用YLD对泌尿系统疾病进行比较，不能全面体现其疾病负担的差异。(3)未包括中国各个省份和地区的BPH相关数据，故未对各地区的疾病负担进行比较，提示开展全国性的流行病学调查以明确我国全国及各地区BPH的疾病负担非常必要。但尽管存在上述局限性，本研究仍为判断我国BPH的疾病负担提供了重要参考。
综上所述，1990—2019年我国BPH的患病例数、发病例数及YLD均大幅增长，但远低于全球水平；而标化患病率、标化发病率及标化YLD率则略微下降，且与全球水平相近。在不同年龄段中，80岁及以上人群BPH的患病率及YLD率最高，发病率最高人群年龄在75~79岁。在各泌尿系统疾病中，BPH的YLD一直居于首位。由此可见，我国BPH的疾病负担仍维持在较高水平，加强BPH防治工作刻不容缓。重点防控中老年男性群体、合理分配医疗资源对减轻我国BPH疾病负担具有重要意义。
