Objective By comparison of the urinary free cortisol (UFC) levels in post- to prior-dexamethasone suppression test (post UFC/pre UFC) with primary pigmented nodular adrenocortical disease (PPNAD), primary bilateral macronodular adrenal hyperplasia (PBMAH) and adrenocortical adenoma (ADA), to explore the optimal differential diagnostic cut-off point of PPNAD from PBMAH and ADA. Methods The clinical data were collected and retrospectively analyzed of in patients admitted from January 2008 to December 2020 with finally diagnosed as PPNAD (n=7), PBMAH (n=31) and ADA (n=130) in the First Medical Center of Chinese PLA General Hospital. Based on the receiver operating characteristic (ROC) curve, the value of post UFC/pre UFC in low-dose dexamethasone suppression test (LDDST) and high-dose dexamethasone suppression test (HDDST)was used as the detection variable, and the diagnosis of PPNAD was used as the status variable to search the best cut-off point for the differential diagnosis of PPNAD. Results The serum cortisol (F) levels of all the 3 groups of patients (PPNAD, PBMAH and ADA) increased significantly in the baseline, LDDST and HDDST, and no statistical difference existed among them (P>0.05). The adrenocorticotropic hormone (ACTH) levels in all the 3 groups of patients were inhibited, and ACTH levels in PBMAH group were the highest at the baseline and after HDDST, and the ACTH levels of ADA group were the lowest among the three groups (P<0.05).The baseline 24 h UFC levels elevated obviously with no statistical difference (P>0.05) among the three groups. For patients in PPNAD group, the LDDST post UFC/pre UFC [1.88(1.39, 2.59)] was obviously higher than that in PBMAH group [1.13(0.82,1.37), P=0.001] and ADA group [1.11(0.70, 1.49), P=0.003]; and the HDDST post UFC/pre UFC in PPNAD group [2.31(1.23,3.08)] was still higher than that in PBMAH group [1.22(0.94, 1.63), P=0.004] and ADA group [1.39(1.08, 1.53), P=0.008].Compared with PBMAH group and ADA group, the diagnostic cut-off point for PPNAD group after LDDST post UFC/pre UFC was 1.385, with 85.7% sensitivity and 71.5% specificity; and after HDDST post UFC/pre UFC was 2.290, with 57.1% sensitivity and 95.0% specificity. Conclusion The cut-off points of post UFC/pre UFC in DST had a clear advantage in distinguishing PPNAD from PBMAH and ADA. The diagnosis of PPNAD may be considered once the value of post UFC/pre UFC after LDDST is greater than 1.385, or after HDDST is above 2.290.

目的 通过比较原发性色素性结节样肾上腺皮质病(PPNAD)、原发性双侧肾上腺大结节增生(PBMAH)和肾上腺皮质腺瘤(ADA)三种疾病的地塞米松抑制试验后尿游离皮质醇变化比值,探讨PPNAD患者的鉴别诊断切点。方法 收集2008年1月-2020年12月于解放军总医院第一医学中心住院病理明确诊断为PPNAD(7例)、PBMAH(31例)及ADA(130例)患者的临床资料进行回顾性分析。通过受试者工作特征(ROC)曲线,以患者小剂量地塞米松抑制试验(LDDST)和大剂量地塞米松抑制试验(HDDST)抑制后24 h尿游离皮质醇(24 h UFC)与抑制前(基线)24 h UFC比值(post UFC/pre UFC)为检测变量,以是否为PPNAD为状态变量,寻找鉴别诊断PPNAD的最佳切点。结果 PPNAD、PBMAH及ADA三组患者基线及LDDST、HDDST后血皮质醇(F)水平均明显升高,组间差异无统计学意义(P>0.05);三组患者促肾上腺皮质激素(ACTH)水平均受抑制,且PBMAH组基线及HDDST后ACTH水平最高,ADA组ACTH水平最低(P<0.05);三组患者基线24 h UFC均明显升高,差异无统计学意义(P>0.05)。与PBMAH、ADA组比较,PPNAD组患者LDDST post UFC/pre UFC[1.88(1.39, 2.59)]明显高于PBMAH组[1.13(0.82, 1.37),P=0.001]及ADA组[1.11(0.70, 1.49),P=0.003],且HDDST抑制后的post UFC/pre UFC[2.31(1.23, 3.08)]亦高于PBMAH组[1.22(0.94, 1.63),P=0.004]及ADA组[1.39(1.08, 1.53),P=0.008]。与PBMAH、ADA组比较,PPNAD组患者LDDST中post UFC/pre UFC诊断切点为1.385,敏感度为85.7%,特异度为71.5%;HDDST中post UFC/pre UFC诊断切点为2.290,敏感度为57.1%,特异度为95.0%。结论 地塞米松抑制试验post UFC/pre UFC切点对PPNAD患者鉴别诊断具有重要临床价值;当LDDST中post UFC/pre UFC为1.385以上,HDDST中post UFC/pre UFC为2.290以上可能诊断为PPNAD。

目的 通过比较原发性色素性结节样肾上腺皮质病(PPNAD)、原发性双侧肾上腺大结节增生(PBMAH)和肾上腺皮质腺瘤(ADA)三种疾病的地塞米松抑制试验后尿游离皮质醇变化比值,探讨PPNAD患者的鉴别诊断切点。方法 收集2008年1月-2020年12月于解放军总医院第一医学中心住院病理明确诊断为PPNAD(7例)、PBMAH(31例)及ADA(130例)患者的临床资料进行回顾性分析。通过受试者工作特征(ROC)曲线,以患者小剂量地塞米松抑制试验(LDDST)和大剂量地塞米松抑制试验(HDDST)抑制后24 h尿游离皮质醇(24 h UFC)与抑制前(基线)24 h UFC比值(post UFC/pre UFC)为检测变量,以是否为PPNAD为状态变量,寻找鉴别诊断PPNAD的最佳切点。结果 PPNAD、PBMAH及ADA三组患者基线及LDDST、HDDST后血皮质醇(F)水平均明显升高,组间差异无统计学意义(P>0.05);三组患者促肾上腺皮质激素(ACTH)水平均受抑制,且PBMAH组基线及HDDST后ACTH水平最高,ADA组ACTH水平最低(P<0.05);三组患者基线24 h UFC均明显升高,差异无统计学意义(P>0.05)。与PBMAH、ADA组比较,PPNAD组患者LDDST post UFC/pre UFC[1.88(1.39, 2.59)]明显高于PBMAH组[1.13(0.82, 1.37),P=0.001]及ADA组[1.11(0.70, 1.49),P=0.003],且HDDST抑制后的post UFC/pre UFC[2.31(1.23, 3.08)]亦高于PBMAH组[1.22(0.94, 1.63),P=0.004]及ADA组[1.39(1.08, 1.53),P=0.008]。与PBMAH、ADA组比较,PPNAD组患者LDDST中post UFC/pre UFC诊断切点为1.385,敏感度为85.7%,特异度为71.5%;HDDST中post UFC/pre UFC诊断切点为2.290,敏感度为57.1%,特异度为95.0%。结论 地塞米松抑制试验post UFC/pre UFC切点对PPNAD患者鉴别诊断具有重要临床价值;当LDDST中post UFC/pre UFC为1.385以上,HDDST中post UFC/pre UFC为2.290以上可能诊断为PPNAD。

Objective By comparison of the urinary free cortisol (UFC) levels in post- to prior-dexamethasone suppression test (post UFC/pre UFC) with primary pigmented nodular adrenocortical disease (PPNAD), primary bilateral macronodular adrenal hyperplasia (PBMAH) and adrenocortical adenoma (ADA), to explore the optimal differential diagnostic cut-off point of PPNAD from PBMAH and ADA. Methods The clinical data were collected and retrospectively analyzed of in patients admitted from January 2008 to December 2020 with finally diagnosed as PPNAD (n=7), PBMAH (n=31) and ADA (n=130) in the First Medical Center of Chinese PLA General Hospital. Based on the receiver operating characteristic (ROC) curve, the value of post UFC/pre UFC in low-dose dexamethasone suppression test (LDDST) and high-dose dexamethasone suppression test (HDDST)was used as the detection variable, and the diagnosis of PPNAD was used as the status variable to search the best cut-off point for the differential diagnosis of PPNAD. Results The serum cortisol (F) levels of all the 3 groups of patients (PPNAD, PBMAH and ADA) increased significantly in the baseline, LDDST and HDDST, and no statistical difference existed among them (P>0.05). The adrenocorticotropic hormone (ACTH) levels in all the 3 groups of patients were inhibited, and ACTH levels in PBMAH group were the highest at the baseline and after HDDST, and the ACTH levels of ADA group were the lowest among the three groups (P<0.05).The baseline 24 h UFC levels elevated obviously with no statistical difference (P>0.05) among the three groups. For patients in PPNAD group, the LDDST post UFC/pre UFC [1.88(1.39, 2.59)] was obviously higher than that in PBMAH group [1.13(0.82,1.37), P=0.001] and ADA group [1.11(0.70, 1.49), P=0.003]; and the HDDST post UFC/pre UFC in PPNAD group [2.31(1.23,3.08)] was still higher than that in PBMAH group [1.22(0.94, 1.63), P=0.004] and ADA group [1.39(1.08, 1.53), P=0.008].Compared with PBMAH group and ADA group, the diagnostic cut-off point for PPNAD group after LDDST post UFC/pre UFC was 1.385, with 85.7% sensitivity and 71.5% specificity; and after HDDST post UFC/pre UFC was 2.290, with 57.1% sensitivity and 95.0% specificity. Conclusion The cut-off points of post UFC/pre UFC in DST had a clear advantage in distinguishing PPNAD from PBMAH and ADA. The diagnosis of PPNAD may be considered once the value of post UFC/pre UFC after LDDST is greater than 1.385, or after HDDST is above 2.290.

原发性色素性结节样肾上腺皮质病(primary pigmented nodular adrenocortical disease,PPNAD)是一种罕见的双侧肾上腺疾病,病因分类属于非促肾上腺皮质激素(ACTH)依赖性库欣综合征,占全部库欣综合征比例不足2%[1]。而非ACTH依赖性库欣综合征病因中,以肾上腺皮质腺瘤(adrenocortical adenoma,ADA)最常见(占库欣综合征10%~22%),原发性双侧肾上腺大结节增生(primary bilateral macronodular adrenal hyperplasia,PBMAH)占库欣综合征的1.0%~6.3%[2-3]。三者临床表现、生化改变类似,且影像学均可表现为双侧占位或腺瘤样改变[4-6],有时难以区分,因处理及预后不同,应加以鉴别。
既往研究发现,PPNAD患者行地塞米松抑制试验后存在24 h尿游离皮质醇(24 h UFC)反常性升高现象。Stratakis等[7]将21例PPNAD患者与对照组(9例大结节性肾上腺皮质病和15例单侧肾上腺皮质醇瘤)比较,结果发现在Liddle试验(6 d地塞米松抑制试验)中,PPNAD患者地塞米松抑制后24 h UFC较基线增加超过50%~100%,提示这一特征可区分PPNAD与其他非ACTH依赖性库欣综合征。Chen等[8]同样以PBMAH及ADA患者作为对照,发现小剂量地塞米松抑制试验(LDDST)、大剂量地塞米松抑制试验(HDDST)序贯进行后24 h UFC与抑制前(基线)24 h UFC比值(post UFC/pre UFC)>1.08可用于PPNAD患者与PBMAH、ADA患者的鉴别。然而,此24 h UFC比值切点小于先前提出的数值,因此,目前关于该切点的确定尚存争议。本研究回顾性分析于解放军总医院第一医学中心住院明确诊断为PPNAD患者的临床资料,以探讨PPNAD患者LDDST、HDDST中post UFC/pre UFC的诊断切点。
收集2008年1月-2020年12月于解放军总医院第一医学中心住院并手术病理明确诊断为PPNAD(n=7)、PBMAH(n=31)、ADA(n=130)的患者的血ACTH、皮质醇(F)、24 h UFC及LDDST、HDDST数据资料进行回顾性分析。非ACTH依赖性库欣综合征的诊断标准:(1)血ACTH水平<2 pmol/L(10 pg/ml)或经典HDDST后24 h UFC/F下降<50%(阳性),影像学除外库欣病或异位ACTH综合征;(2)血清F分泌节律紊乱,午夜血清F水平>50 nmol/L;(3)经典LDDST后血清F水平>50 nmol/L(阳性);(4)除外使用外源性糖皮质激素、妊娠、酗酒、病态肥胖、抑郁症或其他精神疾病[9-10]。Carney综合征诊断标准为2001年公布的Carney综合征诊断标准[11]。如果患者满足两个主要标准或一个主要和一个补充标准,就可以诊断为Carney综合征。纳入标准:(1)实验室检查符合非ACTH依赖性库欣综合征;(2)PPNAD、PBMAH、ADA患者均行手术治疗,病理明确诊断。排除标准:(1)合并严重心、肝、肾功能不全者;(2)资料不全者。本研究获解放军总医院伦理委员会批准(S2015-050-01)。
收集所有患者的ACTH-F节律、24 h UFC、LDDST及HDDST结果,以及PPNAD患者临床、基因检测、手术、病理及术后随访资料。LDDST:基线测量24 h UFC 2 d后,于上午6:00开始每6 h口服地塞米松0.5 mg,连续2 d。在给予地塞米松后的第2天开始留尿测定24 h UFC。与LDDST类似,HDDST期间给予地塞米松2 mg,每6 h口服,连续2 d。给予大剂量地塞米松后的第2天开始留尿测定24 h UFC。血ACTH、F及24 h UFC采用化学发光法测定[血ACTH检测仪(德国Immulite 2000 Analyzer),血F检测仪(德国ADVIA Centaur Analyzer),试剂盒购自德国西门子公司]。参考值范围[血ACTH8:00为<10.12 pmol/L,血F0:00为0~165.7 nmol/L,血F8:00为198.7~797.5 nmol/L,血F16:00为85.3~459.6 nmol/L,24 h UFC为98.0~500.1 nmol/24 h]。
比较PPNAD与PBMAH和ADA患者上述指标;然后以LDDST和HDDST post UFC/pre UFC为检测变量,是否诊断为PPNAD为状态变量,构建受试者工作特征(ROC)曲线,寻找鉴别诊断PPNAD的最佳切点。
采用SPSS 24.0统计软件进行分析。正态分布的计量资料以$\bar{x}±s$表示,多组间比较采用单因素方差分析,进一步两两比较采用LSD-t检验;地塞米松试验抑制前、后24 h UFC比较采用配对t检验;非正态分布的计量资料以M(Q1Q3)表示,三组比较采用Kruskal-Wallis H检验,进一步两两比较时校正显著性水平,P<0.017为差异有统计学意义;计数资料以例(%)表示,组间比较采用χ2检验;采用ROC曲线评估LDDST和HDDST post UFC/pre UFC的鉴别诊断价值并找出最佳切点值。均为双侧检验,P<0.05为差异有统计学意义。
三组患者中,PPNAD组就诊年龄[(19.9±5.7)岁]最小,PBMAH组[(52.3±9.6)岁]年龄最大,三组间差异有统计学意义(P<0.05);三组均以女性居多。三组患者均存在0:00、8:00、16:00血ACTH水平低,血F水平明显升高的特征,但三组间基线及LDDST、HDDST后血F水平差异无统计学意义;三组间ACTH0:00及LDDST、HDDST后ACTH水平差异有统计学意义,ADA组ACTH水平最低(P<0.05)。三组基线24 h UFC均明显升高,组间差异无统计学意义。PPNAD组患者LDDST、HDDST后24 h UFC[分别为1745.40(792.20,3504.87) nmol/24 h,2168.80 (928.80,4592.74) nmol/24 h]较基线水平[703.50(306.30,2529.85)]明显升高(P<0.05)。PPNAD组患者LDDST post UFC/pre UFC、HDDST post UFC/pre UFC均明显高于PBMAH及ADA组(P<0.017)(表1)。
以患者LDDST、HDDST post UFC/pre UFC为检测变量构建ROC曲线,结果发现,与PBMAH、ADA组患者比较,PPNAD组患者LDDST post UFC/pre UFC的ROC曲线下面积(AUC)为0.838(95%CI 0.719~0.958),切点为1.385,敏感度为85.7%,特异度为71.5%;HDDST post UFC/pre UFC的AUC为0.767(95%CI 0.567~0.967),切点为2.290,敏感度为57.1%,特异度为95.0%(图1)。
7例PPNAD患者就诊年龄(19.9±5.7)岁,男女比例为1:6,均具有显性库欣综合征的临床表现。全部行肾上腺手术治疗,病理可见肾上腺皮质多发小结节状增生,切面灰褐色,明确诊断为PPNAD。例4、6合并有Carney综合征,例4表现为口唇、左侧颊黏膜黑色素沉着斑,腹部可见咖啡色斑;例6表现为唇部、臀部、外阴色素沉着,合并心房黏液瘤。这2例患者基因检测均证实为PRKAR1A染色体突变(表2)。
PPNAD与PBMAH、ADA均属于非ACTH依赖性库欣综合征,临床表现相似,生化检查均提示血F水平升高,ACTH水平降低,LDDST、HDDST结果阳性,而影像学又均可表现为双侧肾上腺占位或腺瘤样改变[4-6],鉴别诊断具有挑战性,需要建立有效的临床鉴别方法。
地塞米松抑制试验常用于ACTH依赖性库欣综合征的诊断和鉴别诊断。LDDST用于明确患者是否存在库欣综合征,HDDST用于区分库欣病与其他类型库欣综合征[8]。然而,ACTH非依赖性库欣综合征患者血ACTH基线水平相对较低,应用地塞米松抑制试验鉴别诊断的研究有限。既往研究发现,PPNAD患者口服地塞米松后24 h UFC水平出现了反常性增高,但增高比例报道不一,增高倍数为8%~100%[7-8, 12]。Stratakis等[7]发现,Liddle试验第6天的UFC水平对PPNAD诊断的准确性最高,24 h UFC水平增高50%,可将69.2%(9/13)的PPNAD患者与其他原发性肾上腺皮质疾病的库欣综合征区分,第6天24 h UFC排泄增加100%或以上,仅见于PPNAD患者。Chen等[8]对比分析PPNAD(n=25)与PBMAH(n=27)、ADA(n=84)患者发现,LDDST的post UFC/pre UFC>1.16,诊断PPNAD的敏感度为64.0%,特异度为77.9%;HDDST的post UFC/pre UFC>1.08,诊断PPNAD的敏感度为84.0%,特异度为75.6%。地塞米松抑制试验post UFC/pre UFC可将PPNAD与PBMAH、ADA患者进行区分。
本研究回顾分析了我院12年间7例明确诊断为PPNAD患者地塞米松抑制试验结果,与PBMAH、ADA患者比较,PPNAD患者LDDST post UFC/pre UFC的切点为1.385,诊断敏感度为85.7%,特异度为71.5%;HDDST post UFC/pre UFC的切点为2.290,诊断敏感度为57.1%,特异度为95.0%。提示LDDST后24 h UFC反常性升高38.5%,考虑PPNAD可能性大,可进一步行HDDST,若HDDST后24 h UFC升高129%,则可诊断PPNAD,特异度为95.0%,与既往研究中Liddle试验第6天的UFC水平升高100%时诊断PPNAD的特异度为100%相似[7]
PPNAD患者地塞米松抑制试验24 h UFC反常性升高的机制可能为外源性糖皮质激素增加了PPNAD组织中F的产生,这与PPNAD结节中糖皮质激素受体过度表达相关,而与PRKAR1A编码基因突变无直接关联[13]。反常的地塞米松刺激表现由糖皮质激素受体介导,并可被特异度蛋白激酶A(PKA)抑制剂抑制,提示在PPNAD组织中,地塞米松可能通过糖皮质激素受体介导的PKA催化亚基发挥作用,从而反常性刺激F的释放[12]。虽然对糖皮质激素受体上调的分子学及病因学机制尚不明确,但此现象可作为临床上鉴别影像学表现不典型的PPNAD患者的依据,对PPNAD早期诊断具有较大意义。
此外,PPNAD可能是孤立的或是Carney综合征的一个常见组分。Carney综合征于1985年由Carney等[14]首次报道,是一种常染色体显性遗传的多发性肿瘤综合征,以垂体、甲状腺、胰腺和(或)性腺肿瘤,皮肤、黏膜表面的色素沉着病变及黏液瘤(心脏、皮肤和乳腺),以及包括库欣综合征在内的各种内分泌疾病为特征[15-16]。心脏黏液瘤可能导致卒中和死亡,因此,PPNAD诊断后应进行Carney综合征筛查。本研究7例PPNAD患者中有两例明确合并有Carney综合征,例4表现为口唇、左侧颊黏膜雀斑样痣,腹部咖啡色斑;例6表现为唇部、臀部、外阴雀斑样痣,合并心房黏液瘤。多数PPNAD或Carney综合征被认为与PRKAR1A基因突变相关[17-18],本研究中的两例患者均行基因检测证实PRKAR1A染色体突变,例4父亲亦存在PRKAR1A染色体突变。PRKAR1A被认为是一种抑癌基因,其突变可导致PKA活性增强,从而促进肿瘤的发生发展[17],故PPNAD或Carney综合征患者需长期随访筛查伴发肿瘤。双侧肾上腺切除术联合术后长期激素替代治疗仍为PPNAD患者的标准治疗方案[19],对于部分术前症状较轻的患者,行单侧肾上腺切除术或对侧肾上腺次全切除术,症状也有可能得到长期缓解[20]。本研究2例患者行单侧肾上腺切除,随访3~9年,库欣症状均得到缓解。
综上所述,本研究发现,地塞米松抑制试验post UFC/pre UFC切点对PPNAD患者鉴别诊断具有重要的临床价值。但是,本研究存在一定的局限性:(1)因PPNAD为罕见病,发病率低,研究例数较少,可能导致研究结果存在潜在的偏倚;(2)仅与PBMAH及ADA患者进行比较;(3)数据来源于单中心;(4)国内各中心检测试剂盒尚未标化等。因此,目前的诊断切点能否推广用于所有PPNAD患者的鉴别尚需多中心大样本人群研究验证。
