Surgery and resuscitation are the core capabilities of combat casualty care. Early treatment should be carried out within 3 hours, and divided into two phases, emergency treatment and surgical resuscitation, emphasizing the capability building of resuscitation and surgery. This paper interprets the capability requirements of the early casualty care level. According to the domestic and international progress in the care of civilian trauma and combat casualty, the relationship between "surgery" and "resuscitation" in the early treatment level of combat casualty, and the key technological progress are summarized, indicating that only the coordinated development of surgery and resuscitation ability and mutual support can provide safe, standardized and efficient treatment and reduce the death rate of the combat casualties.

  • 李阳,医学博士,副主任医师,硕士研究生导师,陆军军医大学陆军特色医学中心战创伤医学科副主任,中华医学会灾难医学分会青年学组副组长,重庆市医师协会创伤外科医师分会副会长。主要从事骨创伤临床工作,对骨盆、四肢骨折及多发伤救治具有丰富经验。曾任联合国驻黎巴嫩维和部队司令部(UNIFIL)作战医疗官并荣获联合国和平勋章;多次参与组织国内外军事医学救援演习。主持军队重大及重庆市民生科技创新科研项目6项,总科研经费170万元。发表论文50余篇,其中SCI收录期刊论文7篇,获国家实用新型专利6项。

    张连阳,主任医师,教授,博士研究生导师,陆军军医大学陆军特色医学中心战创伤医学科主任,兼任中华医学会灾难医学分会副主任委员,全军灾难医学专业委员会主任委员等。任《中华创伤杂志》总编辑,《创伤外科杂志》主编。创新了腹腔开放、腹直肌推徙、生物补片+腹壁分离、渐进性关腹术等多项新术式,牵头发布《腹部创伤腔镜诊疗规范专家共识》《创伤后腹腔高压症/腹腔间隙综合征诊疗规范》《负压封闭引流技术腹部应用指南》等8项指南;承担国家科技惠民计划等课题20余项,总经费2000余万元。发表论文200余篇,其中SCI收录期刊论文30余篇。主编、主译专著26部,获国家科技进步二等奖等6项。

手术与复苏是战伤救治的两大核心能力,早期救治应在伤后3 h内实施,包括紧急救治及外科复苏两类职能,因此应强调复苏与手术能力的建设。本文重点阐述早期救治能级的能力要求,并根据国内外平时创伤救治和战时战伤救治的进展,梳理战伤早期救治能级中“手术”与“复苏”的相互关系及其关键技术,提出只有手术和复苏能力协调发展,相互支撑,才能为严重战伤伤员提供安全、规范、高效的救治,降低严重战伤伤员的伤死率。

Surgery and resuscitation are the core capabilities of combat casualty care. Early treatment should be carried out within 3 hours, and divided into two phases, emergency treatment and surgical resuscitation, emphasizing the capability building of resuscitation and surgery. This paper interprets the capability requirements of the early casualty care level. According to the domestic and international progress in the care of civilian trauma and combat casualty, the relationship between "surgery" and "resuscitation" in the early treatment level of combat casualty, and the key technological progress are summarized, indicating that only the coordinated development of surgery and resuscitation ability and mutual support can provide safe, standardized and efficient treatment and reduce the death rate of the combat casualties.

失血性休克及致死性出血是战伤死亡的主要原因,对此类伤员的救治是战伤救治体系建设面临的主要挑战[1]。手术与复苏是战伤救治体系的两大核心能力,可称为战伤救治的“两条腿”;“两条腿”协调前进,才能行快致远。早期救治应在伤后3 h内实施,包括紧急救治与外科复苏两类职能,因此应强调复苏及手术能力的建设。本文重点论述早期救治能级的能力要求,并根据国内外平时创伤救治及战时战伤救治的进展,梳理战伤早期救治能级中手术与复苏的相互关系及其关键技术。
早期救治职能既包括了休克防治、感染防治、深筋膜切开减压及软组织清创等“紧急救治阶梯”的部分内容,又纳入了防治战伤后并发症等“专科治疗阶梯”的部分内容。根据是否有加强力量划分,对早期救治能级的复苏及手术两大核心能力可为两个次能级:(1)没有力量加强的机构,履行紧急救治职能;(2)有力量加强的机构,履行以外科手术及重伤员救治为主的外科复苏职能。
复苏指生物体或离体的器官、组织或细胞等在生理功能极度减缓后又恢复正常的生命活动。一般概念上的复苏术指呼吸心跳骤然停止时采取的一切急救措施。严重战伤面临气道、呼吸及循环功能衰竭,涉及紧急救治的措施均具有复苏的性质,剖胸、剖腹手术止血等本身就是复苏的组成部分,存在持续出血的战伤伤员必须手术,出血的源头不关闭,复苏就达不到终点,此即为复苏性手术的概念。实施战伤早期救治时,要准确理解手术与复苏的关系,同时注意避免两种倾向或错误:(1)将紧急救治和外科复苏两个次能级理解成前后次序,认为伤员应先经过紧急救治次能级后,才转送至外科复苏次能级,从而影响救治的时效性;(2)将外科复苏理解为就是外科手术,片面强调外科手术能力的建设,而忽视了重症复苏能力的建设,这样可能导致虽然手术成功,但伤员却死亡的结局。
战伤救治能力应不低于平时创伤救治能力,承担战伤救治的医疗护理团队能力应基于平时创伤救治的积累,建立可持续的战时战伤-平时创伤救治的协同关系,这也是构建一支随时待命的医疗队的基石[2]
创伤救治救的是伤员,而不是某个具体的损伤。整体观念是创伤中心建设与其他外科专科的重大区别之一。面对高能量损伤伤员,应根据致伤机制预判可能发生的损伤,根据血流动力学状态等评估损伤的严重度及危险性,还应根据伤情的轻重缓急优先处理致命性的损伤。对于胸、腹部等躯干损伤所致出血,紧急手术或介入治疗是控制出血的关键,故应尽快将伤员送至有手术能力的创伤中心实施损害控制性手术,包括剖腹、剖胸等。速度是创伤救治的灵魂,“黄金时间”的概念要求缩短创伤至确定性手术的时间,包括缩短院前救治时间、转运时间及院内救治时间。由以创伤紧急救治为主体责任的多学科团队实施整体救治,避免会诊延长院内术前时间,已成为实体化创伤中心建设的标准模式。
现代重症监护室(intensive care unit,ICU)集中了先进的仪器设备,并配备了熟练掌握生命支持、各种脏器监测及支持技术的专业医护人员,能保证24 h连续密切监测及高质量的脏器支持能力[3]。严重创伤伤员经常出现器官系统功能障碍,可能发生在伤后早期,也可能发生在紧急手术后,应对其实施积极的脏器功能监护、干预等[4]。在严重创伤伤员救治中,外科利用手术方法控制出血、污染、减压、固定及管道管理等,重症医学科则提供生命监测及脏器功能支持。创伤后由于大出血、缺氧、张力性气胸、心脏压塞等原因可导致创伤性心脏骤停(traumatic cardiac arrest,TCA),约占全部心脏骤停的10%,其存活率仅为5.6%(0~17%)。掌握主动脉球囊阻断(resuscitative endovascular balloon occlusion of the aorta,REBOA)、选择性主动脉弓灌注(selective aortic arch perfusion,SAAP)、体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)等TCA复苏新技术,坚持TCA积极复苏理念,持续提高创伤救治团队的复苏能力,也是创伤中心建设的重要抓手[5]。随着损害控制(damage control,DC)在严重创伤救治中的应用,ICU中救治的严重创伤伤员逐渐增加,即使是经过紧急手术后入住ICU的创伤伤员,在ICU期间也有16%的患者需要多次行外科诊疗操作,包括控制出血、遗漏损伤处理、创伤或手术并发症处理、计划性分期手术等,具体术式包括剖胸探查术、剖腹探查术、腹腔扩容术、胸腔闭式引流术、肝动脉栓塞术及骨伤手术等[6]。新近出版的《中华创伤重症医学》[7]总结了近年来创伤重症的主要进展,包括:(1)在创伤重症救治的早期同时开展相关的专科治疗;(2)各种与创伤重症救治有关的高新技术、产品的研发及快速转化应用;(3)创伤重症医学(traumatic critical care medicine,TCCM)的发展促使基础与临床研究、外科与重症救治密切协同,并涌现了一大批创伤重症医学专家。这些都说明以重症医学为平台的多学科团队在严重创伤救治中的作用日益凸显,已被实践证实是提高救治成功率的关键,而ICU已成为创伤复苏单元、手术室之后的又一个严重创伤救治的主战场[3]
伤员与军医间的距离是决定伤员能否存活的关键因素。在阿富汗战争及伊拉克战争期间,美军主要采用非线性作战,即没有确定的前线或敌军,军事行动可能在作战区域内任何地方突然爆发,为实施紧急医疗保障,美军逐渐建设了模块化、小型化、机动性和灵活性兼备的前沿外科手术队(forward surgical team,FST)。美军FST虽然强调手术能力建设(对不能后送的伤员行紧急初期手术,24 h内可完成20台手术,手术范围包括剖胸术、剖腹术、开放性骨折外支架固定等),但也要求具备重症监护能力(包括大出血、多发伤、复合伤伤员的术前评估及术后护理)。FST能对30名伤员提供72 h连续术后治疗及后送前伤员的处置[8]。如在救治1例因简易爆炸装置(IED)爆炸致左侧股骨、臀部受伤的伤员时,首次手术包括骨折外固定、直肠裂伤修补、骶尾部伤口填塞,术后立即转监护病房输血、复苏等,次日剖腹行乙状结肠转流性造口。此例伤员的救治首先是基于可用资源(人、物)进行复苏,遵循最小化的损害控制手术策略控制出血及污染,然后是积极输注全血、主动复温等损害控制性复苏措施。
我军规定战时各级救治机构应当按照职责分工执行相应的救治,根据救治需求、救治能力及救治环境的变化,可以适当扩大或缩小救治范围。紧急救治次能级在以紧急内科重症处置为核心的同时,也要求具备外科手术能力,但不要求具备剖腹、剖胸、开颅的能力;外科复苏次能级则要求具备所有战伤损害控制性外科和急救手术能力,同时具备重症监护能力。总体而言,在高度重视一线救治的基础上,早期救治能力建设对战伤外科手术救治有明确而详细的要求,并高度重视手术能力所需的装备及物资保障、人员配备及技能训练。在实战中承担早期救治能级的各医疗机构须尽可能有外科力量加强,以便遵循时效性救治策略,在时间窗内完成损害控制性外科手术。
在同一医疗机构内提供伤员所需的所有救治资源是最佳策略。随着战略重心及作战样式的不断改变,美国陆军不断改进并重构FST,自2015年建立前沿复苏外科队(forward resuscitation surgical team,FRST),并在2020年底发布前沿复苏外科队条例(ATP 4-02.25)[9],英文名称改为forward resuscitative and surgical detachment(FRSD),主要任务是实施损害控制复苏及损害控制手术,以稳定伤员伤情,使其能够后送到后一级救治机构开展进一步救治,具体包括:(1)剖腹探查(控制出血及污染)、简单剖胸探查(控制胸部穿透伤内出血)、骨折外固定、肢体血管损伤临时再通、损毁肢体截肢、筋膜切开减压、心脏压塞减压、紧急开颅减压等8种外科手术能力;(2)具备快速止血、低压复苏、快速补充循环血容量,通过血液制品、药物预防并治疗低体温及酸中毒,以及术后持续复苏等损害控制复苏能力。FRSD由管理保障组、前沿复苏组及前沿外科组构成,可被认为是FST的重症加强版。
有30%~40%的严重战伤伤员死于难以控制的大出血,输血在降低大出血伤员伤亡率中的重要性可比肩外科止血。战伤输血的主要进展包括:(1)紧急复苏时血液制品输注策略。院前阶段出血尚未控制,须遵循限制性复苏策略,但不能以牺牲心、脑等重要生命器官血供为代价。院前尽早启动血液制品输注,不仅是维持血压的基础,也是纠正凝血功能障碍乃至止血的前提。我军要求休克伤员液体复苏时应遵循以下优先级顺序:全血,按1:1:1比例输注红细胞、血浆、血小板,按1:1比例输注红细胞与血浆,血浆与红细胞,晶体液与胶体液。美军要求如果伤员发生休克且有条件输血时应遵循以下优先级顺序:冷藏低滴度抗体O型全血,预筛低滴度抗体O型新鲜全血,按1:1:1比例输注红细胞、血浆、血小板,按1:1比例输注红细胞与血浆,单用血浆或红细胞[10]。(2)大量输血方案。伤员急性失血量达自身血容量的30%~50%时,往往需要大量输血。美军预测大量输血的指征包括收缩压≤90 mmHg、心率≥120次/min、穿透伤、腹腔创伤超声重点评估(FAST)阳性。出现以下因素中的2个及以上时需要大量输血的概率大于35%:心率>105次/min、收缩压<110 mmHg、pH值<7.25、血细胞比容<32%。(3)新鲜全血采集及输注。一旦储备血液制品告罄,“移动血库(moving blood bank,MBB)”可能是唯一的选项。美军野战条件下新鲜全血输注的适应证为出现威胁生命的严重战伤,库存血液不足或伤员对库存血液制品输注无效时。在阿富汗战争和伊拉克战争中,美军共输注了6000 U的新鲜全血,结果显示其安全、有效。(4)低滴度O型全血(LTOWB)输注。免疫球蛋白IgM滴度在1:256以下时可作为通用供血,新鲜LTOWB可用于未知血型的紧急输血,并明显降低输血反应的风险。根据美军最新实战经验,在战斗环境中使用冷储LTOWB被证明是可行的,并且该产品的使用率正在增高[11]
除了输血能力,重症复苏能力还包括生命体征检测及脏器功能监测、脏器功能支持与并发症防治。胸部爆炸伤是早期致死性最高的爆炸伤类型之一。2022年,中华医学会创伤学分会牵头发布的《胸部爆炸伤紧急救治临床指南(2022年》中的19条推荐意见中,关于院内紧急救治的有7条,其中涉及手术的包括手术指征、胸部出血治疗及肋骨骨折治疗等3条,涉及重症的包括机械通气治疗、ECMO、药物治疗及肺冲击伤治疗等4条[12],从另一侧面佐证了爆炸伤等严重战伤救治时,重症监护需要强大的脏器功能支持及替代能力,外科复苏次能级应具备和平时期创伤中心的重症监护救治能力。
严重战伤可累及多部位、多系统,导致脏器毁损及大出血,并可导致失血性休克、致命三联征、严重感染及脏器功能损害等。伤员救治既需要腹部、心胸、颅脑、骨关节等多外科的密切协作,遵循损害控制外科策略,在恰当的时机实施恰当的外科诊疗操作控制出血、污染等;也需要具备生命监测及多脏器功能监护的重症医学密切协作,遵循损害控制复苏策略,及时发现伤员脏器并发症的高危因素,对危及生命的紧急情况作出迅速反应,积极防治休克、致命三联征、感染及脏器功能损害。理论上损害控制复苏包括损害控制外科,而在实践中二者通常由不同的团队承担、实施。战伤救治要求在一个医疗机构内具备伤员救治需要的所有救治资源,只有手术和复苏能力协调发展、相互支撑,才能为严重战伤伤员提供安全、规范、高效的救治,降低严重战伤伤员的伤死率。
