Objective To explore the effect of miR-491-5p on the apoptosis of trophoblast cells induced by hypoxia and the process of preeclampsia. Methods Human trophoblast cells were divided into control group, hypoxia group, hypoxia+miR-491-5p mimic group and hypoxia+miR-491-5p inhibit group. Cell viability staining was used to detect hypoxia-induced trophoblast cell apoptosis. qRT-PCR was used to detect the expression of miR-491-5p in hypoxia-induced trophoblast cells; TargetScan database to predict miR-491-5p target; Detection of dual-luciferase reporter gene was used to confirm the targeting relationship between miR-149-5p and BCL2l2; miR-491-5p mimic and inhibitor single-stained embryonic trophoblast cells, qRT-PCR was used to detect the expression of miR-491-5p. Western blotting was used to detect changes of BCL2l2 expression in trophoblast after overexpression and inhibition of miR-491-5p; cell viability staining, Western blotting, and flow cytometry to detect trophoblast apoptosis. Results Compared with control group, the hypoxia group had significantly increased cell apoptosis and increased expression of miR-491-5p (2.784±0.214 vs. 1.000±0.000, P<0.01); bioinformatics detection and dual luciferase detection report experiments showed that miR-491-5p can targete and regulate BCL2l2 expression. Compared with control group, the miR-491-5p content in hypoxia+miR-491-5p mimic group was significantly increased (7659.300±191.300 vs. 5.467±0.503, P<0.05), while that in miR-491-5p inhibit group was the opposite (0.139±0.022 vs. 0.323±0.035, P<0.01). Compared with hypoxia group, the apoptosis rate of trophoblast cells was decreased (11.56%±0.452% vs. 16.67%±0.750%, P<0.01), the expressions of BCL2L2 (0.702±0.047 vs. 0.312±0.007, P<0.001) and Bcl-2 (0.752±0.055 vs. 0.415±0.005, P<0.01) were increased, and the expression of Bax (1.221±0.066 vs. 1.652±0.085, P<0.01) was decreased in hypoxia+miR-491-5p inhibit group; but in hypoxia+miR-491-5p mimic group, the apoptosis rate was increased (23.54%±1.233% vs. 16.67%±0.750%, P<0.001), the contents of BCL2L2 (0.211±0.013 vs. 0.312±0.007, P<0.01) and Bcl-2 (0.203±0.011 vs. 0.415±0.005, P<0.001) was decreased, and Bax (2.362±0.284 vs. 1.652±0.085, P<0.01) expression was increased. Conclusions miR-491-5p might negatively regulates the expression of BCL2L2, increases the apoptosis of trophoblast cells induced by hypoxia and accelerates the development of preeclampsia.

目的 探讨microRNA-491-5p(miR-491-5p)在缺氧诱导的滋养细胞凋亡中的作用及其对子痫前期的影响。方法 选取人绒毛膜滋养层细胞,分为4组:对照组、缺氧组、缺氧+miR-491-5p mimic组和缺氧+ miR-491-5p inhibit组。采用细胞活力染色检测滋养细胞凋亡情况,qRT-PCR检测miR-491-5p的表达,TargetScan数据库预测miR-491-5p的靶基因,双荧光素酶报告基因实验验证miR-149-5p与B-细胞淋巴瘤因子2样蛋白2基因(BCL2L2)的靶向关系。miR-491-5p mimic和inhibitor分别转染滋养细胞后,qRT-PCR检测miR-491-5p的表达,Western blotting检测BCL2L2蛋白的表达改变,细胞活力染色、Western blotting、流式细胞术检测滋养细胞凋亡情况。结果 与对照组比较,缺氧组滋养细胞凋亡率明显增高(P<0.001),miR-491-5p表达明显增高(2.784±0.214 vs. 1.000±0.000,P<0.01);生物信息学预测与双荧光素酶检测报告实验显示,miR-491-5p可与BCL2L2结合并调控其表达。与对照组比较,miR-491-5p mimic组miR-491-5p表达明显增高(7659.300±191.300 vs. 5.467±0.503,P<0.01),而miR-491-5p inhibit组miR-491-5p表达明显降低(0.139±0.022 vs. 0.323±0.035,P<0.05)。与缺氧组比较,缺氧+miR-491-5p inhibit组滋养细胞凋亡率降低(11.56%±0.452% vs. 16.67%±0.750%,P<0.01),BCL2L2和Bcl-2蛋白相对表达量增高(0.702±0.047 vs. 0.312±0.007,P<0.001;0.752±0.055 vs. 0.415±0.005,P<0.01),Bax蛋白表达降低(1.221±0.066 vs. 1.652±0.085,P<0.05);缺氧+miR-491-5p mimic组滋养细胞凋亡率增高(23.54%±1.233% vs. 16.67%±0.750%,P<0.001),BCL2L2和Bcl-2蛋白相对表达量降低(0.211±0.013 vs. 0.312±0.007,P<0.01;0.203±0.011 vs. 0.415±0.005,P<0.001),Bax蛋白表达增高(2.362±0.284 vs. 1.652±0.085,P<0.01)。结论 miR-491-5p可负向调控BCL2L2表达,促进缺氧诱导的滋养细胞凋亡,进而加速子痫前期的发展进程。

Objective To explore the effect of miR-491-5p on the apoptosis of trophoblast cells induced by hypoxia and the process of preeclampsia. Methods Human trophoblast cells were divided into control group, hypoxia group, hypoxia+miR-491-5p mimic group and hypoxia+miR-491-5p inhibit group. Cell viability staining was used to detect hypoxia-induced trophoblast cell apoptosis. qRT-PCR was used to detect the expression of miR-491-5p in hypoxia-induced trophoblast cells; TargetScan database to predict miR-491-5p target; Detection of dual-luciferase reporter gene was used to confirm the targeting relationship between miR-149-5p and BCL2l2; miR-491-5p mimic and inhibitor single-stained embryonic trophoblast cells, qRT-PCR was used to detect the expression of miR-491-5p. Western blotting was used to detect changes of BCL2l2 expression in trophoblast after overexpression and inhibition of miR-491-5p; cell viability staining, Western blotting, and flow cytometry to detect trophoblast apoptosis. Results Compared with control group, the hypoxia group had significantly increased cell apoptosis and increased expression of miR-491-5p (2.784±0.214 vs. 1.000±0.000, P<0.01); bioinformatics detection and dual luciferase detection report experiments showed that miR-491-5p can targete and regulate BCL2l2 expression. Compared with control group, the miR-491-5p content in hypoxia+miR-491-5p mimic group was significantly increased (7659.300±191.300 vs. 5.467±0.503, P<0.05), while that in miR-491-5p inhibit group was the opposite (0.139±0.022 vs. 0.323±0.035, P<0.01). Compared with hypoxia group, the apoptosis rate of trophoblast cells was decreased (11.56%±0.452% vs. 16.67%±0.750%, P<0.01), the expressions of BCL2L2 (0.702±0.047 vs. 0.312±0.007, P<0.001) and Bcl-2 (0.752±0.055 vs. 0.415±0.005, P<0.01) were increased, and the expression of Bax (1.221±0.066 vs. 1.652±0.085, P<0.01) was decreased in hypoxia+miR-491-5p inhibit group; but in hypoxia+miR-491-5p mimic group, the apoptosis rate was increased (23.54%±1.233% vs. 16.67%±0.750%, P<0.001), the contents of BCL2L2 (0.211±0.013 vs. 0.312±0.007, P<0.01) and Bcl-2 (0.203±0.011 vs. 0.415±0.005, P<0.001) was decreased, and Bax (2.362±0.284 vs. 1.652±0.085, P<0.01) expression was increased. Conclusions miR-491-5p might negatively regulates the expression of BCL2L2, increases the apoptosis of trophoblast cells induced by hypoxia and accelerates the development of preeclampsia.

子痫前期(preeclampsia,PE)是一种较为常见的妊娠高血压疾病类型,其主要临床表现为妊娠20周后孕妇出现高血压、蛋白尿、水肿等,是孕产妇和胎儿发病及死亡的主要原因之一[1]。研究显示,胎盘滋养细胞浸润不足、凋亡过多、螺旋动脉重构不良是该病的主要病理特征[2-3]。滋养细胞作为一种来自胚胎外滋养层、具有滋养功能的细胞,其功能异常和结构变化与妊娠期多种疾病的发生发展密切相关。胎盘滋养细胞缺氧是PE发生的关键环节。MicroRNAs(miRNAs)是一类广泛存在于真核细胞,具有基因表达调控功能的内源性非编码RNA。研究显示,miRNA与多种生物学功能(细胞自噬、增殖、凋亡等)有关[4-5];PE患者胎盘组织中miRNA表达多存在异常,提示miRNA可能参与滋养细胞的功能异常。miR-491-5p与多种癌细胞的增殖和凋亡有关[6],但其对滋养细胞的影响尚不明确。本研究复制滋养细胞缺氧模型,旨在探讨miR-491-5p对滋养细胞凋亡的影响,并寻找关键靶基因,从而为PE的诊断及治疗提供更多依据。
人绒毛膜滋养层细胞(HTR-8/SV-neo)购自上海瑞鹿生物技术有限公司;胎牛血清、RPMI 1640培养基购自美国Gibco公司;青霉素-链霉素,含酚红、EDTA胰蛋白酶消化液购自中国Solarbio公司;qRT-PCR试剂盒购自日本TaKaRa公司;细胞总RNA提取试剂盒购自中国TIANGEN公司;miR-491-5p mimic、miR-491-5p inhibitor由中国Genepharma公司合成;脂质体(LipofectamineTM 2000) 购自美国 Invitrogen 公司;双荧光素酶活性检测试剂盒购自美国Promega公司;miR-491-5p 引物购自生工生物工程(上海)股份有限公司;B-细胞淋巴瘤因子2样蛋白2(BCL2L2)抗体购自美国Proteintech公司;B淋巴细胞瘤-2(Bcl-2)和Bax蛋白抗体购自英国Abcam公司。
超净工作台(苏州安泰);CO2培养箱(德国Heraeus 公司);5415D 型微量台式离心机(德国Eppendorf 公司);超微量分光光度计(美国SimpliNano 公司);垂直电泳仪、转膜仪、曝光仪(美国Bio-Rad 公司);BS110S 型精密天平(德国Sartorius 公司);水平摇床(美国Thermo Fisher 公司);梯度 RCR 仪(德国Biometra Tone 公司);荧光定量 PCR 仪(上海枫岭);ACEA NovoCyte 流式细胞仪(美国艾森生物);激光共聚焦显微镜(德国ZEISS公司)。
选取HTR-8/SV-neo细胞,设对照组和缺氧组,观察缺氧对细胞凋亡、凋亡相关蛋白(Bcl-2、Bax)和miR-491-5p表达的影响;另设缺氧+miR-491-5p mimic组和缺氧+miR-491-5p inhibitor组,观察miR-491-5p对缺氧诱导的滋养细胞凋亡及BCL2L2、Bax、Bcl-2蛋白表达的影响。
(1)细胞培养:用含有10%胎牛血清、1%青霉素-链霉素的RPMI 1640培养基培养HTR-8/SV-neo细胞,置于37 ℃、5%CO2环境中。(2)细胞转染:将HTR-8/SV-neo细胞接种于6孔板中,培养过夜,待细胞密度达到70%时,采用RPMI 1640培养基,按照Lipofectamine 2000操作说明进行脂质体转染;转染6 h后更换完全培养基,缺氧组细胞置于缺氧袋中缺氧处理,培养48 h,收取细胞用于后续实验。
弃去培养基,PBS清洗3次,按照以下比例配制染料:在15 ml离心管中依次加入5 ml PBS和5 μl NucleiDye、25 μl Dead Dye、3 μl Viable Dye;将3种染料混合后,在每个培养皿中依次加入200 μl混合染料,于37 ℃孵育30 min后取出,激光共聚焦显微镜观察细胞凋亡情况。其中红色(Cy3)提示为凋亡细胞,绿色(FITC)提示为正常细胞,蓝色(DAPI)提示为细胞核。
蛋白裂解液收集细胞蛋白样品,定量后进行SDS-PAGE电泳分离、转膜,5%脱脂奶粉封闭2 h,室温下使用BCL2L2抗体(1:2000)、Bax抗体(1:1000)、Bcl-2抗体(1:1000)4 ℃孵育过夜;PBST洗涤3次后加入HRP标记的二抗,室温下孵育2 h;洗涤后加入化学发光液显色,成像后拍照,利用软件进行灰度值统计。
从各组细胞中提取总RNA,用紫外分光光度计测定RNA纯度和浓度。根据反转录试剂盒说明书操作,扩增引物序列如下。miR-491-5p上游引物:5'-GGAGTGGGGAACCCTTCC-3',下游引物:5'-GTGCAGGGTCCGAGGT-3';内参U6上游引物:5'-CTCGCTTCGGCAGCACA-3',下游引物:5'-AACGCTTCACGAATTTGCGT-3'。取1 μg总RNA作为模板反转录为cDNA;以cDNA为模板,PCR反应条件为:95.0 ℃ 30 s;95.0 ℃ 5 s、61.5 ℃34 s,2~39个循环。结果以2–ΔΔCt法计算。
通过TargetScan (http://www.targetscan.org/vert_72/)在线预测出miR-491-5p与BCL2L2的3'-UTR之间存在碱基互补关系。构建BCL2L2的3'-UTR野生型及突变型载体,分别将miR-491-5p mimic及其对照物miR-NC与BCL2L2 3'-UTR野生型(WT)和突变型(MUT)载体转染HTR-8/SV-neo细胞。48 h后收集细胞,按照双荧光素酶报告基因检测试剂盒操作说明,检测细胞中萤火虫荧光素酶和海肾荧光素酶活性,计算方法为萤火虫荧光值/海肾荧光值。
将各组滋养细胞用1 ml胰酶消化液消化3 min,预冷PBS洗涤后4 ℃离心收集细胞,流式缓冲液重悬细胞,加入5 μl An-nexin V-FITC混匀,4 ℃避光孵育30 min,加5 μl PI染液,孵育5 min,立即用流式细胞仪进行检测。细胞凋亡率为Q1+Q4区细胞所占的比例(Q1位于第一象限,表示晚期凋亡细胞;Q2位于第二象限,表示机械损伤细胞;Q3位于第三象限,表示正常细胞;Q4位于第四象限,表示早期凋亡细胞)。
采用Prism7.0软件进行统计分析。计量资料以$\bar{x}±s$表示,两组间比较采用t检验;多组间比较采用One-way ANOVA检验,进一步两两比较采用SNK-q检验。P<0.05 为差异有统计学意义。
细胞活力染色结果显示,与对照组比较,缺氧组红色荧光增多,绿色荧光减少,滋养细胞凋亡率(%)明显增高(74.33±5.84 vs. 39.54±6.56,P<0.001);Western blotting检测显示,与对照组比较,缺氧组滋养细胞中Bcl-2蛋白相对表达量明显降低(0.514±0.003 vs. 1.000±0.000,P<0.001),Bax蛋白相对表达量明显增高(1.687±0.022 vs. 1.000±0.000,P<0.01),Bax/Bcl-2比值明显增高(3.927±0.264 vs. 1.000±0.000,P<0.001,图1)。
qRT-PCR检测结果显示,与对照组比较,缺氧组miR-491-5p相对表达量明显增高(2.784±0.214 vs. 1.000±0.000,P<0.01)。
采用生物信息学软件TargetScan(http://www.targetscan.org/vert_72/)预测的结果显示,miR-491-5p与BCL2L2的3'-UTR之间存在碱基互补关系(图2)。双荧光素酶报告基因实验显示,与对照组比较,miR-491-5p mimic组共转染WT-BCL2L2后荧光素酶活性明显降低(0.531±0.004 vs. 1.375±0.080,P<0.001),而共转染MUT-BCL2L2后荧光素酶活性与对照组比较差异无统计学意义(P>0.05)。
qRT-PCR检测结果显示,与对照组比较,miR-491-5p mimic组miR-491-5p表达水平明显增高(7659.300±191.300 vs. 5.467±0.503,P<0.01,图3A),而miR491-5p inhibit组miR-491-5p表达水平明显降低(0.139±0.022 vs. 0.323±0.035,P<0.05,图3B)。
Western blotting检测各组BCL2L2蛋白表达情况,结果显示,与对照组比较,缺氧组BCL2L2蛋白相对表达量明显降低(0.312±0.007 vs. 1.000±0.000,P<0.001);与缺氧组比较,miR-491-5P mimic组BCL2L2蛋白相对表达量明显降低(0.211±0.013 vs. 0.312±0.007,P<0.01),miR-491-5p inhibit组BCL2L2蛋白相对表达量明显升高(0.702±0.047 vs. 0.312±0.007,P<0.001,图4)。
Western blotting检测结果显示,与缺氧组比较,miR-491-5p mimic组Bcl-2蛋白表达量明显降低(0.203±0.011 vs. 0.415±0.005,P<0.001),Bax蛋白表达量明显升高(2.362±0.284 vs. 1.652±0.085,P<0.01);而miR-491-5p inhibit组Bcl-2蛋白表达量明显升高(0.752±0.055 vs. 0.415±0.005,P<0.01),Bax蛋白表达量明显降低(1.221±0.066 vs. 1.652±0.085,P<0.05,图5A)。流式细胞术检测结果显示,与缺氧组比较,miR-491-5P mimic组细胞凋亡率明显升高(23.54%±1.233%vs. 16.67%±0.750%,P<0.001),而miR-491-5p inhibit组细胞凋亡率(%)明显降低(11.56%±0.452% vs. 16.67%±0.750%,P<0.01,图5B)。
PE是以孕妇妊娠后因内环境改变导致胎盘功能障碍为主要特征,以高血压、蛋白尿等为主要临床表现的妊娠期特有疾病[2]。据报道,PE的发病率为9%~11%,是世界范围内引起孕产妇及围产儿死亡的主要原因之一[7];相比健康孕妇,PE患者的胎儿更易出现宫内发育迟缓、早产和宫内死亡。因此,PE的发生和发展过程及其机制研究具有重要的临床意义。胎盘作为妊娠期特有的临时器官,承载着母体营养传输和母胎屏障等功能,是妊娠期母胎之间适应性调节的重要纽带。值得关注的是,随着胎儿及附属物(胎盘等)的娩出,PE患者的临床症状可消失,各项指标也恢复正常,提示胎盘是该疾病的关键靶器官,其功能异常可能是PE发生的核心和关键[8-9]。滋养细胞作为胎盘组织的关键组分,其功能异常已成为当前研究的焦点[10-11]。为此,本文选取滋养细胞作为研究对象,开展PE的相关机制研究。
近年来相关研究显示,正常妊娠过程中的细胞凋亡有利于滋养细胞生理功能(浸润、迁移等)的调控,但滋养细胞过度凋亡可致胎盘浅着床、胎盘血液灌注量减少,进而导致PE的发生[12-13],提示滋养细胞凋亡是促进PE进程的重要机制[14]。研究显示,在由缺氧导致PE和胎儿生长受限的妊娠女性中,胎盘滋养细胞凋亡的发生率更高,这提示缺氧促进滋养细胞凋亡可能是PE的关键环节[15-16]。本研究采用体外缺氧处理滋养细胞,模拟PE的胎盘环境,结果显示,缺氧后滋养细胞凋亡率增高,且凋亡相关蛋白Bax表达增高,而Bcl-2蛋白表达降低,提示缺氧可促进滋养细胞凋亡,与文献报道一致[17-18],但其具体机制仍有待进一步研究。
miRNA是一类广泛存在、具有调节功能的转录后水平调节因子。大量研究显示,miRNA可通过降解信使RNA(mRNA)来下调目的基因的表达,从而参与多种生物学过程(增殖、凋亡、自噬等)的调控[19-20]。有研究显示,PE患者胎盘组织中多种miRNA含量发生变化,且这些变化可能影响滋养细胞的功能,进而调节PE的发展过程[21]。miR-491-5p是miR-195的成熟体之一,有研究通过基因芯片和功能分析发现其可调控VPS28基因在乳腺癌细胞中的表达,促进癌症进程[22];有报道称miR-491-5p表达下调能够促进创伤性脑损伤后血管新生[23];在人骨肉瘤中的研究显示,上调miR-491-5p可靶向癌症相关基因FOXP4并抑制其表达,从而抑制骨肉瘤细胞增殖并促进细胞凋亡[24]。以上研究提示,miR-491-5p广泛存在于多种组织细胞中,可通过调控细胞凋亡参与多种疾病的发生发展,但其对缺氧诱导的滋养细胞凋亡的影响尚不明确。本研究通过qRT-PCR检测发现,缺氧后滋养细胞中miR-491-5p含量增加,提示其可能参与了缺氧诱导的滋养细胞凋亡过程。有研究显示,miRNA能够通过作用到靶基因的3'-UTR区实现对靶基因表达的调控,从而影响相关的细胞生物学功能[25-26];BCL2L2蛋白作为Bcl-2蛋白家族的重要成员,有研究显示其可通过抑制凋亡通路中的关键蛋白——半胱氨酸蛋白酶3和7而发挥抗凋亡作用[27]。基于此,本研究利用生物信息学软件预测miR-491-5p的靶基因,采用双荧光素酶报告基因观察到,miR-491-5p能够与BCL2L2结合,提示BCL2L2可能是miR-491-5p的靶基因,miR-491-5p调控BCL2L2的表达可能是促进滋养细胞凋亡的关键机制。为了进一步明确miR-491-5p调控BCL2L2对缺氧诱导的滋养细胞凋亡的影响,本研究分别过表达和抑制miR-491-5p后检测滋养细胞凋亡的变化,结果显示,过表达miR-491-5p后,滋养细胞凋亡率增高,且BCL2L2和Bcl-2蛋白表达降低,Bax蛋白表达升高,而干扰miR-491-5p后结果则相反,提示miR-491-5p可通过负向调节BCL2L2蛋白的表达,促进缺氧诱导的滋养细胞凋亡。
综上所述,本研究结果提示,BCL2L2可能是miR-491-5p的靶基因,miR-491-5p可能是滋养细胞凋亡的关键分子,可负向调控BCL2L2蛋白的表达,促进缺氧诱导的滋养细胞凋亡。滋养细胞凋亡是PE的重要环节,但其具体的分子机制尚不清楚。本研究仅在细胞层面探究了miR-491-5p对滋养细胞凋亡的影响,其在机体内的具体作用尚待进一步验证。关于miR-491-5p是否在机体内促进滋养细胞凋亡及其可能的调控机制可作为后续进一步研究的方向,有望为PE的发生机制研究提供新的视角。
