Objective To analyze the detection characteristics of novel hepatitis B virus (HBV) serum biomarkers including HBV RNA, hepatitis B core-related antigen (HBcrAg), hepatitis B core antibody (anti-HBc), intrahepatic HBV total DNA (tDNA), and covalently closed circle (cccDNA) in patients with different types of HBV infection. Methods A total of 227 HBV-infected patients, who were hospitalized in the Fifth Medical Center of Chinese PLA General Hospital from January 2017 to December 2019 and underwent liver biopsy, were included in this study. The clinical data were retrospectively analyzed and serum HBV RNA, HBcrAg, anti-HBc, intrahepatic HBV tDNA and cccDNA were quantitatively detected. The characteristics and anti-HBV treatment impact on the above virological biomarkers in patients with different types of clinical illness were analyzed. Results Among 16 patients with acute hepatitis B (AHB), 4 were positive for serum HBV RNA (25.0%), 15 were positive for serum HBcrAg(93.8%), 14 were positive for HBV tDNA (87.5%), and 13 were positive for cccDNA (81.3%) in liver tissues. Among 184 patients with chronic hepatitis B (CHB), 34 were treated with nucleos(t)ide analogues (NAs), and their HBV RNA and HBcrAg contents were significantly lower than those without treatment, and the anti-HBc content was significantly higher (P<0.05). Among 18 patients with liver cirrhosis (LC), 9 were treated with NAs, the virological parameters, excepting HBV DNA, showed no significant differences between NAs-treated patients and treatment-naïve patients (P>0.05). Among 9 patients with hepatocellular carcinoma,5 were positive for serum HBV RNA (55.6%), 7 were positive for serum HBcrAg (77.8%), 8 were positive for HBV tDNA (88.9%),and 6 were positive for cccDNA (66.7%) in liver tissues. Conclusions Serum HBcrAg might be used to differentiate whether AHB is cured or not. In NAs-treated patients, even if HBV DNA decreases to the undetectable level, HBVRNA is still detectable and might cause the slow progresses of the disease, so antiviral therapy cannot be discontinued. Serum HBV RNA and HBcrAg might serve as serological substitutes for intrahepatic HBV cccDNA.

目的 分析乙型肝炎病毒(HBV)新型血清学标志物HBV RNA、HBV核心抗原相关抗原(HBcrAg)、HBV核心抗体(anti-HBc)及肝组织HBV总DNA(tDNA)、共价闭合环状DNA(cccDNA)在不同类型HBV感染中的检出特点。方法 纳入2017年1月-2019年12月在解放军总医院第五医学中心住院并接受肝组织活检的227例不同疾病类型的HBV感染患者,回顾性分析其临床资料,检测其血清HBV RNA、HBcrAg、anti-HBc定量及肝组织HBV tDNA、cccDNA水平,比较分析各指标的特征以及抗病毒治疗对其的影响。结果 在16例急性乙型肝炎(AHB)患者中,HBV RNA检出率为25.0%(4/16),HBcrAg检出率为93.8%(15/16),肝组织HBV tDNA及cccDNA检出率分别为87.5%(14/16)及81.3%(13/16)。在184例慢性乙型肝炎(CHB)患者中,34例接受核苷(酸)类似物(NAs)治疗,其HBV RNA和HBcrAg定量均明显低于未治疗组,anti-HBc定量明显高于未治疗组(P<0.05)。在18例肝硬化(LC)患者中,9例接受NAs治疗,除HBV DNA外,其他新型病毒学指标在NAs经治者与未治者间差异无统计学意义(P>0.05)。在9例肝癌(HCC)患者中,HBV RNA检出率为55.6%(5/9),HBcrAg检出率为77.8%(7/9),肝组织HBV tDNA及cccDNA检出率分别88.9%(8/9)及66.7%(6/9)。结论 HBcrAg可作为AHB彻底治愈的鉴别指标;对于NAs经治患者,即使HBV DNA降至不可测,但只要能检测出HBV RNA,则仍可导致疾病缓慢进展,因此应继续抗病毒治疗;HBV RNA及HBcrAg在不同疾病类型中可作为肝组织HBV cccDNA的血清学替代指标。

目的 分析乙型肝炎病毒(HBV)新型血清学标志物HBV RNA、HBV核心抗原相关抗原(HBcrAg)、HBV核心抗体(anti-HBc)及肝组织HBV总DNA(tDNA)、共价闭合环状DNA(cccDNA)在不同类型HBV感染中的检出特点。方法 纳入2017年1月-2019年12月在解放军总医院第五医学中心住院并接受肝组织活检的227例不同疾病类型的HBV感染患者,回顾性分析其临床资料,检测其血清HBV RNA、HBcrAg、anti-HBc定量及肝组织HBV tDNA、cccDNA水平,比较分析各指标的特征以及抗病毒治疗对其的影响。结果 在16例急性乙型肝炎(AHB)患者中,HBV RNA检出率为25.0%(4/16),HBcrAg检出率为93.8%(15/16),肝组织HBV tDNA及cccDNA检出率分别为87.5%(14/16)及81.3%(13/16)。在184例慢性乙型肝炎(CHB)患者中,34例接受核苷(酸)类似物(NAs)治疗,其HBV RNA和HBcrAg定量均明显低于未治疗组,anti-HBc定量明显高于未治疗组(P<0.05)。在18例肝硬化(LC)患者中,9例接受NAs治疗,除HBV DNA外,其他新型病毒学指标在NAs经治者与未治者间差异无统计学意义(P>0.05)。在9例肝癌(HCC)患者中,HBV RNA检出率为55.6%(5/9),HBcrAg检出率为77.8%(7/9),肝组织HBV tDNA及cccDNA检出率分别88.9%(8/9)及66.7%(6/9)。结论 HBcrAg可作为AHB彻底治愈的鉴别指标;对于NAs经治患者,即使HBV DNA降至不可测,但只要能检测出HBV RNA,则仍可导致疾病缓慢进展,因此应继续抗病毒治疗;HBV RNA及HBcrAg在不同疾病类型中可作为肝组织HBV cccDNA的血清学替代指标。

Objective To analyze the detection characteristics of novel hepatitis B virus (HBV) serum biomarkers including HBV RNA, hepatitis B core-related antigen (HBcrAg), hepatitis B core antibody (anti-HBc), intrahepatic HBV total DNA (tDNA), and covalently closed circle (cccDNA) in patients with different types of HBV infection. Methods A total of 227 HBV-infected patients, who were hospitalized in the Fifth Medical Center of Chinese PLA General Hospital from January 2017 to December 2019 and underwent liver biopsy, were included in this study. The clinical data were retrospectively analyzed and serum HBV RNA, HBcrAg, anti-HBc, intrahepatic HBV tDNA and cccDNA were quantitatively detected. The characteristics and anti-HBV treatment impact on the above virological biomarkers in patients with different types of clinical illness were analyzed. Results Among 16 patients with acute hepatitis B (AHB), 4 were positive for serum HBV RNA (25.0%), 15 were positive for serum HBcrAg(93.8%), 14 were positive for HBV tDNA (87.5%), and 13 were positive for cccDNA (81.3%) in liver tissues. Among 184 patients with chronic hepatitis B (CHB), 34 were treated with nucleos(t)ide analogues (NAs), and their HBV RNA and HBcrAg contents were significantly lower than those without treatment, and the anti-HBc content was significantly higher (P<0.05). Among 18 patients with liver cirrhosis (LC), 9 were treated with NAs, the virological parameters, excepting HBV DNA, showed no significant differences between NAs-treated patients and treatment-naïve patients (P>0.05). Among 9 patients with hepatocellular carcinoma,5 were positive for serum HBV RNA (55.6%), 7 were positive for serum HBcrAg (77.8%), 8 were positive for HBV tDNA (88.9%),and 6 were positive for cccDNA (66.7%) in liver tissues. Conclusions Serum HBcrAg might be used to differentiate whether AHB is cured or not. In NAs-treated patients, even if HBV DNA decreases to the undetectable level, HBVRNA is still detectable and might cause the slow progresses of the disease, so antiviral therapy cannot be discontinued. Serum HBV RNA and HBcrAg might serve as serological substitutes for intrahepatic HBV cccDNA.

乙型肝炎病毒(hepatitis B virus,HBV)感染不但会引起急性乙型肝炎(acute hepatitis B,AHB)、慢性乙型肝炎(chronic hepatitis B,CHB),还与肝硬化(liver cirrhosis,LC)及肝细胞癌(hepatocellular carcinoma,HCC)的发生密切相关,在世界范围内造成严重的经济负担,我国77%的LC及84%的HCC是由HBV导致的[1-3]。由于感染HBV的肝细胞内持续存在共价闭合环状DNA(covalently closed circular DNA,cccDNA),导致HBV难以被清除。目前缺乏可准确评价肝组织cccDNA转录及病毒复制的有效血清学指标。
新型血清标志物HBV RNA及HBV核心抗原相关抗原(hepatitis B core-related antigen,HBcrAg)可反映肝组织cccDNA的转录活性,是近年研究的热点[4-6]。其中,HBV RNA是来自肝细胞内cccDNA的直接转录体,主要是未经或仅经部分反转录的前基因组RNA及其剪切变异体;HBcrAg包括HBeAg、HBcAg及中间产物22 ku前核心蛋白,由前-C/C区的开放读码框编码并包含一段由149个相同氨基酸组成的序列,可被同一抗体检测。乙肝核心抗体(anti-HBc)是机体产生的针对HBcrAg的特异性抗体,其定量水平与肝脏组织学炎症坏死程度呈显著正相关[7-8]。检验HBV RNA、HBcrAg及anti-HBc水平对初始治疗方案的选择、临床疗效和预后情况的评估具有重要作用,但目前对这些新指标的研究主要集中于CHB患者,缺少对AHB、LC及HCC患者的研究。在不同疾病类型的HBV感染患者中,系统性地探讨传统及上述新型HBV标志物的特征及意义的研究尚少。本研究选取不同类型的HBV感染患者,分析新型血清学标志物及肝组织HBV总DNA(total DNA,tDNA)和cccDNA的检出特征,以探讨其临床诊断价值。
收集2017年1月-2019年12月在解放军总医院第五医学中心诊治的不同类型HBV感染患者血清样本。诊断标准参照《乙型病毒性肝炎诊断标准》(标准号WS 299-2008)以及《慢性乙型肝炎防治指南(2015更新版)》[9],结合肝脏组织学以及HBV感染病程分期,符合抗病毒治疗适应证者给予核苷(酸)类似物(nucleoside/nucleotide analogues,NAs)治疗。纳入标准:接受肝组织活检,并进行肝组织HBV cccDNA检测的HBV感染患者。排除标准:合并其他病毒性肝炎、药物性肝损伤、自身免疫性肝病及酒精性或非酒精性脂肪肝等其他肝脏疾病。本研究经医院伦理委员会批准(批准号2017050D)。
采用查询病历的方式获取患者的年龄、性别等资料。乙肝抗原抗体系统(HBsAg、anti-HBs、HBeAg、anti-HBe、anti-HBc)采用Roche Cobas e601电化学发光免疫分析仪(英国罗氏诊断产品有限公司)检测,HBsAg定量检测下限为0.05 IU/ml。采用实时荧光定量PCR法检测血清HBV DNA水平(检测下限为20 IU/ml)。
血清HBV RNA荧光定量采用北京热景生物公司研发的HBV pgRNA试剂盒,检测下限为100 拷贝/ml。血清HBcrAg检测采用Lumipulse G夹心免疫检测技术(日本富士瑞必欧株式会社),检测范围3.0~7.0 log10 U/ml。血清anti-HBc检测采用化学发光微粒子免疫检测法,试剂盒购自厦门万泰凯瑞生物技术有限公司,检测有效范围为100~100 000 IU/ml,检测下限为0.25 IU/ml。
检测方法由本课题组自主创建(国家技术发明专利ZL201010273282.9)并临床应用,使用Qiagen公司的QIAamp DNA Mini Kit试剂盒提取新鲜肝组织tDNA,直接进行HBV PCR定量检测,获得结果为HBV tDNA,主要包括肝组织的松弛环状DNA(rcDNA)、双链线性DNA及cccDNA。肝组织tDNA经PSAD酶消化、滚环扩增及跨缺口PCR定量检测,获得结果为HBV cccDNA[6-7]
采用SPSS 20.0软件进行统计分析。正态分布的计量资料以$\bar{x}±s$表示,组间比较采用两独立样本t检验;偏态分布的计量资料以M(Q1Q3)表示,组间比较采用Mann-Whitney U检验。计数资料以例(%)表示,组间比较采用χ2检验或Fisher确切概率法。P<0.05为差异有统计学意义。
共纳入227例患者,男154例,女73例,年龄(39.2±10.3)岁。按疾病类型,AHB组16例,CHB组184例,LC组18例,HCC组9例。227例HBV感染患者的血清HBV RNA检出率为55.5%,定量水平为5.5(2.9,6.7) log10拷贝/ml;血清HBcrAg检出率为90.3%,定量水平4.8(3.4,8.3) log10 IU/ml;血清anti-HBc检出率为96.5%,定量水平3.8(3.1,4.3) log10 IU/ml,不同类型检出特征见表1
AHB组的HBV RNA及anti-HBc定量明显低于CHB组(P<0.05),而两组的HBcrAg定量比较差异无统计学意义(P>0.05,表1)。在4例HBsAg发生血清学转换的AHB患者中,有3例HBV DNA阳性,3例HBcrAg阳性,1例HBV RNA阳性。在184例CHB患者中,34例(18.5%)接受了NAs治疗,其HBV RNA与HBcrAg定量均明显低于未治疗组,anti-HBc定量明显高于未治疗组(P<0.05);在18例LC患者中,9例(50.0%)接受了NAs治疗,其HBV RNA、HBcrAg及anti-HBc定量与未治疗组比较差异均无统计学意义(P>0.05,图1)。
传统血清学指标HBV DNA检出率为86.8%,定量水平为4.2(3.0,7.9) log10 IU/ml,血清HBsAg检出率为92.5%,定量水平为3.4(2.7,4.3) log10 IU/ml;血清HBeAg检出率为45.8%,定量水平为3.1(1.3,3.3) log10 COI。不同类型患者的检出特征见表1
AHB组的HBV DNA、HBsAg及HBeAg定量均明显低于CHB(P<0.05,表1)。在184例CHB患者中,NAs治疗组HBV DNA、HBsAg及HBeAg定量均明显低于未治疗组(P<0.05);在18例LC患者中,NAs治疗组的HBsAg及HBeAg定量与未治疗组比较差异均无统计学意义(P>0.05,图2)。
肝组织HBV tDNA检出率为97.8%,定量水平为5.5(4.8,7.3) log10 拷贝/105,肝组织HBV cccDNA检出率为88.5%,定量水平为4.7(3.8,6.0) log10 拷贝/105。不同类型患者的检出特征见表1
AHB组的肝组织HBV tDNA及HBV cccDNA定量明显低于CHB组(P<0.05,表1)。在184例CHB患者中,NAs治疗组HBV tDNA及HBV cccDNA定量均明显低于未治疗组(P<0.05);在18例LC患者中,NAs治疗组HBV tDNA及HBV cccDNA定量与未治疗组比较差异均无统计学意义(P>0.05,图3)。
近年来,HBV新型血清标志物在HBV感染不同自然病程分期的特征及预测抗病毒应答和新药研发中的价值已成为研究热点[10]。本研究对227例不同疾病类型HBV感染患者的新型血清学标志物的特征进行了比较,结果显示,AHB患者除HBcrAg外其他病毒学标志物定量均低于CHB患者,分析可能与AHB患者体内的免疫清除状态有关,另外在HBsAg血清学转换的4例AHB患者中,仍有HBV DNA、HBV RNA及HBcrAg阳性,推测其原因如下:患者体内Dane病毒颗粒约为108/ml,而HBV RNA病毒样颗粒约106/ml,未包裹核酸的空衣壳颗粒1011/ml,HBsAg的小球型颗粒及管型颗粒约1014/ml[11],AHB患者体内的抗-HBs首先中和大部分有包膜的Dane病毒颗粒、HBV RNA病毒样颗粒以及HBsAg的小球型颗粒及管型颗粒,残余部分无包膜的裸病毒颗粒则更晚被清除,导致HBV DNA及HBcrAg的清除较其他指标更滞后,因此,HBcrAg可作为AHB患者是否彻底治愈的鉴别指标。
在CHB患者中,NAs治疗可以明显降低血清HBV RNA、HBcrAg、HBV DNA、HBsAg、HBeAg及肝组织tDNA、cccDNA水平,并升高anti-HBc定量水平,提示新型血清学标志物与传统标志物均可反映CHB抗病毒治疗过程中HBV DNA复制的变化。但在LC患者中,尽管NAs治疗可以明显降低HBV DNA水平,但其HBV RNA、HBcrAg、anti-HBc定量均无明显变化,表明患者体内低水平的HBV DNA仍可通过其他病毒颗粒如HBV RNA病毒样颗粒或空病毒颗粒促进疾病进展[12-13],故应进一步强调即使HBV DNA降至不可检测水平,但只要仍能检测出HBV RNA,就不能中断抗病毒治疗。另有研究发现,HBcrAg可预测HCC的发生[14-16],而本研究也再次证实LC经抗病毒治疗后HBcrAg没有显著下降,推测这可能是LC患者HCC发生率较高的原因之一。HCC具有更高的HBsAg阴性率及HBeAg阴性率,其机制目前仍不清楚,推测可能由于多数患者已经过渡到免疫控制及再活动期,导致病毒复制减弱。
综上所述,HBcrAg可作为AHB彻底治愈的鉴别指标;若抗病毒治疗后HBV DNA降至不可测,但HBV RNA仍然可测出,则不能停止抗病毒治疗;HBV RNA及HBcrAg或可作为肝组织HBV cccDNA的血清学替代指标。由于本研究为回顾性研究,存在亚组间样本例数不均衡的问题,其结果尚需后续大样本队列研究进一步验证。
