The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is highly infectious and pathogenic.Although it mainly involves the respiratory system, it can also invade the nervous system and lead to a variety of neurological symptoms and diseases, further complicating the clinical conditions of the patients. In order to assist physicians and neurologists in understanding the pathogenesis, clinical features, diagnostic procedures, therapeutic principles, and clinical outcomes of the diseases,the experts of Neurology Branch of Chinese Medical Doctor Association wrote this expert recommendation based on present research articles and clinical practices about the epidemiology, clinical symptoms, diagnostic algorithms, treatment and prognosis of neurological diseases caused by coronavirus disease 2019, in order to provide reference for clinical diagnosis and treatment.

新型冠状病毒(SARS-CoV-2)具有高度传染性及致病性,主要累及呼吸系统,也可侵犯神经系统引发多种神经症状及疾病,进一步导致部分患者症状及病情复杂化。为使广大一线医疗工作者了解此类疾病的发病机制、临床特征、诊治要点及临床转归,中国医师协会神经内科医师分会组织专家依据国内外现有研究报道及临床经验,就疾病流行特点、神经系统损害常见症状、疾病类型、诊断流程、治疗及预后的相关要点等进行讨论并撰写了此推荐意见,以期为临床诊疗工作提供参考。

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is highly infectious and pathogenic.Although it mainly involves the respiratory system, it can also invade the nervous system and lead to a variety of neurological symptoms and diseases, further complicating the clinical conditions of the patients. In order to assist physicians and neurologists in understanding the pathogenesis, clinical features, diagnostic procedures, therapeutic principles, and clinical outcomes of the diseases,the experts of Neurology Branch of Chinese Medical Doctor Association wrote this expert recommendation based on present research articles and clinical practices about the epidemiology, clinical symptoms, diagnostic algorithms, treatment and prognosis of neurological diseases caused by coronavirus disease 2019, in order to provide reference for clinical diagnosis and treatment.

截至2023年2月19日,全球新型冠状病毒肺炎(coronavirus disease 2019,COVID-19)累计确诊病例已超过7.57亿例,死亡人数超680万[1]。临床研究发现,COVID-19可导致或诱发脑血管病、脑炎、癫痫发作及周围神经病等神经系统疾病[2],对此进行早期识别有助于及时治疗并改善预后[3]
中国医师协会神经内科医师分会组织专家根据现有研究报道及临床经验,讨论并总结了本推荐意见,旨在帮助广大急诊、全科、内科及神经科医师加强对COVID-19相关神经系统疾病的识别及处置。
新型冠状病毒(以下简称新冠病毒,SARS-CoV-2)属于β属冠状病毒,包含刺突蛋白(S)、包膜蛋白(E)、膜蛋白(M)及核衣壳蛋白(N)共4种结构蛋白;其中,S蛋白通过结合细胞膜上的血管紧张素转化酶2(angiotensin converting enzyme 2,ACE2)侵入宿主细胞。新冠病毒在流行及传播的过程中基因频繁发生突变,已陆续出现Alpha(B.1.1.7)、Beta(B.1.351)、Gamma(P.1)、Delta(B.1.617.2)及Omicron(B.1.1.529)变异株。Omicron变异株于2021年11月在南非首次检出,随后取代Delta变异株而持续流行演变[4],由于其更强的传染力及传播速度,已经成为现阶段全球的绝对优势流行株。目前,我国监测到的新冠病毒主要是Omicron变异株亚分支BF.7及BA.5.2。与其他变异株相比,Omicron变异株感染后潜伏期明显缩短,中位潜伏期仅为3 d[5]
新冠病毒通过与ACE2结合侵入宿主细胞[6]。除了在人类气道上皮、肺组织细胞、肾脏、小肠、睾丸、肌肉及血管内皮细胞中表达外,ACE2也广泛表达于整个神经系统,在神经元、星形胶质细胞、小胶质细胞及少突胶质细胞等多种细胞表面均有表达,因此神经系统亦为新冠病毒感染的潜在靶标[7]
(1)通过血脑屏障(blood brain barrier,BBB)转运进入中枢神经系统[7-8]。一方面新冠病毒与血管内皮细胞上的ACE2结合,实现病毒的细胞间转移;另一方面新冠病毒与免疫细胞中的ACE2结合,引发免疫反应,免疫细胞携带病毒通过BBB。(2)沿嗅神经/三叉神经通路进入神经系统。新冠病毒可以通过反向轴突转运到神经元,并经神经突触逆行扩散,感染神经系统[9]。(3)通过直接与周围神经轴突、髓鞘及肌肉细胞上的ACE2结合,使病毒包膜与宿主细胞膜偶联,直接感染周围神经及肌肉[10-11](图1)。
(1)炎症免疫反应,包括单核细胞浸润及胶质细胞活化,炎性因子水平升高,介导神经系统及肌肉的炎性损伤[12-13];(2)系统性因素,如COVID-19引发血液高凝状态促进血栓形成,可导致脑血流灌注障碍,以及新冠肺炎导致的低氧血症引起易感脑区(如新皮质、海马及小脑等)的损伤[14]
头痛是COVID-19最常见的神经系统症状,发生率为25%~47%[15-16]。其机制可能与病毒感染后炎性细胞因子释放驱动及三叉神经血管系统激活有关[17]。临床特征包括:(1)中重度持续性疼痛;(2)非甾体类抗炎药物治疗效果欠佳;(3)初始感染症状缓解后头痛可长期持续[18]。有研究发现,感染COVID-19后6个月,仍有8.4%的患者有头痛症状,成为COVID-19神经系统后遗症的主要表现之一[19]。COVID-19急性期头晕的发生率为12%~30%,且女性患者明显高于男性[20],常见症状包括非特异性头晕(44%)、自身不稳感(28%)、晕厥先兆(16%)、眩晕(12%)[21],其中自身不稳感多发生于高龄及有吸烟史的患者。
嗅觉、味觉障碍是COVID-19的常见症状,发生率为40%~65%,远高于其他普通呼吸道病毒感染[22]。嗅觉、味觉障碍常出现在感染后4~5 d,在发病1周后开始好转,在2周内显著改善[23]。对于症状明显的患者,可通过嗅觉训练联合鼻腔冲洗来缓解嗅觉障碍,改善率约为43%;也可以应用左旋肉碱及维生素治疗味觉功能障碍[24]
脑雾指一种“大脑模糊状态”,主要表现为注意力不集中、健忘、疲劳、缺乏动力等[25]。患者感染新冠病毒后,脑雾发生率约为32%,大部分患者的症状可自行缓解,一般不伴有大脑的结构性损伤[26]。认知功能障碍包括记忆、语言、定向、应用、注意力、知觉(视、听、感知)及执行功能障碍等一系列临床症状。COVID-19后认知功能障碍的诊断标准[27]:(1)经实验室检查确认为COVID-19;(2)感染期间或之后出现认知功能减退,持续超过12周,不能用其他疾病解释。认知功能障碍在重症人群中高发[28];中青年患者的发生率约为22%,老年患者约为35%[29]。对于既往已存在认知功能障碍的人群,新发COVID-19可能导致症状加重[30]。痴呆患者在感染早期可出现精神状态改变等不典型COVID-19症状,包括意识混乱、焦虑、迷失方向、食欲不振等,而发热及咳嗽等典型症状往往滞后,易造成诊治延误[31]
COVID-19急性期常出现乏力(72%)、背痛(71%)、肌痛(61%)及关节痛(44%)等症状[32]。即使从急性感染中康复,部分患者仍存在不同程度的不适,其中约22%的患者存在长期的肌痛症状;在重症监护室(ICU)治疗后康复的患者中,一年内出现身体虚弱或疲劳乏力症状的比例高达30%~60%[33]
情绪障碍在COVID-19患者中普遍存在,发生率约为34%[26]。这些症状多出现在急性感染早期,并可持续6个月以上,主要表现为焦虑、抑郁、创伤后应激障碍及失眠等[28-29]。在COVID-19首发症状出现后的90 d内,精神疾病的发生率高达5.8%;对于既往有精神疾病史的人群,精神症状的发生率亦有增高[34]。此外,精神疾病遗传易感性高者更易罹患COVID-19,且进展为重型/危重型的可能性更大;既往有精神疾病史者的精神情绪障碍症状更重;伴精神障碍需要住院治疗者的死亡风险更高[35]
COVID-19可引起热性惊厥、癫痫发作及癫痫持续状态(status epilepticus,SE)[36-37],即使轻症感染者也可能出现这些症状,使得病情加重、治疗棘手,需要临床医师高度重视。目前认为发热、缺氧、炎症风暴及脑血管事件可能与感染后的癫痫发作有关。对于儿童患者,COVID-19所致呼吸系统症状可能轻微,但感染后可能出现复杂性热性惊厥、偏侧惊厥-偏瘫-癫痫、SE及超级难治性SE[38-39]。癫痫患者感染新冠病毒后的死亡风险更高[40-41]。SE多见于既往有癫痫病史的患者,可以表现为惊厥性或非惊厥性SE,还有部分患者出现新发难治性SE[42-43]
COVID-19患者可出现面瘫、膈肌瘫痪及肢体瘫痪等。与贝尔麻痹相比,COVID-19相关面瘫累及双侧的比例更高[44]。膈肌瘫痪少见,主要表现为呼吸困难,影像学显示膈肌抬高,与患者新冠病毒感染恢复后持续存在的呼吸功能障碍有关[45]。感染新冠病毒后中枢神经系统受累所致瘫痪与病变部位有关,偏瘫或交叉瘫常提示大脑半球或脑干病变,截瘫伴尿便障碍常提示脊髓病变。COVID-19导致的单神经病、多发性神经病及广泛的周围神经损害亦有报道,患者表现为与受累神经支配区域相一致的力弱、麻木等[46-47]
COVID-19患者并发缺血性脑卒中的比例为1.6%~4.6%[48-49]。与未感染者相比,COVID-19患者罹患缺血性脑卒中的风险增加3.6倍[50],且具有以下临床特征:(1)发病年龄更低。COVID-19患者卒中发病中位年龄明显低于未感染者(63岁 vs. 70岁)[51]。(2)大血管闭塞型卒中比例高。COVID-19患者大动脉闭塞比例约为未感染者的2倍(31.7% vs. 15.3%)[52]。(3)隐源性卒中多发。COVID-19患者发生隐源性卒中的风险是未感染者的4倍[50]。(4)重症卒中比例更高、预后更差。与未感染者相比,COVID-19感染缺血性卒中患者入院时美国国立卫生研究院卒中量表(National Institute of Health stroke scale,NIHSS)评分、出院时改良Rankin评分(modified Rankin scale,mRS)、住院病死率均显著增高,这可能与大血管闭塞、全身状况差、低氧及合并症增多等因素有关[53-54]
出血性卒中相对少见,约占COVID-19患者总数的0.2%,但此类患者院内死亡风险较高,是未感染者的4.3倍[55-56]。需要注意的是,气管插管、抗凝药物使用、体外膜肺氧合(extracorporeal membrane oxygenation,ECMO)等医源性操作被认为是出血性卒中的危险因素,因此接受上述治疗的患者需密切监测凝血相关指标[57-58]
COVID-19所致脑炎可在病毒感染急性期出现,也可在感染数周后发生,病死率高达13.4%[59]。潜在机制包括:(1)不排除病毒直接侵袭感染中枢神经系统,但缺乏直接的证据;(2)病毒感染后诱发自身免疫性脑炎,如抗N-甲基-D-天冬氨酸(NMDA)受体脑炎、抗富亮氨酸胶质瘤失活因子1(LGI1)抗体相关脑炎、抗接触蛋白相关蛋白2(CASPR2)抗体相关脑炎、抗谷氨酸脱羧酶(GAD65)抗体相关脑炎等[60]。成人及儿童均可发病,成年患者占比约86%,平均发病年龄为55岁,男女比例为2:1[61-62]。COVID-19所致脊髓炎常于病毒感染急性期出现,发病率较低,早期识别并及时启动免疫调节治疗有助于改善患者预后[63]
脑病指一类弥漫性脑功能障碍,通常在短期内(数小时至数天)快速进展,可表现为人格、行为、认知或意识状态的改变。与脑炎不同,脑病患者脑脊液无明确炎性改变。COVID-19相关脑病类型多样,严重的可以表现为急性坏死性脑病(acute necrotizing encephalopathy,ANE)及后部可逆性脑病综合征(posterior reversible encephalopathy syndrome,PRES)[64-65];另外,代谢性脑病的发生多与COVID-19相关低氧血症、电解质紊乱及维生素缺乏有关[66-67]。近来,COVID-19后ANE得到了较多关注,其多于感染症状出现后12~72 h发生并迅速进展,常见于儿童[64]。ANE的临床表现为癫痫发作、意识障碍、认知行为异常。脑脊液分析提示白细胞计数正常而蛋白升高。影像学表现为对称的多灶性病变,丘脑最常受累,脑干、大脑白质、小脑及脊髓亦可累及,增强MRI检查可见环形强化,部分病灶内伴有出血[68]
COVID-19所致中枢神经系统脱髓鞘疾病总体发生比例不高,多于感染后0.5~1个月出现,也有感染急性期发生的报道[69]。可以表现为急性播散性脑脊髓炎(acute disseminated encephalomyelitis,ADEM)、视神经脊髓炎谱系疾病(neuromyelitis optica spectrum disorders,NMOSD)、髓鞘少突胶质细胞糖蛋白抗体相关疾病(myelin oligodendrocyte glycoprotein antibody-associated disease,MOGAD)等[70]。ADEM表现与“经典”表现类似,NMOSD主要表现为视神经炎、脊髓炎及间脑综合征,MOGAD主要的临床表现为头痛、视物模糊、癫痫发作、反应迟钝、肢体麻木无力及脑干综合征等。与免疫功能正常人群相比,既往罹患NMOSD或MOGAD并接受免疫抑制治疗的患者,COVID-19后进展至重症的比例更高[71]
COVID-19所致周围神经病包括吉兰巴雷综合征(Guillain-Barré syndrome,GBS)、多发性神经炎、臂丛神经炎、多发颅神经病变及自主神经功能紊乱等,发病机制可能与病毒感染的炎症反应及随后的自身免疫反应有关,发生率在5%左右[72]
COVID-19所致GBS多发生在感染后2周内,偶见感染3周以后发病的报道[73]。与“典型”GBS相比,COVID-19所致GBS的疾病程度更严重,更多患者需要行机械通气及入住ICU治疗。多数患者存在脑脊液“蛋白-细胞分离”的典型特征,但血清神经节苷脂抗体阳性率较低[74]
COVID-19所致自主神经功能紊乱相对多见,与乏力、心悸、疲劳不耐受等症状有关[75],此外,胃肠道症状、体温调节异常也有报道。多数病例直立倾斜试验结果异常[76]。自主神经功能紊乱可持续6~8个月甚至更久,经治疗后部分缓解[75]。小纤维神经病(small fiber neuropathy,SFN)及多灶性轴索或脱髓鞘性神经病也有报道,常出现于COVID-19后1个月,表现为神经性疼痛,跟腱反射消失,远端针刺觉、振动觉减退,远端肌肉无力及萎缩等症状,以SFN最为多见,多数患者需启动免疫治疗[76]
新冠病毒感染介导的自身免疫反应可诱发重症肌无力(myasthenia gravis,MG),临床表现与“典型”MG一致;还可诱导既往MG的复发或加重[77]。既往有MG病史者,呼吸肌受累、既往长期接受高剂量糖皮质激素、年龄较大、合并肿瘤及接受利妥昔单抗治疗被认为是COVID-19重型/危重型的危险因素[78]
COVID-19所致肌炎临床表现异质性大,可表现为典型皮肌炎、横纹肌溶解,或仅累及椎旁的背部疼痛。临床特征以肌痛、肌肉无力、痉挛及运动不耐受等症状为主,膈肌受累时影响呼吸功能。血清肌酸激酶升高伴发横纹肌溶解患者的肌红蛋白明显升高,从而可引起肾脏功能不全。
COVID-19相关的非特异性神经系统相关症状,如头痛、头晕、乏力、脑雾、肌肉酸痛等,大多症状较轻,无需神经科专科诊治。但当出现以下一个或多个症状时,需及时请神经科专科医师会诊,以便早期发现、早期干预:(1)有明确神经系统疾病史,如脑卒中、痴呆、偏头痛、NMOSD、免疫性脑炎、MG、肌病等,感染后原有症状加重;(2)神经系统症状表现严重,一般对症治疗不缓解,如头痛剧烈伴恶心呕吐,严重肌痛、肢体无力影响行走等;(3)出现无法解释的严重呼吸障碍或意识状态改变;(4)出现明确的急性神经系统功能障碍,如肢体瘫痪、抽搐发作、眩晕发作、精神行为异常等;(5)因基础神经系统疾病,正在服用免疫抑制剂、抗凝药物及抗病毒药物等。
(1)出现突发性平衡障碍、视力模糊、偏瘫、言语不清、面瘫等一个或多个症状,脑卒中可能性较大,应该尽快完善头颅CT检查,迅速评估是否需要行血管再灌注治疗,必要时进行MRI或CT多模式影像评估。(2)出现发作性肢体抽搐,伴或不伴意识障碍,癫痫发作可能性较大,应密切观察;若出现SE,应及时终止发作,并完善脑电图、头颅MRI检查,必要时完善腰椎穿刺检查。(3)出现急性或亚急性发病的意识障碍、癫痫发作、精神症状,脑炎、脑病等可能性较大,应及时完善头颅MRI、腰椎穿刺、脑电图等检查,必要时行脑脊液病原学及相关抗体检查。(4)出现截瘫、传导束性感觉障碍、尿便障碍,脊髓病变可能性大,应及时完善脊髓MRI、腰椎穿刺检查。(5)出现对称性肢体无力,伴或不伴麻木,周围神经损害可能性较大,应及时完善四肢肌电图、神经传导速度、腰椎穿刺检查。(6)出现颅神经支配肌肉或四肢肌肉无力,有病态疲劳、晨轻暮重的特点,MG可能性大,应完善新斯的明试验、神经电生理、乙酰胆碱受体(acetylcholine receptor,AChR)抗体等检查。(7)出现肌痛、肌无力、肌肉压痛,肌炎可能性较大,应及时完善肌酶、肌电图、肌肉影像等检查。
COVID-19的一般治疗策略参考国家现行的新型冠状病毒感染诊疗方案。
对于程度较轻的神经系统相关症状,经对症治疗大多可以缓解。头痛、肌痛的初始治疗药物为对乙酰氨基酚及非甾体抗炎药,当明确存在炎症性肌痛时,糖皮质激素抗炎治疗可能有效;对于COVID-19后的情绪障碍,经心理疏导、适度运动可以缓解,严重者予以抗焦虑抑郁、改善睡眠等药物治疗。精神药物(如5-羟色胺再摄取抑制剂)对缓解乏力症状可能有效。患者情绪状态及失眠的改善有助于缓解脑雾等症状,一般无需给予改善认知的药物治疗。
COVID-19患者的急性卒中管理方法与传统的卒中诊疗指南一致。静脉溶栓及机械取栓术均可使患者获益[54]。对于C反应蛋白、D-二聚体升高及肝功能受损的患者,行溶栓治疗时发生出血的风险可能会增加,机械取栓时需警惕可能存在多血管区域梗死、易再闭塞、易发生血栓碎裂及高血栓负荷等情况[57]。对于脑出血高危人群,如高龄、高血压控制不良、既往脑出血或动脉瘤病史、颅内多发微出血患者,应严密监测其血压、凝血功能等指标,使用抗凝药物时宜谨慎,需评估抗凝治疗与出血的风险获益比,并与患者家属充分沟通。此外,需注意抗病毒药物奈玛特韦/利托那韦不能与利伐沙班、替格瑞洛、氯吡格雷、辛伐他汀等抗凝、稳斑药物联用。
对于COVID-19后首次出现癫痫发作的患者,应该综合考虑是否应用抗癫痫发作药物;若癫痫发作反复发生,应在积极治疗原发病的基础上,短期应用抗癫痫发作药物;对于SE,应尽快终止癫痫发作,具体操作参考相关指南共识[79-80]。对于新冠感染急性期后出现的癫痫发作,应考虑规范应用抗癫痫发作药物治疗。对于既往有癫痫病的患者,若COVID-19诱发癫痫发作的频次或程度加重,应根据个体情况调整药物剂量或种类。
免疫相关药物治疗可能增加某些病原体(包括病毒)感染的风险。当神经免疫性疾病合并COVID-19时,需关注原有免疫调节药物的给药方案,不建议突然停用免疫调节药物;当发生COVID-19所致神经免疫性疾病时,应及时给予相应的免疫调节药物治疗[81]。临床上,免疫调节药物的选择与调整主要依据以下情况而定:COVID-19及神经系统疾病的严重程度、重型/危重型COVID-19的危险因素、药物的药代动力学特点及药物不良反应。同时,需注意免疫调节药物在疾病不同阶段的作用不同,如在COVID-19的初期用药会抑制机体的保护性免疫反应,可能有害;而对于重型/危重型COVID-19患者,则需早期应用,以便尽早减轻炎症反应及细胞因子风暴导致的继发性损害[82]
在治疗COVID-19的基础上,对于神经免疫性疾病较重或快速进展的患者,糖皮质激素及静脉用丙种球蛋白(intravenous immunoglobulin,IVIG)治疗可能是有益的,但需警惕血栓形成的风险[83]。对于严重脑病特别是ANE,应尽早给予甲泼尼龙冲击治疗[儿童:20~30 mg/(kg·d);成人:1000 mg/d],连用3 d,随后根据病情逐渐减量;IVIG总量为2 g/kg,分5 d给予(亦有2 d方案报道),也可酌情选用血浆置换、托珠单抗或改善线粒体代谢的鸡尾酒疗法。
既往接受强效疾病修饰治疗药物(如利妥昔单抗、阿仑单抗等)的患者,由于免疫状态低下更容易发生重型/危重型COVID-19[69],同时更易合并细菌、真菌等感染。对于这类患者,应严密监控并给予相应的预防感染措施,可以考虑给予IVIG或胸腺肽等治疗。
大部分COVID-19患者的神经系统症状及病情比较轻,预后良好,但脑卒中、癫痫发作等可能给患者造成严重损害,因此应加强临床监测及识别,尽量早发现、早处置,避免病情恶化。感染后的长期COVID-19综合征具有包括疲劳、呼吸困难、脑雾、情绪障碍、睡眠障碍、注意力缺陷、肌痛等在内的多种慢性症状[84],已逐渐引起关注,应予重视,进行长期观察、随访及研究。
