Objective To investigate the effect of laryngeal and endotracheal surface anesthesia on cough reflex during extubation after excision of supratentorial tumors, and observe its safety. Methods Forty patients were recruited in present study who underwent supratentorial tumor resection under general anesthesia in the Department of Neurosurgery, Peking University International Hospital from March 2021 to March 2022. The patients were randomly divided into tetracaine group (n=20) and control group (n=20). Patients in tetracaine group were uniformly sprayed 2 ml of 2% tetracaine on the bilateral vocal cords, epiglottis and trachea with an atomized laryngeal anesthetic tube before intubation, and patients in control group received no such treatment Anesthesia management was the same in the both groups. Cough score during extubation, NRS score for throat pain after extubation, NRS score for incision pain, mean arterial pressure (MAP) and heart rate were recorded at 3 min after arterial intubation (T0), before tracheal intubation (T1), 30 s after pulling the cuff after intubation (T2), and when the patient is conscious enough to pull out the tracheal intubation (T3), and the occurrence of hoarseness, swallowing and coughing after extubation. Results Five patients in each group were excluded. The cough score was significantly lower in tetracaine group than in control group (0.4±0.6 vs. 2.6±0.5). Their difference was –2.2 (95%CI –2.5, –1.9), lower than the valid assumed value –1.8. NRS score of throat pain in tetracaine group was significantly lower than control group after extubation [2(0, 2) vs. 4(3, 4.5), P<0.001]. However, no statistically significant difference existed between the two groups in NRS scores of incision pain [3(2, 3) vs. 3(2.5, 3.5), P=0.705]. The MAP was significantly lower in tetracaine group than in control group at T3 [(90.87±13.37) mmHg vs. (102.8±11.52) mmHg, P=0.014]; At T0-T3, there was no significant difference in heart rate between the two groups (P>0.05). No side effects such as hoarseness, swallowing and coughing occurred in both groups after operation. Conclusion Before tracheal intubation in patients undergoing supratentorial tumor resection, laryngeal and endotracheal surface anesthesia with 2% tetracaine could reduce the degree of choking during extubation, relieve pharyngeal pain after extubation, the blood pressure was more stable and no serious adverse reaction was found.

目的 探讨喉及气管内表面麻醉对大脑幕上肿瘤切除术后拔管期呛咳的影响及安全性。方法 选取2021年3月-2022年3月于北京大学国际医院行大脑幕上肿瘤切除术的40例患者作为研究对象,随机分为丁卡因组(n=20)与对照组(n=20)。丁卡因组插管前使用雾化型喉麻管将2 ml 2%丁卡因均匀喷洒于两侧声带、会厌及气管内,对照组不进行此干预,术中两组采取相同的麻醉管理。记录两组拔管时呛咳反应评分、拔管后咽部静息痛NRS评分、手术切口疼痛NRS评分,动脉置管完成3 min时(T0)、置入气管插管前(T1)、插管完成套囊充气后30 s(T2)、患者清醒达拔管条件拔除气管插管时(T3)的平均动脉压(MAP)、心率,以及拔管后声音嘶哑、吞咽呛咳的发生情况。结果 两组各有5例患者脱落。丁卡因组拔管时呛咳反应评分低于对照组[(0.4±0.6)分vs.(2.6±0.5)分],两组差值(95%CI)为–2.2(–2.5,–1.9),低于有效假设数值–1.8。丁卡因组拔管后咽部静息痛NRS评分明显低于对照组[2(0,2)分vs. 4(3,4.5)分,P<0.001],但两组手术切口疼痛NRS评分比较差异无统计学意义[3(2,3)分vs. 3(2.5,3.5)分,P=0.705]。丁卡因组T3时MAP明显低于对照组[(90.87±13.37)mmHg vs.(102.8±11.52)mmHg,P=0.014];T0-T3时,两组心率比较差异无统计学意义(P>0.05)。两组术后均未发生声音嘶哑、吞咽呛咳等不良反应。结论 在幕上肿瘤切除术气管插管前使用2%丁卡因进行喉及气管内表面麻醉,可降低拔管时的呛咳程度,缓解拔管后咽部疼痛,拔管时血压更平稳,且未见严重不良反应。

目的 探讨喉及气管内表面麻醉对大脑幕上肿瘤切除术后拔管期呛咳的影响及安全性。方法 选取2021年3月-2022年3月于北京大学国际医院行大脑幕上肿瘤切除术的40例患者作为研究对象,随机分为丁卡因组(n=20)与对照组(n=20)。丁卡因组插管前使用雾化型喉麻管将2 ml 2%丁卡因均匀喷洒于两侧声带、会厌及气管内,对照组不进行此干预,术中两组采取相同的麻醉管理。记录两组拔管时呛咳反应评分、拔管后咽部静息痛NRS评分、手术切口疼痛NRS评分,动脉置管完成3 min时(T0)、置入气管插管前(T1)、插管完成套囊充气后30 s(T2)、患者清醒达拔管条件拔除气管插管时(T3)的平均动脉压(MAP)、心率,以及拔管后声音嘶哑、吞咽呛咳的发生情况。结果 两组各有5例患者脱落。丁卡因组拔管时呛咳反应评分低于对照组[(0.4±0.6)分vs.(2.6±0.5)分],两组差值(95%CI)为–2.2(–2.5,–1.9),低于有效假设数值–1.8。丁卡因组拔管后咽部静息痛NRS评分明显低于对照组[2(0,2)分vs. 4(3,4.5)分,P<0.001],但两组手术切口疼痛NRS评分比较差异无统计学意义[3(2,3)分vs. 3(2.5,3.5)分,P=0.705]。丁卡因组T3时MAP明显低于对照组[(90.87±13.37)mmHg vs.(102.8±11.52)mmHg,P=0.014];T0-T3时,两组心率比较差异无统计学意义(P>0.05)。两组术后均未发生声音嘶哑、吞咽呛咳等不良反应。结论 在幕上肿瘤切除术气管插管前使用2%丁卡因进行喉及气管内表面麻醉,可降低拔管时的呛咳程度,缓解拔管后咽部疼痛,拔管时血压更平稳,且未见严重不良反应。

Objective To investigate the effect of laryngeal and endotracheal surface anesthesia on cough reflex during extubation after excision of supratentorial tumors, and observe its safety. Methods Forty patients were recruited in present study who underwent supratentorial tumor resection under general anesthesia in the Department of Neurosurgery, Peking University International Hospital from March 2021 to March 2022. The patients were randomly divided into tetracaine group (n=20) and control group (n=20). Patients in tetracaine group were uniformly sprayed 2 ml of 2% tetracaine on the bilateral vocal cords, epiglottis and trachea with an atomized laryngeal anesthetic tube before intubation, and patients in control group received no such treatment Anesthesia management was the same in the both groups. Cough score during extubation, NRS score for throat pain after extubation, NRS score for incision pain, mean arterial pressure (MAP) and heart rate were recorded at 3 min after arterial intubation (T0), before tracheal intubation (T1), 30 s after pulling the cuff after intubation (T2), and when the patient is conscious enough to pull out the tracheal intubation (T3), and the occurrence of hoarseness, swallowing and coughing after extubation. Results Five patients in each group were excluded. The cough score was significantly lower in tetracaine group than in control group (0.4±0.6 vs. 2.6±0.5). Their difference was –2.2 (95%CI –2.5, –1.9), lower than the valid assumed value –1.8. NRS score of throat pain in tetracaine group was significantly lower than control group after extubation [2(0, 2) vs. 4(3, 4.5), P<0.001]. However, no statistically significant difference existed between the two groups in NRS scores of incision pain [3(2, 3) vs. 3(2.5, 3.5), P=0.705]. The MAP was significantly lower in tetracaine group than in control group at T3 [(90.87±13.37) mmHg vs. (102.8±11.52) mmHg, P=0.014]; At T0-T3, there was no significant difference in heart rate between the two groups (P>0.05). No side effects such as hoarseness, swallowing and coughing occurred in both groups after operation. Conclusion Before tracheal intubation in patients undergoing supratentorial tumor resection, laryngeal and endotracheal surface anesthesia with 2% tetracaine could reduce the degree of choking during extubation, relieve pharyngeal pain after extubation, the blood pressure was more stable and no serious adverse reaction was found.

呛咳是气管插管全麻拔管期的常见反应,气管拔管刺激引发的剧烈呛咳会降低患者舒适度,同时易造成心率、血压较大程度的波动,致使颅内压、眼内压升高[1-3]。呛咳时的高张力可能造成颅脑切口部位出血甚至诱发脑疝,尤其对于合并冠心病或其他心脑血管疾病的患者有较大危害[4-5]。气管内表面麻醉已被证实可安全地用于气管插管全身麻醉,能够减轻拔管时的应激反应,且使用成本较低[6-7]。然而,对于全身麻醉下开颅手术患者,目前尚缺乏通过气管插管进行喉及气管内表面麻醉减少拔管期呛咳和血流动力学波动的证据。本研究探讨了喉及气管内表面麻醉对大脑幕上肿瘤切除术后拔管期呛咳的影响及安全性。
选取2021年3月-2022年3月于北京大学国际医院择期行大脑幕上肿瘤切除术的40例患者为研究对象。纳入标准:ASA分级Ⅰ-Ⅱ级;年龄18~65岁;性别不限。排除标准:有严重心肺疾病;严重肝肾功能异常;有恶性心律失常病史;有哮喘或上呼吸道感染;术前可预料的困难气道;对丁卡因或其他酯类局麻药物过敏。剔除标准:术中发生严重心律失常、低血压、失血过多等造成严重不良后果;术后相关数据缺失而造成脱落。应用Randomizer随机数字软件产成随机数字编码,将患者分为丁卡因组(n=20)与对照组(n=20)。本研究经北京大学国际医院伦理委员会批准(2021-KY-0033-01),所有患者均签署知情同意书。
所有患者术前常规禁食6 h,禁清液体2 h。在麻醉准备室由护士建立静脉通路,给予乳酸钠林格液5 ml/kg。入室后常规面罩吸纯氧,新鲜气流量设置为6 L/min,监测心率、无创血压、心电图(electrocardiogram,ECG)、脉搏氧饱和度(saturation of pulse oxygen,SpO2)、脑电双频指数(bispectral index,BIS)等。由外科医师、手术室护士、麻醉医师三方核对患者后静脉滴注甲泼尼龙40 mg、长托宁0.5 mg、咪达唑仑0.03 mg/kg,缓慢滴注舒芬太尼0.1 μg/kg,3 min后进行桡动脉穿刺置管并实时监测动脉压力,待血压、心率稳定后缓慢静脉滴注丙泊酚2 mg/kg、罗库溴铵0.6 mg/kg、舒芬太尼0.3 μg/kg;待患者意识消失后托起面罩进行呼吸机辅助通气,2 min后丁卡因组患者置入可视喉镜暴露会厌及声门,通过喷雾型喉麻管将2 ml 2%丁卡因在气道内,左、右侧声带,以及会厌喉面上均匀喷洒进行表面麻醉,每点0.5 ml,表面麻醉结束后继续面罩通气1 min,然后进行气管插管。对照组面罩通气3 min后进行气管插管。两组患者气管导管与声门下接触的部位均匀涂抹适量利多卡因凝胶。插管完成后妥善固定,调整至合适的呼吸参数进行机械通气。
术中采取容量控制模式通气,呼吸机新鲜气体流量2 L/min,潮气量6~8 ml/kg,呼吸频率12~14次/min,维持呼气末二氧化碳分压在35~45 mmHg。术中以丙泊酚、0.1~0.2 μg/(kg·min)瑞芬太尼复合0.4%七氟烷维持麻醉,调整丙泊酚输注剂量,维持BIS在40~60。缝合颅硬膜时静脉输注舒芬太尼0.2 μg/kg,静脉滴注氟比洛芬酯1 mg/kg、托烷司琼5 mg。手术结束前10 min停止七氟烷吸入,并将新鲜气体流量调整至5 L/h;手术结束时停止泵注丙泊酚、瑞芬太尼。患者轻唤可睁眼,自主呼吸平稳,握手有力,咳嗽、吞咽反射恢复,可完成指令动作时拔除气管导管。拔除气管导管达到离室标准后,将患者护送至病房。
术中出现循环波动后予以记录。当患者心率降低(<50次/min)时可静脉注射阿托品0.5 mg;血压下降(收缩压降低至90 mmHg或平均压低于60 mmHg)时,可静脉注射麻黄碱6 mg。在确认麻醉深度的情况下,若血压升高超过麻醉前20%或血压达160/90 mmHg,且瑞芬太尼单次给药80 μg仍未缓解,可静脉推注乌拉地尔注射液10 mg;若出现心率过快(>110次/min),排除镇静、镇痛与肌松深度不足的情况后,可静脉注射艾司洛尔20 mg。
(1)收集患者一般资料及手术相关基线情况,包括性别、年龄、身高、体重、体重指数(body mass index,BMI)、手术时间、气管插管(带管)时间。(2)主要观察指标为拔管期呛咳反应评分[8]:无呛咳反应记为0分,一次呛咳记为1分,呛咳>一次且持续时间<5 s记为2分,连续呛咳且持续时间≥5 s记为3分。次要观察指标为4个相应时间点患者循环波动情况。记录两组患者动脉置管完成3 min时(T0)、置入气管插管前(T1)、插管完成套囊充气后30 s(T2)、患者清醒达拔管条件拔除气管插管时(T3)的平均动脉压(mean arterial pressure,MAP)、心率,以及拔管后声音嘶哑、吞咽呛咳的发生情况。(3)记录拔管后患者定向力恢复时咽部静息痛数字评分法(numerical rating scale,NRS)评分、手术切口疼痛NRS评分,以及拔管1 min后引流瓶内引流量。
根据预试验结果[丁卡因组呛咳反应评分为(0.2±0.4)分,对照组为(2.6±0.49)分]进行优效性检验计算样本量。检验水准α取单侧0.025,检验效能1-β取0.9,界值选择低优–1.8,两组样本量比例选择1:1,使用PASS软件进行计算,最终n=26,丁卡因组和对照组各13例。本研究预计30%脱落率,因此将样本量扩大至每组20例。本研究中研究对象不知晓分组情况,由两位经过训练且不知晓分组的护士根据手术进度采集各时间节点的相关参数。
采用R软件(4.0.3,www.r-project.org)进行统计分析。主要观察指标采用优效性检验,计算两组差值的95%CI,如95%CI上限低于–1.8,则优效假设成立,否则不成立。计量资料使用Shapiro正态性检验判定样本数据的正态性,如符合正态分布,以$\bar{x}±s$表示,两组间比较采用独立样本t检验;如不符合正态分布,以MQ1Q3)表示,两组间比较采用Wilcox检验。计数资料以例(%)表示,两组间比较采用χ2检验或Fisher's精确检验。双侧P<0.05为差异有统计学意义。
丁卡因组中2例由于术中失血较多予以剔除,3例术后带气管插管进入监护室予以剔除。对照组中1例术中失血较多予以剔除,4例带气管插管进入监护室予以剔除。两组各15例被纳入研究。拔管后两组均未见声音嘶哑及吞咽呛咳发生。丁卡因组中有2例放置术后引流管,对照组4例放置术后引流管,因此引流量数据无法进行统计描述。两组患者性别、年龄、体重、身高、BMI、手术时间、带管时间等比较,差异均无统计学意义(P>0.05,表1)。
丁卡因组患者拔管时呛咳反应评分明显低于对照组[(0.4±0.6)分vs.(2.6±0.5)分],两组差值(95%CI)为–2.2(-2.5,–1.9),两组差值的95%CI上限低于–1.8,因此优效假设成立。丁卡因组患者定向力恢复时咽部静息痛NRS评分明显低于对照组[2(0,2)分vs. 4(3,4.5)分,P<0.001],但两组手术切口疼痛NRS评分差异无统计学意义[3(2,3)分vs.3(2.5,3.5)分,P=0.705]。
T0-T2时,两组MAP比较差异无统计学意义(P>0.05);T3时,丁卡因组MAP明显低于对照组,差异有统计学意义(P=0.014)。T0-T3时,两组心率比较差异无统计学意义(P>0.05)(表2)。
气管插管全麻是临床常用的麻醉方法,然而气管插管对机体是一种强烈的伤害性刺激,拔管期呛咳是气管插管的常见反应[9-10]。因此,减轻拔管应激反应是全麻术后拔管期的关注点之一。Luthra等[11]研究发现,在神经外科手术缝合硬脑膜阶段以0.2 μg/(kg·h)或0.4 μg/(kg·h)的剂量输注右美托咪定,可以有效抑制拔管期呛咳反应并减轻拔管时的血流动力学变化。Zhao等[12]的研究显示,甲状腺手术中使用帕瑞昔布钠复合0.2 μg/(kg·min)瑞芬太尼可有效抑制围拔管期的应激与咳嗽反应。Baftiu等[13]认为,在一定麻醉深度下拔管可减少清醒拔管引起的高血压、呛咳等不良反应,但会导致更高的再插管率。有研究证实,咳嗽可引发颅内压升高,同时呛咳可引起强烈的心血管反应,神经外科手术后发生严重呛咳可能导致手术创面出血,甚至诱发脑疝[14-16]。因此,减轻神经外科开颅患者拔管期应激反应具有重要的临床意义。本研究结果显示,插管前行丁卡因气道表面麻醉能够有效抑制拔管期呛咳反应,减轻拔管期心血管反应,且安全有效。
声门、会厌、气管黏膜处神经分布密集,全麻插管过程中气管导管会对气道黏膜感受器产生强烈的机械性刺激,引发剧烈的神经-体液-内分泌-血流动力学反应[17]。本研究结果显示,丁卡因组患者拔管期呛咳反应评分低于对照组,提示丁卡因表面麻醉有效减轻了气管导管对咽部及气管黏膜的刺激;丁卡因组患者拔管后咽部静息痛NRS评分低于对照组,表明接受丁卡因表面麻醉的患者拔管后具有更高的舒适性。分析其机制可能为丁卡因阻断了会厌、声带以及气管黏膜的电压门控钠离子通道,降低了感觉神经兴奋性,从而减少了伤害性刺激的传入[18]
本研究结果还显示,拔管时丁卡因组MAP明显低于对照组,提示插管前丁卡因表面麻醉可以有效缓解苏醒期拔管引发的血压波动,与魏征等[19]的研究结果一致;丁卡因组T0-T3时的心率低于对照组,但差异无统计学意义,这可能与本研究的样本量较小有关。然而与魏征等[19]的研究相比,本组患者气管带管时间更长,提示丁卡因黏膜表面麻醉效果维持时间较长,这可能与本研究使用更高浓度的丁卡因溶液及手术类型不同有关。
本研究丁卡因组与对照组患者定向力恢复时术后切口疼痛NRS评分无统计学差异,且两组患者均为轻至中度疼痛。韩如泉等[20]的研究显示,开颅手术后疼痛与手术入路相关,其中经蝶入路术后疼痛最为严重,提示可通过合理用药减轻术后切口疼痛,提高患者术后舒适性。强效阿片类药物可安全有效地缓解神经外科术后疼痛[21]。非甾体药物帕瑞昔布钠不影响呼吸与意识,用于开颅手术后的镇痛安全性较高[22]。神经阻滞或手术切口局部浸润麻醉也可有效减轻术后切口疼痛[23]
局麻药物的毒性反应也是临床关注的问题之一。丁卡因属酯类局麻药,脂溶性高、渗透力强,用于黏膜表面麻醉起效迅速,作用时间长[18]。杜威等[24]研究发现,在丁卡因表面麻醉下进行气管插管可有效抑制插管应激反应且安全性高。丁卡因用于成人表面麻醉限量为40 mg,本研究选取2%丁卡因2 ml用于气道表面麻醉,在丁卡因安全剂量内,且两组患者拔管后未见吞咽呛咳及声音嘶哑等不良反应,提示使用丁卡因进行表面麻醉安全性较高。
本研究存在不足之处:由于术后放置引流管的患者过少,未能对两组患者拔管后引流量进行对比,丁卡因表面麻醉能否减少开颅患者术后引流量仍需更大样本量的研究证实。由于条件限制,未能对患者拔管前后血液中儿茶酚胺物质水平进行检测,插管前丁卡因表面麻醉能否抑制拔出气管导管引起的血液儿茶酚胺物质水平增高,仍需进一步探讨。
综上所述,本研究结果表明,在幕上肿瘤切除术患者气管插管前使用2%丁卡因进行喉及气管内表面麻醉,可以降低拔管时的呛咳程度,缓解拔管后咽部疼痛,拔管时血压更平稳,且未见严重不良反应。
