Objective To investigate the correlation between high-density lipoprotein cholesterol (HDL-C) levels and the risk of thyroid carcinoma in the population undergoing surgery for thyroid nodules. Methods The clinical data of 3996 patients who underwent thyroid surgery at the First Medical Center of PLA General Hospital from January 2016 to December 2020 were collected for retrospective analysis, 2846(71.2%) were females and 1150(28.8%) were males, including 3414(85.4%) thyroid carcinoma cases and 582 benign nodules (14.6%). The patients were divided into quintiles groups (Q1-Q5) according to HDL-C level from low to high. The clinical data characteristics of the five groups were compared, and multivariate logistic regression analysis was used to analyze the correlation between HDL-C level and thyroid carcinoma risk. A restricted cubic spline logistic regression model was established to analyze its dose-response relationship. Results As the level of HDL-C climbed from the first to the fifth quintile(Q1-Q5), the proportion of women in the study groups increased, whereas the proportions of thyroid carcinoma, body mass index(BMI), triglyceride levels and fasting blood glucose levels reduced significantly (P<0.05). After adjusting for confounding factors such as age, gender, BMI, thyroid-stimulating hormone (TSH), thyroid peroxidase antibody (TPOAb), and free triiodothyronine/free thyroxine (FT3/FT4) levels, the level of HDL-C remained a significant independent factor for thyroid carcinoma (OR=0.52, 95%CI 0.37-0.72, P<0.001). The trend test revealed statistically significant variations (P for trend <0.001). The E-value analysis indicated E=2.119. Compared with the study population with both TgAb and TPOAb being negative (OR=0.41, 95%CI 0.28-0.60, P<0.001) or positive for either antibody (OR=1.12, 95%CI 0.51-2.46, P=0.777), the correlation between HDL-C and thyroid cancer risk was stronger in patients with both TgAb and TPOAb being negative (P for interaction=0.014). A restricted cubic spline analysis revealed no nonlinear association between HDL-C levels and the risk of thyroid carcinoma in either men or women. Conclusion High HDL-C levels are associated with a lower risk of thyroid carcinoma in the population receiving surgery for thyroid nodules, particularly in patients with negative TgAb and TPOAb.

目的 探讨甲状腺结节手术人群中高密度脂蛋白胆固醇(HDL-C)水平与甲状腺癌发生风险的相关性。方法 收集2016年1月-2020年12月在解放军总医院第一医学中心行甲状腺手术的3996例患者的临床资料进行回顾性分析,其中女2846例(71.2%),男1150例(28.8%),甲状腺癌3414例(85.4%),良性结节582例(14.6%)。按HDL-C水平从低到高等分为五分位数组(Q1-Q5),比较5组患者的临床特征,采用多因素logistic回归分析HDL-C水平与甲状腺癌风险的相关性,建立限制性立方样条logistic回归模型分析其剂量-反应关系。结果 与最低HDL-C五分位数(Q1)的研究人群相比,随着HDL-C水平升高(Q2-Q5),女性的构成比逐渐增高,甲状腺癌的构成比、体重指数(BMI)、三酰甘油水平、空腹血糖水平逐渐降低,差异有统计学意义(P<0.05)。多因素logistic回归分析显示,调整年龄、性别、BMI、促甲状腺素(TSH)、甲状腺球蛋白抗体(TgAb)、甲状腺过氧化物酶抗体(TPOAb)、游离三碘甲状腺原氨酸/游离甲状腺素(FT3/FT4)等相关混杂因素后,HDL-C水平仍是甲状腺癌的独立危险因素(OR=0.52,95%CI 0.37~0.72,P<0.001),且HDL-C从低到高五分位数组趋势性检验差异有统计学意义(P趋势<0.001),敏感性分析显示E值=2.119;亚组分析显示,TgAb和TPOAb均阴性(OR=0.41,95%CI 0.28~0.60,P<0.001)的研究人群与任一抗体阳性(OR=1.12,95%CI 0.51~2.46,P=0.777)的研究人群相比,前者HDL-C与甲状腺癌风险的相关性更明显(P交互=0.014)。限制性立方样条模型分析显示,男性或女性的HDL-C水平与甲状腺癌风险均不存在非线性关系。结论 因甲状腺结节行手术的人群中,高HDL-C水平与甲状腺癌风险降低相关,在TgAb及TPOAb均阴性的患者中该相关性更明显。

目的 探讨甲状腺结节手术人群中高密度脂蛋白胆固醇(HDL-C)水平与甲状腺癌发生风险的相关性。方法 收集2016年1月-2020年12月在解放军总医院第一医学中心行甲状腺手术的3996例患者的临床资料进行回顾性分析,其中女2846例(71.2%),男1150例(28.8%),甲状腺癌3414例(85.4%),良性结节582例(14.6%)。按HDL-C水平从低到高等分为五分位数组(Q1-Q5),比较5组患者的临床特征,采用多因素logistic回归分析HDL-C水平与甲状腺癌风险的相关性,建立限制性立方样条logistic回归模型分析其剂量-反应关系。结果 与最低HDL-C五分位数(Q1)的研究人群相比,随着HDL-C水平升高(Q2-Q5),女性的构成比逐渐增高,甲状腺癌的构成比、体重指数(BMI)、三酰甘油水平、空腹血糖水平逐渐降低,差异有统计学意义(P<0.05)。多因素logistic回归分析显示,调整年龄、性别、BMI、促甲状腺素(TSH)、甲状腺球蛋白抗体(TgAb)、甲状腺过氧化物酶抗体(TPOAb)、游离三碘甲状腺原氨酸/游离甲状腺素(FT3/FT4)等相关混杂因素后,HDL-C水平仍是甲状腺癌的独立危险因素(OR=0.52,95%CI 0.37~0.72,P<0.001),且HDL-C从低到高五分位数组趋势性检验差异有统计学意义(P趋势<0.001),敏感性分析显示E值=2.119;亚组分析显示,TgAb和TPOAb均阴性(OR=0.41,95%CI 0.28~0.60,P<0.001)的研究人群与任一抗体阳性(OR=1.12,95%CI 0.51~2.46,P=0.777)的研究人群相比,前者HDL-C与甲状腺癌风险的相关性更明显(P交互=0.014)。限制性立方样条模型分析显示,男性或女性的HDL-C水平与甲状腺癌风险均不存在非线性关系。结论 因甲状腺结节行手术的人群中,高HDL-C水平与甲状腺癌风险降低相关,在TgAb及TPOAb均阴性的患者中该相关性更明显。

Objective To investigate the correlation between high-density lipoprotein cholesterol (HDL-C) levels and the risk of thyroid carcinoma in the population undergoing surgery for thyroid nodules. Methods The clinical data of 3996 patients who underwent thyroid surgery at the First Medical Center of PLA General Hospital from January 2016 to December 2020 were collected for retrospective analysis, 2846(71.2%) were females and 1150(28.8%) were males, including 3414(85.4%) thyroid carcinoma cases and 582 benign nodules (14.6%). The patients were divided into quintiles groups (Q1-Q5) according to HDL-C level from low to high. The clinical data characteristics of the five groups were compared, and multivariate logistic regression analysis was used to analyze the correlation between HDL-C level and thyroid carcinoma risk. A restricted cubic spline logistic regression model was established to analyze its dose-response relationship. Results As the level of HDL-C climbed from the first to the fifth quintile(Q1-Q5), the proportion of women in the study groups increased, whereas the proportions of thyroid carcinoma, body mass index(BMI), triglyceride levels and fasting blood glucose levels reduced significantly (P<0.05). After adjusting for confounding factors such as age, gender, BMI, thyroid-stimulating hormone (TSH), thyroid peroxidase antibody (TPOAb), and free triiodothyronine/free thyroxine (FT3/FT4) levels, the level of HDL-C remained a significant independent factor for thyroid carcinoma (OR=0.52, 95%CI 0.37-0.72, P<0.001). The trend test revealed statistically significant variations (P for trend <0.001). The E-value analysis indicated E=2.119. Compared with the study population with both TgAb and TPOAb being negative (OR=0.41, 95%CI 0.28-0.60, P<0.001) or positive for either antibody (OR=1.12, 95%CI 0.51-2.46, P=0.777), the correlation between HDL-C and thyroid cancer risk was stronger in patients with both TgAb and TPOAb being negative (P for interaction=0.014). A restricted cubic spline analysis revealed no nonlinear association between HDL-C levels and the risk of thyroid carcinoma in either men or women. Conclusion High HDL-C levels are associated with a lower risk of thyroid carcinoma in the population receiving surgery for thyroid nodules, particularly in patients with negative TgAb and TPOAb.

近年来,甲状腺癌患病率大幅上升[1-2],已成为全球关注的公共卫生问题[3]。高密度脂蛋白胆固醇(HDL-C)被认为是心脑血管疾病的保护因素[4],文献报道其与恶性肿瘤的发生风险降低有关[5-6],尤其是血液系统肿瘤[7]及实体肿瘤(如乳腺癌[8]、肺癌[9])等。目前HDL-C水平与甲状腺癌的相关性研究因纳入排除标准、样本量或研究设计等不同而尚未得出一致性结论[10-12]。本研究通过对因甲状腺结节接受手术治疗患者的病例资料进行回顾性分析,探索HDL-C水平与甲状腺癌的相关性,旨在早期发现甲状腺癌的高危人群。
收集2016年1月-2020年12月在解放军总医院第一医学中心行甲状腺结节手术且术后病理资料完整的3996例患者进行回顾性分析。将研究对象按HDL-C水平从低到高等分为五分位数组(Q1-Q5),其中Q1组HDL-C水平≤0.94 mmol/L(n=799)、Q2组HDL-C水平0.95~1.09 mmol/L(n=743)、Q3组HDL-C水平1.10~1.25 mmol/L(n=849)、Q4组HDL-C水平1.26~1.47 mmol/L(n=800)、Q5组HDL-C水平≥1.48 mmol/L(n=805)。甲状腺癌诊断标准:按照世界卫生组织(WHO)内分泌与神经内分泌肿瘤分类标准(第5版)[13],术后组织病理报告诊断为甲状腺癌。纳入标准:(1)术前甲状腺超声诊断为甲状腺结节;(2)在解放军总医院第一医学中心甲状腺外科接受甲状腺手术;(3)术后病理诊断为甲状腺癌;(4)临床资料可通过电子病历获取。排除标准:(1)血脂资料缺失;(2)有他汀及贝特类药物使用史;(3)既往有甲状腺手术史;(4)有头颈部放射治疗史;(5)年龄<18岁。本研究已获解放军总医院医学伦理委员会审批(伦审第S2022-015-01号)。
通过ICD9 V8.0检索2016年1月-2020年12月在解放军总医院第一医学中心行甲状腺结节手术的住院患者。病理诊断采集自术后组织病理报告单,身高、体重、血压等采集自护理记录单,实验室检查采集自实验室信息管理系统(LIS)术前1周内的最近1次结果,双人录入EpiData v3.1版软件并通过一致性校验。术前HDL-C水平采用均相酶比色法检测(仪器为罗氏Cobas c701)。其他指标包括性别、年龄、体重指数(BMI)及实验室指标包括空腹血糖、促甲状腺素(TSH)、游离甲状腺素(FT4)、游离三碘甲腺原氨酸(FT3)、总甲状腺素(TT4)、总三碘甲腺原氨酸(TT3)、甲状腺球蛋白抗体(TgAb)、甲状腺过氧化物酶抗体(TPOAb)、总胆固醇(TCHO)、三酰甘油、低密度脂蛋白胆固醇(LDL-C)。计算甲状腺激素中枢抵抗指数——甲状腺反馈分位数指数(TFQI),计算公式为:TFQI=cdf(FT4)-[1-cdf (TSH)]。其中cdf为累积分布函数。并计算反映甲状腺激素代谢作用的外周抵抗指数——FT3/FT4[14]
比较不同HDL-C分位数组的临床资料特征;采用多因素logistic回归分析HDL-C水平与甲状腺癌风险的相关性,将单因素分析中P<0.1或具有临床意义的变量逐步纳入logistic回归模型,并排除存在共线性的不同混杂因素,计算关联效应比值比(OR)并进行亚组分析及敏感性分析。
采用R(http://www.R-project.org)软件进行统计分析。正态分布的计量资料以$\bar{x}±s$表示,多组间比较采用单因素方差分析,方差齐时组间两两比较采用Bonferroni法,方差不齐时应用Dunnett T3法;偏态分布资料以M(Q1Q3)表示,多组间比较采用Kruskal-Wallis H检验;计数资料以例(%)表示,组间比较采用χ2检验。采用多因素logistic回归模型计算危险因素比值比(OR),使用E值法对结果进行敏感性分析[15],建立限制性立方样条logistic回归模型分析HDL-C水平与甲状腺癌风险的剂量-反应关系。P<0.05为差异有统计学意义。
2016年1月-2020年12月共6811例患者在解放军总医院第一医学中心行甲状腺结节手术,排除2815例(病理诊断缺失161例,非甲状腺癌9例,既往有甲状腺手术史189例,有头颈部放射治疗史6例,血脂资料缺失2332例,使用他汀或贝特等调脂药物96例,<18岁22例),最终纳入3996例。患者纳入流程图见图1
总体研究人群的年龄为(45.9±11.7)岁;男1150例(28.8%),女2846例(71.2%);甲状腺癌3414例,甲状腺良性结节582例;BMI(24.8±3.6) kg/m2;HDL-C(1.2±0.3) mmol/L(表1)。与最低HDL-C五分位数(Q1)组比较,随着HDL-C水平升高(Q2-Q5组),女性的构成比逐渐增高,甲状腺癌的构成比、BMI、三酰甘油、空腹血糖水平逐渐降低,差异均有统计学意义(P<0.05),而TSH、FT3、FT4、TgAb、TPOAb、TFQI、FT3/FT4差异无统计学意义(表1)。
将单因素分析中P<0.1或具有临床意义的变量纳入logistic回归模型,并逐步排除存在共线性的混杂因素,最终校正年龄、性别、BMI、TSH、总胆固醇、TgAb、TPOAb、FT3/FT4等协变量(模型5)后,HDL-C水平与甲状腺癌风险仍存在独立相关性(OR=0.52,95%CI 0.37~0.72,P<0.001)。按照性别特异性切点(男性:1.0 mmol/L,女性:1.3 mmol/L)将HDL-C转化为二分类变量后,与较低HDL-C水平比较,较高的HDL-C水平与甲状腺癌风险存在独立相关性(OR=0.72,95%CI 0.59~0.88,P=0.001)。与HDL-C最低的五分位数(Q1)组比较,随着HDL-C水平逐渐升高(Q2-Q5组),相关性效应值分别为(OR=0.80,95%CI 0.57~1.12,P=0.195)、(OR=0.73,95%CI 0.53~1.01,P=0.058)、(OR=0.68,95%CI 0.48~0.95,P=0.024)、(OR=0.52,95%CI 0.37~0.74,P<0.001),趋势性检验存在统计学意义(P趋势<0.001)(表2)。
对筛选的混杂因素依据临床意义(年龄、BMI、糖尿病史、TgAb、TPOAb)或中位数(TSH、TFQI、FT3/FT4)的切点进行亚组分析。在不同性别、TSH、TFQI、FT3/FT4,HDL-C与甲状腺癌风险的相关性稳定存在,且亚组间不存在交互作用(P交互>0.05);而在年龄≥55岁、超重和肥胖(BMI 24~28 kg/m2、BMI ≥28 kg/m2)、糖尿病史亚组内HDL-C与甲状腺癌风险无明显相关性(P>0.05),且无明显交互作用(P交互>0.05),但OR<1及较宽的95%CI提示可能为受亚组样本量影响所致(表3)。TgAb和TPOAb均阴性(OR=0.41,95%CI 0.28~0.60,P<0.001)的研究人群与任一抗体阳性(OR=1.12,95%CI 0.51~2.46,P=0.777)的研究人群相比,前者HDL-C与甲状腺癌风险的相关性更明显(P交互=0.014)(图2)。
本研究中可能存在未能收集到重要的影响因素的情况,因此尝试采用E值法对本研究的参数估计进行敏感性分析,结果显示,结局在研究对象中的发生率>15%,$R R=\sqrt{O R}$,得出E值=RR+sqrt[RR×(RR-1)]=2.119,95%CI下限为1.64。
限制性立方样条模型(节点数为4,节点位置为0.05、0.35、0.65、0.95)分析显示,无论男性或女性,血清HDL-C水平与甲状腺癌风险的关联强度均不存在非线性剂量-反应关系(P>0.05,图3)。
近年来多个国家和地区甲状腺癌的发病率有所增高,东亚地区增长尤其迅速,给公共医疗资源带来了极大负担[3]。HDL-C因具有抗氧化、抗炎、抗血管生成、调节免疫与细胞凋亡等作用,一直是癌症领域的研究热点[16-18],尤其是在内分泌系统癌症方面[19]
国内外同类研究由于纳入排除标准、样本量、研究类型及分析方法不同,尚未得出一致性结论,本研究一定程度上提供了有益的临床证据。本研究主要分析了HDL-C水平与甲状腺癌风险的相关性,结果表明,高HDL-C水平与甲状腺癌低风险独立相关。本研究患者虽然来自单中心,但性别构成与全球流行病学研究相近[3],且性别、年龄、血清TSH水平与2011-2015年来自同一医院的甲状腺结节手术人群特征相近[20],但良恶性比例变化较大,这可能与近年来低风险结节的主动监测以及超声引导下细针穿刺活检的广泛应用有关。
Li等[10]纳入2013年11月-2018年11月于北京协和医院就诊的3875例门诊患者,其中,甲状腺癌组女性患者(1255例)的HDL-C水平[1.22(1.06~1.43) mmol/L]明显低于健康对照组女性[1269例,1.32(1.11~1.53) mmol/L],差异有统计学意义(P<0.05),但两组男性HDL-C水平差异无统计学意义。曾幸等[12]对1662例2型糖尿病住院患者中的甲状腺癌与对照组HDL-C水平进行对比分析发现,两组差异无统计学意义,但该研究以细针穿刺活检病理或术后组织病理为诊断标准,且仅针对糖尿病患者这一特殊人群。
一项来自韩国的大型队列研究报道了与本研究类似的结论,该研究纳入2009-2013年参与国民健康保险体检人员3 134 278例,随访至2019年共发生16 129例甲状腺癌,HDL-C水平按性别特异性切点(男1.04 mmol/L,女1.3 mmol/L)划分为高、低水平,在调整年龄、性别、生活方式和代谢性疾病后,年度体检HDL-C低水平次数与甲状腺癌风险上升相关,分别为1次(HR=1.130,95%CI 1.082~1.179)、2次(HR=1.231,95%CI 1.170~1.295)、3次(HR=1.260,95%CI 1.192~1.333)和4次(HR=1.413,95%CI 1.337~1.494),且趋势性检验存在统计学意义(P趋势<0.001)[21]。该研究具有样本量大的优点,但同时也存在多次体检可能导致更多医疗干预从而影响血脂水平的局限性。另一项欧洲的研究针对哥本哈根普通人群(2003-2015年,n=107 341)和哥本哈根城市心脏研究人群(1991-1994年,n=9387)两个队列随访25年,分别观察到8748例和2164例癌症患者,发现随着HDL-C水平下降总体患癌风险增高,但与以甲状腺癌为主的内分泌相关癌症风险无相关性[7]。然而,该研究并未将甲状腺癌作为独立观察终点进行分析,故研究结论尚不能外推至甲状腺癌。
观察性研究中未测量到的或不受控制的混杂因素会对结果造成偏倚,E值代表风险比量度(risk ratio scale)上最小的关联强度,即多大的混杂才能使目前观察到的结果趋于无效。本研究E值=2.119,95%CI下限为1.64,即未测量的混杂因素RR值至少达到1.64,才能完全解释掉本研究结果。
目前,脂代谢与癌症之间的生物学机制仍不明确。当存在与肥胖相关的胰岛素抵抗时,通常会出现脂质分布变化,其特征是HDL-C水平降低[22],三酰甘油水平升高,而本研究也发现HDL-C与BMI呈负相关。另有研究发现,糖尿病患者发生甲状腺癌的风险增加[23],这可能与胰岛素抵抗导致的血脂变化有关。因此,胰岛素抵抗可能是HDL-C与甲状腺癌关联的因素之一。当机体处于正能量状态时,适应性改变表现为甲状腺激素水平下降和TSH水平上升,以免产热增加,这在Framingham子代研究中已得到证实,体重增加与生理范围内的TSH水平呈正相关[24]。TPOAb阳性是代谢综合征的危险因素,且与低HDL-C水平有关[25],自身免疫性甲状腺疾病可能导致TSH水平上升,而TSH水平升高与高脂血症有关[26-27]。TSH在甲状腺细胞的生长及分化中具有重要作用,甲状腺癌患者的TSH水平高于良性甲状腺疾病者[28],故TSH可能是HDL-C与甲状腺癌关联的另一个因素。
从分子机制的角度看,HDL的抗氧化特性是通过相关蛋白和酶而发挥作用的:①HDL相关蛋白能限制氧化低密度脂蛋白的形成[29-30],如载脂蛋白A1可结合并去除氧化低密度脂蛋白中的脂质过氧化氢[31],载脂蛋白E2[32]、载脂蛋白A4和载脂蛋白J也有助于HDL的抗氧化作用;②HDL相关酶如对氧磷酶(PON)-1、卵磷脂胆固醇酰基转移酶和血小板活化因子乙酰水解酶同样具有抗氧化作用。HDL可在细胞水平抑制促炎细胞因子的产生,上调内皮细胞和白细胞黏附分子的表达,而在分子水平可影响核因子kappa-B(NF-κB)调节的一系列促炎通路。HDL抗细胞凋亡的作用同样基于相关蛋白和酶,通过作用于凋亡途径的关键因子caspase 3[33],或诱导抗凋亡的Bcl-2蛋白Bcl-XL表达上调[34],从而抑制内皮细胞的凋亡。此外,载脂蛋白M和1-磷酸腺苷有助于增强HDL的抗凋亡活性[35],鞘磷脂和卵磷脂可能参与了HDL对细胞凋亡的调节作用[36]。HDL的其他功能还包括抑制血栓形成,调节内皮型一氧化氮合酶的定位及表达,调节脂肪组织功能及能量消耗等[37]
综上所述,本研究发现在因甲状腺结节行手术治疗的患者中,高HDL-C水平与甲状腺癌风险降低独立相关,在TgAb及TPOAb阴性的患者中这一相关性更为明显。但本研究为单中心横断面研究,且研究对象为因甲状腺结节行手术的人群,存在一定的局限性。未来仍需纳入更多患者开展多中心、前瞻性研究,以进一步明确HDL-C水平与甲状腺癌风险的相关性。
