Objective To investigate the predictive value of serum C-reactive protein/albumin ratio (CAR) with other inflammatory parameters for in-hospital adverse events in patients with acute ST segment elevation myocardial infarction (STEMI) undergoing emergency percutaneous coronary intervention (PCI). Methods A total of 340 patients with STEMI who underwent emergency PCI in 904 Hospital of PLA Joint Logistics Support Force from January 2016 to December 2020 were consecutively included in this retrospective analysis. Patients with STEMI were divided into two groups according to the presence or absence of adverse cardiovascular events during their hospital stay after emergent PCI: patients with major adverse cardiovascular events (MACEs, n=92) and non-MACEs (n=248). Serum C-reactive protein (CRP), white blood cells, neutrophils, lymphocytes, and other indicators were measured in both groups, and inflammatory parameters such as CAR, neutrophil/lymphocyte ratio (NLR), and platelet/lymphocyte ratio (PLR) were calculated. Univariate and multivariate logistic regression analyses were used to screen the independent risk factors correlated with the occurrence of MACEs during the hospitalization after PCI in STEMI patients. Correlations between each inflammatory parameter and MACEs were analyzed by Spearman correlation analysis. Receiver operating characteristic (ROC) curve to evaluate the efficacy size of each inflammatory parameter in predicting the occurrence of MACEs during PCI hospitalization in patients with STEMI. Results Comparison of general data between the two groups revealed that patients in MACEs group had a higher frequency of age, history of diabetes mellitus, polydactyly, and serum composite inflammatory parameters (CAR, NLR, PLR, MLR, and SII) than those in non-MACEs group, the univariate and multivariate results showed that CAR was a risk factor for the occurrence of MACEs during the hospitalization after PCI in patients with STEMI. Spearman correlation analysis showed that CAR was significantly correlated with the occurrence of MACEs during hospitalization, such as cardiogenic shock (r=0.134, P<0.05), heart failure (r=0.436, P<0.05), and all-cause death (r=0.185, P<0.05). The results of ROC curves showed that CAR (AUC=0.751, 95%CI 0.691-0.811, P<0.001) had better predictive value for the occurrence of MACEs after PCI than NLR (AUC=0.643, 95%CI 0.576-0.711, P<0.001), PLR (AUC=0.598, 95%CI 0.530-0.665, P=0.006) and other inflammatory parameters. Conclusion Compared with traditional inflammatory parameters, CAR has better predictive efficacy for the occurrence of MACEs during hospitalization in STEMI patients undergoing emergency PCI, and can better guide the clinic.

目的 探讨血清C反应蛋白/白蛋白比值(CAR)与其他炎症参数对急性ST段抬高型心肌梗死(STEMI)患者行急诊经皮冠状动脉介入治疗(PCI)术后院内不良事件的预测价值。方法 收集2016年1月-2020年12月在解放军联勤保障部队第904医院行急诊PCI的340例STEMI患者的临床资料进行回顾性分析,根据急诊PCI术后住院期间有无主要不良心血管事件(MACEs)发生将患者分为MACEs组(n=92)与无MACEs组(n=248)。检测两组患者的血清C反应蛋白(CRP)、白细胞、中性粒细胞、淋巴细胞等指标,并计算CAR、中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)等炎症参数。采用单因素和多因素logistic回归分析筛选STEMI患者急诊PCI术后住院期间发生MACEs的独立危险因素,Spearman检验分析各炎症参数与MACEs的相关性,受试者工作特征(ROC)曲线评估各炎症参数预测STEMI患者急诊PCI术后住院期间发生MACEs的效能。结果 MACEs组患者年龄、糖尿病史、病变血管数、血清复合炎症参数(CAR、NLR、PLR、MLR、SII等)均高于无MACEs组。单因素和多因素logistic回归分析结果显示,CAR是STEMI患者急诊PCI术后住院期间发生MACEs的危险因素。Spearman相关分析显示,CAR与心源性休克、心力衰竭、全因死亡等住院期间MACEs呈明显正相关(r分别为0.134、0.436、0.185,P<0.05)。ROC曲线分析结果显示,CAR(AUC=0.751,95%CI 0.691~0.811,P<0.001)对急诊PCI术后MACEs的预测价值优于NLR(AUC=0.643,95%CI 0.576~0.711,P<0.001)、PLR(AUC=0.598,95%CI 0.530~0.665,P=0.006)等炎症参数。结论 与传统炎症参数相比,CAR对STEMI患者急诊PCI术后住院期间MACEs的预测效能更优,可更好地指导临床。

目的 探讨血清C反应蛋白/白蛋白比值(CAR)与其他炎症参数对急性ST段抬高型心肌梗死(STEMI)患者行急诊经皮冠状动脉介入治疗(PCI)术后院内不良事件的预测价值。方法 收集2016年1月-2020年12月在解放军联勤保障部队第904医院行急诊PCI的340例STEMI患者的临床资料进行回顾性分析,根据急诊PCI术后住院期间有无主要不良心血管事件(MACEs)发生将患者分为MACEs组(n=92)与无MACEs组(n=248)。检测两组患者的血清C反应蛋白(CRP)、白细胞、中性粒细胞、淋巴细胞等指标,并计算CAR、中性粒细胞/淋巴细胞比值(NLR)、血小板/淋巴细胞比值(PLR)等炎症参数。采用单因素和多因素logistic回归分析筛选STEMI患者急诊PCI术后住院期间发生MACEs的独立危险因素,Spearman检验分析各炎症参数与MACEs的相关性,受试者工作特征(ROC)曲线评估各炎症参数预测STEMI患者急诊PCI术后住院期间发生MACEs的效能。结果 MACEs组患者年龄、糖尿病史、病变血管数、血清复合炎症参数(CAR、NLR、PLR、MLR、SII等)均高于无MACEs组。单因素和多因素logistic回归分析结果显示,CAR是STEMI患者急诊PCI术后住院期间发生MACEs的危险因素。Spearman相关分析显示,CAR与心源性休克、心力衰竭、全因死亡等住院期间MACEs呈明显正相关(r分别为0.134、0.436、0.185,P<0.05)。ROC曲线分析结果显示,CAR(AUC=0.751,95%CI 0.691~0.811,P<0.001)对急诊PCI术后MACEs的预测价值优于NLR(AUC=0.643,95%CI 0.576~0.711,P<0.001)、PLR(AUC=0.598,95%CI 0.530~0.665,P=0.006)等炎症参数。结论 与传统炎症参数相比,CAR对STEMI患者急诊PCI术后住院期间MACEs的预测效能更优,可更好地指导临床。

Objective To investigate the predictive value of serum C-reactive protein/albumin ratio (CAR) with other inflammatory parameters for in-hospital adverse events in patients with acute ST segment elevation myocardial infarction (STEMI) undergoing emergency percutaneous coronary intervention (PCI). Methods A total of 340 patients with STEMI who underwent emergency PCI in 904 Hospital of PLA Joint Logistics Support Force from January 2016 to December 2020 were consecutively included in this retrospective analysis. Patients with STEMI were divided into two groups according to the presence or absence of adverse cardiovascular events during their hospital stay after emergent PCI: patients with major adverse cardiovascular events (MACEs, n=92) and non-MACEs (n=248). Serum C-reactive protein (CRP), white blood cells, neutrophils, lymphocytes, and other indicators were measured in both groups, and inflammatory parameters such as CAR, neutrophil/lymphocyte ratio (NLR), and platelet/lymphocyte ratio (PLR) were calculated. Univariate and multivariate logistic regression analyses were used to screen the independent risk factors correlated with the occurrence of MACEs during the hospitalization after PCI in STEMI patients. Correlations between each inflammatory parameter and MACEs were analyzed by Spearman correlation analysis. Receiver operating characteristic (ROC) curve to evaluate the efficacy size of each inflammatory parameter in predicting the occurrence of MACEs during PCI hospitalization in patients with STEMI. Results Comparison of general data between the two groups revealed that patients in MACEs group had a higher frequency of age, history of diabetes mellitus, polydactyly, and serum composite inflammatory parameters (CAR, NLR, PLR, MLR, and SII) than those in non-MACEs group, the univariate and multivariate results showed that CAR was a risk factor for the occurrence of MACEs during the hospitalization after PCI in patients with STEMI. Spearman correlation analysis showed that CAR was significantly correlated with the occurrence of MACEs during hospitalization, such as cardiogenic shock (r=0.134, P<0.05), heart failure (r=0.436, P<0.05), and all-cause death (r=0.185, P<0.05). The results of ROC curves showed that CAR (AUC=0.751, 95%CI 0.691-0.811, P<0.001) had better predictive value for the occurrence of MACEs after PCI than NLR (AUC=0.643, 95%CI 0.576-0.711, P<0.001), PLR (AUC=0.598, 95%CI 0.530-0.665, P=0.006) and other inflammatory parameters. Conclusion Compared with traditional inflammatory parameters, CAR has better predictive efficacy for the occurrence of MACEs during hospitalization in STEMI patients undergoing emergency PCI, and can better guide the clinic.

急性ST段抬高型心肌梗死(ST-segment elevation myocardial infarction,STEMI)是临床上常见的急危重症之一,2018年中国新发患者多达18万余例,其中45.9%急诊接受经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)[1-2]。直接PCI是心肌梗死最有效的急诊治疗方法之一,其技术不断成熟,使心肌梗死的院内病死率逐年下降,但术后仍有6.35%~19.05%的患者发生主要不良心血管事件(major adverse cardiovascular events,MACEs)[3-4]。有观点认为,动脉粥样斑块的炎症反应可能是术后发生不良事件的主要原因之一[5],且组织病理学和免疫化学观察结果显示,活动的炎症过程可能触发斑块破裂,明显增加临床发生动脉血栓事件的风险[6]。复合炎症参数较单一炎症参数具有更好的代表性及稳定性,其对心肌梗死后不良事件的预测价值近年来已有研究,如中性粒细胞/淋巴细胞比值(NE/LY ratio,NLR)、血小板/淋巴细胞比值(PLT/LY ratio,PLR)、系统免疫炎症指数(systemic immune-inflammation index,SII,中性粒细胞×血小板/淋巴细胞)、C反应蛋白/白蛋白比值(C-reactive protein/albumin ratio,CRP/ALB,CAR)等被证实与心肌梗死的预后密切相关[7-15]。此外,基于患者入院时血液常规及生化常规检查结果计算的复合炎症参数应用于心肌梗死不良预后的评估具有快速、经济等优点,因此可很好地服务于临床,改善患者的预后。但不同复合炎症参数对心肌梗死患者PCI术后发生MACEs的评估价值有无差异,临床上未见相关报道。为此,本研究对上述复合炎症参数预测STEMI患者急诊PCI术后住院期间发生MACEs的效能进行比较,以期进一步改善患者的预后,更好地指导临床。
收集2016年1月-2020年12月于解放军联勤保障部队第904医院确诊为STEMI且行急诊PCI术的545例患者的临床资料,最终纳入340例进行回顾性分析,其中男239例,女101例,中位年龄59(57,62)岁。STEMI的诊断参考2022年《综述:急性冠脉综合征的诊断与治疗》[16]指南推荐,符合以下条件可明确诊断:(1)持续时间>10 min的典型胸痛症状;(2)急诊入院时心电图在无束支传导阻滞和左心室肥厚的情况下出现新发和持续的连续两个及以上导联的ST段抬高≥1 mm或V2和V3导联ST段抬高(女性患者≥1.5 mm,<40岁的男性患者≥2.5 mm,≥40岁的男性患者≥2.0 mm);(3)对于部分急诊入院时心电图无上述特异性改变的患者,应短时间内重复采集心电图,以降低STEMI漏诊率。纳入标准:(1)年龄18~80岁,因胸痛入院确诊为STEMI;(2)神志清楚,能自然交流沟通,无严重神经及精神类疾病。所有受试者入院后均规范服用阿司匹林、替格瑞洛/氯吡格雷双抗治疗。排除标准:(1)近3个月有大手术史、外伤史或出血史;(2)既往有陈旧性心肌梗死、脑梗死、PCI、冠状动脉旁路移植术病史;(3)合并自身免疫性疾病、活动性感染、恶性肿瘤或严重肝肾功能不全;(4)正在接受抗凝、抗血小板治疗;(5)临床资料不完整。根据患者住院期间是否发生MACEs(包括心源性休克、心力衰竭、再发心肌梗死/心绞痛、恶性心律失常及死亡)分为MACEs组(n=92)与无MACEs组(n=248)。本研究获解放军联勤保障部队第904医院伦理委员会审批(20220804),所有患者均签署知情同意书。
收集所有患者的临床资料,包括年龄、性别及高血压、糖尿病、吸烟史等,术前急诊采集肘静脉血检测血常规(包括血红蛋白、PLT、WBC、NE、LY、MONO等)、CRP、ALB、血肌酐(Scr)、尿酸(UA)、N末端脑钠肽前体(NT-proBNP)、肌钙蛋白I(TnI)等;空腹采集肘静脉血以全自动生化分析仪(美国Beckman公司)检测总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C),并计算CAR、SII、NLR、PLR、单核细胞/淋巴细胞比值(MONO/LY,MLR)、单核细胞/高密度脂蛋白比值(MONO/HDL-C,MHR)及估算肾小球滤过率(eGFR)。
对比分析两组患者的一般临床特征、冠状动脉造影结果、生化指标、血常规指标及复合炎症参数等基线资料,将有差异的参数纳入单因素logistic回归分析。对单因素logistic回归分析中有统计学意义(P<0.05)的参数进行共线性诊断,排除共线性指标后进行多因素logistic回归分析,得出STEMI患者住院期间发生MACEs的独立危险因素;进一步分析各独立危险因素与不良心血管事件之间的相关性。最后比较各独立危险因素对STEMI患者急诊PCI术后住院期间发生MACEs的预测效能。
采用SPSS 20.0软件进行统计分析。计量资料符合正态分布者以$\bar{x}±s$表示,两组间比较采用完全随机设计的独立样本t检验;不符合正态分布者以M(Q1Q3)表示,两组间比较采用两样本Mann-Whitney U检验。计数资料以例(%)表示,两组间比较采用χ2检验。以P<0.1为标准将一般资料中连续性变量进行二分类后纳入单因素及多因素logistic回归分析,采用Spearman相关分析评估炎症参数和PCI术后不良事件的相关性。采用MedCalc15.2.2统计软件绘制受试者工作特征(ROC)曲线,应用Z检验比较各炎症参数对急诊PCI术后不良事件的诊断效能。P<0.05为差异有统计学意义。
与无MACEs组比较,MACEs组患者年龄大,糖尿病及多支血管病变的比例高,TIMI血流分级及eGFR、LY、ALB水平较低,CRP、CAR、NLR、PLR、MLR、SII、WBC、NE、UA、TnI、NT-proBNP水平较高,差异均有统计学意义(P<0.05)(表1)。
将基线资料中有差异的参数(P<0.1)纳入单因素logistic回归分析,结果显示,年龄、病变血管数、糖尿病、WBC、LY、UA、CRP、CAR、NLR、PLR、SII是急诊PCI术后不良事件发生的危险因素(P<0.05,表2)。
将单因素logistic回归分析中有统计学意义(P<0.05)的炎症指标纳入多因素logistic回归分析。共线性分析提示,CRP、CAR两者方差膨胀因子>10,容忍度<0.1,考虑本研究主要探索CAR,故去除CRP后再次行共线性诊断,结果提示各参数方差膨胀因子均<10,容忍度>0.1。将WBC、LY、UA、CAR、NLR、PLR、SII纳入多因素logistic回归分析(模型1),结果显示,CAR(OR=4.016,95%CI 2.311~6.981,P<0.05)是急诊PCI术后不良事件的独立危险因素。随后校准了患者的年龄、高血压、性别、糖尿病、吸烟等参数构建模型2,结果显示,CAR(OR=4.081,95%CI 2.274~7.324,P<0.05)、WBC(OR=3.494,95%CI 1.252~9.748,P<0.05)、UA(OR=1.913,95%CI 1.068~3.429,P<0.05)、NLR(OR=2.628,95%CI 1.031~6.704,P<0.05)是急诊PCI术后不良事件的独立危险因素。随后进一步校准了血管病变情况(靶血管、病变血管数、血栓抽吸、缺血-再灌注时间)及手术操作(模型3)后,结果显示,CAR(OR=3.530,95%CI 1.932~6.450,P<0.05)、WBC(OR=3.114,95%CI 1.065~9.106,P<0.05)、UA(OR=1.936,95%CI 1.064~3.522,P<0.05)依然是急诊PCI术后不良事件的独立危险因素,且CAR的相关性最强(表3)。
Spearman相关分析结果显示,WBC、NE、UA、CRP、CAR、NLR、PLR、MLR及SII与急诊PCI术后复合MACEs呈正相关(P<0.05),而LY与其呈负相关(P<0.05),其中CAR的相关系数最高,为0.386。具体不良事件中,CAR与心源性休克、心力衰竭、死亡呈正相关(P<0.05),而与再发心肌梗死/心绞痛及恶性心律失常无相关性(P>0.05)(表4)。
采用ROC曲线评估CAR、WBC、NE、UA、CRP、CAR、NLR、PLR、MLR、SII对急诊PCI术后不良事件的预测价值(图1表5),并对曲线下面积(AUC)进行比较。结果显示,当CAR取值为0.3567时,对急诊PCI术后不良事件的预测效能最高,敏感度为62.0%,特异度为77.8%,AUC=0.751(95%CI 0.691~0.811,P<0.001)。对所有炎症参数的AUC进行两两比较发现,CAR诊断效能与CRP相当,均高于临床常用的SII、NLR、PLR、MLR等复合炎症参数,差异有统计学意义(P<0.01)(表6)。
STEMI是冠状动脉疾病最严重的一种表现形式。在全球范围内,STEMI具有较高的发病率和病死率。目前,虽然以急诊PCI为主的再灌注治疗在很大程度上降低了STEMI患者的病死率,但是其住院期间及远期心血管不良事件发生率仍较高,严重影响患者生活质量,增加了患者的经济负担[17]。通过简易方法获得的预测不良事件发生的可靠生物标志物将有助于辅助临床医师识别高危患者,进行早期干预,最终做到改善STEMI患者的临床预后。
本研究的主要目的在于探讨不同复合炎症参数对STEMI患者急诊PCI术后住院期间不良事件预测效能的大小,以期更好地服务于临床。结果显示,与WBC、PLT、NLR、PLR、MLR、SII、UA等炎症参数相比,CAR是STEMI患者急诊PCI术后发生院内MACEs的独立预测因子,且预测效能优于其他传统的炎症参数。
既往研究发现,炎症反应能够促进动脉粥样硬化的发生和进展,且炎症活跃状态可诱导斑块破裂而发生临床缺血事件[18-19]。目前,CAR、NLR、PLR、MLR、SII等多种炎症参数已经被广泛应用于冠心病的诊断、血管病变严重程度及冠心病患者预后的评估[11,20],本研究系统性地对上述炎症参数进行比较发现,CAR是STEMI患者急诊PCI术后发生院内MACEs的最强预测因子,其可能的机制如下:(1)冠脉急性闭塞,部分原因是冠脉粥样斑块破裂、斑块侵蚀等引发的一系列炎症细胞参与的级联反应。CRP作为一种急性期蛋白,在炎症反应初期迅速上升,对机体炎症状态具有较高的敏感性,其血清水平可反映急性期炎症反应的强弱及心肌梗死的严重程度,从而可作为心肌梗死患者不良预后的预测指标。(2)白蛋白是人体血浆中的重要蛋白之一,可反映肝功能、一般营养状况等,且在炎症反应急性期被消耗,使其血浆水平下降。在心肌梗死任何时期出现的低白蛋白血症均为患者远期结局不良的预测指标[21-23]。(3)CRP可通过诱导血管紧张素受体的表达、活性氧的产生和释放而促进新生内膜中的巨噬细胞和泡沫细胞释放肿瘤坏死因子-α、白细胞介素(IL)-1β和IL-6,进而导致白细胞、中性粒细胞等炎性细胞增多,最终促进动脉粥样硬化的发生发展[24-26]。综上,越来越多的研究将CRP与白蛋白联合应用于心血管疾病的诊断与预后评估。其中,Aksu等[27]发现,CAR可预测STEMI患者支架内再狭窄的发生;Tanriverdi等[20]发现,CAR可预测稳定性冠心病中血管病变的数量及严重程度。本研究发现,CAR对STEMI患者急诊PCI术后院内不良事件具有良好的预测价值。由于临床多种因素可对STEMI患者急诊PCI术后不良事件产生影响,因此本研究采取分级建模的方法检测CAR的评估价值。模型2校准了年龄、性别、高血压、糖尿病、吸烟等对STEMI患者急诊PCI术后预后有明显影响的临床因素[28-30],结果显示,CAR、WBC、NLR、UA仍是急诊PCI术后不良事件的独立危险因素,且CAR的相关性最强。然后,本研究进一步校准了冠脉病变及手术操作因素(靶病变、病变血管数、缺血-再灌注时间、血栓抽吸等)得到模型3,结果显示CAR依然是急诊PCI术后院内不良事件的最强预测因子,体现了CAR预测PCI术后不良事件的稳定性与独立性。此外,本研究通过ROC曲线分析发现,CAR对STEMI患者急诊PCI术后住院期间MACEs的预测价值高于其他常用的炎症参数,且相较于胆固醇等指标,CAR具有可急诊快速获得等优点,更有利于临床应用及决策。
本研究发现,在ROC曲线分析中CAR及CRP的AUC值分别为0.751和0.750,两者对STEMI患者急诊PCI术后住院期间MACEs的预测效能无差异。推测可能有以下两方面的原因:一方面,CAR是CRP与白蛋白表达水平的比值,该指标从全身炎症反应状态及机体营养状况来预测患者住院期间不良心血管事件发生的可能性大小,相较于CRP,CAR综合了多种影响STEMI患者预后的因素,在预后评估方面具有更好的稳定性和综合性;另一方面,本研究是一项小样本回顾性研究,CRP与CAR预测STEMI患者住院期间MACEs的效能差异不大可能是受样本量的局限所致。
综上所述,本研究发现,CAR是STEMI患者急诊PCI术后院内MACEs的独立危险因素,且与患者发生心源性休克、心力衰竭、死亡呈正相关,在预测住院期间不良事件的发生方面优于其他传统炎症参数。但本研究仍存在不足之处:(1)本研究的血液参数均为单次采样,未进行动态监测;(2)只评估了急诊PCI术后住院期间MACEs的发生,未对出院后的长期预后进行评价;(3)样本量相对较小。因此,未来仍需通过多中心、大样本的前瞻性研究来探讨CAR是否为STEMI患者急诊PCI术后住院期间及远期发生MACEs的最强独立预测因子,并在下一步工作中对STEMI患者入院后不同时间节点进行多次采血,探讨CAR的动态变化对STEMI患者急诊PCI术后住院期间及远期MACEs的预测较单次采样是否有增量价值。
