Objective To investigate the effects and mechnism of abnormal stress promoting macrophage mobility inhibitory factor (MIF), cyclooxygenase 2 (COX2) and prostaglandin E2 (PGE2) in the progression of temporomandibular joint osteoarthritis (TMJOA). Methods From January 2020 to December 2021, TMJOA and temporomandibular joint internal derangement (TMJID) patients (30 cases in each group, we divided the TMJOA into group TMJ Ⅰ, Ⅱ, Ⅲ according to the stage) who were admitted to TMJOA special clinic of the First Affiliated Hospital of Xinjiang Medical University and accompanied by abnormal occlusion were collected. The pain score of the occlusal state of the patients was evaluated by visual analogue scale. The expression levels of MIF, COX2 and PGE2 in synovial fluid were detected by ELISA. We used the unilateral anterior crossbite for TMJOA (UAC) rats model (the grouped into: UAC-4 weeks, UAC-8 weeks and UAC-12 weeks group), and control group at the same time (grouped into: Ctrl-4 weeks, Ctrl-8 weeks and Ctrl-12 weeks group), each group had 6 rats. The expression levels of MIF, COX2 and PGE2 in serum and synovial fluid of rats were detected by ELISA. The expression levels of IL-1β, IL-18, MIF, COX2 and PTGER2 in temporomandibular joint of rats were detected by Western blotting. The fluid flow shear stress (FFSS) model of fibroblast-like synovial cells (FLSs) was established, and the mRNA and protein expression levels of above indexes were detected by RT-PCR and Western blotting. Results Visual analogue scale evaluation showed that the pain score of TMJOA Ⅰ and Ⅱ group was significantly higher than that of TMJID (P<0.001). ELISA results showed that the expression levels of MIF, COX2 and PGE2 in synovial fluid in TMJOA group were higher than those in TMJID group (P<0.05), and the expression levels were the highest in TMJOA Ⅱ group. Compared with control group, the expressions of MIF, COX2 and PGE2 in serum and synovial fluid at UAC-4 weeks, 8 weeks and 12 weeks were slightly higher, and significantly higher at UAC-8 weeks in rat TMJOA model (P<0.05). In addition, the expression trend of protein levels in temporomandibular joint tissues was similar, which showed higher expression levels of IL-1β, IL-18, MIF, COX2 and PTGER2 (P<0.05). In the cell model where FFSS interfered with FLSs, with the increase of FFSS, cell with deformation, incomplete cell membrane and reduced number. Compared with control group, the expression levels of IL-1β, IL-18, MIF, COX2 and PGE2 (PTGER2) of FLSs were increased in 1, 3, 5 and 10 dyn/cm2 intervention groups (P<0.05). Conclusion MIF, COX2 and PGE2 were highly expressed in temporomandibular joint synovial fluid of TMJOA patients with malocclusion. And these three factors were also highly expressed in serum and synovial fluid of UAC rats. The abnormal fluid shear stress promotes the secretion of MIF, COX2 and PGE2 by FLSs to participate in joint microenvironment inflammation and accelerate disease progression.

目的 探讨异常应力对巨噬细胞移动抑制因子(MIF)、环氧合酶2(COX2)和前列腺素E2(PGE2)在颞下颌关节骨关节炎(TMJOA)中的促进作用及其机制。方法 收集2020年1月-2021年12月就诊于新疆医科大学第一附属医院颞下颌关节专病门诊并伴有错𬌗的30例TMJOA患者(TMJOA组,根据分期分为TMJOA Ⅰ期、Ⅱ期、Ⅲ期,n=10)及30例颞下颌关节结构内紊乱病(TMJID)患者(TMJID组),采用视觉模拟评分评价各组患者咬合状态时的疼痛评分,采用ELISA检测各组滑液中MIF、COX2和PGE2的表达水平。将单侧前牙反𬌗(UAC)TMJOA模型大鼠分别处理4、8、12周(分别为UAC-4周组、UAC-8周组和UAC-12周组,n=6),同时设置相应的对照组(Ctrl-4周组、Ctrl-8周组和Ctrl-12周组,n=6,未做处理,饲养条件与UAC组一致)。采用ELISA检测各组大鼠血清和关节滑液中MIF、COX2和PGE2的表达水平,采用Western blotting检测各组大鼠颞下颌关节中白细胞介素(IL)-1β、IL-18、MIF、COX2和PTGER2的表达水平。建立滑膜成纤维样细胞(FLSs)的流体流动剪切应力(FFSS)模型并分为对照组(0 dyn/cm2 FFSS)与1、3、5、10 dyn/cm2 FFSS组,采用RT-PCR和Western blotting检测上述指标mRNA和蛋白的表达水平。结果 视觉模拟评分评价显示,TMJOA Ⅰ、Ⅱ期患者疼痛评分明显高于TMJID患者(P<0.001);ELISA检测结果显示,TMJOA组关节滑液MIF、COX2和PGE2表达水平明显高于TMJID组(P<0.05),且在TMJOA Ⅱ期组表达水平最高。大鼠TMJOA模型中,与相应的对照组比较,UAC-4、8、12周组的血清和关节滑液中MIF、COX2和PGE2表达水平增高,且在UAC-8周时明显增高(P<0.05);与对照组比较,UAC组颞下颌关节组织中IL-1β、IL-18、MIF、COX2和PTGER2蛋白的表达水平也明显升高(P<0.05)。FFSS干预FLSs的细胞模型中,随着FFSS的增大,细胞发生形变、胞膜不完整,且细胞数量减少;与对照组比较,1、3、5、10 dyn/cm2干预组FLSs的IL-1β、IL-18、MIF、COX2和PGE2(PTGER2)的表达水平明显升高(P<0.05)。结论 MIF、COX2、PGE2在伴有错𬌗畸形的TMJOA患者颞下颌关节滑液及UAC大鼠血清及关节滑液中高表达,异常流体流动剪切应力可促进FLSs分泌MIF、COX2、PGE2,参与关节微环境炎症,从而加速疾病的进展。

目的 探讨异常应力对巨噬细胞移动抑制因子(MIF)、环氧合酶2(COX2)和前列腺素E2(PGE2)在颞下颌关节骨关节炎(TMJOA)中的促进作用及其机制。方法 收集2020年1月-2021年12月就诊于新疆医科大学第一附属医院颞下颌关节专病门诊并伴有错𬌗的30例TMJOA患者(TMJOA组,根据分期分为TMJOA Ⅰ期、Ⅱ期、Ⅲ期,n=10)及30例颞下颌关节结构内紊乱病(TMJID)患者(TMJID组),采用视觉模拟评分评价各组患者咬合状态时的疼痛评分,采用ELISA检测各组滑液中MIF、COX2和PGE2的表达水平。将单侧前牙反𬌗(UAC)TMJOA模型大鼠分别处理4、8、12周(分别为UAC-4周组、UAC-8周组和UAC-12周组,n=6),同时设置相应的对照组(Ctrl-4周组、Ctrl-8周组和Ctrl-12周组,n=6,未做处理,饲养条件与UAC组一致)。采用ELISA检测各组大鼠血清和关节滑液中MIF、COX2和PGE2的表达水平,采用Western blotting检测各组大鼠颞下颌关节中白细胞介素(IL)-1β、IL-18、MIF、COX2和PTGER2的表达水平。建立滑膜成纤维样细胞(FLSs)的流体流动剪切应力(FFSS)模型并分为对照组(0 dyn/cm2 FFSS)与1、3、5、10 dyn/cm2 FFSS组,采用RT-PCR和Western blotting检测上述指标mRNA和蛋白的表达水平。结果 视觉模拟评分评价显示,TMJOA Ⅰ、Ⅱ期患者疼痛评分明显高于TMJID患者(P<0.001);ELISA检测结果显示,TMJOA组关节滑液MIF、COX2和PGE2表达水平明显高于TMJID组(P<0.05),且在TMJOA Ⅱ期组表达水平最高。大鼠TMJOA模型中,与相应的对照组比较,UAC-4、8、12周组的血清和关节滑液中MIF、COX2和PGE2表达水平增高,且在UAC-8周时明显增高(P<0.05);与对照组比较,UAC组颞下颌关节组织中IL-1β、IL-18、MIF、COX2和PTGER2蛋白的表达水平也明显升高(P<0.05)。FFSS干预FLSs的细胞模型中,随着FFSS的增大,细胞发生形变、胞膜不完整,且细胞数量减少;与对照组比较,1、3、5、10 dyn/cm2干预组FLSs的IL-1β、IL-18、MIF、COX2和PGE2(PTGER2)的表达水平明显升高(P<0.05)。结论 MIF、COX2、PGE2在伴有错𬌗畸形的TMJOA患者颞下颌关节滑液及UAC大鼠血清及关节滑液中高表达,异常流体流动剪切应力可促进FLSs分泌MIF、COX2、PGE2,参与关节微环境炎症,从而加速疾病的进展。

Objective To investigate the effects and mechnism of abnormal stress promoting macrophage mobility inhibitory factor (MIF), cyclooxygenase 2 (COX2) and prostaglandin E2 (PGE2) in the progression of temporomandibular joint osteoarthritis (TMJOA). Methods From January 2020 to December 2021, TMJOA and temporomandibular joint internal derangement (TMJID) patients (30 cases in each group, we divided the TMJOA into group TMJ Ⅰ, Ⅱ, Ⅲ according to the stage) who were admitted to TMJOA special clinic of the First Affiliated Hospital of Xinjiang Medical University and accompanied by abnormal occlusion were collected. The pain score of the occlusal state of the patients was evaluated by visual analogue scale. The expression levels of MIF, COX2 and PGE2 in synovial fluid were detected by ELISA. We used the unilateral anterior crossbite for TMJOA (UAC) rats model (the grouped into: UAC-4 weeks, UAC-8 weeks and UAC-12 weeks group), and control group at the same time (grouped into: Ctrl-4 weeks, Ctrl-8 weeks and Ctrl-12 weeks group), each group had 6 rats. The expression levels of MIF, COX2 and PGE2 in serum and synovial fluid of rats were detected by ELISA. The expression levels of IL-1β, IL-18, MIF, COX2 and PTGER2 in temporomandibular joint of rats were detected by Western blotting. The fluid flow shear stress (FFSS) model of fibroblast-like synovial cells (FLSs) was established, and the mRNA and protein expression levels of above indexes were detected by RT-PCR and Western blotting. Results Visual analogue scale evaluation showed that the pain score of TMJOA Ⅰ and Ⅱ group was significantly higher than that of TMJID (P<0.001). ELISA results showed that the expression levels of MIF, COX2 and PGE2 in synovial fluid in TMJOA group were higher than those in TMJID group (P<0.05), and the expression levels were the highest in TMJOA Ⅱ group. Compared with control group, the expressions of MIF, COX2 and PGE2 in serum and synovial fluid at UAC-4 weeks, 8 weeks and 12 weeks were slightly higher, and significantly higher at UAC-8 weeks in rat TMJOA model (P<0.05). In addition, the expression trend of protein levels in temporomandibular joint tissues was similar, which showed higher expression levels of IL-1β, IL-18, MIF, COX2 and PTGER2 (P<0.05). In the cell model where FFSS interfered with FLSs, with the increase of FFSS, cell with deformation, incomplete cell membrane and reduced number. Compared with control group, the expression levels of IL-1β, IL-18, MIF, COX2 and PGE2 (PTGER2) of FLSs were increased in 1, 3, 5 and 10 dyn/cm2 intervention groups (P<0.05). Conclusion MIF, COX2 and PGE2 were highly expressed in temporomandibular joint synovial fluid of TMJOA patients with malocclusion. And these three factors were also highly expressed in serum and synovial fluid of UAC rats. The abnormal fluid shear stress promotes the secretion of MIF, COX2 and PGE2 by FLSs to participate in joint microenvironment inflammation and accelerate disease progression.

颞下颌关节紊乱病(temporomandibular joint disease,TMD)中颞下颌关节骨关节炎(temporomandibular joint osteoarthritis,TMJOA)多由结构紊乱病(internal derangement,ID)发展而来[1],其病理特征包括关节软骨退化、骨赘生物形成、滑膜炎症等[2-3]。对于TMD的治疗应遵循规范[4],但基于现有研究,尚无有效方法阻止骨关节炎(osteoarthritis,OA)的进展[5-6]。关节慢性疼痛严重影响患者的生活质量,因此探讨疼痛机制对TMJOA的诊治与疗效具有重要意义。已有大量研究证实𬌗因素可作为独立危险因素参与TMD的发展[7-8]。口颌系统是处于咬合、神经、咀嚼肌与关节支配下的统一整体,TMJOA的发生多归因于颞下颌关节生物力学因素的变化[9],通过三维有限元分析的研究也证实了错𬌗畸形可诱导关节结构的生物力变化[10]。大鼠单侧前牙反𬌗(unilateral anterior crossbite,UAC)咬合紊乱动物模型,在异常力学刺激下诱导颞下颌关节形成经典的OA[11-12]。本课题组前期研究发现,伴有错𬌗的TMJOA患者颞下颌关节滑液中高表达白细胞介素(IL)-1β和IL-18[13]。前列腺素E2(PGE2)作为重要的炎性因子可刺激感觉神经从而引起疼痛,在此过程中还可通过其EP2受体(PTGER2)进行作用。PGE2水平主要由环氧合酶2(COX2)的变化调节。PGE2、COX2的水平增高与炎症性疼痛的病理机制密切相关。有研究表明,COX2和PGE2表达水平升高参与了机械应力引起的骨细胞早期变化[14]。巨噬细胞移动抑制因子(macrophage migration inhibitory factor,MIF)是一种多效细胞因子,可调节先天性和适应性免疫反应。有研究发现,OA滑液中MIF的含量高于正常滑液,且MIF-/-小鼠OA骨质破坏、滑膜炎症及赘生物形成情况较野生型小鼠OA模型明显减轻[15]。因此,研究关节中MIF、疼痛介质的水平可为TMJOA的靶向治疗提供理论基础。本研究初步探讨在异常应力的刺激下,滑膜对MIF、COX2和PGE2分泌的促进在TMJOA进展中的作用及意义,旨在为TMJOA的临床治疗提供新的思路。
36只6周龄雄性Sprague-Dawley(SD)大鼠,体重120~150 g,购自新疆医科大学动物实验中心。IL-1β、IL-18、MIF、COX2和PTGER2购自美国Abcam公司,MIF、COX2和PGE2 ELISA试剂盒购自上海江莱生物科技有限公司,cDNA反转录试剂盒购自美国Thermo Fisher公司,实时定量PCR试剂盒购自德国QiaGen公司。
收集2020年1月-2021年12月就诊于新疆医科大学第一附属医院颞下颌关节专病门诊明确诊断为TMD且需行颞下颌关节盥洗术的60例患者的资料,并根据专科检查及辅助检查分为TMJID组(n=30)与TMJOA组(n=30),TMJOA组再根据分期将患者分为TMJOA Ⅰ期、Ⅱ期、Ⅲ期(n=10)。纳入标准:(1)TMD中诊断为ID(关节盘可复性前移位、关节盘不可复性前移位)、OA(Ⅰ、Ⅱ、Ⅲ期)的患者;(2)临床专科检查和(或)影像学辅助检查中存在个别牙错𬌗的患者。排除标准:(1)确定诊断为两种及以上的关节疾患;(2)颜面部外伤史;(3)骨骼不对称;(4)全身系统性疾病、内分泌系统疾病;(5)近期服用激素及免疫类药物;(6)妊娠期及哺乳期;(7)急性或慢性感染期。本研究已获新疆医科大学第一附属医院伦理委员会批准(K201910-04),所有患者均签署知情同意书。
将SD大鼠随机分为对照组及实验组,每组18只。其中实验组采用单侧前牙反𬌗(unilateral anterior crossbite,UAC)模型[8]诱导大鼠颞下颌关节产生软骨退行性改变。选取25号的牛通乳针切割分段为2.5 mm,作为左上颌中切牙的金属“套筒”装置,选取20号牛通乳针截取8 mm分段,其中3.5 mm处管腔闭合向唇侧弯制形成平面导板,与剩余体部长轴呈135°,体部下端安装在左侧下颌金属“套筒”装置以形成UAC状态。将实验组分为UAC-4周组、UAC-8周组和UAC-12周组(n=6),对照组分为Ctrl-4周组、Ctrl-8周组和Ctrl-12周组(为同等培养条件但无咬合干预,n=6),并分别于4、8、12周处死,左侧颞下颌关节软骨进行分子生物学检测,右侧颞下颌关节用作形态学检测。
告知患者VAS评分刻度代表含义(0分表示无痛不适,10分表示疼痛剧烈),根据患者感受自身疼痛情况评估后填写咬合状态下的关节VAS数值。
于颞下颌关节下腔注射2%利多卡因1 ml,麻醉盥洗,冲洗出关节腔中的免疫物质,收集滑液盥洗物1 ml,以备行ELISA分析。随后同一位点注射透明质酸钠1.5 ml以润滑关节。
大鼠麻醉后,取侧卧位固定大鼠头颈部,常规消毒,使用1 ml注射器抽取200 μl生理盐水后进针入关节腔,有落空感后回抽,无血性渗出物及回抽阻力时进行关节腔盥洗。使用做好标记200 μl EP管收集对应分组的关节滑液,在-80 ℃低温条件下归档保留,检测前3000 r/min离心20 min。随后取仰卧位固定大鼠前肢及左后肢,常规消毒后取腹部正中切口暴露腹主动脉血管鞘,使用一次性采血针及采血管收集动脉血,常温环境下静置30 min时血清血浆初步分离。将动脉血以3000 r/min离心15 min以分离血清及血浆。使用1.5 ml EP管收集上层血清,注意避免吸入血浆,放入-80 ℃低温冰箱保存备用。
取耳前区切口,逐步分离组织后暴露颞下颌关节表面,打开关节囊暴露下颌骨髁突,充分游离并小心取下髁突组织,在体视显微镜下充分观察颞下颌关节髁突的关节表面和滑膜组织,然后将其置于4%多聚甲醛溶液中固定,行Micro-CT扫描检测,分析微观结构骨的相关指标[组织体积(tissue volume,TV)、骨表面积(bone surface,BS)、骨体积(bone volume,BV)、骨表面积/骨体积(bone surface/bone volume,BS/BV)和骨体积分数(骨体积/组织体积,bone volume/tissue volume,BV/TV)]及骨小梁分析指标[骨小梁厚度(trabecular thickness,Tb.Th)及骨小梁数(tabecular number,Tb.N)],以观察骨丢失与改建情况。
从室温平衡60 min后的铝箔袋中取出所需的板条,设置MIF标准品孔、空白孔和样本孔。标准品孔各加不同浓度的MIF标准品50 μl。样本孔中加待测样本50 μl,空白孔加样本稀释液50 μl。每孔加入辣根过氧化物酶标记的检测抗体100 μl,用封板膜封住反应孔,37 ℃恒温箱温育60 min。弃去液体,吸水纸上轻轻拍干,每孔加350 μl洗涤液,静置1 min后弃去洗涤液,重复5次。每孔分别加入底物A、B液各50 μl,37 ℃避光孵育15 min。每孔加入终止液50 μl,于15 min内,在450 nm波长处测定各孔的OD值。采用同样的方法检测关节滑液或血清中COX2和PGE2的浓度。
于大鼠膝关节中线髌骨表面偏内侧垂直切开,逐层分离至滑膜组织,于双侧膝关节关节腔取出滑膜组织,浸泡于含10%双抗的高糖型DMEM(H-DMEM)中,冰盒低温转移入预先消毒完成的超净工作台。PBS冲洗5 min/次×3次,修剪滑膜组织达到1 mm×1 mm×1 mm体积的块状碎片。使用等量的0.1% Ⅱ型胶原酶消化液混匀组织块沉淀,置于37 ℃恒温摇床中,180 r/min条件下消化4 h。1000 r/min离心5 min,吸弃上层混合液后仅留底层沉淀团块。加入1 ml完全培养基重悬组织细胞沉淀,经过200目铜网细胞筛过滤去除残余未消化完全的组织块。将过滤后细胞悬液接种到10 cm培养皿中,在37 ℃、5%CO2细胞培养箱中继续培养。培养12 h后置于倒置显微镜下观察细胞贴壁情况,2~3 d首次更换上层完全培养基,而后根据细胞生长速度及状态进行换液培养,最后得到原代FLSs。
FFSS循环装置由蠕动泵、平板流动腔、细胞载片、连接管及双口烧瓶组成。FFSS干预组分别设定为:0 dyn/cm2(静息状态),1、3、5、10 dyn/cm2,干预1 h。取第3代FLSs,按1×105个/ml接种于载玻片上培养至90%贴壁率,连接流体剪切装置,随机按照以上分组进行干预。
FLSs按照FFSS为0,1、3、5、10 dyn/cm2进行剪切应力干预,用Trizol裂解细胞并提取细胞内总RNA,采用核酸定量仪对各组细胞总RNA进行定量,按照反转录试剂盒说明书反转录得到cDNA。实时定量PCR引物序列见表1。反应总体系为10 μl,其中正、反义引物各0.5 μl,2×SYBR Green 5 μl,cDNA 1 μl,无酶水3 μl。β-actin为内参照基因,采用2-ΔΔCt法计算IL-1β、IL-18、MIF、COX2PGE2 mRNA相对表达水平。
将实验组分为UAC-4周组、UAC-8周组和UAC-12周组(n=6),对照组分为Ctrl-4周组、Ctrl-8周组和Ctrl-12周组(为同等培养条件但无咬合干预,n=6)。大鼠分别于4、8、12周各处死6只,用RIPA-蛋白抑制剂混合裂解液提取动物组织及细胞总蛋白,定量后行蛋白变性以维持其稳定性,-80 ℃保存。将等量的各组总蛋白电泳分离后,电转至PVDF膜上,用一抗(IL-1β抗体1:1000、IL-18抗体1:1000、MIF抗体1:1000、COX2抗体1:500和PTGER2 抗体1:500)4 ℃孵育过夜,用辣根过氧化物酶标记的羊抗兔IgG孵育1.5 h,然后采用化学发光显色方法对PVDF膜进行曝光、显影。
采用SPSS 23.0软件进行统计分析,采用GraphPad Prism 8.0软件绘图。计数资料以例(%)表示,仅进行统计描述。计量资料以$\bar{x}±s$表示,满足正态性及方差齐性时,多组间比较采用单因素方差分析;不符合正态性及方差齐性时,多组间比较采用独立样本Mann-Whitney U检验。P<0.05为差异有统计学意义。
60例患者中,女38例,男22例,女性占比明显高于男性。在TMJOA组中,男10例,女20例,男女比例为1:2,年龄(37.7±8.9)岁,咬合检查可见个别牙反𬌗患者占16.7%(5/30),个别牙锁𬌗患者占23.3%(7/30),伴有其他错𬌗的患者18例。在TMJID组中,女18例,男12例,年龄(29.5±7.0)岁,咬合检查可见个别牙反𬌗患者占10.0%(3/30),个别牙锁𬌗患者占20.0%(6/30),伴有其他错𬌗的患者21例。专科查体发现,TMJOA组患者在咬合状态时均伴有关节疼痛,且11例伴张口受限;TMJID组中23例患者伴有咬合状态时疼痛不适,9例伴张口受限(表2)。
TMJOA Ⅰ、Ⅱ和Ⅲ期患者的VAS评分高于TMJID组,且在TMJOA Ⅰ期(P<0.0001)和TMJOA Ⅱ期(P<0.0001)时差异有统计学意义(图1)。
ELISA检测结果显示,与TMJID组比较,TMJOA组患者关节滑液MIF、PGE2和COX2表达水平明显升高(P<0.05,图2)。此外,TMJOA Ⅰ、Ⅱ、Ⅲ期关节滑液中MIF、PGE2和COX2表达水平也明显高于TMJID组(P<0.05,其中TMJOA Ⅱ期表达水平最高(图3)。
体视显微镜下可见,对照组髁突软骨组织表面连续、平整未见明显凹陷及缺损;与对照组比较,UAC各干预组可见软骨组织出现不同程度的凹陷变薄、充血,伴有裂隙甚至缺损,滑膜组织肿胀增厚伴有不同程度的充血。Micro-CT扫描后可见随着干预时间的延长,髁突体积逐渐缩小,骨小梁数量减少、间隙变宽,在UAC-12周组中可见髁突软骨下骨表面凹陷缺损(图4A)。与Ctrl-4周、Ctrl-8周和Ctrl-12周相应对照组比较,UAC-4周、UAC-8周和UAC-12周组中的BV/TV(%)明显降低(P<0.01);UAC-4周、UAC-8周和UAC-12周组中BS/BV均有所增加,但仅UAC-4周与Ctrl-4组差异有统计学意义(P<0.01);UAC-4周、UAC-8周和UAC-12周组中髁突骨质的Tb.Th有减少趋势,但仅UAC-12周组较Ctrl-12周明显降低,差异有统计学意义(P<0.01);UAC-4周、UAC-8周组中髁突Tb.N较对照组明显降低(P<0.05或P<0.01),但UAC-12周组并无此趋势(图4B)。
ELISA检测显示,UAC-4周、UAC-8周和UAC-12周组血清中MIF、COX2和PGE2的表达水平均高于相应的对照组,其中仅UAC-8周组MIF、COX2与相应对照组比较差异有统计学意义(P<0.05),UAC-4周、UAC-8周和UAC-12周组血清中PGE2与相应对照组比较差异有统计学意义(P<0.05)。UAC-4周、UAC-8周和UAC-12周组关节腔滑液中的MIF、COX2和PGE2表达趋势与血清中基本一致,与相应对照组比较,UAC-8周和UAC-12周组时MIF、COX2差异有统计学意义(P<0.05),UAC-4周、UAC-8周和UAC-12周组关节滑液中PGE2与相应对照组比较差异有统计学意义(P<0.05)(图5)。
Western blotting检测显示,与Ctrl-4周组比较,UAC-4周组IL-1β、MIF表达水平明显升高(P<0.05),COX2蛋白表达水平明显降低(P<0.05),而IL-18及PTGER2蛋白表达水平差异无统计学意义(P>0.05);与Ctrl-8周组比较,UAC-8周组IL-1β、IL-18、MIF、COX2及PTGER2蛋白表达水平均明显升高(P<0.05);与Ctrl-12周组比较,UAC-12周组IL-1β、IL-18、MIF及PTGER2蛋白表达水平升高(P<0.05),而COX2蛋白表达水平差异无统计学意义(图6)。
以0、1、3、5和10 dyn/cm2 FFSS干预FLSs后,与0 dyn/cm2组比较,随着FFSS负载增大,FLSs的形态发生明显改变,表现为细胞肿胀,细胞膜不完整甚至破裂,细胞外可见碎片;FLSs的数量也随FFSS负载增大而减少(图7)。
实时定量PCR检测结果显示,与0 dyn/cm2 FFSS组比较,1 dyn/cm2 FFSS组FLSs中的PGE2、IL-18、MIF mRNA水平明显升高(P<0.05),而IL-1β、COX2 mRNA水平差异无统计学意义;3 dyn/cm2 FFSS组FLSs中IL-18MIFPGE2 mRNA水平明显升高,差异有统计学意义(P<0.05),而IL-1βCOX2 mRNA水平差异无统计学意义(P>0.05);5 dyn/cm2 FFSS组FLSs中IL-1βIL-18MIFPGE2 mRNA水平明显升高(P<0.05),而COX2 mRNA水平差异无统计学意义(P>0.05);10 dyn/cm2 FFSS组中FLSs中炎性因子IL-1βIL-18MIFCOX2PGE2 mRNA水平均明显升高(P<0.05)(图8)。
Western blotting检测显示,与0 dyn/cm2组比较,各FFSS干预组(1、3、5、10 dyn/cm2)的IL-1β、IL-18、MIF、COX2及PTGER2蛋白表达水平均明显升高(P<0.05)。除外3 dyn/cm2组FLSs中的MIF,以及5 dyn/cm2组FLSs中的COX2、PTGER2,随着FFSS负载的增大,FLSs中的IL-1β、IL-18、MIF、COX2及PTGER2蛋白表达水平逐渐增高(P<0.05)(图9)。
促炎细胞因子释放引起的炎症被认为是OA患者疼痛的主要原因[9]。越来越多的证据表明,炎症在TMJOA的发病机制中发挥重要作用[16-17]。滑膜炎症是较常见的早期发现的炎性疾病,通常会引起关节疼痛[18]。在OA中炎症性滑膜中活化的滑膜细胞会产生分解代谢和促炎介质,使分解软骨的蛋白水解酶过量产生,从而导致软骨基质降解。释放到滑液中的软骨分解产物被滑膜细胞吞噬,从而形成正反馈回路加剧滑膜炎症。MIF在体外可抑制巨噬细胞迁移,同时对单核细胞、中性粒细胞、T细胞和B细胞也具有趋化活性[19-20]。研究发现,MIF可调节IL-1家族蛋白(IL-1α、IL-1β和IL-18)的表达与释放[21],本课题组前期发现TMJOA组患者关节滑液中高表达IL-1β和IL-18[13]。本研究发现,与TMJID组比较,TMJOA组MIF、COX2和PGE2表达水平明显升高,且在TMJOA Ⅱ期患者滑液中的表达水平最高,与VAS评分趋势一致,提示MIF、COX2和PGE2表达水平与TMJOA疼痛评分呈正相关。近来有研究发现,PGE2在成骨细胞系中大量分泌[22],并作为疼痛介质可通过感觉神经的信号传递作用反馈调节骨代谢的平衡稳态[14,22]。本研究还发现TMJOA Ⅲ期患者滑液中MIF、COX2和PGE2的表达水平较Ⅱ期患者降低,后续将继续关注MIF参与COX2-PGE2信号通路促进TMJOA发展的潜在作用机制。
近年来,多数研究将咬合作为TMD的主要危险因素,且发现第三磨牙伸长、早接触、锁𬌗[23]和反𬌗[24]可促进TMD的发生。关节在生理条件下受到各种生物力学(压缩应力、拉伸应力、FFSS等)刺激[25]。异常咬合形成过大的应力可直接影响关节组织的结构和功能。目前,UAC诱导模型已广泛用于啮齿类动物OA的研究中,如关节盘退变[26]、咬肌萎缩[27]、软骨中异常矿物沉积[8]、软骨下骨丢失[28]、软骨细胞自噬与凋亡[29]等。本研究发现,UAC干预形成的异常应力会导致关节髁突软骨下骨的吸收及病理性退行性变,且UAC诱导TMJOA大鼠模型的血清和关节滑液中MIF、COX2和PGE2表达水平升高,其中关节滑液局部的MIF水平是血清中的6~8倍,提示MIF可作为TMJOA的生物标记物。此外,本研究还发现,UAC形成的异常应力作用于颞下颌关节组织后可引起无菌性炎症,并促进MIF和疼痛介质COX2和PGE2的释放,提示异常应力可上调上述因子,并参与TMJOA的进展及疼痛。
FLSs可分泌多种可溶性炎性介质和细胞因子,并在免疫调节中发挥重要作用[30]。颞下颌关节的生理功能与咬合因素之间的生物应力关系密切,为了模拟生物应力的刺激,本研究将FLSs暴露于不同FFSS条件下刺激以形成OA细胞模型,结果发现,FLSs暴露于FFSS后出现损伤,且IL-1β、IL-18和COX2、PGE2(PTGER2)的mRNA和蛋白的表达水平均明显升高,促进了炎症的发生。
综上所述,本研究发现,MIF与疼痛相关介质COX2及PGE2在伴有错𬌗畸形的TMJOA滑液中高表达,异常流体流动剪切应力下FLSs分泌MIF、COX2和PGE2参与关节微环境炎症和疼痛。但本研究仍存在一定的局限性:未在异常应力的干预下敲低MIF来观察TMJOA中疼痛介质的动态改变。因此,在异常应力的作用下,FLSs分泌MIF通过COX2-PGE2通路促进TMJOA进展的具体机制仍需进一步探讨。
