Objective To explore the pattern of early expression and secretion of tissue factor (TF) in vascular endothelial cells induced by heat stress. Methods Thirty SPF-rated C57BL/6 male mice were randomly divided into five groups: the control group and groups of indicated recovery time, including 0, 3, 6, and 9 h in room temperature after heat stress (n=6). Mice in the heat stress groups were exposed to an animal incubator to reach 42.5 ℃ for core body temperature for heat stroke. We analyzed the histopathological changes in the liver, lung, and kidney tissues with HE staining. We measured the TF mRNA in mice tissues by RT-qPCR and the plasma concentration of TF in mice with a commercial ELISA kit. Human umbilical vein endothelial cells (HUVECs) were placed in a culture incubator to build an in vitro heat stress model. HUVECs were divided into five groups, including a control group and groups of indicated recovery time, including 0, 3, 6, and 9 h after heat stress. We quantified the expression of TF mRNA and protein in HUVEC cells by RT-qPCR, Western blotting, and immunofluorescence and measured the secreted TF with a commercial ELISA kit. Results No significant pathological injury was observed in the tissues of the control group. Mice treated with heat stress had various degrees of structural injuries and hemorrhagic and inflammatory changes in multiple tissues. Compared to control group, the expression of TF mRNA significantly increased in the kidney of heat stress-treated mice with 0 and 3 h recovery time (1.719±0.018, 1.241±0.178 vs. 1.000±0.063), the lung with 3 h recovery time (2.444±0.511 vs. 1.000±0.106) and the liver with 6 h recovery time (7.312±0.618 vs. 1.000±0.147) (P<0.05). The concentration of TF in plasma also sustainedly elevated in mice with 0, 3, 6, and 9 h recovery time after heat stress as compared to control group [(132.426±17.920) pg/ml, (119.400±10.267) pg/ml, (107.374±13.495) pg/ml, (163.767±22.810) pg/ml vs. (75.479±13.831) pg/ml, respectively, P<0.01]. The expression levels of TF mRNA were higher in heat stress HUVECs with 6 h and 9 h recovery time than the control cells (1.905±0.354, 2.564±0.297 vs. 1.000±0.097, P<0.01). Secreted TF in the supernatant from HUVECs treated with heat stress and different recovery time also increased significantly [(36.309±4.101) pg/ml, (38.425±5.484) pg/ml, (41.655±4.380) pg/ml, (43.586±4.718) pg/ml vs. (14.996±0.254) pg/ml, P<0.01]. Conclusion Heat stress increased early expression and secretion of TF in vascular endothelial cells. Vascular endothelial cells may be a main source of circulating TF in heat stroke.

目的 探讨热打击诱导血管内皮细胞组织因子(TF)早期表达分泌的规律。方法 将30只SPF级C57BL/6雄性小鼠随机分为对照组与热打击后复温0、3、6、9 h组(n=6)。热打击组小鼠暴露于动物孵箱热环境,核心温度达42.5 ℃时即为发生重症中暑。采用HE染色观察小鼠肝脏、肺脏及肾脏组织病理学变化;RT-qPCR检测小鼠组织TF mRNA表达水平;ELISA法检测小鼠血浆TF浓度。体外建立人脐静脉内皮细胞(HUVECs)热打击模型,分为对照组与热打击后复温0、3、6、9 h组。采用RT-qPCR、Western blotting及细胞免疫荧光化学技术分别检测HUVECs中TF mRNA及蛋白表达水平;采用ELISA法检测HUVECs培养上清中TF水平。结果 对照组小鼠各组织未见明显病理损伤,热打击复温小鼠存在不同程度组织细胞结构性损伤及出血性、炎症性损伤。与对照组小鼠比较,热打击复温0、3 h组肾脏(1.719±0.018、1.241±0.178 vs. 1.000±0.063)、复温3 h组肺脏(2.444±0.511 vs. 1.000±0.106)及复温6 h组肝脏(7.312±0.618 vs. 1.000±0.147)中TF mRNA表达量均明显升高(P<0.05)。热打击小鼠复温0、3、6、9 h组血浆TF水平均较对照组小鼠升高[(132.426±17.920) pg/ml、(119.400±10.267) pg/ml、(107.374±13.495) pg/ml、(163.767±22.810) pg/ml vs. (75.479±13.831) pg/ml,P<0.01]。与对照组比较,热打击后复温6、9 h组HUVECs的TF mRNA表达水平明显升高(1.905±0.354、2.564±0.297 vs. 1.000±0.097,P<0.01);热打击复温0、3、6、9 h组HUVECs培养上清TF水平均明显高于对照组[(36.309±4.101) pg/ml、(38.425±5.484) pg/ml、(41.655±4.380) pg/ml、(43.586±4.718) pg/ml vs. (14.996±0.254) pg/ml,P<0.01]。结论 热打击血管内皮细胞中TF的早期表达分泌增加,可能是重症中暑循环TF的主要来源之一。

目的 探讨热打击诱导血管内皮细胞组织因子(TF)早期表达分泌的规律。方法 将30只SPF级C57BL/6雄性小鼠随机分为对照组与热打击后复温0、3、6、9 h组(n=6)。热打击组小鼠暴露于动物孵箱热环境,核心温度达42.5 ℃时即为发生重症中暑。采用HE染色观察小鼠肝脏、肺脏及肾脏组织病理学变化;RT-qPCR检测小鼠组织TF mRNA表达水平;ELISA法检测小鼠血浆TF浓度。体外建立人脐静脉内皮细胞(HUVECs)热打击模型,分为对照组与热打击后复温0、3、6、9 h组。采用RT-qPCR、Western blotting及细胞免疫荧光化学技术分别检测HUVECs中TF mRNA及蛋白表达水平;采用ELISA法检测HUVECs培养上清中TF水平。结果 对照组小鼠各组织未见明显病理损伤,热打击复温小鼠存在不同程度组织细胞结构性损伤及出血性、炎症性损伤。与对照组小鼠比较,热打击复温0、3 h组肾脏(1.719±0.018、1.241±0.178 vs. 1.000±0.063)、复温3 h组肺脏(2.444±0.511 vs. 1.000±0.106)及复温6 h组肝脏(7.312±0.618 vs. 1.000±0.147)中TF mRNA表达量均明显升高(P<0.05)。热打击小鼠复温0、3、6、9 h组血浆TF水平均较对照组小鼠升高[(132.426±17.920) pg/ml、(119.400±10.267) pg/ml、(107.374±13.495) pg/ml、(163.767±22.810) pg/ml vs. (75.479±13.831) pg/ml,P<0.01]。与对照组比较,热打击后复温6、9 h组HUVECs的TF mRNA表达水平明显升高(1.905±0.354、2.564±0.297 vs. 1.000±0.097,P<0.01);热打击复温0、3、6、9 h组HUVECs培养上清TF水平均明显高于对照组[(36.309±4.101) pg/ml、(38.425±5.484) pg/ml、(41.655±4.380) pg/ml、(43.586±4.718) pg/ml vs. (14.996±0.254) pg/ml,P<0.01]。结论 热打击血管内皮细胞中TF的早期表达分泌增加,可能是重症中暑循环TF的主要来源之一。

Objective To explore the pattern of early expression and secretion of tissue factor (TF) in vascular endothelial cells induced by heat stress. Methods Thirty SPF-rated C57BL/6 male mice were randomly divided into five groups: the control group and groups of indicated recovery time, including 0, 3, 6, and 9 h in room temperature after heat stress (n=6). Mice in the heat stress groups were exposed to an animal incubator to reach 42.5 ℃ for core body temperature for heat stroke. We analyzed the histopathological changes in the liver, lung, and kidney tissues with HE staining. We measured the TF mRNA in mice tissues by RT-qPCR and the plasma concentration of TF in mice with a commercial ELISA kit. Human umbilical vein endothelial cells (HUVECs) were placed in a culture incubator to build an in vitro heat stress model. HUVECs were divided into five groups, including a control group and groups of indicated recovery time, including 0, 3, 6, and 9 h after heat stress. We quantified the expression of TF mRNA and protein in HUVEC cells by RT-qPCR, Western blotting, and immunofluorescence and measured the secreted TF with a commercial ELISA kit. Results No significant pathological injury was observed in the tissues of the control group. Mice treated with heat stress had various degrees of structural injuries and hemorrhagic and inflammatory changes in multiple tissues. Compared to control group, the expression of TF mRNA significantly increased in the kidney of heat stress-treated mice with 0 and 3 h recovery time (1.719±0.018, 1.241±0.178 vs. 1.000±0.063), the lung with 3 h recovery time (2.444±0.511 vs. 1.000±0.106) and the liver with 6 h recovery time (7.312±0.618 vs. 1.000±0.147) (P<0.05). The concentration of TF in plasma also sustainedly elevated in mice with 0, 3, 6, and 9 h recovery time after heat stress as compared to control group [(132.426±17.920) pg/ml, (119.400±10.267) pg/ml, (107.374±13.495) pg/ml, (163.767±22.810) pg/ml vs. (75.479±13.831) pg/ml, respectively, P<0.01]. The expression levels of TF mRNA were higher in heat stress HUVECs with 6 h and 9 h recovery time than the control cells (1.905±0.354, 2.564±0.297 vs. 1.000±0.097, P<0.01). Secreted TF in the supernatant from HUVECs treated with heat stress and different recovery time also increased significantly [(36.309±4.101) pg/ml, (38.425±5.484) pg/ml, (41.655±4.380) pg/ml, (43.586±4.718) pg/ml vs. (14.996±0.254) pg/ml, P<0.01]. Conclusion Heat stress increased early expression and secretion of TF in vascular endothelial cells. Vascular endothelial cells may be a main source of circulating TF in heat stroke.

近年来,全球极端气候事件频发,高温日数增加,中暑的发病率随之增长。重症中暑伴高热、器官衰竭或严重代谢紊乱等常危及患者生命[1],即使接受重症监护治疗,患者病死率仍居高不下。尸检结果显示,重症中暑死亡患者的多个器官均有广泛血栓形成,啮齿类及狒狒重症中暑动物模型体内凝血激活,导致弥散性血管内凝血(disseminated intravascular coagulation,DIC)及组织损伤[2-3]。即使治疗及时,多数重症中暑患者早期仍会出现多器官功能障碍,使长期残疾及死亡的风险上升。重症中暑多器官损伤由组织直接热损伤、凝血功能障碍及多因素诱发的全身炎症反应综合征(systemic inflammatory response syndrome,SIRS)发展而来[4],其器官损伤机制尚不完全清楚,凝血激活甚至DIC可能是重症中暑多器官损伤发生的关键机制,并与高病死率有关[4-6]
重症中暑凝血激活的启动机制目前仍不清楚。组织因子(tissue factor,TF)又名凝血因子Ⅲ(F3),是一种细胞膜跨膜糖蛋白,也是凝血酶生成的主要启动因子[7]。TF由多种细胞分泌至血液循环中可启动凝血激活[8],在DIC的发生中发挥核心作用[9]。正常情况下,血管内皮细胞低水平表达TF,受不同刺激后可诱导TF表达增多[8]。Witkowski等[10]提出,血管内皮细胞表达分泌TF在凝血及炎症反应中发挥了重要作用。严重感染及炎症释放的炎性因子诱导内皮细胞生成TF,亦有体外实验佐证,TF介导凝血酶产生,使纤维蛋白原转化为纤维蛋白,同时激活血小板释放P-选择素,促进TF进一步释放[11]。此外,TF与凝血酶结合并激活不同细胞的蛋白酶激活受体(protease-activated receptors,PARs),诱导更多细胞因子表达[12]。血管内皮细胞TF参与炎症反应主要经细胞信号转导途径,而非激活凝血[13]。近年来,TF在炎症及凝血反应中的联结枢纽功能日益得到研究者们的重视。
临床病例分析及动物疾病模型显示,重症中暑宿主体内TF上升,验证了TF对重症中暑诱发凝血反应具有重要作用[3]。重症中暑患者体内的血管内皮损伤标志物血管性血友病因子(von Willebrand factor,vWF)及内皮素增加,表明存在内皮损伤[14-15],但现有研究成果无法明确重症中暑对血管内皮细胞本身的影响。TF是炎症与凝血反应的重要联结,在被激活的单核细胞及受损的血管内皮细胞表面均有表达,但循环TF的具体细胞来源尚不清楚[16]。重症中暑作为一种剧烈的炎症及凝血反应状态,TF的表达分泌是否源自血管内皮细胞更是未知。本研究通过建立体内外热打击模型,初步探讨热打击血管内皮细胞TF早期表达分泌的规律。
DMEM高糖培养基、胎牛血清、CO2培养箱(美国赛默飞世尔科技公司),细胞培养双抗溶液(青霉素-链霉素溶液,美国HyClone公司);BCA蛋白浓度测定试剂盒、彩色预染蛋白Marker(中国白鲨公司)。12.5%聚丙烯酰胺凝胶快速制备试剂盒(上海雅酶生物科技有限公司),兔TF单抗(武汉爱博泰克生物科技有限公司,A4395)、兔TF多抗(北京博奥森生物科技有限公司,bs-4690R)、山羊抗兔IgG DyLight® 488(ab96899)及山羊抗兔IgG(ab97080)购自英国Abcam公司,GAPDH单克隆抗体(美国Proteintech公司,60004-1-Ig),山羊抗小鼠IgG(北京中杉金桥生物技术有限公司,ZB-2305)。含DAPI抗荧光衰减封片剂(北京索莱宝科技有限公司)。去除基因组DNA实时荧光定量PCR(RT-qPCR)专用反转录试剂(日本TaKaRa公司,RR047A),SYBR Green Pro Taq HS预混型qPCR试剂盒(湖南艾科瑞生物工程有限公司,AG11701),Rotor-Gene RT-qPCR仪(德国Qiagen公司)。人TF酶联免疫吸附试验(ELISA)试剂盒、小鼠TF ELISA试剂盒(武汉贝茵莱生物科技有限公司)。三气培养箱(上海力新仪器有限公司),动物专用培养箱(宁波戴维医疗器械有限公司),动物专用体温计(中国康宁公司);光学显微镜(日本Olympus公司),荧光倒置显微镜(德国Leica公司),台式离心机(日本Kubota公司),高速冷冻离心机(德国Eppendorf公司)。
30只8~12周龄SPF级雄性C57BL/6小鼠(体重20~25 g)购自广州南方医大实验动物科技发展有限公司[动物合格证编号:SCXK(粤)2021-0041]。实验前动物自由进食进水,饲养于安静、恒温、恒湿的洁净环境,12 h光照及12 h黑暗交替变化,使用动物专用测温计测量核心体温。本实验获南部战区总医院实验动物伦理委员会审核批准(动物伦理审查编号:2021111801),实验过程符合国家及单位有关实验动物的管理及使用规定。
根据随机数字表法,将30只C57BL/6小鼠分为5组:对照组与热打击后复温0、3、6、9 h组(n=6)。热打击前30 min小鼠禁食禁水。动物专用孵箱温度设置为39 ℃,相对湿度为60%,将热打击组小鼠放入达到热打击条件的孵箱内,每隔15 min测量小鼠核心体温,达42.5 ℃时即认为发生重症中暑[17]。将小鼠移出孵箱,复温3、6、9 h组小鼠置于原正常饲养环境,自由进食进水。
热打击造模完成后采血,颈椎脱臼法处死小鼠,立即解剖留取肝、肾及肺组织,使用4%多聚甲醛固定,脱水、包埋制成石蜡块后切片,并进行HE染色,正置光学显微镜下观察各组织病理学改变并拍照。
取肝脏、肺脏及肾脏组织(10~25 mg)置于洁净EP管,彻底剪碎组织,加入1 ml Trizol吹打混匀,直至无肉眼可见大颗粒组织块,静置5~10 min,Trizol法提取组织总RNA。测定RNA浓度后用RR047A试剂盒反转录为cDNA。采用AG11701试剂盒配制反应体系,在实时定量PCR仪上机完成qPCR反应。每管反应体系为20 μl,包含10 μl 2×qPCR试剂、2 μl cDNA模板、0.4 μl引物F(10 μmol/L)、0.4 μl引物R(10 μmol/L)、7.2 μl DEPC水。实时定量PCR仪反应条件:95 ℃预变性30 s;95 ℃变性5 s,60 ℃扩增延伸反应40 s,重复40次循环。以磷酸甘油醛脱氢酶(GAPDH)作为内参照。采用2-ΔΔCt法计算TF mRNA表达水平。引物序列见表1
腹腔注射1%戊巴比妥钠(50 mg/kg)麻醉小鼠,摘除眼球取血法采集小鼠血液样本,置于含EDTA的EP管,在室温下以3000 r/min离心15 min,收集上层血浆,使用ELISA试剂盒检测小鼠血浆TF水平。
人脐静脉内皮细胞(human umbilical vein endothelial cells,HUVECs)购自武汉普诺赛生命科技有限公司。在37 ℃、5% CO2条件下培养,使用DMEM高糖培养基(含有10%胎牛血清及1%双抗)。将细胞分为对照组与热打击复温0、3、6、9 h组,将HUVECs置于42 ℃、5% CO2培养箱中3 h进行热打击,结束后立即转移至常规培养箱(37 ℃、5% CO2)中复温。
收集各组细胞及细胞培养上清。使用足量4 ℃预冷蛋白裂解液在冰上充分裂解细胞,10 000 r/min离心20 min后提取细胞总蛋白。将细胞培养上清以3000 r/min离心5 min,采用三氯乙酸沉淀蛋白法提取细胞培养上清蛋白[18],用适量蛋白裂解液将蛋白沉淀重悬成蛋白溶液保存。
采用BCA法检测细胞裂解蛋白溶液及细胞培养上清蛋白提取溶液中的蛋白浓度,取蛋白样品溶液,加入1/4体积的蛋白上样缓冲液(5×)混匀后,100 ℃加热5 min使蛋白变性。12.5%聚丙烯酰胺凝胶电泳分离蛋白,再进行转膜、封闭、一抗(A4395)孵育过夜、二抗(ab97080)孵育、洗膜及显影。
将细胞培养上清以1000×g离心10 min,采用ELISA法检测HUVECs培养上清中的TF浓度。
采用Trizol法提取细胞总RNA,RT-qPCR法测定TF mRNA的相对表达水平,细胞实验所用引物序列见表2
实验前于35 mm玻底培养皿接种细胞,热打击后弃培养基,使用4 ℃预冷的PBS缓冲液洗涤细胞3次,使用4 ℃预冷的4%多聚甲醛在冰上固定细胞30 min后洗涤,依次进行抗原封闭、一抗(bs-4690R)孵育过夜、荧光二抗(ab96899)避光孵育,使用含DAPI抗荧光衰减封片剂染核,于倒置荧光显微镜下观察并拍照,使用ImageJ软件进行分析。
采用GraphPad Prism 8.0.2软件进行统计分析。计量资料均符合正态分布,以$\bar{x}±s$表示,多组间比较采用单因素方差分析,进一步两两比较采用Dunnett-t检验。P<0.05为差异有统计学意义。
相较于对照组小鼠,热打击复温0、3 h组的肾TF mRNA (1.719±0.018、1.241±0.178 vs. 1.000±0.063)、复温3 h组的肺TF mRNA (2.444±0.511 vs. 1.000±0.106)及复温6 h组的肝TF mRNA (7.312±0.618 vs. 1.000±0.147)相对表达量均升高,差异均有统计学意义(P<0.05)。肾及肺TF mRNA表达量在热打击结束时即出现上调,肝TF mRNA表达量在复温6 h时明显上调(图1)。
ELISA检测结果显示,热打击小鼠复温0、3、6、9 h组血浆TF水平均较对照组小鼠升高[(132.426±17.920) pg/ml、(119.400±10.267) pg/ml、(107.374±13.495)pg/ml、(163.767±22.810) pg/ml vs. (75.479±13.831) pg/ml,P<0.01],尽管复温3、6 h小鼠血浆TF水平有所下降,但热打击组小鼠整体血浆TF水平仍高于对照组(图2)。
对照组小鼠各组织未见明显病理损伤。热打击组小鼠肺、肾、肝均出现组织结构破坏、出血及炎症表现。重症中暑(即复温0 h组)小鼠肾脏皮质、髓质部均存在出血现象,肾间质小血管及毛细血管明显扩张,有充血出血、血栓形成,肾间质红细胞浸润,肾小球结构有破坏(图3A)。重症中暑小鼠肺泡结构紊乱,细支气管及终末细支气管结构破坏、出血性渗出增多,上皮组织坏死脱落,血管周围、肺间质炎症细胞及红细胞浸润增多,复温6、9 h组小鼠肺泡隔明显变细(图3B)。重症中暑小鼠肝小叶结构破坏明显,有淤血征象,光镜下可见肝小叶中央静脉及肝窦扩张充血,有红细胞充斥其间,肝细胞受压萎缩,肝索变细(图3C)。
Western blotting结果显示,与对照组比较,热打击HUVECs不同复温时长组细胞培养上清能稳定持续检测出TF,对照组细胞培养上清中未检测出TF,提示热打击诱导血管内皮细胞TF胞外分泌可能(图4A)。ELISA检测结果显示,热打击HUVECs复温0、3、6、9 h组细胞培养上清中TF含量均明显高于对照组[(36.309±4.101) pg/ml、(38.425±5.484) pg/ml、(41.655±4.380) pg/ml、(43.586±4.718) pg/ml vs. (14.996±0.254) pg/ml,P<0.01,图4B]。
细胞免疫荧光化学法检测结果显示,对照组HUVECs细胞膜表面TF荧光强度较弱,符合TF在正常生理情况下的表达水平(图5A)。热打击HUVECs复温0、3、6、9 h组细胞膜表面TF相对荧光强度明显高于对照组(3.733±0.419、2.210±0.235、2.618±0.081、4.636±0.373 vs. 1.000±0.214,P<0.01),即热打击使HUVECs细胞膜表面结构性表达TF增强(图5B)。
提取热打击HUVECs的mRNA进行RT-qPCR分析结果显示,热打击复温6、9 h组的TF mRNA相对表达量均明显高于对照组(1.905±0.354、2.564±0.297 vs. 1.000±0.097,P<0.01,图6)。
重症中暑具有高发病率、高死亡率和高致残率的“三高”特征,被认为是最危险的热相关疾病,DIC作为该疾病的常见伴随高危事件,其病理机制的研究是攻克重症中暑治疗难题的关键环节之一。研究显示重症中暑DIC的发生发展与TF的表达分泌存在关联,探索TF的来源及作用是阐明重症中暑DIC发病机制的研究基础。
本研究中,重症中暑小鼠多组织出现炎症反应及凝血障碍;重症中暑早期小鼠多组织TF mRNA表达增加,且其循环内TF水平明显升高;热打击血管内皮细胞膜内TF表达分泌增加,提示血管内皮细胞可能是重症中暑TF分泌的潜在来源之一。
本研究发现,重症中暑小鼠组织发生出血性及炎性损伤,呈现炎性细胞浸润、出血及血栓形成,一定程度上符合既往研究中重症中暑导致多器官广泛出血及血栓形成的DIC病理特征[19]。有研究发现,重症中暑组织炎症反应及凝血障碍与TF表达分泌增加有关[19-20]。Huisse等[20]在2003年巴黎热浪期重症中暑临床病例中发现,患者体内循环TF水平升高。Roberts等[19]在重症中暑狒狒的血液及组织中均检测出TF含量明显上升,并与细胞凋亡相关,提示TF是重症中暑凝血反应启动的潜在重要因素。本研究采用重症中暑小鼠模型进一步证实了多组织TF mRNA表达增加,血浆分泌TF增加。此外,Bouchama等[4]在重症中暑狒狒模型中发现,重组线虫抗凝蛋白c2(组织因子/凝血因子Ⅶ抑制剂)明显改善了重症中暑动物凝血紊乱状态,进一步提示TF在重症中暑凝血紊乱中发挥作用。
血液循环中TF的潜在来源广泛,不同疾病患者体内TF的来源各不相同[21]。Franco等[22]发现,内毒素血症患者血细胞(主要为单核细胞)TF mRNA及蛋白诱导表达与高水平凝血酶-抗凝血酶复合物(凝血激活标志物)有关。脓毒症狒狒的脾脏及主动脉血管内皮细胞、脓毒症小鼠的肾小球内皮细胞均表达TF,血管内皮细胞来源TF可能也在脓毒症凝血中发挥了作用[23]。脂多糖体内外刺激单核细胞及HUVECs均可诱导TF表达,单核细胞及血管内皮细胞被看作是内毒素血症TF的两个最有可能的来源[11]。目前重症中暑表达分泌TF的细胞来源尚不十分清楚。体外热打击细胞模型显示,热打击后不同复温时间HUVECs的TF mRNA表达均有不同程度上调;细胞免疫荧光化学检测表明,HUVECs细胞膜表面TF的荧光强度持续升高。本研究还发现热打击HUVECs后,细胞培养上清分泌TF明显增加,提示热打击可能诱导细胞中TF向分泌形式转化,并可分泌至细胞外,由此推测血管内皮细胞可能是重症中暑TF表达分泌的重要来源。
热打击基于何种机制诱导血管内皮细胞分泌型TF的表达转化是下一步研究的关键。Wu等[24]提出蛋白激酶C(protein kinase C,PKC)及p38在人肺动脉内皮细胞TF的诱导表达分泌中发挥关键作用。血管内皮生长因子(vascular endothelial growth factor,VEGF)基于两种途径诱导HUVECs中TF的表达[7],剪切力激活HUVECs中早期生长反应因子-1(early growth response protein 1,Egr-1)诱导TF表达上调[25]。多项研究表明,不同刺激诱导HUVECs表达TF均基于核因子-κB(nuclear factor kappa-B,NF-κB)[26-28]。然而热打击诱导血管内皮细胞TF表达分泌的分子机制尚未见报道,也是本文的局限性所在。
综上所述,本研究中重症中暑小鼠多组织存在炎症及凝血特征性病理损伤,验证了组织及循环TF可能参与该病理损伤过程。体外热打击可能诱导血管内皮细胞TF向分泌型转变,TF分泌增加,提示血管内皮细胞可能是重症中暑TF的来源之一。探讨热打击血管内皮细胞表达分泌TF的分子机制及有关调控措施将是未来研究的重点。
