Objective To establish and validate the nomogram prediction model of degenerative lumbar disc disease in young and middle-aged people. Methods From January 2020 to March 2022, 232 young and middle-aged people who were examined with MRI of lumbar intervertebral disc and standard standing position lumbar anteroposterior and lateral X-ray films in Suzhou Ninth Hospital Affiliated to Soochow University were selected as the study subjects. Collecting the factors that may affect the occurrence of degenerative lumbar disc disease in young and middle-aged people, the people were divided into degenerative disease group (n=78) and non-degenerative disease group (n=154) according to the presence or absence of degenerative lumbar disc disease. Influencing factors were compared between the two groups, the independent influencing factors screened out by multifactor logistic regression, the nomograph model was constructed by R software, and the model verified. Results Among 232 young and middle-aged people included in this study, 78 (33.6%) had degenerative lumbar disc diseases. The results of multivariate logistic regression analysis showed that the increase of BMI, sedentariness, the increase of IL-1β level were the risk factors of degenerative lumbar disc disease in young and middle-aged people; the increase of pelvic incidence angle and the increase of miRNA-142-3p level were protective factors for degenerative lumbar disc disease in young and middle-aged people (P<0.05). The nomogram model was constructed according to the results of multifactor analysis. ROC results showed that AUC was 0.889 (95%CI 0.844-0.933, P<0.05), the average absolute error of internal validation was 0.017, the prediction curve was basically fitted with the standard curve, and the H-L goodness of fit test showed that the model was consistent (P>0.05). The decision curve shows that the clinical benefit rate is the highest when the model prediction probability threshold is 0.16-0.80. Conclusions Degenerative lumbar disc disease in young and middle-aged people is affected by such factors as BMI, sedentariness, and incidence angle of pelvis, and the nomogram model established based on this has high differentiation and accuracy of prediction.

目的 建立中青年腰椎间盘退行性病变列线图预测模型并进行验证。方法 纳入2020年1月-2022年3月在苏州大学附属苏州九院行腰椎间盘MRI、标准站立位腰椎正侧位X线检查的232名中青年人作为研究对象,根据有无腰椎退行性病变将研究对象分为退行性病变组(n=78)与无退行性病变组(n=154)。收集可能影响中青年腰椎间盘退行性病变发生的因素,比较两组各影响因素,并以多因素logistic回归筛选出独立影响因素,采用R软件构建列线图模型,并对模型进行验证。结果 共纳入232名中青年人,其中78名(33.6%)患有腰椎间盘退行性病变。二元多因素logistic回归分析结果显示,BMI升高、久坐、IL-1β水平升高为中青年腰椎退行性病变的危险因素;骨盆入射角增加、miRNA-142-3p水平升高为中青年腰椎退行性病变的保护性因素(P<0.05)。根据多因素分析结果构建列线图模型,ROC曲线分析显示,AUC为0.889(95%CI 0.844~0.933,P<0.05),内部验证平均绝对误差为0.017,预测曲线与标准曲线基本拟合,H-L拟合优度检验结果显示模型一致性较好(P>0.05)。决策曲线显示,当模型预测概率阈值为0.16~0.80时,临床获益率最高。结论 中青年腰椎间盘退行性病变受BMI、久坐、骨盆入射角等因素的影响,据此建立的列线图预测模型具有较高的区分度与准确度。

目的 建立中青年腰椎间盘退行性病变列线图预测模型并进行验证。方法 纳入2020年1月-2022年3月在苏州大学附属苏州九院行腰椎间盘MRI、标准站立位腰椎正侧位X线检查的232名中青年人作为研究对象,根据有无腰椎退行性病变将研究对象分为退行性病变组(n=78)与无退行性病变组(n=154)。收集可能影响中青年腰椎间盘退行性病变发生的因素,比较两组各影响因素,并以多因素logistic回归筛选出独立影响因素,采用R软件构建列线图模型,并对模型进行验证。结果 共纳入232名中青年人,其中78名(33.6%)患有腰椎间盘退行性病变。二元多因素logistic回归分析结果显示,BMI升高、久坐、IL-1β水平升高为中青年腰椎退行性病变的危险因素;骨盆入射角增加、miRNA-142-3p水平升高为中青年腰椎退行性病变的保护性因素(P<0.05)。根据多因素分析结果构建列线图模型,ROC曲线分析显示,AUC为0.889(95%CI 0.844~0.933,P<0.05),内部验证平均绝对误差为0.017,预测曲线与标准曲线基本拟合,H-L拟合优度检验结果显示模型一致性较好(P>0.05)。决策曲线显示,当模型预测概率阈值为0.16~0.80时,临床获益率最高。结论 中青年腰椎间盘退行性病变受BMI、久坐、骨盆入射角等因素的影响,据此建立的列线图预测模型具有较高的区分度与准确度。

Objective To establish and validate the nomogram prediction model of degenerative lumbar disc disease in young and middle-aged people. Methods From January 2020 to March 2022, 232 young and middle-aged people who were examined with MRI of lumbar intervertebral disc and standard standing position lumbar anteroposterior and lateral X-ray films in Suzhou Ninth Hospital Affiliated to Soochow University were selected as the study subjects. Collecting the factors that may affect the occurrence of degenerative lumbar disc disease in young and middle-aged people, the people were divided into degenerative disease group (n=78) and non-degenerative disease group (n=154) according to the presence or absence of degenerative lumbar disc disease. Influencing factors were compared between the two groups, the independent influencing factors screened out by multifactor logistic regression, the nomograph model was constructed by R software, and the model verified. Results Among 232 young and middle-aged people included in this study, 78 (33.6%) had degenerative lumbar disc diseases. The results of multivariate logistic regression analysis showed that the increase of BMI, sedentariness, the increase of IL-1β level were the risk factors of degenerative lumbar disc disease in young and middle-aged people; the increase of pelvic incidence angle and the increase of miRNA-142-3p level were protective factors for degenerative lumbar disc disease in young and middle-aged people (P<0.05). The nomogram model was constructed according to the results of multifactor analysis. ROC results showed that AUC was 0.889 (95%CI 0.844-0.933, P<0.05), the average absolute error of internal validation was 0.017, the prediction curve was basically fitted with the standard curve, and the H-L goodness of fit test showed that the model was consistent (P>0.05). The decision curve shows that the clinical benefit rate is the highest when the model prediction probability threshold is 0.16-0.80. Conclusions Degenerative lumbar disc disease in young and middle-aged people is affected by such factors as BMI, sedentariness, and incidence angle of pelvis, and the nomogram model established based on this has high differentiation and accuracy of prediction.

人体脊柱有颈椎、胸椎、腰椎与骶椎之分,其中腰椎活动度最大,承受的压力最高,因此腰椎退化及损伤风险最高[1]。近年来随着生活习惯、环境、营养状况等的改变导致腰椎间盘突出、腰椎椎管狭窄等腰椎退行性病变发病率呈明显升高趋势[2-3]。既往腰椎间盘退行性病变多发生于中老年人群,但近年来调查数据显示,中青年腰椎间盘退行性病变发病率呈明显升高趋势,腰椎退行性病变严重影响患者的身心健康,给家庭及社会带来沉重的经济负担[4]。研究证实,中青年腰椎间盘退行性病变为可防、可控的疾病,高危患者进行针对性干预可有效降低其发病率[5]。近年来,中青年腰椎间盘退行性病变的影响因素相关研究虽有报道[6],但建立预测模型者较少。列线图模型是在二元logistic回归分析基础上建立的可视化预测模型,该模型使用方便、易于解释,目前已用于多种临床事件的预测。本研究分析中青年腰椎间盘退行性病变的发生情况,并建立列线图模型,以期为中青年腰椎间盘退行性病变的预测提供新思路。
纳入2020年1月-2022年3月在苏州大学附属苏州九院行腰椎间盘磁共振成像(magnetic resonance imaging,MRI)、标准站立位腰椎正侧位X线检查的232名中青年人作为研究对象。纳入标准:(1)年龄18~44岁;(2)腰椎间盘MRI、标准站立位腰椎正侧位X线片影像学资料完整;(3)神志清醒,读写功能正常,可配合调查。排除标准:(1)临床资料不全;(2)妊娠期及产后1年;(3)合并脊柱肿瘤、强直性脊柱炎、腰椎峡部裂等脊柱疾病;(4)有脊柱骨折、椎间盘突出等脊柱手术史;(5)合并其他全身性疾病。本研究通过苏州大学附属苏州九院伦理委员会审核(2022年伦研批26号),研究对象均已签署知情同意书。
收集可能影响中青年腰椎间盘退行性病变发生的因素:(1)一般资料,包括性别、年龄、体重指数(body mass index,BMI)、久坐情况、饮酒史、吸烟史、是否重体力劳动等;(2)影像学资料,包括骨盆入射角、骨盆倾斜角、骶骨倾斜角、腰椎前凸角、椎间隙高度、腰椎前弯度、腰椎前凸指数、腰骶前凸角、骶倾角等;(3)实验室指标,包括白细胞介素-1β(interleukin 1β,IL-1β)、血清miRNA-142-3p、25(OH)维生素D等。其中久坐指每周至少有4 d连续静坐2 h或静坐总时间>6 h[7];饮酒史指每日乙醇摄入量>60 g,饮酒时间在10年及以上[8];吸烟史指每日吸食20支以上香烟,时间在5年以上[8];重体力劳动指从事装卸工、矿工、建筑工人、挑夫、农民等工作[9]。参照文献[10]的方法对骨盆入射角、骨盆倾斜角、骶骨倾斜角、腰椎前凸角、腰椎前弯度、腰椎前凸指数、腰骶前凸角、骶倾角等脊柱骨盆参数进行测量。IL-1β、血清miRNA-142-3p、25(OH)维生素D检测:抽取患者外周血5 ml,以酶联免疫吸附法检测IL-1β、血清miRNA-142-3p、25(OH)维生素D水平。
腰椎间盘退行性病变诊断由3名具有10年以上临床经验的副主任医师参照Pfirrmann分级评估腰椎间盘退行性病变情况[11]:Ⅰ级,髓核呈均匀的高信号,脑脊液、纤维环与髓核分界清晰,椎间盘高度正常;Ⅱ级,髓核呈不均匀的高信号,内部有或没有水平状信号带,纤维环与髓核界限清晰,椎间盘高度正常;Ⅲ级,髓核呈不均匀的中等信号,纤维环与髓核界限不清,椎间盘高度正常或有轻度下降;Ⅳ级,髓核呈不均匀的中等或低信号,髓核无法与纤维环区分,椎间盘高度正常或有中度下降;Ⅴ级,髓核呈不均匀的信号,无法与纤维环区分,椎间隙重度狭窄。Ⅰ-Ⅱ级为正常椎间盘,Ⅲ-Ⅴ级为椎间盘退行性病变。
根据有无腰椎间盘退行性病变将研究对象分为退行性病变组(n=78)与无退行性病变组(n=154)。比较两组各影响因素,在单因素分析基础上行多因素logistic回归分析,筛选出独立影响因素,根据多因素分析结果采用R4.1.3软件的rms程序包构建列线图模型。
采用R4.1.3软件的rms程序,以Bootstrap法进行内部验证,并以H-L法评估模型拟合度,绘制受试者工作特征(ROC)曲线评估模型的诊断效能,并以决策曲线评估模型的临床效能。
采用SPSS 22.0软件进行统计分析。计数资料以例(%)表示,两组间比较采用χ2检验;计量资料以$\bar{x}±s$表示,两组间比较采用t检验。多因素分析采用二元logistic回归分析,以R4.1.3软件建立列线图模型并进行验证。P<0.05为差异有统计学意义。
共纳入232名中青年人,其中78名(33.6%)患有腰椎间盘退行性病变。两组BMI、久坐情况、吸烟史、骨盆入射角、骶骨倾斜角、腰椎前凸角及IL-1β、miRNA-142-3p、25(OH)维生素D水平差异均有统计学意义(P<0.05),其余指标差异均无统计学意义(P>0.05,表1)。
表1P<0.05的因素赋值后(久坐情况、吸烟史:无=0,有=1;BMI、骨盆入射角、骶骨倾斜角、腰椎前凸角、IL-1β、miRNA-142-3p、25(OH)维生素D均为原值代入),以似然比法筛选变量,纳入多因素二元logistic回归分析,结果显示,BMI升高、久坐、IL-1β水平升高为中青年腰椎退行性病变的危险因素;骨盆入射角增加、miRNA-142-3p水平升高为中青年腰椎退行性病变的保护性因素(P<0.05,表2)。
根据多因素分析中各因素β值构建预测模型,Prob=1/(1+e-Y),Y=1.292×BMI+3.550×久坐情况-0.629×骨盆入射角+0.026×IL-1β-1.183×miRNA-142-3p+1.290。因上述模型计算复杂,为方便临床预测使用,采用R软件构建列线图模型,结果如图1所示。
以ROC曲线评估模型的预测价值,AUC为0.889(95%CI 0.844~0.933)(P<0.05,图2A),提示该模型区分度较好。
以Bootstrap法对列线图模型进行内部验证,以原始数据重复抽样1000次,结果显示,平均绝对误差为0.017,预测曲线与标准曲线基本拟合,提示模型预测准确度较高(图2B)。H-L拟合优度检验结果显示,列线图模型预测概率与中青年腰椎间盘退行性病变实际发生率存在较好的一致性(χ2=2.885,P=0.089)。
为评估列线图预测模型的临床适用性,将攻击行为发生情况作为状态变量,列线图预测模型所获得的风险预测值作为检验变量,以R4.1.3软件绘制临床决策曲线(图3),由该曲线可知,当模型预测概率阈值为0.16~0.80时,临床获益率最高。
腰椎间盘退行性病变为腰背痛的重要病因,其本质是一种生理性过程。近年来,人们的生活、学习、工作方式均发生了巨大的改变,这些改变对于腰椎间盘退行性病变的发生有重要影响。有研究证实,肥胖、久坐、吸烟等也可导致腰椎间盘退行性病变的发生[12]。腰椎间盘退行性病变发病是多方面因素长期作用的结果,本研究旨在通过二元多因素logistic回归分析筛选出中青年腰椎间盘退行性病变的独立影响因素,并据此构建列线图预测模型,以为中青年腰椎间盘退行性病变的早期预测与预防提供参考依据。
列线图模型为近年来常用的可视化预测模型,其建立在logistic回归基础上,实现了可视化。列线图模型根据多因素分析结果整合模型,方便对患者进行个性化预测,可有效满足个性化治疗的需求[13]。本研究获得了BMI、久坐情况、骨盆入射角等5个影响中青年腰椎间盘退行性病变发生的独立影响因素,并以此建立了列线图预测模型。经ROC曲线分析、校准曲线及H-L拟合优度检验结果证实,该列线图模型的预测值具有较高的准确度与区分度,当模型预测概率阈值为0.16~0.80时,临床获益率最高。临床上可使用该模型对具有腰椎间盘突出高风险的中青年制订个性化干预方案,并可在对患者个体化咨询、健康教育时使用该模型,使患者充分认识病情而加深对治疗方案的理解[14]
本研究发现,BMI、久坐均对中青年腰椎退行性病变的发生有显著影响。椎间盘系由硫酸软骨素、透明软骨素等蛋白多糖组成,蛋白多糖的吸水性较强,有良好的流体力学及吸收振荡作用[15-16]。正常生物力学环境下,椎间盘内各成分含量较稳定,但若过度肥胖或长期久坐可致机体生物力学条件改变而引起椎间盘蛋白多糖含量改变,并使各成分比例发生变化,最终导致椎间盘弹性丢失并影响其功能而出现退化[17]。上述是肥胖、长期久坐导致椎间盘退行性病变的主要机制,由此可见,减肥、避免久坐是降低中青年腰椎间盘退行性病变风险的重要手段。
生物力学改变是导致椎间盘退行性病变的重要原因,可使腰椎承受的生物应力发生改变。骨盆入射角等脊柱参数常用于评估腰椎生物应力的变化,其存在一定的个体差异,正常情况下个体骨骼生长停止后骨盆入射角可保持恒定,因此该参数可有效反映个体骨盆解剖形态[18]。骨盆入射角降低可致骨盆后倾而改变腰椎生物应力,引起腰椎间盘质地及功能改变,加速腰椎间盘退行性病变的出现,因此本研究将该指标纳入中青年腰椎退行性病变预测模型[19]
IL-1β为临床常用的炎性细胞因子,本研究发现,IL-1β为中青年腰椎间盘退行性病变的独立影响因素,与相关研究结果一致[20]。IL-1β可通过刺激滑膜细胞及软骨细胞合成前列腺素与胶原膜,同时可破坏软骨基质,并抑制软骨细胞合成蛋白多糖。蛋白多糖为腰椎间盘髓核的主要成分,胶原则为纤维环的主要成分,髓核中的蛋白多糖减少,胶原强度下降是腰椎间盘退行性病变的主要表现[21]
miRNA是一类约有22个核苷酸长度的非编码单链小分子RNA,在基因表达中起重要的调控作用[22]。miRNA-142-3p为miRNA家族的一员,既往体外研究发现,其低表达会促进椎间盘退变,可能对应的靶基因为高迁移率族蛋白B1(high-mobility group box 1,HMGB1),对终板软骨细胞的增殖、凋亡、迁移和自噬发挥重要的负反馈调节作用[23-24]。本研究发现,血清miRNA-142-3p在中青年腰椎间盘退行性病变中呈低表达,但其机制目前尚不完全明确,有待进一步探讨。
综上所述,中青年腰椎间盘退行性病变发生率较高,主要受BMI、久坐、骨盆入射角等因素的影响。本研究建立的列线图预测模型对中青年腰椎间盘退行性病变预测具有较高的准确度与区分度,当模型预测概率阈值为0.16~0.80时,临床获益率最高。但受样本量的限制,本研究仅进行了内部验证,未进行外部验证,且纳入分析的因素较少,模型仍有进一步优化的空间,后续研究将采取多中心、增加样本量、增加因素的方法以进一步优化模型,并对模型进行外部验证。
