Objective To analyze the changes of B lymphocyte (B cells) subsets in peripheral blood of patients with chronic hepatitis B (CHB) and to explore its clinical significance. Methods Peripheral blood samples were collected from 37 treatment-naïve CHB patients who were admitted to the Fifth Medical Center of PLA General Hospital from July 2022 to October 2022, and peripheral blood samples collected from 18 healthy individuals who have received the hepatitis B vaccine as healthy controls (HC). The study subjects' clinical indexes such as age, HBV DNA viral load, HBsAg quantification, HBeAg semi quantification, ALT, AST, and AST/ALT ratio were collected. The change characteristics of the frequency, phenotypic and functional markers of peripheral blood B lymphocytes and their subsets were compared between CHB and HC. Using multi-color flow cytometry, and the correlation between them and clinical indexes was analyzed. Results Frequency analysis of each subset of B cells showed that compared with HC, the frequency of total B cells, transitional B cells and naive B cells was decreased (P<0.05), while the frequency of mature B cells, memory B cells, atypical memory B cells and activated memory B cells was increased in CHB patients (P<0.01). And there was no significant difference in the frequency of resting memory B cells between the two groups (P>0.05). The results of functional analysis showed that compared with HC, the expression levels of CD79b on total B cells, mature B cells, memory B cells, naive B cells, activated memory B cells, atypical memory B cells and resting memory B cells in CHB patients were increased (P<0.05). The expression level of programmed cell death protein-1(PD-1) on atypical memory B cells in CHB patients was also higher than that in HC group (P<0.05). The results of correlation analysis showed that the frequency of total B cells in CHB patients was slightly negatively correlated with age (r=-0.39, P<0.05), while the expression of programmed death-1 (PD-1) on total B cells, mature B cells, transitional B cells, memory B cells and naive B cells were slightly positively correlated with age (r>0.36, P<0.05). Conclusions Chronic HBV infection leads to depletion of the frequency and function of a portion of B cells in the peripheral blood of CHB patients, and age is a potential risk factor for the decline in humoral immune function in CHB patients.

目的 观察慢性乙型肝炎(CHB)患者外周血B淋巴细胞(B细胞)亚群的变化特征,并探讨其临床意义。方法 收集2022年7-10月在解放军总医院第五医学中心就诊的37例CHB未治疗患者的外周血标本,同时收集18名接种过乙肝疫苗的健康人外周血标本作为健康对照(HC),并收集研究对象的年龄、HBV DNA病毒载量、HBsAg定量、HBeAg半定量、谷丙转氨酶(ALT)、谷草转氨酶(AST)及AST/ALT比值等临床指标。采用多色流式细胞术检测并比较两组外周血B细胞及各亚群的频率、表型及功能标志物的变化特征,并探究其与临床指标之间的相关性。结果 B细胞各亚群频率分析显示,与HC组比较,CHB组整体B细胞、过渡B细胞及幼稚B细胞频率均降低(P<0.05),而成熟B细胞、记忆B细胞、非典型记忆B细胞及激活记忆B细胞频率均升高(P<0.01);两组静息记忆B细胞频率比较,差异无统计学意义(P>0.05)。B细胞各亚群功能分析显示,与HC组比较,CHB组整体B细胞、成熟B细胞、记忆B细胞、幼稚B细胞、激活记忆B细胞、非典型记忆B细胞及静息记忆B细胞的CD79b表达水平均升高(P<0.05);此外,CHB组非典型记忆B细胞的程序性死亡受体1(PD-1)表达水平也高于HC组(P<0.05)。相关性分析显示,CHB组整体B细胞频率与年龄呈负相关(r=-0.39,P<0.05),且整体B细胞、成熟B细胞、过渡B细胞、记忆B细胞、幼稚B细胞的PD-1表达水平与年龄均呈正相关(r>0.36,P<0.05)。结论 慢性HBV感染导致CHB患者外周血部分B细胞的频率及功能发生耗竭,而年龄是导致CHB患者体液免疫功能下降的潜在危险因素。

目的 观察慢性乙型肝炎(CHB)患者外周血B淋巴细胞(B细胞)亚群的变化特征,并探讨其临床意义。方法 收集2022年7-10月在解放军总医院第五医学中心就诊的37例CHB未治疗患者的外周血标本,同时收集18名接种过乙肝疫苗的健康人外周血标本作为健康对照(HC),并收集研究对象的年龄、HBV DNA病毒载量、HBsAg定量、HBeAg半定量、谷丙转氨酶(ALT)、谷草转氨酶(AST)及AST/ALT比值等临床指标。采用多色流式细胞术检测并比较两组外周血B细胞及各亚群的频率、表型及功能标志物的变化特征,并探究其与临床指标之间的相关性。结果 B细胞各亚群频率分析显示,与HC组比较,CHB组整体B细胞、过渡B细胞及幼稚B细胞频率均降低(P<0.05),而成熟B细胞、记忆B细胞、非典型记忆B细胞及激活记忆B细胞频率均升高(P<0.01);两组静息记忆B细胞频率比较,差异无统计学意义(P>0.05)。B细胞各亚群功能分析显示,与HC组比较,CHB组整体B细胞、成熟B细胞、记忆B细胞、幼稚B细胞、激活记忆B细胞、非典型记忆B细胞及静息记忆B细胞的CD79b表达水平均升高(P<0.05);此外,CHB组非典型记忆B细胞的程序性死亡受体1(PD-1)表达水平也高于HC组(P<0.05)。相关性分析显示,CHB组整体B细胞频率与年龄呈负相关(r=-0.39,P<0.05),且整体B细胞、成熟B细胞、过渡B细胞、记忆B细胞、幼稚B细胞的PD-1表达水平与年龄均呈正相关(r>0.36,P<0.05)。结论 慢性HBV感染导致CHB患者外周血部分B细胞的频率及功能发生耗竭,而年龄是导致CHB患者体液免疫功能下降的潜在危险因素。

Objective To analyze the changes of B lymphocyte (B cells) subsets in peripheral blood of patients with chronic hepatitis B (CHB) and to explore its clinical significance. Methods Peripheral blood samples were collected from 37 treatment-naïve CHB patients who were admitted to the Fifth Medical Center of PLA General Hospital from July 2022 to October 2022, and peripheral blood samples collected from 18 healthy individuals who have received the hepatitis B vaccine as healthy controls (HC). The study subjects' clinical indexes such as age, HBV DNA viral load, HBsAg quantification, HBeAg semi quantification, ALT, AST, and AST/ALT ratio were collected. The change characteristics of the frequency, phenotypic and functional markers of peripheral blood B lymphocytes and their subsets were compared between CHB and HC. Using multi-color flow cytometry, and the correlation between them and clinical indexes was analyzed. Results Frequency analysis of each subset of B cells showed that compared with HC, the frequency of total B cells, transitional B cells and naive B cells was decreased (P<0.05), while the frequency of mature B cells, memory B cells, atypical memory B cells and activated memory B cells was increased in CHB patients (P<0.01). And there was no significant difference in the frequency of resting memory B cells between the two groups (P>0.05). The results of functional analysis showed that compared with HC, the expression levels of CD79b on total B cells, mature B cells, memory B cells, naive B cells, activated memory B cells, atypical memory B cells and resting memory B cells in CHB patients were increased (P<0.05). The expression level of programmed cell death protein-1(PD-1) on atypical memory B cells in CHB patients was also higher than that in HC group (P<0.05). The results of correlation analysis showed that the frequency of total B cells in CHB patients was slightly negatively correlated with age (r=-0.39, P<0.05), while the expression of programmed death-1 (PD-1) on total B cells, mature B cells, transitional B cells, memory B cells and naive B cells were slightly positively correlated with age (r>0.36, P<0.05). Conclusions Chronic HBV infection leads to depletion of the frequency and function of a portion of B cells in the peripheral blood of CHB patients, and age is a potential risk factor for the decline in humoral immune function in CHB patients.

慢性乙型肝炎(chronic hepatitis B,CHB)是最常见的病毒性疾病之一,与肝硬化及肝细胞癌(hepatocellular carcinoma,HCC)的发生密切相关[1]。我国目前约有7000万慢性乙型肝炎病毒(hepatitis B virus,HBV)感染者,其中CHB患者2000万~3000万[2-3]。调查发现,机体发生慢性HBV感染后,乙型肝炎表面抗原(hepatitis B surface antigen,HBsAg)自发清除的年发生率低于1%。核苷(酸)类似物单药或与干扰素(interferon,IFN)联合治疗能使部分患者实现HBsAg持续清除,即功能性治愈或临床治愈,但CHB患者经过连续5年以上的有效抗病毒治疗后,HBsAg转阴率仅有8%~10%[3-7]。HBsAg的持续存在是导致肝硬化及HCC的重要危险因素之一[3-7]。研究发现,使用利妥昔单抗治疗B淋巴细胞瘤合并HBV感染者可导致机体B淋巴细胞(B细胞)耗竭,从而使临床治愈患者发生HBV病毒再激活,导致与HBV感染相关的疾病再次进展[8],提示B细胞介导的体液免疫反应对HBV的清除及预防再次感染发挥着重要作用,但目前CHB患者体液免疫的特征尚未完全阐明。
研究发现,CHB患者的外周B细胞过度活化,但体外增殖能力降低[9-10]。早期研究将记忆B细胞作为一个整体,然而近期在慢性感染或自身免疫性疾病患者中鉴定出一群CD27及CD21分子表达双重缺失的非典型记忆B细胞[11]。该细胞在急性及慢性HBV感染者中呈高表达[12-14],其频率随临床疾病的发展而下降,并与HBV DNA水平呈正相关[12]。这些研究在一定程度上对CHB患者体液免疫的特征进行了描述,但由于早期对记忆B细胞分型认识的局限性,对CHB患者B细胞各亚群特征变化的描述不够详尽,同时对CHB患者B细胞变化的临床意义也尚未完全阐明。本研究观察CHB患者外周血B细胞及其各细胞亚群的频率、表型及功能特征等变化,并分析这些变化与临床指标间的相关性,以阐明其临床意义。
本研究为前瞻性研究。收集2022年7-10月于解放军总医院第五医学中心诊治的未治疗CHB患者的外周血标本,同期收集接种过乙肝疫苗的健康人外周血标本作为对照。CHB诊断标准参照《慢性乙型肝炎防治指南(2019年版)》[3]。纳入标准:HBsAg阳性持续时间为6个月以上,并且在入组前6个月内未接受过核苷类似物或IFN等抗病毒治疗的CHB患者定义为CHB未治疗患者;乙型肝炎核心抗体(hepatitis B core antibody,HBcAb)检测阴性者定义为健康对照。排除标准:已接受乙型肝炎抗病毒治疗;肝硬化或HCC;合并感染其他病毒,如甲、丙、丁、戊型肝炎病毒或人类免疫缺陷病毒(human immunodeficiency virus,HIV)等;患有药物性肝损伤、自身免疫性肝病及酒精性或非酒精性脂肪肝等其他肝脏疾病;患有其他严重疾病或未同意入组等。本研究通过解放军总医院第五医学中心伦理委员会批准(2020-053-D),所有入组对象均知情同意并签署同意书。
记录研究对象的年龄及性别等基本信息,同时记录研究对象的HBV DNA水平、HBsAg定量、血清学五项指标(HBsAg、HBsAb、HBeAg、HBeAb、HBcAb)、常规血液检查、生化、肝功能评估及既往史等临床资料。另外,使用EDTA抗凝真空采血管采集8~10 ml外周静脉血,用于单个核细胞分离。
使用EDTA抗凝真空采血管无菌采集外周静脉血8~10 ml,2000 r/min室温离心10 min,吸取上层血浆保存于-80℃冰箱,下层加入等体积PBS,混匀后缓慢铺于淋巴细胞分离液(中国美德太平洋科技公司)上方,2500 r/min离心20 min,吸取中间白膜层细胞,PBS清洗1~2次,进行细胞计数后,加入细胞冻存液,于液氮中保存备用。
对液氮中存放的PBMCs进行复苏处理,37 ℃水浴锅中快速融化后,加入恢复室温的RPMI 1640完全培养基(美国Sigma公司)中,清洗细胞后将其重悬于RPMI 1640完全培养基,并置于37 ℃、含有5% CO2的培养箱中孵育6~8 h。孵育结束后,使用流式细胞分选(fluorescence activated cell sorting,FACS)缓冲液(添加2%胎牛血清的PBS缓冲液,胎牛血清购自美国Gibco公司)对PBMCs进行清洗,并将细胞浓度调整为5×106个/ml,每个流式检测管取5×105个细胞,并添加5 μl的Fc受体阻断试剂及单核细胞阻断试剂(美国Biolegend公司),在4 ℃孵育20~30 min。孵育结束后,用FACS缓冲液清洗细胞并重悬,然后加入可固定死细胞的染料Fixable Viability Stain 510(美国BD Biosciences公司)、CD3 BV510、CD14 BV510、CD56 BV510、CD19 APC-Cy7、CD20 BV786、CD10 PerCP-Cy5.5、CD21 BUV496、CD27 PE-CF594、PD‑1 FITC、CD79b APC等流式表面染色抗体(美国Biolegend公司),4 ℃避光孵育30 min,清洗细胞后加入300 μl的1%多聚甲醛(美国Sigma公司)固定液重悬细胞,并于24 h内使用FACS Symphony A5流式细胞仪进行检测。
采用SPSS 25.0软件进行统计分析,使用GraphPad Prism 9.0软件(美国GraphPad Software公司)作图,流式原始数据采用FlowJo 10.8.1软件(美国BD Biosciences公司)分析。符合正态分布的计量资料以$\bar{x}±s$表示,两组间比较采用t检验;非正态分布的计量资料以M(Q1,Q3)表示,两组间比较采用Mann-Whitney U检验。计数资料以例(%)表示,两组间比较采用χ2检验。相关性分析采用Spearman秩相关检验。P<0.05为差异有统计学意义。
共纳入18名接种过乙型肝炎疫苗的健康对照个体(HC组)及37例CHB未治疗患者(CHB组)。两组研究对象的性别、年龄及AST/ALT比值差异无统计学意义(P>0.05),HBsAg、HBsAb、HBeAg、HBeAb、HBcAb阳性及阴性例数比较,差异有统计学意义(P<0.001);CHB组患者ALT及AST均高于HC组,差异有统计学意义(P<0.001,表1)。
外周B淋巴细胞亚群流式细胞术分析的设门策略如图1A所示。在圈定淋巴细胞并去除粘连体后,利用CD3、CD14、CD56分子及可固定死细胞的染料排除T淋巴细胞、单核细胞、NK细胞及死细胞;再利用CD19及CD20分子排除CD19-、CD20-的非B细胞后,从而圈定整体B细胞亚群;最后以CD10分子的表达与否区分成熟B细胞(CD10-)及过渡B细胞(CD10+)(图1A)。结果显示,与HC组比较,CHB组整体B细胞频率降低(4.08% vs. 5.69%,Z=-2.018,P=0.044),同时在整体B细胞中过渡B细胞频率也降低(3.89% vs. 4.53%,Z=-3.086,P=0.002),而成熟B细胞频率升高(96.10% vs. 95.45%,Z=-3.010,P=0.003)(图1B)。
利用CD21及CD27分子的表达差异将成熟B细胞区分为记忆B细胞(CD21- C27-,CD21- C27+及CD21+ C27+)及幼稚B细胞(CD21+ C27-)(图2A)。分析慢性HBV感染对记忆B细胞及幼稚B细胞频率的影响,结果显示,CHB组整体B细胞中记忆B细胞的频率较HC组升高(27.85% vs. 18.15%,Z=-3.059,P=0.002),而幼稚B细胞的频率较HC组降低(69.05% vs. 76.95%,Z=-3.067,P=0.002)(图2B)。此外,根据CD21及CD27分子的表达量差异,进一步将记忆B细胞区分为非典型记忆B细胞(CD21- C27-)、激活记忆B细胞(CD21- C27+)及静息记忆B细胞(CD21+ C27+)等3种记忆表型,流式细胞分析的设门策略如图2C所示。与HC组比较,CHB组非典型记忆B细胞的频率明显升高(9.09% vs. 3.71%,Z=-4.960,P<0.0001),同时激活记忆B细胞的频率较HC组也升高(1.05% vs. 0.59%,Z=-2.987,P=0.003),而两组静息记忆B细胞的频率比较,差异无统计学意义(15.85% vs. 14.15%,Z=-0.350,P=0.726)(图2D)。
为了探究慢性HBV感染对B细胞各亚群功能产生的影响,本研究分别检测了两组人群不同B细胞亚群表面功能标志物CD79b及PD-1表达的平均荧光强度(mean fluorescent intensity,MFI),典型图分别如图3A图4A所示(以整体B细胞为例)。CD79b表达水平分析显示,与HC组比较,CHB组整体B细胞及成熟B细胞表面CD79b的表达水平升高(整体B细胞:2214 vs. 1853,Z=-2.736,P=0.006,图3B;成熟B细胞:2158 vs. 1789,Z=-2.816,P=0.005,图3C),同时成熟B细胞中记忆B细胞及幼稚B细胞表面CD79b的表达水平也升高(记忆B细胞:2275 vs. 1976,Z=-2.215,P=0.027,图3D左;幼稚B细胞:2028 vs. 1736,Z=-2.269,P=0.023,图3D右)。进一步分析发现,CHB组激活记忆B细胞、非典型记忆B细胞及静息记忆B细胞这3种表型的记忆B细胞表面CD79b的表达水平均较HC组升高(激活记忆B细胞:2198 vs. 1496,Z=-3.695,P<0.001;非典型记忆B细胞:1590 vs. 1040,Z=-4.413,P<0.0001;静息记忆B细胞:2701 vs. 2250,Z=-2.161,P=0.031,图3E)。PD-1表达水平分析显示,CHB组非典型记忆B细胞表面PD-1的表达水平较HC组升高(329 vs. 279,Z=-1.964,P=0.049),而两组其余B细胞亚群表面PD-1的表达水平比较,差异均无统计学意义(P>0.05,图4B-E)。
CHB患者淋巴细胞中整体B细胞频率与年龄呈负相关(r=-0.39,P<0.05,图5A);而B细胞各亚群表面CD79b的表达水平与临床指标之间的相关性分析,相关性均无统计学意义(P>0.05,图5B);整体B细胞、成熟B细胞、过渡B细胞、记忆B细胞、幼稚B细胞表面PD-1的表达水平与CHB患者的年龄均呈正相关(P<0.05,图5C)。
体液免疫反应在清除HBV及预防再次感染方面发挥重要作用[3-6,8,15-16],但由于早期对B细胞各亚群分型的认识存在局限性,针对CHB患者外周B细胞的特征变化及其临床意义的描述不够充分[9-10]。为此,本研究全面分析了CHB患者B细胞各亚群的频率、表型及功能特征,并与重要的临床指标进行了相关性分析。结果显示,与HC组比较,CHB组患者外周整体B细胞、幼稚B细胞频率降低,记忆B细胞频率升高;记忆B细胞中非典型记忆B细胞及激活记忆B细胞频率明显升高;非典型记忆B细胞表面PD-1及CD79b的表达水平升高;年龄是影响B细胞频率及PD-1表达水平的重要因素。
美国的一项研究报道,与HC人群相比,CHB患者的整体B细胞、浆母细胞及浆细胞频率上调,而整体B细胞中记忆B细胞的频率差异无统计学意义[17]。然而本研究的结果显示,慢性HBV感染导致淋巴细胞中整体B细胞频率下降,而B细胞中成熟B细胞及记忆B细胞频率却明显升高,与上述研究结果不一致。导致该差异的主要原因可能包括:(1)本研究在B细胞的流式设门策略中增加了排除T细胞、单核细胞及NK细胞的步骤,使用了CD19及CD20两个表面分子标志物共同定义整体B细胞,提高了B细胞在整体淋巴细胞中的分辨率;(2)在记忆B细胞的流式圈定中,使用CD21表面分子与CD27分子共同定义记忆B细胞,避免CD21及CD27双阴性的非典型记忆B细胞被归为幼稚B细胞;(3)CHB患者感染的HBV基因型不同也可能是导致结果差异的重要原因。调查显示,欧美人群感染HBV病毒多以A基因型为主,而我国以B、C基因型为主,基因型差异导致CHB患者对IFN的应答能力、肝硬化及HCC的进展风险均存在差异,且影响临床预后[18]。因此,不同基因型HBV感染患者的B细胞特征可能存在差异。另外,B细胞在分化成浆细胞的过程中发生CD19及CD20的表达下调甚至缺失,也可能是导致B细胞频率降低的重要原因[19]
CD79b分子是B细胞抗原受体(B cell receptor,BCR)的组成亚基之一,与BCR的表达水平高度一致[20-21]。BCR在B细胞的发育、存活及活化中发挥着重要作用[22-23]。本研究发现,CHB患者成熟B细胞及其中的记忆B细胞频率均明显上调,并且各B细胞亚群上CD79b的表达水平也普遍升高。尽管CHB患者的年龄、HBV DNA病毒载量、HBsAg定量水平及HBeAg半定量水平与B细胞各亚群上CD79b的表达水平呈较为一致的正相关趋势,但差异并无统计学意义。该结果提示,慢性HBV感染可能影响B细胞表面BCR的表达水平,然而具体的临床意义及可能的作用机制目前尚不清楚,仍需要扩大样本量进行进一步验证。
研究报道,CHB患者外周存在非典型记忆B细胞的富集,这群细胞高表达Fc受体样家族成员(Fc receptor-like,FcRL)、T-betCD11cSiglec6等抑制性基因,并且与其他记忆B细胞比较,非典型记忆B细胞分化成浆细胞的能力降低,产生的抗病毒细胞因子较少,提示非典型记忆B细胞处于功能耗竭状态,这是导致病毒特异性免疫应答缺陷的重要原因[12-14]。本研究同样发现,CHB患者外周记忆B细胞中非典型记忆B细胞富集,同时非典型记忆B细胞高表达耗竭标志物PD-1,提示慢性HBV感染导致部分B细胞功能耗竭,而靶向PD-1分子可能使B细胞功能恢复。该结果与Salimzadeh等[12]的观点一致。他们认为:PD-1阻断能够部分恢复CHB患者功能障碍的病毒特异性B细胞。这些证据均为慢性HBV感染的临床治疗提供了新思路,且目前PD-1阻断剂的治疗作用已经在HBV及HBV相关HCC的临床试验中进行了测试[24]
一项关于HBsAg对CHB患者特异性及整体T细胞影响的研究发现,HBsAg特异性T细胞的数量减少只与CHB患者的年龄密切相关,即CHB患者暴露HBsAg的持续时间与HBsAg特异性T细胞数量呈负相关,而HBsAg的定量水平与HBsAg特异性T细胞数量减少并无显著关联[25]。此外,国内一项淋巴细胞频率的调查研究发现,只有CHB患者的B细胞频率与年龄呈负相关,而在健康人群中没有这种相关性[26]。本研究将CHB患者B细胞的特征与重要临床指标进行相关性分析发现,淋巴细胞中整体B细胞频率与CHB患者的年龄同样呈负相关,而整体B细胞及各亚群B细胞表面耗竭标志物PD-1的表达水平与年龄亦呈正相关。尽管在本研究中均只表现出较弱的相关性,但该结果提示CHB患者暴露HBsAg的持续时间可能是导致CHB患者体液免疫功能下降的潜在危险因素之一。由此可见,提倡早发现、早诊断、早治疗的策略对于CHB患者的病毒控制,甚至临床治愈均极为重要。与此同时,B细胞作为体液免疫的重要效应细胞,其频率及功能与年龄的相关性也部分解释了慢性HBV感染的老年人群免疫力下降,且容易发生合并感染的原因。
综上所述,慢性HBV感染后记忆B细胞的功能耗竭与HBV感染时长有关,这可能是导致HBV持续存在的重要原因。本研究进一步完善了对CHB患者体液免疫特征的理解,但由于入组患者病例数较少,且缺乏动态的随访观察,所以对于不同自然史分期的CHB患者体液免疫特征的详细描述有所欠缺。因此,后续仍需扩大样本量,并增加动态随访样本,详细解析B细胞的动态变化特征及临床意义,以期为阐明慢性HBV感染的发病机制提供更多的参考信息。
