Exertional heat stroke (EHS) is the most serious and life-threatening acute heat-related illness. It can develop from heat-related illnesses, and the prehospital management is a crucial in the diagnosis and treatment of EHS. The prognosis of EHS patients is largely determined by the rapid recognition, effective cooling, and standardized transportation during the prehospital period. Extensive research has demonstrated that medical security personnel still have fatal misconceptions in the prehospital recognition and management of EHS, mainly due to the lack of recognition and inappropriate treatment. This review summarizes the top 10 misconceptions in prehospital recognition and management of EHS, based on domestic and international research findings and combined with practical scenarios. It also provides an accurate prehospital management plan according to the China Expert Consensus on Diagnosis and Treatment of Heatstroke, aiming to reduce or avoid irreversible damage to EHS patients caused by these misunderstandings and to decrease the incidence, disability rate, and mortality rate of EHS.

劳力型热射病(EHS)是威胁生命的最严重的急性热致疾病。热致疾病可持续进展为EHS,院前处置是EHS救治的重要关口,院前是否能做到EHS患者的快速识别、有效降温及规范转运,一定程度上决定了患者的预后。根据广泛调研发现,在EHS院前认识和处置过程中,医疗保障人员仍存在亟须纠正的致命误区,这些误区集中在认识缺失和处置失当。本文借鉴国内外研究成果并结合现实情况,归纳出“EHS院前认识及处置十大误区”,并参照《中国热射病诊断与治疗专家共识》给出正确的院前处置方案,以期减少或避免这些误区对EHS患者造成的不可逆损害,降低EHS的发生率、致残率及致死率。

Exertional heat stroke (EHS) is the most serious and life-threatening acute heat-related illness. It can develop from heat-related illnesses, and the prehospital management is a crucial in the diagnosis and treatment of EHS. The prognosis of EHS patients is largely determined by the rapid recognition, effective cooling, and standardized transportation during the prehospital period. Extensive research has demonstrated that medical security personnel still have fatal misconceptions in the prehospital recognition and management of EHS, mainly due to the lack of recognition and inappropriate treatment. This review summarizes the top 10 misconceptions in prehospital recognition and management of EHS, based on domestic and international research findings and combined with practical scenarios. It also provides an accurate prehospital management plan according to the China Expert Consensus on Diagnosis and Treatment of Heatstroke, aiming to reduce or avoid irreversible damage to EHS patients caused by these misunderstandings and to decrease the incidence, disability rate, and mortality rate of EHS.

人体在高温和(或)高湿环境下剧烈运动易出现产热与散热失衡,当产热大于散热且未得到及时调节时,可能发生劳力型热射病(exertional heat stroke,EHS)。EHS是热射病(heat stroke,HS)中最严重的类型,多见于运动员、体力劳动者、士兵等,以高热、脑病、多器官功能障碍为特征表现,是威胁生命的急性热致疾病[1-3]。院前处置是EHS救治的重要关口,是否做到快速识别、有效降温及规范转运事关患者的预后。为提高现场第一目击者及医护人员的EHS院前认识及处置能力,并切实降低EHS的致残率及致死率,笔者与全军热射病防治专家组结合我国院前处置现状及国内外的研究进展,归纳出EHS院前认识及处置的十大误区。
EHS发生率会随着环境温度升高及活动强度增强而升高,然而EHS的发生往往是多种危险因素共同作用的结果。医护人员如仅考虑体弱者在高温环境下进行剧烈活动引发EHS的可能性,会导致其他易发人群被忽视,极易造成误诊、漏诊。许多EHS患者为体格健壮者,且以往在类似的热环境下进行同等强度的运动并未发生任何热相关病症[4]。EHS的危险因素可分为个体、环境、组织3个方面(表1)[1,5-7]
有些危险因素容易被忽视,如既往疾病、前期用药、睡眠不足、高湿环境及过高的精神压力等。探究常用药物的作用机制,可以此排查出由于药物诱发的EHS,如前期应用解热镇痛药。解热镇痛药作用于体温调节中枢从而发挥解热作用,体温调节功能被干扰则易诱发EHS[8-9]。睡眠不足亦是EHS不可忽视的致病因素,超高温的环境会缩短并破坏睡眠,睡眠不足及睡眠质量欠佳会使人明显感到精神不济、行动迟缓,在影响运动成绩的同时易诱发EHS[10-11]。组训者及医护人员应注意,在高湿环境下EHS的发生率明显增高,因为当环境湿度>90%时,人体出汗无法实现蒸发散热,易导致体温调节失衡[4]。过大的精神压力也是诱发EHS的要素之一,在运动、工作或军事活动中接受了超出机体运动负荷要求的健康运动者亦有发生EHS的案例报道[2]
核心体温被广泛用于评估EHS患者的病情严重程度,在HS的诊断标准中,院前核心体温的升高以往也被认为是核心标准。Bouchama等[12]提出的HS诊断标准(B-HS)须符合以下3点:(1)核心体温>40 ℃;(2)中枢神经系统异常;(3)明确的环境暴露史。日本急性中暑协会(JAAM-HS)的HS诊断标准是在医师怀疑存在热致疾病的基础上出现相应的临床表现和异常检验指标,除关注中枢神经系统异常外,还明确了肝、肾功能损伤及凝血障碍为重要的临床表现,不再强调核心体温是否>40 ℃[13]。《中国热射病诊断与治疗专家共识》根据多年临床经验及研究成果对EHS的诊断标准进行了调整,指出核心体温<40 ℃并不能排除EHS,强调应根据患者病史信息并结合临床表现来诊断EHS[6]。临床上亦有核心体温<38.4 ℃而确诊EHS的病例[14]。相较于JAAM-HS诊断标准,《中国热射病诊断与治疗专家共识》明确提出在院前处置整体过程中均应实时监测核心体温,且HS涉及的损伤器官多,不仅包括肝、肾,还包括横纹肌、胃肠、心脏等[6]。以上3个HS的诊断标准具体见表2
体表温度(腋温、前额温、颞温、手腕温)受气温、风速、湿度等周围环境影响较大,不应用于热致疾病的临床决策,且剧烈活动导致大量出汗或局部使用冰块、擦拭、水浴等方式进行降温均会使体表温度下降,因此体表温度不能准确反映患者的核心体温[15]。推荐以直肠温度为代表的核心温度作为评估病情的最佳指标[16]。有研究强调应尽一切努力在怀疑EHS诊断后测量患者的核心体温,如无法在院前测量患者的直肠温度,可测量耳蜗鼓室温度来评估其核心体温[17-18]
EHS常以意识障碍为主要临床表现,当体力活动者进行活动期间或之后出现晕厥或将要晕厥时,医师应立即对其进行评估,但意识障碍并不是EHS唯一可能出现的临床症状。美国户外工作者关于HS预防的相关文章提议,如果人摸起来又热又干,或者持续出汗但意识模糊,就应当怀疑是HS[19]
EHS意识障碍的临床表现具有多样性,如头晕、疲劳、虚弱、非理性行为、短暂失忆、歇斯底里、易激惹、癫痫发作、谵妄、昏迷等都可能是EHS所导致的中枢神经系统异常[20]。心动过速、呼吸急促、呕吐、腹痛、腹泻也是常见的院前EHS临床表现,这些异常表现提示存在轻度至中度横纹肌溶解,以及急性肾、肝、心脏损伤及凝血障碍的发生[21]。值得注意的是,训练停止或短暂降温后,可能会在中枢神经系统恶化前存在清醒间歇期。有研究证实,超过30%的EHS患者可能会出现体温及意识的一过性好转,甚至部分患者有长达1~2 d的完全清醒间歇期[3,22]。因此,现场第一目击者及医护人员不可忽视对早期完全清醒EHS患者的处置。
当EHS患者发病时症状轻或有所缓解,现场第一目击者及医护人员可能会低估其病情而误诊为脱水或轻度热致疾病,甚至可能在短期症状好转后任由患者再次进行体力活动。热致疾病是一种会持续进展的疾病,分为轻、中、重度,完整的热致疾病诊断应包含分类及分级,随着体内热量蓄积,轻度热致疾病可进展为热衰竭等中度热致疾病,如处理不及时,可能会进展为危及生命的HS[23]
病情由轻度热致疾病进展为EHS一般在24~48 h会出现多器官损伤,并可能迅速发展为多器官功能障碍甚至导致死亡[5]。EHS的器官功能障碍情况较经典型热射病(classic heat stroke,CHS)更为突出[24],康复后数月乃至数年内,其发生心血管事件、房颤及缺血性卒中的概率及死亡风险明显高于普通人群[2,4]。因存在病情进展为EHS的可能,诊断为轻度热致疾病的患者不建议在2 h内恢复训练,如考虑发生热致疾病后,应尽快联系有救治经验的医疗专家及有救治能力的医疗机构。
经过评估后若怀疑EHS,应在院前第一时间内进行降温,除发生心脏骤停必须先行心肺复苏外,不能因呼救、后送医院等延误降温时机。未得到充分降温的EHS患者,其发生并发症的风险较有效降温者高4.57倍,甚至导致死亡或遗留严重后遗症[25]。有研究发现,当患者被送到急诊时,若测肛温≥40.5 ℃,其病死率可高达80%[3];若能在现场30 min内有效降温,即30 min内将EHS患者的核心温度降至39 ℃以下[7,16],患者的病死率可降为0。
降温是指器官、组织的高温被降低,导致皮肤及浅表组织的血管收缩,维持了组织灌注,实现了对高温最敏感器官(中枢神经系统、肝脏)的最大保护[2]。降温速率即每分钟的降温速度,是判断降温是否有效的重要标志,EHS降温速度>0.15 ℃/min可最大限度地提高患者的存活率,且不会出现并发症,被认为是安全有效的[4,26]
目前对EHS患者最有效的降低核心体温的方法是CWI,其被公认为降温的“金标准”,而顾虑冷水刺激的认知是错误的[18]。当怀疑发生急性热致疾病时,应立即停止活动,脱离热环境(移至阴凉通风处并脱衣),且即刻启动主动降温。EHS院前降温手段(图1)中,降温速率<0.15 ℃/min的降温法不能作为单一的降温手段,且没有证据表明存在有效的药物可替代物理降温[7]。CWI是将便携式水浴盆装满加入冰块的冷水,使水温维持在2~20 ℃,将患者颈部以下完全浸泡在水中即最大限度地使体表接触冷水[26]。施救过程中应当避免患者头部入水,保持呼吸道通畅以防止误吸及溺水,揉搓全身皮肤以减少血管收缩。
行浸泡降温时,2~5 ℃冰水的降温速率为0.12~0.36 ℃/min,8~14 ℃冷水的降温速率是0.16~0.26 ℃/min[15]。有关CWI降温的生存率研究发现,10 ℃冷水浸泡15 min左右,EHS患者的生存率可达100%,其平均降温速率为0.22 ℃/min[27]。建议在可能发生EHS的运动场地配备CWI相应设施,并进行基本操作培训及考核,如现场处置时无标准CWI设施,需要灵活运用如洗手池、小溪、油布等创造临时CWI设施。如院前不具备行CWI的条件,可在保证降温速率的前提下选择适宜的降温手段联合应用,如冷水淋浴,湿毛巾全身擦拭,静脉输液,扇风,颈部两侧、腋窝及腹股沟上敷冰袋等[5,18,28]
EHS患者由于高温、高湿、高强度运动导致容量不足,未及时充分进行补液扩容治疗可能会出现低血容量休克、电解质紊乱、降温困难、高凝、炎症反应加剧等不良后果并影响预后。根据患者意识状态可选择口服和(或)静脉注射途径,建议选择晶体液(生理盐水、平衡液)行补液治疗[18]。口服冰盐水或输注4 ℃冷盐水均有降温作用,冷盐水输注与常规降温手段相结合有助于预防器官衰竭,适用于高温脱水伴休克的EHS患者[29-30]
EHS补液治疗的补液量应依据患者的生命体征、体力活动持续时间、个人出汗情况及转运时长等进行调整。在低血压的情况下,应立即静脉输注250~500 ml的晶体液[5]。《中国热射病诊断与治疗专家共识》建议在院前第1 h内输液量为30 ml/kg或者总量为1500~2000 ml[6]。应注意避免院前大量静脉输液带来的稀释性低钠血症、高氯血症和液体过负荷。
EHS院前处置一定要贯彻“降温第一、转运第二”的原则,未达到降温目标而盲目后送就是延误治疗。院前处置者应对EHS患者的核心体温进行动态评估,在达到有效降温目标(39 ℃以下,最佳为38.0~38.5 ℃)、低血压逐渐纠正且心率降至120次/min以下才可启动转运。
对疑似EHS的患者,若尽一切可能仍无法进行院前降温,需5 min内就近转运到具备降温治疗条件的医疗单位[15]。如EHS患者院前经降温等治疗后症状好转,应考虑到可能的病情进展情况,送往具备救治能力的医疗机构[6,9]。转运途中尽量缩短时间,以降低热致疾病的死亡风险[4,31]
EHS患者的院前降温不仅要迅速、有效,还必须贯穿始终,不可中断降温,因为在已存在体温调节障碍的情况下,运动停止后肌肉仍会继续产热,随着逐步加剧的产热、散热失衡,患者病情会再次加重[32]。在患者核心温度<39 ℃时,应在监测核心体温的同时持续予以降温,除需心肺复苏可暂停对复苏操作有影响的降温手段外,不可因为转运而中断降温操作[4,16]
急救车内空间狭小、大量电器、路途颠簸等导致无法行CWI的情况下,为控制病情恶化,需采用其他降温手段以保证降温速率。转运途中的常用降温手段包括降低环境温度(空调设置低温、打开车窗),降低患者体温(降温毯、冷水擦拭、冰块多部位冷敷、扇风),补液(清醒:口服加静脉输注液体;意识不清:静脉输注4 ℃冷盐水)[33]
院前忽视对EHS患者生命体征的持续监测,可能会出现对急重症EHS识别、分诊的错误,甚至导致紧急情况处理的延误。医护人员需持续监测EHS患者的核心体温、体表体温、意识状态、心率、血压、呼吸、血氧饱和度及大小便等情况。监测过程中,建议依据上述生理指标建立制式记录表格并规范填写。
核心体温被用来明确CWI的降温终点并指导后续的降温治疗[16],建议测温最少1次/10 min,并根据情况随时进行调整[15]。观察EHS患者的意识状态,如出现抽搐、躁动,院前应行药物(地西泮、苯巴比妥)控制以降低产热及耗氧。监测心率、血压、脉搏及尿量可评估院前患者的容量状况,调整补液量及补液速度[6]。注意保护气道,监测呼吸及血氧饱和度有助于发现呕吐所致的误吸,给予鼻导管或面罩吸氧等氧疗措施可使血氧饱和度≥95%[6]。日本HS的现场处置相关研究提出,院前除意识障碍外,心率>120次/min为心动过速、呼吸频率>24次/min为呼吸急促,心动过速及呼吸急促适用于非专业人员早期识别重症HS[31]
综上所述,EHS院前处置包括识别、降温、转运3个环节,要求现场第一目击者、医护人员能够快速识别EHS患者,果断采取CWI降温方式,并灵活应用一切降温手段实现有效降温,在持续监测生命体征的情况下完成补液、镇静、氧疗等,并在实现降温目标后规范化转运。在EHS院前处置中应遵循快速识别、有效降温、规范转运的现场处置规范,着力避免本文所述误区,以期最大程度地提高HS的救治成功率。
