Objective To explore the efficacy and safety of modified meibomian gland massage combined with ultrasonic atomization and drug therapy for dry eye in high-altitude military personnel. Methods A total of 180 patients (360 eyes) with dry eye who were diagnosed and treated by the medical team of General Hospital of Central Theater Command of Chinese PLA from July to October 2022 in Linzhi, Tibet (average altitude of 3100 meters) were selected as the research subjects. Patients were divided into four block groups based on the course of the disease: <1 month, 1-3 months, 3-6 months, and ≥6 months, and each block group was randomly assigned to control group, traditional group and modified group by random number table method, with 60 cases in each group. Control group received routine treatment (artificial tear drops and atomization fumigation); on the basis of routine treatment, traditional group underwent traditional meibomian gland massage, and modified group underwent modified meibomian gland massage. After 3 months of treatment, the scores of symptoms and signs, total effective rate, duration of treatment and incidence of adverse events were compared among the 3 groups. Results In the intra-group comparison before and after treatment, except for control group's symptom scores difference which was not statistically significant (P>0.05), all other scores decreased significantly (P<0.05). In the inter-group comparison after treatment, the scores of both traditional group and modified group were significantly better than those of control group (P<0.05), but no significant difference was observed in symptom and sign scores between traditional and modified groups (P>0.05). Compared with control group, the total effective rates of traditional group and modified group both significantly increased (P<0.01), but there was no significant difference in total effective rate between traditional and modified groups (P>0.05). The treatment time in modified group was significantly longer than that in control group (P<0.05), but significantly shorter than that in traditional group (P<0.05). The incidence of adverse events was significantly lower in modified group than that in traditional group (P<0.001). Conclusion In high-altitude areas, modified meibomian gland massage combined with ultrasonic atomization and local drug therapy for dry eye is safe and effective, non-invasiveness and easy to perform, and suitable for promoting and application in military field training.

目的 探究改良睑板腺按摩联合超声雾化及药物治疗高原官兵眼干燥症的疗效及其安全性。方法 选取2022年7-10月中部战区总医院医疗队诊治的驻西藏林芝(平均海拔3100 m)官兵眼干燥症患者180例(360眼)作为研究对象,按病程将患者分为4个区组(<1个月、1~3个月、3~6个月、≥6个月),分别将各区组内的患者采用随机数字表法随机均分为对照组、传统组及改良组(n=60)。对照组给予常规治疗(人工泪液滴眼和雾化熏蒸);在常规治疗基础上,传统组行传统睑板腺按摩,改良组行改良睑板腺按摩。疗程3个月。比较3组患者治疗后的症状与体征评分、总有效率、治疗时间及不良事件发生率。结果 3组治疗前后组内对比,除对照组症状评分差异无统计学意义(P>0.05)外,其他评分均明显下降(P<0.05)。治疗后3组间比较,传统组与改良组评分均明显优于对照组(P<0.05);传统组与改良组症状及体征评分差异无统计学意义(P>0.05)。与对照组比较,传统组与改良组的总有效率均明显增高(P<0.01);传统组与改良组间总有效率比较差异无统计学意义(P>0.05)。3组治疗时间比较,改良组治疗时间较对照组明显延长(P<0.05),但较传统组明显缩短(P<0.05)。与传统组比较,改良组不良事件发生率明显降低(P<0.001)。结论 在高原地区,改良睑板腺按摩联合雾化及局部药物治疗眼干燥症安全有效,操作简便,适合在官兵野训中推广应用。

目的 探究改良睑板腺按摩联合超声雾化及药物治疗高原官兵眼干燥症的疗效及其安全性。方法 选取2022年7-10月中部战区总医院医疗队诊治的驻西藏林芝(平均海拔3100 m)官兵眼干燥症患者180例(360眼)作为研究对象,按病程将患者分为4个区组(<1个月、1~3个月、3~6个月、≥6个月),分别将各区组内的患者采用随机数字表法随机均分为对照组、传统组及改良组(n=60)。对照组给予常规治疗(人工泪液滴眼和雾化熏蒸);在常规治疗基础上,传统组行传统睑板腺按摩,改良组行改良睑板腺按摩。疗程3个月。比较3组患者治疗后的症状与体征评分、总有效率、治疗时间及不良事件发生率。结果 3组治疗前后组内对比,除对照组症状评分差异无统计学意义(P>0.05)外,其他评分均明显下降(P<0.05)。治疗后3组间比较,传统组与改良组评分均明显优于对照组(P<0.05);传统组与改良组症状及体征评分差异无统计学意义(P>0.05)。与对照组比较,传统组与改良组的总有效率均明显增高(P<0.01);传统组与改良组间总有效率比较差异无统计学意义(P>0.05)。3组治疗时间比较,改良组治疗时间较对照组明显延长(P<0.05),但较传统组明显缩短(P<0.05)。与传统组比较,改良组不良事件发生率明显降低(P<0.001)。结论 在高原地区,改良睑板腺按摩联合雾化及局部药物治疗眼干燥症安全有效,操作简便,适合在官兵野训中推广应用。

Objective To explore the efficacy and safety of modified meibomian gland massage combined with ultrasonic atomization and drug therapy for dry eye in high-altitude military personnel. Methods A total of 180 patients (360 eyes) with dry eye who were diagnosed and treated by the medical team of General Hospital of Central Theater Command of Chinese PLA from July to October 2022 in Linzhi, Tibet (average altitude of 3100 meters) were selected as the research subjects. Patients were divided into four block groups based on the course of the disease: <1 month, 1-3 months, 3-6 months, and ≥6 months, and each block group was randomly assigned to control group, traditional group and modified group by random number table method, with 60 cases in each group. Control group received routine treatment (artificial tear drops and atomization fumigation); on the basis of routine treatment, traditional group underwent traditional meibomian gland massage, and modified group underwent modified meibomian gland massage. After 3 months of treatment, the scores of symptoms and signs, total effective rate, duration of treatment and incidence of adverse events were compared among the 3 groups. Results In the intra-group comparison before and after treatment, except for control group's symptom scores difference which was not statistically significant (P>0.05), all other scores decreased significantly (P<0.05). In the inter-group comparison after treatment, the scores of both traditional group and modified group were significantly better than those of control group (P<0.05), but no significant difference was observed in symptom and sign scores between traditional and modified groups (P>0.05). Compared with control group, the total effective rates of traditional group and modified group both significantly increased (P<0.01), but there was no significant difference in total effective rate between traditional and modified groups (P>0.05). The treatment time in modified group was significantly longer than that in control group (P<0.05), but significantly shorter than that in traditional group (P<0.05). The incidence of adverse events was significantly lower in modified group than that in traditional group (P<0.001). Conclusion In high-altitude areas, modified meibomian gland massage combined with ultrasonic atomization and local drug therapy for dry eye is safe and effective, non-invasiveness and easy to perform, and suitable for promoting and application in military field training.

眼干燥症(ophthalmoxerosis)的特点是眼表失稳态伴泪膜不稳定,可导致炎症反应、眼表损伤甚至神经感觉异常,并出现一系列眼部症状[1]。高原环境的低气压、低氧、低温及紫外线辐射强等特点[2],导致泪液蒸发增加,致眼干燥症患病率显著增高[3]。有调查显示,驻高原地区官兵眼干燥症的患病率达26.1%,明显高于平原地区官兵的11.9%[4]。眼干燥症的常规治疗以局部用药为主,虽然近年针对泪液和眼表健康的新技术方案不断出现,但其治疗效果尚待进一步验证。临床上,药物联合眼睑物理清洁治疗眼干燥症的疗效确切[5-6]。但因高原环境的特殊性,睑板腺按摩用于眼干燥症是否安全有效,传统睑板腺按摩在高原官兵中推广是否适用,会否增加不良事件的风险仍有争议。本研究引入改良版睑板腺按摩,并与传统睑板腺按摩比较,旨在探究其联合雾化及局部用药在高原官兵眼干燥症治疗中的适用性。
选取2022年7-10月中部战区总医院医疗队诊治的驻西藏林芝某地(平均海拔3100 m)官兵眼干燥症患者180例(360眼)作为研究对象。男119例,女61例,年龄19~51(29.5±6.2)岁;体重48~86(68.29±7.12) kg;文化程度:高中及以下26例,专本科117例,研究生及以上37例;病程0.5~84.0(6.32±10.49)个月。纳入标准:(1)眼部干涩感、异物感、疲劳感、烧灼感、眼胀、眼红、眼痛、畏光或视力波动等,上述9项主观症状至少1项;(2)眼表疾病指数(ocular surface disease index,OSDI)评分≥13分;(3)荧光素染色泪膜破裂时间(fluorescein breakup time,FBUT)≤5 s,或无麻醉泪液分泌试验(Schirmer Ⅰ test,SIT)≤5 mm/5 min,或5 s<FBUT≤10 s、5 mm/5 min<SIT≤10 mm/5 min且角结膜荧光素钠染色阳性(≥5个点)。排除标准:(1)合并角结膜、虹膜病变或有眼外伤、眼手术史;(2)患甲状腺相关眼病、类风湿关节炎等自身免疫病以及糖尿病、高血压及肝肾等全身性疾病;(3)长期服药、近3个月眼部用药;(4)不愿配合治疗。
眼干燥症亚型[7-8]:SIT≤2 mm/5 min,考虑水液缺乏型;FBUT≤5 s但SIT为(5~10) mm/5 min,倾向于蒸发过强型;否则为其他类型(如黏蛋白异常型、泪液动力学异常型或混合型)。本研究获中部战区总医院临床伦理委员会审批,所有程序遵循《赫尔辛基宣言》。患者知晓研究内容并自愿签署知情同意书。
按病程不同将患者分为4个区组:<1个月,1~3个月,3~6个月,≥6个月。分别将各区组患者采用随机数字表法随机均分为对照组、传统组和改良组,每组60例。对照组患者给予常规治疗(人工泪液滴眼和雾化熏蒸);传统组在常规治疗基础上,行传统睑板腺按摩;改良组在常规治疗基础上,行改良睑板腺按摩。为减少偏倚,对患者及治疗者采取双盲,3组的常规基础治疗方法均一致。随机分配后,比较不同治疗方案的评分、疗效、治疗时间及不良事件发生率。
(1)人工泪液滴眼与雾化熏蒸。人工泪液即玻璃酸钠滴眼液(中山万汉 H202003255),滴眼4次/d;雾化熏蒸指将生理盐水10 ml+玻璃酸钠滴眼液1 ml加入超声眼干燥症雾化仪(粤华WH-2000)中,雾量中等,通过超声波能量使药液振荡形成气化雾粒,患者佩戴密闭眼罩并睁眼熏蒸,15 min/次,2次/周。(2)传统睑板腺按摩[9]。患者仰卧,表面麻醉并眼周皮肤消毒,左氧氟沙星滴眼液润湿无菌棉签后,嘱患者眼球下转,翻转上睑后湿润棉签从睑板穹隆部向睑缘处反复按摩挤压,力度适中,排出分泌物;同法按摩下睑;2次/周。(3)改良睑板腺按摩。患者取坐位,无须麻醉。操作者手消毒,戴无菌橡胶手套后单手示指置于患眼上睑缘与眉下中部皮肤,拇指贴于下睑缘下方皮肤,双指合力加压睑板,使上下睑缘外翻暴露睑板腺开口;随后双指腹从穹窿部睑板向睑缘合力轻柔反复挤压,至直视下睑板腺分泌物排出,疏通睑板腺管;2次/周(图1)。
疗程共3个月,共计治疗24次。治疗期间,所有患者均予以眼健康宣传,治疗前睑缘清洁、热敷及舒适护理。
疗效评估指标包括评分、总有效率、治疗时间和不良事件发生率。(1)评分,于治疗前及治疗3个月后分别进行评分,包括症状和体征评分。评分标准见表1。(2)疗效判定:痊愈,症状消失,FBUT>10 s且SIT>10 mm/5 min;好转,症状缓解,FBUT或SIT较前延长但尚未至痊愈标准;无效,症状无缓解甚至加剧,或FBUT及SIT均较前无延长。总有效率=痊愈率+好转率。(3)治疗时间以分钟(min)计。不良事件指角膜擦伤、难以忍受的不适感、结膜下出血及急性结膜炎等。上述指标通过现场问卷及眼表专科检查获取,每眼上下睑分别记录评分,双眼取均值,所有操作均由同一眼科医师完成。
采用SPSS 22.0软件进行统计分析。Shapiro-Wilk检验分析计量数据的正态性,不符合正态分布时,采用多个独立样本比较的Kruskal-Wallis H检验,进一步两两比较采用Bonferroni法。符合正态分布的计量资料以$\bar{x}±s$表示,组内比较采用t检验,多组间比较采用单因素或随机区组设计的单因素分析,进一步两两比较采用LSD-t法。计数资料以例(%)表示,组间比较采用χ2检验,进一步两两比较采用χ2分割法。P<0.05为差异有统计学意义。
3组眼干燥症患者的基线资料比较差异无统计学意义(P>0.05,表2),具有可比性。水液缺乏型、蒸发过强型及其他亚型患者分别为14例(7.8%)、59例(32.8%)及107例(59.4%)。
与治疗前比较,治疗后对照组症状评分差异无统计学意义(P>0.05),传统组和改良组症状评分均明显下降(P<0.001);3组治疗后的体征评分均明显下降(P<0.001)。治疗前,3组症状评分和体征评分差异均无统计学意义(P>0.05)。治疗后,3组症状和体征评分比较差异有统计学意义(P<0.001);传统组与改良组症状及体征评分差异无统计学意义(P>0.05),传统组与改良组症状及体征评分均明显优于对照组(P<0.05,表3)。
随着病程进展,症状评分降低而体征评分增高(F=4.255、17.694,P=0.006、<0.001)。两两比较,症状评分除病程1个月内较其余明显增高外(P<0.05),余任两区组间差异无统计学意义(P>0.05);体征评分除病程1~3个月和3~6个月间差异无统计学意义(P>0.05)外,余任两区组间差异均有统计学意义(P<0.05,图2)。
3组总有效率比较,差异有统计学意义(χ2=31.957,P<0.001);与对照组比较,传统组与改良组的总有效率均明显增高(P<0.01);传统组与改良组总有效率比较差异无统计学意义(P>0.05,表4)。
3组治疗时间比较,差异有统计学意义(P<0.001);改良组治疗时间较对照组明显延长[(17.5±0.5) min vs. (16.0±0.5) min,P<0.05],但较传统组明显缩短[(17.5±0.5) min vs. (31.0±2.0) min,P<0.05]。3组不良事件共计18次;其中对照组0次;传统组16次(发生率26.7%),结膜炎9例,难以忍受的不适感6例,角膜擦伤1例;改良组2次(发生率3.3%),结膜炎1例,难以忍受的不适感1例。与传统组比较,改良组不良事件发生率明显降低(P<0.001)。
2022年中国干眼专家共识提出,可考虑改善症状、炎症管理、上皮保护、眼睑管理及神经营养等综合个体化治疗,充分考虑眼干燥症的亚型和严重程度,在缓解症状、保护视力的基础上尽可能去除病因[5]。由于驻防点位分散、医疗条件有限,且治疗的依从性有待提高,高原眼干燥症官兵常难以及时有效地接受详尽的诊断评估和规范治疗。
本研究依托林芝某驻防地展开,统计显示眼干燥症例数在该部官兵门诊量中排名第三(11.3%),仅次于呼吸道感染和皮肤病。流行病学调查结果显示,新冠疫情期间林芝地区官兵眼干燥症普遍,且多有频繁近距离用眼、吸烟、熬夜等不规律生活史,故而推测高海拔环境下官兵机体处于应激状态,易诱发或加重一系列眼干燥症症状[10-11];另外,高原地区的眼干燥症以蒸发过强型为主,睑酯改变影响泪膜脂质层,同时泪液渗透压增高,可诱发或加重睑缘或角膜、结膜的炎症,破坏睑缘结构,久之也会出现水样液缺乏[12]。此外,眼干燥症可由多种因素导致,临床多数患者的单纯型仅为发病早期,病情如未及时控制,往往发展为混合型。因此,疏通、恢复睑板腺的结构和功能,对于水样液缺乏型眼干燥症也有一定疗效。高原地区强光辐射、干燥致使泪液蒸发快,且寒冷、多风使得泪膜不稳定[11],致使眼干燥症呈年轻化、症状重、治疗周期长且易反复的特点[13]。传统睑板腺按摩须器械辅助,且需麻醉[14],对无菌及技术要求相对较高,在高原地区开展受限。本研究设计了3组眼干燥症治疗方案并进行比较,旨在探究针对高原地区眼干燥症睑板腺按摩是否仍安全可靠,以及改良版睑板腺按摩的适用性。疗效判定指标采用评分和总有效率等,综合了症状、体征及眼科检查[15-16]
本研究中,与治疗前比较,3组眼干燥症治疗后,除对照组症状评分差异无统计学意义外,其余两组症状评分均明显下降;提示高原眼干燥症仅采用雾化联合局部人工泪液处理可减轻体征,但患者症状无明显缓解,而改良组和传统组症状和体征评分均明显改善;提示睑板腺按摩治疗高原眼干燥症有效。3组间比较,治疗前评分差异均无统计学意义;而治疗后上述症状和体征评分均出现明显差异,且相较对照组,改良组和传统组症状和体征评分改善更明显;传统组与改良组症状和体征评分差异无统计学意义。此结果提示,眼睑按摩治疗有效,且改良组眼干燥症症状和体征的改善与传统组相近。马金兰等[13]采用无菌棉签按摩联合雾化及局部用药治疗高原地区睑板腺中度功能障碍(疗程1个月),效果良好。本研究为使治疗更加便捷,简化了麻醉、耗材及患者术前准备流程,同时严控纳入标准、规范疗程至3个月。有研究显示,睑板腺结构和功能改变是眼干燥症进展的重要因素,睑缘深层的清洁对于蒸发过强型眼干燥症安全有效[17-18]。同时,随着病程进展,症状评分降低而体征评分增高。有研究报道,检查的准确性随眼干燥症病程而增高,但检查指标与症状的相关性较差[19]。结合本研究结果,提示检查指标更多地反映体征,而眼干燥症症状更具有多样性、影响因素更复杂。
本研究各组的疗效比较结果显示,改良组与传统组总有效率相近,均高于对照组。此结果与评分比较相似。本研究疗效判定引入了FBUT和SIT。FBUT主要反映脂质层,SIT体现水样液的状况。García-Marqués等[20]比较热敷及睑板腺按摩治疗眼干燥症年轻患者与老年患者的效果,结果显示症状均缓解,但仅年轻患者BUT及脂质层厚度改善。有效的眼睑按摩甚至可改善睡眠质量,年轻患者获益更明显[21]。高原官兵以青年为主,眼睑按摩可取得良好疗效。近期研究认为泪膜黏蛋白聚合物可调节泪膜流动性,其丢失与泪膜过早破裂密切相关[22-23]。Szczesna-Iskander[24]的泪液动力学研究显示,眼干燥症者自发眨眼后达到泪膜动态平台期明显快于健康者,这与泪膜表面质量及泪膜稳定性密切相关。高原眼干燥症患者FBUT和SIT普遍不达标,分析原因:一是环境刺激使得睑板腺终末导管过度角化、睑酯黏滞及腺泡萎缩,从而抑制脂质分泌;二是高原官兵防护意识尚待增强,紫外线眼表损伤后结膜杯状细胞黏蛋白分泌亦受干扰,进一步破坏泪膜稳定性[25]
本研究还显示,改良组治疗时间较对照组稍有延长,但较传统组明显缩短;对照组无不良事件,改良组不良事件明显少于传统组;提示改良组在缓解眼干燥症症状的同时改善了眼表微环境,其操作简单、安全、易耐受,患者依从性良好。不良事件中,结膜炎均为轻度感染性,对视功能影响不明显,考虑由野外条件无菌标准不够或继发感染所致。因此,治疗流程中仍需严格规范操作,控制好各个环节的无菌要求。高原眼干燥症的治疗应兼顾眼干燥症和睑板腺功能,以局部治疗为重点;必要时辅以人工泪液、抗感染甚至手术治疗。黏稠度高、不含防腐剂并含脂质的人工泪液更适合于高原眼干燥症[26]。近年来,针对眼干燥症出现了生物衍生品、强脉冲光、睑板腺探测及神经调节刺激器等治疗手段[27-28],这些手段可大幅改善眼干燥症者的生活质量,但仍需致力于逆转其潜在病因及核心机制[16],未来须探究针对不同亚型眼干燥症的新疗法[29-30]。值得注意的是,眼干燥症的防护非常必要,例如,紧密贴合的防护镜可提供良好的紫外线辐射防护,且能减缓泪液蒸发[31]
综上所述,本研究结果显示,在高原地区,改良睑板腺按摩治疗眼干燥症有效,且安全性、时效性及舒适性较高。由于在野外伴随保障条件下展开,一定程度阻碍睑板腺功能的系统评估,结果中病程对评分有影响,尚需针对高原眼干燥症开展全面系统的检查,纳入多因素进一步深入研究。简言之,改良睑板腺按摩联合雾化及药物治疗高原眼干燥症效果确切,用时较短、安全无创且操作简便,在高原驻防、野战等条件下具有可行性,适合在广大官兵外训中推广应用。
