Objective To investigate the relationship between single nucleotide polymorphisms (SNPs) in the eNOS gene and genetic susceptibility to systemic lupus erythematosus (SLE) in Hainan. Methods Blood samples were collected from SLE patients (SLE group, n=214) and healthy controls (control group, n=214) from January 2020 to December 2022 at the First Affiliated Hospital of Hainan Medical College and Hainan Provincial People's Hospital. The bases of eNOS gene rs3918188, rs1799983 and rs1007311 loci in each group were detected by SNaPshot sequencing technology. Logistic regression was used to analyze the correlation between genotypes, alleles and gene models (dominant model, recessive model, and overdominant model) of the above 3 target loci of the eNOS gene and genetic susceptibility to SLE. Haplotype analysis was conducted using HaploView 4.2 software to investigate the relationship between haploid and genetic susceptibility to SLE at each site. Results The results of logistic regression analysis revealed that the CC genotype and the C allele at rs3918188 locus were risk factors for genetic susceptibility to SLE (CC vs. AA: OR=2.449, P<0.05; C vs. A: OR=2.133, P<0.001). In recessive model at rs3918188 locus, CC genotype carriers had an increased risk of SLE development compared with AA+AC genotype carriers (OR=2.774, P<0.001). In contrast, in overdominant model at this locus, AC genotype carriers had a decreased risk of SLE occurrence compared with AA+CC genotype carriers (OR=0.385, P<0.001). In addition, polymorphisms of rs1799983 and rs1007311 were not associated with susceptibility to SLE in genotype, allele type and the 3 genetic models (P>0.05). Haplotype analysis revealed a strong linkage disequilibrium between the rs1007311 and rs1799983 loci of the eNOS gene, but no significant correlation was found between haplotype and genetic susceptibility to SLE (P>0.05). Conclusion The CC genotype and C allele at rs3918188 locus of eNOS gene may be risk factors for SLE in Hainan, while the risk of SLE occurrence is reduced in carriers of AC genotype under the overdominant model.

目的 探讨海南地区内皮型一氧化氮合酶(eNOS)基因单核苷酸多态性(SNP)与系统性红斑狼疮(SLE)遗传易感性的关系。方法 收集2020年1月-2022年12月在海南医学院第一附属医院及海南省人民医院的海南籍SLE患者(SLE组,n=214)及健康对照者(对照组,n=214)的血液样本,通过SNaPshot测序技术检测两组eNOS基因rs3918188、rs1799983及rs1007311位点的碱基情况,并采用logistic回归分析eNOS基因上述3个目的位点的基因型、等位基因及基因模型(显性模型、隐性模型、超显性模型)与SLE遗传易感性之间的相关性。采用HaploView 4.2软件分析各位点的单倍体与SLE遗传易感性的关系。结果 Logistic回归分析结果显示,rs3918188位点中,CC基因型及C等位基因是SLE遗传易感性的危险因素(CC vs. AA:OR=2.449,P<0.05;C vs. A:OR=2.133,P<0.001)。在rs3918188位点的隐性模型中,与AA+AC基因型携带者比较,CC基因型携带者的SLE发生风险增高(OR=2.774,P<0.001);而在该位点的超显性模型中,与AA+CC基因型携带者比较,AC基因型携带者的SLE发生风险降低(OR=0.385,P<0.001)。此外,在基因型、等位基因型以及3种基因模型中,rs1799983和rs1007311的多态性均与SLE的易感性无明显相关(P>0.05)。单倍型分析显示,eNOS基因的rs1007311和rs1799983位点之间存在强连锁不平衡,但单倍型与SLE的遗传易感性无明显相关(P>0.05)。结论 海南地区eNOS基因rs3918188位点的CC基因型和C等位基因可能是SLE的危险因素,而在超显性模型下该位点AC基因型携带者的SLE发生风险降低。

目的 探讨海南地区内皮型一氧化氮合酶(eNOS)基因单核苷酸多态性(SNP)与系统性红斑狼疮(SLE)遗传易感性的关系。方法 收集2020年1月-2022年12月在海南医学院第一附属医院及海南省人民医院的海南籍SLE患者(SLE组,n=214)及健康对照者(对照组,n=214)的血液样本,通过SNaPshot测序技术检测两组eNOS基因rs3918188、rs1799983及rs1007311位点的碱基情况,并采用logistic回归分析eNOS基因上述3个目的位点的基因型、等位基因及基因模型(显性模型、隐性模型、超显性模型)与SLE遗传易感性之间的相关性。采用HaploView 4.2软件分析各位点的单倍体与SLE遗传易感性的关系。结果 Logistic回归分析结果显示,rs3918188位点中,CC基因型及C等位基因是SLE遗传易感性的危险因素(CC vs. AA:OR=2.449,P<0.05;C vs. A:OR=2.133,P<0.001)。在rs3918188位点的隐性模型中,与AA+AC基因型携带者比较,CC基因型携带者的SLE发生风险增高(OR=2.774,P<0.001);而在该位点的超显性模型中,与AA+CC基因型携带者比较,AC基因型携带者的SLE发生风险降低(OR=0.385,P<0.001)。此外,在基因型、等位基因型以及3种基因模型中,rs1799983和rs1007311的多态性均与SLE的易感性无明显相关(P>0.05)。单倍型分析显示,eNOS基因的rs1007311和rs1799983位点之间存在强连锁不平衡,但单倍型与SLE的遗传易感性无明显相关(P>0.05)。结论 海南地区eNOS基因rs3918188位点的CC基因型和C等位基因可能是SLE的危险因素,而在超显性模型下该位点AC基因型携带者的SLE发生风险降低。

系统性红斑狼疮  /  内皮型一氧化氮合酶  /  单核苷酸多态性

Objective To investigate the relationship between single nucleotide polymorphisms (SNPs) in the eNOS gene and genetic susceptibility to systemic lupus erythematosus (SLE) in Hainan. Methods Blood samples were collected from SLE patients (SLE group, n=214) and healthy controls (control group, n=214) from January 2020 to December 2022 at the First Affiliated Hospital of Hainan Medical College and Hainan Provincial People's Hospital. The bases of eNOS gene rs3918188, rs1799983 and rs1007311 loci in each group were detected by SNaPshot sequencing technology. Logistic regression was used to analyze the correlation between genotypes, alleles and gene models (dominant model, recessive model, and overdominant model) of the above 3 target loci of the eNOS gene and genetic susceptibility to SLE. Haplotype analysis was conducted using HaploView 4.2 software to investigate the relationship between haploid and genetic susceptibility to SLE at each site. Results The results of logistic regression analysis revealed that the CC genotype and the C allele at rs3918188 locus were risk factors for genetic susceptibility to SLE (CC vs. AA: OR=2.449, P<0.05; C vs. A: OR=2.133, P<0.001). In recessive model at rs3918188 locus, CC genotype carriers had an increased risk of SLE development compared with AA+AC genotype carriers (OR=2.774, P<0.001). In contrast, in overdominant model at this locus, AC genotype carriers had a decreased risk of SLE occurrence compared with AA+CC genotype carriers (OR=0.385, P<0.001). In addition, polymorphisms of rs1799983 and rs1007311 were not associated with susceptibility to SLE in genotype, allele type and the 3 genetic models (P>0.05). Haplotype analysis revealed a strong linkage disequilibrium between the rs1007311 and rs1799983 loci of the eNOS gene, but no significant correlation was found between haplotype and genetic susceptibility to SLE (P>0.05). Conclusion The CC genotype and C allele at rs3918188 locus of eNOS gene may be risk factors for SLE in Hainan, while the risk of SLE occurrence is reduced in carriers of AC genotype under the overdominant model.

系统性红斑狼疮(systemic lupus erythematosus,SLE)是一种自身免疫性疾病[1],其特征是破坏性的全身器官受累,可导致患者脏腑功能下降、发病率和病死率增高[2]。目前,SLE在全球的发病率为0.003‰~0.233‰[3],而在中国的发病率为0.3‰~0.7‰,位居全世界第二[4],给社会和家庭带来了沉重的负担[5]。与健康人群比较,SLE患者的死亡风险高18~19倍[3],最常见的病死原因是肾脏疾病、心血管等系统疾病及感染[6]。尽管SLE的治疗已取得了较大进展,但SLE患者的死亡率仍然很高,标化死亡率为2.4%~5.9%[7]。SLE的发病机制尚未阐明,目前研究认为主要由遗传和环境因素影响而发病[8]。随着分子生物学的发展,很多研究证实,遗传易感性对SLE的发展有重要影响[9-10]。研究表明,内皮型一氧化氮合酶(eNOS)基因被认为是SLE易感基因之一,能催化一氧化氮(NO)的产生,该过程与SLE的发病机制密切相关[11-13]。但关于eNOS基因rs3918188、rs1799983、rs1007311位点的多态性与SLE患者的易感性尚不明确。eNOS基因位点与海南地区SLE易感性的关联研究也少见报道。本研究探讨海南地区eNOS基因rs3918188、rs1799983、rs1007311位点的单核苷酸多态性(single nucleotide polymorphism,SNP)与SLE患者遗传易感性的关系,以期为SLE的机制研究提供参考。
收集2020年1月-2022年12月在海南医学院第一附属医院及海南省人民医院就诊的海南籍SLE患者(SLE组,n=214)和健康对照者(对照组,n=214)的临床资料进行回顾性分析。SLE组纳入标准:年龄18~70岁;符合2019年欧洲抗风湿病联盟(EULAR/ACR)的SLE诊断标准。对照组纳入标准:年龄18~70岁;健康检查正常;无心、脑、肝、肾、肺等主要器官的实质性病变。排除标准:(1)合并其他风湿病,如类风湿关节炎、硬皮病、多发性肌炎;(2)合并循环系统、泌尿系统和造血系统等严重原发性疾病,严重代谢失调;(3)肿瘤;(4)职业运动员、妊娠、哺乳及月经期;(5)不愿意参加本研究。本研究已获得海南医学院伦理委员会审批(HYLL-2017-001)。
PCR引物购自美国Life Technologies Corporation公司;Taq酶、dNTP、ExoI酶购自美国Fermentas公司;Taq DNA ligase购自美国New England Biolabs公司;qPCR PreMix(Probe)购自北京天根生化科技有限公司;各型号tips购自美国KIRGEN公司;DEPC水购自上海吉真生物科技有限公司;FastAP酶购自美国Thermo Scientific公司;SNaPshot试剂盒购自美国Applied Biosystems公司。微量核酸检测仪购自美国Thermo Scientific公司;SNaPshot测序PCR仪、基因测序仪购自美国Applied Biosystems公司;实时荧光定量PCR仪购自美国Bio-Rad公司;电泳仪购自美国Major Science公司;电泳槽购自美国Labnet 公司;普通PCR仪购自北京东胜创新生物科技有限公司;台式高速冷冻离心机、精密移液器购自德国Eppendorf 公司;荧光定量PCR96孔板、Platemax Ultraclear Sealing购自美国Axygen公司。
取SLE组及对照组血液样本解冻,进行DNA抽提与纯度分析后配制反应体系:DNA模版1 μl,10×buffer 1.5 μl,MgCl2(25 mmol/L)1.5 μl,dNTP(10 mmol/L)0.3 μl,引物(10 μmol/L)0.15 μl/条,Taq酶(5 U/μl)0.3 μl,DEPC水补至15 μl。反应条件:94 ℃预变性3 min;94 ℃变性15 s、55 ℃退火15 s、72 ℃延伸30 s,循环35个周期;72 ℃延伸3 min。待反应结束后,各位点取3 μl PCR产物混匀,置于冰上,加入0.2 μl核酸外切酶Ⅰ和0.5 μl碱性磷酸酶纯化,在37 ℃反应15 min,随后在80 ℃反应15 min,主要用核酸外切酶Ⅰ去除反应产物中的剩余引物,用碱性磷酸酶去除反应中剩余的dNTP。目的基因相对表达量采用2-ΔΔCt法计算,实验重复至少3次。以上产物在-20 ℃冰箱保存。引物序列见表1
根据目标位点的序列,设计SNaPshot测序反应引物(表2)。SNaPshot测序反应体系:PCR扩增纯化产物2 μl,SNaPshot Mix试剂1 μl,延伸引物(10 μmol/L)0.2 μl/条,ddH2O补至6 μl。反应条件:96 ℃预变性1 min;96 ℃变性10 s、52 ℃退火5 s、60 ℃延伸30 s,循环30个周期。反应结束后,将反应产物置于冰上,加入0.2 μl碱性磷酸酶混匀纯化,在37 ℃反应60 min,随后在75 ℃反应15 min,主要是用碱性磷酸酶去除反应中剩余的dNTP。纯化后的SNP位点各取1 μl,加入9 μl 甲酰胺(HIDI)混匀,在95 ℃条件下静置反应3 min,随后冰水浴,上机电泳进行测序。其余产物在4 ℃冰箱中保存。
使用GeneMarker软件对测序结果进行判读,并与NCBI数据库(https://www.ncbi.nlm.nih.gov/snp/)进行比对,以获得多态性的位点碱基。利用Hardy-Weinberg遗传平衡定律检验本研究所选血液样本的遗传稳定性是否良好。本研究所收集的数据均釆用双录入方式,并对数据进行核查、整理。采用SPSS 25.0软件进行统计分析。计量资料以$\bar{x}±s$表示,两组间比较采用t检验;计数资料以率(%)表示,两组间比较采用χ2检验;各位点基因型、等位基因、基因模型采用logistic回归分析,并进行比值比(OR)和95%置信区间(95%CI)的计算。采用HaploView 4.2软件对eNOS基因的3个SNP位点进行单倍体构建和连锁不平衡分析,并将R2>80%视为强连锁不平衡[14],分析SLE组与对照组间单倍型频率差别。P<0.05为差异有统计学意义。
对照组年龄为(33.3±11.2)岁,男25例,女189例;SLE组年龄为(33.8±12.5)岁,男23例,女191例。两组年龄、性别比较差异无统计学意义(P>0.05)。
海南地区eNOS基因的rs3918188、rs1799983和rs1007311位点在健康对照组的分布符合Hardy-Weinberg遗传平衡定律(P>0.05,表3)。
在海南地区的SLE组中,rs3918188位点的CC基因型和C等位基因频率明显高于对照组(P<0.05),而rs1007311与rs1799983位点的基因型和等位基因频率差异无统计学意义(P>0.05)(表4)。
rs3918188位点在隐性模型中,CC基因型携带者相较AA+AC基因型携带者的SLE发生风险升高(P<0.001);而在超显性模型中,AC基因型携带者相较AA+CC基因型携带者的SLE发生风险降低(P<0.001)。此外,rs1799983和rs1007311的多态性在基因型、等位基因以及3种基因模型中差异均无统计学意义(P>0.05)(表5)。
采用HaploView 4.2软件对eNOS基因上3个SNP位点进行单倍体构建和连锁不平衡分析,构建一个由rs1007311和rs1799983组成的连锁不平衡(linkage disequilibrium,LD)图(图1)。结果显示,rs1007311与rs1799983位点之间存在强连锁不平衡关系,2个SNPs形成GG、AG、AT这3种单倍型,SLE组与对照组间差异无统计学意义(P>0.05,表6)。
SLE是一种系统性自身免疫性疾病[15],其特点为免疫失调,常导致多系统、多器官受累[16]。SLE的病因既有遗传因素,也有环境因素,是一种多因素导致的疾病[17]。目前,遗传易感性对SLE发展的影响已被证实[18-21]。有学者基于全基因组关联研究(genome-wide association studies,GWASs)发现了东亚和欧洲人群中的100多个SLE易感位点[22]。有研究发现,eNOS基因是SLE遗传易感性的候选基因之一,能催化一氧化氮(NO)的产生,其过程与SLE的发病机制密切相关[11-13]。为了进一步探索eNOS基因的SNP位点rs3918188、rs1799983和rs1007311与SLE遗传易感性之间的关系,笔者收集了428例海南地区人群血液样本进行SNP分析。
本研究首先进行Hardy-Weinberg遗传平衡检验,在确认3个位点均符合平衡之后,再进行基因型、等位基因和基因模型频率的logistic回归分析。结果发现,在海南地区的人群中,rs1799983位点的基因型和等位基因均与SLE的遗传易感性无关,且在隐性、显性或超显性模型中也未发现SLE与该位点多态性之间的关联性。rs1799983位点与巴西[23]、伊朗东南部[24]和阿拉伯等地区[25]的SLE遗传易感性无关,与本研究结果一致。Lee等[25]的Meta分析纳入亚洲人群,使用固定效应模型发现rs1799983位点与SLE的遗传易感性有关,但在随机效应模型下,未发现该位点与SLE的遗传易感性相关。在我国东北地区,rs1799983位点的变异与SLE肾病的发生存在明显的相关性,该位点T等位基因在SLE肾损伤病理中发挥明显的保护作用,但在其显性模型下可增加SLE肾病的患病风险[26],与本研究结果存在一定的差异。Tang等[27]对我国湖南省汉族人群进行研究发现,rs1799983位点的多态性在SLE患者与对照组之间无明显差异,与本研究结果一致。造成这些研究结果差异的原因可能与样本量、环境因素和民族差异有关[28]。有学者发现,rs1799983位点多态性与SLE合并股骨头坏死的遗传易感性明显相关,但与SLE无关,提示该位点可能与SLE患者遗传易感性无明显相关性,但与SLE进展导致的某些临床亚型的遗传易感性有关[29]。但目前仍无相关证据可以证明该推测。因此,未来可以SLE临床亚型作为切入点,更深入地研究rs1799983位点与SLE遗传易感性的关系及机制。
目前,eNOS基因的rs1007311位点与疾病的遗传易感性研究较少,本研究未发现该位点与SLE的遗传易感性有关。然而,rs3918188在其他疾病领域的研究范围较广,有学者评估该位点对中国汉族人群2型糖尿病和糖尿病肾病易感性的潜在作用时,发现该位点多态性可影响2型糖尿病的遗传易感性[30]。刘玲等[31]发现,rs3918188位点多态性可能与汉族原发性高血压无明显相关性。Al-Forkan等[32]认为,rs3918188位点的AC基因型明显增高了来自砷污染地区的脑血管疾病患者心脏组织损伤的概率。然而,目前rs3918188与SLE的相关研究尚未见报道。本研究发现,在海南地区的SLE患者中,rs3918188位点的CC基因型和C等位基因是SLE的危险因素;在隐性模型分布下,rs3918188位点CC基因型携带者相较于AA+AC基因型携带者的SLE发生风险增高,而在超显性模型中,AC基因型携带者相较于AA+CC基因型携带者的SLE发生风险降低。
为了进一步探索这些位点之间的关系,本研究进行了单倍型分析。结果显示rs1007311与rs1799983位点之间存在强连锁不平衡关系。然而,rs1007311和rs1799983位点之间形成的单倍型与SLE的遗传易感性无相关性,这可能与样本量、地区和种族的差异有关[28]
综上所述,本研究基于海南地区的SLE患者及健康人eNOS基因的SNP分析,发现rs3918188位点与该地区SLE患者的遗传易感性有关,而rs1007311、rs1799983位点与该地区SLE患者的遗传易感性无关。该结果有助于深入探究eNOS基因与SLE遗传易感性的机制,并为SLE的早期诊断及治疗提供一定的遗传学证据。但本研究仍存在不足:(1)未进行民族分层研究。由于海南地区除汉族外,还分布有黎族等少数民族,在同一地区不同民族人群中可能会有不同的结果。(2)侧重于遗传标记与疾病易感性的关系,未探讨这些遗传变异的功能影响。未来可扩大样本量,针对民族进行分层比较,并补充相关遗传学实验,进一步分析eNOS基因位点的多态性与海南地区不同民族SLE遗传易感性之间的关系,并探索这些遗传变异的功能影响,这对早期筛查SLE、阐明SLE的发病机制有重大意义。
