Lipids, including fats (triglycerides) and lipoids (phospholipids and sterols), not only serve as an energy source for the body but also play a pivotal role throughout the reproductive process, particularly in the establishment and maintenance of early pregnancy. This encompasses the regulate of early embryonic development and uterine tolerance, and the facilitation of embryo implantation. Given the diversity of lipids, this review focuses on extensively studied lipid mediators such as polyunsaturated fatty acids, endocannabinoids, prostaglandins, lysophosphatidic acid, sphingolipids and steroid hormones. It systematically elaborates on the regulatory effects of fatty acid, phospholipid, and cholesterol metabolism on the formation of endometrial receptivity and embryo implantation, as well as the potential underlying mechanisms. The review aims to provide new insights and feasible intervention approaches for predicting and improving the outcomes of natural pregnancy and/or assisted reproductive technology.

脂类包括脂肪(三酰甘油)及类脂(磷脂、固醇类),除为机体提供能量外,脂质代谢也在整个生殖过程中发挥重要作用,尤其是早期妊娠的建立和维持(包括调节早期胚胎的发育和子宫的容受性,以及促进胚胎的着床)阶段。鉴于脂质种类繁多,本文着重从多不饱和脂肪酸、内源性大麻素、前列腺素、溶血磷脂酸、鞘磷脂、类固醇激素等被广泛研究的脂质介质,系统阐述脂肪酸、磷脂、胆固醇代谢对子宫内膜容受性形成及胚胎着床的调控作用,以及可能的潜在机制,以期为预测和改善自然妊娠和(或)辅助生殖助孕的结局提供新的切入点及可行的干预途径。

Lipids, including fats (triglycerides) and lipoids (phospholipids and sterols), not only serve as an energy source for the body but also play a pivotal role throughout the reproductive process, particularly in the establishment and maintenance of early pregnancy. This encompasses the regulate of early embryonic development and uterine tolerance, and the facilitation of embryo implantation. Given the diversity of lipids, this review focuses on extensively studied lipid mediators such as polyunsaturated fatty acids, endocannabinoids, prostaglandins, lysophosphatidic acid, sphingolipids and steroid hormones. It systematically elaborates on the regulatory effects of fatty acid, phospholipid, and cholesterol metabolism on the formation of endometrial receptivity and embryo implantation, as well as the potential underlying mechanisms. The review aims to provide new insights and feasible intervention approaches for predicting and improving the outcomes of natural pregnancy and/or assisted reproductive technology.

胚胎着床是复杂而精细的过程,需要依赖优质的胚胎、良好的子宫内膜容受性,以及胚胎和子宫内膜的同步发育与相互配合[1]。临床上胚胎着床失败是早期妊娠丢失的主要原因之一[2]。尽管辅助生殖技术(assisted reproductive techniques,ART)中胚胎筛选、体外培养及低温储藏技术已取得了巨大进步[3],但临床妊娠率仍仅30%~40%。因此,因子宫内膜容受性受损或降低导致的胚胎着床失败逐渐受到重视。研究发现,许多因素可调控子宫内膜容受性,包括子宫解剖结构、免疫因素及内分泌代谢等[4]。近年来研究表明,子宫内膜含有明显影响生殖的脂质成分,且脂类代谢的脂质介质涉及女性从卵子发生到输卵管运输、子宫内膜容受性形成、胚胎植入、妊娠的建立及维持,以及分娩等多个生殖事件[5-6]。然而,脂代谢影响妊娠的确切机制尚未明确。因此,增进对脂质介质及其信号通路调控子宫内膜蜕膜化及胚胎植入的分子机制的理解,有利于更好地指导相关不孕症的治疗。本文系统阐述血脂异常对女性生殖结局的影响,多不饱和脂肪酸(poly-unsaturated fatty acid,PUFA)、内源性大麻素(endocannabinoids,eCBs)、前列腺素(prostaglandin,PG)等脂质代谢在子宫内膜容受性形成及胚胎着床中可能的潜在机制,以及脂代谢异常与多囊卵巢综合征等生殖相关性疾病关系,旨在为预测和提高自然妊娠和(或)辅助生殖助孕的结局提供新的切入点和可行的干预途径。
临床中通常根据血清总胆固醇(total cholesterol,TC)、三酰甘油、低密度脂蛋白胆固醇(low density lipoprotein total cholesterol,LDL-C)及高密度脂蛋白胆固醇浓度评估血脂状况。近年的研究发现,血脂异常不仅与子痫、早产等妊娠并发症密切相关,也会对接受ART助孕女性的生殖结局产生不良影响[7-8]。大型临床随机对照试验研究显示,高浓度LDL-C的多囊卵巢综合征(polycystic ovarian syndrome,PCOS)不孕症患者妊娠率、活产率明显降低,且流产率明显增高(P<0.05)[9]。同样,Gao等[10]以TC>6.11 mmol/L作为高胆固醇血症的诊断标准,发现TC水平升高与PCOS患者接受体外受精及胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)助孕后的活产率呈负相关(P<0.05),并建立了PCOS患者辅助生殖活产率的预测模型。但Yang等[11]认为,当TC≥5.20 mmol/L时,IVF-ET不孕患者的活产率及足月出生率明显降低(P<0.05)。虽然诊断标准还未达成共识,但以上证据表明高水平TC是不孕症女性IVF-ET生殖结局的高危因素。以上研究表明,脂代谢对女性生殖结局有重要影响,但脂代谢异常是通过影响卵母细胞-胚胎质量还是子宫内膜微环境进而影响女性生殖功能,仍有待深入研究。Yang等[11]基于5030例不孕症患者的大型回顾性真实世界研究发现,TC水平升高不仅影响IVF-ET生殖结局,且血脂异常组患者的子宫内膜厚度明显降低,C型子宫内膜的发病率明显高于健康对照组(P<0.05),但两组间成熟卵子数及优质胚胎数差异并无统计学意义(P>0.05),故推测TC水平升高可能对子宫内膜容受性产生不良影响,进而影响女性的生殖结局。
FA是脂质氧化及合成的代谢枢纽物质。有些FA如亚油酸、α-亚麻酸及花生四烯酸(arachidonic acid,AA)等人体无法合成,只能从食物或营养补充剂中获得,为必需脂肪酸。FA除为人体提供主要能源外,还参与细胞膜形成、离子稳态、信号转导、基因表达及脂类生物调节剂的合成。近年来许多研究表明,FA尤其是PUFA对性腺功能及生殖调控有重要影响。Matorras等[12]利用脂质组学方法比较了接受IVF-ET助孕患者的着床期与非着床期的子宫内膜液,发现甘油磷脂及ω-6 PUFA发生明显改变,提示磷脂及PUFA对子宫容受性形成具有重要作用。多项研究发现PUFA尤其是必需脂肪酸,可能通过影响前列腺素、雌激素、孕激素合成以干扰子宫内膜容受性的建立。如Shahnazi等[13]在着床期前小鼠的饮食中添加ω-3及ω-6 PUFA,结果发现ω-3和ω-6 PUFA均可促进小鼠子宫内前列腺素E2(prostaglandin E2,PGE2)合成酶的表达,且PGE2合成酶微粒体mRNA的表达与着床率呈正相关。Zeng等[14]也证实,富含必需脂肪酸的饮食可明显增加卵泡中雌、孕激素的浓度,并促进胚胎着床。
包含PGE2、PGF、前列腺素I2等亚型,参与包括排卵、月经来潮、着床及子宫收缩等在内的许多生理过程,其中PGE2对女性生育能力的不同阶段均有积极影响。PGE2可通过促进卵丘颗粒细胞扩张诱导卵母细胞减数分裂成熟,进而参与排卵级联反应,且可通过维持黄体功能及诱导激活趋化因子促进胚胎发育、早期着床,以及子宫的血管扩张等[5,15]。Moraes等[6]发现,着床期间子宫腔中PGE2、PGF浓度明显增高,提示PG有利于胚胎着床。而反复IVF-ET失败的女性PG合成减少,提示人子宫内膜PG合成异常可导致子宫内膜容受性降低[16]。PG合成酶也在子宫内膜容受性的建立及调控胚胎着床中发挥关键作用。Vilella等[17]发现,与容受前期相比,容受期的子宫上皮细胞中环加氧酶(cyclooxygenase,COX-2)表达水平明显升高。且反复着床失败患者的子宫内膜PG合成异常,以COX-2及胞质磷脂酶A2(cytoplasmic phospholipase A2,cPLA2)水平明显降低为特征。关于PG影响子宫内膜容受性及胚胎着床的潜在机制,有研究认为着床期间高水平的PGE2可增加子宫血管的通透性及血流量,并招募炎性细胞因子、趋化因子及免疫细胞,促进子宫内膜向容受状态转化[18-19]。此外,Baryla等[20]的体外研究发现,子宫内膜PGF可通过与其受体结合激活丝裂原活化蛋白激酶信号通路,诱导人绒毛外滋养细胞系对子宫基质细胞的黏附,进而促进胚胎着床。总之,子宫中PG尤其是PGE2和PGF可正向调控子宫内膜容受性的建立及胚胎着床。
eCBs系统(endocannabinoid system,ECS)广泛分布于子宫内膜等整个生殖道,由eCBs、大麻素受体(cannabinoid receptor,CBR)1/2及其相关代谢酶构成。近年来,ECS通过下丘脑-垂体-卵巢轴对女性生殖健康的影响备受关注,尤其是eCBs中的N-花生四烯酸氨基乙醇(N-arachidonic acid aminoethanol,AEA)及CBR1/2对卵子发生、蜕膜化、胚胎着床及胎盘发育的影响被广泛研究[21-22]。Almada等[23]发现,容受期子宫内AEA水平降低可促进早期妊娠的建立及维持。而Maia等[24]发现,非容受期子宫内AEA及CBR1均高表达,且容受期外周血中较高水平的AEA与自发性妊娠失败有关。以上研究均提示,高浓度的AEA不利于子宫内膜容受性的建立及胚胎着床。对此,有研究指出,AEA可能通过直接作用于子宫内膜基质细胞分化,抑制PG的生物合成,进而抑制蜕膜化所需的血管重塑,最终导致流产[20]。此外,也有研究认为,AEA是通过下调细胞色素P450超家族成员CYP19A1 mRNA的表达及雌激素的分泌,诱导抗芳香化酶作用,进而导致了蜕膜化受损[25]。eCBs信号通路通过激活CBR1、CBR2发挥生物学效应,因此,CBR1、CBR2的缺陷均可导致妊娠失败。Li等[26]从转录水平研究发现,双重敲除编码CBR1/2的Cnr1Cnr2以沉默eCBs信号不利于早期妊娠的建立,同时还会增加妊娠中期流产及蜕膜化受损的风险。
LPA由自分泌运动因子(secretase autotaxin,ATX)水解溶血磷脂或由cPLA2水解磷脂酸产生,并通过与其同源受体LPA1-6结合发挥生物学效应。LPA在生殖领域也发挥重要的调控作用,如参与排卵、受精、早期胚胎发育、子宫蜕膜化及血管生成、重构等。Ye[27]发现,LPA主要通过与LPA3结合触发子宫内膜容受性及类固醇激素信号转导来调节局部平衡。此外,Achache等[16]发现,LPA3在围着床期子宫上皮细胞中高表达,且在反复着床失败患者中的表达明显降低。以上研究均提示LPA-LPA3信号通路参与了子宫内膜容受性的建立及胚胎着床。而LPA影响子宫内膜容受性及胚胎着床的具体机制仍在探索阶段。Aikawa等[28]发现,在小鼠子宫蜕膜化过程中,ATX-LPA-LPA3信号通路可诱导肝素结合表皮生长因子及COX2的表达,调控子宫内膜血管发育及基质细胞增殖,提示LPA3信号通路位于PG合成的上游,可能通过PG间接影响胚胎着床。但也有研究认为,LPA-LPA3信号通路直接参与了母胎界面的血管重构,保证充足的血流量以应对子宫内膜向蜕膜的转变、胚胎着床生长,以及日益增加的代谢需求[29]
鞘磷脂代谢可产生许多信号分子,如神经酰胺、鞘氨醇及1-磷酸鞘氨醇(sphingosine-1-iphosphate,S1P),参与调控细胞生长、衰老等信号转导。Ding等[30]发现,小鼠的子宫基质细胞在孕激素调控下发生蜕膜化,在此过程中,鞘磷脂合成被高度激活,若阻碍这一合成通路将导致着床位点明显减少及蜕膜化过程缺陷。S1P是一种多变的脂质介质,与其受体S1P1-5结合参与调节细胞增殖、迁移、免疫细胞招募及子宫内膜蜕膜化[31]。进一步研究发现,S1P促进子宫蜕膜化的可能机制是促进子宫血管生成[32]。Skaznik-Wikiel等[33]证实,S1P也可诱导子宫蜕膜基质细胞中COX-2的高表达,提示鞘磷脂代谢与PG信号通路在蜕膜化组织中存在联系,进而影响子宫蜕膜化质量及胚胎着床。作为S1P合成的关键酶,鞘氨醇激酶(sphingosine kinase 1/2,SphK1/2)在内膜蜕膜化、胚胎着床中也发挥重要作用。Mizugishi等[34]发现,SphK-/-SphK-/+小鼠子宫蜕膜基质细胞多在孕早期死亡,且SphK缺乏也可引起未分化的子宫内膜基质细胞增殖减少及蜕膜血管大量破裂,使蜕膜化受损而无法受孕。SphK1/S1P能够参与免疫炎症反应,巨噬细胞等免疫细胞群体对其具有感知能力,从而激活免疫系统发挥免疫作用[35-36],故中性粒细胞的大量浸润及母体的过度免疫反应可能是SphK缺陷介导妊娠丢失的另一种可能机制。
雌、孕激素是由胆固醇衍生的两种类固醇激素,二者协同调节子宫内膜的结构及功能改变,尤其对子宫内膜容受性的建立至关重要,为囊胚植入做准备。Suthaporn等[37]证实,孕激素可通过与其同源核受体结合激活下游通路,驱动间质细胞产生蜕膜样变,从而促使子宫内膜转化为容受状态,为胚胎着床做准备。此前,Csabai等[38]发现,孕激素诱导的阻断因子缺乏可改变子宫免疫反应(包括T细胞活化受损、Th1分化紊乱及自然杀伤细胞活性增加),导致小鼠胚胎植入失败。因此,临床上对疑似黄体不足的孕妇,常规建议补充孕激素以支持黄体继续维持妊娠。在卵泡期,雌激素刺激子宫内膜上皮细胞增殖,并诱导孕激素受体募集,是实现子宫内膜容受性的必要步骤[39]。Zhang等[40]证实,雌激素通过细胞外信号调节激酶(extracellular signal-regulated kinase,ERK)-p65信号轴调节内质网应激介导的凋亡,可促进子宫内膜的血管生成,从而改善子宫内膜容受性。雌、孕激素也可通过影响PG生成影响子宫内膜容受性及胚胎着床。有研究发现,雌、孕激素可增加子宫PGF的分泌[41]。以上研究表明,月经周期中雌、孕激素水平对子宫内膜容受性的建立及胚胎着床至关重要,但其确切的作用机制仍有待进一步研究。
PCOS患者子宫内膜容受性受损可能与其胚胎植入率低、早期流产率高等不良妊娠结局有关。研究发现,PCOS患者存在孕激素抵抗现象,导致胞饮突在着床期表达不良[42],进而影响子宫内膜容受性的建立[43]。此外,Regidor等[44]发现,与健康对照组相比,PCOS女性AA的代谢产物如COX、PG的表达异常。PCOS患者的AEA水平明显高于健康对照组(P<0.05)[45]。综上所述,PCOS患者脂代谢异常可降低子宫内膜容受性,导致其生育力受损。
子宫内膜异位症(endometriosis,EMs)可增加育龄期妇女不孕的风险,可能与其子宫内膜容受性受损有关[46]。目前,EMs的发病机制尚未明确,有研究证实,ECS通过影响细胞增殖、凋亡、免疫等生物过程参与EMs的发生发展[24]。Andrieu等[47]发现,EMs患者增殖期的2-花生四烯酸甘油水平较对照组明显升高,而AEA水平明显下降(P<0.05)。Wei等[48]证实,EMs患者在分泌期中晚期(围着床期)子宫内膜LPA3水平明显降低,且LPA3表达水平降低与其他子宫内膜容受性生物标志物如骨桥蛋白、同源异型盒基因A10的表达密切相关。由于这些基因表达均受孕激素调控,故这些基因的表达降低可能与EMs相关的孕激素抵抗有关。以上研究表明,EMs患者脂代谢异常可影响其子宫内膜容受性及胚胎着床,最终导致生育力降低。
孕早期乃至孕前期母体的脂肪酸、磷脂、胆固醇代谢(如PUFA、PG、eCBs等脂质介质)参与了子宫内膜容受性的建立及胚胎着床,并产生正向或负向影响[49]。脂质代谢对女性生殖健康具有重要影响。本文通过阐述对脂质介质及其信号通路调控子宫内膜蜕膜化及胚胎植入的分子机制,有利于更好地指导相关不孕症的治疗,为预测及改善自然妊娠和(或)辅助生殖助孕的结局提供新的切入点及可行的干预途径。但是,脂质代谢的研究目前仍存在一定的局限性,如脂质种类多样、结构复杂等增加了脂质代谢分析的困难,进而导致脂质代谢对生殖调控的基础及临床研究相对较少。此外,目前的研究大多数源于动物实验,基于人体试验的数据相对较少。相信随着脂质组学等多组学技术的迅速发展,未来会有更多基于人体研究的循证证据来探讨脂质代谢对女性生殖过程的调控机制,为促进女性生殖健康提供新的思路及策略。
