Refractory prolactinoma is the most common pituitary neuroendocrine tumor. Dopamine receptor agonists (DA) are the primary choice for drug treatment. Most patients with prolactinomas respond well to DA. However, a minority of prolactinomas patients still show resistance to DA. Although drug-resistant and refractory prolactinomas are rare in clinical practice, their treatment is extremely challenging. Even a combination of drug therapy, multiple surgeries, and radiotherapy may not yield satisfactory outcomes. Therefore, standardizing the diagnosis and treatment process and pathway for refractory prolactionmas and exploring more effective multidisciplinary collaborative treatment strategies are urgent problems to be solved. In the clinical management of refractory prolactinomas, it is often necessary to consider the patient's condition comprehensively, replace other types of DA, or consider surgery, radiotherapy, and immunotherapy, which requires multidisciplinary diagnosis and treatment. This review synthesizes the latest literature at home and abroad to systematically discuss the latest advances in drug therapy, surgery, and radiotherapy treatments for refractory prolactionmas, aiming to provide new ideas for basic research, clinical diagnosis and treatment.

垂体催乳素瘤是常见的垂体神经内分泌肿瘤,一般首选药物治疗,主要为多巴胺受体激动剂(DA)。大部分催乳素瘤患者对DA反应良好,然而,仍有少数患者对DA表现出抵抗性、耐药性。DA抵抗型难治性催乳素瘤临床少见,但其治疗极其困难,即使是药物治疗联合多次手术和放射治疗,也不能达到满意的疗效。因此,规范难治性催乳素瘤的诊疗流程和路径、探索更为有效的多学科协作治疗策略是目前亟待解决的难题。临床针对难治性催乳素瘤常需要综合患者情况,更换其他种类的DA,或考虑手术、放疗以及免疫治疗等,此过程需要多学科联合诊治。本文综合国内外最新文献,系统论述了难治性催乳素瘤的治疗药物、手术及放射治疗的最新进展,以期为难治性催乳素瘤的基础研究和临床诊治提供新的思路。

垂体催乳素瘤是常见的垂体神经内分泌肿瘤,一般首选药物治疗,主要为多巴胺受体激动剂(DA)。大部分催乳素瘤患者对DA反应良好,然而,仍有少数患者对DA表现出抵抗性、耐药性。DA抵抗型难治性催乳素瘤临床少见,但其治疗极其困难,即使是药物治疗联合多次手术和放射治疗,也不能达到满意的疗效。因此,规范难治性催乳素瘤的诊疗流程和路径、探索更为有效的多学科协作治疗策略是目前亟待解决的难题。临床针对难治性催乳素瘤常需要综合患者情况,更换其他种类的DA,或考虑手术、放疗以及免疫治疗等,此过程需要多学科联合诊治。本文综合国内外最新文献,系统论述了难治性催乳素瘤的治疗药物、手术及放射治疗的最新进展,以期为难治性催乳素瘤的基础研究和临床诊治提供新的思路。

Refractory prolactinoma is the most common pituitary neuroendocrine tumor. Dopamine receptor agonists (DA) are the primary choice for drug treatment. Most patients with prolactinomas respond well to DA. However, a minority of prolactinomas patients still show resistance to DA. Although drug-resistant and refractory prolactinomas are rare in clinical practice, their treatment is extremely challenging. Even a combination of drug therapy, multiple surgeries, and radiotherapy may not yield satisfactory outcomes. Therefore, standardizing the diagnosis and treatment process and pathway for refractory prolactionmas and exploring more effective multidisciplinary collaborative treatment strategies are urgent problems to be solved. In the clinical management of refractory prolactinomas, it is often necessary to consider the patient's condition comprehensively, replace other types of DA, or consider surgery, radiotherapy, and immunotherapy, which requires multidisciplinary diagnosis and treatment. This review synthesizes the latest literature at home and abroad to systematically discuss the latest advances in drug therapy, surgery, and radiotherapy treatments for refractory prolactionmas, aiming to provide new ideas for basic research, clinical diagnosis and treatment.

催乳素瘤是常见的垂体神经内分泌肿瘤(pituitary neuroendocrine tumors,pitNETs),占功能性垂体腺瘤的50%以上[1]。女性最常见的症状为月经不调、闭经、溢乳和不孕症,男性可出现性欲减退、勃起功能障碍、不育症、骨质疏松等症状。微腺瘤常见于年轻女性,在起病初期即可出现闭经、泌乳等典型症状,而男性和绝经后女性微腺瘤缺乏典型的临床特征,因此易发展为较大的腺瘤[2]。催乳素(prolactin,PRL)大腺瘤或巨大腺瘤可造成占位效应并引起相应的症状,主要包括头痛、视力下降、视野缺损等神经功能障碍,对患者健康产生巨大威胁,甚至发生垂体卒中,危及生命。
近年来,药物在垂体肿瘤治疗中的地位逐步提升。催乳素瘤首选药物治疗,且目前已成为临床共识。其一线用药为多巴胺受体激动剂(dopamine receptor agonist,DA),主要包括溴隐亭(bromocriptine,BRC)和卡麦角林(cabergoline,CAB)[3]。90%的催乳素瘤患者对BRC和CAB反应良好,药物治疗后肿瘤体积明显缩小。然而,仍有10%~15%的催乳素瘤患者对DA治疗反应不敏感,存在抵抗,此部分患者很难通过药物治疗达到临床缓解[4]。此外,DA抵抗型催乳素瘤往往具有肿瘤体积大、增殖指数高的特点,易侵袭海绵窦、颈内动脉、脑神经等周围重要结构,导致手术难度大、全切率低、复发率高,仅约34%的DA抵抗型患者术后能够达到临床缓解[5]。对于不能完全切除的DA抵抗型催乳素瘤患者,即使术后予以辅助放化疗等手段,有时也难以达到理想的治疗效果。因此,2017版WHO垂体腺瘤新分类中正式提出了“难治性垂体腺瘤”的概念,难治性垂体腺瘤的特点是肿瘤呈侵袭性生长,且生长快速,虽经手术、药物治疗及放疗等多种模式的标准化治疗后,肿瘤仍继续生长和(或)激素超量分泌,严重影响患者的生活质量,甚至危及生命[6]。难治性催乳素瘤的治疗依旧是亟待解决的难题,但近年来新的药物和临床研究不断涌现,为其诊疗提供了新的思路。
DA抵抗是指使用最大可耐受剂量的DA仍不能使血PRL水平恢复至正常范围,和(或)垂体瘤体积缩小未能达到原始体积的50%。一般来说,指南普遍推荐BRC的最大剂量为15 mg/d,CAB的最大剂量为2 mg/周[3]。但在使用BRC的患者中,约1/3需加量至20~30 mg/d;而在使用CAB达到2 mg/周的患者中,继续加量至3 mg/周,仅有约1/3的患者PRL可下降至正常水平,部分患者甚至需要高达12 mg/周的剂量才能使PRL恢复至正常水平[7]。因而对于最大可耐受剂量仍存在争议,但目前比较认可的是BRC为20~30 mg/d,CAB为4 mg/周,治疗3~6个月仍未达到临床缓解即可定义为DA抵抗。在此定义下的临床研究表明,DA抵抗在BRC治疗中可能更为常见,占BRC治疗患者的20%~30%,而在接受CAB治疗的患者中仅占10%~25%[3]
目前关于垂体催乳素瘤DA耐药机制的研究主要集中在以下几个方面:(1)多巴胺D2受体(dopamine D2 receptor,DRD2)及其介导的下游信号通路。部分DA耐药与催乳素瘤DRD2 mRNA表达水平降低有关。DRD2存在异构体,有DRD2s(short)和DRD2l(long)两种亚型,DRD2s亚型的比例越高,组织对DA的反应性越好;与DA敏感者相比,DRD2l mRNA表达水平在DA耐药和继发性耐药者中明显降低[8]。此外,DRD2还可调控细丝蛋白A(filamin-A)的表达,在DA敏感的催乳素瘤患者中,细丝蛋白A表达静默,而当细丝蛋白A过表达时,催乳素瘤患者则表现出明显的DA抵抗[7]。(2)TGF-β通路。TGF-β1可通过类似于DA的方式抑制PRL的分泌和催乳素细胞的增殖。敲除DRD2或使用DRD2拮抗剂(舒必利,sulpiride)可减少催乳素细胞TGF-β1的分泌和TGF-β 型受体(TGF-βR)的表达。与健康人相比,在因DA耐药而接受手术的患者中,其垂体TGF-β/Smad信号通路明显下调[9]。TGF-β1可抑制催乳素细胞的增殖以及PRL的合成与分泌,因而恢复局部TGF-β1的活性可能是提高DA抵抗型催乳素瘤治疗效果的一种新策略[10]。(3)遗传综合征背景下的DA抵抗。多发性内分泌腺瘤综合征1型(multiple endocrine neoplasia type-1,MEN-1)和MEN-4患者发生DA抵抗的比例明显增高。存在MEN1基因突变(MEN1mut)的患者更容易出现DA抵抗,在组织学上,与无MEN1突变的催乳素瘤相比,存在MEN1基因突变的催乳素瘤通常更具侵袭性;在MEN-4和家族性孤立性垂体腺瘤(familial isolated pituitary adenomas,FIPA)综合征患者中,催乳素瘤瘤体往往更大,更具侵袭性,对DA的反应更弱[11]。当发生芳烃相互作用蛋白(arylhydrocarbon interacting protein,AIP)基因突变时,77%的男性患者肿瘤较大,对DA的敏感性也较低[12]。(4)DA抵抗中的其他分子机制。研究发现,DA耐药组中Focal adhesion信号通路明显被激活,其通路分子FAK及Paxillin的表达水平明显升高[13]。此外,microRNA的异常也参与了DA抵抗,如miR-93-5p、miR-1299、miR-145等。还有许多新的靶基因不断被证实与催乳素瘤的发生发展密切相关,如SF3B1基因突变可能会引起垂体瘤细胞的过度增殖及PRL分泌增多,早期生长应答因子1(early growth response 1,EGR1)激动剂genipin可抑制催乳素瘤细胞的增殖,pou6f2基因可抑制MMQ细胞(大鼠垂体瘤细胞)的增殖[10]
BRC是最早用于治疗催乳素瘤的DA,为我国推荐治疗催乳素瘤的首选药物,起始应从小剂量开始,一般于晚餐后或睡前服用,待患者逐渐耐受和适应后,缓慢滴定至最大剂量,一般不超过20 mg/d。而对于已经使用到BRC最大可耐受剂量的DA抵抗患者,可考虑换用长效的高选择性DRD2激动剂,如CAB、喹高利特(仅在欧洲获批),并逐渐增加至最大可耐受剂量。与BRC相比,CAB对DRD2具有更高的亲和力和更强的细胞杀伤作用,因此其治疗效果及耐受性更好,已成为美国的一线用药[3]。CAB半衰期较BRC长(2~3 d),故起始剂量一般为0.25 mg(1次/周),逐渐加量至0.5~4.0 mg(2次/周)。研究表明,在催乳素微腺瘤和催乳素大腺瘤患者中,CAB可分别使95%和80%的患者PRL正常化及肿瘤缩小[4]。仅对BRC耐药的催乳素瘤患者中,在换用CAB后约有80%的患者PRL可降至正常水平,而对BRC和喹高利特双重耐药的患者中,在换用CAB后也有81%的患者催乳素恢复至正常水平,50%的患者肿瘤体积缩小至50%,92%的患者临床症状得到明显缓解[14]。对CAB耐药的患者最常见的治疗方法是增加CAB的剂量。虽然对CAB耐药的患者一般不太可能表现出对BRC敏感,但有文献报道2例分别为催乳素微腺瘤和催乳素大腺瘤的患者在对CAB耐药后,换用BRC后的治疗反应极好。需要注意的是,CAB可能存在诱发心脏瓣膜病的风险,在长时间接受高剂量CAB治疗的帕金森病患者中,累积剂量达到2000 mg后,肺胸膜和胸膜后纤维化的风险增加[15]。尽管应用常规剂量的CAB治疗催乳素瘤是安全的,不会使患者暴露于发生瓣膜病变的危险中,但难治性催乳素瘤患者接受长时间大剂量治疗时,CAB累积剂量增大,也可增加相关的风险[16]。此外,大剂量使用BRC时需要关注血管周围纤维化、肺及腹膜后纤维化等少见的严重不良反应。因此,英国内分泌学会、超声心动图学会和心瓣膜协会2019年发布联合声明,针对接受DA治疗的高催乳素患者,建议在开始使用DA之前常规进行心脏超声检查,CAB 2 mg/周以下剂量者每5年监测一次心脏超声,2 mg/周以上剂量者需每年监测一次,而对于有瓣膜基础疾病者,不论剂量如何均需每年监测一次[17]
非一线DA激动剂如培高利特(pergolide)已因心脏瓣膜纤维化的不良反应在2007年退市。新一代药物QNA已在欧洲批准上市。QNA是一种非麦角衍生的DRD2选择性激动剂,常规给药剂量为150~300 μg/d,1次/d。值得注意的是,QNA在开始使用的前3 d剂量为25 μg/d,之后3 d加量至50 μg/d,从第7天开始,剂量可调整为75 μg/d,后续视PRL水平可逐渐增加至最大可耐受剂量。QNA可使90%催乳素大腺瘤患者的肿瘤明显缩小,PRL水平明显降低,约50%的患者PRL水平可降至正常范围。与BRC治疗相比,QNA给药期间恶心、呕吐、头晕和嗜睡症状较轻[18]
麦角乙脲(lisuride)是另一种麦角衍生物DA,具有抑制PRL分泌的能力。特麦角脲(terguride)类似麦角乙脲,可使少数催乳素微腺瘤患者的PRL水平降至正常范围,且可缩小肿瘤体积,但目前尚未投入商业生产。
在DA应用于临床之前,手术是治疗催乳素瘤的主要手段[19]。目前的手术适应证主要有:(1)对DA不能耐受;(2)对DA治疗无效的催乳素瘤(耐药或逃逸);(3)巨大垂体催乳素瘤伴有明显视力视野障碍,药物治疗一段时间(2周)后无明显缓解;(4)自发或DA诱导的脑脊液漏;(5)合并精神疾病,且使用DA治疗仍加重;(6)催乳素大腺瘤患者计划短期内怀孕;(7)患者偏好(拒绝长期服用药物治疗)。近十年来,随着神经内镜技术的普及和精进,由经验丰富的神经外科医师行经蝶窦手术可使高达93%的催乳素微腺瘤和75%的催乳素大腺瘤患者实现PRL水平正常化。同时,神经内镜微创手术的围手术期及术后并发症发生率较低(0~4%),死亡发生率为0[20]。2022年意大利临床内分泌协会和国际临床内分泌学分会联合发布的催乳素瘤临床实践声明及2023年国际垂体协会发布的垂体催乳素瘤诊断和治疗国际共识声明指出,经蝶窦切除可作为根治性切除腺瘤的一线可选方案,包括微腺瘤和包裹良好/边界清晰的大腺瘤[21-22]。近期一项纳入184例行手术治疗的男性催乳素瘤患者的回顾性分析发现,178例为催乳素大腺瘤,其中61例具有侵袭性[即鞍上和(或)鞍旁和(或)蝶窦受侵袭],约10%(6/61)的侵袭性垂体瘤患者PRL水平术后即明显下降;长期随访中,约50%(30/61)的患者PRL水平逐步达到正常化。由此可见,手术对DA耐药的患者是一种有价值的治疗方法[23]。此外,手术切除后的标本应行肿瘤组织学分析,如发现与肿瘤侵袭性相关的病理标志物,如Ki-67和p53等,可为进一步针对性治疗提供依据。未来仍需更多的随机对照研究,针对特定亚型的微腺瘤和界限清楚的大腺瘤(Knosp分级0级和1级、Knosp分级2级),比较药物与手术的优劣性,以提供循证依据。
放射治疗是催乳素瘤的治疗手段之一,通常用于接受DA治疗和(或)经蝶窦手术后仍存在有临床意义和临床症状的血清PRL升高者。这意味着大多数情况下放射治疗是三线治疗手段。欧洲内分泌协会发布的侵袭性垂体瘤和垂体癌诊治指南指出,对存在侵袭性生长的垂体瘤,即使已接受手术治疗或药物治疗,也需考虑行放射治疗[24]。放射治疗主要包括常规放射治疗及立体定向放射治疗(stereotactic radiotherapy,SRT)/立体定向放射外科治疗(stereotactic radiosurgery,SRS),其中SRT/SRS的常用技术有伽马刀、直线加速器和射波刀等。与伽马刀放射外科治疗(GKRS)相比,常规放射治疗主要有以下不足:(1)激素缓解时间较GKRS延长5~10年,需要在治疗潜伏期进行有效的药物治疗;(2)存在发生不良反应的风险,包括垂体前叶功能减退(80%以上的患者)、视神经炎、放射性脑肿瘤、脑梗死和(或)认知功能障碍。这些晚期不良反应的发生时间多在放疗后的10~20年[25]。鉴于常规放疗的弊端,SRT/SRS是目前的优先选择方式,但当肿瘤过大(直径3 cm)、边界形态不规则或紧邻视神经(视交叉)时,常规放射治疗更安全。放射治疗对催乳素瘤的抗肿瘤疗效(残留肿瘤稳定或体积缩小)较好,在70%~100%的病例中观察到放射治疗的中位缓解时间为12~36个月。在一项针对DA抵抗或侵袭性催乳素大腺瘤患者的研究中,38例患者均接受SRS放射治疗,平均及中位照射剂量分别为21.3 Gy和25.0 Gy,治疗后50%的患者PRL水平降至正常范围内,提示SRS对DA不耐受或耐药抵抗的患者是一种有效的治疗方式。值得注意的是,SRS放射治疗起效较慢,中位缓解时间为20~40个月。此外,因DA可能具有放射保护作用,因此指南建议催乳素瘤患者在进行放射治疗时最好停用DA[26]
TMZ为常用于脑胶质瘤等恶性肿瘤的口服烷化剂。近年来,TMZ探索性用于治疗难治性垂体瘤并取得良好的疗效,是目前难治性催乳素瘤最常用的化疗药物[27],欧洲内分泌协会指南(2018年版)提到,TMZ可作为侵袭性垂体瘤和垂体癌的一线化疗药物[24],文献报道其对侵袭性垂体腺瘤和垂体癌的治疗有效率分别为55%和58%[28]。Almalki等[29]发现,在42例受试者(其中23例分泌型垂体腺瘤、19例垂体癌)中,接受TMZ治疗后76%的患者肿瘤体积明显缩小、激素水平降低,常见的不良反应有头痛、头晕、恶心、脱发等,3例出现白细胞和血小板减少,大多数患者未出现严重不良反应。除TMZ外,长春新碱、顺铂、洛莫司汀与5-氟尿嘧啶联合等也被探索性地用于治疗难治性垂体瘤,但疗效欠佳[27]。虽然TMZ疗效确切,不良反应较少,但部分患者可能出现耐药,在服药时间过长或剂量过大时也可能出现药物毒性反应。TMZ用于难治性催乳素瘤的起始剂量、维持剂量及治疗持续时间目前尚无明确的共识和规范。
SSA被广泛用于治疗生长激素腺瘤。因催乳素瘤细胞也同时表达生长抑素受体(somatostatin receptor,SSTR),如SSTR1、SSTR2和SSTR5等,SSA可与催乳素瘤细胞膜上的SSTR结合而抑制PRL的分泌。Sosa-Eroza等[30]发现,5例DA抵抗型催乳素大腺瘤患者接受奥曲肽和CAB联合治疗,其中1例治疗后PRL水平降低97.0%,肿瘤体积缩小93.0%;1例PRL水平降低81.0%,肿瘤体积缩小93.6%。另有1例个案报道表明,DA抵抗型催乳素大腺瘤患者使用CAB联合兰瑞肽不能使PRL降至正常水平,改为CAB联合帕瑞肽治疗1个月后,血浆PRL降至正常范围内,治疗3个月后闭经、溢乳等症状明显好转,仅糖化血红蛋白轻度升高,无其他不良反应[31]。分析其机制可能为SSTR5在催乳素瘤细胞中的表达水平更高,而帕瑞肽对SSTR5具有更高的选择性,因此在DA抵抗型催乳素瘤患者中,帕瑞肽可能较奥曲肽和兰瑞肽(主要激活SSTR2)更有效,提示新型SSA在难治性催乳素瘤的治疗中具有一定的价值。
雌激素可促进催乳素瘤细胞增殖,使其合成和分泌的PRL增多[32]。雌激素受体调节剂(estrogen receptor modulators,ERM)雷洛昔芬临床上主要用于预防绝经后女性骨质疏松。Choudhary等[33]发现,14例DA抵抗型催乳素瘤患者使用雷洛昔芬治疗后,10例PRL水平明显降低,平均下降约25.9%;2例PRL水平降至正常范围。ERM对DA抵抗型催乳素瘤的有效性仍有待更多基础和临床研究验证。
PRLRA可竞争性结合PRL受体,减少内源性PRL与PRL受体的结合,从而减轻高催乳素血症产生的不良反应[27]。PRLRA可能是治疗DA抵抗型催乳素瘤的新方向,但目前该类药物尚未投入临床使用,且其是否可抑制催乳素瘤细胞的增殖仍不清楚,尚需进一步临床验证。
此类药物主要包括:(1)哺乳动物雷帕霉素靶蛋白(mammalian target of rapamycin,mTOR)抑制剂。临床上主要用于预防排异反应以及治疗乳腺癌、神经内分泌癌、肾癌、结节性硬化症等疾病。有个案报道1例DA抵抗型催乳素瘤患者使用依维莫司联合CAB治疗5个月后,血PRL水平降低,肿瘤体积缩小[34]。基础研究也有类似发现,依维莫司联合CAB干预后,G3细胞(催乳素瘤模型)的PRL分泌受到抑制。(2)表皮生长因子受体-酪氨酸激酶抑制剂(epidermal growth factor receptor-tyrosine kinase inhibitors,EGFR-TKI)。EGFR基因的突变或异常表达在肿瘤的发生发展中起着重要作用,临床上以EGFR为靶点的药物主要用于非小细胞肺癌、乳腺癌、结直肠癌、头颈癌等的治疗。研究证实,EGFR/人表皮生长因子受体-2(HER-2)在正常垂体前叶细胞中的表达可能与垂体瘤的发生相关,且使用拉帕替尼治疗后小鼠垂体瘤模型的肿瘤体积明显缩小[35]。临床也有类似报道,2例DA抵抗型催乳素瘤患者在接受CAB联合拉帕替尼治疗6个月后,PRL水平分别降低78%和42%,其中1例肿瘤体积明显缩小[36]。(3)抗血管内皮生长因子(vascular endothelial growth factor,VEGF)靶向药物[37]。大多数恶性肿瘤中VEGF mRNA及蛋白呈高表达,与肿瘤侵袭、转移、复发和预后密切相关。因此,VEGF是抗肿瘤治疗的重要靶点。但在垂体瘤中抗VEGF治疗的效果尚存在争议。有研究表明,VEGF在PRL型、促肾上腺皮质激素(ACTH)型及无功能型垂体腺瘤中高表达[38],使用贝伐单抗联合DA治疗后肿瘤新生血管明显减少,肿瘤体积缩小。(4)神经生长因子(nerve growth factor,NGF)。NGF与肿瘤生长密切相关,一般而言,其对肿瘤细胞主要表现为抗凋亡作用,可促进肿瘤细胞的生长。研究发现,在DA敏感的催乳素瘤中NGF表达水平明显高于DA抵抗的催乳素瘤,推测NGF可提高催乳素瘤患者对DA的敏感性[39]。Missale等[40]发现,NGF干预后,DA抵抗型小鼠催乳素瘤细胞的D2受体表达上调,细胞增殖也受到抑制。但NGF不能透过血脑屏障,这也是NGF治疗DA抵抗的难治性催乳素瘤的难点。(5)TGF-β1激活或抑制药物。目前处于研发阶段,尚无药品面世。TGF-β1可抑制催乳素细胞增殖,减少PRL的合成与分泌,因此局部激活TGF-β1的活性可作为DA抵抗型催乳素瘤的治疗策略。研究发现,ABT-510、ABT-898等TGF-β1激活剂类似物可缩小肿瘤体积,使PRL水平下降[41]。但也有研究发现,侵袭性催乳素瘤和垂体大腺瘤的TGF-β1表达水平明显高于非侵袭性催乳素瘤和垂体微腺瘤,TGF-β1水平与肿瘤的侵袭性呈正相关,因此可通过抑制TGF-β1来抑制肿瘤的生长[42]。总之,目前靶向治疗及靶基因的临床证据仍不充分,疗效也存在争议,尚需进一步的临床及基础研究证实。
PRRT治疗是用放射性核素标记SSA,将放射性核素导向至SSTR高表达的细胞而发挥作用[43]。一项纳入3例难治性催乳素瘤患者(2例催乳素瘤,1例无功能性垂体瘤)的研究发现,进行PRRT(111In-DTPA-octreotide,177Lu-DOTATOC)治疗后,1例的残余肿瘤被控制,2例未见明显疗效;3例均未出现骨髓和肾脏等的不良反应[44]。但目前尚缺乏更多的大规模临床研究验证PRRT治疗的有效性和安全性。
近年来,随着肿瘤学及免疫学的不断发展,肿瘤免疫治疗也越来越受到关注。免疫治疗是指通过调动机体自身免疫系统,抑制和杀伤肿瘤细胞,主要包括细胞免疫疗法、肿瘤疫苗以及免疫检查点抑制剂等。程序性死亡受体-1(programmed death-1,PD-1)及其配体(programmed death-ligand 1,PD-L1)是目前广泛研究的免疫检查点,在垂体瘤方面的应用研究较多。Wang等[45]研究发现,PD-L1和CD8+ T淋巴细胞染色与PRL水平呈正相关,并与肿瘤侵袭性明显相关。1例DA抵抗型催乳素瘤患者接受免疫治疗后PRL水平降低,肿瘤体积缩小,且治疗过程中未见明显不良反应,证实免疫治疗对催乳素瘤有效[46]。研究发现,PD-L1在催乳素瘤及生长激素瘤中的表达明显高于其他类型的垂体瘤,提示免疫治疗可用于DA抵抗的难治性催乳素瘤[47],但其有效性和安全性仍缺少大样本临床数据支持,尚需更多基础及临床研究进一步验证。
二甲双胍是使用最广泛的降糖药,作为2型糖尿病治疗中的“神药”,同时具有抗肿瘤作用。多项研究表明,二甲双胍可抑制MMQ细胞增殖并促进其凋亡[48]。2例DA抵抗的难治性催乳素瘤患者使用DA与二甲双胍联合治疗后PRL水平降至正常范围,肿瘤体积明显缩小,提示二甲双胍对DA抵抗的难治性催乳素瘤可能有效,但仍缺乏大样本的临床研究证实。
氯喹主要用于治疗疟疾。临床研究发现,氯喹可增加肿瘤对化疗药物的敏感性,诱导肿瘤细胞凋亡,氯喹联合DA治疗可增强DA对垂体瘤细胞增殖的抑制作用,在雌激素诱导的垂体瘤动物模型中也观察到同样的结果[49]。一项个案报道中,1例DA抵抗型催乳素瘤患者在CAB基础上加用羟基氯喹200 mg/d后,其PRL水平降至正常范围[50]。但这些研究仅为少量个案报道,其具体机制仍有待进一步阐明。
大多数催乳素瘤的治疗并不复杂,通过药物或手术可达到治愈或长期缓解。然而,仍有10%~30%的催乳素瘤对DA表现出耐药性。虽然难治性催乳素瘤相对少见,但治疗极其复杂,有时即使是药物治疗联合多次手术和放疗,仍难以达到满意的效果。因此,规范难治性催乳素瘤的诊疗流程和路径,探索更为有效的多学科协作治疗策略是目前亟待解决的难题。在治疗过程中应在坚持以药物治疗作为基础的前提下,联合多学科治疗手段,采取阶梯式治疗策略,做好与难治性催乳素瘤长期斗争的准备,石以砥焉,化钝为利,通过坚持不懈的综合治疗,最终使难治性催乳素瘤得到理想的控制,提高患者的生存质量并延长预期寿命。
