Objective To explore the regulatory role of transient receptor potential channel 6(TRPC6) on podocyte autophagy under the influence of homocysteine (Hcy) in mouse kidney. Methods Mouse renal podocytes were divided into control group and Hcy groups (stimulated by Hcy at 40, 60, 80 and 100 μmol/L for 48 h). The level of TRPC6 mRNA was assessed using quantitative reverse transcription polymerase chain reaction (qRT⁃PCR) to identify the optimal Hcy concentration for subsequent experiments. Western blotting was employed to evaluate the expression levels of autophagy-related proteins LC3 Ⅱ and p62, as well as the expression levels of podocyte structural proteins Nephrin and Podocin. The expression levels of TRPC6 mRNA and protein in both groups were determined using qRT-PCR, Western blotting and immunofluorescence. Transfections of cells with TRPC6 overexpression or interference were set as follows: (1) control group (untreated), negative control group of TRPC6 overexpression, and TRPC6 overexpression group; (2) control group (untreated), negative control group of TRPC6 interference, and TRPC6 interference group (si-1, si-2, si-3).The expression level of TRPC6 was detected using qRT⁃PCR. The cells after overexpressing or interfering of TRPC6 were further set as follows: (1) control group (untreated), Hcy group (80 μmol/L Hcy added), TRPC6 overexpression control+Hcy group, TRPC6 overexpression+Hcy group; (2) control group (untreated), Hcy group, TRPC6 interference control+Hcy group, and TRPC6 interference+Hcy group. The expression levels of p62, LC3 Ⅱ, and TRPC6 proteins were detected using Western blotting. Results qRT⁃PCR detection results showed that compared with control group, the expression level of TRPC6 mRNA in Hcy group increased with the increase of Hcy concentration, with the highest expression level observed at 80 μmol/L Hcy. Therefore, 80 μmol/L Hcy was selected as the optimal concentration for intervention. At this time, the expression level of autophagy-related protein LC3 Ⅱ increased, and the expression level of p62 decreased (P<0.05). Western blotting results showed that compared with control group, the expression levels of podocyte-related proteins Nephrin and Podocin in Hcy group were significantly decreased (P<0.05). qRT⁃PCR results showed that compared with control group, the expression level of TRPC6 mRNA in Hcy group was significantly increased (P<0.05). Compared with negative control group for TRPC6 overexpression, both mRNA and protein expression levels of TRPC6 in TRPC6 overexpression group were significantly higher (P<0.05). Compared with negative control group for TRPC6 interference, both mRNA and protein expression levels of TRPC6 in TRPC6 interference group were significantly decreased (P<0.05). Western blotting results showed that compared with negative control group for TRPC6 overexpression, the expression level of autophagy-related protein LC3 Ⅱ in TRPC6 overexpression+Hcy group was significantly increased, and the expression level of p62 was significantly decreased (P<0.05). Compared with TRPC6 negative control+Hcy group for TRPC6 interference+Hcy, the expression level of autophagy-related protein LC3 Ⅱ in TRPC6 interference+Hcy group was significantly decreased, and the expression level of p62 was significantly increased (P<0.05). Conclusion Hcy can induce autophagy of renal podocytes. Inhibiting the expression of TRPC6 can significantly reduce the autophagy damage to podocytes.

目的 探讨在同型半胱氨酸(Hcy)作用下瞬时感受电位通道6(TRPC6)对小鼠肾脏足细胞自噬的调控作用。方法 将小鼠肾脏足细胞分为对照组与Hcy组(40、60、80、100 μmol/L的Hcy刺激48 h),采用qRT⁃PCR检测TRPC6 mRNA水平,筛选出Hcy的最佳浓度后,后续均以此浓度进行实验。采用Western blotting检测自噬相关蛋白LC3 Ⅱ、p62,以及足细胞结构蛋白Nephrin、Podocin的表达水平;采用qRT-PCR、Western blotting及免疫荧光检测两组TRPC6 mRNA及蛋白的表达水平。转染过表达或干扰TRPC6的细胞分别设置为:(1)对照组(不做处理)、TRPC6过表达阴性对照组、TRPC6过表达组;(2)对照组(不做处理)、TRPC6干扰阴性对照组和TRPC6干扰组(si-1、si-2、si-3),并采用qRT⁃PCR检测TRPC6表达水平。再将过表达或干扰TRPC6后的细胞分别设置为:(1)对照组(不做处理)、Hcy组(加入80 μmol/L Hcy)、TRPC6过表达对照+Hcy组、TRPC6过表达+Hcy组;(2)对照组(不做处理)、Hcy组、TRPC6干扰对照+Hcy组、TRPC6干扰+Hcy组,并采用Western blotting检测p62、LC3 Ⅱ、TRPC6蛋白的表达水平。结果 qRT⁃PCR检测结果显示,与对照组比较,TRPC6 mRNA表达水平随Hcy浓度增加而升高,其中80 μmol/L Hcy干预后的表达水平最高,因此将80 μmol/L作为Hcy的最佳干预浓度。此时,自噬相关蛋白LC3 Ⅱ的表达水平升高、p62表达水平降低(P<0.05)。Western blotting检测结果显示,与对照组比较,Hcy组中足细胞相关蛋白Nephrin、Podocin的表达水平明显降低(P<0.05)。qRT⁃PCR检测结果显示,与对照组比较,Hcy组中TRPC6 mRNA表达水平明显升高(P<0.05);与TRPC6过表达阴性对照组比较,TRPC6过表达组的TRPC6 mRNA及蛋白表达水平均明显升高(P<0.05);与TRPC6干扰阴性对照组比较,TRPC6干扰组的TRPC6 mRNA及蛋白表达水平均明显降低(P<0.05)。Western blotting检测结果显示,与TRPC6过表达对照组相比,TRPC6过表达+Hcy组中自噬相关蛋白LC3 Ⅱ表达水平明显升高,p62表达水平明显降低(P<0.05);与TRPC6干扰对照+Hcy组相比,TRPC6干扰+Hcy组中自噬相关蛋白LC3 Ⅱ表达水平明显降低,p62表达水平明显升高(P<0.05)。结论 Hcy可诱导小鼠肾脏足细胞自噬,抑制TRPC6的表达可明显减轻足细胞的自噬性损伤。

目的 探讨在同型半胱氨酸(Hcy)作用下瞬时感受电位通道6(TRPC6)对小鼠肾脏足细胞自噬的调控作用。方法 将小鼠肾脏足细胞分为对照组与Hcy组(40、60、80、100 μmol/L的Hcy刺激48 h),采用qRT⁃PCR检测TRPC6 mRNA水平,筛选出Hcy的最佳浓度后,后续均以此浓度进行实验。采用Western blotting检测自噬相关蛋白LC3 Ⅱ、p62,以及足细胞结构蛋白Nephrin、Podocin的表达水平;采用qRT-PCR、Western blotting及免疫荧光检测两组TRPC6 mRNA及蛋白的表达水平。转染过表达或干扰TRPC6的细胞分别设置为:(1)对照组(不做处理)、TRPC6过表达阴性对照组、TRPC6过表达组;(2)对照组(不做处理)、TRPC6干扰阴性对照组和TRPC6干扰组(si-1、si-2、si-3),并采用qRT⁃PCR检测TRPC6表达水平。再将过表达或干扰TRPC6后的细胞分别设置为:(1)对照组(不做处理)、Hcy组(加入80 μmol/L Hcy)、TRPC6过表达对照+Hcy组、TRPC6过表达+Hcy组;(2)对照组(不做处理)、Hcy组、TRPC6干扰对照+Hcy组、TRPC6干扰+Hcy组,并采用Western blotting检测p62、LC3 Ⅱ、TRPC6蛋白的表达水平。结果 qRT⁃PCR检测结果显示,与对照组比较,TRPC6 mRNA表达水平随Hcy浓度增加而升高,其中80 μmol/L Hcy干预后的表达水平最高,因此将80 μmol/L作为Hcy的最佳干预浓度。此时,自噬相关蛋白LC3 Ⅱ的表达水平升高、p62表达水平降低(P<0.05)。Western blotting检测结果显示,与对照组比较,Hcy组中足细胞相关蛋白Nephrin、Podocin的表达水平明显降低(P<0.05)。qRT⁃PCR检测结果显示,与对照组比较,Hcy组中TRPC6 mRNA表达水平明显升高(P<0.05);与TRPC6过表达阴性对照组比较,TRPC6过表达组的TRPC6 mRNA及蛋白表达水平均明显升高(P<0.05);与TRPC6干扰阴性对照组比较,TRPC6干扰组的TRPC6 mRNA及蛋白表达水平均明显降低(P<0.05)。Western blotting检测结果显示,与TRPC6过表达对照组相比,TRPC6过表达+Hcy组中自噬相关蛋白LC3 Ⅱ表达水平明显升高,p62表达水平明显降低(P<0.05);与TRPC6干扰对照+Hcy组相比,TRPC6干扰+Hcy组中自噬相关蛋白LC3 Ⅱ表达水平明显降低,p62表达水平明显升高(P<0.05)。结论 Hcy可诱导小鼠肾脏足细胞自噬,抑制TRPC6的表达可明显减轻足细胞的自噬性损伤。

Objective To explore the regulatory role of transient receptor potential channel 6(TRPC6) on podocyte autophagy under the influence of homocysteine (Hcy) in mouse kidney. Methods Mouse renal podocytes were divided into control group and Hcy groups (stimulated by Hcy at 40, 60, 80 and 100 μmol/L for 48 h). The level of TRPC6 mRNA was assessed using quantitative reverse transcription polymerase chain reaction (qRT⁃PCR) to identify the optimal Hcy concentration for subsequent experiments. Western blotting was employed to evaluate the expression levels of autophagy-related proteins LC3 Ⅱ and p62, as well as the expression levels of podocyte structural proteins Nephrin and Podocin. The expression levels of TRPC6 mRNA and protein in both groups were determined using qRT-PCR, Western blotting and immunofluorescence. Transfections of cells with TRPC6 overexpression or interference were set as follows: (1) control group (untreated), negative control group of TRPC6 overexpression, and TRPC6 overexpression group; (2) control group (untreated), negative control group of TRPC6 interference, and TRPC6 interference group (si-1, si-2, si-3).The expression level of TRPC6 was detected using qRT⁃PCR. The cells after overexpressing or interfering of TRPC6 were further set as follows: (1) control group (untreated), Hcy group (80 μmol/L Hcy added), TRPC6 overexpression control+Hcy group, TRPC6 overexpression+Hcy group; (2) control group (untreated), Hcy group, TRPC6 interference control+Hcy group, and TRPC6 interference+Hcy group. The expression levels of p62, LC3 Ⅱ, and TRPC6 proteins were detected using Western blotting. Results qRT⁃PCR detection results showed that compared with control group, the expression level of TRPC6 mRNA in Hcy group increased with the increase of Hcy concentration, with the highest expression level observed at 80 μmol/L Hcy. Therefore, 80 μmol/L Hcy was selected as the optimal concentration for intervention. At this time, the expression level of autophagy-related protein LC3 Ⅱ increased, and the expression level of p62 decreased (P<0.05). Western blotting results showed that compared with control group, the expression levels of podocyte-related proteins Nephrin and Podocin in Hcy group were significantly decreased (P<0.05). qRT⁃PCR results showed that compared with control group, the expression level of TRPC6 mRNA in Hcy group was significantly increased (P<0.05). Compared with negative control group for TRPC6 overexpression, both mRNA and protein expression levels of TRPC6 in TRPC6 overexpression group were significantly higher (P<0.05). Compared with negative control group for TRPC6 interference, both mRNA and protein expression levels of TRPC6 in TRPC6 interference group were significantly decreased (P<0.05). Western blotting results showed that compared with negative control group for TRPC6 overexpression, the expression level of autophagy-related protein LC3 Ⅱ in TRPC6 overexpression+Hcy group was significantly increased, and the expression level of p62 was significantly decreased (P<0.05). Compared with TRPC6 negative control+Hcy group for TRPC6 interference+Hcy, the expression level of autophagy-related protein LC3 Ⅱ in TRPC6 interference+Hcy group was significantly decreased, and the expression level of p62 was significantly increased (P<0.05). Conclusion Hcy can induce autophagy of renal podocytes. Inhibiting the expression of TRPC6 can significantly reduce the autophagy damage to podocytes.

同型半胱氨酸(homocysteine,Hcy)是甲硫氨酸循环中的代谢产物[1]。目前,慢性肾脏疾病(chronic kidney disease,CKD)已成为危害健康的一大杀手[2-4]。相关研究显示,CKD患者占全球人口的10%~13%,而我国CKD总患病率为10.8%,估计有1.195亿CKD患者,而高同型半胱氨酸(hyperhomo-cysterinemia,HHcy)是CKD的诱发因素[5-8]。瞬时感受电位通道6(transient receptor potential channel 6,TRPC6)是一种非选择性Ca2+通道,对调节肾脏生理平衡起着关键作用,如近端小管经Na+-Ca2+交换体重吸收Ca2+,或经细胞旁途径吸收Ca2+[9-10]。有研究发现,TRPC6通道异常开放可增加致病的风险,且其在多种疾病中表达异常[10]。如Jain等[11]发现,TRPC6通道大量激活可使肺动脉压增高,导致疾病的发生发展;揭育祯等[12]发现,在胰岛β细胞发生细胞焦亡的过程中,TRPC6高表达与疾病的发生发展呈正相关,而TRPC6通道异常激活后则通过DNA甲基化的方式损伤胰岛β细胞,最终导致疾病的发生。目前越来越多的学者关注到TRPC6异常表达是影响疾病发展的关键因素之一,但其在Hcy诱导CKD中的作用鲜有报道[13]。本研究探讨TRPC6对Hcy诱导的足细胞自噬中的调控作用,旨在为深入研究CKD的发生发展提供实验依据。
DMEM培养基、胎牛血清购自美国Gibco公司;蛋白提取试剂盒购自上海凯基生物技术有限公司;总RNA提取试剂盒购自日本宝日医生物技术有限公司;反转录试剂盒、荧光PCR试剂盒购自北京天根生化科技有限公司。小鼠肾脏足细胞株(MPC-5)购自深圳豪地华拓生物科技有限公司;TRPC6干扰片段及过表达质粒购自上海吉玛制药技术有限公司。LSM 510 SYSTEM激光共聚焦显微镜购自德国ZEISS公司;电泳仪、电转仪、Model680全自动酶标仪购自美国Bio-Rad公司;超净工作台购自中国苏州安泰有限公司;CO2培养箱和5415D型微量台式离心机购自美国Eppendorf公司;BS110S型精密天平购自美国Sar-Toriu公司;荧光定量PCR仪购自上海枫岭生物技术有限公司;制冰机购自美国Scotsman公司。
用含有10%胎牛血清的DMEM培养液培养肾脏足细胞MPC-5,置于37 ℃、5% CO2培养箱中,2 d传代一次,取对数生长期(第3~4代)细胞用于实验。
(1)将细胞分为对照组及Hcy组,其中Hcy组浓度分别设置为0、40、60、80、100 μmol/L干预48 h,采用qRT⁃PCR检测TRPC6 mRNA水平,以筛选最佳浓度,后续均以此浓度进行实验。采用Western blotting检测两组LC3 Ⅱ、P62自噬相关蛋白的表达水平,以及足细胞结构蛋白Nephrin、podocin的表达水平;采用qRT-PCR、Western blotting及免疫荧光检测两组TRPC6 mRNA及蛋白的表达水平。(2)将转染过表达或干扰TRPC6的细胞分别设置为:①对照组(不做处理)、TRPC6过表达阴性对照组、TRPC6过表达组;②对照组(不做处理)、TRPC6干扰阴性对照组和TRPC6干扰组(si-1、si-2、si-3),并采用qRT⁃PCR检测过表达或干扰TRPC6后的足细胞TRPC6表达水平。(3)再将过表达或干扰TRPC6后的细胞分别设置为:①对照组(不做处理)、Hcy组(加入80 μmol/L Hcy)、TRPC6过表达对照+Hcy组、TRPC6过表达+Hcy组;②对照组(不做处理)、Hcy组、TRPC6干扰对照+Hcy组、TRPC6干扰+Hcy组,并采用Western blotting检测过表达或干扰TRPC6后的足细胞P62、LC3 Ⅱ、TRPC6蛋白的表达水平。
根据LipofectamineTM 2000说明书将Ad-TRPC6及si-TRPC6转染至肾脏足细胞中。转染前1 d,在250 μl的DMEM培养基中接种2×105个细胞。A液:250 μl DMEM纯培养基分别与Ad-TRPC6、si-TRPC6 5 μl混合,吹打2~3次混匀。B液:250 μl DMEM纯培养基与6 μl LipofectamineTM 2000混合,吹打2~3次混匀,静置5 min。A液加B液吹打2~3次混匀,室温下静置20 min。补充3.5 ml纯培养基混匀,将培养瓶置于37 ℃、5% CO2培养箱培养6~8 h,Hcy干预48 h后收取细胞,以便进行后续实验。
采用总RNA试剂盒提取小鼠肾脏足细胞的总RNA,通过NCBI查询TPRC6的ID序列号,经GeneBank查询上下游引物序列,引物由上海生物生工公司合成。TRPC6引物,上游:5'-CTGATCCTCAGATCATCTCT-3',下游:5'-GAGTAAAGGTTGAACATTCC-3';GAPDH引物,上游:5'-GGTTGTCTCCTGCGACTTCA-3',下游:5'-TGGTCCAGGGTTTCTTACTCC-3'。反转录反应体系:Prime Script buffer for Real Time 2 μl、Oligo Diprimer 1 μl、Random 1 μl、Prime Script RT Enlyme Mlxi 1 μl、H2O与RNA总体积之和为13 μl;PCR反应体系:TB Green 10 μl、上下游引物各0.8 μl、H2O 6.4 μl、反转录产物2 μl。qRT-PCR扩增反应,共45个循环。以小鼠GADPH为内参照,目的基因扩增后通过2-ΔΔCt计算目的基因相对表达量。
收集小鼠肾脏足细胞,5000 r/min离心5 min后弃上清,根据凯基试剂盒说明书提取总蛋白,BCA法测定蛋白浓度,加200 μl蛋白上样缓冲液,金属浴99 ℃煮沸5 min变性,配制8%、10% SDS-PAGE凝胶,经SDS-PAGE凝胶电泳80 V、20 min,120 V、40 min,湿转2 h后转移至PVDF膜上,5%脱脂奶粉封闭2 h,加入1:1000稀释的TRPC6、Nephrin、Podocin、LC3 Ⅱ及p62兔一抗4 ℃孵育过夜。PBST洗膜10 min×3次,用1:5000稀释的兔HRP二抗(ZB-2301,购自北京中杉金桥生物技术有限公司)孵育2 h,PBST洗膜10 min×3次。采用Image Lab软件扫描并采集图像,以β-action为内参照,计算TRPC6、Nephrin、Podocin、LC3 Ⅱ、p62的相对表达量。
将传代时生长状态良好的肾脏足细胞以70%的密度铺到共聚焦小皿中,Hcy干预48 h后收集细胞。PBS洗5 min×3次,4%多聚甲醛溶液固定30 min,PBS洗5 min×3次;0.2% Triton X-100溶液通透20 min,PBS洗5 min×3次;山羊血清覆盖封闭30 min,按照说明书配制一抗,置于4 ℃暗盒中孵育过夜。隔天取出小皿用PBS洗5 min×3次,加入荧光二抗,37 ℃避光孵育1 h,PBS洗5 min×3次;滴加DAPI液,避光孵育5 min,PBS洗5 min×3次,滴入适量封片淬灭剂后用LSM 510 SYSTEM激光共聚焦显微镜采集图像。
胰酶消化收集小鼠肾脏足细胞,制备细胞悬液,PBS洗5 min×2次;12 000 r/min离心5 min,用500 μl Annexin V结合细胞悬液,加入5 μl Annexin V-FITC和PI染色并标记单染管、双染管、空白管,在单染管仅加PI,双染管加Annexin V-FITC和PI,空白对照组管不加,静置20 min后上机检测足细胞凋亡率。
采用Prism 8.0软件进行统计分析。实验数据均为计量资料且符合正态分布以$\bar{x}±s$表示,两组间比较采用t检验;多组间比较采用单因素方差分析,进一步两两比较采用Tukey检验。P<0.05为差异有统计学意义。
qRT-PCR检测结果显示,与对照组比较,40、60、80 μmol/L Hcy组TRPC6 mRNA表达水平逐渐升高,但Hcy浓度达到100 μmol/L时,TRPC6 mRNA表达水平反而降低,因此,后续实验选择80 μmol/L作为最佳干预浓度(图1A)。Western blotting检测结果显示,与对照组比较,Hcy组TRPC6蛋白表达水平明显升高(P<0.05,图1B)。免疫荧光染色结果显示,与对照组比较,Hcy组TRPC6的免疫荧光表达强度明显增强(P<0.05,图1C)。
Western blotting检测结果显示,与对照组比较,Hcy组中Nephrin、Podocin蛋白表达水平明显降低(P<0.05,图2A),LC3 Ⅱ、p62蛋白表达水平明显升高(P<0.05,图2B)。流式细胞术检测结果显示,与对照组比较,Hcy组细胞凋亡率明显增高(P<0.05,图2C)。
qRT-PCR检测结果显示,对照组与TRPC6过表达阴性对照组的TRPC6 mRNA表达水平差异无统计学意义(P>0.05),提示转染成功;与TRPC6过表达阴性对照组比较,TRPC6过表达组TRPC6 mRNA表达水平明显增高,差异有统计学意义(P<0.05,图3A)。对照组与TRPC6干扰阴性对照组的TRPC6 mRNA表达水平差异无统计学意义(P>0.05),提示转染成功;与TRPC6干扰阴性对照组比较,TRPC6干扰组(si-1、si-2、si-3)TRPC6 mRNA表达水平明显降低(P<0.05,图3B),且si-1干扰效果最强,据此选择si-1干扰片段用于后续实验。
Western blotting检测结果显示,与对照组比较,Hcy组、TRPC6过表达对照+Hcy组及TRPC6过表达+Hcy组TRPC6蛋白表达水平明显升高(P<0.05或P<0.01);与TRPC6过表达对照+Hcy组比较,TRPC6过表达+Hcy组TRPC6蛋白表达水平明显升高(P<0.05);而Hcy组与TRPC6过表达对照+Hcy组TRPC6蛋白表达水平差异无统计学意义(P>0.05,图4A)。与对照组比较,Hcy组及TRPC6干扰对照+Hcy组TRPC6蛋白表达水平明显升高(P<0.05),而TRPC6干扰+Hcy组TRPC6蛋白表达水平明显降低(P<0.05);与TRPC6干扰对照+Hcy组比较,TRPC6干扰+Hcy组TRPC6蛋白表达水平明显降低(P<0.01);而Hcy组与TRPC6干扰对照+Hcy组TRPC6蛋白表达水平差异无统计学意义(P>0.05,图4B)。
Western blotting检测结果显示,与对照组比较,Hcy组、TRPC6过表达对照+Hcy组、TRPC6过表达+Hcy组中LC3 Ⅱ蛋白表达水平明显升高,p62蛋白表达水平明显降低(P<0.05或P<0.01);与TRPC6过表达对照+Hcy组比较,TRPC6过表达+Hcy组LC3 Ⅱ蛋白表达水平明显升高,p62蛋白表达水平明显降低(P<0.05);而Hcy组与TRPC6过表达对照+Hcy组LC3 Ⅱ、p62蛋白表达水平差异无统计学意义(P>0.05,图5A)。与对照组比较,Hcy组、TRPC6干扰对照+Hcy组及TRPC6干扰+Hcy组中LC3 II蛋白表达水平明显升高,p62蛋白表达水平明显降低(P<0.05);与TRPC6干扰对照+Hcy组比较,TRPC6干扰+Hcy组LC3 Ⅱ蛋白表达水平明显降低,p62蛋白表达水平明显升高(P<0.05);而Hcy组与TRPC6干扰对照+Hcy组LC3 Ⅱ、p62蛋白表达水平差异无统计学意义(P>0.05,图5B)。
肾脏足细胞是肾小球滤过膜的重要组成部分,位于肾小球基底膜(glomerular basement membrane,GBM)最外侧。足细胞损伤可使肾小球滤过率降低,是导致CKD的因素之一[14]。细胞自噬是以胞内吞噬降解为特点的一种程序性死亡方式;在生理状态下细胞自噬活性通常较低,但在病理状态下,细胞自噬明显增强。自噬作为一种防御机制,溶酶体降解是其重要途径之一。当机体出现衰老或蛋白质功能紊乱时,胞内溶酶体大量被激活、释放,吞噬衰老及功能异常的细胞,在人体内环境稳态中发挥重要作用[15-16]
多项研究证实了Hcy在CKD进展中的作用及其与细胞自噬的关系[17-19]。细胞自噬的特点是胞内物质的吞噬和降解,已被证实与多种疾病的发展密切相关[20-23]。本研究发现,Hcy可使足细胞结构蛋白Podocin和Nephrin的表达水平降低,并诱导足细胞自噬,表现为自噬相关蛋白LC3 Ⅱ表达水平升高,以及p62蛋白表达水平降低。以上结果提示,Hcy可通过上调自噬水平导致肾脏损伤。
此外,吴凡等[21]发现,Hcy能够抑制血管内皮细胞中SIRT1的表达,从而促进血管内皮细胞凋亡,而SIRT1水平升高则可减少Hcy诱导的血管内皮细胞凋亡及动脉粥样硬化斑块的形成。揭育祯等[12]发现,Hcy可促进胰岛β细胞焦亡,而TRPC6甲基化可能是这一过程的关键。基于此,笔者推测,在CKD中TRPC6也可促进肾脏足细胞的自噬,加速CKD的进展。本研究结果证实了这一推测,提示高水平的Hcy作为损伤因子,可能通过自噬性损伤来诱导疾病的发生。
TRPC6是一种非选择性钙离子通道,属于TRPC家族,在CKD中存在异常表达[24-29]。本研究发现,Hcy干预后TRPC6表达上调,进而引起足细胞自噬,导致肾脏损伤;干扰TRPC6的表达则可降低足细胞自噬水平,增加p62蛋白的表达,从而主动协同转运、降解自噬小体,减少LC3 Ⅰ向LC3B-Ⅱ的转换;相反,过表达TRPC6则会加重自噬性损伤,提示TRPC6的表达调控对于Hcy诱导的自噬水平具有明显影响,而自噬水平的改变对慢性肾损伤至关重要。由此可见,TRPC6能够调控自噬水平,减轻自噬对慢性肾损伤的影响,同时TRPC6还可作为一种新型的诊断标志物或治疗靶点,控制钙离子通道的开放和关闭,调控钙离子平衡,在慢性肾损伤的治疗中具有潜在的应用价值。
综上所述,本研究在揭示Hcy在CKD中的关键作用方面取得了重要进展,并为进一步理解自噬在疾病进展中的复杂作用机制提供了新的视角。临床上,对Hcy水平的监测可能成为早期诊断和干预CKD的有效手段,有望显著改善患者的预后。然而,本研究也存在一定的局限性:(1)主要是基于细胞水平进行验证,缺乏动物模型的支持,这可能影响了结果的普遍性和可靠性;(2)未充分考虑遗传因素和其他潜在的调控机制,这些因素可能与Hcy和自噬在CKD中的作用。未来的研究应致力于克服这些局限,在动物模型中验证研究结果,并考虑更多的因素,以进一步深化对Hcy和TRPC6在CKD中作用机制的理解,这不仅有望为CKD的治疗带来更多希望,也可能为其他相关疾病的治疗提供新的思路和方法。
