Objective To investigate the characteristics of coagulation function changes in patients with exertional heat illness (EHI) at different core temperatures(Tc). Methods A retrospective analysis was conducted on the clinical data of 346 EHI patients admitted to the emergency or intensive care units of 24 military hospitals from March 2021 to November 2022. According to the Tc at admission, patients were divided into 4 groups: Tc <39 ℃ group (n=223), 39 ℃ ≤Tc <40 ℃ group (n=60), 40 ℃ ≤Tc <41 ℃ group (n=35), 41 ℃ ≤Tc <42 ℃ group (n=17), and Tc ≥42 ℃ group (n=11). Based on the occurrence of heat stroke, the 346 EHI patients were further divided into heat stroke group (n=63) and non-heat stroke group (n=283). Basic information, complete blood count, coagulation function, liver and kidney function, and other laboratory indicators of the patients in each group were collected and statistically analyzed. Multifactorial logistic regression analysis was used to identify independent risk factors for the development of heat stroke in EHI patients. The diagnostic value of prothrombin time (PT), D-dimer, and platelet count for EHI patients developing heat stroke was assessed using the receiver operating characteristic (ROC) curve. Results When Tc exceeded 39 ℃, D-dimer levels in EHI patients increased significantly and further elevated with rising Tc (P<0.05). When Tc exceeded 40 ℃, platelet count and fibrinogen levels decreased, and PT was prolonged (P<0.05). When Tc exceeded 41 ℃, activated partial thromboplastin time (APTT) was significantly prolonged, and platelet count and fibrinogen level decreased (P<0.05). Multivariate logistics regression analysis showed that PT (OR=1.120, 95%CI 1.015-1.236), D-dimer (OR=1.322, 95%CI 1.129-1.549), and platelet count (OR=0.991, 95%CI 0.985-0.997) were independent risk factors for heat stroke (P<0.05). The area under the ROC curve (AUC) for D-dimer in diagnosing heat stroke was 0.796 (95%CI 0.732-0.860, P<0.001) with sensitivity and specificity of 69% and 80%, respectively, when D-dimer was greater than 0.9 μg/ml. The AUC for PT in diagnosing heat stroke was 0.708 (95%CI 0.628-0.788, P<0.001), with sensitivity and specificity of 42% and 97%, respectively, when PT was greater than 16.4 s. The AUC for platelet count in diagnosing heat stroke was 0.724 (95%CI 0.642-0.807, P<0.001), with the sensitivity and specificity of 52% and 94%, respectively, when the platelet count was less than 140×109/L. Conclusions The degree of Tc elevation in EHI patients is positively correlated with the severity of coagulation dysfunction. Prolonged PT, increased D-dimer level, and decreased platelet count are independent risk factors for the development of exertional heat stroke in EHI patients.

目的 探讨不同核心体温(Tc)的劳力性中暑(EHI)患者凝血功能的变化特征。方法 回顾性分析2021年3月-2022年11月24家军队医院急诊或重症医学科收治的346例EHI患者的临床资料,根据入院时的Tc分为Tc<39 ℃组(n=223)、39 ℃≤Tc<40 ℃组(n=60)、40 ℃≤Tc<41 ℃组(n=35)、41 ℃≤Tc<42 ℃组(n=17)和Tc≥42 ℃组(n=11)。根据是否发生热射病,将346例EHI患者分为热射病组(n=63)与非热射病组(n=283)。收集各组EHI患者的基础资料,血常规、凝血功能及肝肾功能等实验室检查结果,比较不同Tc组基础资料及实验室指标的差异。采用多因素logistic回归分析筛选EHI患者发生热射病的独立危险因素。采用受试者操作特征(ROC)曲线评估凝血酶原时间(PT)、D-二聚体、血小板计数对EHI患者发生热射病的诊断价值。结果 Tc超过39 ℃时,EHI患者的D-二聚体水平升高,且随Tc升高而进一步升高(P<0.05);Tc超过40 ℃时,EHI患者的血小板计数和纤维蛋白原水平降低,PT延长(P<0.05);Tc超过41 ℃时,EHI患者的活化部分凝血活酶时间(APTT)明显延长,血小板计数和纤维蛋白原水平降低(P<0.05)。多因素logistics回归分析显示,PT(OR=1.120,95%CI 1.015~1.236)、D-二聚体(OR=1.322,95%CI 1.129~1.549)、血小板计数(OR=0.991,95%CI 0.985~0.997)是热射病发生的独立危险因素(P<0.05)。D-二聚体诊断热射病的ROC曲线下面积(AUC)为0.796(95%CI 0.732~0.860,P<0.001),当D-二聚体>0.9 μg/ml时,诊断热射病的敏感度和特异度分别为69%和80%。PT诊断热射病的AUC为0.708(95%CI 0.628~0.788,P<0.001),当PT>16.4 s时,诊断热射病的敏感度和特异度分别为42%和97%。血小板计数诊断热射病的AUC为0.724(95%CI 0.642~0.807,P<0.001),当血小板计数<140×109/L时,诊断热射病的敏感度和特异度分别为52%和94%。结论 EHI患者的Tc升高幅度与凝血功能障碍严重程度呈正相关,PT延长、D-二聚体水平升高及血小板计数减少是EHI患者发生劳力性热射病的独立危险因素。

Objective To investigate the characteristics of coagulation function changes in patients with exertional heat illness (EHI) at different core temperatures(Tc). Methods A retrospective analysis was conducted on the clinical data of 346 EHI patients admitted to the emergency or intensive care units of 24 military hospitals from March 2021 to November 2022. According to the Tc at admission, patients were divided into 4 groups: Tc <39 ℃ group (n=223), 39 ℃ ≤Tc <40 ℃ group (n=60), 40 ℃ ≤Tc <41 ℃ group (n=35), 41 ℃ ≤Tc <42 ℃ group (n=17), and Tc ≥42 ℃ group (n=11). Based on the occurrence of heat stroke, the 346 EHI patients were further divided into heat stroke group (n=63) and non-heat stroke group (n=283). Basic information, complete blood count, coagulation function, liver and kidney function, and other laboratory indicators of the patients in each group were collected and statistically analyzed. Multifactorial logistic regression analysis was used to identify independent risk factors for the development of heat stroke in EHI patients. The diagnostic value of prothrombin time (PT), D-dimer, and platelet count for EHI patients developing heat stroke was assessed using the receiver operating characteristic (ROC) curve. Results When Tc exceeded 39 ℃, D-dimer levels in EHI patients increased significantly and further elevated with rising Tc (P<0.05). When Tc exceeded 40 ℃, platelet count and fibrinogen levels decreased, and PT was prolonged (P<0.05). When Tc exceeded 41 ℃, activated partial thromboplastin time (APTT) was significantly prolonged, and platelet count and fibrinogen level decreased (P<0.05). Multivariate logistics regression analysis showed that PT (OR=1.120, 95%CI 1.015-1.236), D-dimer (OR=1.322, 95%CI 1.129-1.549), and platelet count (OR=0.991, 95%CI 0.985-0.997) were independent risk factors for heat stroke (P<0.05). The area under the ROC curve (AUC) for D-dimer in diagnosing heat stroke was 0.796 (95%CI 0.732-0.860, P<0.001) with sensitivity and specificity of 69% and 80%, respectively, when D-dimer was greater than 0.9 μg/ml. The AUC for PT in diagnosing heat stroke was 0.708 (95%CI 0.628-0.788, P<0.001), with sensitivity and specificity of 42% and 97%, respectively, when PT was greater than 16.4 s. The AUC for platelet count in diagnosing heat stroke was 0.724 (95%CI 0.642-0.807, P<0.001), with the sensitivity and specificity of 52% and 94%, respectively, when the platelet count was less than 140×109/L. Conclusions The degree of Tc elevation in EHI patients is positively correlated with the severity of coagulation dysfunction. Prolonged PT, increased D-dimer level, and decreased platelet count are independent risk factors for the development of exertional heat stroke in EHI patients.

劳力性中暑(exertional heat illness,EHI)是在高温高湿环境下进行高强度体力活动后,人体体温调节失衡引起热代谢紊乱的疾病[1]。劳力性热射病是最严重的中暑类型,是人体核心体温(core temperature,Tc)超过40 ℃且以中枢神经系统功能障碍为特征的综合征,具有进展快、病情重、病死率高的特点[2]。凝血功能障碍是EHI的常见并发症[3-4],临床表现为皮肤淤点、淤斑、穿刺点出血、结膜出血、黑便、咯血、血尿,甚至发生颅内出血。据报道,合并凝血功能障碍的中暑患者病死率可达48%左右[5]。Tc升高是导致EHI相关凝血障碍的重要因素[6],但Tc升高幅度与凝血功能障碍严重程度的相关性尚未见报道。本研究对24家军队医院急诊或重症医学科收治的346例中暑患者的临床资料进行回顾性分析,旨在探讨不同Tc的EHI患者凝血功能的变化特征。
选取2021年3月-2023年9月24所军队医院(解放军总医院海南医院、解放军总医院第三医学中心、解放军东部战区总医院、解放军南部战区总医院、解放军中部战区总医院、解放军北部战区总医院、陆军军医大学第一附属医院、武警特色医学中心、解放军新疆军区总医院、解放军联勤保障部队第900医院、解放军联勤保障部队第902医院、解放军联勤保障部队第908医院、解放军联勤保障部队第909医院、解放军联勤保障部队第910医院、解放军联勤保障部队第947医院、解放军联勤保障部队第964医院、解放军联勤保障部队第967医院、解放军联勤保障部队第969医院、解放军联勤保障部队第970医院、海军第971医院、解放军联勤保障部队第980医院、解放军联勤保障部队第990医院、陆军第七十一集团军医院,解放军联勤保障部队第920医院)被确诊为中暑的346例患者进行回顾性分析。根据美国运动医学会2021年发布的EHI专家共识[7],EHI包括劳力性热衰竭、劳力性热损伤及劳力性热射病。纳入标准:符合《中暑的定义与分级诊断》中的EHI诊断标准[8],具体为暴露于高温(高湿)环境和(或)剧烈运动一定时间后,出现下列症状或体征中的至少一项且不能用其他疾病解释:(1)头晕、头痛、反应减退、注意力不集中、动作不协调;(2)口渴、心悸、心率明显增快、血压下降、晕厥;(3)恶心、呕吐、腹泻、少尿或无尿;(4)大汗或无汗、面色潮红或苍白、皮肤灼热或湿冷、肌痛、抽搐;(5)发热。排除标准:(1)非劳力性中暑;(2)存在先天性凝血功能紊乱;(3)合并慢性肝肾功能不全;(4)临床数据不完整者。经筛选后共346例EHI患者被纳入研究,根据入院时的Tc分为Tc<39 ℃组(n=223)、39 ℃≤Tc<40 ℃组(n=60)、40 ℃≤Tc<41 ℃组(n=35)、41 ℃≤Tc<42 ℃组(n=17)和Tc≥42 ℃组(n=11)(图1)。依据《中国热射病诊断与治疗专家共识》,采用如下的热射病诊断标准:满足高温高湿环境下或强体力活动任意一条,加上以下器官损害中的任意一条:(1)中枢神经系统功能障碍表现(如昏迷、抽搐、谵妄、行为异常等);(2)Tc>40 ℃;(3)多器官(器官≥2个)功能损伤表现(肝、肾、胃肠等);(4)严重凝血功能障碍[9-10]。将EHI患者按热射病诊断标准分为热射病组(n=63)与非热射病组(n=283)。本研究获解放军联勤保障部队第908医院伦理委员会审批(908yyLL031)。
通过住院电子病历系统收集EHI患者的一般资料(包括年龄、性别)和入院时的实验室指标,包括血红蛋白、血小板计数、谷丙转氨酶(alanine aminotransferase,ALT)、谷草转氨酶(aspartate aminotransferase,AST)、总胆红素、肌酐、肌酸激酶、凝血酶原时间(prothrombin time,PT)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)、纤维蛋白原及D-二聚体等。比较不同Tc组一般资料及实验室指标的差异。采用单因素分析筛选可能的影响因素,再通过多因素logistic回归分析筛选EHI患者发生热射病的独立危险因素。采用受试者操作特征(receiver operator characteristic,ROC)曲线评估PT、D-二聚体、血小板计数对EHI患者发生热射病的诊断价值。
采用SPSS 26.0及Prism 6软件进行统计分析及作图。计量资料符合正态分布时以$\bar{x}±s$表示,组间比较采用单因素方差分析,进一步两两比较采用LSD-t检验(方差齐时)或Tamhane's T2检验(方差不齐时);不符合正态分布时以M(Q1Q3)表示,组间比较采用Kruskal-Wallis H检验。计数资料以例(%)表示,组间比较采用χ2检验或Fisher确切概率法。为确定热射病的危险因素,先行单因素回归分析,将检验水准设为0.05,再用线性回归确认多重共线性后采用向后逐步回归的方法进行多因素分析。多因素logistic回归分析结果以比值比(odds ratio,OR)和95%可信区间(confidence interval,CI)表示。筛选变量后进行ROC曲线分析。P<0.05为差异有统计学意义。
各组EHI患者的年龄及性别比较差异均无统计学意义(P>0.05)。与Tc<39 ℃组和39 ℃≤Tc<40 ℃组比较,40 ℃≤Tc<41 ℃组、41 ℃≤Tc<42 ℃组及Tc≥42 ℃组EHI患者的心率、AST、ALT、肌酐及肌酸激酶升高,且均随Tc升高而逐渐升高(P<0.05);与Tc<39 ℃组、39 ℃≤Tc<40 ℃组、40 ℃≤Tc<41 ℃组及41 ℃≤Tc<42 ℃组比较,Tc≥42 ℃组的血红蛋白明显降低,总胆红素水平明显升高(P<0.05)(表1)。
与Tc<39 ℃组和39 ℃≤Tc<40 ℃组比较,40 ℃≤Tc<41 ℃组EHI患者的血小板计数降低(P<0.05,图2A),但仍在正常范围内。相比之下,41 ℃≤Tc<42 ℃组及Tc≥42 ℃组EHI患者的血小板计数已降至正常范围以下,且随Tc升高而进一步下降(P<0.05,图2A)。与Tc<39 ℃组和39 ℃≤Tc<40 ℃组比较,40 ℃≤Tc<41 ℃组、41 ℃≤Tc<42 ℃组及Tc≥42 ℃组EHI患者的纤维蛋白原水平均降低,且随Tc升高而下降(P<0.05,图2B)。与Tc<39 ℃组和39 ℃≤Tc<40 ℃组比较,40 ℃≤Tc<41 ℃组、41 ℃≤Tc<42 ℃组及Tc≥42 ℃组EHI患者的PT、APTT明显延长,并随Tc升高而进一步延长(P<0.05,图2C、D)。与Tc<39 ℃组比较,39 ℃≤Tc<40 ℃组EHI患者的D-二聚体水平相对升高(P<0.05,图2E),而40 ℃≤Tc<41 ℃组、41 ℃≤Tc<42 ℃组及Tc≥42 ℃组EHI患者的D-二聚体水平绝对升高,并随Tc升高而进一步升高(P<0.05,图2E)。
单因素logistics回归分析显示,PT、D-二聚体、血小板计数、ALT、AST、总胆红素、肌酐、APTT均是EHI患者发生热射病的危险因素(P<0.05,表2)。经线性回归确认多重共线性后,将PT、D-二聚体、血小板计数、ALT、总胆红素、肌酐、APTT纳入多因素logistics回归分析,结果显示PT、D-二聚体及血小板计数是EHI患者发生热射病的独立危险因素(P<0.05,表3)。

热射病患者常出现凝血功能障碍,表现为凝血因子活性下降、血小板减少等[11]。本研究进一步探讨了EHI患者Tc与凝血功能障碍的相关性,结果显示:Tc>39 ℃时,EHI患者的D-二聚体水平升高;Tc>40 ℃时,血小板计数减少,PT开始延长;Tc>41 ℃时,APTT明显延长,而血小板计数减少。血小板计数<140×109/L、D-二聚体>0.9 μg/ml、PT>16.4 s是EHI患者发生热射病的独立危险因素。
中暑导致凝血系统紊乱的因素包括高体温、缺血缺氧、血流低切应力、炎症反应等,中暑甚至会诱发弥散性血管内凝血[9]。临床可见多个器官出现充血、出血、广泛微血栓形成、坏死等病理表征[12]。实验室指标表现为血小板减少、PT和APTT延长、纤维蛋白原水平降低、D-二聚体水平升高等[13]。本研究结果显示,Tc达到39 ℃时,EHI患者最早出现的是D-二聚体水平升高,且是发生热射病的独立危险因素,其截断值为0.9 μg/ml,表明D-二聚体是中暑早期凝血功能变化较为敏感的指标。其机制可能是高温致内皮损伤后激活了凝血反应[14],内皮细胞对体温升高很敏感[15],导致循环中的血小板来源微粒和内皮来源微粒浓度升高。血小板来源微粒可刺激内皮细胞释放炎性因子并表达各种黏附分子,参与促进内皮氧化应激,降低一氧化氮的生物利用度,增强细胞黏附分子的表达,并损害内皮血管的运动功能[16]。内皮受损后释放促凝因子血管性血友病因子(von willebrand factor,vWF)、凝血Ⅷ因子,与血小板结合后可上调纤溶酶原激活物抑制物-1,从而促进血栓形成及抑制纤溶,导致D-二聚体水平升高[17]。在劳力性热射病大鼠模型的研究中发现,Tc达到40 ℃时,vWF水平升高,且随Tc升高而升高,表明内皮损害也随Tc升高而逐渐加重[6]
在高温致内皮受损的基础上,中暑后机体产生的炎性因子将进一步损伤内皮,内皮受损后暴露的vWF与血小板糖蛋白Ⅰbα/Ⅱa复合物结合,从而激活血小板[18]。此外,Tc升高时伴随体液大量丢失,出现脱水及血管通透性增高,导致血浆外渗,进一步造成血液浓缩,血流速度减慢,血流的切应力降低。血流低切应力会诱导缺氧,刺激内皮细胞释放P选择素和vWF,进一步促进凝血反应[19]。此病理过程中,血小板、纤维蛋白原会被不断消耗。本研究结果显示,Tc为40 ℃时血小板和纤维蛋白原水平开始降低,D-二聚体水平升高,而直到Tc>41 ℃时血小板计数才出现减少。血小板计数是发生热射病的独立危险因素,但其截断值为140×109/L,仍处于正常范围内,因而血小板计数下降在反映中暑相关凝血功能障碍时相对滞后。结合既往的动物实验研究结果,D-二聚体和血小板的这种变化特征可能是内皮损伤逐渐加重的结果[6]
组织因子(tissue factor,TF)是激活外源性凝血途径的重要启动因子,也是热射病中凝血反应激活的主要途径[20]。中暑致内皮受损后暴露的vWF结合激活的血小板及炎症反应过程中释放的单核细胞和中性粒细胞,可上调TF进而诱导外源性凝血反应[21-22]。本研究结果显示,Tc达到40 ℃时,反映外源性凝血途径的PT开始延长,且PT是发生热射病的独立危险因素,其截断值为16.4 s。同时,炎症反应过程中的中性粒细胞可通过形成中性粒细胞胞外陷阱促进血栓过度形成,激活因子Ⅻ,进而激活内源性凝血途径[23]。直至Tc>41 ℃时,APTT明显延长,此时随着凝血的进展,血小板计数进一步减少,而D-二聚体水平逐渐升高,PT亦明显延长。
综上所述,EHI患者随着Tc升高,中暑相关凝血功能障碍越加严重。PT延长、D-二聚体水平升高及血小板计数减少是EHI患者发生劳力性热射病的独立危险因素。本研究仍存在不足之处:(1)研究对象的性别构成比例不均,其中女性患者仅4例,故结果仅能反映男性群体的凝血功能变化特点;(2)仅为单时间点的观察,缺乏对热射病患者凝血功能的动态监测。后续可收集连续数据,动态评估凝血功能指标的变化趋势,进一步明确其在热射病早期诊断和预后预测中的应用价值。
