Abdominal war trauma is a common and high-risk type of injury in the modern battlefield, with rapid changes in condition and a high mortality rate. There is an urgent need for emerging medical technologies to improve the efficiency and success rate of first aid for military casualties. With the development of artificial intelligence (AI), 5G, and other emerging technologies, the concept of intelligent medical treatment is gradually forming and can assist in the diagnosis and treatment of abdominal trauma. This paper reviews the characteristics of abdominal war trauma in modern wars, discusses the application of intelligent medical treatment for abdominal war trauma and its drawbacks to be solved, aiming to provide reference for research related to abdominal war trauma.

腹部战创伤是现代战场环境下常见的战创伤类型,其发生率较高,病情变化迅速,病死率高,目前亟需采用新的医疗技术以提高我军腹部战创伤急救的效率及成功率。随着人工智能(AI)、5G移动通信等新兴技术的发展并与医疗相结合模式的推进,智能医疗的概念逐渐形成,并可为腹部战创伤的诊治提供帮助。本文就数次现代战争中腹部战创伤的伤情特点、智能医疗在腹部战创伤救治中的应用及其待解决的弊端进行综述,旨在为腹部战创伤的相关研究提供参考。

Abdominal war trauma is a common and high-risk type of injury in the modern battlefield, with rapid changes in condition and a high mortality rate. There is an urgent need for emerging medical technologies to improve the efficiency and success rate of first aid for military casualties. With the development of artificial intelligence (AI), 5G, and other emerging technologies, the concept of intelligent medical treatment is gradually forming and can assist in the diagnosis and treatment of abdominal trauma. This paper reviews the characteristics of abdominal war trauma in modern wars, discusses the application of intelligent medical treatment for abdominal war trauma and its drawbacks to be solved, aiming to provide reference for research related to abdominal war trauma.

腹部战创伤的发生率居现代战争所有战创伤类型的前3位,如不及时进行救治,病死率可达70%以上。闭合性腹部战创伤在腹部战创伤中最为常见,因其发病突然、病情变化迅速、病死率高,一直是战创伤救治的难点[1-2]。近年来,由于人工智能(artificial intelligence,AI)及5G移动通信等新兴技术在医疗领域的应用取得突破性进展,智能医疗得以诞生并取得了飞速发展。本文主要对智能医疗在腹部战创伤的诊断、治疗,生命体征监测,疗效和预后评估等方面的研究进展及其可能存在的问题进行综述,旨在为腹部战创伤的相关研究提供参考。
腹部战创伤是指在战场环境中由于子弹或爆炸装置等产生的不同外源性致伤因素(包括物理、化学和生物因素等)作用于人体的腹部,导致腹壁和(或)腹腔内部组织器官结构的完整性受损以及同时或相继出现的一系列器官功能障碍。在现代化战争中,腹部战创伤仍是战场环境下常见的损伤类型,约占全身各部位损伤的11%,仅次于四肢战创伤和头颈部战创伤[1-2]。其中腹部闭合性战创伤占腹部战创伤的70%以上,是临床上较为严重的急诊战创伤类型之一[3],具有发病突然、伤势严重、病情变化迅速、病死率高等特点,且常合并脑、胸、四肢等其他部位的战创伤,如不及时救治,病死率可高达70%~80%[4-5]。进入21世纪以后,随着更多高新技术武器的出现和发展,现代战争作战环境与模式不断变化,导致战创伤的伤类伤情愈发复杂,致残、致死率不断提高,显著增加了战创伤救治的难度[6]。而腹部面积较大,涉及脏器较多,导致其发生战创伤的概率较大,因此,腹部战创伤是战场环境下的常见战创伤伤情。
美军在2003年伊拉克战争(第二次海湾战争)中的伤情分析显示,腹部战创伤占整体战创伤的比例为10%~20%[7]。腹部战创伤通常涉及空腔脏器损伤,多数伤员合并多处战创伤(每例伤员平均合并4或5处战创伤)。腹部战创伤的情况往往较为复杂,常伴有全身其他部位和系统的损伤,可能导致出血性休克、感染等,进而引发死亡。此外,腹部战创伤的救治难度大,病死率高,是当前军队作战减员和战斗力下降的主要原因之一[8]。因此腹部战创伤一直以来都是我军战创伤救治研究的重点和热点。野外战场环境下腹部战创伤伤员救治策略、救治技术和救治方法的改进与创新对伤员的救治起到了重要作用。随着现代化战争对腹部战创伤现场救治和转运后送等环节任务需求的不断提升,迫切需要将一些新兴技术应用于腹部战创伤的救治。
智能医疗目前尚无明确定义,本文中的智能医疗是指利用目前多种新兴技术如AI及5G技术等与医学相结合形成的一些新的用于诊断及治疗疾病的医学技术。
AI是基于电子计算机来对人类智力进行模拟、开发和拓展的新型技术,也是一门由计算机、物理、数学、心理学和哲学交叉形成的新兴学科。随着电子计算机等科学技术的迅猛发展,AI逐渐呈现出多元化、综合化发展的趋势,并衍生出计算机视觉、语音识别、自然语言处理、规划决策以及大数据统计分析等多个新兴技术领域,且逐步融入到医学的各个领域之中,形成了“AI辅助医疗”的模式。“AI辅助医疗”作为智能医疗的重要分支,改变了医疗系统运行的传统模式,也促进了创伤医学的新发展。AI有望通过高效、精准、智能的特性提高创伤救治的安全性和效率,同时降低临床医师的工作负荷,并在病情评估、风险预测、辅助诊断、辅助治疗等多个方面为创伤救治带来新的思路[9]
5G(the fifth generation of wireless transmission technology)指第5代无线技术,其建立在前身2G、3G和4G技术的基础上,可提供更快的数据传输速度、更低的延迟和更大的网络通信容量。5G技术结合了新的基础设施、无线电频率和通信协议来实现高速的数据传输,为包括医疗保健在内的各个领域开辟了新的可能性。虽然在现有有线网络下也能实现远程会诊及远程手术指导等远距离信息交换,但在许多腹腔镜下操作如术中穿孔、大量出血等紧急情况下,对于网络的带宽、低延迟、实时性以及图像的清晰度要求极高,稍有偏差即可能导致处理失败甚至危及患者生命。5G技术具有高速率、低功耗和低延迟的特点,可解决4G网络条件下远程手术中实时性差、视野清晰度低、远程控制延迟高等问题[10]。5G网络可实现高清音视频实时交互、操控信号双向传输、生命体征数据监测实时共享等,手术医师能实时且更清晰地看到患者远程手术的场景,掌握手术的即时数据,在低延迟下完成手术的远程操作[11]
开放性腹部战创伤通常易发现及诊断,可及时手术处理并明确出血或损伤的部位及器官。而闭合性腹部战创伤大多发生突然,不易发现及诊断,病情变化迅速,如不及时救治,伤员死亡率极高。闭合性腹部战创伤患者常有合并伤,易造成漏诊、误诊,而准确诊断、及时救治闭合性腹部战创伤可使其死亡率降至15%以下[4-5],因此对于腹部战创伤快速准确的诊断尤为重要,同时复杂的战场环境对目前已有的诊断技术也提出了严苛的要求。诊断性腹腔穿刺及腹腔灌洗术、腹部CT和腹部创伤超声重点检查(focused abdominal sonography for trauma,FAST)是目前用于评估腹部战创伤伤情的常用技术[12]
诊断性腹腔穿刺及腹腔灌洗术的诊断阳性率高达90%以上[13],但诊断性腹腔穿刺仅在抽吸出10 ml以上的血液时才能作为诊断伤员存在来自肝、脾等腹腔脏器损伤的阳性指标[14]。尽管诊断性腹腔穿刺和腹腔灌洗术简便易行,但由于这两种技术均为有创操作,可能造成腹腔脏器损伤,不宜用于腹腔胀气或粘连的患者,且可重复性差,诊断所依赖的精密检验设备不适合在战场环境中携带。这些缺点限制了其在战场环境中的应用。
腹部CT相较于诊断性腹腔穿刺和腹腔灌洗术特异性高,且为无创检查,因此很多创伤治疗中心将其作为首选的诊断方式[15]。但在战场环境下,由于CT设备体积及重量大,对造影剂的依赖性强,往往需要将伤员运送到后方才能进行检查;而且腹部CT主要适用于血流动力学稳定的闭合性腹部战创伤伤员的伤情评估,对于血流动力学不稳定的伤员会延误诊断时间,对较小的空腔脏器损伤也难以进行诊断[16]
相较于诊断性腹腔穿刺和腹腔灌洗术及腹部CT,FAST因具有装置便携、无创的优点,无疑是腹部战创伤的重要筛查和诊断手段。FAST能够初步检测伤员是否有腹腔出血,并明确出血量和出血部位,可辨别出100 ml以上的腹腔游离液体,且灵敏度与腹腔游离液体的量呈正比。通过FAST初筛可明确伤员的治疗优先次序[17]。对于FAST检查结果阳性的循环不稳定者,则需要及时进行腹腔探查手术。因此FAST更适用于战场环境下腹部战创伤的诊断。
AI、5G技术辅助FAST诊断将会是智能医疗在腹部战创伤诊断中的重要发展方向。目前AI技术在腹部超声影像中器官、组织和病变的自动检测及分割方面取得了显著进展。与传统的图像处理技术相比,基于卷积神经网络(convolutional neural networks,CNN)的深度学习方法表现出了更好的准确度和效能。U-Net网络结构及其变体模型被广泛应用于医学图像的检查及区分。深度学习的CNN模型已被用于肝分割、肝肿块检测,以及脾、胰腺、肾和组织图像分割。此外,机器学习和深度学习技术已被用于通过器官组织表征对疾病进行诊断和预测[18]。Pavlopoulos等[19]使用AI超声图像诊断模型分析了肝脏超声图像,构建了基于5个特征参数[分形维数纹理分析(FDTA)、空间灰度依赖矩阵(SGLDM)、灰度共生矩阵(GLCM)、灰度游程统计量(RUNL)和一阶灰度参数(FOP)]的AI诊断模型,该模型可用于智能识别播散性肝脏病变。基于相关技术,利用AI分析腹部超声图像来辅助诊断腹部战创伤也是可实现的。
目前已经出现基于AI与5G技术相结合的AI便携式超声检测系统,该系统与5G通信平台相结合,有助于战场或灾害现场的救援,能自主进行检查、诊断、操作等,可减少战场救援时对人工操作的依赖,完成远程超声检查、图像传输、快速远程会诊等,从而节省人力物力[20]。AI便携式超声检测系统采用增强现实(augmented reality,AR)技术及通过5G技术进行远程操作也是未来的发展趋势[21]。通过5G通信可将图像等相关数据实时传输到后方医院,后方医务人员可以进行诊断,一定程度上提高诊断的准确性。利用5G低延迟、高速率的特点进行信息传输,以AR技术进行远程引导,使具有丰富床旁超声诊断经验的医师能够远程指导战场急救人员更准确地使用超声装置,做出诊疗决策并确保诊断正确。此类AI辅助便携式超声诊断系统显示了良好的应用前景,对于战时腹部战创伤的诊断有重要作用。
随着影像学、重症医学和微创技术的进步,以腹腔镜技术为代表的微创外科手术方式已逐渐成为腹部战创伤诊疗中不可或缺的重要手段。腹腔镜微创技术不仅可满足战时创伤救治的实际需求,更被视为未来腹部战创伤救治研究的重要方向,有望在继续改进后成为创新的战时创伤救治途径。该技术凭借其微创优势,在处理腹部钝性伤和刺伤方面应用越来越广泛,可避免非治疗性剖腹手术并缩短住院时间[22-23]。相较于剖腹探查术,腹腔镜微创探查术存在误诊率高、无法探查腹膜后脏器损伤及可能造成气腹并发症等问题,使得其在腹部战创伤中的应用相较于其他腹部疾病显著滞后。然而一项纳入9058例腹部创伤患者的Meta分析发现,腹腔镜探查术是改善围手术期结局及减少血流动力学稳定的腹部创伤患者并发症的有效方法,值得在临床实践中进一步推广[24]。战场条件下,由于医疗资源及环境的限制,腹腔镜微创技术因具有创伤小、术后恢复快、伤员感染减少等突出优势,成为腹部战创伤诊治中值得大力推广运用的技术。
相较于传统的诊疗技术,腹腔镜对腹部创伤的诊治有诸多优势:对于开放性腹部战创伤伤员,腹腔镜探查术可利用开放的伤道进行探查诊断,不仅避免了常规剖腹探查造成的腹部二次损伤,而且诊断的准确率与常规剖腹探查没有明显差异[25]。腹腔镜探查术可在探查腹部器官组织损伤的同时进行修补及止血治疗。此外,腹腔镜技术也适用于损伤控制性手术,通过小切口完成手术,对周围组织器官的影响更小,可降低手术并发症的发生率。由于腹腔镜具有放大视野的作用,相较于传统剖腹探查术可实现更精准的止血、修补、切除和吻合,最大限度地保留和修复伤员重要脏器的生理功能。同时腹腔镜手术造成的手术创伤小,有助于术后快速康复[26]。以腹腔镜技术为代表的腹部微创技术因其手术快速精准、创伤小、术后恢复快的优势,已成为腹部战创伤诊疗的重要部分,也是未来腹部战创伤救治的发展趋势。
应用5G技术远程指导手术,也可能成为腹腔镜手术治疗腹部战创伤的一种常用辅助手段。2019年,西班牙巴塞罗那医疗团队通过5G网络远程指导并完成了2例直肠腺癌切除手术,被认为是全球首次使用5G通信远程指导的临床手术。巴塞罗那和上海的两个手术团队通过5G技术在电脑上实时发送和接收手术信息,患者的手术图像经腹腔镜摄像头发送到远程外科医师的电脑,远程外科医师通过耳机与手术外科医师进行交流,手术外科医师从手术室实时接收远程外科医师在观看手术图像上的语音指令和图画,他的视觉指示被投射到手术室的屏幕上,进而辅助进行腔镜外科手术[27]。通过5G技术还可进行远程机器人手术,如解放军总医院开展了距离3000 km的保留肾单位手术的动物实验,验证了基于5G技术的超远距离机器人手术的安全性和可行性[28]。但目前手术机器人普遍体积较大,移动不便,用于战场环境下的远程战创伤手术机器人的设计还需要继续改进。
当前在机器人、腹腔镜手术中已经有结合AI技术的探索性应用。AI可通过计算机视觉和深度学习对机器人手术和腔镜微创手术期间录制的手术视频进行识别与分析,有可能改善手术流程。有研究人员开发了手术视频的自动相位识别算法[29-30];该算法拥有自动识别手术时期的能力,有可能优化手术室的工作流程。有研究开发了一个计算机视觉模型的实时软件,并在机器人腹股沟疝修补术期间进行手术阶段的实时预测,平均预测准确率为68.7%[31]。这种拥有微创手术时空分析能力的计算机视觉模型可进一步转移到临床应用中,类似模型还可集成到医院和手术室软件中,以帮助优化手术的工作流程。手术室工作人员可通过AI模型的预测提示来了解手术情况,并明确外科医师当前术中所需要的手术器械或其他帮助的时机。例如,若某个手术阶段花费的时间比预估的平均时间延长较多,则可通过软件连接的警报系统提示指派另一名经验丰富的外科医师检查手术外科医师的手术情况并在需要时提供帮助。这类程序有利于保障患者手术的安全性。此类AI模型还可帮助手术室工作人员了解手术不同阶段所需的器械,以便他们在下一个手术阶段开始之前提前做好器械准备工作,缩短手术准备时间。实时自动识别手术阶段对优化手术室的运转流程及提高围手术期的护理质量具有重要意义[32]。此外,还可开发AI模型预测出血事件的发生,并提醒外科医师重新评估下一步的手术操作是否能保障患者安全[31]。AI预测模型的这些功能对于腹部战创伤的腔镜手术治疗可提供辅助指导,从而提高手术效率。
目前在术后护理及体征监测方面出现了一些基于5G技术的可穿戴的体征监测设备[33]。通过5G的连续数据传输技术可实时传输战创伤伤员的生命体征,并对其进行监控和诊断。该技术更快的数据传输速度和更低的延迟为患者数据的连续传输提供了保障。这也意味着生命体征和其他相关信息可实时发送给战时后方医院,而不易中断或延迟;专业的医务人员可随时访问获得患者的实时数据,并据此做出准确的临床决策[34]
AI技术可对一些体征监测数据进行分析,进而对术后患者在重症监护病房(ICU)中的特定事件进行预测。Hatib等[35]使用两个大型数据库(共1334例患者)中的动脉管路血压波形数据开发并测试了一种AI预测模型;该模型可提前15 min预测低血压事件(定义为平均动脉压<65 mmHg持续1 min)的发生。AI也可利用其他ICU监测数据库创建机器学习模型来预测发病率、气管插管拔管时间、临床病情恶化趋势、病死率或再入院率,甚至预测脓毒症[36]。虽然战场环境中腹部战创伤的伤口感染率为14%,与严重的平民创伤感染相差不大,但基于感染的脓毒症仍是公认的腹部战创伤术后的主要并发症之一[37]。对于AI技术在检测脓毒症的发生方面,一项单中心的随机对照试验比较了以6项生命体征参数[收缩压/舒张压、心率、体温、呼吸频率、外周毛细血管氧饱和度(SpO2)和年龄]作为特征的AI警报系统与基于电子健康记录的警报系统(使用其他标准预测脓毒症)对术后脓毒症的预测能力,结果显示,AI警报系统的预测效能优于全身炎症反应综合征标准、序贯器官衰竭评估评分量表,且使住院时间缩短了20.6%,住院死亡率降低了58%[38]。基于面部识别及光电容积脉搏波图和皮肤电导波形、脑电图信号进行机械学习建立的AI模型也可用于术后患者的疼痛管理,虽然准确率仅有65%,但对于腹部战创伤伤员的术后疼痛起到了一定监测作用[39-40]
目前的AI算法已具备了一定的准确预测术后并发症的能力,但仍应在内部和外部进行适当的测试及验证。此外,拥有完整的数据库和没有主观遗漏的数据是AI算法模型准确预测的绝对先决条件[41]
智能医疗的应用首先应注意医疗设备中的网络安全问题。随着5G及AI技术的出现,医疗设备在医疗环境中的互联互通达到了前所未有的水平,显著提高了通信的速度和效率,但增加了潜在的网络威胁和受攻击面[42],在战创伤诊治过程中易被敌方网络黑客进行网络攻击,恶意访问盗取机密信息,造成数据泄露,暴露伤员的个人信息甚至诊治的位置信息,从而被敌方重点打击[43]。敌方网络黑客还可能通过网络攻击扰乱医疗程序和治疗,篡改输液泵或改变剂量造成护理中断,甚至可能导致药物剂量使用错误等严重后果,危及伤员的生命。5G技术对这些漏洞的独特放大作用值得特别考虑。因此保护网络安全是保障我军智能医疗发展的根本。
使用5G技术优化远程指导手术的理论体系尚未完全建立,在安全性、可靠性、传输质量、易用性、成本等方面还需建立标准。目前使用5G进行远程指导在外科培训中存在一些实际局限性[44],如远程外科医师无法承担所指导的手术的医疗责任。此外,手术治疗经验是一个关键因素,经验不足的外科医师可能难以理解远程指导的指令,致使远程指导难以进行。能熟练运用智能医疗设备的同时还要保证有充足的经验,这对未来智能医疗人才的培养提出了挑战。
AI做出的临床决策存在不可解释性,引起了人们对其安全性及准确性的担忧。由于AI系统无法以一种可被人理解的方式来解释自己的决策,决策建议内在的因果联系尚不清楚,这造成了AI系统的不透明。黑箱问题是指当AI系统不透明时,算法自身的缺陷遮掩了数据处理结果的可预测性,从而引发的对临床决策的质疑。这使人们越来越担心AI系统因黑箱问题无法保证算法提供的临床决策的可靠性,从而怀疑由AI作出的医疗决策的安全性[45]。AI算法的黑箱问题会造成人们对医学理解和解释的缺失。如果人们只重视治疗结果,而忽略了数据的呈现方式及其中的原理,将导致临床医师对患者的关怀转变成对治疗效益的追求,这会造成临床医师以AI系统决策为中心,从而威胁到当代医疗实践中以患者为中心的传统。
AI的医疗决策如果出现错误,侵害了患者的权益时会产生责任归属的相关法律问题。随着AI被赋予更多的自主权,医疗AI设备可在没有人为干预的情况下进行自主决策,并依据算法选择临床诊疗决策。因此,未来必须要有相关责任人来承担因采用拥有更多决策权力的AI作出的医疗决策而产生的法律责任[46]。如果未来AI算法被证实存在缺陷,且AI诊疗系统提出的医疗建议对伤员抢救产生了不良影响,就需要更完善的法律来解决因此产生的责任归属问题。
越来越多的证据表明,5G及AI技术为腹部战创伤智能医疗的发展及应用开辟了更多的可能性,智能医疗对腹部战创伤诊疗的影响将是深远且多方面的,本文仅在远程医疗、AI数据学习模型建立及增强现实(augmented reality,AR)及虚拟现实(virtual reality,VR)领域选择了一些应用5G及AI技术重构腹部战创伤诊治形式的代表性案例进行综述。目前基于5G、AI技术的智能医疗在腹部战创伤的诊断、手术治疗、术后生命体征监测及并发症预防等方面显示出良好的前景,但仍存在网络安全问题、数据泄露风险、缺乏全网部署和硬件支持、AI决策的责任归属等问题,对数据安全保密工作、现代化设施部署、相关伦理讨论等提出了新的挑战。目前战场环境多变,战创伤也变得复杂多样,智能化战创伤诊治技术和设备也应跟随时代步伐持续革新,以提高诊治效率,促进伤员康复。
