Objective To evaluate the predictive value of dose-surface histogram (DSH) for radiation proctitis (RP) in prostate cancer (PCa) patients undergoing radiotherapy. Methods This prospective randomized controlled clinical trial included 380 PCa patients who underwent image-guided radiotherapy in the First Affiliated Hospital of Hebei Northern University from January 2018 to January 2023. Patients were randomly divided into observation group (n=200) and control group (n=180). The rectal dose distribution of patients in the two groups was evaluated by using DSH and dose-volume histogram (DVH), respectively. The receiver operating characteristic (ROC) curve was utilized to evaluate the predictive value of DSH for acute RP, with DVH serving as a reference. Results The difference was not statistically significant in clinical information such as age, KPS score, and body mass index (BMI) between the observation and control groups (P>0.05), as well as in acute RP incidence (P>0.05). There were significant differences in S40 and V40, S50 and V50, S60 and V60, S70 and V70, and S78 and V78 between the two groups (P<0.05). S40, S50, V40, and V50 showed low efficacy (P<0.001) in predicting acute RP at each level, with AUC ≤0.700. S60 and V60 showed moderate efficacy (P<0.001) in predicting acute RP at each level, with AUC 0.700-0.900. S70, S78, V70 and V78 showed high efficacy (P<0.001) in predicting acute RP at each level, with AUC >0.900. Conclusions The predictive value of DSH for rectal toxicity in patients with PCa is basically consistent with that of DVH. It is expected to become a novel and valuable tool for evaluating radiotherapy plans in the future.

目的 评价剂量表面积直方图(DSH)对前列腺癌(PCa)患者急性放射性直肠炎(RP)的预测价值。方法 本研究为前瞻性随机对照临床研究。收集2018年1月-2023年1月河北北方学院附属第一医院放疗科收治的380例PCa患者,采用抽签法随机分为观察组(n=200)与对照组(n=180)。采用DSH评价观察组直肠剂量分布,剂量体积直方图(DVH)评价对照组直肠剂量分布。采用受试者操作特征(ROC)曲线评价两种计划评价工具对急性RP的预测效能。结果 两组年龄、RPS评分、体重指数(BMI)等临床资料比较差异均无统计学意义(P>0.05)。两组急性RP级别比较差异无统计学意义(P>0.05)。两组S40与V40、S50与V50、S60与V60、S70与V70、S78与V78比较差异均有统计学意义(P<0.001)。S40、S50、V40、V50预测各级急性RP的曲线下面积(AUC)≤0.700,提示低等效能(P<0.001),S60、V60预测各级急性RP的AUC为0.700~0.900,提示中等效能(P<0.001),S70、S78、V70、V78预测各级急性RP的AUC>0.900,提示高等效能(P<0.001)。结论 DSH对PCa患者直肠毒性的预测效能与DVH较为相似,在未来放射治疗计划设计中可弥补DVH的不足或单独用于计划评价。

目的 评价剂量表面积直方图(DSH)对前列腺癌(PCa)患者急性放射性直肠炎(RP)的预测价值。方法 本研究为前瞻性随机对照临床研究。收集2018年1月-2023年1月河北北方学院附属第一医院放疗科收治的380例PCa患者,采用抽签法随机分为观察组(n=200)与对照组(n=180)。采用DSH评价观察组直肠剂量分布,剂量体积直方图(DVH)评价对照组直肠剂量分布。采用受试者操作特征(ROC)曲线评价两种计划评价工具对急性RP的预测效能。结果 两组年龄、RPS评分、体重指数(BMI)等临床资料比较差异均无统计学意义(P>0.05)。两组急性RP级别比较差异无统计学意义(P>0.05)。两组S40与V40、S50与V50、S60与V60、S70与V70、S78与V78比较差异均有统计学意义(P<0.001)。S40、S50、V40、V50预测各级急性RP的曲线下面积(AUC)≤0.700,提示低等效能(P<0.001),S60、V60预测各级急性RP的AUC为0.700~0.900,提示中等效能(P<0.001),S70、S78、V70、V78预测各级急性RP的AUC>0.900,提示高等效能(P<0.001)。结论 DSH对PCa患者直肠毒性的预测效能与DVH较为相似,在未来放射治疗计划设计中可弥补DVH的不足或单独用于计划评价。

Objective To evaluate the predictive value of dose-surface histogram (DSH) for radiation proctitis (RP) in prostate cancer (PCa) patients undergoing radiotherapy. Methods This prospective randomized controlled clinical trial included 380 PCa patients who underwent image-guided radiotherapy in the First Affiliated Hospital of Hebei Northern University from January 2018 to January 2023. Patients were randomly divided into observation group (n=200) and control group (n=180). The rectal dose distribution of patients in the two groups was evaluated by using DSH and dose-volume histogram (DVH), respectively. The receiver operating characteristic (ROC) curve was utilized to evaluate the predictive value of DSH for acute RP, with DVH serving as a reference. Results The difference was not statistically significant in clinical information such as age, KPS score, and body mass index (BMI) between the observation and control groups (P>0.05), as well as in acute RP incidence (P>0.05). There were significant differences in S40 and V40, S50 and V50, S60 and V60, S70 and V70, and S78 and V78 between the two groups (P<0.05). S40, S50, V40, and V50 showed low efficacy (P<0.001) in predicting acute RP at each level, with AUC ≤0.700. S60 and V60 showed moderate efficacy (P<0.001) in predicting acute RP at each level, with AUC 0.700-0.900. S70, S78, V70 and V78 showed high efficacy (P<0.001) in predicting acute RP at each level, with AUC >0.900. Conclusions The predictive value of DSH for rectal toxicity in patients with PCa is basically consistent with that of DVH. It is expected to become a novel and valuable tool for evaluating radiotherapy plans in the future.

近年来,前列腺癌(prostate cancer,PCa)图像引导放射治疗(image-guided radiotherapy,IGRT)技术的应用明显提高了肿瘤靶区位置的准确度,降低了多数危及器官(organ at risk,OAR)的毒性[1-3]。但是,PCa患者放射性直肠炎(radiation proctitis,RP)发生率仍较高[4-6]。传统上,放疗物理师采用剂量体积直方图(dose and volume histogram,DVH)评价直肠剂量的分布并预测其毒性,该方法已被证实具有一定的科学性和可行性[7-9]。健康的直肠壁厚仅约2 mm甚至更薄,因此将其视为平面并进行剂量分布评价或许更具研究价值[6]。2009年,Buettner等[10]提出了剂量表面积直方图(dose and surface histogram,DSH)的理念。DSH是一种通过统计承受不同累积输出剂量的靶区或OAR表面积占其总表面积的比值来评价受照射部位剂量水平的放疗计划系统工具。与DVH相似,不同水平的DSH或许与靶区或OAR放疗后的疗效或毒性相关。受技术水平限制,直至2019年,Vanneste等[11]才首次应用DSH对直肠壁剂量分布进行评价,并评价了其对RP的预测效能。为了更深入量化DSH的临床应用价值,本研究比较和评价了DSH与DVH对PCa患者放射治疗后急性RP的预测效能,以期为补充和优化放疗计划评价工具提供参考。
本研究为前瞻性随机对照临床研究(临床试验注册号:ChiCTR2000032827)。收集2018年1月-2023年1月河北北方学院附属第一医院放疗科收治的PCa患者。诊断标准:影像学肉眼可见肿瘤但未侵犯其他组织,前列腺特异性抗原(prostate specific antigen,PSA)异常但<20 ng/ml,格里森评分(Gleason score,GS)异常但<7分。纳入标准:(1)年龄≥18岁;(2)临床资料完整;(3)患有局限期中、低危PCa,PSA <20 ng/ml,GS评分<9分;(4)肿瘤临床分期T1–T3a期;(5)除PSA外,其他检验指标正常。排除标准:(1)有盆腔手术史;(2)有盆腔放疗史;(3)有前列腺良性疾病史;(4)有结直肠疾病史。共380例患者纳入,其PSA为(12.39±1.19) ng/ml,GS评分为(7.29±0.61)分。采用抽签法将患者随机分为观察组(采用DSH进行放射治疗计划评价,n=200)与对照组(采用DVH进行放射治疗计划评价,n=180)。本研究获河北北方学院附属第一医院伦理委员会审批(20181002),所有患者入组前均签署知情同意书。
嘱患者膀胱充盈,直肠排空,采用Philips Big Bore CT采集盆腔范围内的计划CT影像(扫描上界为腰椎L4,下界为闭孔下缘,扫描层厚为3 mm)并上传至Monaco计划系统用于计划设计。根据美国肿瘤放射治疗协作组织(RTOG)前列腺癌靶区勾画指南勾画靶区并设置目标剂量[12],其中直肠目标限量为接受40 Gy累积输出剂量的体积(V40,下同)≤50%,V50≤30%,V60≤20%,V70≤10%,V78≤5%。Infinity医用电子直线加速器(瑞典医科达公司)治疗模式为双弧设计(1个360°全弧和1个260°弧)的容积旋转调强放疗模式,射线能量选择6 MV,剂量率为600 MU/min,计划靶区(plan target volume,PTV)处方剂量为67.5 Gy,共25个分次,每周治疗5次[6]
采用DSH评价观察组患者直肠壁剂量分布,包括7个步骤:(1)导出患者CT影像至OpenCV图像处理软件,该软件由Intel公司开发和维护,是一个基于BSD许可(开源)发行的跨平台计算机视觉和机器学习软件库,提供了包括Matlab在内的多种语言接口;(2)截取单层CT上直肠区域图像,对其进行灰阶处理;(3)计算直肠壁平均灰度值,对处于平均灰度值一定范围内的直肠壁外轮廓进行二值化处理;(4)采用主成分分析法计算直肠壁外轮廓在单层CT上的质心坐标并设置为原点;(5)从原点出发,对直肠壁进行每4°一等分的切割,共90等分;(6)量化每等分切割长度,宽度为CT层厚(3 mm),每一等分表面积计算方法为长度×宽度(图1);(7)通过人工统计中心处点累积剂量和观察组接受40 Gy累积输出剂量的直肠壁表面积占直肠壁总表面积的比值(S40,余同)、S50、S60、S70、S78。采用DVH评价对照组患者直肠剂量分布。由临床医师在计划CT影像上勾画直肠轮廓,各项剂量输出参数设置完毕后,由物理师通过计划系统软件自动统计对照组接受40 Gy累积输出剂量的直肠体积占直肠总体积的比值(V40,余同)、V50、V60、V70、V78
收集两组患者的临床资料。(1)人口统计学资料,包括年龄、体重指数(body mass index,BMI)、卡氏功能状态(KPS)评分等[6];(2)前列腺特征,包括PSA、GS评分、前列腺体积、PTV体积等[6];(3)剂量学特征,包括PTV最小剂量、PTV最大剂量、PTV平均剂量等[6];(4)直肠特征,包括直肠表面积、直肠体积等。
每周患者接受3次基于锥形束CT的治疗前验证[1-6]。急性RP的时间界定为自IGRT开始至结束后3个月[6]。根据常见不良事件评价标准(CTCAE)5.0对急性RP进行分级(共分为4级)[13]。IGRT开始后患者随访频率为每周1次,IGRT结束后患者随访频率为每月1次。随访时间截至2023年12月31日。
采用SPSS 19.0软件进行统计分析。不符合正态分布的计量资料以M(Q1Q3)表示,组间比较采用Mann-Whitney U检验;符合正态分布的计量资料以$\bar{x}±s$表示,组间比较采用独立样本t检验。计数资料和等级资料以例(%)表示,前者组间比较采用χ2检验,后者组间比较采用Wilcoxon秩和检验。采用受试者操作特征(receiver operating characteristic,ROC)曲线下面积(arear under the curve,AUC)分析DVH和DSH对急性RP的预测效能。AUC≤0.700提示低等预测效能,0.700~0.900提示中等预测效能,>0.900提示高等预测效能。P<0.05为差异有统计学意义。
两组年龄、KPS评分、BMI等临床资料差异均无统计学意义(P>0.05,表1)。
两组S40与V40、S50与V50、S60与V60、S70与V70、S78与V78比较差异均有统计学意义(P<0.05,表2)。
截至2023年12月31日试验结束,无患者失访。两组急性RP整体级别分布较为一致(Z=0.053,P=0.958);观察组与对照组各级急性RP发生率较为一致(P>0.05,表3)。
两种计划评价工具预测各级急性RP的结果(表4)显示:(1)S40、S50、V40、V50预测各级急性RP的AUC≤0.700(P<0.001),提示低等预测效能;(2)S60、V60预测各级急性RP的AUC为0.700~0.900(P<0.001),提示中等预测效能;(3)S70、S78、V70、V78预测各级急性RP的AUC>0.900(P<0.001),提示高等预测效能。S70预测Ⅰ级急性RP的截断值为4.44%,预测Ⅱ级急性RP的截断值为5.82%,预测Ⅲ级急性RP的截断值为6.31%;S78预测Ⅰ级急性RP的截断值为1.28%,预测Ⅱ级急性RP的截断值为1.84%,预测Ⅲ级急性RP的截断值为2.80%。高预测效能计划评价指标(S70、S78、V70、V78)的ROC曲线如图2所示。
目前,已有多种新技术应用于PCa的IGRT中,如基于黄金基准标志物校准的图像引导技术[14-15]明显提高了靶区位置的准确度,奠定了PCa大分割乃至超分割放疗的基础,通过提高剂量率而最终提高了肿瘤局部控制率。然而,由于直肠前壁容易进入靶区高剂量范围,多数患者存在较高的RP发生率[6]。国内外学者采用了多种方法来降低RP发生率[5-6],如置入水凝胶或充气球囊,以分隔前列腺与直肠前壁。就临床应用效果来看,置入间隔物后RP发生率明显下降。另一种方法为通过更加科学的计划设计及评价指标调整和优化放疗计划以降低RP的发生率或减轻其严重程度。自问世以来,前述各种技术日益成熟,应用效果日益显著。本研究评价和比较了DSH与DVH对PCa IGRT致各级直肠毒性的预测效能,发现两种计划评价工具结果较为相似。
本研究与其他研究存在部分异同,例如Vanneste等[11]研究。相同处:(1)对图像的处理过程较为相似,均采用了灰阶处理、二值化处理、轮廓等分切割等过程;(2)均评价了DSH对急性RP发生的预测效能。Vanneste等[11]认为DSH对Ⅱ级及以上的RP具有较高的预测效能。不同处:(1)不同于本研究的主成分分析法,Vanneste等[11]采用了具有一定偏心率的椭圆同直肠壁外轮廓进行曲线拟合并计算了质心坐标;(2)Vanneste等[11]未量化DSH对其他级别RP的预测效能,且未采用DVH作为参考,结果的科学性欠佳。
本研究发现,DSH的S40、S50、S60、S70、S78分别高于DVH的V40、V50、V60、V70、V78。笔者认为,造成此结果的原因如下。(1)直肠壁轮廓输出误差:DSH应用的第3步为“计算直肠壁平均灰度值,对处于平均灰度值一定范围内的直肠壁外轮廓进行二值化处理”,该过程受CT设备软组织分辨率影响较大。较低的软组织分辨率会降低直肠壁灰度值的准确度,从而混淆直肠壁与其他组织的边界,最终造成直肠壁轮廓的输出误差。该误差会导致后续一系列计算误差,最终影响DSH的评价结果。(2)直肠壁轮廓切割精度:DSH应用的第5步为“以质心为原点对直肠壁行90等分切割,每一等分为4°”,该过程直接决定了DSH数据源的体量。在宽度(CT层厚)固定前提下,等分数量越多,等分角度越小,等分面积也就越小,DSH数据源的体量就越大,对剂量分布细微差别的描述就越精确。相比国外研究中的100等分[11],本研究中等分数量相对较小,对剂量分布的描述能力相对较差,最终影响了DSH的评价结果。总之,随着CT软组织分辨率、图像切割精度的提高和CT扫描层厚的降低,DSH评价结果的准确度将会进一步提高。
临床医师在靶区或OAR轮廓勾画过程中,经常会受到计划CT影像质量不高的困扰。当计划CT影像质量不理想时,极薄部分靶区或OAR轮廓的勾画存在一定难度,容易出现勾画偏差甚至轮廓重合。将三维的极薄结构降维为二维的平面是解决上述问题的方法之一。本研究中DSH的应用为极薄靶区或OAR轮廓的降维奠定了基础,为其剂量分布评价开辟了新的思路。对于极薄靶区或OAR,大部分可应用DSH进行评价;对于薄厚不均匀的靶区或OAR,可考虑分段联合应用DSH和DVH。目前,国内外关于DSH的研究和应用均处于初级阶段,大部分操作过程需人工手动完成,难度较大且过程复杂,同时流程欠标准化,结果可重复性差。因此,亟待开发DSH相关的程序和软件,最终实现操作的标准化和自动化。
综上所述,本研究结果表明,DSH和DVH对PCa IGRT致Ⅰ、Ⅱ、Ⅲ级急性RP的预测效能较为相似,提示DSH也具有较为理想的直肠毒性预测效能,在未来放射治疗计划设计中可弥补DVH的不足或单独用于计划评价。但本研究也存在不足之处:(1)未深入分析DSH对PCa IGRT致晚期RP的预测效能;(2)由于技术水平的限制,未对直肠壁按薄厚进行分段处理,后续研究中对直肠壁按薄厚进行分段处理(极薄部分应用DSH,较厚部分应用DVH)可能会取得更为理想的结果。
