Heatstroke, especially in high-temperature and high-humidity environments, is a life-threatening acute heat-injury disease that seriously endangers human health. Timely and effective on-site treatment is crucial for patients' survival and prognosis. Early recognition, rapid assessment, and on-site cooling are the core of pre-hospital treatment of heatstroke. Currently, there is a lack of standardized application procedures for pre-hospital emergency care of heatstroke. Therefore, the "Expert Consensus on Pre-hospital Emergency Management of Heatstroke (2024 edition)" was initiated by the Expert Group on Heatstroke Prevention of the People's Liberation Army and developed in collaboration with experts from local pre-hospital emergency care, emergency departments, and intensive care units. This consensus focuses on heatstroke prevention, on-site and ambulance-based treatment, and early emergency room interventions, and puts forward 10 evidence-based recommendations, aiming to provide a reference for scientific and standardized pre-hospital emergency care of heatstroke.

高温高湿环境下发生的热射病是威胁生命的急性热损伤性疾病,严重影响人类健康。及时有效的现场救治对患者生存和预后至关重要,早期识别、快速评估和现场降温是热射病院前救治的核心。目前,热射病的院前急救尚缺乏规范的应用流程。为此,由全军热射病防治专家组发起,联合地方院前急救、急诊、重症等领域的专家,聚焦于热射病的预防、现场及救护车上的处置、急诊室早期救治等方面,共同编写了《热射病院前急救专家共识(2024版)》,形成了10条推荐意见,旨在为科学规范地进行热射病的院前急救提供参考。

高温高湿环境下发生的热射病是威胁生命的急性热损伤性疾病,严重影响人类健康。及时有效的现场救治对患者生存和预后至关重要,早期识别、快速评估和现场降温是热射病院前救治的核心。目前,热射病的院前急救尚缺乏规范的应用流程。为此,由全军热射病防治专家组发起,联合地方院前急救、急诊、重症等领域的专家,聚焦于热射病的预防、现场及救护车上的处置、急诊室早期救治等方面,共同编写了《热射病院前急救专家共识(2024版)》,形成了10条推荐意见,旨在为科学规范地进行热射病的院前急救提供参考。

Heatstroke, especially in high-temperature and high-humidity environments, is a life-threatening acute heat-injury disease that seriously endangers human health. Timely and effective on-site treatment is crucial for patients' survival and prognosis. Early recognition, rapid assessment, and on-site cooling are the core of pre-hospital treatment of heatstroke. Currently, there is a lack of standardized application procedures for pre-hospital emergency care of heatstroke. Therefore, the "Expert Consensus on Pre-hospital Emergency Management of Heatstroke (2024 edition)" was initiated by the Expert Group on Heatstroke Prevention of the People's Liberation Army and developed in collaboration with experts from local pre-hospital emergency care, emergency departments, and intensive care units. This consensus focuses on heatstroke prevention, on-site and ambulance-based treatment, and early emergency room interventions, and puts forward 10 evidence-based recommendations, aiming to provide a reference for scientific and standardized pre-hospital emergency care of heatstroke.

全球变暖加剧和极端天气事件频发使高温对人类健康的影响愈加凸显[1]。2015-2019年欧洲发生了多次严重的极端高温事件[1],同时,我国上海市和浙江省等地也记录了大量高温日和热浪事件,导致热相关疾病的发病率明显增高[2-4]。热相关疾病包括热痉挛、热晕厥、热衰竭和热射病,其严重程度逐渐加重。热射病是一种高温环境引起的严重疾病,其特征包括核心体温明显升高(常超过40 ℃)、中枢神经系统异常(如精神状态改变、抽搐、昏迷等),以及多器官功能受损[5],可分为经典型(非劳力型)和劳力型两种类型;如处理不当,病情发展迅速,预后差,死亡发生率高[6]。一项多中心研究显示,自2022年我国西南地区83家三甲医院收治了873例热射病患者,其中ICU收治的重症热射病患者病死率达20.7%,出院死亡发生率为34.1%[7];美国军队2023年的统计数据显示,热射病和热衰竭的罹患率分别为31.7/10万人年和172.7/10万人年,对军队战斗力造成了严重影响[8]。因此,普及推广热射病预防及处置方法,减少其对公共健康和军事行动的不利影响十分必要。
快速识别、快速评估、迅速降温和及时对症支持治疗是成功治疗热射病的关键环节[6,9-14]。研究显示,在发病后30 min内将患者体温降至38.5 ℃以下可明显降低病死率[2,10,12]。《中国热射病诊断与治疗专家共识》[6]《热射病急诊诊断与治疗专家共识(2021版)》[12]《军事训练防治中暑/热射病降温方法专家共识》[15]等强调,在发现中暑或热射病伤员后应在5 min内启动降温措施,目标是在30 min内将核心体温降至39.0 ℃以下,并在2 h内将其降至38.5 ℃以下。因此,有学者将热射病发生后的最初30 min、1 h称为“黄金半小时”和“黄金1小时”。由于道路拥堵、急救车数量不足、120电话占用等各种因素的综合影响,我国院前救护车到达现场的时间城市为10~15 min,乡镇则超过30 min[16-18]。但热射病患者发病时通常处于医院外甚至是荒山野地,专业医护人员不能第一时间到达现场,等待救护车到达再开展救治极有可能错过“黄金半小时”或“黄金1小时”,严重影响其救治成功率。因此,热射病的院前急救对于提高热射病患者的生存率和改善预后至关重要[9-10,12-13,16,19-21]
基于临床实践需求,根据循证医学证据并结合当前我国国情,在《中国热射病诊断与治疗专家共识》[6]和《热射病急诊诊断与治疗专家共识(2021版)》[12]的基础上制定热射病院前急救专家共识十分必要,对于提高我国院前热射病急救水平有着重要的指导意义。军队热射病防治专家组联合地方院前急救、急诊医学、重症医学等学科的专家,组成编写小组,围绕关键问题开展系统文献检索与筛选,提出相关推荐意见,2024年8月各方达成了共识,完成第4轮讨论,10月份定稿。专家成员对推荐意见逐一进行了表决,在最终轮投票进行表决时达成一致。推荐强度和循证证据等级见表12,参考推荐意见分级的评估、制定及评价方法(Grading of Recommendations Assessment, Development and Evaluation,GRADE)[22]
推荐意见1:热射病严重危害人类健康,加强对热射病的认识,推动早期干预、快速识别和迅速现场降温的开展,直接影响患者的预后和生存率(Ⅰ级证据,A级推荐)
推荐意见2:推荐对公众进行热射病防治健康教育,热射病防治“8-4-6”黄金法则可用于指导培训热射病的预防和院前处置(Ⅳ级证据,B级推荐)
《中国现场救护第一目击者行动公众指南》[23]和《院前急救待援期公众应对措施专家共识》[24]强调,当意外事件发生时,每个公民都应主动进行自救、互救和他救,这是一种社会责任。这一理念同样适用于热射病患者的救治。对于热射病,第一目击者及时有效的现场干预对于提高患者的生存率和改善预后至关重要。一项对1300多名研究对象进行的调查显示,我国普通居民对中暑的常见症状和风险因素有相对良好的认知,但对重度中暑的评估及处置相关知识了解较少,参加热射病急救培训的人数较少[25]
有效的健康教育可使公众意识到热射病不同于普通中暑,它是十分严重的急诊急症,可能危及生命。教育内容应包括热射病的定义、易感因素、预防措施以及早期处理方法等[23-24]。教育的对象不仅应包括高危人群(如消防员、运动员、军人、高温高湿环境下的劳动者、老人、儿童及患有慢性疾病或特定健康问题的人群),还应包括他们的单位管理人员、训练组织人员、工地安全监督人员及特殊人群的家属等[23-27]。大众传播媒体是公众获取热射病信息的主要途径[26-27]。通过网络、广播、电视、报纸、宣传板、宣传册及讲座等多种形式,定期开展广泛的健康教育活动,可让公众认识热射病、了解热射病。2023年发布的《中国现场救护第一目击者行动公众指南》针对中暑的现场救治,提出了立即撤离高温环境、物理降温、补充含盐饮料和重症者送医的院前急救链条[23]。宋青教授领导的全军热射病防治专家组在多年的实践中提出了劳力型热射病防治的“8-4-6”黄金法则(预防8项措施、预警4信号和现场处置6步法),简单易记,可用于指导热射病的普及培训[28]
推荐意见3:为提高热射病急救的效率和质量,推荐对全体院前急救人员进行热射病防治知识、技能的培训及考核(Ⅲ级证据,A级推荐)
院前急救的效率和质量对于热射病患者的预后至关重要,因此加强急救人员的专业培训是提高院前急救能力的必要前提[16]。然而,目前急诊医护人员对热射病的认知和熟悉度较低。一项研究显示,国内急诊医护人员仅30.9%掌握了热射病的诊断要点,而救治要点的掌握率更低,仅为13.3%[29]。国外也存在类似问题[30],即使是部分曾诊治过热射病的美国院前急救人员,对于热射病的认知和处理方法仍然存在较大不足,尤其是在核心体温测量和冷水浸泡降温方面的认知存在明显不足。急救人员须全面了解热射病的危险因素、临床表现及规范的急救流程。美国多数州的院前急救方案没有建议将冷水浸泡作为首选治疗方法,也没有强调“先降温、后转运”的指导原则[16,30]
针对这些问题,对院前急救人员(包括调度人员)进行专业培训和考核尤为重要[16,29-30]。这种培训能提高热射病的精准识别率、有效处置率、转运分流正确率,有效缩短降温时间、改善治疗效果、降低重症发生率。因此,熟练掌握热射病院前急救的诊疗常规、操作规范和时间指标应是每位急救人员的必备技能[31]。急救人员的专业培训应按照最新的专家共识和推荐指南进行,并及时更新、反复强化,以提升其认知水平和处理能力,显著提高热射病救治的整体质量。
推荐意见4:在发生热射病风险较高的行业、地区或季节,为提高救治效率,建议制定详尽的热射病救治预案,并确保充足的保障措施(Ⅲ级证据,A级推荐)
有效的保障措施和预案能够明显降低热射病的发生率和病死率,尤其在军事训练、大型赛事等高强度作业过程中。部队在这方面拥有丰富的经验和成功的防治方案。在高强度训练和大型演练中,部队通过科学的宣传教育、热习服训练、重点监控、快速识别和有效降温等措施,成功将热射病的发生率降至0[6,12,15,32]。2020年东京奥运会[19]和2023年杭州第19届亚运会[21]均使用了热射病防治措施,有效保障了运动员的健康及安全。这些方案包括设立专门的热射病现场救治单元、制定详尽的救治方案,以及实施预防措施。热射病防治“8-4-6”黄金法则中的8项预防措施(表3)要求,在高强度体力作业前,要进行必要的热习服,注意休息,避免带病劳作,准备好降温设备、耗材和水电解质等;作业过程中要注意观察及监测人员的身体状态,及时补充水分和盐分。为了更好地保障消防员、军人及进行高强度作业或高体力劳动的人员,在活动前应备足防暑降温设备和设施,制定详尽的应急预案,并根据可能发生的情况进行实际场景培训和流程推演。综合应用这些措施[现场救治需要的必备设备、耗材和药品等(表4)],可有效提升人员在高温环境下的安全性和健康保障水平,避免热射病的发生。
推荐意见5:现场人员应迅速识别热射病早期症状,注意预警“4”信号,以便及时采取适当措施进行处理。急救调度员接到报警电话后应指导现场人员根据病史和症状等进行识别处置(Ⅲ级证据,B级推荐)
早期识别是院前处置的关键步骤[6,9-10,12-13]。热射病临床表现不一,早期症状包括中枢神经系统功能紊乱,如混乱、烦躁或谵妄等非理性行为,严重时可导致昏倒或意识丧失。有时患者可能出现短暂的清醒期,随后中枢神经功能急剧下降。并排所有患者均会停止出汗、皮肤发热或昏迷,部分患者可能仍在出汗,皮肤感觉凉爽或湿冷。早期表现往往具有迷惑性,尤其是在经典型热射病或非典型病例中,可能仅表现为头晕、乏力、发热、腹痛或腹泻。在运动或劳动后,现场人员需要迅速识别潜在的劳力型热射病征兆。尽管热射病通常发生在高温高湿环境中,但在较冷条件下,剧烈运动也可能引发热射病。热射病的发展是动态的,早期临床表现可能较轻,但是短时间内有迅速进展的可能。热射病防治“8-4-6”黄金法则中的预警“4”信号强调“烫”“晃”“晕”“乱”4种主要表现(图1),尤其是神经功能障碍,简化了诊断的关键要点,易于掌握[25]。急救医疗服务系统(Emergency Medical Service System,EMSS)涵盖了热射病的院前处理,包括拨打120急救电话、响应、派遣、现场处理和转运,以及与接诊医院的协调。EMSS调度员是热射病院前急救的首要环节,他们在接到急救电话后,需要迅速识别可能的热射病患者,并指导现场人员进行降温治疗。
推荐意见6:现场评估热射病时,鼓励优先测量直肠温度,迅速评估中枢功能、生命体征,迅速处理和转运不稳定的患者,如出现心搏骤停,应及时实施心肺复苏(Ⅲ级证据,B级推荐)
核心体温监测对于热射病的病情评估至关重要,对于一些非典型或经典型热射病,核心体温测定直接影响现场评估和处置策略。直肠温度测量是现场评估核心体温的最佳方法;其他常见的体温测量部位,如鼓膜、口腔、前额和腋窝,均不能准确地评估核心体温[9-10,13]。如果无法测量直肠温度,现场应评估导致中枢功能障碍的情况、生命体征和中枢神经系统功能,以确定是否应将伤员作为中暑患者进行处理,不应延迟降温过程。高温高湿环境下,运动员或劳动者可能会出现晕倒的情况,要注意可能的诱因,包括心脏骤停、脑震荡、低钠血症、低血糖、运动相关体位性低血压等。因此,现场人员、急救人员应及时、反复地评估患者的意识状态、生命体征等变化,并密切关注核心体温的变化;如果患者出现生命体征不稳定甚至心跳骤停,应及时进行转运,并在必要时实施心肺复苏[9-10,13]
推荐意见7:在热射病的现场救治中,应迅速实施全身冷水浸泡以快速降低核心体温至38.5 ℃以下,同时密切关注并处理可能出现的其他急症(Ⅲ级证据,A级推荐)
一旦怀疑或诊断热射病,应按照热射病防治“8-4-6”黄金法则中的现场救治6步法进行处置(图2),立即启动降温措施。快速、有效、持续地降温是缓解过度热应激反应的关键,有效的治疗依赖于现场快速降温,目标是半小时内将体温降至38.9 °C以下,2 h内降至38.5 ℃以下,首选方法是全身冷水浸泡(CWI)。CWI通常将患者浸泡至20~26 ℃的冷水中,以0.11 ℃/min的速率进行降温。使用该方法患者几乎没有不适、颤抖或潜在的冷休克反应,并能达到很好的降温效果。也有研究提出水温越低,降温速率越快,治疗效果越好[33]。一旦体温降至38.5 ℃以下,应停止降温以避免低体温的风险。实际操作中可根据现场条件选择不同的降温方式(图3),但基本原则是:范围要覆盖足够的身体表面积以及确保足够的降温速率。在处理热射病时,患者可能出现腹泻、呕吐或意识不清等情况,应在不影响快速降温的前提下进行干预。少数情况下可能出现心律失常、癫痫或其他严重症状,应优先处理这些紧急情况,待稳定后立即重新开始降温[9-10,13]。应注意的是,降温对于在高温高湿环境中出现发热或意识障碍等非热射病患者不会造成严重的不良反应[20]。迅速降温是热射病救治的最有效处置措施。此外,降温有时间要求和注意要点(表5)。许多民间做法或偏方[23],如指掐人体穴位、口服中药制剂以及局部涂抹清凉油或风油精等做法,缺少循证依据,本文不做评价,但不能替代降温。
推荐意见8:救护车应配备直肠温度计和有效的降温设备,并在转运热射病患者时持续监测体温,确保体温维持在38.5 ℃以下,同时进行液体补充和对症支持,以提高救治效果(Ⅲ级证据,B级推荐)
热射病现场救治的标准做法是“先降温、后转运”[3,9-10,12-15]。根据2020年东京奥运会热射病管理指南,强调必须首先进行有效的降温[19]。需确认运动员体温在降温后能稳定至少15 min,且在此期间意识恢复正常,医疗团队才能启动转运程序,将患者送往医院。然而,如果现场没有合适的降温设施,或者即使施行了降温措施患者的意识状态仍未改善,或体温仍然居高不下,或生命体征不稳定,应考虑尽快将患者转移到能够提供适当治疗的医院。在救护车转运过程中,关键是能够持续监测患者的核心体温,且确保核心体温维持在38.5 ℃以下;同时要求能够进行快速的液体补充和对症支持治疗,必要时给予人工通气、镇静等措施。常见的降温方法包括降低环境温度(如调整空调设置或者打开车窗),使用降温毯、多部位冰块冷敷、扇风等。救护车可配备专用的降温设备(图4),如转运降温担架或足够的冰水,用冷毛巾全身擦拭或冰水被子覆盖患者的躯干;也可进行低温静脉输液。在夏季热射病高发地区,救护车必须具备持续监测核心体温并实施降温的能力,以确保患者在转运过程中能够得到及时有效的救治(救护车车上救治需要的部分设备、耗材和药品等见表4)。国内针对降温设备有不少发明和改进,如《中华危重病急救医学杂志》报道的基于相变材料的热射病降温组件和分体式热射病急救服,可在快速降温的同时实时监测鼓膜温度,效果确切、携带便捷、可重复使用,可用于现场急救、转运及院内持续降温[34-35]
推荐意见9:医院急诊科应配备核心体温监测和持续降温设备,优化热射病处理流程,进行病情评估和鉴别诊断,同时与院前急救保持沟通,以确保患者得到及时、有效的救治(Ⅲ级证据,B级推荐)
当怀疑患有热射病时,应将患者转至医院进行进一步的评估和治疗[9-10,13]。院内急诊科应具备持续降温和监测核心体温的能力,并优化流程,必要时转入重症监护室(ICU)进行进一步治疗。在转运前或途中,要及时与目的地医院急诊科沟通,共同评估病情。一旦患者抵达急诊室,医师应进行全面的医学评估,并根据需要完善相关的化验检查,如血常规、凝血功能、生化检查,以及血浆肌红蛋白和肌酸激酶水平测定等,这些检查有助于评估热射病的严重程度、判断可能的器官损伤,并为制定治疗方案提供重要依据[36]。鉴别诊断必不可少,需要排除其他可能导致发热和意识障碍的疾病,如颅内感染、感染引起的高热和高血压脑出血等;同时,需要与凝血功能障碍相关的疾病如脓毒症和某些血液疾病进行鉴别[12]。急诊科应具备完善的热射病救治能力,包括准确的分诊和识别、持续的核心体温监测、快速有效的降温措施及对症支持治疗。高发地区或季节可考虑设立专门的热射病抢救单元(图5)。已有研究证实,急诊科实施早期快速降温、加强监测等集束化治疗策略,可明显提高热射病的抢救成功率,缩短住院时间[37]。连续性血液净化和亚低温治疗等技术手段能够有效用于热射病的降温处理[37-41]。然而,在应用这些治疗措施时,仍需综合考虑其经济成本。
推荐意见10:建议建立区域性热射病救治网络,通过优化急救资源配置、规划不同水平医院的救治能力,并利用移动和远程医疗技术,实现信息共享和协同救治,确保热射病的及时、有效处理(Ⅲ级证据,B级推荐)
热射病的诊治与急性心肌梗死、脑卒中和创伤等疾病一样,具有明显的时间敏感性[9-10,13,42-45]。早期救治窗口短暂,因此,卫生健康部门应当引导建立区域性的热射病救治体系(图6),合理设置和规划热射病院前医疗急救网络,评估和建设不同水平的热射病救治能力医院,形成急救中心(站)与不同能力医院的区域协同救治体系,同时要特别重视完善热射病批量伤员的救治流程[35]。此外,应利用移动和远程医疗技术,建立信息双向衔接共享机制,促进院前急救和网络医院间转诊、接诊流程的科学化。急救系统应制定热射病院前急救的质控标准和关键性绩效指标,加强各个环节的质控,最大限度地落实现有的循证医学指南。建立热射病急救质量评价和改进相关数据库,及时、完整地收集数据,以便于量化考核及改进各项指标。院前急救机构应定期对热射病急救质量进行评估,建立持续改进机制。
在热射病的急救中,迅速降低核心体温是关键,尤其是在“黄金半小时”或“黄金1小时”内将体温降至安全水平[1,6,12]。为减少院前和院内的时间延误,可借鉴胸痛救治单元的成功经验,采取一系列优化措施[46-47]。首先,可在基层卫生单位设立专门的热射病降温单元,配置冷却设备和温度监测仪器,以提供初步的紧急处理。其次,救护车应装备核心体温监测和降温系统,形成移动降温单元,实现对患者体温的实时监测和调控,从而在转运过程中迅速有效地降低体温,提高治疗效果。院内急诊科应设立专门的热射病抢救单元,配备先进的监测设备和快速降温系统,以确保患者到达医院后能立即接受有效治疗。
此外,参照《中国心脏猝死防治救体系建设蓝皮书(2022)》[48],热射病预防和院前救治也应该纳入相应的健康公益体系中,将相关处置指南科普化,科技成果民用化,做到科普惠民。增强公众对热射病急救的意识和能力至关重要。应开展广泛的急救培训,教育公众如何识别和应对热射病,以形成“公众现场自救互救—院前急救—院内急救”三位一体的急救链条[23,25]。利用移动智能终端,建立综合的热射病急救信息平台,实现患者信息的实时传输、急救过程的记录、GPS定位服务,以及急救医院的查询推荐。这些措施不仅可提升急救响应速度和处理效率,还能加强院前和院内的协同作业,确保热射病患者得到及时和有效的治疗。因此,这些优化措施将明显提升热射病急救工作的效果,成为未来急救领域的重要发展方向。
