Objective To assess the effectiveness of the evaluation of military physical function (EMPF) system in predicting the occurrence of military training injuries among new recruits to provide scientific guidance and methodological choice for military training. Methods A total of 527 new recruits from 5 grassroots units from July 2016 to February 2018 were selected for the study. The recruits underwent EMPF testing, and their military training injuries were monitored over a 2-year follow-up period. Those who sustained injuries during training were divided into injury group (n=163), while the remaining recruits were placed in healthy group (n=364). The predictive ability of the total EMPF score for training injuries was assessed using the receiver operating characteristic curve (ROC), and the correlation between the total EMPF score, individual test scores, and military training injuries were analyzed using binary logistic regression. Results The total EMPF score of new recruits in injury group (19.52±1.97) was significantly lower than that of healthy group (24.31±1.54) (P<0.001), which also demonstrated a high diagnostic value in predicting the risk of military training injuries, with an area under the curve (AUC) of ROC of 0.971 (P<0.001). A cut-off value of 22 scores was found to have the highest accuracy in predicting future training injuries, with an odds ratio (OR) of 25.63, sensitivity of 0.939, specificity of 0.879, positive likelihood ratio of 7.76, and a post-test probability of 0.67. Binary logistic regression analysis revealed that 6 EMPF tests, including holding the ball over and leaning back, bending forward and touching the ground with the ball, lunge squat and twist, swallow balance with holding the ball afterward, vertical jump, and respiratory pattern assessment, were negatively associated with the risk of military training injuries (P<0.0001). Conclusion The EMPF system can effectively predict the risk of military training injuries, with military personnel whose total EMPF score is less than 22 being at higher risk of sustaining such injuries.

目的 分析身体运动功能评估(EMPF)系统在新兵军事训练伤风险评估中的应用价值,以期为军事训练提供科学指引和方法选择。方法 选取2016年7月-2018年2月5支基层部队中的527名新兵进行回顾性分析。所有新兵使用EMPF系统进行测试,并通过为期2年的随访记录其军事训练伤的发生情况。将军事训练伤的新兵作为伤病组(n=163),其余作为健康组(n=364),利用ROC曲线评估EMPF总分预测训练伤风险的能力,并采用二元logistic回归分析EMPF各项评分及总分与军事训练伤之间的相关性。结果 伤病组新兵的EMPF总分明显低于健康组[(19.52±1.97)分 vs. (24.31±1.54)分,P<0.001];利用EMPF总分评估训练伤风险具有较高的价值,其ROC曲线下面积为0.971(P<0.001),当截断值为22分时,预测后期发生训练伤的准确度最高,其比值比(OR)为25.63,敏感度为0.939,特异度为0.879,阳性似然比为7.76,验后概率为0.67。二元logistic回归分析显示,EMPF系统的10项测试中,双手持球过顶后仰触背、立位体前屈持球触地、持球弓箭步行走转体、持球后跨步燕式平衡、原地纵跳及呼吸模式评估6项评分与军事训练伤的发生呈明显负相关(P<0.001)。结论 EMPF系统可用于预测军事人员的训练伤风险,EMPF总分低于22分的人员后期更可能罹患军事训练伤。

目的 分析身体运动功能评估(EMPF)系统在新兵军事训练伤风险评估中的应用价值,以期为军事训练提供科学指引和方法选择。方法 选取2016年7月-2018年2月5支基层部队中的527名新兵进行回顾性分析。所有新兵使用EMPF系统进行测试,并通过为期2年的随访记录其军事训练伤的发生情况。将军事训练伤的新兵作为伤病组(n=163),其余作为健康组(n=364),利用ROC曲线评估EMPF总分预测训练伤风险的能力,并采用二元logistic回归分析EMPF各项评分及总分与军事训练伤之间的相关性。结果 伤病组新兵的EMPF总分明显低于健康组[(19.52±1.97)分 vs. (24.31±1.54)分,P<0.001];利用EMPF总分评估训练伤风险具有较高的价值,其ROC曲线下面积为0.971(P<0.001),当截断值为22分时,预测后期发生训练伤的准确度最高,其比值比(OR)为25.63,敏感度为0.939,特异度为0.879,阳性似然比为7.76,验后概率为0.67。二元logistic回归分析显示,EMPF系统的10项测试中,双手持球过顶后仰触背、立位体前屈持球触地、持球弓箭步行走转体、持球后跨步燕式平衡、原地纵跳及呼吸模式评估6项评分与军事训练伤的发生呈明显负相关(P<0.001)。结论 EMPF系统可用于预测军事人员的训练伤风险,EMPF总分低于22分的人员后期更可能罹患军事训练伤。

Objective To assess the effectiveness of the evaluation of military physical function (EMPF) system in predicting the occurrence of military training injuries among new recruits to provide scientific guidance and methodological choice for military training. Methods A total of 527 new recruits from 5 grassroots units from July 2016 to February 2018 were selected for the study. The recruits underwent EMPF testing, and their military training injuries were monitored over a 2-year follow-up period. Those who sustained injuries during training were divided into injury group (n=163), while the remaining recruits were placed in healthy group (n=364). The predictive ability of the total EMPF score for training injuries was assessed using the receiver operating characteristic curve (ROC), and the correlation between the total EMPF score, individual test scores, and military training injuries were analyzed using binary logistic regression. Results The total EMPF score of new recruits in injury group (19.52±1.97) was significantly lower than that of healthy group (24.31±1.54) (P<0.001), which also demonstrated a high diagnostic value in predicting the risk of military training injuries, with an area under the curve (AUC) of ROC of 0.971 (P<0.001). A cut-off value of 22 scores was found to have the highest accuracy in predicting future training injuries, with an odds ratio (OR) of 25.63, sensitivity of 0.939, specificity of 0.879, positive likelihood ratio of 7.76, and a post-test probability of 0.67. Binary logistic regression analysis revealed that 6 EMPF tests, including holding the ball over and leaning back, bending forward and touching the ground with the ball, lunge squat and twist, swallow balance with holding the ball afterward, vertical jump, and respiratory pattern assessment, were negatively associated with the risk of military training injuries (P<0.0001). Conclusion The EMPF system can effectively predict the risk of military training injuries, with military personnel whose total EMPF score is less than 22 being at higher risk of sustaining such injuries.

军事体能训练是提高官兵身体素质及部队战斗力的重要方法,随着军事体育训练大纲的颁布实施与实战化训练的深入推进,训练科目的增多及训练强度的增大对官兵的身体素质、战斗素养提出了更高的标准和更严的要求。军人具有健康体魄是保证高水平战斗力的前提,预防官兵训练伤的发生是目前研究的重点[1]。功能性动作筛查(functional movement screen,FMS)是运动康复领域的重要评估工具,可通过评估个体基本运动模式有效识别无症状个体运动模式中的薄弱环节,通过针对性纠正训练加以强化,从而预防运动损伤的发生[2],在我国多项体育运动中已广泛应用[3],在部分军队中亦有所涉及[4-5]。然而,FMS作为复合性筛查工具,虽然测试过程简易,但应用于基层部队综合分析的难度较大;此外,其设置的测试项目也未针对我国官兵军事训练内容加以优化。鉴于此,全军军事训练医学研究所在多年指导部队体能训练及运动损伤防治现场调研和临床实践的基础上,逐渐发展并完善了对于军人身体功能进行系统化评估的方法体系,即军人身体运动功能评估(evaluation of military physical function,EMPF)系统,该系统也适用于地方卫生机构及体育部门[6]
EMPF系统是一套用以检测受训人员整体动作控制稳定性、身体平衡能力、柔韧性以及本体感觉等能力的检测方法;通过其中的功能动作检测,可简易识别个体的功能限制和不对称发展。EMPF系统的特点是测试易操作、评价简单。虽然EMPF系统已逐渐走入大众视野,但目前其实际应用相关的研究暂无报道,且在识别新兵军事训练伤风险中的应用价值尚未验证。本研究以5支基层部队新兵为研究对象,采用EMPF系统对其运动功能进行评估并依据《军事训练伤诊断与防治原则专家共识(2022版)》[7]进行为期2年的训练伤诊断与统计,验证EMPF系统预测新兵军事训练伤的有效性,以期为EMPF系统的实际应用提供科学指引和方法支撑。
选取全军军事训练研究所建立的军事训练伤防治健康档案数据库2016年7月-2018年2月5支基层部队中属于《军事体育训练大纲》中陆军二类人员的新兵共计527名进行回顾性分析。均为男性,年龄18~24(20.5±3.2)岁。纳入标准:(1)有明确军事训练参训史;(2)因军事训练导致的软组织损伤、骨与关节损伤、器官损伤等;(3)体格检查有对应损伤症状和体征,或影像学检查和生化检查存在与损伤相关的异常改变。排除标准:(1)病理性损伤,如周围血管病变、软组织病变、器官病变、骨质疏松、骨肿瘤等;(2)生理性疼痛,如训练导致的肌肉酸痛、肌肉痉挛等;(3)非军事训练导致的软组织、骨与关节或器官外伤。参照《军事训练伤诊断与防治原则专家共识(2022版)》[7],由各部军医随访记录新兵伤病情况,经上级医院复查,将罹患军事训练伤的新兵作为伤病组(n=163),其余作为健康组(n=364)。本研究获全军军事训练医学研究所伦理委员会批准(WZLL-2024-012)。
收集所有新兵的一般资料,包括年龄、身高、体重、体重指数(body mass index,BMI)等。采用EMPF系统测试军人的身体功能,包括7个动作模式测试和3个身体功能测试,其中动作模式测试包括:(1)双手持球过顶后仰触背;(2)立位体前屈持球触地;(3)立位持球旋转;(4)双手持球过顶深蹲;(5)持球弓箭步行走转体;(6)持球后跨步燕式平衡;(7)原地纵跳;主要考量受试者的灵活性、稳定性、力量和柔韧性等关键身体素质。身体功能测试包括:(1)呼吸模式;(2)折返跑;(3)心血管功能;主要评估受试者的呼吸功能、速度和灵敏性以及心血管功能水平。整套测试具体内容、示意图及评分标准详见附表1(https://dx.doi.org./10.11855/j.issn.0577-7402.1428.2024.0813FJ)。
分别于2016年7月21日-2016年7月31日及2017年7月14日-2017年7月24日,由获得全军军事训练医学研究所EMPF筛查培训课程合格证书的军医分别前往5支基层部队,携带测试器材(篮球、秒表、量角器、皮尺等),于现场完成EMPF测试及评分。测试时间均为上午8:00-12:00,地点为各部体能训练场。新兵集合后,由军医解释测试目的、方法、流程,并向参与测试的新兵示范标准动作后,正式开始测试,流程如下:(1)测量受试者的静息心率;(2)受试者进行呼吸模式测试;(3)受试者依次完成7项动作模式测试,每个动作完成3次,终末动作停留2 s;(4)受试者完成10 m×4折返跑,记录用时;(5)测量受试者运动后即刻心率;(6) 1 min后再次测量心率;(7)完成评分并记录。
比较两组新兵的一般资料,分析EMPF总分及各项评分与训练伤发生的相关性,并确定EMPF总分的截断值;以>截断值和≤截断值为暴露因素,分析暴露与非暴露新兵伤病率之间的差异;最后分析EMPF各项评分与运动损伤的相关性,评估其对运动损伤危险性的预测价值。
采用SPSS 23.0软件进行统计分析。经Kolmogorov-Smirnov检验后,所有计量资料满足正态分布,以$\bar{x}±s$表示,两组间比较采用独立样本t检验;计数资料以例(%)表示,两组间比较采用Person χ2检验。采用二元logistic回归分析EMPF总分及各项评分与训练伤发生的相关性,采用ROC曲线及曲线下面积(AUC),并计算Youden指数以确定EMPF总分的截断值。采用Pearson χ2检验分析暴露与非暴露新兵伤病率之间的差异,并计算比值比(OR);采用二元logistics回归分析,采用向前剔除法逐步拟合预测模型,Hosmer-Lemeshow检验验证模型拟合度,分析EMPF各项评分与运动损伤的相关性。P<0.05为差异有统计学意义。
伤病组与健康组年龄、身高、体重、BMI等一般资料比较,差异均无统计学意义(P>0.05,表1)。
伤病组新兵EMPF总分明显低于健康组,差异有统计学意义[(19.52±1.967) 分 vs. (24.31±1.539)分,t=27.59,P<0.001]。以新兵是否罹患伤病为因变量,以EMPF总分为自变量进行二元logistic回归分析,结果显示,EMPF总分与罹患伤病之间呈明显负相关(β=-1.567,P<0.001)。
以新兵是否罹患伤病为因变量,EMPF各项评分为自变量进行多元logistic回归分析,结果显示,经6步迭代拟合后,模型总体拟合度良好,Hosmer-Lemeshow检验示“观测值”与“期望值”差异无明显统计学意义(χ2=1.54,P=0.992)。由表2可见,双手持球过顶后仰触背、立位体前屈持球触地、持球弓箭步行走转体、持球后跨步燕式平衡、原地纵跳及呼吸模式评估6项评分与罹患伤病之间呈明显负相关(P<0.001)。
ROC曲线分析结果显示,EMPF总分预测新兵运动损伤风险的AUC为0.971(95%CI 0.958~0.984,P<0.001,图1)。
基于SPSS 23.0软件图形下ROC曲线分析,输出结果中各可能分界点分值的敏感度和特异度(表3)。当截断点分值为22时Youden指数最大(0.818),因而以EMPF总分22分为截断值进行后续分析。
以新兵EMPF总分分为>22分与≤22分,以及是否伤病绘制四格表(表4),EMPF总分>22分的新兵共330名,其中10名后期罹患军事训练伤,伤病率为3.0%;EMPF总分≤22分的新兵共197名,其中153名后期罹患军事训练伤,伤病率为77.7%。Pearson χ2检验显示,EMPF总分≤22分新兵的伤病率高于EMPF总分>22分的新兵,差异有统计学意义(χ2=321.65,P<0.001),OR值为25.63。
以22分为截断值时,EMPF总分预测伤病情况的敏感度为0.939,特异度为0.879,阳性似然比为7.76。根据2015年全军新兵运动损伤流行病学调查,以22%为验前概率,验前概率比为0.28,验后概率比为2.03,计算得验后概率为0.67。因而以EMPF总分22分预测伤病具有较高的敏感度和特异度。
近年来,我军军事训练更加面向实战,训练强度、难度有所加大,军事训练伤的发生率亦有增高趋势,因此,新兵军事训练伤的预防尤为重要[8]。EMPF系统是由全军军事训练伤研究所提出,根据新《军事体育训练大纲》对军人体质体能的新要求,遵循战斗力生成模式和人体运动结构功能设计而成,旨在通过评估识别出使受试者处于较高受伤风险的危险信号和代偿动作,确认受试者运动模式最薄弱的环节,并对薄弱环节进行矫正训练或康复治疗,从而达到降低训练损伤风险和训练伤发生率、促进训练效果提升的目的[6]。为进一步推广EMPF系统在我军各部的应用,并为其应用提供科学指引及方法支撑,本研究通过为期2年的随访,验证了EMPF系统预测新兵军事训练伤的有效性。
有研究表明,伤病组EMPF评分明显低于健康组,其ROC曲线下面积为0.971,验证了EMPF对于军事训练伤的预测价值[9]。此外,本研究确定了EMPF预测发生军事训练伤的阈值为22分(敏感度为0.939,特异度为0.879),EMPF评分≤22分的新兵训练损伤发生率明显高于>22分新兵(77.7% vs. 3.0%),OR高达25.63,提示在应用EMPF系统时,可以22分作为最佳风险预测阈值指导筛查,对于总分≤22分者应予以特别重视,尽早通过针对纠正性训练改善其潜在的肌力不平衡、关节功能不良等薄弱环节。然而,军事训练伤的致伤因素复杂多样,且本研究仅纳入二类人员作为评估对象,因此建议各单位具体使用时可根据实际情况对该阈值做出适当调整。本研究logistics回归分析确定了EMPF总分及6项测试评分可更好地预测军事训练伤的发生风险,其回归系数β<0,同时OR<1,表明得分越高,新兵未来罹患军事训练伤的风险越低[9]。然而,有4项测试(立位持球旋转、双手持球过顶深蹲、折返跑及心血管功能)未被纳入回归模型,表明其预测军事训练伤风险的能力较差,笔者认为可能是由测试时标准不够统一,或样本量较小所致,亦有可能是其评测方法及评分标准存在不足,因此,EMPA系统仍需不断优化以提高其应用价值。
相较于国际上已广泛使用的FMS系统[2],EMPF系统以其分层性设计具有独特优势:第一级3个动作模式可评估运动结构,分别对应人体屈伸旋转结构功能,可筛查受试者由于疼痛或双侧对称性欠佳造成的运动能力异常,当全身各关节出现结构异常或炎症反应(如腰椎间盘突出症、膝关节炎、肩周炎等)时,更容易在这3个动作模式中有所表现[10]。第二级4个动作模式可评估运动功能,分别对应重心改变、旋转、位移改变以及上下肢推拉,可识别受试者机体运动功能不良,如关节功能异常、肌力薄弱等导致的动作完成不佳[11]。最后3个测试项目属运动效能评估,综合性评估受试者的运动表现:如呼吸模式与机体核心功能紧密联系,可极大程度地影响身体的稳定性;心血管功能测试可反映心肺耐力;折返跑则是直线与变向速度素质的体现[12]。这种评价方法可以测评出受试人员的基本体能素质(力量、耐力、柔韧、速度、灵敏),从灵活性到稳定性,从静态到动态对人员的功能性动作做出完整的评估。此外,采用EMPF系统进行评估的过程中,“疼痛”是整个评价系统的分水岭,决定了受试新兵接下来的3个去向:第一,评估过程中出现疼痛的受试者应该寻求医师或物理治疗师的帮助;第二,评估过程中没有疼痛,但出现功能动作障碍的受试者则需要进行纠正性练习[13];第三,评估过程中没有疼痛,也没有动作障碍的受试者可直接进行体适能水平提高和专项技能方面的训练。
从现代运动训练视角来看,军人的体能训练和专项动作技能均应建立在基本动作模式之上。然而现实中很多新兵不仅没有科学系统地完成这一步,反而通过挤占本应发展基本动作模式的空间来发展体能和专项动作技能,其结果就是急性非接触损伤和慢性劳损的高频率发生[14]。本研究通过在基层部队实际应用EMPF系统,发现部分在入伍前有一定体适能水平的新兵,其在呼吸功能与心血管功能方面表现优异,却无法高效完成一些简单的评估动作,而是使用了代偿性的动作模式[3]。这种代偿性动作并不符合最佳力学结构,且通常无显著临床症状[15],军人个体主观难以发现。这种错误的动作模式不利于训练中以最少的能量消耗获得最佳的运动表现,也是导致未来易发生训练伤的主要原因[5]。因而部队各基层单位应利用好EMPF系统这一工具,提高对入伍新兵基本动作模式评估的重视,广泛、常态化开展对入伍新兵的EMPF系统筛查,从而有效识别受试者存在的运动结构、动作模式及运动表现异常,进而指导对不同类型人群开展更为合理的军事训练,从而协助部队实施个体化科学组训,实现训练伤的早发现、早纠正、早预防。
综上所述,本研究通过对新兵进行功能性动作测试,建立了EMPF系统,可用于观察新兵动作功能变化与体能素质变化,预测运动损伤风险及了解纠正性训练效果。在EMPF总分22分为截断值时具有最高的预测运动损伤风险效度,测试总分≤22分的新兵更易发生运动损伤。未来应利用好EMPF系统这一工具,促进EMPF系统在我军军事体能训练过程中的常态化开展,在我军军事体能训练效果提升及训练伤病防控中发挥更重要的作用。
