Objective To investigate the diagnostic efficacy of serum lipids in patients with neuromyelitis optica spectrum disorders (NMOSD), and to analyze the relationship between serum lipids and clinical features of NMOSD. Methods The clinical and laboratory diagnosis information of 324 patients with NMOSD (NMOSD group), 147 patients with multiple sclerosis (MS, MS group) and 252 healthy controls (HCs, HCs group) were collected from the First Medical Center of PLA General Hospital from January 2018 to July 2020. Logistic regression analysis was used to control the age, gender, disease duration, affected sites and the interaction of various serum lipid indexes. The differences of serum total cholesterol (TC), triglyceride (TG), Apolipoprotein A1 (Apo A1), Apolipoprotein B (Apo B), high-density lipoprotein (HDL) and low-density lipoprotein (LDL) in three groups of patients were analyzed, and the concentration differences of each index in the acute stage, relapse stage and remission stage of NMOSD were also analyzed. According to the clinical features of NMOSD, such as AQP4 antibody titer and affected sites, the concentration difference of lipid index in different subgroups was analyzed. Results The average disease duration in the NMOSD group was 20 months, and magnetic resonance imaging (MRI) showed that the most affected region is the optic nerve (198/324, 61.1%). In the MS group, the disease duration was longer (48 months) and multiple sites were affected (56/147, 38.1%). There were significant differences in TC, Apo A1, LDL, TG and Apo B between NMOSD group and MS group or HCs group (P<0.05). Subgroup analysis showed that only Apo B was significantly different in the acute stage, relapse stage and remission stage of NMOSD group (P<0.05). Further analysis on the subgroups of NMOSD showed that there were significant differences in TC, Apo A1, Apo B, HDL and LDL in different sites(P<0.05). The receiver operating characteristic (ROC) analysis showed that the combination of serum TC, Apo A1, Apo B and LDL could specifically diagnose NMOSD with the AUC of 0.732, sensitivity of 57.86% and specificity of 81.15%. Conclusion The combination of serum TC, Apo A1, Apo B and LDL can specifically diagnose NMOSD, and Apo A1 and Apo B can be used as indicators of affected sites and disease activity of NMOSD respectively.

目的 探讨视神经脊髓炎谱系疾病(NMOSD)患者血清脂类指标的诊断价值,并分析其与NMOSD临床特征的关系。方法 回顾性分析2018年1月—2020年7月于解放军总医院第一医学中心神经内科和眼科确诊的324例NMOSD患者(NMOSD组)、147例多发性硬化(MS)患者(MS组),以及252名无神经系统受累者(健康对照组)的临床及实验室诊断相关信息。采用logistic回归控制年龄、性别、病程、病变部位、各血清脂类指标间的相互作用,分析三组研究对象的血清总胆固醇(TC)、三酰甘油(TG)、载脂蛋白A1(Apo A1)、载脂蛋白B(Apo B)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)水平,以及各指标在NMOSD急性期、复发期、缓解期中的水平差异。根据AQP4抗体效价高低、受累部位不同等NMOSD临床特征进一步分组,分析各指标在不同亚组间的水平差异。结果 NMOSD患者中位病程为20个月,MRI显示受累部位主要为视神经(198/324,61.1%);MS患者中位病程较长(48个月),呈多部位受累(56/147,38.1%)。NMOSD组与MS组、健康对照组TC、Apo A1、LDL、Apo B水平差异有统计学意义(P<0.05)。亚组分析结果显示,NMOSD急性期、复发期、缓解期三个亚组中,仅急性期与复发期Apo B水平差异有统计学意义(P<0.05);NMOSD不同主要受累部位TC、Apo A1、Apo B、HDL、LDL水平差异有统计学意义(P<0.05)。ROC曲线分析显示,联合血清TC、Apo A1、Apo B、LDL预测NMOSD的AUC为0.732,敏感度为57.86%,特异度为81.15%。结论 联合血清TC、Apo A1、Apo B、LDL能较好地特异性诊断NMOSD,且Apo A1和Apo B可分别作为提示受累部位和NMOSD疾病活动程度的指标。

目的 探讨视神经脊髓炎谱系疾病(NMOSD)患者血清脂类指标的诊断价值,并分析其与NMOSD临床特征的关系。方法 回顾性分析2018年1月—2020年7月于解放军总医院第一医学中心神经内科和眼科确诊的324例NMOSD患者(NMOSD组)、147例多发性硬化(MS)患者(MS组),以及252名无神经系统受累者(健康对照组)的临床及实验室诊断相关信息。采用logistic回归控制年龄、性别、病程、病变部位、各血清脂类指标间的相互作用,分析三组研究对象的血清总胆固醇(TC)、三酰甘油(TG)、载脂蛋白A1(Apo A1)、载脂蛋白B(Apo B)、高密度脂蛋白(HDL)和低密度脂蛋白(LDL)水平,以及各指标在NMOSD急性期、复发期、缓解期中的水平差异。根据AQP4抗体效价高低、受累部位不同等NMOSD临床特征进一步分组,分析各指标在不同亚组间的水平差异。结果 NMOSD患者中位病程为20个月,MRI显示受累部位主要为视神经(198/324,61.1%);MS患者中位病程较长(48个月),呈多部位受累(56/147,38.1%)。NMOSD组与MS组、健康对照组TC、Apo A1、LDL、Apo B水平差异有统计学意义(P<0.05)。亚组分析结果显示,NMOSD急性期、复发期、缓解期三个亚组中,仅急性期与复发期Apo B水平差异有统计学意义(P<0.05);NMOSD不同主要受累部位TC、Apo A1、Apo B、HDL、LDL水平差异有统计学意义(P<0.05)。ROC曲线分析显示,联合血清TC、Apo A1、Apo B、LDL预测NMOSD的AUC为0.732,敏感度为57.86%,特异度为81.15%。结论 联合血清TC、Apo A1、Apo B、LDL能较好地特异性诊断NMOSD,且Apo A1和Apo B可分别作为提示受累部位和NMOSD疾病活动程度的指标。

Objective To investigate the diagnostic efficacy of serum lipids in patients with neuromyelitis optica spectrum disorders (NMOSD), and to analyze the relationship between serum lipids and clinical features of NMOSD. Methods The clinical and laboratory diagnosis information of 324 patients with NMOSD (NMOSD group), 147 patients with multiple sclerosis (MS, MS group) and 252 healthy controls (HCs, HCs group) were collected from the First Medical Center of PLA General Hospital from January 2018 to July 2020. Logistic regression analysis was used to control the age, gender, disease duration, affected sites and the interaction of various serum lipid indexes. The differences of serum total cholesterol (TC), triglyceride (TG), Apolipoprotein A1 (Apo A1), Apolipoprotein B (Apo B), high-density lipoprotein (HDL) and low-density lipoprotein (LDL) in three groups of patients were analyzed, and the concentration differences of each index in the acute stage, relapse stage and remission stage of NMOSD were also analyzed. According to the clinical features of NMOSD, such as AQP4 antibody titer and affected sites, the concentration difference of lipid index in different subgroups was analyzed. Results The average disease duration in the NMOSD group was 20 months, and magnetic resonance imaging (MRI) showed that the most affected region is the optic nerve (198/324, 61.1%). In the MS group, the disease duration was longer (48 months) and multiple sites were affected (56/147, 38.1%). There were significant differences in TC, Apo A1, LDL, TG and Apo B between NMOSD group and MS group or HCs group (P<0.05). Subgroup analysis showed that only Apo B was significantly different in the acute stage, relapse stage and remission stage of NMOSD group (P<0.05). Further analysis on the subgroups of NMOSD showed that there were significant differences in TC, Apo A1, Apo B, HDL and LDL in different sites(P<0.05). The receiver operating characteristic (ROC) analysis showed that the combination of serum TC, Apo A1, Apo B and LDL could specifically diagnose NMOSD with the AUC of 0.732, sensitivity of 57.86% and specificity of 81.15%. Conclusion The combination of serum TC, Apo A1, Apo B and LDL can specifically diagnose NMOSD, and Apo A1 and Apo B can be used as indicators of affected sites and disease activity of NMOSD respectively.

视神经脊髓炎谱系疾病(neuromyelitis optica spectrum disorders,NMOSD)和多发性硬化(multiple sclerosis,MS)都属于中枢神经系统的炎性脱髓鞘疾病[1-3],两者早期均表现为视力异常和脊髓炎等症状,在临床上难以区分。2004年,Lennon等[4]首次发现水通道蛋白4抗体(aquaporin-4-IgG,AQP4-IgG)对NMOSD的诊断价值,显著提高了NMOSD的临床诊断率并可有效与MS鉴别,但该抗体的诊断特异度仅为75%[5]。随后,髓鞘少突细胞胶质蛋白(myelin oligodendrocyte glycoprotein IgG,MOG-IgG)被发现,补充了对AQP4-IgG阴性NMOSD患者的诊断[6-7]。但这两种与NMOSD密切相关的抗体均无法反映疾病的活动度和严重程度,目前缺少能够预测NMOSD发生发展的良好指标,增加了早期NMOSD临床诊断及鉴别的难度[8]。脂蛋白参与脂质代谢且与炎症反应密切相关[9]。有研究发现,MS患者血清中胆固醇和低密度脂蛋白(low-density lipoprotein,LDL)水平增高,且与疾病的活动度和残疾程度相关,其中LDL可能在MS早期即通过损伤的血脑屏障进入病变部位并被氧化修饰,且很可能与患者脊髓T2病变加重有关[10-12]。目前针对血清脂蛋白在NMOSD发生发展中的变化以及NMOSD与MS之间差异的研究较少。本研究探讨了NMOSD患者血清脂类指标的诊断效能,并分析其与NMOSD临床特征的关系,以期发现有意义的指标用于指导临床早期诊断。
回顾性分析2018年1月—2020年7月于解放军总医院第一医学中心神经内科和眼科确诊的324例NMOSD患者(设为NMOSD组)、147例MS患者(设为MS组),以及252名确定无神经系统受累的健康查体成人(设为健康对照组)的临床及实验室诊断相关信息。根据国际诊断小组制定的NMOSD和MS诊断标准[13-14],入组的NMOSD和MS患者满足临床症状和影像学诊断标准;排除高血压、糖尿病、肿瘤、感染、流产、外伤等相关干扰性疾病的影响。NMOSD和MS的疾病进程均分为急性期、复发期、缓解期。其中急性期是指首次发病,未进行血清及脑脊液相关检测,仅因视力异常或脊髓异常等其他症状就诊;复发期是指原有症状加重或者出现新的症状或体征;缓解期是指病情在1个月内未加重或未出现新的临床症状,病情稳定。
收集研究对象的一般资料[包括年龄、性别、体重指数(body mass index,BMI)]、疾病相关信息[包括首发症状、NMOSD诊断分期、病程、特异性抗体表达及其效价(部分患者无效价信息)、磁共振成像(magnetic resonance imaging,MRI)高信号部位、临床诊断]及血清脂类指标检测结果。病程定义为起始发病到该次复发就诊的时间。MRI高信号部位分为视神经、脊髓、颅脑、合并多处等。本研究删除了敏感信息,并对数据进行匿名处理。本研究经解放军总医院医学伦理委员会批准(S2021-128-01)。
患者入院次日清晨采集空腹静脉血3~5 ml,使用罗氏Cobas®8000全自动生化分析仪及原装诊断配套试剂[胆固醇检测试剂盒(酶比色法)、三酰甘油(triglyceride,TG)检测试剂盒(比色法)、载脂蛋白A1(apolipoprotein A1,Apo A1)检测试剂盒(免疫比浊法)、载脂蛋白B(apolipoprotein B,Apo B)检测试剂盒(免疫比浊法)、高密度脂蛋白(high-density lipoprotein,HDL)检测试剂盒(酶比色法)、LDL检测试剂盒(酶比色法)]检测血清总胆固醇(total cholesterol,TC)、TG、Apo A1、Apo B、HDL、LDL水平。
采用SPSS 25.0和MedCalc 19软件进行统计分析。计量资料采用Kolmogorov-Smirnov法进行正态性检验,符合正态分布的数据以$\bar{x}±s$表示,多组间比较采用方差分析,进一步两两比较采用Bonferroni法检验;采用logistic回归控制混杂因素(年龄、性别、病程、病变部位、各血清脂类指标间的相互作用等),分析各指标在各组和各分期中的差异。非正态分布的数据以M(Q1Q3)表示,组间比较采用Mann-Whitney U检验。计数资料以例(%)表示,组间比较采用χ2检验。采用MedCalc 19软件绘制受试者工作特征(receiver operating characteristic,ROC)曲线预测指标的诊断效能,并根据Youden指数选择最佳截断值。P<0.05为差异有统计学意义。
纳入的324例NMOSD患者中,多数处于复发期(63.9%,207/324),其次为急性期(29.6%,96/324),仅21例(6.5%,21/324)处于缓解期;147例MS患者中,多数处于复发期(68.0%,100/147),其次为缓解期(23.1%,34/147),仅13例(8.8%,13/147)处于急性期。NMOSD组患者的平均发病年龄为40.0岁,较MS患者发病晚;NMOSD组与MS组患者均以女性为主(分别为85.5%、61.2%),健康对照组男女比例接近1:1(53.6%:46.4%),三组性别差异有统计学意义(χ2=100.48,P<0.001)。NMOSD组患者中位病程为20个月,多因视力异常(67.0%,217/324)就诊,MRI检查显示以视神经受累为主(61.1%,198/324)。MS组患者中位病程(48个月)约为NMOSD患者的2倍,多因脊髓炎等其他症状(68.7%,101/147)就诊,约1/3的患者MRI检查显示多部位受累。NMOSD组与MS组患者的受累部位差异有统计学意义(F=183.7,P<0.001)。NMOSD患者分期不同,主要受累部位差异亦有统计学意义(F=23.2,P<0.001)(表1)。
表2所示,NMOSD组与MS组TG、HDL水平差异无统计意义(P=0.484、P=0.160),TC、Apo A1、Apo B、LDL水平差异有统计学意义(P<0.05)。NMOSD组各指标水平均高于MS组和健康对照组,且血清脂类指标能明显区分NMOSD组与健康对照组(P<0.05)。对NMOSD不同分期患者血清脂类指标进行分析,结果显示,急性期与复发期患者仅血清Apo B水平差异有统计学意义(P=0.031,表3)。
进一步分析血清脂类指标与不同主要受累部位的关系,结果显示,NMOSD主要受累部位不同的患者TG水平差异无统计学意义(F=1.647,P=0.178),TC、Apo A1、Apo B、HDL、LDL水平差异有统计学意义(F=3.742、5.384、4.748、6.553、2.839,P<0.05),其中TC、Apo A1和HDL在视神经受累时水平较高。多重比较结果显示,仅Apo A1在视神经受累时可与脊髓受累和颅脑受累明显区分(P=0.016、0.032)。MS主要受累部位不同的患者各指标差异均无统计学意义(P>0.05)(表4)。NMOSD患者血清AQP4-IgG效价为1:10~1:3200,仅LDL在不同抗体效价亚组间差异有统计学意义(F=2.467,P<0.05,表5),且急性期与复发期NMOSD患者AQP4-IgG效价主要为1:32(表6)。
ROC曲线分析结果显示,HDL预测NMOSD的敏感度较高(72.98%),但NMOSD组与MS组患者HDL水平差异无统计学意义(P=0.160,表2)。TC、Apo A1预测NMOSD的曲线下面积(area under curve,AUC)分别为0.700、0.701,但敏感度较低(表7)。如图1所示,联合TC、Apo A1、Apo B、LDL预测NMOSD的AUC为0.732,敏感度为57.86%,特异度为81.15%。
本研究发现,NMOSD与MS患病人群主要为40岁左右的女性,与既往文献报道一致[15-16]。虽然女性群体NMOSD发病率较高的具体原因尚不清楚,但在中枢神经系统自身免疫性疾病中,这种性别差异的确存在[17]。NMOSD患者主要因双眼同时或相继出现视神经炎或纵向长节段横贯性脊髓炎等症状而就诊[18],本研究结果显示,NMOSD组患者血清脂类指标水平较健康对照组明显升高,且67.0%的NMOSD患者主要因视力异常就诊,而同期68.7%的MS患者则主要因脊髓炎等其他症状就诊,这可能是由于两者的发病机制不同所致。NMOSD和MS均属于中枢神经系统的炎性脱髓鞘疾病,中枢神经系统内强烈的炎症反应会引起胶质细胞、神经元和髓鞘损伤,进而影响大脑不同区域、神经和脊髓的功能,表现出视神经炎和脊髓炎等症状[18-19]。本研究MRI检查结果显示,NMOSD患者多表现为视神经受累。既往研究发现,NMOSD对视觉通路的损伤较MS严重,前者通过更广泛的神经变性途径致使疾病进程加快[20-21]。目前NMOSD的诊断多依赖抗体检测,本研究中,AQP4-IgG(+)患者急性期抗体效价主要为1:32,复发期主要为1:32和1:100,但抗体效价与疾病进展无关(P>0.05)。既往观察性研究大多报道抗体滴度无法预测复发[22],但个别研究发现,MOG-IgG(+) NMOSD患者复发期抗体滴度较急性期增高[23],本研究由于该抗体阳性的患者较少,并未观察到类似的结果。
有研究发现,TC、TG和LDL水平与MS患者的残疾程度呈正相关,而HDL水平越高,炎症活动性却越低[24]。也有研究发现,与健康对照组相比,NMOSD组患者仅表现出HDL水平降低和TG水平升高,且并未表现出明显的差异以及与病变受累部位的关联[25],由于该研究并未进行与主要鉴别诊断组——MS组的比较,故在临床鉴别诊断中的提示作用比较有限。本研究结果显示,NMOSD组与MS组TG和HDL水平差异无统计学意义,其余指标差异均有统计学意义,且Apo A1水平在视神经受累时明显升高,可与其他部位受累相区分。中性粒细胞等炎性细胞在NMOSD疾病进展中发挥着重要作用[26-27],Apo A1可能通过抑制中性粒细胞的募集、趋化而抑制炎症反应[28]。NMOSD可呈现明显的脱髓鞘现象。完整髓鞘主要由少突胶质细胞、星形胶质细胞和神经元等形成的胆固醇组成[29]。正常情况下,外周血中的胆固醇和脂蛋白不能跨越血脑屏障,但在外周高胆固醇时,可能会继发血脑屏障损伤[30],大分子AQP4抗体可透过血脑屏障,与星形胶质细胞足突上的AQP4交联,造成以星形胶质细胞为主的一系列损伤[31]
既往研究发现,AQP4抗体及MOG抗体阳性的NMOSD患者与MS患者血脂代谢存在明显差异[32],可能反映了脱髓鞘、中枢神经系统炎症以及血脑屏障损伤等导致的代谢途径改变。本研究发现,NMOSD与MS患者血清Apo A1、Apo B、TC和LDL水平存在明显差异,且这些指标的血清水平受到病变受累部位的影响,其中血清Apo A1可明显区分NMOSD患者的受累部位。作为HDL主要的载脂蛋白,Apo A1是HDL发挥功能的主体[33]。既往研究发现,HDL可能是一种抗炎因子[34],这可能是Apo A1在不同病变部位受累时表达不同的原因。Rhoads等[35]发现,LDL可能在炎症反应中发挥促炎作用。强烈的炎症反应是NMOSD病情进展及复发的主要因素。本研究结果显示,血清LDL水平在疾病进展期(急性期和复发期)高于缓解期,且在不同抗体效价的患者中存在明显差异,推测LDL可能在其中发挥作用,但抗体效价与病情进展的关系尚未明确,故LDL在NMOSD疾病进展中是否发挥促炎作用、炎症进展是否会引起抗体效价的变化仍需通过进一步研究进行验证。
目前脂蛋白在炎症反应中的作用尚未完全明确。Chiurchiu等[36]发现,脂质信号与炎症反应处于动态平衡和相互交织的状态,脂质信号对炎症反应起着积极或消极的调节作用,而先天免疫驱动的炎症反应可影响脂质代谢。NMOSD常由疫苗接种或细菌、病毒等感染诱发,既往研究表明,在细菌或病毒感染时,机体脂质和脂蛋白代谢会发生明显变化[37],由此推测,感染可能是NMOSD患者血脂变化的原因之一。本研究ROC曲线分析显示,联合TC、Apo A1、Apo B、LDL预测NMOSD的特异度较高,而Apo A1在NMOSD患者视神经受累时表达较高,Apo B可预测疾病复发,提示血清脂类指标可作为预测疾病进展和损伤部位的良好指标。
综上所述,联合血清TC、Apo A1、Apo B、LDL能较好地特异性诊断NMOSD,且Apo A1和Apo B可分别作为提示受累部位和疾病进展的指标。但NMOSD炎症改变与脂质代谢改变的因果关系,以及血脂改变在疾病进展中的作用机制仍需进一步深入研究。
