Objective: To analyze the characteristics of serious adverse reactions (ADRs) caused by rituximab in order to provide reference for rational clinical use of rituximab.
Methods: A total of 35 serious ADR reports associated with rituximab were collected in Beijing from 2003 to 2019 and further analyzed.
Results: Among the 35 cases of serious ADRs, there were 18 males and 17 females involved. The average age of the patients was 60±17. Systemic symptoms and respiratory system damage ranked the first, and the occurrence time varied from 30 min to 7 mo after treatment. As a result of serious ADRs, 1 case died, 4 cases aggravated, 12 cases prolonged the course of disease, and 18 cases had no obvious effect on the original disease.
Conclusion: The monitoring over middle-aged and elderly patients and serious ADR patients should be strengthened. If serious ADRs occur, the drug should be stopped in time, and the treatment and rescue should be actively taken so as to ensure the safety of the drug application.

结果： 35例严重不良反应中，男18例，女17例。患者平均年龄（60±17）岁。全身症状及呼吸系统损害并列居首位，发生时间在用药后30 min至7个月不等。不良反应结果导致死亡1例，病情加重4例，病程延长12例，对原患疾病影响不明显18例。
结论： 建议用药时对中老年患者及发生严重不良反应高危患者加强监测，一旦发生严重不良反应及时停药并积极治疗抢救，以保证用药安全。
