Objective:

To mine the security alert signals of risk signals of adverse drug events (ADEs) associated with post-marketing use of avacopan based on the FDA Adverse Event Reporting System (FAERS) database. The findings are intended to provide a reference for clinical safety in medication practices.

Methods:

The study collected ADE reports related to Apixaban from the FAERS database for the period of Q4 2021 to Q2 2024. Potential safety signals associated with Apixaban were identified using various methodologies, including reporting odds ratio (ROR), proportional reporting ratio (PRR), bayesian confidence propagation neural network (BCPNN), and multi-item gamma poisson shrinker (MGPS). Weber distribution test was used to determine the occurrence rule of ADE. The reporting odds ratio was used to estimate the relative risk of arvaracopam ADE in different genders.

Results:

A total of 6 519 ADE reports involving 2 730 patients with avacopan as the primary suspected drug were collected, and 75 avacopan ADE signals were mined involving 15 systems. ADE occurs mostly within 30 days after administration of avacopan. Consistent with the description in the package insert, ADEs of hepatobiliary system, infection and infection system were commonly observed. In addition, ADEs such as epistaxis, pulmonary hemorrhage, pharyngeal swelling and deep vein thrombosis were not included in the package insert. The analysis of gender differences in the risk signals associated with arvalacopam revealed that female patients were more susceptible to conditions such as jaundice, pulmonary vasculitis, esophageal candidiasis, cheilitis, Escherichia coli urinary tract infection, etc. In contrast, male patients were more likely to have dental hypersensitivity, pulmonary hemorrhage, elevated serum ferritin, and cardiac pacemaker implantation.

Conclusion:

In the real-world application of avacopan, it is essential to focus on the ADE related to the hepatobiliary system, respiratory system, thoracic and mediastinal systems, as well as infectious conditions. Additionally, appropriate strategies should be implemented based on gender differences to mitigate the occurrence of ADEs.

目的:

基于美国FDA不良事件报告系统(the FDA Adverse Event Reporting System, FAERS)数据库挖掘阿伐可泮上市后药品不良事件(adverse drug event,ADE)风险信号,为临床安全用药提供参考。

方法:

收集FAERS数据库中2021年第4季度—2024年第2季度关于阿伐可泮ADE报告。采用报告比值比法(reporting odds ratio,ROR)、比例报告比值法(proportional reporting ratio,PRR)、贝叶斯置信区间神经网络传播法(Bayesian confidence propagation neural network,BCPNN)和多重伽马-泊松收缩估计法(multi-item gamma Poisson shrinker,MGPS)识别阿伐可泮相关不良反应的潜在安全信号;使用韦伯分布检验确定ADE发生规律;使用ROR法估算不同性别发生阿伐可泮ADE的相对危险度。

结果:

共收集到阿伐可泮为首要怀疑药物的ADE报告6 519份,涉及患者2 730例,共挖掘出阿伐可泮ADE信号75个,累及15个系统;ADE主要发生在用药后30 d以内;与说明书描述一致,常见肝胆系统、感染及侵染类系统ADE,此外鼻衄、肺出血、咽部肿胀、深静脉血栓等ADE未被说明书收录;阿伐可泮的性别差异风险信号分析,女性更易发生黄疸、肺血管炎、食管念珠菌病、唇炎、埃希菌性尿路感染等;男性更易发生牙齿感觉过敏、肺出血、血清铁蛋白升高、心脏起搏器植入等。

结论:

在真实世界中使用阿伐可泮,应重点关注肝胆系统,呼吸、胸及纵隔系统,感染及侵染类系统的ADE,并基于性别差异性采取相应对策减少ADE的发生。

目的:

基于美国FDA不良事件报告系统(the FDA Adverse Event Reporting System, FAERS)数据库挖掘阿伐可泮上市后药品不良事件(adverse drug event,ADE)风险信号,为临床安全用药提供参考。

方法:

收集FAERS数据库中2021年第4季度—2024年第2季度关于阿伐可泮ADE报告。采用报告比值比法(reporting odds ratio,ROR)、比例报告比值法(proportional reporting ratio,PRR)、贝叶斯置信区间神经网络传播法(Bayesian confidence propagation neural network,BCPNN)和多重伽马-泊松收缩估计法(multi-item gamma Poisson shrinker,MGPS)识别阿伐可泮相关不良反应的潜在安全信号;使用韦伯分布检验确定ADE发生规律;使用ROR法估算不同性别发生阿伐可泮ADE的相对危险度。

结果:

共收集到阿伐可泮为首要怀疑药物的ADE报告6 519份,涉及患者2 730例,共挖掘出阿伐可泮ADE信号75个,累及15个系统;ADE主要发生在用药后30 d以内;与说明书描述一致,常见肝胆系统、感染及侵染类系统ADE,此外鼻衄、肺出血、咽部肿胀、深静脉血栓等ADE未被说明书收录;阿伐可泮的性别差异风险信号分析,女性更易发生黄疸、肺血管炎、食管念珠菌病、唇炎、埃希菌性尿路感染等;男性更易发生牙齿感觉过敏、肺出血、血清铁蛋白升高、心脏起搏器植入等。

结论:

在真实世界中使用阿伐可泮,应重点关注肝胆系统,呼吸、胸及纵隔系统,感染及侵染类系统的ADE,并基于性别差异性采取相应对策减少ADE的发生。

Objective:

To mine the security alert signals of risk signals of adverse drug events (ADEs) associated with post-marketing use of avacopan based on the FDA Adverse Event Reporting System (FAERS) database. The findings are intended to provide a reference for clinical safety in medication practices.

Methods:

The study collected ADE reports related to Apixaban from the FAERS database for the period of Q4 2021 to Q2 2024. Potential safety signals associated with Apixaban were identified using various methodologies, including reporting odds ratio (ROR), proportional reporting ratio (PRR), bayesian confidence propagation neural network (BCPNN), and multi-item gamma poisson shrinker (MGPS). Weber distribution test was used to determine the occurrence rule of ADE. The reporting odds ratio was used to estimate the relative risk of arvaracopam ADE in different genders.

Results:

A total of 6 519 ADE reports involving 2 730 patients with avacopan as the primary suspected drug were collected, and 75 avacopan ADE signals were mined involving 15 systems. ADE occurs mostly within 30 days after administration of avacopan. Consistent with the description in the package insert, ADEs of hepatobiliary system, infection and infection system were commonly observed. In addition, ADEs such as epistaxis, pulmonary hemorrhage, pharyngeal swelling and deep vein thrombosis were not included in the package insert. The analysis of gender differences in the risk signals associated with arvalacopam revealed that female patients were more susceptible to conditions such as jaundice, pulmonary vasculitis, esophageal candidiasis, cheilitis, Escherichia coli urinary tract infection, etc. In contrast, male patients were more likely to have dental hypersensitivity, pulmonary hemorrhage, elevated serum ferritin, and cardiac pacemaker implantation.

Conclusion:

In the real-world application of avacopan, it is essential to focus on the ADE related to the hepatobiliary system, respiratory system, thoracic and mediastinal systems, as well as infectious conditions. Additionally, appropriate strategies should be implemented based on gender differences to mitigate the occurrence of ADEs.

抗中性粒细胞胞质抗体相关血管炎(antineutrophil cytoplas-mic antibody associated vasculitis, ANCA-AAV)是一类可侵犯多个器官系统的自身免疫性疾病,病变累及微动脉、微静脉及毛细血管[1]。其包括显微镜下多血管炎(microscopic polyangiitis,MPA)、肉芽肿性多血管炎(granulomatosis with polyangiitis,GPA)和嗜酸性肉芽肿性多血管炎(eosinophilic granulomatosis with polyangiitis,EGPA)[2]。GPA和MPA年发病率为每百万人中有2~30例,患病率为每百万人中有25~250例;EGPA比GPA,MPA罕见,发病率每百万人中有1~4例,患病率每百万人中有10~25例[3]。抗中性粒细胞胞质抗体相关血管炎发病率低、复发率及死亡率高,是累及多器官(包括肾脏、呼吸系统、胃肠系统和皮肤等)的全身性疾病[4]。目前治疗推荐环磷酰胺或利妥昔单抗联合糖皮质激素进行缓解诱导治疗,硫唑嘌呤或利妥昔单抗进行缓解维持治疗[2],然而长期使用糖皮质激素可能导致显著临床风险。阿伐可泮(avacopan,商品名:Tavneos)是选择性补体C5a受体抑制剂,可显著减少治疗AAV所用糖皮质激素剂量,于2021年10月获美国FDA批准上市,是美国FDA审批通过的第一款补体5a受体拮抗剂。Jayne等[5-6]报道Ⅱ期和Ⅲ期临床试验证实了阿伐可泮减少糖皮质激素或完全取代糖皮质激素之后的疗效。
随着阿伐可泮广泛使用,其不良反应受到越来越多关注,有关阿伐可泮的安全性数据主要来自上市前临床试验,缺乏基于真实世界数据的阿伐可泮药品不良事件(adverse drug event,ADE)系统性研究。美国FDA不良事件报告系统(FDA adverse event reporting system,FAERS)是一个监测上市后药品和治疗性生物制品所有ADE信息和用药错误信息的数据库[7],在一定程度上反映了药物上市后不良反应发生概况,因此本文基于美国FAERS数据库挖掘阿伐可泮ADE信号,评价其在真实世界中可能产生的ADE、发病时间、性别差异及危险因素,为阿伐可泮临床合理应用提供参考。
收集阿伐可泮批准上市后自2021年第4季度—2024年第2季度,共11个季度的原始数据,按照美国FDA推荐方法进行数据的去重与清洗。以“avacopan”“Tavneos”为检索词进行检索,获得以阿伐可泮为首要怀疑目标ADE报告6 519份。对于报告中的ADE名称,采用国际医学用语词典(medical dictionary for regulatory activities,MedDRA)的首选系统器官分类(system organ class,SOC)和首选术语(preferred term,PT)进行规范化描述和统计。
本研究采用比例失衡法中的报告比值比法(reporting odds ratio,ROR)和比例报告比值法(proportional reporting ratio,PRR)、贝叶斯置信区间神经网络传播法(Bayesian confidence propagation neural network,BCPNN)和多重伽马-泊松收缩估计法(multi-item gamma Poisson shrinker,MGPS)同时检测药物的ADE信号,检测方法及标准参考Zou等[9]的研究,具体内容见表1
本研究经数据清洗后得到以阿伐可泮为首要怀疑药物的ADE报告6 519份,共涉及2 730例患者。主要上报国家为美国(2 146例,78.6%)、日本(319例,11.7%);排除未注明性别的报告,其余报告中女性为1 001例(占36.7%),男性为659例(占24.1%);年龄集中于18~85岁(1 447例,53.1%);ADE报告结局为住院的患者558例(占20.4%),死亡的患者164例(6%)。
对收集的2 730例报告进行ADE发生时间分析(去除不准确、缺失或未知的发病时间报告),结果见图1。阿伐可泮ADE发生时间主要集中于给药30 d以内(144例,36.18%),中位时间48 d,30 d之后发生ADE的报告数明显减少,在给药150~180 d内,有轻微提升,此后又有所下降。为进一步确定ADE发生率的变化比例,对ADE发生事件进行韦伯分布检验,进而确定ADE的发生率随时间变化的规律。韦伯分布的尺度参数α控制着分布的宽度或范围:较大的α值会导致分布变得更加分散或宽广,而较小的α值则使分布更加集中或紧凑;韦伯分布的形状参数β影响分布的偏态特征:较大的β值会导致分布呈现左偏曲线,而较小的β值则导致分布呈现右偏曲线[10]。当形状参数β<1且其95%CI<1时,ADE发生率被认为是随着时间的推移而下降的(早期失败型曲线);当形状参数β等于或接近于1且其95%的CI值包括1时,ADE发生率随时间的推移不断发生(随机失败型曲线);当形状参数β>1且其95%的CI值不包括1时,ADE发生率被认为是随着时间的推移而增加的(磨损型故障曲线)[11]。具体结果见表2
同时满足4种分析方法阈值判定标准的阿伐可泮相关ADE阳性信号数有75个,共涉及15个SOC,详细结果见表3。ADE发生频次高的SOC分布主要集中在各类检查,肝胆系统疾病,各种手术及医疗操作,胃肠系统疾病,呼吸系统、胸及纵隔疾病,感染及侵染类疾病。由表2可见,说明书中【警告与注意事项】提及严重不良反应,除发生率较低(<5%)的血管神经水肿未检测到信号,转氨酶升高及相关肝胆事件,严重感染(常见肺炎及尿路感染)均被检测到信号。阿伐可泮信号强度排名前15位的ADE信号,以慢性活动性爱泼斯坦-巴尔病毒感染、抗中性粒细胞胞质抗体增加、胆管消失综合征、肾脏活检、疑似药物性肝损伤等PT为主,除肝脏相关及疾病相关ADE外均未被说明书记载,详见表4
已有的关于阿伐可泮的安全性数据主要来自上市前随机对照临床试验,缺乏真实世界研究,使其在药品不良反应的评判上存在缺陷,尤其在评价不同性别的不良反应发生情况方面存在不足。阿伐可泮信号挖掘结果显示女性(1 001例)ADE报告数明显高于男性(659例),为了分析性别对于阿伐可泮的ADE影响,用【1.2信号挖掘方法及标准】分别筛选出男性、女性的ADE阳性信号,再对相同ADE使用ROR法估算不同性别发生的相对危险度。当a≥3,ROR>1或女性产生ADE而男性没有时,则判定女性患者发生目标药品ADE的风险更高;而当c≥3,ROR<1或男性产生ADE而女性没有时,则判定男性患者更易发生目标药品ADE。具体原理见表5[11]
筛选出阿伐可泮相关ADE阳性信号数男性共46个,女性共47个,其中16个阳性信号为共同发生的,对相同ADE使用森林图进行可视化展示,详细结果见图2。去除男女ADE中有歧义的PT,结果显示女性较男性更易发生黄疸、肺血管炎、食管念珠菌病、埃希菌性尿路感染等,男性较女性更易发生牙齿感觉过敏、肺出血、血清铁蛋白升高、心脏手术等,结果见表6
据阿伐可泮SOC分布挖掘结果显示,产生PT信号较多的SOC分别为各类检查(15个,20%)及肝胆系统疾病(12个,16%),其中各类检查中有7项检查为肝胆异常相关指标,这与药品说明书及临床试验中结果一致,进一步证实肝胆相关ADE为阿伐可泮严重不良反应。然而,临床试验及药品说明书中并没有具体说明有哪些肝胆相关ADE,本研究挖掘到阿伐可泮上市后可能产生的肝胆异常指标及相关肝胆系统ADE,涉及指标包括肝酶升高(44例)、谷丙转氨酶升高(20例)、谷草转氨酶升高(20例)、肝脏功能检查值升高(16例)、肝功检查异常(14例)、血胆红素升高(8例)、血碱性磷酸酶升高(7例),肝胆系统ADE类型包括肝功能异常(56例)、肝脏疾病(50例)、黄疸(35例)、药物诱导的肝损伤(30例)、胆汁淤积(15例)、胆管消失综合征(14例)、高转氨酶血症(8例)、胆管炎(5例)、疑似药物性肝损伤(5例)、肝细胞损伤(3例)、胆囊扩大(3例)、肝纤维化(3例)。其中胆管消失综合征、疑似药物性肝损伤、黄疸、胆囊扩大与阿伐可泮呈现强关联性,查阅目前有关阿伐可泮肝损伤不良反应的个案报道,Yamaguchi等[13]、Shirota等[14]各报道1例使用阿伐可泮导致的胆管消失综合征,2例患者均为女性,年龄分别为75和70岁;Kojima等[15]报道了1例76岁女性患者使用阿伐可泮导致的严重的长期肝异常伴黄疸。以上3例患者均使用熊去氧胆酸治疗相关ADE,症状得到缓解;3例阿伐可泮ADE均被包含在本研究挖掘PT信号中,进一步证实本研究的可靠性。提示临床在使用阿伐可泮时应关注黄疸、胆汁淤积、胆管消失综合征、胆管炎、胆囊扩大、肝纤维化,并在出现肝损伤时,可使用熊去氧胆酸进行治疗。
严重感染是阿伐可泮说明书【注意事项】中强调的一项重要的不良反应,其中以肺炎和尿路感染最常见。本研究同样挖掘到这两类ADE:耶氏肺孢子虫肺炎(14例)、埃希菌性尿路感染(3例);此外挖掘到感染及侵染类疾病有巨细胞病毒感染(9例)、巨细胞病毒病毒血症(4例)、食管念珠菌病(4例)、慢性活动性爱泼斯坦-巴尔病毒感染(3例);其中慢性活动性爱泼斯坦-巴尔病毒感染(EBV)是阿伐可泮相关ADE信号强关联PT。EBV多为隐性感染,且未被清除的EBV可以在记忆B淋巴细胞内建立持续终身的潜伏状态[16],在机体抵抗力差时,可引起多种疾病;食管念珠菌病发生可能与阿伐可泮作用机制有关,阿伐可泮降低了中性粒细胞和巨噬细胞对真菌的摄取和杀伤,从而增加人类感染真菌疾病的风险,尤其在免疫功能下降的患者中;同样巨细胞病毒感染也与个体的免疫功能状态密切相关,免疫功能正常的人群中,多以隐性感染为主,可自限,部分潜伏在体内,在免疫功能下降的患者中,可重新被激活[17-18]。因此提示临床在使用阿伐可泮期间,针对此类高危人群,充分考虑风险与益处,并及时进行适当的抗感染治疗。
本研究挖掘到鼻衄、肺出血、咽部肿胀、咯血、肺血管炎、换气过度等多个呼吸、胸及纵隔系统相关的ADE风险信号,6项ADE均未被说明书收录;其中肺出血与阿伐可泮具有强关联性。目前其作用机制尚不明确,且无相关个案报道,推测可能与其原发疾病有一定关联,因为抗中性粒细胞胞质抗体相关血管炎可累及呼吸道;但在阿伐可泮相关ADE信号强度中,肺出血与阿伐可泮呈现强关联性,提示临床医师保持警惕,当出现呼吸、胸及纵隔系统相关的ADE,仍要评估是否与阿伐可泮相关,以采取进一步治疗措施。此外挖掘到一项严重ADE:深静脉血栓形成(95%CI:2.25~6.69;PRR=3.88, χ2=27.72;EBGM05=2.45;IC025=0.29)。Alberici等[19]追踪了一项59例患者使用阿伐可泮52周以上用药经验,其中1例患者同样发生了深静脉血栓,分析其原因可能与剂量改变及药物联用有关,然而引起此ADE的作用机制尚不明确,仍需要进一步的前瞻性研究,同时因该项不良反应严重,临床医师在实际用药过程中,仍需要警惕与预防。
对阿伐可泮ADE诱发时间分布情况进行分析,结果显示阿伐可泮ADE发生时间集中在用药90 d以内,占比高达71.35%(284例),其中30 d以内发生ADE比例为36.18%(144例),之后发生ADE的报告数明显减少,在给药150~180 d内,有轻微提升,之后有所下降,误差可能与数据来源有一定关联,但整体趋势明显。为进一步确定阿伐可泮ADE发生率的变化比例,使用韦伯分布检验进行验证。在开始治疗后,ADE的发生率取决于药物的作用机制,而且往往随时间变化而变化,同时与药物治疗无关的ADE则以恒定的速度发生。阿伐可泮ADE诱发时间韦伯分布检验,形状参数为0.77(β<1),其95%CI(0.71~0.83)<1,发生率被认为是随着时间的推移而下降的,符合早期失败型的分布特征;与FAERS系统中上报的阿伐可泮ADE诱发时间分布情况一致,提示阿伐可泮ADE发生时间多见于用药早期,临床使用时应重点关注。
同时满足4种分析方法阈值判定标准的阿伐可泮相关ADE男性阳性信号数为46个,女性为47个,其中16个阳性信号为共同发生的。去除有歧义的PT,其中男性产生阳性信号而女性未产生信号的有24个,具有强关联性的为牙齿感觉过敏(95%CI:86.48~399.89;PRR=185.15, χ2= 1 205.38; EBGM05=91.76;IC025=5.76)、肺出血(95%CI:10.85~63.28;PRR=26.12,χ2=119.72;EBGM05=12.38;IC025=3.02)、血清铁蛋白升高(95%CI:7.1~69.01;PRR=22.1,χ2=59.98; EBGM05=8.47;IC025=2.78)、心脏起搏器植入(95%CI:6.13~59.48;PRR=19.06,χ2=51; EBGM05=7.32;IC025=2.57);女性产生阳性信号而男性未产生信号的有14个,具有强关联性的为肺血管炎(95%CI:163.78~1 931.45;PRR=561.77, χ2=1 414.16;EBGM05=168.55;IC025=7.1)、食管念珠菌病(95%CI:8.32~80.85;PRR=25.9,χ2=71.21;EBGM05=9.92;IC025=3.01)、唇炎(95%CI:5.66~54.82;PRR=17.59,χ2=46.67;EBGM05=6.76;IC025=2.46)、埃希菌性尿路感染(95%CI:5.13~49.65;PRR=15.94,χ2=41.78;EBGM05=6.13;IC025=2.32)。在16个共同阳性信号中,女性发生黄疸(ROR=3.75,95%CI:1.3~10.83)的危险度更高。提示临床医师应基于性别差异性更好地制定个体化治疗方案,以采取相应对策减少ADE的发生[20]
本研究存在以下几点局限性:①阿伐可泮ADE上报国家主要为美国,亚洲人群少,使用时可能因种族差异而导致ADE不同。②上报人群主要为消费者,报告质量参差不齐,可能存在上报信息不完善、漏报、错报,导致结果存在偏倚。③因药物使用人群基数不能确定,因此本研究结果只证明ADE发生与药物使用强关联性,不能确定其发生率。但总体而言,基于FAERS数据库对阿伐可泮ADE进行挖掘与分析,确定了阿伐可泮ADE诱发时间分布,发现了一些说明书未提及的风险信号及阿伐可泮的性别差异风险信号,为临床安全用药提供参考,仍具有重要的研究意义。
