Objective:

To establish the 14th batch of national reference standards for histamine phosphate.

Methods:

Histamine phosphate was identified using infrared spectroscopy and bioactivity assay. Its hygroscopicity, moisture content, and uniformity were also examined. Using the 13th batch of national reference standard for histamine phosphate as the standard, 4 laboratories were selected to conduct collaborative calibration of the potency of the reference standard using the cat blood pressure method.

Results:

The final combined potency of this batch of standard products was determined to be 97.8%.

Conclusion:

This batch of reference materials can be used as the 14th batch of national reference materials for histamine phosphate, with a relative potency set at 100%, batch numbers 150510-202214.

目的:

建立第14批磷酸组胺国家对照品。

方法:

采用红外光谱和生物活性测定2种方法对磷酸组胺进行鉴别,并检查其引湿性、水分和均匀性,以第13批磷酸组胺国家对照品为标准,选择4个实验室,采用猫血压法对待标品效价进行协作标定。

结果:

该批待标品最终合并计算相对效价为97.8%。

结论:

本批待标品可作为第14批磷酸组胺国家对照品使用,相对效价定为100%,批号:150510-202214。

Objective:

To establish the 14th batch of national reference standards for histamine phosphate.

Methods:

Histamine phosphate was identified using infrared spectroscopy and bioactivity assay. Its hygroscopicity, moisture content, and uniformity were also examined. Using the 13th batch of national reference standard for histamine phosphate as the standard, 4 laboratories were selected to conduct collaborative calibration of the potency of the reference standard using the cat blood pressure method.

Results:

The final combined potency of this batch of standard products was determined to be 97.8%.

Conclusion:

This batch of reference materials can be used as the 14th batch of national reference materials for histamine phosphate, with a relative potency set at 100%, batch numbers 150510-202214.

组胺是一种具有广泛生理作用的生物胺,进入体内后作用于内皮和平滑肌细胞的H1受体,可产生血管舒张、血压下降,支气管和胃肠道平滑肌收缩等生理反应[1]。磷酸组胺(histamine phosphate)是组胺的磷酸盐,即1H-咪唑-4-乙胺磷酸盐,又名二磷酸组胺、2-(4-咪唑)乙胺二磷酸盐,分子式为C5H9N3·2H3PO4,是《中华人民共和国药典》2020年版四部通则1145降压物质检查法(猫血压法)和1146组胺类物质检查法(离体豚鼠回肠法)所必需的标准物质[2-3],结构式详见图1。磷酸组胺可直接作用于血管,扩张H1和H2受体介导的动脉和毛细血管。我国自1959年开始标定第1批磷酸组胺国家对照品,至2013年制备第13批[4],目前第13批磷酸组胺国家对照品即将售罄,因此研制第14批磷酸组胺国家对照品。
本批磷酸组胺(二磷酸组胺)原料药购于上海蓝木化工有限公司,产品编号:S4117,批号:S411702,白色粉末,纯度:99.73%,色谱级,共计250 g。乙腈和三氟乙酸(TFA)为色谱纯,分别购自Fisher公司和Sigma公司。
标准物质:第13批磷酸组胺国家对照品(二磷酸组胺一水合物),批号:150510-201313。
LC—20A型高效液相色谱(Shimadzu公司);V305型水分测定仪(Mettler公司);MP150多导生理记录仪(Biopac公司);Nexus傅里叶变换红外光谱仪(Thermo公司);Advantage动态蒸气吸附仪(SMS公司)。
第14批磷酸组胺国家对照品由中国食品药品检定研究院标准物质与标准化管理中心标准物质制备室分装,分装条件:20 ℃以下,30% RH以下,避光。最小包装单元容器为棕色安瓿,约20 mg·支-1,分装数量约11 000支,批号:150510-202214。
外观:白色结晶性粉末。
采用溴化钾压片法制样,样品扫描次数15次;背景扫描次数15次;分辨率为4.000;Sample Gain: 1.0;Mirror Velocity: 0.474 7;Aperture: 80.0。本品的红外光吸收图谱与磷酸组胺对照品(批号:150510-201313)一致,见图2
参考《中华人民共和国药典》2020年版四部通则1145降压物质检查法[2],本品具有降低猫血压生物活性,具体方法参见“4”项效价测定。
采用SMS-动态吸附蒸汽仪,待标品在25 ℃、20%~80%~0%RH条件下放置16 h,其动态吸附曲线见图3,参照《中华人民共和国药典》2020年版四部指导原则9103药物引湿性实验指导原则规定[5],引湿增重<0.2%,为几乎无引湿性。
采用库仑滴定法对样品进行水分测定,磷酸组胺原料药水分测定结果为5.73%,分装后的磷酸组胺待标品水分测定结果为5.79%。
取12支待标品,采用HPLC法进行均匀性检查,每支待标品独立平行配制2份相同浓度溶液,溶剂为流动相B。流动相A为0.1% TFA乙腈,流动相B为0.1% TFA水,采用等度洗脱A∶B=9∶1;C18色谱柱(100 mm×4.6 mm,3.5 μm);进样体积2 μL;柱温25 ℃;流速1.0 mL·min-1;检测波长214 nm;PDA检测器。色谱图详见图4图5,测定结果以主峰峰面积为指标。采用GraphPad Prism 9.5.1软件进行统计,比较采用单因素方差分析,P<0.05为差异有统计学意义。结果F<FαP=0.871 2,表明本批磷酸组胺对照品均匀性符合要求,结果详见图6
中国食品药品检定研究院邀请上海市食品药品检验研究院、河南省药品医疗器械检验院、浙江省食品药品检验研究院和北京市药品检验研究院对第14批磷酸组胺国家对照品进行协作标定。
第13批磷酸组胺对照品(批号:150510-201313),装量每支约20 mg,效价设定为100%。待标品批号:150510-202214,装量每支约20 mg。
参考《中华人民共和国药典》2020年版四部通则1145降压物质检查法,采用猫血压法进行测定。将动物麻醉后固定于手术台上,分离气管,剥离一侧颈动脉,插入另一端连接多导生理记录仪的动脉插管并注入适量抗凝剂,然后剥离一侧股静脉,插入静脉插管供注射药液用,药液注射速度和间隔时间应相同,每次给药后立即注入适量生理盐水[6]。实验设计为2×2法,对照品和待标品分别设高、低2个剂量(即SH,SL,TH,TL),剂量比值为1∶0.6。采用SL和SH进行灵敏度检测,重复2~3次。灵敏度符合要求后,可以进行正式实验。对照品和待标品高、低4个剂量为一组,按照拉丁方排列顺序给药,即abcd, bcda, cdab, dabc…(a:SH,b:SL;c:TH,d:TL),在同一只猫上分别给药4~6组,记录每次注射药液的反应值和血压曲线,典型血压曲线图见图7
要求每次实验成立,且可信限率(fiducial limit, FL)不得大于10%。以第13批磷酸组织胺国家对照品(编号:150510-201313)作标准,标定待标品(原料药分装的4个时间段各取2支,即d 1上午、d 2上午、d 2下午、d 3上午)。每支待标品提供1个独立标定结果,每个实验室需提供6~8个结果。
所有试验数据均以量反应平行线2×2法[7],采用BS 2000软件按随机区组设计进行统计计算生物效价,合并计算结果详见表1
以上实验数据经BS 2000软件合并计算:结果均一,测得相对效价为PT=97.8%,可信限率为FL=0.973 77%,测得效价的可信限范围为96.8%~98.7%。本批次磷酸组胺国家对照品测定的相对效价为97.8%,与100%仅相差2.2%,远小于测定方法猫血压法的可信限率10%,因此认为本批对照品相对效价可定为100%,即与第13批磷酸组胺国家对照品效价一致。
各协作标定单位结果散点分布如图8所示,结果频数分布如图9所示。
本批待标品可作为第14批磷酸组胺国家对照品使用,批号:150510-202214。
由动物组织或体液提取,或微生物发酵获得的多组分生化药物在制备过程中可能混入组胺或类组胺等物质,因为中药成分复杂,也可能含有多种降低血压的已知和未知杂质,引起血压下降、变态反应甚至休克等不良反应,影响临床用药安全性[8]。《中华人民共和国药典》2020年版四部9301“注射剂安全性检查法应用指导原则”规定,对于该类药物注射剂,应考虑设置降压物质或组胺类物质检查项[9]。第14批磷酸组胺国家对照品可用于《中华人民共和国药典》2020年版四部通则1145降压物质检查法及1146组胺类物质检查法。
《中华人民共和国药典》自1977年开始收载降压物质检查法,一直沿用至今。降压物质检查法系比较组胺对照品与供试品引起麻醉动物血压下降的程度,以决定供试品中降压物质的含量限度是否符合规定的一种方法[10],因此,该方法所用的磷酸组胺对照品赋值的准确性十分重要,直接影响着多种药品的临床用药安全。
《中华人民共和国药典》2005年版以前,采用的实验动物为猫或犬,从2005版开始,删除了犬,规定猫为唯一实验动物品种。猫循环系统发达,能在麻醉状态下保持血压稳定[6],相同实验条件下,猫对血压降低的敏感性高于比格犬和兔,对组胺降压作用有较好的重现性[11, 8]。但猫作为实验动物购买较困难,价格昂贵,实验成本高,且目前我国没有规范化的繁殖、饲养和质量检测标准,许多实验室用猫为家养杂种猫,购买自农场和饲养场,其遗传、年龄、微生物和寄生虫等携带状况不清,个体差异不可控,健康状况无法保证,对实验结果的准确性及实验人员的健康安全产生严重威胁和影响[12-13]
《中华人民共和国药典》2010年版在第二部附录《化学药物注射剂安全性检查法应用指导原则》增加了组胺类物质检查项;《中华人民共和国药典》2015年版首次收载了组胺类物质检查法(即豚鼠离体回肠收缩法),作为猫降压物质检查法的补充和替代方法。豚鼠作为标准化饲养的常规实验动物,易于获取,且实验操作更易掌握,实验结果更能得到保障。目前,已有多家实验室针对某些品种展开降压物质检查的研究,通过对比2种方法实验结果的一致性,为组胺类物质检查法替代降压物质检查法提供背景数据[14-22]。然而,组胺类物质检查法也有一定的局限性,供试品组胺限值有限制,若药物临床使用剂量较大,供试品试验浓度过高时,可能影响豚鼠回肠的灵敏度[19]。此外,豚鼠离体回肠的体外试验可能不能完全模拟哺乳动物体内的代谢条件,引起哺乳动物血压降低的原因除组胺外,还有许多其他物质如预存的介质激肽原酶,新合成的介质白三烯、血小板活化因子、前列腺素D2等[14]。因此,组胺类物质检查法代替降压物质检查法还需要加大基础研究工作,获得更多的研究报告和数据支撑。对于成分复杂的多组分药物的降压物质检查,应根据其主药成分的性质和临床适应证谨慎选择适合的方法。
磷酸组胺国家对照品第13批次为二磷酸组胺一水合物,第12批次为二磷酸组胺(与本批次一致),上述2批次的长期稳定性考察结果均表明:本品种对照品在现有储存温度2~8 ℃条件下性质稳定,可达十余年。此外,本品种现有的运输条件亦不会对本对照品性质产生影响。因此,本批次磷酸组胺对照品的稳定性评估在依据可靠的质量检测历史数据资料及实际经验的基础上,未进行实验考察。
第13批磷酸组胺对照品(批号:150510-201313)为二磷酸组胺一水合物,因此,在实验称量后换算成组胺含量时,需乘以系数0.342;而本批次磷酸组胺待标品为二磷酸组胺,因此,在实验称量后换算成组胺含量时,需乘以系数0.362(致谢:上海市食品药品检验研究院、河南省药品医疗器械检验院、浙江省食品药品检验研究院及北京市药品检验研究院)。
