Objective:

To investigate the intestinal absorption characteristics of harmine derivative H-2-168 using Caco-2 cells model and in situ single-pass intestinal perfusion method.

Methods:

The Caco-2 cell monolayer model and in situ single pass intestinal perfusion model of rat were established. The concentrations of H-2-168 in cell permeation fluid and intestinal perfusion fluid were determined by HPLC. The changes of apparent permeability coefficient (Papp) of H-2-168 in Caco-2 cell model and those of absorption rate constant (Ka) and Papp in in situ single pass intestinal perfusion model of rat under different influence factors (intestinal segment, concentration, pH and P-glycoprotein inhibitor) were investigated.

Results:

In Caco-2 cell model, Papp values at medium and high concentrations of H-2-168 were significantly higher than that at low concentration (P<0.05), but Papp at high concentration was slightly lower than that at medium concentration, showing a high concentration saturation phenomenon. H-2-168 was absorbed in the whole intestine of rats, with Ka and Papp decreased significantly with the increase of drug concentration (P<0.05). Ka and Papp at pH 7.4 were significantly higher than those at pH 5.4. There were no significant changes in the absorption parameters before and after the addition of P-glycoprotein inhibitor (P>0.05).

Conclusion:

H-2-168 is a well-absorbed drug, and its absorption mechanism may involve active transport or facilitated diffusion, and it is not a substrate of P-glycoprotein.

目的:

结合Caco-2细胞模型和在体单向肠灌流法考察去氢骆驼蓬碱衍生物H-2-168的肠吸收特性。

方法:

建立Caco-2细胞单层模型及大鼠在体单向肠灌流模型,采用高效液相色谱法测定细胞渗透液及肠灌流液中H-2-168的浓度,考察H-2-168在Caco-2细胞模型不同影响因素下表观渗透系数(Papp)的变化及在大鼠在体单向肠灌流模型不同影响因素(肠段、浓度、pH及P-糖蛋白抑制剂)下的吸收速率常数(Ka)和Papp的变化。

结果:

在Caco-2细胞模型中,中、高浓度H-2-168的Papp均显著高于低浓度(P<0.05),但高浓度Papp略低于中浓度,呈高浓度饱和现象;H-2-168在大鼠全肠段均有吸收,随药物浓度增加KaPapp呈明显下降趋势(P<0.05);pH 7.4时KaPapp明显高于pH 5.4时;加入P-糖蛋白抑制剂前后,吸收参数无显著变化(P>0.05)。

结论:

H-2-168为吸收良好的药物,其吸收机制可能涉及主动转运或促进扩散,不是P-糖蛋白的底物。

目的:

结合Caco-2细胞模型和在体单向肠灌流法考察去氢骆驼蓬碱衍生物H-2-168的肠吸收特性。

方法:

建立Caco-2细胞单层模型及大鼠在体单向肠灌流模型,采用高效液相色谱法测定细胞渗透液及肠灌流液中H-2-168的浓度,考察H-2-168在Caco-2细胞模型不同影响因素下表观渗透系数(Papp)的变化及在大鼠在体单向肠灌流模型不同影响因素(肠段、浓度、pH及P-糖蛋白抑制剂)下的吸收速率常数(Ka)和Papp的变化。

结果:

在Caco-2细胞模型中,中、高浓度H-2-168的Papp均显著高于低浓度(P<0.05),但高浓度Papp略低于中浓度,呈高浓度饱和现象;H-2-168在大鼠全肠段均有吸收,随药物浓度增加KaPapp呈明显下降趋势(P<0.05);pH 7.4时KaPapp明显高于pH 5.4时;加入P-糖蛋白抑制剂前后,吸收参数无显著变化(P>0.05)。

结论:

H-2-168为吸收良好的药物,其吸收机制可能涉及主动转运或促进扩散,不是P-糖蛋白的底物。

Objective:

To investigate the intestinal absorption characteristics of harmine derivative H-2-168 using Caco-2 cells model and in situ single-pass intestinal perfusion method.

Methods:

The Caco-2 cell monolayer model and in situ single pass intestinal perfusion model of rat were established. The concentrations of H-2-168 in cell permeation fluid and intestinal perfusion fluid were determined by HPLC. The changes of apparent permeability coefficient (Papp) of H-2-168 in Caco-2 cell model and those of absorption rate constant (Ka) and Papp in in situ single pass intestinal perfusion model of rat under different influence factors (intestinal segment, concentration, pH and P-glycoprotein inhibitor) were investigated.

Results:

In Caco-2 cell model, Papp values at medium and high concentrations of H-2-168 were significantly higher than that at low concentration (P<0.05), but Papp at high concentration was slightly lower than that at medium concentration, showing a high concentration saturation phenomenon. H-2-168 was absorbed in the whole intestine of rats, with Ka and Papp decreased significantly with the increase of drug concentration (P<0.05). Ka and Papp at pH 7.4 were significantly higher than those at pH 5.4. There were no significant changes in the absorption parameters before and after the addition of P-glycoprotein inhibitor (P>0.05).

Conclusion:

H-2-168 is a well-absorbed drug, and its absorption mechanism may involve active transport or facilitated diffusion, and it is not a substrate of P-glycoprotein.

去氢骆驼蓬碱(harmin,HM)为蒺藜科骆驼蓬属植物骆驼蓬(Peganum harmala L.)中主要的生物碱[1],具有抗肿瘤、改善阿尔茨海默病、杀虫、抗炎等多种药理作用[2]。新疆为包虫病高发区,因此课题组多年来聚焦于抗包虫病新药研究。据文献报道,HM具有良好的抗包虫作用且前期课题组研究也已证实这一结论[3],然而其明显的神经毒性限制了临床转化应用[4]。因此,为减毒增效,课题组以HM母核为先导化合物对其进行结构修饰获得多种衍生物,前期通过体内外药效及安全性筛选,从众多衍生物中筛选出了高效低毒的候选化合物-衍生物H-2-168,分子式为C18N3H15[5]
药物吸收研究是新药开发和评价的首要环节,直接影响药物生物利用度及口服制剂的开发[6],因此有必要对H-2-168的吸收特性进行考察。在体单向肠灌流法因保留了肠道神经及内分泌系统的完整性和血液及淋巴液的供应,成为体内法广泛应用的模型[7-8]。体外法目前常采用细胞模型法,其中Caco-2细胞因具有与小肠上皮细胞相同的细胞特性且可以表达各种蛋白载体和酶,成为目前最成熟且应用最多的体外吸收模型[9-10]。因此,本文采用Caco-2细胞模型和大鼠在体单向肠灌流模型综合评价H-2-168的肠吸收特性,为其进一步研究开发提供生物药剂学依据。

H-2-168(新疆华世丹药业股份有限公司提供,批号:180815,纯度≥98%);替硝唑(tinidazole,TNZ)对照品(中国食品药品检定研究院,批号:100336-201704,纯度≥98%);MEM/EBSS培养基(批号:AF29584106)、胰蛋白酶(批号:J190002)、非必需氨基酸(批号:AF29585030)、青链霉素(批号:J190007)、胎牛血清(批号:RB35949)、D-hank's液(批号:20150504)均购自美国Hyclone公司;噻唑蓝(MTT,批号:H8130,北京索莱宝科技有限公司);二甲基亚砜(dimethyl sulfoxide,DMSO,批号:D2650,美国Sigma公司);盐酸维拉帕米(verapamil,Ver)对照品(批号:823A024,纯度≥98%)和环孢素A(cyclosporine A,CsA)对照品(批号:045K403,纯度≥98%)均购自中国药品生物制品检定所;氯化钠(批号:20120926)、氯化钾(批号:20191212)、氯化钙(批号:20220721)、磷酸二氢钠(批号:20230110)、碳酸氢钠(批号:20240426)、氯化镁(批号:20190618)均购自国药集团化学试剂有限公司;葡萄糖(批号:20200414,天津市鑫铂特化工有限公司);甲醇、乙腈为色谱纯;水为超纯水。
人结肠腺癌Caco-2细胞株购自武汉普诺赛生命科技有限公司(Procell Life Science & Technology Co,. Ltd),批号:CL-0050。
SPF级SD大鼠,54只,雌雄各半,体质量(200±20) g,购自湖南斯莱克景达实验动物有限公司提供,合格证号:SCXK(湘)2020-0006。本动物实验经新疆医科大学第一附属医院动物实验医学伦理委员会批准,批准编号:20220309-175。
精密称取H-2-168对照品10.00 mg,置10 mL棕色量瓶中,加甲醇适量溶解,定容至刻度,摇匀配制成1.00 mg·mL-1的储备液,4 ℃冷藏备用。
精密称取TNZ对照品20.40 mg,置于10 mL棕色量瓶中,用甲醇溶解并定容,摇匀后,得质量浓度为2.04 mg·mL-1的TNZ储备液,4 ℃冷藏备用。
色谱柱为Inertsil ODS-SP C18柱(250 mm×4.6 mm,5 μm);流动相为甲醇-0.1%甲酸水(30∶70);进样量10 μL;流速0.8 mL·min-1;柱温35 ℃;检测波长302 nm。
分别精密移取H-2-168细胞渗透液或肠灌流液100 μL,加入TNZ内标溶液20 μL,加入80 μL乙腈,涡旋,于12 000 r·min-1离心10 min(4 ℃),取上清液进样分析。
Caco-2细胞接种于25 cm2的细胞培养瓶中,加入5 mL的MEM/EBSS培养基,含10%胎牛血清、1%非必需氨基酸、1%青链霉素,于CO2恒温培养箱中培养,隔3 d换1次液。细胞以1×105个·mL-1接种在Transwell板中,肠腔侧(apical,AP)加0.5 mL细胞悬液,基底侧(basolateral,BL)加入1.0 mL培养液。将Transwell板放在37 ℃、5% CO2培养箱中,每2 d更换培养基1次,每3 d采用细胞电阻仪测定单层细胞跨上皮细胞电阻(TEER),TEER>500 Ω·cm2的膜用于转运实验[12]
Caco-2细胞以1×105个·mL-1的密度接种于96孔板,每孔体积为200 μL,培养3 d后更换培养基。细胞分为空白组、阴性对照组和实验组。空白组不加细胞仅加培养基;阴性对照组加入细胞、培养基和DMSO(终体积分数1%);实验组加入不同浓度梯度的H-2-168药液,使终浓度分别为7.81,15.63,31.25,62.50,125.00,250.00,500.00 μmol·L-1,每浓度设置5个平行复孔。孔板置于培养箱中孵育4 h,避光每孔加入20 μL的5 mg·mL-1 MTT试液,再孵育4 h,除去细胞上清液。每孔加入150 μL DMSO,振荡摇匀15 min,用酶标仪在490 nm波长处测定每孔吸光度(A)。计算细胞存活率,存活率>90%药物浓度用于下一步实验。细胞存活率=(A实验组-A空白组)/(A对照组-A空白组)×100%。
取符合条件的细胞单层,D-Hank's液荡洗3次后置于培养箱中孵育30 min,吸弃废液。用D-hank's液配制低、中、高浓度(62.50,125.00,250.00 μmol·L-1)的药液。分别取0.5 mL加入细胞AP侧,细胞BL侧加入D-hank's液1.0 mL,置于培养箱中孵育,分别于15,30,45,60,90,120 min从BL侧取样,同时立即向BL侧补加D-hank's液100 μL;测定药物浓度,计算药物累计转运量Q(μg),见公式1。绘制Q-t曲线,计算各给药浓度下药物Papp,见公式2。式中Ci为第i个时间点测得的药物浓度(μg·mL-1),dQ/dt为单位时间药物转运量(μg·sec-1),A为Transwell板接种细胞的表面积(1.12 cm2),C0为AP侧中加入药物的初始浓度(μg·mL-1)。
AP-BL转运实验:同“8.3”项下;BL-AP转运实验:BL侧加入34.10 μg·mL-1 H-2-168药液1.0 mL,AP侧加入D-hank's液0.5 mL,与上述同样时间点从AP侧取样100 μL。处理后测定,计算外排比(efflux ratio,ER),见公式3。其中Papp (BL-AP)Papp (AP-BL)分别为药物从BL侧→AP侧、AP侧→BL侧转运的表观渗透系数。
取符合条件的细胞单层,AP侧加入100 μmol·L-1 Ver或20 μmol·L-1 CsA置于培养箱中孵育30 min后,小心吸去P-gp抑制剂,然后用D-hank's液荡洗细胞3次,AP侧加入34.10 μg·mL-1的药液,BL侧加入1.0 mL D-hank's液,不同时间取样后测定,分别计算Ver和CsA作用后H-2-168的Pinhibitor (A-B),见公式4。
Krebs-Ringer's(K-R)溶液:称取NaCl 7.8 g、KCl 0.35 g、CaCl2 0.37 g、NaH2PO4 0.32 g、NaHCO3 1.37 g、MgCl2 0.02 g、葡萄糖1.4 g,用超纯水溶解。其中需将CaCl2单独充分溶解后加入,避免产生沉淀。溶解后用定容至1 L,用稀盐酸调节pH至7.4,即得。
空白肠灌流液:用37 ℃生理盐水将肠内容物冲洗干净后,用不含药物的K-R液迅速充盈肠段,以0.2 mL·min-1的速度连续泵入肠段,在出口处收集流出液,作为空白肠灌流液。
H-2-168灌流液的制备:称取H-2-168适量,先加入0.5 mL吐温80(10%)溶解,后用K-R液稀释定容至100 mL,配制成浓度分别为20,40,80 μg·mL-1的肠灌流液。
取禁食过夜的大鼠(自由饮水),采用异氟烷麻醉。按照王丽峰等[13]方法分离出十二指肠、回肠、盲肠、结肠。用预热至37 ℃的生理盐水将肠内容物冲洗干净,K-R溶液灌流15 min,再用40 μg·mL-1的含药灌流液(预热37 ℃)先以0.5 mL·min-1的流速灌流10 min,再将流速调为约0.2 mL·min-1,平衡30 min后,每隔15 min迅速更换1次已知质量的供试液小瓶和收集液小瓶,直至120 min,称质量,计算灌入和收集的供试液质量。实验结束后,剪下被考察肠段,测量其长度(L)和横截面半径(r)。吸取灌流液,按“7”项下方法处理后进样测定。按公式(5)和(6)计算药物KaPapp
式中VinVout分别为灌入和收集的供试液体积;CinCout分别为进口和出口处灌流液中的药物质量浓度;Lr分别为被考察肠段长度和半径;t为灌流时间;Qin为灌流速度。
选择十二指肠作为目标肠段,分别采用低(20 μg·mL-1)、中(40 μg·mL-1)、高(80 μg·mL-1)浓度的肠灌流液对十二指肠进行灌流,考察不同浓度对H-2-168吸收参数的影响。
采用40 μg·mL-1浓度H-2-168肠灌流液对十二指肠进行灌流,每组6只大鼠。考察不同pH值(5.4,6.4,7.4)对药物吸收的影响。
采用含有P-gp抑制剂(Ver/CsA)和不含P-gp抑制剂的中浓度H-2-168肠灌流液对大鼠十二指肠进行灌流,比较P-gp抑制剂加入前后H-2-168的吸收情况。
采用SPSS 21.0统计软件分析,实验结果以表示,多组均值比较采用方差分析(analysis of variance,ANOVA)法,组间两两比较采用双侧Student t-test,以P<0.05表示有显著性差异。
连续培养20 d时于倒置显微镜下观察,可见细胞排列紧密、细胞间无间隙,形成了良好的紧密连接,见图1A。观察培养电阻值变化,随着培养天数的增加,TEER值不断增加。当培养至d 20,TEER值>600 Ω·cm2,见图1B,表明形成了紧密连接的单层膜,可用于后续转运实验。
实验结果见图2。在浓度范围7.81~500.00 μmol·L-1范围内时,细胞存活率均在90%~110%范围内,表明H-2-168浓度在此范围内不会对细胞产生明显抑制或促进作用,因此可用该浓度范围进行后续转运实验。
分别给予低(62.5 μmol·L-1)、中(125 μmol·L-1)、高(250 μmol·L-1)浓度的H-2-168细胞渗透液,考察其不同取样点的累计透过量(Q)及不同浓度下的Papp,结果见图3图4。结果显示,随着时间增加,药物Q逐渐增加,但Q增加幅度降低。低浓度Papp明显低于中、高浓度(P<0.05),但高浓度时Papp出现下降,推测原因可能为吸收饱和现象。提示H-2-168吸收可能有主动转运或促进扩散等转运机制参与。
H-2-168在BL-AP方向的Papp为(4.51±0.18)×10-5 cm·s-1,AP-BL方向的Papp(3.51±0.29)×10-5 cm·s-1,ER=1.13。ER<2,表明在所考察浓度范围内H-2-168转运不具有外排现象,推测其可能不是P-gp底物[14]
表1结果可看出,加入Ver和CsA后,H-2-168 Papp均减小,但与不加抑制剂无显著性差异(P>0.05),因此推测H-2-168可能不是P-gp的底物。
用40 μg·mL-1肠灌流液灌流2 h后各肠段的PappKa值结果见图5。结果显示,H-2-168在各个肠段均有吸收,且十二指肠段的KaPapp显著高于空肠、回肠和结肠(P<0.05),提示十二指肠可能为H-2-168的主要吸收部位。
不同浓度的H-2-168肠灌流液灌流十二指肠2 h后结果见表2。结果显示,低浓度吸收参数显著高于中、高浓度(P<0.05),表明药物浓度影响H-2-168在十二指肠的吸收,吸收具有高浓度饱和现象,提示H-2-168的吸收过程可能涉及主动转运或促进扩散。
采用不同pH的H-2-168肠灌流液在对十二指肠进行灌流2h后结果见图6。结果显示,当灌流液pH值升高时,H-2-168在十二指肠的KaPapp值均显著增高(P<0.05),提示H-2-168在偏碱性条件下吸收较好。
P-gp抑制剂加入前后对H-2-168吸收参数的影响结果见图7。含抑制剂的H-2-168组与不含抑制剂组比较,差异无统计学(P>0.05),表明H-2-168可能不是P-gp的底物。
药物的吸收特性是决定药物生物利用度及影响药物疗效的关键,对药物在体内的吸收的研究有助于对药物进行结构修饰、选择合适的剂型及处方等[15]。本研究建立Caco-2单层细胞模型和大鼠在体单向肠灌流模型,结合体外和在体法综合评价H-2-168的肠吸收特性。
大鼠在体单向肠灌流结果显示,H-2-168在小肠全肠段均有吸收且十二指肠的吸收速率常数明显大于其他肠段,这可能与十二指肠具有丰富的毛细血管网利于药物吸收有关。研究认为,当药物的Papp<1×10-6 cm·s-1表明化合物吸收差,而当Papp>10×10-6 cm·s-1则表明化合物吸收良好[16]。在本实验2种模型中,H-2-168的Papp均>10×10-6 cm·s-1,表明H-2-168属于吸收良好的药物。前期研究显示盐酸H-2-168溶解度差,约为2.5 μg·mL-1,根据生物药剂学分类系统,属于初步判定盐酸H-2-168为低溶解度/高渗透性药物。
大鼠在体单向肠灌流结果显示,随着药物浓度的增加,KaPapp不断递减,呈现高浓度抑制现象,Caco-2细胞模型实验中高浓度Papp低于中、低浓度,也证实了H-2-168的吸收具有高浓度饱和现象,因此推测H-2-168的吸收机制可能为主动吸收或促进扩散。大鼠在体单向肠灌流实验还显示,随pH值的增大吸收参数不断增大,原因可能为H-2-168的pKa为7.4左右(课题组前期实验测的结果),当溶液的pH值接近其pKa值时,H-2-168主要以分子形式存在,易于药物的吸收。
P-gp是肠道内主要的外排转运蛋白,能够将底物从肠上皮细胞主动泵出细胞外,影响药物吸收,降低生物利用度。本研究结果测得H-2-168的ER为1.13,根据美国FDA指导原则,当ER接近于1时可认为该药物不是P-gp的底物;当ER>2时,则可认为该药物是P-gp的底物[17],H-2-168的ER接近1,提示H-2-168不是P-gp的底物,且通过加入抑制剂前后Papp的变化,进一步证实了168不是P-gp的底物,但H-2-168是否是其他外排转运蛋白如多药耐药相关蛋白2(multidrug resistance related protein 2,MRP2)或乳腺癌耐药蛋白(breast cancer resistance protein,BCRP)等的底物尚待研究。
综上所述,本研究结合Caco-2细胞模型和大鼠在体单向肠灌流模型对H-2-168的肠吸收特性进行了评价,为后续剂型设计及进一步开发提供了重要的实验基础和科学依据。
