Objective:

To evaluate the safety and tolerance of a single administration versus multiple administrations of recombinant human parathyroid hormone 1-84 (PTH1-84) for injection in Chinese healthy volunteers, as well as to observe the pharmacokinetic characteristics in health individuals following drug administration.

Methods:

A single center, open, single administration/multiple administration trial was designed. The healthy subjects were divided into four dose groups: 25, 50, 75 and 100 μg, in which the 25 μg dose group was only given at a single dose, and the other three dose groups continued to conduct continuous administration after a single dose. The physical signs of the subjects were recorded, and the biological samples were collected at the time specified in the scheme to calculate and analyze the pharmacokinetic parameters. The safety of different doses of recombinant PTH1-84 was evaluated by the pharmacokinetic parameters of healthy subjects and the occurrence of adverse reactions.

Results:

A total of 28 healthy subjects were included in the study, with 4 participants in the 25 μg dosage group and 8 participants in each of the remaining three groups. In the safety evaluation, no correlation between the incidence of adverse events and dosages was observed, and no serious adverse events occurred. The reported adverse reactions resolved favorably. The pharmacokinetic results indicate that recombinant human parathyroid hormone 1-84, within the dosage range of 50 to 100 μg, demonstrates a dose-dependent relationship for both AUC and Cmax following single and multiple administrations. This suggests a positive correlation between drug exposure and dosage.

Conclusion:

PTH1-84 has good clinical safety and tolerance at the experimental dose.

目的:

评价中国健康受试者对注射用重组人甲状旁腺素1-84单次给药和多次给药的安全性和耐受性,同时观察给药后健康人体内的药动学特征。

方法:

采用单中心、开放、单次给药/多次给药的实验设计。纳入健康受试者,设置25,50,75,100 μg这4个剂量组,其中25 μg剂量组只单次给药,其余3个剂量组在单次给药后继续进行连续给药试验。记录受试者体征,按方案规定的时间点采集生物样本进行药动学参数计算和分析,通过健康受试者的药动学参数以及不良反应的发生来评价不同剂量的重组人甲状旁腺素1-84的安全性。

结果:

共纳入健康受试者28例,25 μg剂量组4例,剩余3组每组8例,男女均有,在安全性评价中,未观察到不良事件发生率与剂量存在相关性,且未发生严重不良事件,不良反应转归良好。药动学结果显示,重组人甲状旁腺素1-84在50~100 μg剂量范围内,单次给药和多次给药的AUC和Cmax均与剂量相关,提示药物暴露与剂量具有正相关性。

结论:

重组人甲状旁腺素1-84在试验剂量下具有良好的临床安全性及耐受性。

目的:

评价中国健康受试者对注射用重组人甲状旁腺素1-84单次给药和多次给药的安全性和耐受性,同时观察给药后健康人体内的药动学特征。

方法:

采用单中心、开放、单次给药/多次给药的实验设计。纳入健康受试者,设置25,50,75,100 μg这4个剂量组,其中25 μg剂量组只单次给药,其余3个剂量组在单次给药后继续进行连续给药试验。记录受试者体征,按方案规定的时间点采集生物样本进行药动学参数计算和分析,通过健康受试者的药动学参数以及不良反应的发生来评价不同剂量的重组人甲状旁腺素1-84的安全性。

结果:

共纳入健康受试者28例,25 μg剂量组4例,剩余3组每组8例,男女均有,在安全性评价中,未观察到不良事件发生率与剂量存在相关性,且未发生严重不良事件,不良反应转归良好。药动学结果显示,重组人甲状旁腺素1-84在50~100 μg剂量范围内,单次给药和多次给药的AUC和Cmax均与剂量相关,提示药物暴露与剂量具有正相关性。

结论:

重组人甲状旁腺素1-84在试验剂量下具有良好的临床安全性及耐受性。

Objective:

To evaluate the safety and tolerance of a single administration versus multiple administrations of recombinant human parathyroid hormone 1-84 (PTH1-84) for injection in Chinese healthy volunteers, as well as to observe the pharmacokinetic characteristics in health individuals following drug administration.

Methods:

A single center, open, single administration/multiple administration trial was designed. The healthy subjects were divided into four dose groups: 25, 50, 75 and 100 μg, in which the 25 μg dose group was only given at a single dose, and the other three dose groups continued to conduct continuous administration after a single dose. The physical signs of the subjects were recorded, and the biological samples were collected at the time specified in the scheme to calculate and analyze the pharmacokinetic parameters. The safety of different doses of recombinant PTH1-84 was evaluated by the pharmacokinetic parameters of healthy subjects and the occurrence of adverse reactions.

Results:

A total of 28 healthy subjects were included in the study, with 4 participants in the 25 μg dosage group and 8 participants in each of the remaining three groups. In the safety evaluation, no correlation between the incidence of adverse events and dosages was observed, and no serious adverse events occurred. The reported adverse reactions resolved favorably. The pharmacokinetic results indicate that recombinant human parathyroid hormone 1-84, within the dosage range of 50 to 100 μg, demonstrates a dose-dependent relationship for both AUC and Cmax following single and multiple administrations. This suggests a positive correlation between drug exposure and dosage.

Conclusion:

PTH1-84 has good clinical safety and tolerance at the experimental dose.

重组人甲状旁腺素1-84(recombinant human parathyroid hormone1-84,rhPTH1-84)临床拟用于绝经后女性骨质疏松症治疗,国外已有研究证明其临床使用较为安全[1-3],但中国人群使用相关研究及报道较少。本研究以中国健康受试者为研究对象,通过评价中国健康人群使用rhPTH1-84单次及连续给药的安全性、耐受性及药动学参数,为rhPTH1-84的后续试验及临床合理用药提供依据。
受试制剂:注射用重组人甲状旁腺素1-84(中国山东丹红制药有限公司,规格:100 μg·支-1,批号:S20170601);重组人甲状旁腺素1-34(BACHE公司,规格:1 mg·瓶-1,批号:1067417);正常人血清(Bioreclamation Inc公司,批号:HPS170518JIZ)。试剂盒信息: PTH (Intact,1-84) ELISA Kit(Alpco公司,货号:21-IPTHU-E01);HIGH Sensitivity Human PTH (1-34) ELISA Kit (Immutopics公司,货号:60-3900)。
本研究经中国中医科学院西苑医院伦理委员会的审查和批准(批准号:2017XL020),所有受试者均签署知情同意书。试验共入组28例健康受试者,其中A1组4例,A2组、A3组、A4组各8例,男女各半,平均年龄为(28.6±2.8)岁,平均身高(164.24±6.42) cm,平均体质量(60.20±7.21) kg,体质量指数(BMI)为(22.27±1.82) kg·m-2
纳入标准:①年龄在18~45周岁的中国健康志愿者,同批年龄相差<10岁,男女各半。②男性体重≥50 kg,女性体重≥45 kg;BMI(BMI=体重/身高2)介于19~26 kg·m-2之间(含临界值)。③受试者经病史询问、体格检查、生命体征检查、实验室检查等结果正常,女性尿妊娠试验阴性。④受试者理解并接受本研究的流程和限制要求,自愿参加本临床研究,并签署了书面的知情同意书。
排除标准:①存在研究者判定为有临床意义的疾病史(如甲状旁腺疾病史、高钙血症、高钾血症、高尿酸血症等病史)。②存在研究者判定为有临床意义的食物、药物过敏史。③有嗜烟、酗酒史,吸毒史或药物滥用史者。④不同意避免影响药物吸收、分布、代谢、排泄等因素者。⑤3个月内参加过其他任何临床试验者。⑥试验首次给药前3个月内曾使用抗骨质疏松药物或影响钙、磷平衡的药物者;或试验首次给药前14 d内服用过任何药物者。⑦研究者认为不适合参加本临床研究的受试者。
本试验采用开放、单中心设计,共设置A1组(25 μg),A2组(50 μg),A3组(75 μg)及A4组(100 μg) 4个剂量组。其中,A1组仅进行单次给药试验,A2~A4组在完成单次给药试验后,继续给予药物干预至d 7,进行多次给药试验观察。每个剂量组末次用药后至少2周进行安全性评估,A2组、A3组、A4组的试验均于上一剂量组的安全性评估合格后进行。
每支粉针(100 μg)溶于1 mL无菌注射用水中,现用现配。用无菌注射器抽取药品溶液,A1组抽取0.25 mL液体,A2组抽取0.50 mL液体,A3组抽取0.75 mL液体,A4组抽取全部液体。所有受试者均统一于早餐后在腹部进行皮下注射。
A2组、A3组、A4组受试者在第1次给药前(-2 h,-1 h,0 h),第1次给药后5 min,15 min,30 min,45 min,1 h,1.5 h,2 h,3 h,4 h,6 h,8 h,10 h,12 h,24 h,第5~7次早晨给药前,第7次给药后5 min,15 min,30 min,45 min,1 h,1.5 h,2 h,3 h,4 h,6 h,8 h,10 h,12 h,24 h的时间节点分别采集5 mL血样[4]。血样采集后置于事先已贴好标签的促凝管中,冰浴放置至少0.5 h,待血块收缩后在4 ℃以3 000 r·min-1离心15 min后分取血清。样本在-20 ℃冰箱内暂存,采集后24 h内转存至(-80±10) ℃冰箱保存。
将空白基质、rhPTH1-84 HQC(300.01 pg·mL-1)和LQC(25.73 pg·mL-1)分别加入以下浓度的PTH1-34 (Cat:4011474.0001):500,200,100,0 pg·mL-1(对照组),配制一套特异性样品进行检测。
稀释标准品,使其浓度分别为5,10,20,40,80,120,240,400,600 pg·mL-1。检测对应OD值,以四参数逻辑回归方程进行拟合,得到标准曲线。
配制rhPTH1-84及rhPTH1-34 LLOQ,LQC,MQC,HQC及ULOQ的质控样本,每个质量浓度6份样品,在不同天内连续测定3批,计算得到日内及日间精密度及准确度数据。
选择人血清HQC(299.75 pg·mL-1)和LQC(25.55 pg·mL-1)两组浓度的rhPTH1-84质控样品进行稳定性检测,rhPTH1-34质控样品的稳定性试验选择人血清HQC(120.00 pg·mL-1)和LQC(25.00 pg·mL-1)两组浓度的样品。考察4 h室温储存稳定性,处理后的样品2~8 ℃、20 h稳定性,5次冻融循环稳定性及长期存储稳定性。
药动学(PK)参数计算由Phoenix WinNonlin 8.0完成,其他使用SAS 9.4软件分析,其中proc mixed用于分析连续型变量,proc freq用于分析离散型变量。GraphPad Prism v.9.1用于统计分析和绘图。统计学显著性设定为P<0.05。
结果显示每个特异性样品检测结果的偏差及精密度均在20%之内,且当PTH1-34浓度达到500 pg·mL-1,PTH1-84均具有很好的特异性。
rhPTH1-84的曲线方程为y=-23.99·[1+(x·3 292-1)1.2]-1+24.08(R2=0.998),权重因子为y-2,定量范围为10.00~400.00 pg·mL-1。人血清中rhPTH1-34的曲线方程为y=-28.038 62·[1+(x·609-1)1.698]-1+28.11(R2=0.996),权重因子为y-2,定量范围为10.00~200.00 pg·mL-1
结果显示,各组样品精密度与准确度均符合预设接受标准。具体结果见表1
结果显示,各检测条件下,样品检测浓度偏差不超过20%,精密度在20%以内,具有良好的稳定性。
将d 1给药前-2 h,-1 h和0 h的血药浓度取平均值为“平均基线浓度”,基线调整后的血药浓度=血药浓度-平均基线浓度。对于扣除基线以后浓度为负值者,按0处理。A2组~A4组受试者基线校正后PTH1-84和PTH1-34的血药浓度-时间曲线详见图1
单次及连续皮下注射rhPTH1-84后的主要药动学参数及与剂量的相关系数见表2表3。本试验中绝大多数受试者仅能在达峰时间附近检测到PTH1-34的浓度,而其他大部分时间点的PTH1-34浓度低于检测定量下限,这使得PTH1-34的药动学参数无法准确计算。
单次给药A2组、A3组和A4组三组的剂量差异对PTH1-84的AUC0-24和AUC0-∞具有极显著影响(P<0.001),药动学参数与用药剂量的Pearson相关系数检验结果显示,在50~100 μg剂量范围内,单次皮下注射rhPTH1-84后,PTH1-84的AUC0-24和AUC0-∞与用药剂量呈明显的正相关性。
多次给药A2组、A3组和A4组三组的剂量差异对PTH1-84的AUCssCavg具有极显著影响(P<0.001),对PTH1-84的Css, max具有显著影响(P<0.05),对PTH1-84的Tss,max,DF无显著影响(P>0.05)。在50~100 μg剂量范围内,多次皮下注射rhPTH1-84后,PTH1-84的AUCssCss与用药剂量呈明显的正相关性。
rhPTH1-84在28例健康受试者中完成了单次及连续给药耐受性临床观察,试验期间共26例受试者发生96例次不良事件,没有发生严重不良事件,所有不良事件的严重程度皆为“1级”或“2级”。不良反应发生率为92.9%,其中发生频率大于10%的常见不良反应有:血钙升高(64.3%)、尿钙升高(21.4%)、血磷降低(17.9%)和贫血(14.3%)、谷丙转氨酶升高(10.7%)和尿酸升高(10.7%)。所有的药物不良反应严重级别均较低,且转归良好,提示rhPTH1-84临床安全性、耐受性较好。
rhPTH1-84在体内可以降解为PTH1-34,刺激骨松质层和骨皮质层的新骨形成,改善骨小梁微结构[5],增加骨密度和骨强度[6]。同类产品Preotact已于欧盟被批准用于治疗患有骨质疏松症和骨折风险高的绝经后妇女[7]
本研究结果显示,在药动学方面,本品单次给药各剂量组PTH1-84及PTH1-34的Tmax均在1 h以内,药物达峰较快,PTH1-84的t1/2约1~2 h,而PTH1-34作为PTH1-84具有药理药效作用的降解产物,在50及100 μg剂量组的t1/2在35~40 h,有利于药效的发挥。已有研究显示单次皮下注射rhPTH1-84后,血清中的PTH1-84很快达到峰浓度,并呈现明显的双峰,提示rhPTH1-84可能通过一个快速和一个相对较慢的途径从注射部位吸收进入体循环,另外PTH1-84和PTH1-34在体内的清除非常快[8]。这与本次试验中观察到的rhPTH1-84的药动学特征基本一致。
PTH作为调节血内钙、磷水平的主要激素之一,主要作用于肾脏和骨骼[9],作用于肾脏时可促进肾小管对Ca2+的重吸收,减少钙质流失,并促成活性维生素D的合成,由于肾小管的重吸收作用,导致尿酸排泄减少,可能出现高尿酸血症的症状[10-11]。因此认为,试验过程中出现的常见不良反应与PTH的药理作用相关,是可预期的不良反应。Tay等[11]进行了一项为期8年的rhPTH1-84治疗甲状旁腺功能减退症患者安全性前瞻性研究,结果表明rhPTH1-84在25,50及100 μg剂量的治疗方案中具有良好的安全性。本次试验中,在25~100 μg剂量范围内,未观察到严重的不良事件,表明此剂量范围内rhPTH1-84在中国受试者中具有良好的安全性及耐受性。
