AIM

To investigate and analyze the adverse drug event (ADE) signals associated with ceftriaxone in children in the real world, and provide references for the safety evaluation of ceftriaxone in children.

METHODS

ADE reports in the US FDA Adverse Event Reporting System (FAERS) from the first quarter of 2004 to the third quarter of 2022 involving children aged 0-17 years with ceftriaxone as the primary suspected drug were extracted. ADEs were classified into organ systems using the Medical Dictionary for Regulatory Activities (MedDRA). The reporting odds ratio (ROR) and proportional reporting ratio (PRR) methods were utilized to identify ADE signals associated with ceftriaxone.

RESULTS

A total of 999 ADE reports were collected for children aged 0-17 years with ceftriaxone as the primary suspected drug. There were 139 related ADE signals affecting 18 organ systems. The top 3 ADEs in terms of frequency were acute kidney injury, cholelithiasis, and postrenal acute renal failure. The top 3 ADEs in terms of signal strength were postrenal acute renal failure, ureteral calculus, and intravascular hemolysis. The top 10 ADEs in terms of signal strength for ceftriaxone in different age groups of children primarily involved the hepatobiliary system, renal and urinary system, and the blood and lymphatic system. After the update of the ceftriaxone package insert in July 2007, the proportion of pediatric stone-related ADEs caused by ceftriaxone decreased from 5.88% to 5.61% among all ADEs. Among the top 10 concomitant medications reported in ADEs, other types of antibiotics and nonsteroidal anti-inflammatory drugs were the most commonly used.

CONCLUSION

When ceftriaxone is used in children, caution should be exercised regarding its potential adverse events on the liver, gallbladder, and renal system. Additionally, attention should be paid to the risk of adverse events when used in combination with other medications.

目的

挖掘分析真实世界中儿童使用头孢曲松的药物不良事件(ADE)信号,为儿童使用头孢曲松的安全性评估提供参考。

方法

提取美国食品和药物管理局不良事件报告系统(FAERS)中2004年第一季度至2022年第三季度以头孢曲松为首要怀疑药物的0~17岁儿童的ADE报告,利用国际医学用语词典(MedDRA)对ADE进行器官系统归类,采用报告比值比(ROR)法和比例报告比值比(PRR)法挖掘头孢曲松ADE信号。

结果

收集到0~17岁儿童中以头孢曲松为首要怀疑药物的ADE报告共999份,相关ADE信号139个,累及18个器官系统。发生频次前3位ADE分别为急性肾损伤、胆石症和肾后性肾衰竭;信号强度前3位ADE分别为肾后性肾衰竭、输尿管结石和血管内溶血。各年龄段儿童头孢曲松信号强度前10位ADE均主要累及肝胆系统、肾脏及泌尿系统和血液及淋巴系统。2007年7月头孢曲松说明书更新后,头孢曲松所致儿童结石类ADE在全部ADE中所占构成比由5.88%降至5.61%。ADE报告中联合用药排名前10位的药物以其他类型的抗生素和非甾体抗炎药为主。

结论

头孢曲松在儿童中应用时,应警惕其对肝胆和肾脏系统可能造成的不良事件,同时注意与其他药物联合使用带来的不良事件风险。

目的

挖掘分析真实世界中儿童使用头孢曲松的药物不良事件(ADE)信号,为儿童使用头孢曲松的安全性评估提供参考。

方法

提取美国食品和药物管理局不良事件报告系统(FAERS)中2004年第一季度至2022年第三季度以头孢曲松为首要怀疑药物的0~17岁儿童的ADE报告,利用国际医学用语词典(MedDRA)对ADE进行器官系统归类,采用报告比值比(ROR)法和比例报告比值比(PRR)法挖掘头孢曲松ADE信号。

结果

收集到0~17岁儿童中以头孢曲松为首要怀疑药物的ADE报告共999份,相关ADE信号139个,累及18个器官系统。发生频次前3位ADE分别为急性肾损伤、胆石症和肾后性肾衰竭;信号强度前3位ADE分别为肾后性肾衰竭、输尿管结石和血管内溶血。各年龄段儿童头孢曲松信号强度前10位ADE均主要累及肝胆系统、肾脏及泌尿系统和血液及淋巴系统。2007年7月头孢曲松说明书更新后,头孢曲松所致儿童结石类ADE在全部ADE中所占构成比由5.88%降至5.61%。ADE报告中联合用药排名前10位的药物以其他类型的抗生素和非甾体抗炎药为主。

结论

头孢曲松在儿童中应用时,应警惕其对肝胆和肾脏系统可能造成的不良事件,同时注意与其他药物联合使用带来的不良事件风险。

AIM

To investigate and analyze the adverse drug event (ADE) signals associated with ceftriaxone in children in the real world, and provide references for the safety evaluation of ceftriaxone in children.

METHODS

ADE reports in the US FDA Adverse Event Reporting System (FAERS) from the first quarter of 2004 to the third quarter of 2022 involving children aged 0-17 years with ceftriaxone as the primary suspected drug were extracted. ADEs were classified into organ systems using the Medical Dictionary for Regulatory Activities (MedDRA). The reporting odds ratio (ROR) and proportional reporting ratio (PRR) methods were utilized to identify ADE signals associated with ceftriaxone.

RESULTS

A total of 999 ADE reports were collected for children aged 0-17 years with ceftriaxone as the primary suspected drug. There were 139 related ADE signals affecting 18 organ systems. The top 3 ADEs in terms of frequency were acute kidney injury, cholelithiasis, and postrenal acute renal failure. The top 3 ADEs in terms of signal strength were postrenal acute renal failure, ureteral calculus, and intravascular hemolysis. The top 10 ADEs in terms of signal strength for ceftriaxone in different age groups of children primarily involved the hepatobiliary system, renal and urinary system, and the blood and lymphatic system. After the update of the ceftriaxone package insert in July 2007, the proportion of pediatric stone-related ADEs caused by ceftriaxone decreased from 5.88% to 5.61% among all ADEs. Among the top 10 concomitant medications reported in ADEs, other types of antibiotics and nonsteroidal anti-inflammatory drugs were the most commonly used.

CONCLUSION

When ceftriaxone is used in children, caution should be exercised regarding its potential adverse events on the liver, gallbladder, and renal system. Additionally, attention should be paid to the risk of adverse events when used in combination with other medications.

头孢曲松(ceftriaxone)属于第三代头孢菌素类药物,1984年获得美国食品和药物管理局(FDA)批准上市[1],1994年进入中国市场,抗菌谱广且半衰期长,在儿童抗感染治疗中应用广泛,主要用于治疗敏感菌株引起的呼吸道感染、腹腔感染、败血症、脑膜炎等[2]。头孢曲松最常见的药物不良事件(ADE)包括皮疹、过敏反应、消化道反应及血液学检查异常等[3]。有研究报道了国内儿童使用头孢曲松的ADE文献分析,但大多基于单中心数据或文献案例报道,样本数量较少[4]。近年来,对药物安全性及ADE的研究常使用大型药物警戒系统数据库进行,以了解ADE发生的规律和特点。本研究旨在挖掘FDA不良事件报告系统(FAERS)中儿童使用头孢曲松相关ADE信号,为儿童的临床安全用药提供参考。
本研究数据来源于FAERS。通过在线OpenVigil检索工具[5]对FAERS数据库中的数据进行检索,限定目标药物名称为“ceftriaxone”。设定最大年龄为“17岁”,获取2004年第一季度至2022年第三季度首要怀疑药物为头孢曲松的ADE报告;分别设定年龄范围为“0~2岁”、“3~6岁”、“7~12岁”及“13~17岁”,获取不同年龄段儿童以头孢曲松为首要怀疑药物的ADE报告。2007年7月,FDA发布信息更新说明书,对头孢曲松与含钙产品同时使用的潜在风险进行了警示。分别设定报告提交时间为“2004年1月—2007年7月”及“2007年8月—2022年9月”,获取以2007年7月为节点头孢曲松说明书更新前后结石类ADE与全部ADE报告。
使用《国际医学用语词典》(MedDRA)25.1版本对ADE用语进行标准化和汉化。将属于同一个首选术语(PT)的ADE合并,并按照系统器官分类(SOC)对PT进行分类整理。
采用比例报告比值比(PRR)法和报告比值比(ROR)法进行信号挖掘与统计分析。PRR和ROR法的计算均基于比例失衡测量法四格表,见表1。两种比值的计算公式为:ROR=(a/c)/(b/d),;PRR=[a/(a+b)]/[c/(c+d)],χ2=(ad-bc)2(a+b+c+d)/[(a+b)(c+d)(a+c)(b+d)]。PRR与ROR值越大,表示目标药物与该ADE之间相关性越强。信号判断标准:ROR法——满足a≥3,且ROR 95%CI下限>1时为检出信号;PRR法——满足a≥3,χ2≥4,PRR≥2时为检出信号。规定同时满足两种算法标准则生成1个ADE信号。将不同年龄段儿童分组为0~2岁(婴幼儿)、3~6岁(学龄前期)、7~12岁(学龄期)、13~17岁(青少年),统计各年龄段儿童头孢曲松信号强度前10位ADE。统计头孢曲松说明书更新前后结石类ADE在全部ADE中所占构成比。统计在儿童以头孢曲松为首要怀疑药物的ADE中,与头孢曲松联合用药排名前10位的药物。统计分析均采用Microsoft Excel 2016软件完成。
按照上述限定标准从FAERS数据库中检索到以头孢曲松为首要怀疑药物的ADE报告999份,其中男性、女性、性别未知占比分别为55.6%、40.0%、4.4%,0~2岁、3~6岁、7~12岁、13~17岁患者占比分别为29.0%、28.5%、27.1%、15.3%。报告国家中美国上报213例,占比21.3%;其次为中国,上报196例,占比19.6%。给药方式主要为静脉注射、静脉滴注和肌内注射。
按照上述信号判定标准挖掘出头孢曲松相关ADE信号143个,排除各类损伤、中毒及操作并发症和各种手术及医疗操作等与药物本身无关的信号后,最终获得有效ADE信号139个,共累及18个SOC。ADE例数占比前3位的SOC为肾脏及泌尿系统疾病、肝胆系统疾病和皮肤及皮下组织类疾病,信号数占比前3位的SOC为肾脏及泌尿系统疾病、各类检查和皮肤及皮下组织类疾病,见表2。主要ADE比例失衡四格表数值见表3。将139个ADE信号按照发生频次及信号强度大小进行排序,发生频次前3位ADE分别为急性肾损伤、胆石症和肾后性肾衰竭;信号强度前3位ADE分别为肾后性肾衰竭、输尿管结石和血管内溶血,见表4表5
将不同年龄段儿童分组为0~2岁(婴幼儿)、3~6岁(学龄前期)、7~12岁(学龄期)、13~17岁(青少年)。各年龄段儿童头孢曲松信号强度前10位ADE均主要累及肝胆系统、肾脏及泌尿系统和血液及淋巴系统,见表6
2004年1月—2007年7月头孢曲松所致儿童结石类ADE在全部ADE中所占构成比为5.88%,2007年8月—2022年9月头孢曲松所致儿童结石类ADE在全部ADE中所占构成比为5.61%。
在儿童以头孢曲松为首要怀疑药物的ADE中,与头孢曲松联合用药前10位以其他类型的抗生素和非甾体抗炎药为主,包括万古霉素64例(6.4%)、对乙酰氨基酚46例(4.6%)、布洛芬29例(2.9%)、甲硝唑27(2.7%)、阿奇霉素23例(2.3%)、克林霉素23例(2.3%)、阿米卡星20例(2.0%)、氨苄青霉素18例(1.8%)、地塞米松16例(1.6%)、头孢氨苄14例(1.4%)。
头孢曲松属β-内酰胺类半合成抗生素,抗菌谱可以覆盖多种革兰阳性菌、革兰阴性菌及厌氧菌,属于国家基本药物,在儿童抗感染治疗中应用较为广泛[6]。说明书中列出的头孢曲松常见不良反应包括皮肤及皮下组织类疾病(表现为皮疹、瘙痒、荨麻疹、水肿、多形性红斑等)、消化道反应(表现为恶心、呕吐、腹痛、腹泻等)、神经系统反应(表现为头痛、眩晕等)、血液学改变(多表现为嗜酸粒细胞增多、白细胞减少、血小板减少、溶血性贫血等),肝肾功能异常发生率分别为5%和1.4%(多数表现为一过性血清转氨酶升高、血尿素氮和血肌酐升高等),严重不良反应少见,整体耐受性较好[7]。多项研究表明头孢曲松不良反应在各年龄段均有发生,其中18岁以下的儿童所占构成比较高[8,9]。因此,在儿童中应用头孢曲松时需特别注意。
本研究挖掘了FAERS数据库中儿童使用头孢曲松的ADE,并采用ROR法和PRR法进行统计分析,共获得139个ADE信号,涉及18个系统和器官。说明书中提及的ADE所累及的器官系统包括皮肤及皮下组织、各类检查、消化道系统、神经系统、肝胆系统、泌尿系统等在本研究中均有信号检出。报告数前20位和信号强度前20位的ADE中,溶血性贫血、荨麻疹、腹痛、瘙痒、嗜酸粒细胞增多等已收录于说明书中,其余多数ADE未收录在说明书中,但在临床研究中已有报道。其中肾脏及泌尿系统和胆道系统的严重ADE占比较大,包括胆石症、急性肾衰竭、输尿管结石、肾盂积水、无尿等。各年龄段儿童头孢曲松信号强度前10位ADE主要累及肝胆系统、肾脏及泌尿系统和血液及淋巴系统,表明各年龄段儿童使用头孢曲松后都可能发生胆道及肾脏不良反应。头孢曲松经静脉注射或肌内注射后,大部分由肾小球滤过后以原型形式经尿排出(约占33%~67%),其余则经胆汁排出,因此在尿液及胆汁中浓度较高[10]。DAUDON等[11]的研究表明,头孢曲松可通过促使尿钙排泄及促进形成钙结晶诱导药物性肾结石及输尿管结石形成。多项研究报道了头孢曲松用于儿童抗感染治疗后出现肾结石、胆道结石、假性胆囊结石及急性肾衰竭等ADE,且大多数病例中治疗剂量的头孢曲松就可导致尿液结晶、结石形成。儿童由于肾结石导致尿路梗阻,表现为少尿或无尿,进而引起肾绞痛或急性肾衰竭[12-14]。相较于儿童,成年人在使用头孢曲松后出现肾结石及急性肾衰竭的案例较为罕见,且主要在头孢曲松使用过量时出现[10,15]。AZARKAR等[16]的研究显示,成年人接受头孢曲松治疗后胆结石和肾结石的发生率分别为8.8%和1.5%,而一项病例对照研究结果表明儿童接受头孢曲松治疗后假性结石的发生率为25%~50%,风险高于成人[17]。LIU等[18]使用ROR法和贝叶斯置信度递进神经网络法检测头孢曲松相关肝胆结石信号,发现头孢曲松所致肝胆结石与患者年龄有关,儿童及老年女性患者可能是其危险因素,且无论何种剂量的头孢曲松,在肝胆结石发生中均显示出阳性信号,信号值随药物剂量增加而增加。肾盂积水、肾乳头坏死、胆囊炎、胆道病、胆管炎、胆管扩张可能为肾结石及胆道结石的继发疾病。另外,一项病例对照研究结果显示,头孢曲松有诱发儿童肝炎及肝转氨酶升高的风险,可能与头孢曲松促进钙沉淀所致的毒性作用以及胆汁淤积有关[19]。头孢曲松造成的药物性结石一般较小,且停药后结石可自行消失[13]。因此,在儿童中应用头孢曲松时医师应密切关注患者的胆道及肾脏功能,检测到功能异常时评估是否需停药观察。
2007年7月FDA发出警示:头孢曲松与含钙剂或含钙产品合并用药有产生头孢曲松-钙盐沉淀的风险。本研究以2007年7月为节点,统计头孢曲松说明书更新前后儿童结石类ADE在全部ADE中所占构成比,结果显示,说明书更新后头孢曲松所致结石类ADE在全部ADE中所占构成比由5.88%降至5.61%。此前已有多例病例报道显示,新生儿在接受静脉注射头孢曲松和静脉注射钙后出现心脏骤停,在肺和肾内发现头孢曲松-钙盐沉淀物[20]。对于大于28 d的患者,使用头孢曲松和含钙产品在体内形成沉淀的危险较低[21],但仍有少量病例报道显示大于28 d的儿童及成年人在合用头孢曲松和含钙产品后出现严重ADE[22,23]。因此,医师在应用头孢曲松时应严格遵循其使用方法及注意事项,实时监测患者情况,避免产生严重后果。
头孢曲松与皮肤血管炎相关文献报道较少。ALMASOUDI等[24]指出药物性白细胞破碎性血管炎约占所有血管炎病例的10%,β-内酰胺类、磺胺类和喹诺酮类药物可能是其病因。婴儿急性出血性水肿(AHEI)是一种好发于2岁以下儿童的自限性白细胞破碎性小血管炎,病因尚不明确,目前认为可能与感染和药物诱导有关[25]。头孢曲松相关的AHEI仅有罕见文献报道,本研究中AHEI阳性信号较强,提示头孢曲松可能诱导AHEI的发生。其余信号较弱的ADE包括呼吸系统、肌肉骨骼及结缔组织、神经系统、精神类、代谢及营养类相关ADE等均未在说明书中提及,且在文献中报道也较少,因此有待进一步研究。
在儿童以头孢曲松为首要怀疑药物的ADE中,与头孢曲松联合用药前10位分别为万古霉素、对乙酰氨基酚、布洛芬、甲硝唑、阿奇霉素、克林霉素、阿米卡星、氨苄青霉素、地塞米松及头孢氨苄。药物以其他类型的抗生素及非甾体抗炎药为主。万古霉素是治疗严重革兰阳性菌感染的首选药物,而头孢曲松抗菌谱可以覆盖部分革兰阳性菌和革兰阴性菌,两者常用于联合治疗严重细菌感染的儿童。但研究表明,万古霉素与儿童急性肾损伤的相关性高达22%[26],将其与头孢曲松联合使用可能加剧儿童肾损伤的风险。KHALILI等[27]研究显示,使用头孢曲松单药治疗的患者中有1.3%出现急性肾损伤,而同时使用头孢曲松和氨基糖苷类药物治疗的患者中17%出现急性肾损伤。因此,头孢曲松在与其他具有肝肾毒性的药物联用时,应加强对患者肝肾功能的监测。
综上所述,在儿童使用头孢曲松的临床用药过程中应警惕其在肝胆、肾脏中的严重ADE,注意对神经系统、呼吸系统、肌肉骨骼及心脏等说明书中未提及或较少提及的器官系统进行ADE监测,同时关注头孢曲松与其他类型抗生素联合用药对ADE的影响。另外,本研究存在一定局限性。FAERS属于自发呈报系统,数据存在错报、描述模糊等问题;统计分析时,由于无法获得头孢曲松的所有使用数据,因此仅能计算ADE发生的构成比,无法计算发生率;ROR法和PRR法的计算均基于比例失衡四格表法,该方法敏感性高,但容易出现假阳性;最后,信号挖掘结果仅说明头孢曲松与ADE之间存在统计学关联,具体是否存在因果联系仍需进一步证实。
