Preeclampsia is a common hypertensive disorder of pregnancy, which seriously endangers the health of mothers and infants. The early application of low-dose aspirin can effectively prevent preeclampsia, and effectively reduce the probability of hemolysis, elevated liver function and low platelet count syndrome, placental abruption, and fetal distress. In recent years, with the deepening of research, more experimental evidences show that there is a correlation between gene polymorphisms and aspirin resistance. Because some patients have gene polymorphisms, the expected therapeutic effect cannot be achieved. This paper reviewed the research progress of precision medication of aspirin in high-risk pregnant women with preeclampsia, in order to guide the medication of aspirin in special groups such as pregnant women with preeclampsia and improve pregnancy outcomes.

先兆子痫是一种常见的妊娠期高血压疾病,严重危害母婴健康。早期应用低剂量阿司匹林在可有效预防先兆子痫,并可以有效降低发生溶血肝功能异常血小板减少综合征、胎盘早剥及胎儿窘迫等并发症的概率。近年来,随着研究的不断深入,较多实验证据表明基因多态性与阿司匹林抵抗存在相关性,由于部分患者存在基因多态性,从而达不到预期治疗效果。本文就先兆子痫高危孕妇阿司匹林精准用药的研究进展进行综述,以指导阿司匹林在预防先兆子痫高危孕妇这类特殊人群的用药,改善妊娠结局。

先兆子痫是一种常见的妊娠期高血压疾病,严重危害母婴健康。早期应用低剂量阿司匹林在可有效预防先兆子痫,并可以有效降低发生溶血肝功能异常血小板减少综合征、胎盘早剥及胎儿窘迫等并发症的概率。近年来,随着研究的不断深入,较多实验证据表明基因多态性与阿司匹林抵抗存在相关性,由于部分患者存在基因多态性,从而达不到预期治疗效果。本文就先兆子痫高危孕妇阿司匹林精准用药的研究进展进行综述,以指导阿司匹林在预防先兆子痫高危孕妇这类特殊人群的用药,改善妊娠结局。

Preeclampsia is a common hypertensive disorder of pregnancy, which seriously endangers the health of mothers and infants. The early application of low-dose aspirin can effectively prevent preeclampsia, and effectively reduce the probability of hemolysis, elevated liver function and low platelet count syndrome, placental abruption, and fetal distress. In recent years, with the deepening of research, more experimental evidences show that there is a correlation between gene polymorphisms and aspirin resistance. Because some patients have gene polymorphisms, the expected therapeutic effect cannot be achieved. This paper reviewed the research progress of precision medication of aspirin in high-risk pregnant women with preeclampsia, in order to guide the medication of aspirin in special groups such as pregnant women with preeclampsia and improve pregnancy outcomes.

先兆子痫(preeclampsia)是一种妊娠期高血压疾病,其发生发展可较大影响母婴健康和安全[1],若不及时干预,严重者可能会发生溶血肝功能异常血小板减少综合征、胎盘早剥及胎儿窘迫等严重并发症。阿司匹林是常用抗血小板药物,其在预防先兆子痫中的应用已得到广泛验证,但心脑血管疾病临床研究显示部分患者易出现阿司匹林抵抗(aspirin resistance,AR)现象[2],即在规范接受阿司匹林治疗后,心脑血管疾病仍复发。据统计,AR受基因多态性的调控,AR的发生有1/3是由基因决定的[3]。因此,对阿司匹林实施精准个体化用药十分必要。现就先兆子痫高危孕妇阿司匹林精准用药的研究进展进行综述。
先兆子痫的发病机制目前尚未完全明确[4],现今较为公认胎盘血管的发育异常和功能障碍是其主要致病因素[5]。第一阶段发生于妊娠前半期,其特点主要表现为滋养细胞对子宫肌层的浸润异常、子宫螺旋动脉重铸不良、胎盘灌注减少,临床症状不明显;第二阶段即妊娠后半期,异常的胎盘会释放多种可溶性有毒因子,促进炎症反应,加重血管内皮功能障碍和母体全身小血管痉挛,导致多器官功能受损,出现多种临床症状,如新发高血压、微血栓和蛋白尿等[6]
先兆子痫的基本病理改变是全身小血管动脉痉挛。由于小动脉痉挛,血流减少,全身各器官组织缺血、缺氧并产生氧化应激反应,干扰前列腺素合成平衡,血栓素A2(TXA2)增加,前列环素(PGI2)减少,使PGI2与TXA2比值下降,导致血压升高等病理改变[7]
环氧合酶2(COX-2)是COX基因合成前列腺素的关键限速酶之一,介导调节全身各种炎症介质,COX-2的低表达对改善炎症反应及内皮细胞损伤等有一定影响[8,9]。基质金属蛋白酶9(MMP-9)为一类由绒毛外滋养细胞层产生的有效蛋白水解酶,在滋养细胞侵袭力和重塑血管动态平衡中发挥重要作用,MMP-9的高表达有利于胎盘着床[10,11]。血小板膜表面的血清糖蛋白(GP)Ⅱb/Ⅲa是与血小板聚集有关的纤维蛋白原受体,介导血小板与纤维蛋白原结合,使血小板聚集,导致血栓形成。GP Ⅱb/Ⅲa受体的低表达有利于降低先兆子痫的发生[12]。脂联素(ADP)是脂源性细胞胰岛素增敏因子,参与机体糖代谢、脂代谢,具有调脂降糖等作用,并调节内皮细胞炎症反应,与先兆子痫患者胰岛素抵抗增加、血管内皮损伤及炎症反应关系密切[13]
阿司匹林是临床常用非甾体抗炎药,具有抗血小板、解热、镇痛、抗炎等作用[7],阿司匹林预防先兆子痫的作用机制主要包括以下两个方面:(1)通过抑制COX使TXA2的合成减少,降低TXA2与PGI2的比值(TXA2/PGI2),避免先兆子痫的发生;(2)可溶性酪氨酸激酶1(soluble Fms-like tyrosine kinase-1, sFlt-1)水平升高是先兆子痫的发病机制之一[14],阿司匹林通过抑制COX,抑制氨基末端激酶JNK/AP-1通路,减少血浆sFlt-1的产生,避免血小板聚集和黏附。当血小板被激活时,释放出的花生四烯酸(AA)在COX的作用下生成前列腺素G2(PGG2),并在MMP-9的参与下增加血管内皮生长因子(VEGF)的释放[15],合成TXA2。同时被激活的血小板通过脱颗粒作用释放TXA2和ADP,两者分别与血小板表面的TXA2受体(TXA2R)及ADP受体(P2Y1和P2Y12受体)结合,使GP Ⅱb/Ⅲa受体结合位点暴露,导致血小板聚集。低剂量阿司匹林的使用可通过干扰AA的转化,抑制COX及血小板聚集,以减少血栓的形成。但是,与血小板聚集途径相关的TXA2、COX、MMP-9、GP受体及ADP受体基因的多态性在作用过程中会影响阿司匹林的血小板抑制作用,从而导致AR的发生[3]
TXA2R基因位于人19号染色体p13.3上,包含3个内含子、3个外显子和1个附加外显子。在胎盘组织中TXA2主要由滋养细胞生成,参与妊娠期全过程,其合成调控异常可能会导致妊娠期间发生先兆子痫、胎儿窘迫等并发症。许晶晶等[16]在2 881例冠心病患者的研究中对TXA2R的4个代表性单核苷酸多态性(tagSNP)位点进行检测(包括rs2238630、rs2238631、rs2238633、rs3786989),研究结果显示SNP与AR的发生不存在明显关系。笔者经过文献查阅,发现国内外在对TXA2R基因SNP与AR发生相关性方面的临床研究较少,因此需要进一步进行相关基因检测,从而确定该基因多态性与孕妇AR发生的相关性。
COX是参与合成前列腺素的重要限速酶,在人体中以COX-1和COX-2两种亚型存在。阿司匹林是非选择性COX酶抑制剂,对两种亚型均产生抑制,从而抑制机体前列腺素的分泌。阿司匹林使其中COX-1丝氨酸530产生不可逆的乙酰化作用而阻断TXA2的合成。
COX-1基因位于染色体9q32~q33.3,含10个内含子和11个外显子。由于阿司匹林直接作用于COX-1而发挥血小板聚集作用,因此COX-1基因多态性可能影响阿司匹林的抗血小板作用。WENG等[17]通过Meta分析发现,COX-1基因上A842G(rs10306114)、C50T(rs3842787)位点突变可能与AR相关,但YI等[18]发现COX-1 C50T和A842G在中国人群中位点突变率很低而研究意义不大。COX-2基因位于染色体1q25.2-q25.3区,包含10个外显子和9个内含子,WENG等[17]通过Meta分析发现,COX-2的SNP位点rs20417被认为可能与阿司匹林抵抗相关。许晶晶等[16]通过筛选COX-2基因中国人群携带频率大于5%的tagSNP位点(rs3218625),并进行基因多态性检测,发现该位点在中国冠心病人群中发生基因突变的几率十分微小。
通过上述对COX基因的研究可知,COX基因存在基因多态性,部分位点突变可能与AR有关,但可能受到研究人群的种族、地域或身体条件的限制,出现不同的研究结果。因此,在孕妇这一特殊人群中进行的基因多态性与AR相关性研究需要进一步扩大样本量,充分考虑各种因素的限制。
MMP-9基因位于染色体20q11.2~q13.1,长度为26 000 bp,相对分子质量为92 000,是MMPs中分子质量最大的酶。其启动子区内包含活化蛋白(AP)-1、AP-2、刺激蛋白1因子、表达序列标签结合位点、核因子κB(NF-κB)和转化生长因子(TGF)-β控制元件。
MATSUMURA等[19]证实MMP-9基因启动子区存在1562C/T多态性,会影响基因的转录水平,导致基因功能发生改变,产生高活性(C/T、T/T)和低活性(C/C)的启动子基因型。有研究表明MMP-9表达主要由1562C/T调控,与先兆子痫的发病有关,1562C/T通过提高MMP-9的浓度及活性,影响螺旋动脉的重塑,有利于使胎盘浅着床,可以改善胎盘缺血、缺氧和血管内皮细胞受损的情况,从而降低发生妊娠期高血压疾病如先兆子痫的风险。有关研究指出,1562C/T的T等位基因在妊娠期高血压疾病中较少,C等位基因可能是妊娠期高血压疾病的易感基因之一[20]
血小板膜表面包含多种与血小板黏附、激活、聚集有关的GP受体。GP Ⅱb/Ⅲa通过纤维蛋白原的桥梁作用最终使血小板聚集,其基因多态性位点主要集中在GPⅢa rs5918,包括PLA1PLA2两种等位基因。FLOYD等[21]的荟萃分析指出,成人卒中与PLA2等位基因有较强关联,且和PLA1纯合子比较,PLA2的纯合个体与缺血性卒中的相关性更强。张璐[22]在对上述位点进行检测后发现,AR患者不存在GP受体基因多态性,且AR与该多态性不存在联系。因此,GP受体基因多态性与孕妇AR的关系还需要进行更深入的研究,寻找新的基因位点以得出更多结论。
ADP是诱导血小板聚集的重要物质之一,包括P2Y1P2Y12两种等位基因,均位于3号染色体上,P2Y1基因长度为4 000 bp,包括1个外显子,ADP通过促进血小板表面G蛋白与P2Y链接,诱导血小板聚集。张文婷等[23]研究指出,携带P2Y1 rs701265G等位基因是大动脉粥样硬化性卒中患者AR的独立危险因素;张璐[22]在对AR患者P2Y1 rs701265G等位基因的研究中发现了该基因位点的多态性,即GA、AA两种基因型,但尚未发现AR与该多态性的联系。KARAZHANOVA等[24]的研究发现,冠心病患者P2Y12基因中H2单倍体与AR有关,并且H2单倍体突变在疾病组与健康对照组之间的差异有显著意义。因此,ADP受体的基因多态性可能与孕妇的AR有关,但还需要进一步研究。
目前,应用低剂量阿司匹林预防先兆子痫已被国内外众多指南推荐[25]。2021年,世界卫生组织发布了应用抗血小板药物预防先兆子痫的建议,推荐低剂量阿司匹林(75 mg·d-1)用于预防妇女中度或高危先兆子痫[26]。据中国《妊娠期高血压疾病诊治指南(2020)》推荐,存在先兆子痫史以及先兆子痫高危因素者可以在妊娠早中期(妊娠12~16周)开始每日服用小剂量阿司匹林50~150 mg,依据个体因素决定用药时间,预防性应用可维持到妊娠26~28周[27]。中国《妊娠期血压管理中国专家共识(2021)》建议先兆子痫高危孕妇重视血压管理,及时发现血压异常,降低不良妊娠结局发生风险[28]。2020版美国妇产科医师学会(ACOG)实践指南推荐,具有先兆子痫高危因素和具有中等危险因素的妇女应在妊娠12~28周(最好在16周前)之间接受低剂量阿司匹林(81 mg·d-1)预防先兆子痫[29]。2021年美国预防服务工作组(USPSTF)建议以低剂量阿司匹林作为先兆子痫高危妊娠人群妊娠12周后预防用药[30]。2021年,昆士兰临床指南建议具有中高危先兆子痫风险的孕妇,从孕16周前至孕36周期间每晚口服阿司匹林100~150 mg[31]
2021年ROLNIK等[32]根据阿司匹林预防先兆子痫的循证研究得出,在孕16周或之前开始每日夜间服用阿司匹林150 mg,对于先兆子痫的预防效果可能更显著。2019年DULEY等[33]研究表明,妊娠妇女使用阿司匹林较为安全,未出现孕妇胎盘早剥、产后出血或胎儿脑出血等风险的报道。周文龙等[34]对早发型先兆子痫患者给予小剂量的阿司匹林联合硫酸镁进行治疗,结果显示,患者肝肾功能显著转好,且干预后发生早产、胎儿窘迫等的概率降低。
根据以上国内外研究报道,妊娠期间使用阿司匹林对孕妇和胎儿都有较高的安全性。为预防先兆子痫的发生,对于中、高危先兆子痫的孕妇,建议在妊娠早中期(妊娠12~16周)给予低剂量阿司匹林进行预防,但在用药过程中建议时刻注意阿司匹林的不良反应,如出血风险等。然而,目前最新的临床研究发现,使用低剂量阿司匹林并不能有效降低先兆子痫发生率。TOLCHER等[35]在3 135名妇女的低剂量阿司匹林试验中,选取血压正常、未生育的妇女在妊娠13~26周之间入组,随机分为每日服用阿司匹林60 mg或安慰剂,试验结果表明,接受阿司匹林的56名低风险非西班牙裔白人妇女的先兆子痫发生率显著降低,但总体而言,西班牙裔和57名非西班牙裔黑人妇女的先兆子痫发生率没有明显降低,因此发现阿司匹林治疗先兆子痫的疗效可能与不同民族和种族有一定关联。由此可见,种族差异和基因多态性可能影响阿司匹林预防先兆子痫的临床试验结果。不同临床研究的差异,可能与使用的剂量、不同人群基因分布的差异有关,在使用低剂量阿司匹林预防先兆子痫的过程中可能需要根据个体病情的不同选择合适的剂量。
阿司匹林作为经典抗血小板药物,在临床中应用范围广、频率高,但其在不同个体中的疗效差异明显,因此应针对各患者不同的风险因素采取个体化给药方案,以期达到精准用药的目的。(1)孕妇在孕期体重波动较大,对于存在先兆子痫高危因素的孕妇,建议在孕早期就注意体重控制及相关营养指导,在服用阿司匹林过程中根据身高体重制订个性化给药方案(如用药剂量、停药时间等),较好地改善妊娠结局;(2)对合并其他慢性疾病或自身免疫性疾病(如系统性红斑狼疮等)的孕妇,建议在孕前就做好相关筛查工作,并积极治疗,在怀孕期间应用低剂量阿司匹林的同时,与其他对控制原发疾病有效的药物进行联合、针对性治疗,发挥多学科会诊的作用,从而改善母婴结局;(3)对于存在先兆子痫高危因素的孕妇,应用阿司匹林期间建议进行血小板聚集率、胎盘生长因子等临床与生化监测,对病情实行精准调控,对阿司匹林有效抑制血小板聚集程度进行量化研究,从而达到精准用药的目的[22]
低剂量阿司匹林可用于预防先兆子痫的发生和发展,目前的研究推荐小剂量阿司匹林预防先兆子痫的起始时间在孕16周或之前,但停药时间不一,因此,对于先兆子痫的最佳预防方案尚未明确。尽管已被证明其对于母婴相对安全,但由于不同个体对阿司匹林反应的差异,如因种族不同产生的基因多态性而引发的不同程度的AR现象。因此,现阶段需要综合医生的经验,结合基因检测、体重、其他合并症等个体化信息,针对不同个体严格把握用药指征,降低用药风险,精准使用阿司匹林。
