AIM

To evaluate the influences of vitamin D2 injection on serum 25-hydroxyvitmin D (25(OH)D) and immune function in men with vitamin D deficiency aged 80 and over.

METHODS

A total of 48 men with vitamin D deficiency aged 80 and over were given intramuscular vitamin D2 600 000 IU every month until the serum 25(OH)D reached and over 30 µg·L-1. The levels of serum 25(OH)D2, 25(OH)D3, T lymphocyte cells (including CD3+, CD4+, CD8+), B lymphocyte cells (CD19+), and blood calcium and phosphorus were detected before and after treatment. And adverse reactions were observed.

RESULTS

A total of 40 patients completed the study. It took (6.2±1.4) months for 25(OH)D to reach targeted serum concentration. Compared with those before treatment, the serum levels of 25(OH)D2 after the treatment increased by (27.42±5.47) µg·L-1, 25(OH)D3 decreased by (5.25±3.38) µg·L-1, and the total 25(OH)D increased by (22.45±6.15) µg·L-1 (all P<0.01). And the serum levels of CD4+ T cells increased by (3.71±6.26)%, CD4+/CD8+ increased 0.31±0.93, and CD19+ B cell increased by (1.50±3.31)% (all P<0.05). There were no significant differences in serum CD3+ T cells, alanine aminotransferase, blood glucose, creatinine, calcium, and phosphorus before and after the treatment (P>0.05). No severe adverse drug reactions occurred during the treatment.

CONCLUSION

Vitamin D2 injection is helpful for increasing serum 25(OH)D2 levels and regulating immune function in men with vitamin D deficiency aged 80 and over.

目的

研究维生素D2注射液对80岁以上老年男性维生素D缺乏患者血清25-羟基维生素D[25(OH)D]和免疫功能的影响。

方法

选择48例年龄80岁以上男性维生素D缺乏[血清25(OH)D<20 μg·L-1]患者,给予维生素D2注射液(60万单位,肌内注射,每月1次)治疗,直至血清25(OH)D≥30 μg·L-1后停用。检测治疗前后血清25(OH)D2、25(OH)D3,T淋巴细胞亚群(包括CD3+、CD4+、CD8+)和CD19+ B细胞、血钙、血磷等水平,并观察不良反应发生情况。

结果

共40例患者完成研究,25(OH)D达标用时(6.2±1.4)个月。与治疗前比较,治疗后25(OH)D2升高(27.42±5.47)μg·L-1,25(OH)D3降低(5.25±3.38)μg·L-1,25(OH)D升高(22.45±6.15)μg·L-1,治疗前后比较均有非常显著差异(P<0.01)。CD4+ T细胞升高(3.71±6.26)%,CD4+/CD8+增高0.31±0.93,CD19+ B细胞升高(1.50±3.31)%,与治疗前比较均有显著差异(P<0.05)。血CD3+ T细胞、丙氨酸转氨酶、血糖、肌酐、血钙、血磷均无显著变化(P>0.05),无严重不良反应发生。

结论

维生素D2注射液可提高80岁以上老年男性维生素D缺乏患者的25(OH)D2水平,并可能调节其免疫功能。

目的

研究维生素D2注射液对80岁以上老年男性维生素D缺乏患者血清25-羟基维生素D[25(OH)D]和免疫功能的影响。

方法

选择48例年龄80岁以上男性维生素D缺乏[血清25(OH)D<20 μg·L-1]患者,给予维生素D2注射液(60万单位,肌内注射,每月1次)治疗,直至血清25(OH)D≥30 μg·L-1后停用。检测治疗前后血清25(OH)D2、25(OH)D3,T淋巴细胞亚群(包括CD3+、CD4+、CD8+)和CD19+ B细胞、血钙、血磷等水平,并观察不良反应发生情况。

结果

共40例患者完成研究,25(OH)D达标用时(6.2±1.4)个月。与治疗前比较,治疗后25(OH)D2升高(27.42±5.47)μg·L-1,25(OH)D3降低(5.25±3.38)μg·L-1,25(OH)D升高(22.45±6.15)μg·L-1,治疗前后比较均有非常显著差异(P<0.01)。CD4+ T细胞升高(3.71±6.26)%,CD4+/CD8+增高0.31±0.93,CD19+ B细胞升高(1.50±3.31)%,与治疗前比较均有显著差异(P<0.05)。血CD3+ T细胞、丙氨酸转氨酶、血糖、肌酐、血钙、血磷均无显著变化(P>0.05),无严重不良反应发生。

结论

维生素D2注射液可提高80岁以上老年男性维生素D缺乏患者的25(OH)D2水平,并可能调节其免疫功能。

AIM

To evaluate the influences of vitamin D2 injection on serum 25-hydroxyvitmin D (25(OH)D) and immune function in men with vitamin D deficiency aged 80 and over.

METHODS

A total of 48 men with vitamin D deficiency aged 80 and over were given intramuscular vitamin D2 600 000 IU every month until the serum 25(OH)D reached and over 30 µg·L-1. The levels of serum 25(OH)D2, 25(OH)D3, T lymphocyte cells (including CD3+, CD4+, CD8+), B lymphocyte cells (CD19+), and blood calcium and phosphorus were detected before and after treatment. And adverse reactions were observed.

RESULTS

A total of 40 patients completed the study. It took (6.2±1.4) months for 25(OH)D to reach targeted serum concentration. Compared with those before treatment, the serum levels of 25(OH)D2 after the treatment increased by (27.42±5.47) µg·L-1, 25(OH)D3 decreased by (5.25±3.38) µg·L-1, and the total 25(OH)D increased by (22.45±6.15) µg·L-1 (all P<0.01). And the serum levels of CD4+ T cells increased by (3.71±6.26)%, CD4+/CD8+ increased 0.31±0.93, and CD19+ B cell increased by (1.50±3.31)% (all P<0.05). There were no significant differences in serum CD3+ T cells, alanine aminotransferase, blood glucose, creatinine, calcium, and phosphorus before and after the treatment (P>0.05). No severe adverse drug reactions occurred during the treatment.

CONCLUSION

Vitamin D2 injection is helpful for increasing serum 25(OH)D2 levels and regulating immune function in men with vitamin D deficiency aged 80 and over.

老年男性由于皮肤中的7-脱氢胆固醇含量下降、外出活动减少,容易出现维生素D缺乏[1],从而减少钙的吸收,诱发骨质疏松,并会降低机体免疫功能,诱发肺部感染、恶性肿瘤等疾病[2,3]。已有研究表明,维生素D2注射液能提高80岁以上老年男性维生素D缺乏患者的血清25-羟基维生素D[25-hydroxyvitamin D,25(OH)D]水平[4],笔者进一步观察其对患者血清25(OH)D2、25(OH)D3水平和免疫功能的影响,以期为临床使用提供参考。
选择2022年1月来本院就医的男性患者,年龄≥80岁,血清25(OH)D < 20 μg·L-1(1 μg·L-1=2.5 nmol·L-1),能接受维生素D注射液每月1次肌内注射(肌注)和定期回访。排除有自身免疫性疾病、重度感染、恶性肿瘤晚期、慢性肾功能不全(肌酐清除率≤30 mL·min-1)者,以及曾接受过维生素D2注射液或维生素D软胶丸治疗的患者。研究经本院伦理委员会审核通过(2021KT017),患者均知情同意。共纳入患者48例,年龄(89.3±3.0)岁,80~97岁,其中合并原发性高血压38例、慢性阻塞性肺疾病21例、2型糖尿病17例、慢性胃炎11例。
给予维生素D2注射液(规格为每支20万单位,批号为20121401,江西赣南海欣药业股份有限公司)60万单位,肌注,每月1次,连续给药至患者血清25(OH)D≥30 μg·L-1。所有患者在测定血清25(OH)D、血生化和免疫功能后,接受维生素D2注射液肌注。
从2022年1月至2022年9月,观察每位患者肌注维生素D2注射液后,血清25(OH)D、血生化指标、淋巴细胞水平和不良反应。每次注射维生素D2注射液前1日的清晨6:30~7:30抽取空腹肘静脉血。(1) 25(OH)D:采用液相色谱-串联质谱法(以下简称质谱法,美国AB SCIEX 4500MD液相质谱联用仪)检测血清25(OH)D2和25(OH)D3,合计为25(OH)D量。(2)血生化指标:采用日本日立7080全自动生化分析仪测定血丙氨酸转氨酶(ALT)、血糖、肌酐、血钙、血磷水平。(3)淋巴细胞:采用流式细胞术(美国Beckman Coulter Navios流式细胞仪)测量CD3+、CD4+、CD8+ T细胞亚群和CD19+ B细胞,计算CD4+/CD8+。(4)不良反应:观察有无头晕、疲乏、胃纳下降、便秘、腹泻和皮下硬结等发生。
应用SPSS 17.0统计软件进行分析,计量资料以$\bar{x}\pm s$表示,治疗前后比较选用配对样本t检验,P<0.05为差异有显著意义。
至2022年9月观察结束。48例患者中,有7例未按时回访,另有1例因为出现注射处皮下硬结而拒绝继续用药,共40例患者完成研究。
40例患者25(OH)D达标用时(6.2±1.4)个月,4~9个月。其中4个月达标3例、5个月4例、6个月27例、7个月3例、8个月2例、9个月1例,其中27例6个月达标患者每月25(OH)D变化情况见图1
与治疗前相比,40例患者治疗后25(OH)D、25(OH)D2上升,而25(OH)D3下降,差异均有非常显著意义(P<0.01),而ALT、血糖、肌酐、血钙、血磷均无明显变化(P>0.05)。见表1
与治疗前比较,治疗后CD4+ T细胞、CD4+/CD8+和CD19+ B细胞增加,CD8+ T细胞下降,差异均有显著意义(P<0.05),而CD3+ T细胞在治疗前后差异无显著意义(P>0.05)。见表1
48例患者中仅1例出现臀部注射处皮下硬结,经热敷后硬结消失,未发现有头晕、疲乏、胃纳下降等不良反应。
本研究结果表明,在每月肌注维生素D2 60万单位后,患者血清25(OH)D2水平明显增高,这是由于肌肉中维生素D2缓慢吸收,在肝脏25-羟化酶的作用下转变为25(OH)D2后释放入血,从而增高血清25(OH)D2水平[4]。肌注维生素D2后,患者体内25(OH)D3水平明显下降,这是因为维生素D2在肝脏与维生素D3竞争25-羟化酶,从而导致体内维生素D3转化为25(OH)D3减少[5]。此外,本研究采用了质谱法检测,发现80岁以上老年男性血清25(OH)D平均水平从11.68 μg·L-1到达标平均用时6个月,明显低于化学发光免疫法(以下简称免疫法)检测所用时间(平均12个月)[6]。考虑原因为质谱法是通过物质不同的质荷比来区分并检测25(OH)D2、25(OH)D3,然后再合计出25(OH)D的总量,而免疫法是通过抗原抗体反应来间接检测25(OH)D,且对25(OH)D2检出能力不足[5],因而质谱法检测25(OH)D比免疫法更加精准[7]
CD4+ T细胞能辅助CD8+ T细胞靶向杀伤病毒等,CD8+ T细胞增多时,机体易患类风湿关节炎、系统性红斑狼疮等自身免疫病,而CD4+/CD8+降低时,机体易患病毒感染、恶性肿瘤等疾病[8]。本研究中80岁以上老年男性维生素D缺乏患者血清维生素D补充达标后,虽然总T细胞(CD3+)数未见显著增高,但是CD4+ T细胞数、CD4+/CD8+显著增高,CD8+ T细胞数显著降低,说明补充维生素D可能调节患者的细胞免疫功能。此外,维生素D补充达标后,患者B细胞(CD19+)数也出现明显增高,提示维生素D2注射液还可能提高患者的体液免疫功能[9]
本研究发现,80岁以上老年男性维生素D缺乏患者在肌注维生素D2后,血ALT、血糖、肌酐、血钙、血磷无明显变化,且仅1例患者出现注射处皮下硬结,未出现头晕、疲乏、胃纳下降、便秘、腹泻等不适,说明维生素D2注射液肌注较安全。此外,维生素D2注射液是国家基本药物,医保甲类,费用低廉,且使用方便(每月只需注射1次),因而是维生素D缺乏患者的理想治疗药物。
综上所述,维生素D2注射液在提高患者血清25(OH)D2水平的同时,可能改善其机体细胞免疫和体液免疫功能,值得临床推荐使用。另外,患者在接受维生素D2注射液治疗时,建议选择质谱法来检测指标、判断疗效。但本研究样本量较小,且未设立对照组,因而有待进一步扩大样本,并制定更严密的试验设计,来分析维生素D2注射液在80岁以上老年男性维生素D缺乏患者中的疗效。
