AIM

To evaluate the economics of pembrolizumab in the first-line treatment of patients with unresectable or metastatic microsatellite instability-high/mismatch repair-deficient (MSI-H/dMMR) colorectal cancer.

METHODS

Based on the Chinese healthcare system, a partitioned survival model was constructed to compare the cost and utility between pembrolizumab and standard of care using KEYNOTE-177 clinical trial data. The cycle length and time horizon of the model was set as 21 days and 10 years, respectively. The incremental cost-effectiveness ratio (ICER) was used as the evaluation indicator. Scenario analysis and sensitivity analysis were also performed.

RESULTS

Compared with standard of care, the ICER of pembrolizumab was 221 546.85 yuan·QALY-1, which was lower than 3 times China’s per capita gross domestic product (GDP) in 2021. The pembrolizumab was dominant when patient assistance program was considered. The results of one-way sensitivity analysis showed that the proportion of pembrolizumab on subsequent treatment in the standard of care, pembrolizumab price and discounting exhibited the significant impact on the ICER. The results of probability sensitivity analysis showed that under the 3 times China’s per capita GDP in 2021, the probability of pembrolizumab being cost-effectiveness was 63.36%.

CONCLUSION

Pembrolizumab has a cost-effective advantage over standard of care as first-line treatment for unresectable or metastatic MSI-H/dMMR colorectal cancer in China.

目的

评价帕博利珠单抗用于不可切除或转移性微卫星高度不稳定/错配修复缺陷(MSI-H/dMMR)结直肠癌患者一线治疗的经济性。

方法

基于中国卫生体系角度,利用KEYNOTE-177临床试验数据,构建分区生存模型评价帕博利珠单抗对比标准治疗的成本与效用,模型周期与研究时限分别设置为21 d与10年。以增量成本效果比(ICER)为评价指标,并对关键参数进行情景分析和敏感性分析。

结果

与标准治疗相比,帕博利珠单抗的ICER为221 546.85元·QALY-1,低于2021年3倍中国人均国内生产总值(GDP)。若有患者援助计划,帕博利珠单抗为绝对优势。单因素敏感性分析表明,对ICER影响最大的3个参数分别为标准治疗后续治疗中帕博利珠单抗比例、帕博利珠单抗价格和贴现率。概率敏感性分析显示,在2021年3倍中国人均GDP下,帕博利珠单抗具有经济性的概率为63.36%。

结论

相比标准治疗,帕博利珠单抗在不可切除或转移性MSI-H/dMMR结直肠癌的一线治疗中具有经济学优势。

目的

评价帕博利珠单抗用于不可切除或转移性微卫星高度不稳定/错配修复缺陷(MSI-H/dMMR)结直肠癌患者一线治疗的经济性。

方法

基于中国卫生体系角度,利用KEYNOTE-177临床试验数据,构建分区生存模型评价帕博利珠单抗对比标准治疗的成本与效用,模型周期与研究时限分别设置为21 d与10年。以增量成本效果比(ICER)为评价指标,并对关键参数进行情景分析和敏感性分析。

结果

与标准治疗相比,帕博利珠单抗的ICER为221 546.85元·QALY-1,低于2021年3倍中国人均国内生产总值(GDP)。若有患者援助计划,帕博利珠单抗为绝对优势。单因素敏感性分析表明,对ICER影响最大的3个参数分别为标准治疗后续治疗中帕博利珠单抗比例、帕博利珠单抗价格和贴现率。概率敏感性分析显示,在2021年3倍中国人均GDP下,帕博利珠单抗具有经济性的概率为63.36%。

结论

相比标准治疗,帕博利珠单抗在不可切除或转移性MSI-H/dMMR结直肠癌的一线治疗中具有经济学优势。

AIM

To evaluate the economics of pembrolizumab in the first-line treatment of patients with unresectable or metastatic microsatellite instability-high/mismatch repair-deficient (MSI-H/dMMR) colorectal cancer.

METHODS

Based on the Chinese healthcare system, a partitioned survival model was constructed to compare the cost and utility between pembrolizumab and standard of care using KEYNOTE-177 clinical trial data. The cycle length and time horizon of the model was set as 21 days and 10 years, respectively. The incremental cost-effectiveness ratio (ICER) was used as the evaluation indicator. Scenario analysis and sensitivity analysis were also performed.

RESULTS

Compared with standard of care, the ICER of pembrolizumab was 221 546.85 yuan·QALY-1, which was lower than 3 times China’s per capita gross domestic product (GDP) in 2021. The pembrolizumab was dominant when patient assistance program was considered. The results of one-way sensitivity analysis showed that the proportion of pembrolizumab on subsequent treatment in the standard of care, pembrolizumab price and discounting exhibited the significant impact on the ICER. The results of probability sensitivity analysis showed that under the 3 times China’s per capita GDP in 2021, the probability of pembrolizumab being cost-effectiveness was 63.36%.

CONCLUSION

Pembrolizumab has a cost-effective advantage over standard of care as first-line treatment for unresectable or metastatic MSI-H/dMMR colorectal cancer in China.

2020年全球癌症统计数据显示,结直肠癌(colorectal cancer,CRC)新发病例超过190万例,死亡病例超过93万例,是第三大常见癌症和第二大癌症死亡原因[1]。DNA错配修复缺陷(mismatch repair-deficient,dMMR)可导致微卫星高度不稳定(microsatellite instability-high,MSI-H),其在转移性CRC(mCRC)患者中发生的比例为4%~5%[2,3]。与错配修复功能完整的mCRC患者相比,MSI-H/dMMR mCRC患者从单用化疗或化疗联用单克隆抗体(单抗)方案中获益更少,即无进展生存期(PFS)和总生存期(OS)更短[2]
近年来,肿瘤免疫疗法兴起,备受关注。由于MSI-H/dMMR mCRC有高肿瘤突变负荷、高水平肿瘤新抗原及增加的免疫细胞浸润等生物学特性,因此其更适合使用免疫检查点抑制剂治疗[3]。帕博利珠单抗(pembrolizumab)是一种可与程序性死亡受体1(PD-1)结合的人源化单抗,于2018年在我国上市。2021年6月,国家药品监督管理局基于Ⅲ期临床试验(KEYNOTE-177)[4]的研究结果,批准帕博利珠单抗用于MSI-H/dMMR mCRC患者的一线治疗,这也是其在国内批准的第7个适应证。2022年4月,KEYNOTE-177的长期临床试验数据显示,与标准治疗相比,帕博利珠单抗可显著延长MSI-H/dMMR mCRC患者的PFS [8.2个月vs. 16.5个月,风险比(HR)=0.59,95%置信区间(CI):0.45~0.79][5]。中国临床肿瘤学会结直肠癌诊疗指南[6]在MSI-H/dMMR mCRC患者的ⅠA类一线治疗方案中推荐使用帕博利珠单抗。
鉴于国内缺乏相关经济学证据支持,本研究基于我国卫生体系角度,利用KEYNOTE-177临床试验[5]最新结果,评价帕博利珠单抗对比标准方案一线治疗1随机分配至帕博利珠单抗组(n=153)或标准治疗组(n=154)。帕博利珠单抗组每3周接受一次帕博利珠单抗200 mg静脉输注。标准治疗组根据研究人员选择接受化疗或化疗联合贝伐珠单抗和西妥昔单抗治疗,静脉给药,每2周一次。标准治疗组用药方案包括:(1)mFOLFOX6:第1日使用奥沙利铂85 mg·m-2+亚叶酸钙400 mg·m-2+氟尿嘧啶400 mg·m-2静脉推注,随后每日予氟尿嘧啶1 200 mg·m-2,持续2 d;(2)mFOLFOX6+贝伐珠单抗:在mFOLFOX6的基础上,第1日予贝伐珠单抗5 mg·kg-1;(3)mFOLFOX6+西妥昔单抗:在mFOLFOX6的基础上,第1周静脉输注西妥昔单抗400 mg·m-2,后每周250 mg·m-2;(4)FOLFIRI:第1日使用伊立替康180 mg·m-2+亚叶酸钙400 mg·m-2+氟尿嘧啶400 mg·m-2静脉推注,随后每日予氟尿嘧MSI-H/dMMR mCRC患者的成本效果,以期为国家医保药品目录动态调整、临床合理用药和精准医疗决策提供参考依据。
数据来源于KEYNOTE-177试验[5],该研究为一项随机、开放、阳性对照的Ⅲ期临床试验,在全球23个国家或地区的193个学术医学中心和医院开展。纳入标准:患者年龄≥18岁,通过影像学评估证实为MSI-H/dMMRⅣ期CRC;根据实体瘤疗效评价标准(RECIST 1.1)有可测量病变;东部肿瘤协作组体能状态评分为0或1分;且血常规及肝肾功能检查正常[5]。排除标准:既往接受过系统治疗;在过去2年内患有需要系统治疗的自身免疫疾病;在随机分组前至少7 d被诊断出患有免疫缺陷、正在接受系统类固醇治疗或其他免疫抑制治疗[5]。共纳入患者307例,按1:啶1 200 mg·m-2,持续2日;(5)FOLFIRI+贝伐珠单抗:在FOLFIRI的基础上,第1日予贝伐珠单抗5 mg·kg-1;(6)FOLFIRI+西妥昔单抗:在FOLFIRI的基础上,第1周静脉输注西妥昔单抗400 mg·m-2,后每周250 mg·m-2。所有患者治疗直至疾病进展(PD)、出现无法耐受的不良反应或完成帕博利珠单抗35个疗程治疗为止。患者出现PD后,帕博利珠单抗组和标准治疗组接受后续治疗的比例分别为34.0%和78.6%[5]。其中,帕博利珠单抗组可继续接受帕博利珠单抗17个疗程治疗;标准治疗组可交叉接受帕博利珠单抗35个疗程治疗,且停药后PD的患者可再次接受17个疗程[5]。本研究假设除接受帕博利珠单抗治疗的患者外,其余患者均接受最佳支持治疗。
采用Microsoft Excel 2019构建分区生存模型,从中国卫生体系角度评价帕博利珠单抗组对比标准治疗组长期的成本效果,按照患者的发展过程将模型划分为3种健康状态:PFS、PD和死亡(death),见图1。患者由PFS状态进入模型,依据疾病转归和进展发生状态转移,每个周期患者只能处于1种健康状态并接受相应健康状态下的治疗方案。根据帕博利珠单抗的用药规律将模型的循环周期设定为21 d,模拟患者10年的治疗成本和健康产出。模型产出包括总成本、质量调整生命年(QALYs)和增量成本效果比(ICER)。根据《中国药物经济学评价指南(2020)》,成本和健康产出均选用5%进行贴现,将我国2021年3倍人均国内生产总值(GDP,242 928元·QALY-1)设定为意愿支付阈值(WTP)[7]
不同健康状态下的患者比例从KEYNOTE-177试验[5]报道的生存曲线中获取,模拟时限超过随访期时,采用参数生存模型外推生存曲线。先采用GetData Graph Digitizer 2.26软件(http://www.getdata-graph-digitizer.com/download.php)从KEYNOTE-177试验报道的Kaplan-Meier(K-M)曲线上取点。后通过Stata 15.1软件拟合重构患者的个体数据,并对生存资料进行贝叶斯参数生存模型分析,采用5种参数生存模型对生存数据进行拟合外推:exponential分布、weibull分布、log-logistic分布、log-normal分布和Gompertz分布[8]。最后,采用赤池信息准则、贝叶斯信息准则并结合视觉检查进行拟合优度检验,判断最优拟合分布,计算生存函数[9]。对于长拖尾分布,采用更为稳健的weibull分布拟合。各组最优拟合分布及其参数见表1
由于本研究基于中国卫生体系角度开展药物经济学评价,因此仅纳入治疗方案相关的直接医疗成本,包括药品费用、检查费用、最佳支持治疗费用以及不良事件相关的管理费用。成本参数主要来源于药智网(https://www.yaozh.com/)和已发表的研究[10-14]。本研究仅考虑3~5级且发生率> 3%的不良事件相关的管理费用,其中,不良事件发生率来源于KEYNOTE-177临床试验,包括:腹泻、疲劳、贫血、口腔炎、白细胞减少和中性粒细胞减少[5]。基于我国2020年发布的男女平均身高和体重[15]及KEYNOTE-177临床试验的男女患者比例[5],设定患者的平均身高与平均体重分别为163.83 cm和64.28 kg,因此患者平均体表面积为1.79 m2(体表面积=0.006 1×身高+0.012 4×体重-0.009 9)[16],以此计算标准治疗组药品用量与药品费用。
本研究的健康效用值来源于英国国家卫生与临床优化研究所网站提交的帕博利珠单抗经济学评价报告。该报告基于KEYNOTE-177试验获得相关数据,试验中采用EQ-5D-3L量表测量MSI-H/dMMR mCRC患者健康相关生命质量,并基于英国健康效用积分体系计算健康效用值。帕博利珠单抗组和标准治疗组PFS状态的健康效用值分别为0.852和0.800,2组PD状态的健康效用值均为0.730[17]。成本和健康效用值数据具体见表1
考虑帕博利珠单抗的患者援助计划。为提高帕博利珠单抗的可负担性,“生命之钥-肿瘤免疫治疗患者援助项目”推出赠药政策,用于KRASNRASBRAF基因均为野生型的MSI-H/dMMR mCRC患者,即:患者自费2个疗程的帕博利珠单抗治疗,经基金会审核通过后,可为其援助2个疗程;后续患者继续使用2个疗程的帕博利珠单抗,基金会可继续为其提供援助直至PD,但患者累计帕博利珠单抗用量最多不超过24个月[18]
本研究采用单因素敏感性分析(one-way sensitivity analysis,OWSA)和概率敏感性分析(probabilistic sensitivity analysis,PSA),以评估参数变化对结果稳定性的影响。在OWSA中,采用各参数的最大值和最小值为上下限分别计算ICER以确定对模型具有较大影响的参数。将95%CI或者基础分析值上下浮动20%作为模型参数的上下限范围,根据《中国药物经济学评价指南(2020)》,将0~8%设定为贴现率的浮动范围[7]。OWSA结果通过龙卷风图呈现。在PSA中,根据参数的取值范围和概率分布形式执行10 000次Monte Carlo模拟,成本数据服从Gamma分布,不良事件发生率、健康效用值、标准治疗组用药比例、后续治疗用药比例以及贴现率数据均服从Beta分布。PSA结果通过成本效果散点图与成本效果可接受曲线呈现。
在10年的模拟时限内,与标准治疗组相比,帕博利珠单抗组的增量成本为165 682.53元,增量QALYs为0.75 QALYs,ICER为221 546.85元·QALY-1,低于WTP阈值。见表2
当考虑患者援助计划时,2年(35个周期)内,每个周期仅需花费约4 120元,帕博利珠单抗组在增加QALYs的基础上可节省成本469 280.67元,为绝对优势方案。
OWSA结果显示,由于ICER仅低于WTP 21 381.15元·QALY-1,对研究结果影响最大的模型参数是标准治疗组后续治疗中帕博利珠单抗的比例,其取最小值时,会改变经济性结果。其次是帕博利珠单抗价格与贴现率,其取最大值时,会改变经济性结果。其他模型参数对研究结果的影响较小。见图2
散点均分布于第一象限内,说明帕博利珠单抗组给患者带来更多健康产出的同时会增加成本支出,成本效果散点图见图3
当WTP阈值为3倍中国人均GDP时,帕博利珠单抗组具有经济性的概率为63.36%,成本效果可接受曲线见图4
MSI-H/dMMR mCRC患者治疗有限,免疫检查点抑制剂的出现为患者提供了新的治疗选择,帕博利珠单抗是中国首个CRC一线免疫治疗药物。KEYNOTE-177临床试验证实帕博利珠单抗能够给患者带来显著的临床获益,但考虑到其价格昂贵,因此本研究基于我国卫生体系角度构建分区生存模型,评价帕博利珠单抗用于MSI-H/dMMR mCRC一线治疗的成本效果。本研究结果显示,与标准治疗相比,帕博利珠单抗可获得更高的健康产出,但同时增加成本支出,ICER为221 546.85元·QALY-1。若有患者援助计划,患者治疗两年需14万元,帕博利珠单抗成为绝对优势方案。本研究OWSA结果显示,由于ICER略低于WTP阈值,标准治疗组后续治疗中帕博利珠单抗的比例、帕博利珠单抗价格和贴现率一定程度上会影响模型分析结果的稳定性。PSA结果显示,在当前价格与WTP之下,帕博利珠单抗组具有经济性的概率为63.36%。
帕博利珠单抗用于MSI-H/dMMR mCRC患者的经济性在美国已得到证实。AGUIAR-IBANEZ等[19]从美国卫生体系角度构建分区生存模型评估帕博利珠单抗用于一线治疗的经济性,结果显示,帕博利珠单抗相比标准治疗的ICER为6 984美元·QALY-1,远低于WTP阈值(100 000美元·QALY-1)。若从全社会角度出发,帕博利珠单抗为绝对优势方案。美国国家综合癌症网络指南[20]推荐帕博利珠单抗用于MSI-H/dMMR晚期CRC的一线和二线治疗,TAN等[21]基于美国支付方角度,评估帕博利珠单抗分别用于一线或二线治疗相比标准治疗的成本效果,结果显示,一线用帕博利珠单抗相比二线用帕博利珠单抗为绝对优势,相比标准治疗的ICER为13 441美元·QALY-1,远低于WTP阈值(150 000美元·QALY-1)。本研究在中国背景下评估帕博利珠单抗一线治疗MSI-H/dMMR晚期CRC的成本效果,研究结论与国外两项研究相似,可为未来医保循证决策经济学证据,为临床合理用药提供参考依据。
本研究存在一定的局限性:(1)KEYNOTE-177试验是一项全球多中心临床试验,人群可能存在潜在差异,后续仍需采用国内大样本临床试验或真实世界研究数据进一步验证。且由于该试验未报道原始生存数据,因此本研究采用参数生存模型进行拟合外推,可能会导致偏倚。(2)本研究仅纳入3~5级且发生率>3%的不良事件相关管理费用,在一定程度上会低估治疗方案的成本,但OWSA结果显示不良事件发生率与管理费用对基础分析结果影响均较小。(3)由于KEYNOTE-177临床试验与国内诊疗指南未明确其他后续治疗方案,因此本研究假设除帕博利珠单抗外,其余均采用最佳支持治疗,与临床试验和真实用药可能存在偏差。(4)帕博利珠单抗的患者援助计划用于KRASNRASBRAF基因均为野生型的MSI-H/dMMR mCRC患者。由于临床试验未报告该亚组数据,因此本研究假设KRASNRASBRAF基因均为野生型的患者与总人群的临床效果相似,情景分析结果需谨慎解释。
综上所述,基于中国卫生体系角度,在当前价格和WTP之下,相比标准方案,帕博利珠单抗一线治疗MSI-H/dMMR mCRC具有经济学优势。
