AIM

To explore the regulation effect of hyperoside on renal ischemia-reperfusion(IR)injury and nuclear factor E2 related factor 2(Nrf2)/heme oxygenase 1(HO-1)/quinone oxidoreductase 1(NQO1)signaling pathway in rats.

METHODS

The models of renal IR rat were constructed by ligating bilateral renal arteries(60 minutes)and recovering perfusion(120 minutes). Forty SD rats were randomly divided into control group, model group and hyperoside 12.5, 25, 50 mg·kg-1 groups. The samples of blood and renal tissues were collected to detect levels of 24 h proteinuria,serum creatinine, blood urea nitrogen and superoxide dismutase(SOD), malondialdehyde(MDA). The renal tissue lesions were observed by HE staining. The expression of caspase-3 was detected by immunohistochemistry. The expression of inducible nitric oxide synthase(iNOS), interleukin(IL)-6 and IL-10 mRNA was detected by qRT-PCR. The expression of Bcl-2, Bax, Nrf2, HO-1 and NQO1 proteins in renal tissues was detected by Western blot. A total of 32 rats were randomly and averagely divided into control group, model group, hyperoside(50 mg·kg-1)group and hyperoside+ML385(30 mg·kg-1)group for relevant detection.

RESULTS

Compared with the model group, the levels of 24 h proteinuria, serum creatinine and blood urea nitrogen, and MDA were significantly decreased, while the levels of SOD were significantly increased in the hyperoside 25 and 50 mg·kg-1 groups(P<0.05). HE staining showed that there was obvious pathological injury of renal tissues and inflammatory cells infiltration was severe in the model group. The pathological injury were relieved in the hyperoside groups, and the improvement were more significant in the hyperoside 50 mg·kg-1 group. Compared with the model group, the expression of Bax/Bcl-2, caspase-3, iNOS and IL-6 mRNA were decreased significantly, while the expression of IL-10 mRNA, p-Nrf2/Nrf2, HO-1 and NQO1 and SOD were significantly upregulated in the hyperoside 25 and 50 mg·kg-1 groups(P<0.05). The expression of Bax/Bcl-2 decreased significantly in the hyperoside 12.5 mg·kg-1 group(P<0.05). ML385 can partially block the improvement effect of hyperoside on renal IR rats.

CONCLUSION

Hyperoside may inhibit apoptosis of renal cells, excessive oxidative stress and inflammatory response by up-regulating Nrf2/HO-1/NQO1 signaling pathways, and relieve renal IR injury.

目的

探究金丝桃苷对大鼠肾缺血再灌注(IR)损伤及核因子E2相关因子2(Nrf2)/血红素加氧酶-1(HO-1)/醌氧化还原酶1(NQO1)信号通路的调节。

方法

以结扎双肾动脉60 min再恢复灌注120 min构建肾IR大鼠模型,将40只SD大鼠随机均分为对照组,模型组,金丝桃苷12.5、25、50 mg·kg-1组,留取大鼠血液和肾脏组织标本,检测24 h蛋白尿、血肌酐及尿素氮、超氧化物歧化酶(SOD)和丙二醛(MDA)水平,HE染色观察肾脏组织病变,免疫组化法检测caspase-3表达,qRT-PCR法检测诱导型一氧化氮合酶(iNOS)、白细胞介素(IL)-6、IL-10 mRNA表达,Western blot法检测肾组织中Bcl-2、Bax、Nrf2、HO-1、NQO1蛋白表达。另取32只大鼠随机均分为对照组、模型组、金丝桃苷50 mg·kg-1组和金丝桃苷+ML385(30 mg·kg-1)组,进行上述检测。

结果

与模型组比较,金丝桃苷25、50 mg·kg-1组24 h蛋白尿、血肌酐、尿素氮、MDA水平均显著下降,SOD水平显著升高(P<0.05)。模型组肾组织出现明显病理损伤,炎症细胞浸润严重,金丝桃苷组肾组织病理损伤均减轻,50 mg·kg-1组改善更为明显。与模型组比较,金丝桃苷25、50 mg·kg-1组Bax/Bcl-2、caspase-3蛋白表达和iNOSIL-6 mRNA表达显著下降,IL-10 mRNA和p-Nrf2/Nrf2、HO-1及NQO1蛋白表达显著上调(P<0.05),金丝桃苷12.5 mg·kg-1组Bax/Bcl-2表达显著下降(P<0.05)。ML385可部分阻断金丝桃苷对肾IR大鼠的改善作用。

结论

金丝桃苷可能通过上调Nrf2/HO-1/NQO1信号通路,抑制肾脏细胞的凋亡、过度的氧化应激和炎症反应,减轻肾脏IR损伤。

目的

探究金丝桃苷对大鼠肾缺血再灌注(IR)损伤及核因子E2相关因子2(Nrf2)/血红素加氧酶-1(HO-1)/醌氧化还原酶1(NQO1)信号通路的调节。

方法

以结扎双肾动脉60 min再恢复灌注120 min构建肾IR大鼠模型,将40只SD大鼠随机均分为对照组,模型组,金丝桃苷12.5、25、50 mg·kg-1组,留取大鼠血液和肾脏组织标本,检测24 h蛋白尿、血肌酐及尿素氮、超氧化物歧化酶(SOD)和丙二醛(MDA)水平,HE染色观察肾脏组织病变,免疫组化法检测caspase-3表达,qRT-PCR法检测诱导型一氧化氮合酶(iNOS)、白细胞介素(IL)-6、IL-10 mRNA表达,Western blot法检测肾组织中Bcl-2、Bax、Nrf2、HO-1、NQO1蛋白表达。另取32只大鼠随机均分为对照组、模型组、金丝桃苷50 mg·kg-1组和金丝桃苷+ML385(30 mg·kg-1)组,进行上述检测。

结果

与模型组比较,金丝桃苷25、50 mg·kg-1组24 h蛋白尿、血肌酐、尿素氮、MDA水平均显著下降,SOD水平显著升高(P<0.05)。模型组肾组织出现明显病理损伤,炎症细胞浸润严重,金丝桃苷组肾组织病理损伤均减轻,50 mg·kg-1组改善更为明显。与模型组比较,金丝桃苷25、50 mg·kg-1组Bax/Bcl-2、caspase-3蛋白表达和iNOSIL-6 mRNA表达显著下降,IL-10 mRNA和p-Nrf2/Nrf2、HO-1及NQO1蛋白表达显著上调(P<0.05),金丝桃苷12.5 mg·kg-1组Bax/Bcl-2表达显著下降(P<0.05)。ML385可部分阻断金丝桃苷对肾IR大鼠的改善作用。

结论

金丝桃苷可能通过上调Nrf2/HO-1/NQO1信号通路,抑制肾脏细胞的凋亡、过度的氧化应激和炎症反应,减轻肾脏IR损伤。

AIM

To explore the regulation effect of hyperoside on renal ischemia-reperfusion(IR)injury and nuclear factor E2 related factor 2(Nrf2)/heme oxygenase 1(HO-1)/quinone oxidoreductase 1(NQO1)signaling pathway in rats.

METHODS

The models of renal IR rat were constructed by ligating bilateral renal arteries(60 minutes)and recovering perfusion(120 minutes). Forty SD rats were randomly divided into control group, model group and hyperoside 12.5, 25, 50 mg·kg-1 groups. The samples of blood and renal tissues were collected to detect levels of 24 h proteinuria,serum creatinine, blood urea nitrogen and superoxide dismutase(SOD), malondialdehyde(MDA). The renal tissue lesions were observed by HE staining. The expression of caspase-3 was detected by immunohistochemistry. The expression of inducible nitric oxide synthase(iNOS), interleukin(IL)-6 and IL-10 mRNA was detected by qRT-PCR. The expression of Bcl-2, Bax, Nrf2, HO-1 and NQO1 proteins in renal tissues was detected by Western blot. A total of 32 rats were randomly and averagely divided into control group, model group, hyperoside(50 mg·kg-1)group and hyperoside+ML385(30 mg·kg-1)group for relevant detection.

RESULTS

Compared with the model group, the levels of 24 h proteinuria, serum creatinine and blood urea nitrogen, and MDA were significantly decreased, while the levels of SOD were significantly increased in the hyperoside 25 and 50 mg·kg-1 groups(P<0.05). HE staining showed that there was obvious pathological injury of renal tissues and inflammatory cells infiltration was severe in the model group. The pathological injury were relieved in the hyperoside groups, and the improvement were more significant in the hyperoside 50 mg·kg-1 group. Compared with the model group, the expression of Bax/Bcl-2, caspase-3, iNOS and IL-6 mRNA were decreased significantly, while the expression of IL-10 mRNA, p-Nrf2/Nrf2, HO-1 and NQO1 and SOD were significantly upregulated in the hyperoside 25 and 50 mg·kg-1 groups(P<0.05). The expression of Bax/Bcl-2 decreased significantly in the hyperoside 12.5 mg·kg-1 group(P<0.05). ML385 can partially block the improvement effect of hyperoside on renal IR rats.

CONCLUSION

Hyperoside may inhibit apoptosis of renal cells, excessive oxidative stress and inflammatory response by up-regulating Nrf2/HO-1/NQO1 signaling pathways, and relieve renal IR injury.

肾缺血再灌注(ischemia-reperfusion,IR)损伤是指肾脏组织在缺血后恢复血流灌注过程中,缺血损伤无法恢复至正常状态,甚至恶化的病理表现[1]。肾IR损伤多继发于肾脏手术、心肺复苏、休克等,是引起急性肾损伤的重要因素[2]。金丝桃苷属于黄酮醇苷类化合物,具有多种生理活性,如抗炎、利尿、局部镇痛等[3]。研究证实,金丝桃苷可介导多种信号途径,在心脑IR损伤中起保护作用,如磷脂酰肌醇-3激酶/蛋白激酶B信号通路、一氧化氮信号通路、氧化应激信号通路等[4]。WU等[5]研究表明,金丝桃苷可改善肾IR损伤,可能与调节线粒体裂变、氧化应激和凋亡有关。KWON等[6]研究发现,金丝桃苷可激活核因子E2相关因子2(Nrf2)/血红素加氧酶-1(HO-1)信号途径,抑制6-羟基多巴胺诱导的氧化应激。经典氧化应激通路Nrf2/HO-1/醌氧化还原酶1(NQO1)在肾IR损伤中有重要作用[6],本研究通过建立肾IR损伤大鼠模型,观察金丝桃苷对肾IR损伤的影响及与Nrf2/HO-1/NQO1通路相关的分子机制。
金丝桃苷(上海源叶生物科技有限公司,规格为100 mg,批号为190601),Nrf2抑制剂ML385(上海泽叶生物科技有限公司,纯度为99.59%,批号为191025),肌酐、尿素氮、超氧化物歧化酶(SOD)和丙二醛(MDA)试剂盒(上海碧云天生物技术有限公司),引物合成(北京赛百盛基因技术有限公司),兔Bcl-2、Bax、Nrf2、HO-1、NQO1、内参Actin蛋白抗体(美国Abcam公司),免疫组化试剂盒、辣根过氧化物酶标记山羊抗兔IgG蛋白二抗、ECL发光剂(武汉博士德生物工程有限公司)。垂直电泳仪(北京六一仪器厂),多功能酶标仪、PCR仪(美国Bio-Rad)。
健康SD大鼠,雄性,6~8周龄,体重180~200 g,由川北医学院提供,动物许可证号:SCXK(川)2018-18。
将40只SD大鼠随机分为对照组,模型组,金丝桃苷12.5、25、50 mg·kg-1组,每组8只。将大鼠固定于操作台,腹腔注射戊巴比妥(60 mg·kg-1)麻醉,腹部备皮后消毒,于剑突下1.5 cm处切口打开腹腔,小心分离肾动脉并夹闭,夹闭60 min后松开,恢复血流再灌注120 min,充分夹闭后肾脏呈苍白色,表明缺血成功,恢复灌注后肾脏呈鲜红色,表明再灌注成功[5]。对照组大鼠仅分离肾动脉,但不作结扎处理,其余组均建立肾IR损伤模型。金丝桃苷组于造模前7 d分别灌胃给予12.5、25、50 mg·kg-1金丝桃苷,每日1次,连续给药7 d,对照组和模型组灌胃给予同等容积的生理盐水。另取32只大鼠随机为对照组、模型组、金丝桃苷(50 mg·kg-1)组和金丝桃苷+ML385(灌胃金丝桃苷50 mg·kg-1后30 min腹腔注射Nrf2抑制剂ML385 30 mg·kg-1)组。
实验结束后,采集大鼠24 h尿液检测尿蛋白,另取血液和肾脏组织标本用于后续病理和分子生物学检测。
取血液样本2 mL,室温静置60 min后,1 500×g离心10 min,参照试剂盒说明书操作,检测血肌酐、尿素氮、SOD和MDA水平。留取24 h尿液,应用全自动生化分析仪(焦酚红/钼酸盐法)检测尿蛋白含量。
取新鲜的肾脏组织制成石蜡切片,行常规HE染色,于400倍视野下观察大鼠肾组织病理变化。免疫组化染色按试剂盒说明书操作,caspase-3一抗稀释比为1:200,黄色至棕褐色指示caspase-3阳性表达,计算各视野下阳性面积占比。
采用qRT-PCR法。iNOS上游引物:5’-CAGCTGGGCTGTACAAACCTT-3’,下游引物:5’-CATTGGAAGTGAAGCGTTTCG-3’;IL-6上游引物:5’-AGCGATGATGCACTGTCAGA-3’,下游引物:5’-GG AACTCCAGAAGACGAGAGC-3’;IL-10上游引物:5’-TC AGGGTGGCGACTCTAT-3’,下游引物:5’-TGGGCTTCT TTCTAAATCGTTC-3’;内参GADPH上游引物:5’-GG TGAAGGTCGGTGTGAACG-3’,下游引物:5’-CTCG CTCCTGGAAGATGGTG-3’[78]。取适量肾脏组织提取总RNA,反转录合成cDNA链,应用PCR仪扩增,反应参数:95 ℃ 60 s、95 ℃ 30 s、60 ℃ 40 s、72 ℃ 2 min,共35个循环。采用2-ΔΔCT法,对目的基因进行相对定量。
采用Western blot法检测。取适量肾脏组织提取总蛋白,检测蛋白浓度,取等量蛋白进行10% SDS-PAGE凝胶电泳,电转至硝酸纤维素膜上,室温下于脱脂牛奶中封闭2 h,加入相应的蛋白一抗,4 ℃下孵育过夜,再加入相应的HRP标记蛋白二抗,室温下孵育1 h,最后加入ECL发光剂进行显色,应用凝胶成像系统扫描各蛋白条带的灰度值,以Actin为内参,定量分析Bcl-2、Bax、Nrf2、HO-1及NQO1蛋白相对表达量。
采用统计学软件SPSS 18.0分析处理,数据以表示,组间比较采用单因素方差分析,多重比较采用LSD-t检验,以P<0.05为差异有显著意义。
与对照组比较,模型组24 h蛋白尿、血肌酐、尿素氮、MDA水平升高,SOD水平下降(P<0.01);与模型组比较,金丝桃苷25、50 mg·kg-1组24 h蛋白尿、血肌酐、尿素氮、MDA水平显著下降,SOD水平显著升高(P<0.05),见表1
HE染色显示,模型组肾小管上皮细胞大量坏死,暴露基底膜,炎症细胞浸润严重。金丝桃苷各剂量组上述病理改变均有所减轻,高剂量改善更显著。见图1
与对照组比较,模型组Bax/Bcl-2、caspase-3阳性表达率以及iNOSIL-6IL-10 mRNA表达水平均显著升高(P<0.01)。与模型组比较,金丝桃苷25、50 mg·kg-1组Bax/Bcl-2、caspase-3阳性表达率以及iNOSIL-6 mRNA显著下降(P<0.01),IL-10 mRNA显著升高(P<0.05);金丝桃苷12.5 mg·kg-1组Bax/Bcl-2表达显著下降(P<0.05)。见图2图3表2
与对照组比较,模型组p-Nrf2/Nrf2、HO-1及NQO1表达显著下调(P<0.01)。与模型组比较,金丝桃苷25、50 mg·kg-1组p-Nrf2/Nrf2、HO-1及NQO1表达显著上调(P<0.01)。见图4表3
与金丝桃苷组比较,金丝桃苷+ML385组肾组织病理损伤加重,SOD水平和p-Nrf2/Nrf2、HO-1、NQO1蛋白表达及IL-10 mRNA表达水平显著下降(P<0.05),MDA水平、Bax/Bcl-2和IL-6 mRNA水平显著升高(P<0.05)。见图5图6表4
肾脏是IR损伤容易危及的器官之一。肾IR损伤的发生机制尚不完全清楚,但细胞凋亡、过度激活的氧化反应和炎症反应是其主要的病理机制[9]。细胞凋亡主要表现为促凋亡蛋白Bax、凋亡执行蛋白caspase-3上调,抗凋亡蛋白Bcl-2的下调。氧化应激涉及SOD活性降低,氧化应激因子iNOS和代谢终产物MDA的增加。促炎性细胞因子IL-6的增加是炎症反应的主要表现,IL-10属于多功能细胞因子,随着炎性损伤的进展,具有抗自由基、免疫抑制、抗炎及抗凋亡等作用[1011]。本研究中大鼠肾IR损伤后肾脏组织出现明显的病理变化,金丝桃苷各剂量组肾组织病理损伤明显减轻,24 h蛋白尿、血肌酐和尿素氮明显降低,同时伴随着凋亡、氧化应激及炎症反应的下调,提示金丝桃苷能有效减轻肾IR损伤。
正常生理状态下,体内存在氧化-抗氧化机制,维持动态平衡状态。Nrf2是体内抗氧化反应的关键转录因子,当体内触发氧化应激时,Nrf2可与Kelch样环氧氯丙烷相关蛋白1解联,激活其下游HO-1、NQO1等多种抗氧化基因的表达,启动抗氧化,快速清除活性氧,从而减轻氧化损伤[12]。LI等[13]在肾IR损伤中证实,Nrf2/HO-1/NQO1信号通路激活可调控SOD/MDA变化,抑制caspase-3的表达,从而减轻凋亡与氧化应激反应。本研究结果发现,肾脏IR能抑制Nrf2/HO-1/NQO1信号通路,给予金丝桃苷后,Nrf2磷酸化、HO-1、NQO1表达明显上调,而且给予Nrf2抑制剂ML385后可减弱金丝桃苷对肾IR大鼠的改善作用,表明金丝桃苷可能通过激活Nrf2/HO-1/NQO1信号通路,在肾IR中发挥保护作用。但金丝桃苷是否还依赖其他分子途径,还需要进一步研究。
综上所述,金丝桃苷可有效减轻肾IR损伤,其机制可能是通过上调Nrf2/HO-1/NQO1信号通路,抑制肾脏细胞的凋亡、氧化应激和炎症反应。
