AIM

To visually analyze the research hotspots and trends of Sparganii Rhizoma in the past 10 years based on the knowledge mapping method to provide references for future research in this field.

METHODS

The research on Sparganii Rhizoma from January 1st, 2012 to January 1st, 2022 were selected from CNKI, and the sample literature included in the study was analyzed using CiteSpace software, and the annual publication volume, author collaboration network,institutional collaboration network, keyword co-occurrences, keyword clustering, and keyword emergent mapping were drawn.

RESULTS

A total of 592 papers were included, and the annual publication volume was relatively stable. The team represented by SHI HY, HOU RJ and LIANG QL was formed, and there was less cooperation among institutions. The keywords with higher frequency include data mining, medication law, Sparganii Rhizoma, etc., forming 10 clusters. The research hotspots mainly focus on gynecological diseases and tumors, clinical research, mechanism of action, etc. The keyword emergence analysis obtained a total of 43 emergent words, with more research on data mining, clinical research, and treatment of tumors and gynecological diseases, and the laboratory research pays more attention to the extraction of the active ingredient and its active ingredients and the mechanism of action. Laboratory studies paid more attention to the extraction of active ingredients and their active components.

CONCLUSION

In recent years, the research hotspots of Sparganii Rhizoma mainly focuses on the clinical medication for gynecological and oncological diseases, and gradually extends to the treatment of refractory diseases such as organ fibrosis. In the future, the research direction of Sparganii Rhizoma will tend to be more towards the research of the target mechanism and the summary of the law of medication.

目的

基于知识图谱方法对三棱近10年研究热点及趋势进行可视化分析,为未来该领域的研究提供参考。

方法

选取2012年1月1日至2022年1月1日中国知网(CNKI)中有关三棱的研究,运用CiteSpace软件对纳入研究的样本文献进行分析,并绘制年发文量、作者合作网络、机构合作网络、关键词共现、关键词聚类、关键词突现图谱。

结果

最终纳入592篇文献,年发文量较为平稳;形成以时海燕、侯睿捷、梁侨丽为代表的团队,各机构间合作较少。频次较高的关键词包括数据挖掘、用药规律、三棱等,共形成10个聚类;研究热点主要集中于妇科及肿瘤等疾病的用药规律、临床研究和相关作用机制;关键词突现分析共得到43个突现词,以数据发掘、临床研究及治疗肿瘤与妇科疾病的研究较多,实验室研究对有效成分的提取及其活性成分作用机制较为重视。

结论

近年来,三棱的研究热点主要集中在妇科和肿瘤疾病的临床用药总结以及抗纤维化机制的初步探究,未来或将深入探求其抗肿瘤作用靶点,并逐渐延伸至对器官纤维化等难治性疾病的治疗靶点研究及临床用药规律总结。

目的

基于知识图谱方法对三棱近10年研究热点及趋势进行可视化分析,为未来该领域的研究提供参考。

方法

选取2012年1月1日至2022年1月1日中国知网(CNKI)中有关三棱的研究,运用CiteSpace软件对纳入研究的样本文献进行分析,并绘制年发文量、作者合作网络、机构合作网络、关键词共现、关键词聚类、关键词突现图谱。

结果

最终纳入592篇文献,年发文量较为平稳;形成以时海燕、侯睿捷、梁侨丽为代表的团队,各机构间合作较少。频次较高的关键词包括数据挖掘、用药规律、三棱等,共形成10个聚类;研究热点主要集中于妇科及肿瘤等疾病的用药规律、临床研究和相关作用机制;关键词突现分析共得到43个突现词,以数据发掘、临床研究及治疗肿瘤与妇科疾病的研究较多,实验室研究对有效成分的提取及其活性成分作用机制较为重视。

结论

近年来,三棱的研究热点主要集中在妇科和肿瘤疾病的临床用药总结以及抗纤维化机制的初步探究,未来或将深入探求其抗肿瘤作用靶点,并逐渐延伸至对器官纤维化等难治性疾病的治疗靶点研究及临床用药规律总结。

AIM

To visually analyze the research hotspots and trends of Sparganii Rhizoma in the past 10 years based on the knowledge mapping method to provide references for future research in this field.

METHODS

The research on Sparganii Rhizoma from January 1st, 2012 to January 1st, 2022 were selected from CNKI, and the sample literature included in the study was analyzed using CiteSpace software, and the annual publication volume, author collaboration network,institutional collaboration network, keyword co-occurrences, keyword clustering, and keyword emergent mapping were drawn.

RESULTS

A total of 592 papers were included, and the annual publication volume was relatively stable. The team represented by SHI HY, HOU RJ and LIANG QL was formed, and there was less cooperation among institutions. The keywords with higher frequency include data mining, medication law, Sparganii Rhizoma, etc., forming 10 clusters. The research hotspots mainly focus on gynecological diseases and tumors, clinical research, mechanism of action, etc. The keyword emergence analysis obtained a total of 43 emergent words, with more research on data mining, clinical research, and treatment of tumors and gynecological diseases, and the laboratory research pays more attention to the extraction of the active ingredient and its active ingredients and the mechanism of action. Laboratory studies paid more attention to the extraction of active ingredients and their active components.

CONCLUSION

In recent years, the research hotspots of Sparganii Rhizoma mainly focuses on the clinical medication for gynecological and oncological diseases, and gradually extends to the treatment of refractory diseases such as organ fibrosis. In the future, the research direction of Sparganii Rhizoma will tend to be more towards the research of the target mechanism and the summary of the law of medication.

三棱(Sparganii Rhizoma)为黑三棱科植物黑三棱(Sparganium stoloniferum Buch.-Ham.)的干燥块茎。三棱最早记载于唐代的《本草拾遗》,距今已有1 200多年的历史。其味辛、苦,性平,归肝、脾经,功用于破血行气、消积止痛,主治癥瘕痞块、瘀血经闭、胸痹心痛、食积胀痛[1]。现代药理学研究表明三棱具有抗血小板聚集、抗肿瘤、抗炎镇痛等功效,常用于心血管疾病及子宫内膜异位症等妇科疾病的治疗[2]。CiteSpace是一款文献计量学可视化软件,可以从时间及空间维度分析引文,来探寻、归纳、分析、展示学科或领域的发展进程、规律以及现状,其结果可以为未来的研究和决策提供建议[3]。本研究运用CiteSpace软件,对近10年中国知网(CNKI)发表相关三棱文献的发文量、作者、研究机构、关键词等进行可视化分析,探讨三棱的研究热点以及相关趋势,以求为相关领域的后续研究提供参考与借鉴。
中文文献以CNKI为数据来源。采用CiteSpace软件(5.8.R3版本)进行分析。
CNKI检索条件设置为高级检索,检索式:主题(“三棱”OR“黑三棱”OR“荆三棱”OR“京三棱”)AND主题(“中医”OR“中药”OR“中医药”OR “药材”)NOT主题(“三棱针”OR“针灸”OR“刺络拔罐”OR“火针”OR“放血”OR“针刺”),检索文献发表时间为2012年1月1日—2022年1月1日,共检索到中文文献672篇。
符合检索策略的与三棱研究相关的文献,如与中医药相关的三棱的临床应用、方药分析、成分研究、名医经验类等文献。
(1)如与三棱主题无关的针刺穴位、书籍校正、药品生产标准、饮片调剂规程等文献;(2)重复发表的文献;(3)会议论文、通知、科普宣传、报道以及信息不全的文献。最终纳入分析范围的中文文献为592篇,文献筛选流程见图1
CNKI文献以“Refworks”格式导出并分别存储为“download_xxx.txt”格式,导出的题录中包含论文的题目、作者、关键词、摘要、发文机构等。CiteSpace 5.8.R3参数设置:时区范围(time slicing)为2012—2022年,时间切片选定1年;阈值(top N per slice)为10%;节点类型(node type)为作者(author)、关键词(key word)、发表机构(institution)、数据来源(source)。图谱修剪算法为pathfinder、prunning sliced networks、pruning the merged network,绘制知识图谱,并对发文量、中心性、关键词和突现词等参数进行数据分析。中介中心性(centrality)表示节点在网络中的重要程度,起到枢纽的作用,可以反映研究领域的热点方向,一般以中介值≥0.1为条件评价该网络节点的重要性[4]。在共现网络的基础上,利用Loglikelihood Ratio test(LLR)算法对关键词进行知识图谱的聚类分析。聚类序号与聚类大小成反比,聚类序号越小则其聚类的节点数越多,代表其研究热度越高。以聚类模块值(Q值)>0.3时认为聚类结构准确且显著,即图谱聚类有效;聚类平均轮廓值(S值)>0.7时,聚类结果可信[5]。利用关键词突现分析展示三棱研究热点随时间的变化情况[6]
2012—2022年三棱相关研究发文量一直较为平稳,2015—2017年发文量持续上涨,2018—2021年发文量基本稳定在较高水平,均保持在50篇以上。见图2
近十年三棱相关中文研究发文量较高的机构均为中医药领域相关高校及其附属医院。其中南京中医药大学发文量最多,其余发文量大于30篇的有北京中医药大学、山东中医药大学、成都中医药大学。对研究机构合作网络进行可视化分析后获得节点数247个,连线147条,见图3。提示各研究机构间合作较少,主要聚集在不同地区的中医药大学及相应的附属医院,缺乏跨区域和多学科合作。
近年来有较多学者开展了三棱相关研究,共有274位作者被纳入,但研究均不够深入,单人发表三棱相关研究文献最多仅有4篇。对发文作者合作网络进行可视化分析,可见目前国内主要有5个三棱研究团队。分别为:(1)时海燕团队,主要致力于三棱分子机制、活性部位以及有效成分分析相关研究。(2)侯睿捷团队,主要研究内容为挖掘三棱治疗子宫内膜异位性疾病的证治规律。(3)梁侨丽团队,在三棱的化学成分提取和抗炎作用机制方面获得较多成果。(4)李有杰团队,主要进行三棱治疗肝癌、胃癌、卵巢癌的组方规律研究。(5)高智团队,主要研究三棱在妇科疾病中的治疗。另有个别作者开展了一些独立研究,在其相应研究领域获得一定成果,如卢青进行了三棱-莪术药对抗动脉粥样硬化作用及机制探究;华雯对三棱治疗肝癌的作用机制展开了研究。各研究团队内部成员合作密切,团队间外部合作较少,见图4
对2012—2022年关于三棱研究的关键词进行共现分析,得到260个节点,380条连线。除三棱本身外出现频次最高的是前五名分别是数据挖掘(87次)、用药规律(67次)、中药(32次)、子宫肌瘤(25次)、临床研究(19次)。除三棱外,中介值≥0.1的关键词共有4个,分别为数据挖掘(0.32)、用药规律(0.25)、中药(0.15)和莪术(0.10)。其余中介中心性较大的关键词为子宫肌瘤、痛经、肿瘤,见图5
对260个关键词进行标签聚类,共形成了10个有代表性的聚类结果,分别为#0中医药,#1三棱,#2用药规律,#3临床研究,#4作用机制,#5子宫肌瘤,#6胃癌,#7配伍,#8关联分析,#9本草考证。见图6。Q值=0.706 8,S值=0.903 5,该结果显著可信。
对各聚类关键词进行总结归纳,可将10个聚类分为4个方面,结果见表1,包括:(1)数据研究,包含#0、#8,主要关键词为医案、数据挖掘、关联规则、中医证型等。(2)组方用药规律,包含#2、#7,主要关键词为莪术、药对、活血药、用药规律等。(3)临床应用,包含#3、#5、#6,主要关键词为子宫肌瘤、胃癌、痛经、米非司酮、临床疗效、甲氨蝶呤等。(4)成分提取与药理作用,包含#1、#4、#9,主要关键词为制备工艺、化学成分、靶点、通路、炮制、产地等。
利用Timeline视图显示各聚类的研究时长和聚类间的联系,直观展现三棱相关的研究方向随时间变化的情况,见图7。聚类#4(作用机制)、聚类#9(本草考证)研究时间跨度最长。
突现的起止时间构成了关键词的突现时期,体现关键词在该时期内出现频次显著变化,在图8中用红色线条表示。根据43个关键词突现的起止时间,将研究前沿分为早期前沿、中期前沿和近期前沿三类[7]。早期前沿(2012—2014年):主要集中于三棱治疗妇科疾病的研究及建立三棱指纹图谱、药材品质评价及制备工艺等方面;中期前沿(2015—2017年):三棱在胃癌、肝癌等肿瘤治疗中的应用成为新的研究热点;近期前沿(2018—2022年):三棱抗肿瘤(尤其是胃癌、肝癌、子宫肌瘤)作用机制的研究成果较多逐渐深入,利用分子对接进行网络药理学研究等生物信息学分析也逐渐出现。
三棱始载于唐代《本草拾遗》,是临床常用的活血化瘀药物。本研究运用CiteSpace软件对近10年所发表的三棱相关研究中文文献进行可视化分析,直观展示出近年来三棱相关文献的作者与合作机构特征、发文趋势,提示三棱目前主要研究内容、热点及未来研究方向。
研究结果显示,近10年来,学者们对中药三棱相关研究持续保持较高热度;形成了以时海燕、侯睿捷、梁侨丽、李有杰和高智为代表的5个研究团队。各团队研究方向不同,内部成员合作密切,但外部联系较少。
目前三棱的研究热点主要集中于治疗妇科疾病和肿瘤的临床用药规律总结以及抗肿瘤作用机制方面。研究结果显示,三棱的主要化学成分为挥发油类、黄酮类、有机酸类、皂苷类、苯丙素类及生物碱类[8];具有抗血小板凝集、抗血栓、镇痛抗炎、抗纤维化、抗肿瘤[9-13]以及保护心脑血管等药理作用,在临床常用于治疗胃癌、肝癌、子宫肌瘤、卵巢癌及妇科疾病[14-16]
近年来针对三棱治疗肿瘤及妇科疾病的药理机制及临床用药经验挖掘的相关研究不断深入;同时也出现了一些三棱用于治疗其他疾病的新研究方向。周岚等[17]研究发现三棱可以通过调节主动脉Cyr61Sgk1Ch25h基因表达水平实现抗动脉粥样硬化的作用;LOPEZNOVOA等[18-20]发现三棱可通过激活PPARγ表达从而抑制TGFβ1/Smads信号通路介导的IEC-6细胞上皮间质转化,进而抑制肺、肝、肠道纤维化的进程,延缓组织凋亡;董毅玲等[2122]发现从三棱中提取出的三棱内酯B可与一种TAR(HIV-1结构区)区域相互作用,从而阻断HIV-1转录,降低HIV的传染性。这些具有创新意义的研究拓宽了三棱的研究方向,为指导三棱的临床使用提供了一定的参考和科学依据。
三棱-莪术作为药对,常相须为用,故有学者以此为出发点构建Copula函数,论证了三棱-莪术不同剂量伍用的耦合效应,探索相须配伍关系中的数理模型[23];还有学者研究发现,三棱与芒硝配伍,会使其效用减弱[24],而与延胡索配伍可有效治疗胁痛[25],这些三棱与他药配伍关系的总结和研究,必将为指导三棱在临床的合理使用提供重要的科学依据。
三棱作为传统中药,在临床广泛应用。近年来受到了越来越多的关注,对其药理、作用机制、合理临床使用等方面开展了大量研究,取得了系列成果,但仍存在不足:(1)各研究团队间缺乏跨区域和多学科合作,应加强沟通;(2)主要集中在妇科及肿瘤的治疗方面,在其他疾病方面研究较少,有待深入开展其对器官纤维化、艾滋病等西医难治性疾病作用机制的研究,为三棱临床合理、安全、有效使用提供科学依据。
