AIM To evaluate the efficacy and safety of oxycodone in acute heart failure (AHF). METHODS In this prospective randomized controlled trial, 50 AHF patients admitted to the ICU between January 2022 and December 2023 were allocated to morphine group (n=25) or oxycodone group (n=25) using random number table, received intravenous injection of 5 mg morphine or oxycodone based non-invasive ventilation, respectively. Effectiveness outcomes (comfort score,NT-proBNP, oxygenation index (OI), ICU length of stay, non-invasive ventilation duration, and hospitalization outcomes)and safety parameters (mean arterial pressure (MAP), heart rate (HR), respiratory rate (RR), minute ventilation (MV),intubation rate, and respiratory depression) were assessed at baseline (T0), 10 min (T1), 20 min (T2), 30 min (T3),and 24 h (T4) post-treatment in the two groups. RESULTS Both groups showed significant reduction in comfort scores at T3 and increased OI at T4 compared with those of T0P<0.01), and the oxycodone group demonstrated significantly lower NT-proBNP at T4P<0.05), with no significant differences between the two groups(P>0.05). No between-group differences were observed in ICU stay or ventilation duration. MAP decreased transiently in both groups: the morphine group maintained lower MAP in T1-T3 than T0P<0.05), whereas the oxycodone group only showed reductions at T1 and T2P<0.05),with significantly higher MAP than the morphine group during T1-T3P<0.05). HR exhibited biphasic changes while RR and MV progressively declined. The morphine group showed sustained RR reduction in T1-T3 than T0P<0.05), and the oxycodone group showed reduction at T2 and T3P<0.05). Neither group required intubation or developed respiratory depression. CONCLUSION Oxycodone demonstrates favorable hemodynamic stability and potential prognostic benefits in AHF management, suggesting its promise as a novel therapeutic option.

目的 观察羟考酮治疗急性心力衰竭的有效性及安全性。方法 纳入2022年1月至2023年12月重症医学科收治的急性心力衰竭患者50例,采用随机数字表法分成吗啡组和羟考酮组(均n=25),在无创机械通气治疗基础上分别给予静脉推注吗啡5 mg或羟考酮5 mg,于治疗前(T0)和治疗后10 min(T1)、20 min(T2)、30 min(T3)、24 h(T4)观察2组药物治疗的有效性[舒适度评分、N末端脑钠肽前体(NT-proBNP)、氧合指数(OI)、ICU住院天数、无创机械通气时间和住院结局]及安全性[平均动脉压(MAP)、心率(HR)、呼吸频率(RR)、分钟通气量(MV)及气管插管和呼吸抑制发生情况]。结果 与T0相比,2组T3时舒适度评分显著降低、T4时OI显著升高(P<0.01),羟考酮组T4时NT-proBNP显著降低(P<0.05),但组间比较均无显著差异(P>0.05)。2组间ICU住院天数、无创通气时间均无显著差异(P>0.05)。吗啡组MAP在T1~T3均低于T0P<0.05),羟考酮组MAP仅T1、T2低于T0P<0.05),且T1~T3的MAP显著高于吗啡组(P<0.05)。2组HR呈先升高后降低趋势,RR、MV则随时间降低,而吗啡组T1~T3的RR显著低于T0P<0.05),羟考酮组仅T2、T3低于T0P<0.05)。2组均未发生气管插管或无呼吸抑制。结论 羟考酮对急性心力衰竭患者的循环呼吸影响小,并能有效改善预后,可能成为该类疾病的新选择。

目的 观察羟考酮治疗急性心力衰竭的有效性及安全性。方法 纳入2022年1月至2023年12月重症医学科收治的急性心力衰竭患者50例,采用随机数字表法分成吗啡组和羟考酮组(均n=25),在无创机械通气治疗基础上分别给予静脉推注吗啡5 mg或羟考酮5 mg,于治疗前(T0)和治疗后10 min(T1)、20 min(T2)、30 min(T3)、24 h(T4)观察2组药物治疗的有效性[舒适度评分、N末端脑钠肽前体(NT-proBNP)、氧合指数(OI)、ICU住院天数、无创机械通气时间和住院结局]及安全性[平均动脉压(MAP)、心率(HR)、呼吸频率(RR)、分钟通气量(MV)及气管插管和呼吸抑制发生情况]。结果 与T0相比,2组T3时舒适度评分显著降低、T4时OI显著升高(P<0.01),羟考酮组T4时NT-proBNP显著降低(P<0.05),但组间比较均无显著差异(P>0.05)。2组间ICU住院天数、无创通气时间均无显著差异(P>0.05)。吗啡组MAP在T1~T3均低于T0P<0.05),羟考酮组MAP仅T1、T2低于T0P<0.05),且T1~T3的MAP显著高于吗啡组(P<0.05)。2组HR呈先升高后降低趋势,RR、MV则随时间降低,而吗啡组T1~T3的RR显著低于T0P<0.05),羟考酮组仅T2、T3低于T0P<0.05)。2组均未发生气管插管或无呼吸抑制。结论 羟考酮对急性心力衰竭患者的循环呼吸影响小,并能有效改善预后,可能成为该类疾病的新选择。

AIM To evaluate the efficacy and safety of oxycodone in acute heart failure (AHF). METHODS In this prospective randomized controlled trial, 50 AHF patients admitted to the ICU between January 2022 and December 2023 were allocated to morphine group (n=25) or oxycodone group (n=25) using random number table, received intravenous injection of 5 mg morphine or oxycodone based non-invasive ventilation, respectively. Effectiveness outcomes (comfort score,NT-proBNP, oxygenation index (OI), ICU length of stay, non-invasive ventilation duration, and hospitalization outcomes)and safety parameters (mean arterial pressure (MAP), heart rate (HR), respiratory rate (RR), minute ventilation (MV),intubation rate, and respiratory depression) were assessed at baseline (T0), 10 min (T1), 20 min (T2), 30 min (T3),and 24 h (T4) post-treatment in the two groups. RESULTS Both groups showed significant reduction in comfort scores at T3 and increased OI at T4 compared with those of T0P<0.01), and the oxycodone group demonstrated significantly lower NT-proBNP at T4P<0.05), with no significant differences between the two groups(P>0.05). No between-group differences were observed in ICU stay or ventilation duration. MAP decreased transiently in both groups: the morphine group maintained lower MAP in T1-T3 than T0P<0.05), whereas the oxycodone group only showed reductions at T1 and T2P<0.05),with significantly higher MAP than the morphine group during T1-T3P<0.05). HR exhibited biphasic changes while RR and MV progressively declined. The morphine group showed sustained RR reduction in T1-T3 than T0P<0.05), and the oxycodone group showed reduction at T2 and T3P<0.05). Neither group required intubation or developed respiratory depression. CONCLUSION Oxycodone demonstrates favorable hemodynamic stability and potential prognostic benefits in AHF management, suggesting its promise as a novel therapeutic option.

急性心力衰竭是常见急重症,临床表现为心输出量在短期内急剧下降,血流动力学紊乱,甚至丧失排血功能,因其发病急、病情变化快,严重危及患者的生命安全,正确的处理方法和安全有效的药物在临床上的应用对于控制患者的症状和改善预后至关重要[1]。长期以来,吗啡一直是治疗急性心力衰竭的经典一线药物,也是多年的临床实践共识,国内外心力衰竭治疗指南都曾做推荐[2]。但近年,特别是急性失代偿性心力衰竭的国际注册研究(ADHERE)[3]结果让吗啡在急性心力衰竭救治中应用的安全性和有效性不断受到质疑,而且急性心力衰竭中国急诊管理指南(2022)[4]已不推荐常规应用吗啡,仅建议对烦躁不安又除外持续低血压、意识障碍、严重慢性阻塞性肺疾病的急性心力衰竭患者,可小剂量缓慢静脉注射,同时需注意用药个体化。而羟考酮与吗啡的镇痛效果相当且不良反应较少[5],同时羟考酮能抑制心肌缺血再灌注损伤后的细胞凋亡和保护心脏微血管内皮细胞完整性而发挥心脏保护作用[6-8],但其用于急性心力衰竭镇痛的报道较少,因此,本研究应用羟考酮治疗急性心力衰竭,明确其对心脏重症患者治疗的安全性及有效性。
研究选取2022年1月至2023年12月重庆大学附属涪陵医院重症医学科收治的急性心力衰竭患者50例,纳入标准:(1)年龄18~85岁;(2)诊断符合急性心力衰竭诊断标准[4];(3)应用无创机械通气。排除标准:(1)妊娠或哺乳期妇女;(2)对阿片类药物过敏或有使用禁忌;(3)总胆红素≥34.2 μmol·L-1;(4)慢性肾脏病、急性肾损伤或需连续性肾脏替代治疗(CRRT)者;(5)心源性休克;(6)重症肌无力;(7)肠梗阻;(8)下丘脑综合征、腺垂体功能减退、肾上腺皮质功能减退症及甲状腺功能减退;(9)严重酸中毒(pH<7.2);(10)意识障碍(Glasgow昏迷评分≤14分)。采用随机数字表法将患者分成吗啡组和羟考酮组,每组各25例,本研究经医院伦理委员会批准(2022CQSFLZXYYEC-018),同时与患者或家属签署知情同意书。
全部患者均给予半卧位、利尿、抗感染相关常规治疗,同时应用经鼻(面)罩无创机械通气治疗,模式:S/T(IPAP:8~14 cmH2O,EPAP:4~8 cmH2O,吸氧浓度30%~60%)。在此基础上,吗啡组静脉缓慢推注吗啡5 mg(东北制药集团沈阳第一制药有限公司,批号:210908),羟考酮组静脉缓慢推注羟考酮5 mg(江苏恩华药业股份有限公司,生产批号:QK220102)[4,9,10]
记录2组患者治疗前(T0)和治疗后30 min(T3)的舒适度评分,采用视觉模拟评分法[11]进行评价(1~10分,评分越低表示患者舒适度越高),记录2组患者T0和治疗后24 h(T4)的N末端脑钠肽前体(NT-proBNP)、氧合指数(OI)以及2组患者无创机械通气时间、ICU住院天数、住院结局。
2组患者气管插管及呼吸抑制发生情况(呼吸抑制定义为呼吸频率≤8次·min-1[12]),2组患者T0和治疗后10 min(T1)、20 min(T2)、T3的平均动脉压(MAP)、心率(HR)、呼吸频率(RR)及分钟通气量(MV)变化情况。
采用SPSS 19.0对数据进行统计学分析,符合正态分布的计量资料以均数±标准差()表示。组内比较采用配对t检验,组间比较采用两样本独立t检验;不同时间点重复测量数据采用重复设计方差分析,若存在交互效应,则应进行单独效应分析,即通过单因素方差分析组内效应,采用多变量方差分析组间效应;非正态分布的计量资料以中位数(四分位数)[MP25P75)]表示,组间比较采用秩和检验;计数资料采用χ2检验。P<0.05为有显著差异。
2组患者的年龄、性别、体重指数,及心脏超声、降钙素原、血常规、肾功能、肝功能的比较均无显著差异(P<0.05)。见表1
T0时,2组舒适度评分、NT-proBNP及OI无显著差异(P>0.05)。与T0相比,2组T3时舒适度评分显著降低、T4时OI 显著升高(P<0.01),羟考酮组T4时NT-proBNP显著降低(P=0.019);2组间比较均无显著差异(P>0.05)。见表2。吗啡组和羟考酮组住院天数分别为12(8.5,15)d和10(7,12.5)d,且均好转出院,无创通气时间分别为10.5(6.5,26.5)h和10(5,13.5)h,2组比较均无显著差异(P>0.05)。
2组患者MAP随时间先降低后升高(P组间=0.036,P时间<0.001,P交互<0.001),其中吗啡组MAP在T1~T3均低于T0P<0.05),羟考酮组MAP仅T1、T2低于T0P<0.05),且羟考酮组T1、T2、T3的MAP均高于吗啡组(P<0.05)。2组患者HR随时间先升高后恢复(P组间=0.905,P时间<0.001,P交互=0.542),2组患者RR、MV则随时间降低(分别为P组间=0.605,P时间<0.001,P交互=0.601;P组间=0.714,P时间<0.001,P交互=0.766),其中吗啡组的RR在T1~T3均低于T0P<0.05),羟考酮组的RR仅T2、T3低于T0P<0.05)。见表3。另外,2组患者均未发生气管插管和呼吸抑制。
急性心力衰竭是老龄患者住院的主要原因,且患病率呈逐年升高趋势[13],已经成为威胁中老年群体的首要疾病。急性心力衰竭住院病死率为3%,6个月再住院率50%,5年病死率高达60%[14],且发病急、进展迅速,可危及生命,在发病时需及时治疗来提升存活率、改善预后。急性心衰的药物治疗中,曾推荐早期应用吗啡治疗,其治疗急性心力衰竭是基于动静脉舒张作用,减轻心脏前后负荷,改善心功能;另一方面,吗啡具有的镇痛镇静作用能缓解烦躁、恐惧相关不良情绪,抑制交感神经过度亢进、降低氧耗。近来研究[15]发现,应用吗啡与气管插管、使用正性肌力药物和病死率呈正相关,吗啡也可能导致心输出量减低和低血压,同时伴随着中枢性呼吸抑制相关不良反应,因此,吗啡治疗急性心力衰竭的安全性受到质疑。然而,对于急性心肌梗死合并不伴有低血压的急性心力衰竭,应用阿片类药物兼具镇痛、镇静和减轻心脏负荷的多重效应[4],获益明确。与此同时,适宜的镇痛镇静可以减轻疼痛、缓解焦虑,可提高无创机械通气治疗成功率[16],有研究表明[17],临床应用镇静镇痛药物可显著提高急性心力衰竭患者无创机械通气的疗效及依从性。除此以外,阿片类药物亦具有心脏保护作用,此作用依赖于阿片受体的激活,κ、δ受体的激活直接参与心肌保护[18],而血管系统存在μ、κ及δ受体,在人体心房肌中此三种受体均有分布[19],因此,阿片类药物在治疗急性心力衰竭中仍存在重要作用。
羟考酮是一种由生物碱蒂巴因中提取的半合成阿片类药物,通过激动μ、κ双阿片受体产生镇痛作用[20]。研究表明,健康受试者在疼痛刺激下给予吗啡和羟考酮静脉推注时,与吗啡相比,羟考酮的受试者血流动力学相对更加平稳[21];另一方面,羟考酮在择期手术(非心脏疾病)全麻期间减弱气管插管相关的血流动力学反应方面比芬太尼更有效而且具有安全的血流动力学变化[22];除此以外,与舒芬太尼相比,在心脏瓣膜置换术患者麻醉前给予羟考酮,患者在麻醉诱导至手术开始过程中血流动力学更加稳定,同时也不会导致过度镇静和呼吸抑制[23]。与此同时,研究证实羟考酮的呼吸抑制发生率显著低于吗啡[24]。阿片类药物是通过作用于阿片受体而发挥作用,阿片受体主要存在μ、κ及δ三种,其中μ受体与镇痛的关系最为密切,激动μ1受体可发挥镇痛作用和药物依赖,激动μ2受体可能会出现恶心、呕吐、便秘、呼吸抑制相关不良反应,而激动κ受体可产生镇痛和镇静作用,激动δ受体则产生镇痛、血压下降相关作用。吗啡可作用于μ、κ及δ受体,主要作用于μ受体,而羟考酮则主要作用于κ受体却对μ受体作用轻微[25-27],因此,羟考酮与循环呼吸相关的不良反应更少。本研究亦证实,羟考酮对急性心力衰竭患者的循环呼吸影响更小,因此,对于缺血性心脏病、心脏重症患者而言羟考酮可能更加有利。
NT-proBNP是世界公认的心功能不全判定标志物,主要由心室肌细胞分泌,左室牵张和室壁张力对NT-proBNP的释放具有调节作用。NT-proBNP是脑钠肽前体裂解过程中所生成的无生物活性的氨基酸片段,其半衰期长、稳定性好,与急性心力衰竭患者的病情严重程度及预后密切相关[28],同时亦可用于药物治疗的疗效监测[29]。本研究发现,羟考酮与吗啡相比可降低急性心力衰竭患者的NT-proBNP,因此能够更好地改善急性心力衰竭患者的预后,与此同时,羟考酮亦能促进无创机械通气的舒适性、改善氧合。
需要说明的是,本研究存在一定的局限性,羟考酮未能减少急性心力衰竭患者的无创机械通气及住院时间可能与样本量偏少有关。另外,研究亦未对急性心力衰竭患者的远期预后进行评价,以后需更多样本量的临床研究来进一步证实。
综上所述,羟考酮对急性心力衰竭患者的循环呼吸影响小,且能更好地改善预后,可能是治疗急性心力衰竭的新选择。
