Alzheimer’s disease (AD) is a slowly progressive degenerative disease of the central nervous system, and its incidence, prevalence and mortality are increasing year by year. At present, there is no cure for AD, mainly sympotomatic treatment with drugs, including cholinesterase inhibitors and megaline, but these drugs can only relieve clinical symptoms,but can not stop or reverse the development of AD. Aducanumab and lecanemab are targeted drugs developed for amyloid β-protein (Aβ) plaques in the brain in recent years, but they have not been fully popularized, and the safety and effectiveness of drugs need to be further verified. Sodium oligomannate is an innovative drug developed in China for the treatment of mild to moderate AD patients. It mainly improves cognitive function by regulating gut microbiota-brain axis. Other possible mechanisms include reducing brain Aβ deposition and tau protein phosphorylation, activating intestinal endocrine cell-vagus afferent pathway, inhibiting intracranial neuritidine, etc. With the clinical application of sodium oligomannate, more evidence shows that sodium oligomannate can not only improve the cognitive function of AD patients, but also have certain effects on the behavioral and psychiatric symptoms and daily living ability of patients. Combined treatment with other drugs may improve the clinical efficacy and reduce adverse reactions.

阿尔茨海默病(AD)是一种缓慢进展的中枢神经系统退行性疾病,其发病率、患病率和死亡率呈逐年上涨趋势。目前尚无根治AD的方法,以药物对症治疗为主,包括胆碱酯酶抑制剂和美金刚,但这些药物仅能缓解临床症状,未能终止或逆转AD发展。aducanumab和lecanemab是近年针对脑内淀粉样蛋白(Aβ)斑块研发的靶向药,但目前尚未完全普及,药物的安全性和有效性有待进一步验证。甘露特钠是我国研发用于治疗轻中度AD患者的创新性药物,主要通过调节肠道微生物群-脑轴来改善认知功能,其他的可能机制包括减少脑内Aβ沉积和tau蛋白磷酸化,激活肠内分泌细胞-迷走神经传入通路,抑制脑内神经炎症等。随着该药的临床应用,更多证据表明甘露特钠不仅可改善AD患者的认知功能,对患者的行为精神症状和日常生活能力也有一定疗效,与其他药物联合治疗可能具有提高临床疗效,降低不良反应的作用。

阿尔茨海默病(AD)是一种缓慢进展的中枢神经系统退行性疾病,其发病率、患病率和死亡率呈逐年上涨趋势。目前尚无根治AD的方法,以药物对症治疗为主,包括胆碱酯酶抑制剂和美金刚,但这些药物仅能缓解临床症状,未能终止或逆转AD发展。aducanumab和lecanemab是近年针对脑内淀粉样蛋白(Aβ)斑块研发的靶向药,但目前尚未完全普及,药物的安全性和有效性有待进一步验证。甘露特钠是我国研发用于治疗轻中度AD患者的创新性药物,主要通过调节肠道微生物群-脑轴来改善认知功能,其他的可能机制包括减少脑内Aβ沉积和tau蛋白磷酸化,激活肠内分泌细胞-迷走神经传入通路,抑制脑内神经炎症等。随着该药的临床应用,更多证据表明甘露特钠不仅可改善AD患者的认知功能,对患者的行为精神症状和日常生活能力也有一定疗效,与其他药物联合治疗可能具有提高临床疗效,降低不良反应的作用。

Alzheimer’s disease (AD) is a slowly progressive degenerative disease of the central nervous system, and its incidence, prevalence and mortality are increasing year by year. At present, there is no cure for AD, mainly sympotomatic treatment with drugs, including cholinesterase inhibitors and megaline, but these drugs can only relieve clinical symptoms,but can not stop or reverse the development of AD. Aducanumab and lecanemab are targeted drugs developed for amyloid β-protein (Aβ) plaques in the brain in recent years, but they have not been fully popularized, and the safety and effectiveness of drugs need to be further verified. Sodium oligomannate is an innovative drug developed in China for the treatment of mild to moderate AD patients. It mainly improves cognitive function by regulating gut microbiota-brain axis. Other possible mechanisms include reducing brain Aβ deposition and tau protein phosphorylation, activating intestinal endocrine cell-vagus afferent pathway, inhibiting intracranial neuritidine, etc. With the clinical application of sodium oligomannate, more evidence shows that sodium oligomannate can not only improve the cognitive function of AD patients, but also have certain effects on the behavioral and psychiatric symptoms and daily living ability of patients. Combined treatment with other drugs may improve the clinical efficacy and reduce adverse reactions.

阿尔茨海默病(Alzheimer’s disease, AD)是最常见的老年性痴呆,约占老年性痴呆的60%~70%,以进行性认知功能障碍和行为损害为特征[1]。AD的主要病理特点为细胞外β-淀粉样蛋白(amyloid β-protein,Aβ)沉积形成的老年斑和细胞内tau蛋白过度磷酸化形成的神经纤维缠结[2],临床主要表现为认知功能障碍,行为精神症状和日常生活能力下降三大症状[3]。目前我国已进入深度老龄化阶段,社会人口老龄化及随年龄增长伴发的认知功能障碍,尤其是AD,对家庭乃至社会造成沉重的经济负担[4]。据统计,2021年我国AD及其他痴呆患病人数为16 990 827例,患病率为1 194.2/10万,死亡率为34.6/10万,死亡人数高于全球平均水平,约占全球的25.2%[5]。AD已成为当前医疗花费最昂贵、最致命和负担最重的疾病之一,预计到2050年全球痴呆的患病率将增加2倍[6]。然而,目前尚无根治AD的方法,主要为对症治疗,包括药物治疗和非药物治疗。中英指南均推荐胆碱酯酶抑制剂(cholinesterase inhibitor,ChEI)治疗轻、中度AD,美金刚治疗重度AD[7],但这些药物仅能缓解临床症状,未能终止或逆转AD进展。因此,临床仍迫切需要寻找新的药物来阻止疾病发展。
2019年11月我国首次批准的甘露特钠(sodium oligomannate, GV-971)[8]、2021年6月美国食品药品管理局(FDA)批准的aducanumab [9]和2023年1月FDA加速审批通过的lecanemab [10]均为近年来批准用于治疗AD的新药。aducanumab 和lecanemab是针对脑内Ab斑块的靶向药。aducanumab的两项Ⅲ期临床试验将Aβ斑块清除率作为研究终点,研究结果相互矛盾,一经上市便引来巨大争议;lecanemab以改善认知功能为试验终点,2023年7月获得FDA完全批准,2024年1月经国家药监局批准在我国上市[11]。中国是继美国和日本之后批准lecanemab使用的第三个国家,但该药价格昂贵且目前尚未完全普及,药物安全性和有效性有待进一步验证。甘露特钠是从天然产物海藻中分离出来的寡糖化合物,主要通过调节肠道菌群,降低肠道菌群代谢产物的累积来改善认知功能,填补了我国抗AD原创药物的空白,目前已被纳入我国医保政策,价格降幅超60%[12, 13]。现有的多项临床研究表明,甘露特钠不仅可改善AD患者的认知功能,对患者的行为精神症状和日常生活能力同样具有良好的治疗效果,且耐受性和安全性高,但相关证据较为琐碎,有必要系统的整理与归纳。本文通过总结既往有关甘露特钠治疗AD患者的最新研究成果,以期为临床实践提供参考依据。
AD是一种缓慢进展的中枢神经系统退行性疾病,关于AD的发病机制目前尚不明确,可能的机制包括Aβ异常沉积、tau蛋白过度磷酸化、胆碱能神经元损伤、神经炎症、氧化应激、线粒体功能障碍等,目前的治疗药物多针对不同的途径来发挥作用[14]。甘露特钠是从褐藻中提取的一种线性多糖,主要通过肾脏排泄,可透过血-脑屏障,临床前研究显示其对轻中度AD患者的认知功能具有改善作用[15]。目前甘露特钠改善认知功能的机制尚未完全阐明,潜在的调控机制总结为以下四个方面:(1)通过重塑肠道菌群,抑制肠道细菌氨基酸样神经炎症,改善认知功能。WANG等[16]通过5XFAD转基因小鼠模型发现,肠道菌群组成的改变导致外周血中氨基酸异常升高,特别是苯丙氨酸和异亮氨酸,异常升高的氨基酸刺激外周促炎T辅助细胞(Th1)的分化和增殖,外周Th1细胞浸润中枢神经系统激活中枢M1小胶质细胞,促进AD相关的神经炎症。甘露特钠可通过重塑5XFAD转基因小鼠的肠道微生物群,抑制苯丙氨酸诱导的外周Th1细胞增殖,降低中枢神经炎症,改善认知功能[16]。(2)减少脑内Aβ沉积和tau蛋白磷酸化。甘露特钠的羧基与Aβ40/ Aβ42的3个组氨酸残基之间的多位点静电结合,抑制Aβ的聚集[17]。还可使先前形成的Aβ原纤维稳定为无毒单体,同时减少脑内tau蛋白磷酸化[16]。最近,国外的两项独立研究观察到,在雄性AD模型小鼠中,甘露特钠可改变肠道微生物群的物种丰度和代谢水平,显著降低大脑皮质和海马的Aβ负荷,调节外周和大脑皮质中细胞因子和趋化因子的水平[18]。(3)通过激活肠内分泌细胞(enteroendocrine cells,EECs)-迷走神经传入通路,调节认知功能。甘露特钠可直接或间接激活甜味受体和瞬时受体电位锚蛋白1(TRPA1),增强TRPA1介导的钙离子进入EECs引发的钙振荡,导致缩胆囊素和5-羟色胺释放增加,增强迷走神经传入活性,改善认知功能[19]。(4)通过降低脑组织中BACE1基因的表达,抑制神经炎症,改善认知功能。BACE1是Aβ生成的关键酶,甘露特钠可降低淀粉样前体蛋白/早衰蛋白1(APP/PS-1)转基因小鼠脑组织中BACE1的mRNA和蛋白的表达,减少大脑皮层和海马区Aβ斑块面积,降低促炎因子白细胞介素1β(IL-1β)和肿瘤坏死因子α(TNF-α)水平,升高抗炎因子IL-10水平,部分恢复治疗小鼠的空间记忆和行动能力[20]
轻度认知功能障碍是介于正常衰老和痴呆的中间认知状态,可出现记忆、注意力、语言、执行和视觉空间等领域的异常,早期可不影响个人的日常生活,如果不及时控制,认知功能恶化速度可能会加快,甚至造成老年痴呆,严重影响个人健康[21]。在65岁以上的轻度认知障碍患者中,随访2年痴呆的发生率为14.9%[22]。Ⅲ期临床试验[23]显示,与安慰剂治疗相比,口服甘露特钠450 mg bid可有效改善轻中度AD患者的认知功能,主要观察指标AD评估量表-认知子量表(ADAS-Cog)评分在第4周随访时出现差异趋势,第36周时两组间已具有显著差异,与基线相比,36周时两组间的变化差异为-2.15(P<0.000 1)。亚组分析评估了患者简易智力状态检查量表(mini-mental state examination, MMSE)评分在11~14、15~19和20~26分间的治疗效果,结果显示,MMSE评分在11~14分治疗效果最佳,表明甘露特钠在认知能力下降越严重的人群,治疗效果可能越好[23]
甘露特钠从国内批准再到进入临床其改善认知功能的作用得到进一步的验证。一项纳入78例轻中度AD患者的临床研究显示,与多奈哌齐相比,甘露特钠治疗6个月可显著提高患者的蒙特利尔认知评估量表(MoCA)和老年性痴呆生活质量量表(QOL-AD)评分,提示甘露特钠可改善AD患者的认知能力、提高生活质量[24]。另一项随机、双盲前瞻性研究共纳入72例患者,按1:1:1分为安慰剂组、多奈哌齐组和甘露特钠组,结果显示,与安慰剂相比,甘露特钠可较早改善患者的临床印象变化量表(CIBIC-plus)评分,第24周ADAS-Cog评分也有改善,提示甘露特钠治疗轻中度AD有效,但需要长期持续治疗才能改善患者的认知功能;与多奈哌齐相比,甘露特钠治疗48周在改善AD患者整体临床印象和抑郁症方面较好,但在改善患者认知功能、生活自理能力和精神症状方面两者相当[25]。上述研究结果的不一致可能与研究设计不同有关,前者以MoCA评分作为评估认知功能的主要观察指标,后者则以ADAS-Cog评分为主,虽然前者研究的治疗周期仅为6个月,但可能由于MoCA量表仅强化对注意力和执行功能的评估[5],故显示认知功能改善更显著。
由于AD发病机制复杂,单药治疗往往效果欠佳,研究表明,甘露特钠联合多奈哌齐可显著提高疗效。王云霞等[26]的研究显示,与单用多奈哌齐治疗相比,联合甘露特钠可显著降低患者的ADAS-Cog评分,提高MMSE评分、日常生活能力量表(activity of daily living scale,ADL)评分及总体有效率,提示甘露特钠联合多奈哌齐有助于改善患者的认知功能,提高日常生活能力及治疗的总体有效率。一项前瞻性队列研究显示,40例服用多奈哌齐治疗3月主观疗效不满意或认知功能下降较快的AD患者,联合甘露特钠治疗6个月,可不依赖患者的病程和基线认知水平,显著改善MoCA、Addenbrooke认知功能检查-Ⅲ评分,延缓患者认知功能的下降,且两组不良反应发生率无明显差异[27]。一项纳入95例轻中度AD患者的研究表明,甘露特钠联合多奈哌齐与单用多奈哌齐治疗6个月相比,患者的ADAS-Cog、MMSE和ADL评分均明显改善,但两组治疗3个月的评分与治疗前相比未见显著差异[28]。另一项治疗周期为8周的观察性研究也显示,在治疗第4周和第8周末MMSE评分较治疗前未见显著学差异[29],提示甘露特钠的治疗效果与治疗周期有关,治疗时间越长,治疗效果可能越好。
非药物治疗方式中的神经调控治疗和中医药治疗目前发展迅速,重复经颅磁刺激(rTMS)是神经调控治疗的一种,不仅可改善认知功能,对AD患者的精神症状和睡眠障碍也有治疗效果[5]。有研究显示,甘露特钠联合rTMS与单用rTMS、单用甘露特钠相比,可显著降低患者的ADAS-Cog和AD行为病理评定量表(BEHAVE-AD)评分,改善患者的认知功能,随访24周甘露特钠联合rTMS的总有效率78.79%仍高于单用rTMS治疗54.55%[30]。夏俊博等[31]的研究也进一步表明,甘露特钠联合rTMS治疗3个月可改善AD患者的炎症状态、调节神经递质的水平,提高MMSE、MoCA评分,改善认知功能。潘亚平等[32]的研究显示,中药汤剂联合甘露特钠可提高MMSE评分,降低ADAS-Cog、BEHAVE-AD评分,有效改善患者的认知功能和痴呆症状,显著提高治疗总有效率。
痴呆的行为精神症状包括抑郁、易激惹、冷漠、焦虑、躁动、妄想等,可出现在AD的任何阶段,研究表明,即使是轻度的行为改变,也会增加AD的患病风险,行为精神症状常导致高的发病率、住院率和死亡率,严重增加照护者负担[3, 33]。目前,针对AD患者的行为精神症状非药物治疗为一线治疗方式,药物治疗包括抗痴呆药物、抗精神病药物、抗抑郁药物和心境稳定药物[34]。抗痴呆药物多奈哌齐和卡巴拉汀具有改善精神症状作用,卡巴拉汀改善中重度AD精神症状优于多奈哌齐(Ⅰ级证据),但多奈哌齐安全性更高[7]。甘露特钠的临床前研究由于AD患者的基线NPI评分非常低,平均为3分,故尚未观察到患者行为精神症状的明显改善情况。有报道证实,甘露特钠治疗2周末AD患者的被害妄想症状明显减少,治疗3个月末症状基本消失,停药后精神症状再发,重复治疗后症状得到改善[35],提示甘露特钠对AD患者的行为精神症状具有改善作用。一项纳入60例轻中度AD患者的研究表明,甘露特钠联合多奈哌齐与单用多奈哌齐相比,可显著降低治疗4周末、8周末的NPI评分,尤其对患者的焦虑、抑郁、激越、易激惹症状改善明显[29]。另有研究表明,美金刚联合甘露特钠较美金刚联合多奈哌齐治疗3个月,患者的BEHAVE-AD评分下降更明显,表明美金刚联合甘露特钠可显著改善AD患者的精神症状[36]
AD是导致老年人丧失日常生活能力的最常见疾病之一[1]。日常生活能力包括基本生活能力和工具使用能力,认知功能障碍与AD早期的工具使用能力下降有关,认知功能恶化与AD后期的基本生活能力下降有关,及早的日常生活能力评估有助于预测认知障碍的发生风险,改善AD患者的预后[3]。研究表明,甘露特钠在改善AD患者认知功能的同时可提高患者的日常生活能力。一项纳入60例轻中度AD患者的临床研究,按1:1分为单用多奈哌齐组和甘露特钠联合多奈哌齐组,甘露特钠联合多奈哌齐治疗12个月,患者的MMSE和ADL评分显著高于治疗前,且患者血液中的脂联素浓度明显升高,表明甘露特钠联合多奈哌齐有利于改善患者的营养状态,提高生活质量[37]。最近的研究表明,甘露特钠可改善早期突触核蛋白病小鼠的运动功能障碍,显著减少小鼠大脑皮质、中脑和小脑中的α-突触核蛋白,对α-突触核蛋白诱导的神经元损伤起保护作用[38]。α-突触核蛋白也是AD最新指南[39]中非AD病理生物标志物之一,目前关于甘露特钠提高AD患者日常生活能力的机制尚不明确,提示可能与降低脑组织中α-突触核蛋白有关。
随着AD病程的延长,患者伴发癫痫的概率也在增加,AD患者中约7%的人群伴发癫痫,癫痫可使患者的认知功能进一步恶化,多奈哌齐和美金刚是治疗AD的常用药物,可降低癫痫发作阈值,诱发或加重癫痫发作[40]。有研究表明,甘露特钠联合左乙拉西坦对AD伴发的癫痫有治疗作用[40, 41],目前尚无大样本的临床研究证实该结论,但存在动物研究证实甘露特钠50 mg·kg-1联合左乙拉西坦150 mg·kg-1可改善慢性癫痫大鼠的认知功能,降低癫痫发作频率[42]。自主神经功能障碍可增加AD患者的感染风险、住院周期和死亡风险,随着疾病进展,AD患者常出现自主神经功能障碍,并且自主神经功能障碍与认知功能呈正相关,有研究表明甘露特钠可能具有改善AD患者大小便失禁的潜力[43]。另外,也有研究观察到,甘露特钠在改善AD患者认知功能的同时,可能有下调甲状腺球蛋白抗体滴度和血脂水平,缓解肠易激综合征患者腹痛腹泻症状的作用[44, 45]。总之,上述研究尚待更多证据支持,但提示甘露特钠的作用可能不局限于改善AD患者的三大症状。
甘露特钠是一种海洋来源的寡糖,临床前研究纳入的研究对象为50~85岁的轻中度AD患者,结果显示,大多数的不良事件为轻至中度,无需治疗[46]。甘露特钠常见的不良反应包括尿路感染、鼻咽炎、上呼吸道感染、血糖升高、头晕、高脂血症等,其中高脂血症和鼻咽炎发病率高于安慰剂组,其他的不良事件两组间无显著差异[23]。Ⅱ期临床研究报告的14例严重不良反应中,600 mg组的1例痴呆的行为和精神症状可能与药物相关[46]。Ⅲ期临床研究的治疗周期为9个月,有研究报道,1例合并糖尿病的AD患者使用甘露特钠联合多奈哌齐、美金刚治疗3年,未见明显不良反应,且甘露特钠疗效和耐受性好[47]。多奈哌齐属于ChEI,是AD治疗的一线药物,可出现心动过缓、腹泻、低血压和尿失禁等副作用,且不良反应多出现在高龄患者,研究表明,对于心率偏慢及服用多奈哌齐出现精神紊乱的患者,使用甘露特钠治疗安全性更高[48-52]
AD已成为影响我国公民健康和社会可持续发展的重大公共卫生问题。甘露特钠通过靶向肠道微生物群-脑轴改善患者的认知功能,为人类战胜AD提供新的治疗思路,甘露特钠是一种线性多糖,近年来的研究表明,多糖在体内和体外均对AD有潜在作用,不同来源的多糖具有相似的抗AD作用机制,其减轻病理损伤和改善认知功能的机制可能有:(1)拮抗异常淀粉样蛋白和tau蛋白毒性;(2)减轻氧化应激和促炎作用;(3)重建神经可塑性[53]。甘露特钠的作用机制目前仍未完全阐明,未来仍需探讨其改善认知功能的其他可能机制。
现有的临床研究显示,甘露特钠可改善AD患者的认知功能和行为精神症状,提高患者的日常生活能力,评估的方式多采用神经心理测评,包括MMSE、MoCA、ADAS-Cog、NPI、ADL等评估量表,但因评估者个体差异及量表自身的局限性,如MMSE用于识别痴呆早期患者灵敏度差、MoCA用于低教育水平人群适用性不强等问题[5],有待其他评价标准进行补充。目前,最新修订的AD诊断和分期指南[39]纳入了血液生物标志物,尤其是血浆P-tau217显示出更大的诊断优势,其他的生物标志物如脑脊液中的Aβ42/Aβ40、P-tau181,影像学标志物Aβ-PET、tau-PET等同样具有诊断价值,未来可结合生物标志物检查进一步量化评估甘露特钠改善AD症状与Aβ和tau蛋白的相关性。基因突变也是导致AD发病的危险因素之一,载脂蛋白E(ApoE)的等位基因ApoE ε4被认为是相关性最强的易感基因[5], 有报道证明甘露特钠联合多奈哌齐治疗ApoE ε3/ε4型AD患者的认知功能、行为精神症状和日常生活能力是安全有效[54] ,未来可进一步验证并探讨可能的机制。多项研究表明延长治疗周期治疗效果更佳,未来期待更长的观察周期以评估甘露特钠的疗效。
除治疗AD外,目前尚有研究证实,甘露特钠对卒中后认知障碍、混合性痴呆、路易体痴呆和行为变异型额颞叶痴呆的认知功能也有治疗作用,提示甘露特钠可能通过共同的通路来发挥作用,为未来的研究提供方向。
综上所述,甘露特钠对AD患者的认知功能、行为精神症状和日常生活能力显示出较好的治疗效果,相关的作用机制有待进一步研究,药物的安全性和耐受性高,与其他药物联合治疗可能具有提高临床疗效,降低不良反应的作用,未来期待国内外更多大规模、多中心的研究提供甘露特钠的循证医学证据指导AD治疗。
