AIM To explore the effect of Ginkgo biloba diterpene lactone glucamine injection on inflammation and oxidative indicators and proteins related to the phosphatidylinositol 3-kinase (PI3K)/serine-threonine protein kinase(Akt) signaling pathway in the blood of patients with post-stroke epilepsy. METHODS A total of 150 patients with post-stroke epilepsy in our hospital form May 2021 to April 2023 were randomly divided into two groups, each containing 75 cases. The control group was given sodium valproate for 3 months, and on this basis, the observation group was given Ginkgo biloba diterpene lactone glucamine injection for 2 weeks. The clinical efficacy and adverse reactions of the two groups were compared, as well as seizure number, duration, serum interleukin-1β (IL-1β), matrix metalloproteinase-9 (MMP-9),superoxide dismutase (SOD), tumor necrosis factor-alpha (TNF-α), glutathione peroxidase (GSH-Px), malondialdehyde(MDA), and protein expression related to PI3K/Akt signaling pathway before and after the treatment. RESULTS The total clinical effective rate of the observation group was 92%, which was higher than 72% of the control group (P<0.05). After 3 months of treatment, the number of seizures and seizure duration in the observation group were less than those in the control group (P<0.05). After 2 weeks and 3 months of treatment, the levels of IL-1β, MMP-9, MDA, TNF-α, and the expression of PI3K and Akt proteins in the observation group were lower than those in the control group, while the activities of SOD and GSH-Px were higher than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). CONCLUSION Ginkgo biloba diterpene lactone glucamine injection can reduce the inflammatory level and oxidative stress response in patients with epilepsy after stroke, and regulate the PI3K/Akt signaling pathway, thereby effectively controlling seizures, with significant effects.

目的 探究银杏二萜内酯葡胺注射液对脑卒中后癫痫患者血液中炎症、氧化指标和磷脂酰肌醇-3激酶(PI3K)/丝氨酸-苏氨酸蛋白激酶(Akt)信号通路相关蛋白的影响。方法 选取2021年5月至2023年4月商丘市第三人民医院脑卒中后癫痫患者150例,随机分为2组,各75例。对照组予以丙戊酸钠治疗3个月,于此基础上,观察组予以银杏二萜内酯葡胺注射液治疗2周。比较2组临床疗效、不良反应及治疗前后癫痫发作次数、持续时间,血清白细胞介素-1β(IL-1β)、基质金属蛋白酶-9(MMP-9)、超氧化物歧化酶(SOD)、肿瘤坏死因子-α(TNF-α)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醛(MDA)水平和PI3K/Akt信号通路相关蛋白表达。结果 观察组临床总有效率显著高于对照组(92% vs. 72%,P<0.05);治疗3个月,观察组癫痫发作次数及发作持续时间较对照组少(P<0.05);治疗2周及3个月,观察组IL-1β、MMP-9、MDA、TNF-α水平及PI3K、Akt蛋白表达均低于对照组,SOD、GSH-Px活性均高于对照组(P<0.05);2组不良反应发生率比较无显著差异(P>0.05)。结论 银杏二萜内酯葡胺注射液可降低脑卒中后癫痫患者机体炎症水平及氧化应激反应,并调控PI3K/Akt信号通路,从而有效控制癫痫发作,且效果较为显著。

目的 探究银杏二萜内酯葡胺注射液对脑卒中后癫痫患者血液中炎症、氧化指标和磷脂酰肌醇-3激酶(PI3K)/丝氨酸-苏氨酸蛋白激酶(Akt)信号通路相关蛋白的影响。方法 选取2021年5月至2023年4月商丘市第三人民医院脑卒中后癫痫患者150例,随机分为2组,各75例。对照组予以丙戊酸钠治疗3个月,于此基础上,观察组予以银杏二萜内酯葡胺注射液治疗2周。比较2组临床疗效、不良反应及治疗前后癫痫发作次数、持续时间,血清白细胞介素-1β(IL-1β)、基质金属蛋白酶-9(MMP-9)、超氧化物歧化酶(SOD)、肿瘤坏死因子-α(TNF-α)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醛(MDA)水平和PI3K/Akt信号通路相关蛋白表达。结果 观察组临床总有效率显著高于对照组(92% vs. 72%,P<0.05);治疗3个月,观察组癫痫发作次数及发作持续时间较对照组少(P<0.05);治疗2周及3个月,观察组IL-1β、MMP-9、MDA、TNF-α水平及PI3K、Akt蛋白表达均低于对照组,SOD、GSH-Px活性均高于对照组(P<0.05);2组不良反应发生率比较无显著差异(P>0.05)。结论 银杏二萜内酯葡胺注射液可降低脑卒中后癫痫患者机体炎症水平及氧化应激反应,并调控PI3K/Akt信号通路,从而有效控制癫痫发作,且效果较为显著。

AIM To explore the effect of Ginkgo biloba diterpene lactone glucamine injection on inflammation and oxidative indicators and proteins related to the phosphatidylinositol 3-kinase (PI3K)/serine-threonine protein kinase(Akt) signaling pathway in the blood of patients with post-stroke epilepsy. METHODS A total of 150 patients with post-stroke epilepsy in our hospital form May 2021 to April 2023 were randomly divided into two groups, each containing 75 cases. The control group was given sodium valproate for 3 months, and on this basis, the observation group was given Ginkgo biloba diterpene lactone glucamine injection for 2 weeks. The clinical efficacy and adverse reactions of the two groups were compared, as well as seizure number, duration, serum interleukin-1β (IL-1β), matrix metalloproteinase-9 (MMP-9),superoxide dismutase (SOD), tumor necrosis factor-alpha (TNF-α), glutathione peroxidase (GSH-Px), malondialdehyde(MDA), and protein expression related to PI3K/Akt signaling pathway before and after the treatment. RESULTS The total clinical effective rate of the observation group was 92%, which was higher than 72% of the control group (P<0.05). After 3 months of treatment, the number of seizures and seizure duration in the observation group were less than those in the control group (P<0.05). After 2 weeks and 3 months of treatment, the levels of IL-1β, MMP-9, MDA, TNF-α, and the expression of PI3K and Akt proteins in the observation group were lower than those in the control group, while the activities of SOD and GSH-Px were higher than those in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). CONCLUSION Ginkgo biloba diterpene lactone glucamine injection can reduce the inflammatory level and oxidative stress response in patients with epilepsy after stroke, and regulate the PI3K/Akt signaling pathway, thereby effectively controlling seizures, with significant effects.

脑卒中后癫痫多因脑卒中后脑神经元损伤形成的局部病灶发生神经元放电异常所致,为脑卒中较为严重的并发症之一,不仅会加剧神经功能缺损程度,还可增加全身系统性疾病风险,增加治疗难度,患者致残率及致死率较高[1-3]。现阶段,临床治疗脑卒中后癫痫以抗癫痫药物为主,其中丙戊酸钠作为不含氮的广谱抗癫痫药物应用较为广泛,该药物可有效缓解症状,控制癫痫发作[4-6],但服药疗程较长,影响治疗依从性。近年来,随着中医中药的不断推广,其在癫痫治疗中的优势逐渐凸显。脑卒中后癫痫属中医学“痫病”范畴,主要以火、痰、风、瘀为患,治疗通常采用平肝熄风、平肝泻火、祛痰开窍、活血化瘀等方法,而银杏作为传统活血化瘀中药被应用于脑卒中后癫痫治疗中,但其具体作用机制尚未阐明。有学者指出,银杏内酯可通过增强中枢神经系统胆碱能、调节神经肽Y表达等途径发挥抗癫痫作用[7,8]。基于此,本研究对脑卒中后癫痫患者采用银杏内酯化学单体注射液——银杏二萜内酯葡胺注射液治疗,观察治疗期间炎症反应、氧化应激反应指标及磷脂酰肌醇-3激酶(PI3K)/丝氨酸-苏氨酸蛋白激酶(Akt)信号通路相关蛋白等变化,旨在为临床工作提供参考。
选取2021年5月至2023年4月150例本院脑卒中后癫痫患者进行前瞻性研究。纳入标准:(1)符合脑卒中[9,10]、癫痫[11]相关诊断标准,并经颅脑CT、MRI及脑电图等检查确诊;(2)脑卒中为轻中度[美国国立卫生研究院卒中量表(NIHSS)评分<15分];(3)脑卒中后局灶性癫痫病程3个月以上,每月平均发作次数≥3次;(4)年龄60~78岁;(5)参与本研究前1个月内未接受过抗癫痫药物治疗;(6)患者可配合完成本研究,并签署知情同意书。排除标准:(1)原发性癫痫或既往癫痫病史者;(2)颅脑手术、肿瘤、脑外伤及严重感染等其他原因所致癫痫者;(3)入组前接受其他癫痫相关治疗者;(4)对本研究药物过敏或禁忌者;(5)伴有精神类疾病者;(6)合并重要器官功能不全者;(7)伴有新冠肺炎等急慢性感染者;(8)因疫情封控等原因中断治疗者。本研究经院伦理委员会批准(医学伦理审批号:20210426)。共纳入150例患者,依据随机数字表法分对照组75例,观察组75例。
2组均根据患者病情,予以基础治疗,如控糖、降压、营养神经、抗凝及调脂等。在此基础上,对照组予以丙戊酸钠(信东生技股份有限公司,国药准字HC20191001)治疗,口服,初始剂量10~15 mg·kg-1·d-1,随后递增至20~30 mg·kg-1·d-1,维持剂量,每日2次,早晚饭后30 min服用,疗程为3个月。观察组予以银杏二萜内酯葡胺注射液(江苏康缘药业股份有限公司,国药准字Z20120024)联合丙戊酸钠治疗,银杏二萜内酯葡胺注射液25 mg混溶于氯化钠注射液250 mL中,静脉滴注,初始剂量10~15滴·min-1,随后逐渐提高滴注速度,最大滴注速度不超过30滴·min-1,每日1次,连续治疗2周;丙戊酸钠用量同对照组,疗程为3个月。
收集患者治疗前性别、年龄、病程、体重指数、合并症等资料;治疗前、治疗3个月后癫痫发作次数及发作持续时间;治疗前、治疗2周及治疗3个月后实验室资料,包括炎症反应指标[血清白细胞介素-1β(IL-1β)、基质金属蛋白酶-9(MMP-9)、肿瘤坏死因子-α(TNF-α)]、氧化应激指标[超氧化物歧化酶(SOD)、谷胱甘肽过氧化物酶(GSH-Px)、丙二醛(MDA)]和PI3K/Akt信号通路相关蛋白;观察2组不良反应。
受检者均于空腹状态下采集肘静脉血8 mL,其中3 mL离心取血清,参数设置:速率3 500 r·min-1、半径18.5 cm、时间10 min,取上层清液采用上海江莱生物科技有限公司提供的酶联免疫吸附双抗体夹心试验试剂盒测定GSH-Px、SOD活性及IL-1β、MMP-9、TNF-α、MDA水平;另5 mL采用Ficoll密度梯度离心法提取外周血单个核细胞,以Western blot法测定PI3K及Akt蛋白表达,提取单个核细胞中总蛋白,BCA法测定蛋白浓度。采用10%SDS-PAGE分离胶电泳,转至PVDF膜;常温下采用5%脱脂牛奶封闭2 h,4 ℃下加入一抗(1:500),孵育过夜,洗膜后,常温下加入二抗(1:500),孵育2 h;洗膜后在PVDF膜上滴入现配的ECL发光液,以β-actin为内参,采用Image J软件分析蛋白灰度值。
治疗3个月后进行疗效判定。临床治愈:无癫痫发作,症状消失;显效:癫痫发作频率下降75%以上,症状明显减轻;好转:癫痫发作频率下降50%~75%,症状有所缓解;无效:症状无明显改善或加剧,癫痫发作频率下降程度未达到上述标准。总有效率= [(显效例数+临床治愈例数+好转例数)/总例数]×100%。
采用SPSS 26.0对数据进行统计分析,计量资料经正态性检验,符合正态分布的资料采用均数±标准差,描述比较采用重复测量方差分析并采用Bonferrion两两比较。分类计数资料用例数(百分比)表示,组间比较采用χ2检验。以P<0.05表示差异有统计学意义。
2组患者治疗前性别、年龄、病程、体重指数、合并症等资料均无显著差异(P>0.05),见表1
观察组临床总有效率为92%,显著高于对照组的72%(P<0.05),见表2

2组治疗期间均未出现严重不良反应,治疗后肝肾功能均未见异常。其中观察组出现5例恶心呕吐,3例皮疹,2例头晕头昏,不良反应发生率为13%(10/75);对照组出现3例恶心呕吐,2例皮疹,1例头晕头昏,不良反应发生率为8%(6/75),但均未影响继续治疗。2组不良反应发生率比较,差异无显著意义(χ2=1.119,P=0.290)。
脑卒中是癫痫发作的常见原因之一,癫痫也是脑卒中常见临床表现[12]。据调查显示,我国脑卒中后癫痫发生率约为9%,且近年来,随着人口老龄化趋势加剧,脑卒中患病人数不断增长,脑卒中后癫痫发病率随之升高[13,14],受到临床高度关注。
作为一种广谱抗癫痫药物,丙戊酸钠能够加快脑组织内抑制性神经递质氨基丁酸分泌,进而抑制神经元异常放电,达到保护有效神经元作用,同时还可通过对钙离子内流的抑制作用,使得神经元细胞膜保持稳定,从而有效控制癫痫发作[15-17]。但随着临床研究的不断深入,发现丙戊酸钠单独用于治疗脑卒中后癫痫效果不佳,仅可控制癫痫,且长期使用,不良反应较多[18]。有学者指出,对于大脑而言,癫痫发作过程是一个缺氧过程,可加剧脑卒中患者脑组织缺氧,进而加剧炎症及氧化应激反应,致使血脑屏障破坏,最终影响神经功能,增加致残、致死风险[19,20]。因此,减轻炎症及氧化应激反应,纠正脑组织细胞缺氧状态对改善癫痫患者预后尤为关键。
银杏叶中提取出的银杏内酯,属于倍半萜内酯化合物,抗炎及抗氧化作用显著,且有研究指出,银杏内酯可参与调控海马内缺氧诱导因子表达,而缺氧诱导因子与脑组织细胞缺氧密切相关[21]。因此,本研究采用银杏内酯化学单体注射液——银杏二萜内酯葡胺注射液治疗脑卒中后癫痫,结果显示,治疗3个月,观察组临床总有效率(92%)高于对照组(72%),观察组癫痫发作次数及发作持续时间少于对照组,且2组不良反应无明显差异,充分说明银杏二萜内酯葡胺注射液治疗脑卒中后癫痫,可有效控制癫痫发作,安全可靠。既往研究表明,银杏内酯可降低血脑屏障、具有脑保护作用[22]。另有学者指出,银杏内酯可通过减轻谷氨酸造成的钙离子超载,保护神经元[23]。由此提示,银杏二萜内酯葡胺可通过保护神经元,减少癫痫作用。
银杏内酯是一种具有抗炎、抗氧化等药理作用的小分子活性单体化合物,动物实验显示,银杏内酯可通过抑制炎症及氧化应激反应减轻大鼠缺血性脑损伤[24]。癫痫发病时,脑组织内炎症细胞因子释放过度,引发系列炎症反应,进而损伤海马神经元。IL-1β、TNF-α作为炎症趋化因子,大量释放可促使钙离子大量进入神经细胞内部,导致神经元内钙离子浓度过高,参与癫痫发生发展[25];MMP-9参与血脑屏障破坏,可促进神经元凋亡,可作为判定癫痫疗效的重要指标[26]。此外,癫痫发作过程中神经元异常放电及过度兴奋促使活性氧自由基大量合成、释放,导致抗氧化酶SOD、GSH-Px消耗过度,而过剩的活性氧自由基攻击破坏蛋白质、核酸等发生氧化应激反应,导致具有生物毒性的MDA大量生成[27]。为进一步探究银杏二萜内酯葡胺注射液治疗脑卒中后癫痫的可能机制,本研究观察患者治疗期间炎症及氧化应激反应相关指标变化,结果发现治疗2周及3个月,观察组SOD、GSH-Px活性高于对照组,IL-1β、MMP-9、TNF-α、MDA水平低于对照组,充分说明银杏内酯对脑卒中后癫痫患者炎症及氧化应激反应具有明显抑制作用,这可能是银杏二萜内酯葡胺注射液治疗脑卒中后癫痫的重要机制之一。
此外,癫痫发作过程中,脑组织低氧/缺氧,在低氧状态下缺氧诱导因子-1参与低氧反应性基因转录的调控,在缺氧性脑损伤中发挥至关重要作用。研究指出,PI3K/Akt信号通路参与调控癫痫发作后大鼠海马缺氧诱导因子-1的产生与表达[28]。而本研究中银杏二萜内酯葡胺注射液治疗2周及3个月,观察组PI3K及Akt蛋白表达较对照组降低,充分说明银杏内酯可有效调控脑卒中后癫痫患者PI3K/Akt信号通路,进而改善脑组织缺氧环境,这可能是银杏二萜内酯葡胺注射液治疗脑卒中后癫痫的另一机制。但本研究受限于科研人员及经费不足等情况,未开展银杏二萜内酯葡胺注射液治疗脑卒中后癫痫的动物实验,未观察服药前后脑组织各区域病理变化,有待日后开展动物实验二次证实本研究结论,并在病理方面进一步拓展思路。
综上所述,对脑卒中后癫痫予以银杏二萜内酯葡胺注射液、丙戊酸钠联合治疗,可降低炎症及氧化应激反应、调控PI3K/Akt信号通路,有效控制癫痫发作,效果较为显著。
