Innovative biologics are the most powerful tools for preventing and treating infectious diseases. During the containment of major public health emergencies, ensuring the production and supply of clinically urgent medications through post-approval changes carries dual requirements of compliance and urgency. This study was conducted from the perspective of enhancing prevention and control of public health emergencies. The needs and difficulties of emergency changes were analyzed, with a focus on how marketing authorization holders can efficiently carry out emergency changes to expand production capacity and accelerate release testing. Based on the analysis, relevant recommendations were proposed, such as conducting pre-research on changes, building “dual-use for routine and emergency” capabilities, implementing graded and categorized management, optimizing registration testing, and promoting the rapid microbiological methods and AI.

创新生物制品是预防和治疗传染性疾病最有力的武器。在防控突发重大公共卫生事件时,紧急开展上市后变更,确保临床急需药品的生产供应,具有合规性和紧迫性双重要求。本文从提升重大突发公共卫生事件防控响应能力的角度,分析了应急变更的需求和难点,对药品上市许可持有人如何规范高效地扩大产能、加快放行检验速度等应急变更策略进行深入研究,并提出开展变更预研、建设“平急两用”产能、实施分级分类管理、优化注册检验、推进快速微生物检测法和AI新技术应用等建议。

Innovative biologics are the most powerful tools for preventing and treating infectious diseases. During the containment of major public health emergencies, ensuring the production and supply of clinically urgent medications through post-approval changes carries dual requirements of compliance and urgency. This study was conducted from the perspective of enhancing prevention and control of public health emergencies. The needs and difficulties of emergency changes were analyzed, with a focus on how marketing authorization holders can efficiently carry out emergency changes to expand production capacity and accelerate release testing. Based on the analysis, relevant recommendations were proposed, such as conducting pre-research on changes, building “dual-use for routine and emergency” capabilities, implementing graded and categorized management, optimizing registration testing, and promoting the rapid microbiological methods and AI.

作为一个有着14亿多人口的大国,防范化解重大疫情和重大突发公共卫生风险,是须臾不可放松的大事[1],疫苗、免疫球蛋白、中和抗体等创新生物制品是预防和治疗传染性疾病最有力的武器。突发重大公共卫生事件时,保证急需生物制品的充分供应,关系民众健康、社会稳定和国家安全,具有重大经济意义和社会意义。
相对于常态下药品上市许可持有人(以下简称持有人)为提高自身产品质量、生产效率和经济效益而自发开展的药品上市后变更,为应对重大突发公共卫生事件而紧急开展的药品上市后变更(以下简称应急变更),具有合规性和紧迫性双重要求。《药品管理法》《疫苗管理法》规定对于重大突发公共卫生事件急需的疫苗和药品,国务院药品监督管理部门可以附条件批准。《药品注册管理办法》也为药品加快上市注册设立了突破性治疗药物、附条件批准、优先审评审批和特别审批四条路径[2]。但是上述政策主要针对新药注册,对于如何加速急需药品应急变更,政策法规尚无明确规定。目前,业内对于生物制品上市后变更的研究主要针对常态化变更[3,4],而关于应急变更的研究鲜有报道。
本文从提升重大突发公共卫生事件防控能力视角,探讨创新生物制品的持有人和生产企业如何在现有政策制度框架内,选择适合的变更策略,规范高效地实施应急变更,以满足防控药品的紧急保供需求。
生物制品因其在传染病预防和治疗方面不可替代的作用,当重大突发公共卫生事件发生时,其用药需求可能大幅增加,需要企业在短时间内提升供应能力。如在近年的新冠病毒疫情和后续甲型流感爆发期间,新冠病毒疫苗、流感疫苗、静脉用丙种球蛋白和单抗类治疗药物的需求激增。当发生地震、海啸等严重地质灾害时,霍乱、鼠疫等甲类传染病疫苗的大规模接种需求也可能出现。而在国外爆发严重传染病疫情时,我国政府还可能需要对外供应或捐赠相关疫苗和治疗药物。但是,由于生物制品生产大多涉及微生物或细胞逐级放大培养和收获,批产量和生产周期基本固定,试图通过压缩原有工艺周期,提高生产批次、增加产量比较困难。另一方面,生物制品生产场地对无菌环境等要求苛刻,维护成本极高,企业出于经济考量,一般处于产需紧平衡状态。尤其是产品刚上市的创新生物制品,产线和产能会更加紧张。因此,当发生重大突发公共卫生事件时,持有人需要根据政府的防控保供部署,通过开展应急变更来紧急提高产能。由于生物制品生产一般分为原液生产和制剂生产两段工序,因此可以从提升原液产能、提升制剂产能或委托生产三个方面来研究变更策略。
生物制品的成品放行检验包括鉴别、物理检查、化学检定、含量、纯度、杂质、生物学活性、无菌、内毒素、异常毒性、免疫力实验(疫苗)、病毒滴定(疫苗)等项目,是产品上市放行的重要依据,也是影响产品供应速度的关键环节。正常情况下,除了细胞治疗产品等少数药品,大多数生物制品企业并不特别强调放行时限的要求,通常采用标准化的检验方法。但是当重大突发公共卫生事件发生时,在保证检验质量的前提下,通过应急变更,改用快速微生物检查法等更加高效的检验方法,可以显著缩短检验周期,加快产品上市,对于疫情防控有重大意义。
与通常情况下企业自发进行的上市后变更不同,应对突发重大公共卫生事件的应急变更通常是持有人根据紧急保供要求进行的。常规生物制品放大产能的变更周期往往需要2~5年,但应急变更必须在数月内完成。因此应急变更的难点是如何在符合上市后变更管理法律和规范要求的前提下,用最短的时间完成变更程序,尽快启动保供生产。保供需求往往具有突发性和暂时性的特点,导致应急变更的时间窗口很窄。若持有人延迟完成变更,就可能会错过需求关键时期,导致投入的大量成本无法实现预期效果。因此,应急变更的完成速度非常关键。
生物制品的生产涉及生物过程和生物材料,相较化学药和中药,生产更复杂,质量控制点更多,无菌控制要求更严格,相应的变更管理要求也更高[5]。为了确保变更不会对产品质量安全造成不利影响,正式实施变更前必须开展严格的变更研究,充分评估和识别该变更在工艺、质量、稳定性、临床数据方面的潜在风险点。一般而言,变更等级越高,变更涉及内容越多,变更研究所需的时间越长[6]。例如,涉及临床/非临床研究、长期稳定性考察、遗传稳定性研究的变更研究周期通常超过6个月,甚至可能持续数年,而且研究失败风险较高。
根据《药品注册管理办法》要求,生物制品的重大变更是审评类事项,持有人向国家药品监督管理局药品审评中心提交补充注册申请,批准后方能实施。根据《药品上市后变更管理办法(试行)》,审评难度和变更等级及内容复杂度密切相关[7]。单项变更的审评时限为60日,多项合并申报的审评时限为80日;如果触发临床试验研究数据审查或者药品注册核查检验,审评时限将延长为200日。因此,审评周期是应急变更策略研究中需要着重考量的因素。
在制定应急变更策略时,持有人应专注于提升产能和加速供应这两个核心任务,对照《已上市生物制品药学变更研究技术指导原则(试行)》(以下简称为《变更指导原则》)要求,优先选择用时较短,风险最可控的变更策略。一是争取将变更等级控制在备案后即可实施的中等变更或先实施再报告的微小变更级别。二是尽量规避需要开展临床/非临床研究、长期稳定性考察、注册检验等耗时较长的变更内容。三是控制变更范围,避免将多个项目合并申请。例如,将反应器放大或培养基变更等项目合并到生产场地变更中,可能会提高变更研究的难度,增加延期和失败的风险。四是要确保变更研究方案的周密性,防止发生因研究不充分、数据不规范导致的退回补充情况。此外,在启动应急变更之前,持有人应当就变更策略和监管要求,及时与药品监管部门沟通,达成共识。在明确应急变更的优先级与风险管控框架后,以下将针对不同生产环节的优化路径展开具体分析。
生物制品的原液生产工序一般包括种子复苏、细胞扩增、生物反应器培养、转染、深层过滤、超滤浓缩、层析、置换缓冲液等环节。其中的生物反应器培养是核心环节。提升原液产能一般围绕增加生物反应器培养量开展。
路径一:放大生物反应器容量。如将现用的500 L生物反应器,变更为容量更大的2 000 L一次性反应器或10 000 L不锈钢反应器。但是生物培养规模放大并不是简单的线性放大,往往需要同时进行传代代次变更、培养基变更、控制参数以及各种关键工艺变更,才能保证变更前后的生产和质量稳定性。根据《变更指导原则》,不但需要进行连续3批生产工艺验证和稳定性研究,还可能需要开展动物安全性、有效性评估、非临床/临床的桥接研究、外源因子检测和遗传稳定性研究[8]。该变更一般为审批类的重大变更,变更研究内容极多,往往需要数年才能完成。因此,虽然增加单个生物反应器容量对于生产企业降低生产成本很有意义,但是并不适宜用于快速提升产能。
路径二:为尽量缩短变更耗时,更可行的扩大产能的做法是在原生产线上增加相同的生物反应器,或生产线整体复制。在传代代次、生产工艺和过程控制不发生实质性改变的前提下,单纯增加同种生产设备或复制生产线,可以显著减少变更研究的范围和内容。如果所在生产车间或厂房已被批准用于生产同类产品,且受控于同一质量管理体系,还可以进一步由重大变更降低为中等变更。持有人不需要补充申请或专门备案,只要向省级药监部门申请增加生产场地的《药品生产许可证》变更申请,再通过生产许可检查和药品生产管理规范(GMP)符合性检查(两检查可同步开展)后即可实施,而且接受GMP符合性检查的产品可以上市供应[9]
制剂工序包括原液解冻、配制、除菌过滤、内包材清洗、干热灭菌、烘干、制剂灌装、压塞、冻干、轧盖等全部或部分环节。加速变更的关键是能否将变更等级控制在中等变更,缩短变更周期。
路径一:放大批生产规模。放大批生产规模可能会涉及无菌过滤、灌装时间、冻干装量、冻干曲线、以及原液和制剂批次对应关系等变化。如果变化超出了产品《制检规程》核准的范围,则应当先开展变更研究,经过备案或补充申请获批后才能实施。如果还涉及配制系统、除菌过滤系统等设备变更,或病毒去除灭活工艺改变,就会升级为重大变更,甚至可能需要开展非临床/临床的桥接研究。因此,为了缩短变更时间,应急变更时应当尽量不改变原关键设备和工艺或使用等效设备。
路径二:增加生产线。制剂生产的车间厂房是无菌环境的重要保障。如果选择新建或者改建车间/厂房/生产线,除了基建工作,还需要开展培养基模拟灌装试验等无菌验证。模拟灌装实验连续合格3次后,才能申请GMP检查和补充申请,通过后方能实施,变更周期耗时很长。因此,建议应急变更时应选择此前已被批准,生产过相同类别产品的场地进行生产,一方面可以减少验证工作量,另一方面,该变更为中等变更,可以不用补充申请或专门提交备案,只要向省级药监部门申请新增生产场地即可。若还需要增加生物制品的包装线/贴签线,则应当在已批准的生产场地进行,且不改变包装材料、容器及贮藏条件,从而使该应急变更控制在微小变更级别。
生物制品生产厂房、车间和生产线建设周期长,投资巨大,而且后期运行维护成本高昂,因此持有人通过委托生产的方式,利用其他企业闲置产能,提高临床急需药品的产量是一种可行的方式。在新冠疫情期间,一些疫苗和治疗药物就曾采取全部或分段委托生产的方式,取得了良好的效果[10]
在应急委托生产中,持有人承担质量安全主体责任[11],需要谨慎选择合适的受托生产方企业,如果受托方与当前厂家受控同一质量体系(QA/QC),生产场地已批准过生产同类产品,且原液生产线与当前生产线一致,该变更等级可以控制在中等变更级别。一般而言,隶属同一集团的企业更容易符合上述要求。
此外,跨省委托生产审批还涉及双方所在地的省级药监部门的监管协同。例如,需要受托方所在地省局对受托生产场地开展现场检查,出具GMP符合性检查结果告知书和《同意受托生产的意见》。如果双方药监部门已建立了监管合作和信息传递机制,如长三角地区间的药品监管合作机制,将有利进一步加快委托生产的许可审批,这也可以作为选择委托生产方的重要考量。
在成品放行检验中,无菌检查是最关键的检验项目之一。绝大多数生物制品的无菌检查方法采用传统的微生物培养法(《中国药典》通则1101),该方法的检验周期至少需要14日,是最耗时的检验环节之一。如果能将微生物培养法变更为更先进的快速微生物检测法(rapid microbiological methods,RMM),无菌检查周期有望缩短50%以上,可以极大提高产品供应临床的速度。目前业内开发的RMM有呼吸信号法、固相细胞法、核酸扩增法、ATP法等[12, 13]。根据《变更指导原则》,分析方法的替换一般属于重大变更,需要提交补充申请,变更周期较长。不过,鉴于呼吸信号法已经在无菌要求更严格的细胞治疗产品检验中得到应用,而且2024年发布的《中国药典》(2020年版)第一增补本9406《细胞类制品微生物检查指导原则》中也已将呼吸信号法作为RMM推荐方法,如果持有人将微生物培养法变更为药典已认可的呼吸信号法,或者作为培养法的补充,则该应急变更可考虑申请降级管理。
需要注意,呼吸信号法使用特定的仪器和耗材,企业变更前需要按《药品类微生物检验替代方法验证指导原则》进行适用性研究,对专属性、检测限、重现性、耐用性进行验证,证明和原方法等效。另外,由于疫苗和血液制品必须批签发后才能上市,相关企业应当提前与对应的批签发机构对接,做好检验方法的转移和衔接。
各级政府可考虑将提升急需药品的生产能力纳入应对突发公共事件总体应急预案中,统筹提升应急变更能力。一是鼓励持有人将应急变更事宜纳入日常工作中,定期梳理可能有保供需求的品种,制定应急方案,适时开展变更预研,尽早启动扩产所需的对比分析、工艺验证、稳定性研究等耗时较长的验证工作。二是推动“平急两用”产能建设,鼓励持有人在厂房、车间、生产线的设计与建设中,考虑产品的兼容性和易切换性,预留出扩展空间,并预先规划共线生产的评估与验证工作。对于集团型企业,应鼓励建立各子公司间的生产互备份机制,统一质量管理体系和生产追溯信息化管理系统。
对于药品监管部门,建议实施分级分类管理,将信用等级较高、变更管理记录良好的企业纳入“白名单”,为其应急变更的审评或备案开辟快速通道;做好跨前服务,对涉及应急变更的车间厂房改造、委托生产质量协议签署和申报资料准备给予前置指导;建议将“优化药品补充申请改革试点”范围逐步扩大至生物制品,减少应急变更的注册检验批次要求[14]。此外,建议推动RMM、参数放行、人工智能以及检验机器人等先进技术在大规模放行检验中的应用。
对于行业协会,建议充分发挥行业自律作用,收集最佳案例,开展专题培训,牵头建设上市后应急变更的团体标准,指导企业提高变更合规意识。
上市后变更是生物制品全生命周期管理的关键内容。在应对重大突发公共卫生事件中,生物制品应急变更的合规性和紧迫性显得尤为重要,不仅要求持有人增强责任意识和合规意识,持续强化变更研究和管理水平,优化应急变更策略,提升药品应急生产供应能力,同时也需要政府部门做好统筹规划,完善制度设计,加强指导服务。相信通过多方共同努力,国家应对化解重大公共卫生风险挑战的能力将不断增强,为人民健康安全提供更加坚实的保障。
